Abstract:Although numerous studies have been performed, there are no clear findings providing a meaningful statement about how foot strike changes as a result of muscular fatigue. In studies on running fatigue, it is not possible to differentiate which kinematic or kinetic changes are a direct result of local muscle fatigue and varied speeds. This study aimed to investigate the effect of foot strike technique and localized muscle fatigue of the plantar and dorsal flexors on plantar pressure distribution and selected kinematic features of treadmill running. Twenty-six voluntary forefoot and rearfoot runners of similar age and body mass participated in the study. Each group completed two tests with a time interval of 3-7 days. The kinematic data was measured with the help of a three-dimensional measuring and analysis system while the volunteers ran on the treadmill. The fatigue protocol included an isometric maximal force test and an isokinetic endurance test. In order to check the differences, a variance analysis with repeated measurements was used. The strength values of the two groups of runners showed significant average differences in the plantar flexors in the endurance test and in the fatigue index for both the left and right legs. The results of the strength values are surprising because the plantar flexors of this group of runners should be well-trained due to the forefoot strike during regular running. In response to fatigue, the pressure maxima decreased under the exposed foot regions, i.e. under the forefoot at forefoot strike and under the heel at rearfoot strike. The two groups of runners differed in foot angle at foot-on with higher values measured in the forefoot runners. The larger foot angle of the forefoot runners improved shock absorption and can thus reduce the risk of injury.
Does high-intensity running to fatigue influence lower limb injury risk?

Abstract:Objectives: The aim of this study was to quantify changes in peak bending moments at the distal tibia, peak patellofemoral joint contact forces and peak Achilles tendon forces during a high-intensity run to fatigue at middle-distance speed. Design: Observational study. Methods: 16 high-level runners (7 female) ran on a treadmill at the final speed achieved during a preceding maximum oxygen uptake test until failure (~3 min). Three-dimensional kinetics and kinematics were used to derive and compare tibial bending moments, patellofemoral joint contact forces and Achilles tendon forces at the start, 33 %, 67 % and the end of the run. Results: Average running speed was 5.7 (0.4) m·s-1. There was a decrease in peak tibial bending moments (-6.8 %, p = 0.004) from the start to the end of the run, driven by a decrease in peak bending moments due to muscular forces (-6.5 %, p = 0.001), whilst there was no difference in peak bending moments due to joint reaction forces. There was an increase in peak patellofemoral joint forces (+8.9 %, p = 0.026) from the start to the end of the run, but a decrease in peak Achilles tendon forces (-9.1 %, p < 0.001). Conclusions: Running at a fixed, high-intensity speed to failure led to reduced tibial bending moments and Achilles tendon forces, and increased patellofemoral joint forces. Thus, the altered neuromechanics of high-intensity running to fatigue may increase patellofemoral joint injury risk, but may not be a mechanism for tibial or Achilles tendon overuse injury development.

Abstract:ABSTRACT Objective: To understand the influence of sports fatigue on plantar pressure distribution of healthy male college students and provide a theoretical basis for improving their awareness of foot health. Methods: Forty-nine ordinary male college students jogged along the 800-meter runway to moderate fatigue. All the subjects took off their shoes and socks and walked naturally with their usual gait. The dynamic plantar pressure of each foot was measured twice in one step. FootscanUSB2 Belgian flat-plate plantar pressure testing system was used for testing. Results: The average dynamic peak plantar pressure was (206.38 44.59) N for boys, and the changes of AA and CB walking speed in the arch did not change significantly. After fatigue, the peak pressure of FM, AA, RH5 in left foot and FM, AA, CB in right foot decreased significantly (P < 0.05, P < 0.01). The peak time of RH and CB in the left foot was significantly shorter than that before fatigue (P < 0.05, P < 0.01), while FMF, AA, C areas had no significant change but tended to be delayed. There were significant differences in peak force-time between boys’ left and right feet except for the fifth metatarsal bone (P < 0.05). There is a significant difference in the peak force-time between the second and fifth toes of the left foot (P< 0.01), and there is a gender difference in the peak force-time between the second metatarsal and the third metatarsal (P < 0.05). Conclusion: Sports fatigue leads to the decrease of physiological functions such as muscle strength of lower limbs, which leads to the corresponding changes in gait stages, plantar pressure distribution parameters, and foot balance. Larger Achilles and plantar fascia induce lower duty factor during barefoot running

