Abstract:Background The peculiar and highly variable C2 anatomy can make screw fixation more challenging and prone to potential vertebral artery or neurologic injury. Conventional C-arm fluoroscopy has several drawbacks. The aim of this research was to evaluate the accuracy of posterior C2 screw fixation using intraoperative three-dimensional fluoroscopy-based navigation (ITFN) and assess the perioperative complication rate related to screw placement.Methods A retrospective review identified patients who underwent operative management with C2 instruments using ITFN at our hospital between January 2006 and December 2012. Clinical data were obtained from medical records and final screw positions were graded according to a modified classification of Gertzbein and Robbins. Grade A and B screws were considered well positioned.Results The study included 99 patients (53 males and 46 females) who underwent posterior C2 screw fixation using ITFN. The mean Japan Orthopedic Association score improved from (6.7 +/- 1.9) points before surgery to (12.5 +/- 2.7) points at 6-month follow-up (z=+8.628, P<0.01). The mean visual analogue scale improved from (4.1 +/- 1.2) points before surgery to (0.7 +/- 0.9) points at 6-month follow-up, with an improvement of 83.7% (z=8.638, P < 0.01). Of the 196 screws analyzed using computed tomography and chart review, 126 transarticular, 64 pedicle, and 6 pars screws were placed with 82.5% (104/126), 89.1% (57/64), and 100% (6/6) accuracy (grade A), respectively; 98.5% (193/196) of screws were grade A or B (grade C, 1.5% (3/196)), and no neurologic injuries occurred. In normal C2 cases, 93 transarticulars and 47 pedicles were placed with high accuracy rates of 90.3% (84/93) and 93.6% (44/47) (grade A), respectively. However, in cases with C2 deformity, 33 transarticular, 17 pedicle, and 6 pars screws were placed with only 60.6% (20/33), 76.5% (13/17), and 100% (6/6) accuracy (grade A), respectively.Conclusion ITFN is a safe, accurate, and effective tool for posterior C2 fixation.

Abstract:Background: The incidence of fractures in the upper cervical spine is a common entity among elderly patients. The incidence depends on the health care system and the adopted protocols for a computer tomogram in the emergency unit, in order to assure an easier and earlier detection. Injuries to the vertebral artery could occur with devastating consequences, due to this type of fracture. The aim of the current surgical strategies is to stabilize the fractures and avoid further injuries to the vertebral arteries. Methods: We adopted a previously hypothesized method for intraoperative imaging of the vertebral artery when performing an O-Arm navigated cervical osteosynthesis. Three patients were included in this group: one patient with dens fracture type II after Anderson and D’Alonzo, one patient with a complex C1/C2 fracture and dissection of the vertebral artery on the left side and one patient with a kyphotic deformity due to Larynx carcinoma Metastasis in C4-C6. After positioning the patients, a 100 ml contrast was applied prior to the primary O-Arm scan, thus obtaining real-time imaging of the vertebral artery in the navigational CT series. Results: A total of 18 screws were placed. None of the screws bridged the vertebral canal. The average operative time was 174 mins. No new neurological deterioration was observed in the postoperative period. Conclusions: With the development of this technique, it is possible to reduce the surgery-related injury of the vertebral artery to 0%. Further studies are necessary to assess the feasibility of this technique. This technique could be especially helpful in the cases of distorted anatomical relations.

Comparative Analysis of Surgical Outcomes of C1–2 Fusion Spine Surgery between Intraoperative Computed Tomography Image Based Navigation-Guided Operation and Fluoroscopy-Guided Operation

Abstract:OBJECTIVE: Fixation of the C1-2 segment is challenging because of the complex anatomy in the region and the need for a high degree of accuracy to avoid complications. Preoperative 3D-computed tomography (CT) scans can help reduce the risk of complications in the vertebral artery, spinal cord, and nerve roots. However, the patient may be susceptible to injury if the patient's anatomy does not match the preoperative CT scans. The intraoperative 3D image-based navigation systems have reduced complications in instrument-assisted techniques due to greater accuracy. This study aimed to compare the radiologic outcomes of C1-2 fusion surgery between intraoperative CT image-guided operation and fluoroscopy-guided operation.METHODS: We retrospectively reviewed the radiologic images of 34 patients who underwent C1-2 fusion spine surgery from January 2009 to November 2018 at our hospital. We assessed 17 cases each of degenerative cervical disease and trauma in a study population of 18 males and 16 females. The mean age was 54.8 years. A total of 139 screws were used and the surgical procedures included 68 screws in the C1 lateral mass, 58 screws in C2 pedicle, nine screws in C2 lamina and C2 pars screws, four lateral mass screws in sub-axial level. Of the 34 patients, 19 patients underwent screw insertion using intraoperative mobile CT. Other patients underwent atlantoaxial fusion with a standard fluoroscopy-guided device.RESULTS: A total of 139 screws were correctly positioned. We analyzed the positions of 135 screws except for the four screws that performed the lateral mass screws in C3 vertebra. Minor screw penetration was observed in seven cases (5.2%), and major pedicle screw penetration was observed in three cases (2.2%). In one case, the malposition of a C2 pedicle screw was confirmed, which was subsequently corrected. There were no complications regarding vertebral artery injury or onset of new neurologic deficits. The screw malposition rate was lower (5.3%) in patients who underwent intraoperative CT-based navigation than that for fluoroscopy-guided cases (10.2%). And we confirmed that the operation time can be significantly reduced by surgery using intraoperative O-arm device.CONCLUSION: Spinal navigation using intraoperative cone-beam CT scans is reliable for posterior fixation in unstable C1-2 pathologies and can be reduced the operative time.