Abstract:Objectives: Tendons play a crucial role allowing the storage and release of mechanical energy during the running cycle. Running kinematics, including duty factor, constitute a basic element of the runner's biomechanics, and can determine their performance. This study aimed to analyze the link between Achilles tendon and plantar fascia morphology and running parameters, considering the influence of wearing shoes versus running barefoot. Design: Cross-sectional study. Methods: 44 participants (30 men and 14 women) engaged in two running sessions, one with shoes and one without, both lasting 3 min at a consistent speed of 12 km/h. We captured running kinematic data using a photoelectric cell system throughout the sessions. Before the trials, we measured the thickness and cross-sectional area of both the Achilles tendon and plantar fascia using ultrasound. Results: The Pearson test revealed a significant correlation (p < 0,05) between Achilles tendon and plantar fascia morphology and contact time (r > -0.325), flight time (r > -0.325) and duty factor (ratio of ground contact to stride time) (r > -0.328) during barefoot running. During the shod condition, no significant correlation was found between connective tissue morphology and kinematic variables. Conclusions: In barefoot running, greater size of the Achilles tendon and plantar fascia results in a reduced duty factor, attributed to longer flight times and shorter contact times.
Differentiation of Candida dubliniensis on chrom agar and Pal's agar.

Abstract:Difference in expression of putative virulence factors and in antifungal susceptibility among different Candida species has raised the need for species-level identification. The close relationship of Candida dubliniensis with C. albicans has led to misidentification of C. dubliniensis isolates as C. albicans. Phenotypic tests include ability to produce chlamydospore on casein agar, colony colour development on differential media CHROM agar Candida medium and ability to form hyphal fringe on Pal's agar, have been used to differentiate these two Candida species. Fifty isolates of Candida species were recovered from various specimens (blood, urine, tissue and respiratory secretions) from diabetic and cancer patients between April and July 2007. The isolates were tested for chlamydospore production on casein agar. These were also streaked simultaneously on CHROM agar, Pal's agar and a combination of CHROM agar supplemented with Pal's agar for identification and differentiation of C. dubliniensis from C. albicans. On CHROM agar, 19 isolates were identified as C. dubliniensis, nine as C. albicans, 10 as C. krusei, nine as C. tropicalis and two as C. glabrata. One was indeterminate and later identified as C. dubliniensis. Out of the 20 C. dubliniensis isolates, 19 isolates exhibited hyphal fringe on Pal's agar. On CHROM agar supplemented with Pal's agar, 16 out of the 19 fringe-positive isolates exhibited fringe surrounding the bluish green-coloured colonies of C. dubliniensis. Additional identification tests like growth at 45 degrees C and ability to reduce 2,3,5-triphenyltetrazolium chloride were time efficient, inexpensive and easy-to-use methods for differentiation of C. dubliniensis and C. albicans isolates. CHROM agar when supplemented with Pal's agar gave definitive identification between C. dubliniensis and C. albicans.
[Massive subcutaneous emphysema following percutaneous tracheostomy].

Abstract:Although percutaneous dilatational tracheostomy (PDT) is fast becoming the method of choice for securing an airway in chronic ventilated patients in an intensive care unit (ICU). Subcutaneous emphysema is an unusual and sometimes lethal complication which may extend the length of stay in the ICU. We report a case of massive subcutaneous emphysema without tracheal wall laceration that occurred in the ICU after PDT. An 81-year-old woman was admitted to our ICU due to infective exacerbation of chronic obstructive airways disease. Her medical therapy included nebulized bronchodilators, steroids and empirical antibiotics. Within thirty-six hours of initial endotracheal intubation PDT was performed. Several hours following the procedure, the patient developed massive subcutaneous emphysema encompassing her entire body. No pneumothorax was identified on subsequent chest X-ray. Laryngoscopic and bronchoscopic examination showed no evidence of tracheal wall laceration. We discuss the etiology and management plan for this rare complication.

Abstract:Abstract We determined whether fatigue modifies the effect of custom foot orthoses manufactured from ethyl-vinyl acetate (EVA) and expanded thermoplastic polyurethane (TPU) materials, both compared to standardized footwear (CON), on running mechanics, running economy, and perceived comfort. Eighteen well-trained, males ran on an instrumented treadmill for 6 min at the speed corresponding to their first ventilatory threshold (13.8 ± 1.1 km/h) in three footwear conditions (CON, EVA, and TPU). Immediately after completion of a repeated-sprints exercise (8 × 5 s treadmill sprints, rest = 25 s), these run tests were replicated. Running mechanics, running economy and perceived comfort were determined. Two-way repeated measures ANOVA [condition (CON, EVA, and TPU) × fatigue (fresh and fatigued)] were conducted. Flight time shortened ( P = 0.026), peak braking ( P = 0.016) and push-off ( P = 0.032) forces decreased and vertical stiffness increased ( P = 0.014) from before to after the repeated-sprint exercise, independent of footwear condition. There was a global fatigue-induced deterioration in running economy (− 1.6 ± 0.4%; P < 0.001). There was no significant condition × fatigue [except mean loading rate ( P = 0.046)] for the large majority of biomechanical, cardio-respiratory [except minute ventilation ( P = 0.020) and breathing frequency ( P = 0.019)] and perceived comfort variables. Acute intense fatigue does not modify the effect of custom foot orthoses with different resilience characteristics on running mechanics, running economy and perceived comfort.

Quantifying Active and Passive Stiffness in Plantar Flexor Muscles Following Intermittent Maximal Isometric Contractions Using Shear Wave Elastography

Abstract:Objective: This study aimed: (i) to investigate the impact of fatigue, triggered by maximal isometric contraction exercises, on the active and passive stiffness of plantar flexors (PF), and (ii) to examine the relationship between changes in mechanical parameters and neuromuscular alterations after fatigue. Methods: A healthy cohort (n = 12; age = 27.3 ± 5.5 y; BMI = 24.4 ± 2.35 kg/m²) was instructed to perform 60 isometric contractions, each lasting 4 s with a 1-s rest interval, using an ergometer. Several measures were taken before and after the fatigue protocol. First, the stiffness of the PF-tendon complex (PFC) was quantified during passive ankle mobilization both during and after the fatigue protocol using the ergometer. Additionally, from shear wave elastography, the active and passive stiffness of the gastrocnemius medialis (GM) were measured during passive ankle mobilization and isometric maximal voluntary contraction (MVC), respectively. Finally, the peak torque and the rate of torque development (RFD) of PF were assessed during the MVC using the ergometer. Ankle muscle activities (surface electromyograph [SEMG]) were recorded during all evaluations using electromyography. Results: After the fatigue protocol, the results revealed a decline in active stiffness, peak torque of PF, RFD and SEMG activity of the GM (p < 0.001). Furthermore, significant correlation was identified between the decrease of the peak torque of PF and the active stiffness of the GM (r = 0.6; p < 0.05). A decrease in the PFC stiffness (p < 0.001) and a decrease in the shear modulus of the GM at 20° (p < 0.001) were also observed. Conclusion: Isometric fatiguing exercises modify the mechanical properties of both the contractile and elastic components. Notably, decreases in both passive and active stiffness may be critical for athletes, as these changes could potentially increase the risk of injury.
Effect of Fatigue and Graded Running on Kinematics and Kinetics Parameters in Triathletes

Abstract:This study aimed to evaluate the effect of cycling-induced fatigue on subsequent running with different ground inclinations on triathletes. Twenty male triathletes ran on a treadmill at individual speeds, on three inclines: level (0%), 7% uphill (7%), and 7% downhill (-7%). Subsequently, the athletes performed an incremental ramp-to-exhaustion (RTE) protocol on an ergometric bicycle until task failure and repeated the running protocol (POST-RTE). During running and cycling was monitored perceived exertion (RPE) and heart rate (HR). Plantar pressure distribution, peak plantar pressure, total foot area, and ground contact time were measured by an in-shoe sensor. Analysis of variance showed that HR (p=0.001; η2=0.63) and RPE (p=0.001; η2=0.82 large effect) were higher in POST-RTE, without interactions between slopes and fatigue. The heel load showed difference between 0 and 7% and 7 and -7% (p=0.001; η 2=0.44 large effect) with higher value in -7%. The midfoot showed differences between 0 and -7% with higher value in 0% (p=0.03; η 2=0.15 small effect). The contact time showed interactions between fatigue and slopes with a higher value in POST-RTE. Coaches should be aware that training on the uphill and downhill surfaces can alter the plantar load on different foot parts.

Abstract:This study aimed to determine how fatigue affects factors associated with injury, neuromuscular activity, and control in recreational runners. Previously identified injury risk factors were defined as peak vertical instantaneous loading rates (pVILR) for tibial stress fracture (TSF) and peak hip adduction (pHADD) for patellofemoral pain syndrome and iliotibial band syndrome. Kinematics, kinetics, and electromyography data were collected from 11 recreational runners throughout a fatiguing run. Three trials were collected in the first and final minutes of the run. Coactivation was quantified about the knee and ankle for the entire stance phase and anticipatory, weight acceptance (WA), and propulsion sub-phases of stance. Trunk control was quantified by the peak mediolateral lean, peak forward lean, and flexion range of motion (ROM). There were significant increases in pHADD and pVILR when fatigued. Significant decreases in coactivation around the knee were found over the entire stance phase, in the anticipatory phase, and WA phase. Coactivation decreased about the ankle during WA. Lateral trunk lean significantly increased when fatigued, but no significant changes were found in flexion ROM or lean. Mediation analyses showed changes in ankle coactivation during WA, and lateral trunk lean are significant influences on pVILR, a measure associated with TSF. Fatigue-induced adaptations of decreasing ankle coactivation during WA and increased lateral trunk lean may increase the likelihood of TSF. In this study, a fatiguing run influenced changes in control in recreational runners. Further investigation of causal fatigue-induced injuries is necessary to better understand the effects of coactivation and trunk control.

Abstract:This study investigates factors associated with the self-reported use of complementary therapies, types of therapies used, and sources of complementary therapy information among HIV-positive patients attending a public, HIV outpatient clinic in New Orleans. A convenience sample of 287 clients (220 men and 67 women) was given a self-administered anonymous questionnaire. Overall, complementary therapy use was 31%. Patients who used complementary therapy were more likely to be white (O.R., 2.5), female (O.R. 3.3), a high school graduate (O.R. 2.9), and to know another complementary therapy user (O.R. 7.8). Age, sexual orientation, CD4 cell count, injection drug use, living with another HIV-infected person, having pain, and HIV support group membership were not associated. Men were more likely than women, and whites were more likely than nonwhites, to use vitamins/minerals, imagery/meditation, and dietary regimens. Nonwhites were more likely than whites, and women more likely than men, to use spiritual healing. Of those using complementary therapy, men were more likely than women, and whites more likely than nonwhites, to get information about complementary therapy from HIV organizations, friends, and homosexual-oriented media. Doctors and nurses were the most frequently cited source of complementary therapy information for women. Frequency, type of therapies used, and source of information about complementary therapy among HIV-infected persons vary by race and gender. Clinicians should be educated about complementary therapies so that they can provide information to their patients and be aware of self-treatment behavior.
Effect of foot position during plantarflexion on the neural drive to the in runners with Achilles tendinopathy

Abstract:Runners with Achilles tendinopathy (AT) have reduced neural drive to the (GL). This study investigated if the strategy of pointing feet-inward ( ) during isometric plantarflexion would increase electromyography root mean square amplitude (RMS) and motor unit discharge rates, compared to feet-in neutral position ( ), in runners with Achilles Tendinopathy (AT). High-density electromyograms were recorded from and , during 20-s and isometric heel raise, in runners with (n=18) and without (n=19) AT. During , GL RMS was higher during in both groups and GM RMS was lower only during in the AT. Conversely, motor unit discharge rates were lower during in GL (p<0.001) and in GM in the AT group. The AT group had lower endurance during single leg heel raise. In summary, increases GL RMS in both groups, conversely reducing motor unit discharge rates in the AT group, compared to . Additionally, reduces GM RMS and motor unit discharge rates only in the AT group, compared to . This would shift the ratio excitation, favouring . Nonetheless, while this strategy holds promise, it remains uncertain whether performing plantarflexion exercise with feet pointed inwards would provide additional benefits for the treatment of runners with Achilles tendinopathy. Our findings suggest that the increased GL RMS during is effective in increasing GL excitation but not as consequence of increased MUDR and, but it might be a result of recruitment of more motor units.

Plantar load characteristics among runners with different strike patterns during preferred speed

Abstract:Objectives: This study aimed to compare the plantar loads between habitual rearfoot strike (RFS) and non-RFS (NRFS) during running under the participant's preferred speed. Methods: A total of 66 (36 RFS, 30 NRFS) healthy amateur male runners were included in our study. In-shoe pressure sensors were utilised to the test plantar loads when participants were running using their preferred foot strike pattern and running speed (RFS: 3.2 ± 0.3 m/s; NRFS: 3.4 ± 0.4 m/s). Results: Results indicated that running speed has a significant effect on the total contact area [F (1, 64) = 7.061, P = 0.01, η 2 = 0.101], which also affects midfoot and forefoot regions. No significant difference was found on the total maximum force, force-time-integral, peak pressure (PP) and pressure-time-integral (PTI), but the total contact area of RFS was higher than that of NRFS runners [F (1, 64) = 77.406, P < 0.001, η 2 = 0.551]. Plantar loads were mainly focused on the heel and midfoot for RFS runners in all variables, and NRFS runners experienced increased PP and PTI in medial forefoot regions. Conclusion: Habitual runners tend to adjust their contact area according to the running speed through midfoot and forefoot regions. RFS runners remain susceptible to high impact force on the heel and midfoot, and NRFS runners experience high impact force in the first metatarsal regions. Therefore, runners should note this situation to avoid running-related injuries.

Abstract:Abstract Two long-distance runners developed MTSS (male age=26.3 years, female age=47.5 years) after baseline assessment of plantar flexor muscle structure and function and spatiotemporal running variables. B-mode ultrasound and lean leg girth characterized plantar flexor muscle structure. Hand-held dynamometry and a single-leg heel raise to failure protocol characterized plantar flexor muscle function. Finally, spatiotemporal running variables were determined during a treadmill protocol. The two runners who developed MTSS demonstrated less plantar flexor strength and endurance capacity than published comparative control data and marked variability in muscle structure. Reduced plantar flexor strength and endurance capacity were thought to contribute to an impaired ability to resist tibial bending moments during midstance due to earlier muscle fatigue. Earlier muscle fatigue could, in turn, contribute to increased tibial bending moments and MTSS development. Therefore, assessing plantar flexor muscle strength and endurance might help to identify athletes at risk of developing MTSS.

Abstract:Evaluation of potential fatigue for the elderly could minimize their risk of injury and thus encourage them to do more physical exercises. Fatigue-related gait instability was often assessed by the changes of joint kinematics, whilst planar pressure variability and asymmetry parameters may complement and provide better estimation. We hypothesized that fatigue condition (induced by the treadmill brisk-walking task) would lead to instability and could be reflected by the variability and asymmetry of plantar pressure. Fifteen elderly adults participated in the 60-min brisk walking trial on a treadmill without a pause, which could ensure that the fatigue-inducing effect is continuous and participants will not recover halfway. The plantar pressure data were extracted at baseline, the 30th minute, and the 60th minute. The median of contact time, peak pressure, and pressure-time integrals in each plantar region was calculated, in addition to their asymmetry and variability. After 60 min of brisk walking, there were significant increases in peak pressure at the medial and lateral arch regions, and central metatarsal regions, in addition to their impulses (p < 0.05). In addition, the variability of plantar pressure at the medial arch was significantly increased (p < 0.05), but their asymmetry was decreased. On the other hand, the contact time was significantly increased at all plantar regions (p < 0.05). The weakened muscle control and shock absorption upon fatigue could be the reason for the increased peak pressure, impulse, and variability, while the improved symmetry and prolonged plantar contact time could be a compensatory mechanism to restore stability. Abstract:Exercise-induced fatigue causes changes within the central nervous system that decrease force production capacity in fatigued muscles. The impact on unrelated, non-exercised muscle performance is still unclear. The primary aim of this study was to examine the impact of a bilateral forearm muscle contraction on the motor function of the distal and unrelated ankle plantar-flexor muscles. The secondary aim was to compare the impact of maximal and submaximal forearm contractions on the non-fatigued ankle plantar-flexor muscles. Maximal voluntary contractions (MVC) of the forearm and ankle plantar-flexor muscles as well as voluntary activation (VA) and twitch torque of the ankle plantar-flexor muscles were assessed pre-fatigue and throughout a 10-min recovery period. Maximal (100 % MVC) and submaximal (30 % MVC) sustained isometric handgrip contractions caused a decreased handgrip MVC (to 49.3 ± 15.4 and 45.4 ± 11.4 % of the initial MVC for maximal and submaximal contraction, respectively) that remained throughout the 10-min recovery period. The fatigue protocols also caused a decreased ankle plantar-flexor MVC (to 77 ± 8.3 and 92.4 ± 6.2 % of pre-fatigue MVC for maximal and submaximal contraction, respectively) and VA (to 84.3 ± 15.7 and 97.7 ± 16.1 % of pre-fatigue VA for maximal and submaximal contraction, respectively). These results suggest central fatigue created by the fatiguing handgrip contraction translated to the performance of the non-exercised ankle muscles. Our results also show that the maximal fatigue protocol affected ankle plantar-flexor MVC and VA more severely than the submaximal protocol, highlighting the task-specificity of neuromuscular fatigue.

Abstract:Fatigue can be classified as peripheral or central depending on the extent of its effects. Muscle strength reduction, associated with the appearance of fatigue during running, produces kinetics and kinematics modifications which could lead to an increased risk of injury. This study aimed to analyze the effect of peripheral and central fatigue protocols in running kinematics and to investigate the relationship between isokinetic strength and dynamic stability in fatigue related changes. Eighteen male recreational runners participated in the study. The dynamic postural stability index (DPSI) and quadriceps and hamstring isokinetic strength were assessed before the fatigue test. Then, angular kinematics during treadmill running were evaluated in pre- and post-fatigue states (central and peripheral). The results showed that runners with higher hamstring isokinetic strength and better DPSI had lower modifications after central fatigue of stance time, knee flexion, vertical and leg stiffness, and ankle dorsiflexion during the absorption and propulsion phases (r > 0.400, p < 0.05). Moreover, small changes in ankle dorsiflexion at initial contact after peripheral fatigue are related to a better DPSI and higher hamstring isokinetic strength (r > 0.400, p < 0.05). In summary, high values of hamstring isokinetic concentric strength and dynamic stability are related to lower increases of range of movements during running after central and peripheral fatigue. So, fatigue may affect to a lesser extent the running technique of those runners with higher hamstring strength and stability values.
The Influence of Plantar Short Foot Muscle Exercises on the Lower Extremity Muscle Strength and Power in Proximal Segments of the Kinematic Chain in Long-Distance Runners

Abstract:The aim of this study was to evaluate the influence of plantar short foot muscles exercises on the performance of lower extremities in long-distance runners. 47 long-distance runners aged 21-45 years took part in this study. The participants were divided into two groups based on baseline measurement of Foot Posture Index: Group 1 (n=27) with neutral foot and Group 2 (n=20) with slight and increased pronation. The participants performed the exercises daily for 6 weeks. The knee flexors and extensors torque, work, and power on Isokinetic Dynamometer and Running-Based Anaerobic Sprint Test (RAST) were checked at baseline and after 6 weeks of exercises. Higher values of peak torque of knee flexors were observed. This change was statistically significant at high load with angular velocity 90°/s (73.55 Nm at baseline and 89.05 Nm after 6 weeks) and 160°/s (69.40 Nm at baseline and 79.00 Nm after 6 weeks) in Group 2. In both groups higher values of maximum power were noted. Participants in Group 2 achieved lower values in each 35-metre run time and higher values of power. In Group 2 there was significant improvement of total time (35.26 s at baseline and 34.79 s after 6 weeks) compared to Group 1 (37.33 s at baseline and 37.56 s after 6 weeks). Exercises strengthening short foot muscles may improve energy transfer through body segments and increase strength and values of generated power. They should be included as a part of daily training programme of runners. This study was registered in the Australian New Zealand Clinical Trials Registry (ANZCTR). Abstract:BACKGROUND: The risk factors for the development of plantar fasciitis (PF) have been associated with the medial longitudinal arch (MLA), rearfoot alignment and calcaneal overload. However, the relationships between the biomechanical variables have yet to be determined.OBJECTIVE: The goal of this study was to investigate the relationships between the MLA, rearfoot alignment, and dynamic plantar loads in runners with unilateral PF in acute and chronic phases.METHOD: Cross-sectional study which thirty-five runners with unilateral PF were evaluated: 20 in the acute phase (with pain) and 15 with previous chronic PF (without pain). The MLA index and rearfoot alignment were calculated using digital images. The contact area, maximum force, peak pressure, and force-time integral over three plantar areas were acquired with Pedar X insoles while running at 12 km/h, and the loading rates were calculated from the vertical forces.RESULTS: The multiple regression analyses indicated that both the force-time integral (R²=0.15 for acute phase PF; R²=0.17 for chronic PF) and maximum force (R²=0.35 for chronic PF) over the forefoot were predicted by an elevated MLA index. The rearfoot valgus alignment predicted the maximum force over the rearfoot in both PF groups: acute (R²=0.18) and chronic (R²=0.45). The rearfoot valgus alignment also predicted higher loading rates in the PF groups: acute (R²=0.19) and chronic (R²=0.40).CONCLUSION: The MLA index and the rearfoot alignment were good predictors of plantar loads over the forefoot and rearfoot areas in runners with PF. However, rearfoot valgus was demonstrated to be an important clinical measure, since it was able to predict the maximum force and both loading rates over the rearfoot.