Abstract:PurposeIn surgery of C1–C2 fractures, standard navigation for screw placement based on preoperative image data was compared with intraoperative imaging guidance applying intraoperative computed tomography (iCT) with a special focus on accuracy of screw placement, workflow, and radiation exposure.MethodsA single surgeon series of 16 consecutive patients with C1–C2 trauma was retrospectively analyzed. Seven patients were operated with standard navigation; preoperative image data were registered by a 20-point surface-matching process for each vertebra. Nine patients were operated with iCT guidance, allowing automatic navigation registration. Screw placement was examined and graded with either iCT or postoperative CT. Dose length product of CT and dose area products of fluoroscopy scans were assessed; effective radiation doses were estimated based on conversion factors. Radiation doses of intraoperative and postoperative X-ray and/or CT diagnostics for each group were summarized to compare the total effective doses.ResultsA total number of 72 screws were placed, 26 in the standard navigation group including 24 screws in C1 and C2, and 46 screws in the iCT group including 34 screws in C1 and C2. 15.38% (n = 4) of the C2 screws showed a grade 1 deviation and 3.8% (n = 1) a grade 2 deviation applying standard navigation. There was no misplacement of screws in the iCT group. Mean operating time in the standard navigation group was 186.57 min versus 157.11 min in the iCT group, while the mean summarized effective dose was 1.129 mSv in the standard navigation and 2.129 mSv in the iCT group.ConclusioniCT navigated surgery can lead to higher accuracy and shorter operating time compared to standard navigated operations. iCT is a safe and straightforward procedure allowing reduction in radiation exposure of the medical staff, while modified scan protocols resulted in a radiation exposure that is lower than in standard diagnostic neck CT.

Abstract:This study explored the combined use of the O-arm and real-time navigation during spinal fixation. The clinical data for 60 patients undergoing spinal fixation, at Beijing Tsinghua Chang Gung Hospital between May 5, 2015 and May 1, 2017, were retrospectively analyzed. Pre-, intra-, and postoperative imaging findings were assessed. The patients were classified into the occipitocervical fusion (32 cases) and thoracic/lumbar/sacral spine fixation (28 cases, including 6 cases of percutaneous pedicle screw) groups. Lesion resections were performed microscopically. An O-arm, combined with real-time navigation, was used to assess spinal fixation. Efficacy was evaluated using operative times, Xray times, screw positioning, and complications. Within the occipitocervical fusion group, 182 screws were placed in the cervical spine and 96 in the occipital bone. However, 6 screws penetrated the bone cortex and were adjacent to the vertebral arteries, based on O-arm three-dimensional imaging; therefore, the precision rate was 96.7%. Within the thoracic/lumbar/sacral spine fixation group, 148 pedicle screws were implanted, with 4 initially outside the vertebral body, yielding a precision rate of 97.3%. Ten percutaneous pedicle screws were implanted and well positioned. O-arm scans were performed 3 times/patient, with an average of 20–30 min/time. Screw implantation times were 5–7 min (cervical spine), 8–10 min (thoracic spine), and 6–8 min (lumbar spine). Intraoperative O-arm scans, combined with real-time navigation technology, allow real-time observation of screw angles and depths, improving the accuracy and safety of posterior screw fixations and reducing the radiation dose and frequency experienced by patients and surgeons.
Posterior C1-C2 Transarticular Screw Fixation for Atlantoaxial Instability Assisted by Intraoperative 3-Dimensional Fluoroscopy-Based Navigation

Abstract:Objective To assess the feasibility,efficacy and indication of posterior C1-C2 transarticular screw fixation for atlantoaxial instability assisted by intraoperative 3-dimensional fluoroscopybased navigation.Methods From November 2005 to October 2012,58 patients with atlantoaxial instability underwent posterior C1-C2 transarticular screw fixation assisted by intraoperative 3-dimensional fluoroscopybased navigation.Those with excellent atlantoaxial reduction and no need for decompression underwent percutaneously minimally invasive procedures.Operative duration,blood loss volume,reduction of atlantoaxial joints and postoperative neurovascular complications were recorded.Screw positions were studied by postoperative computed tomography (CT) and divided into 3 categories.The follow-up period was at least 3 months.Local symptoms (assessed by visual analogue scale (VAS) score),neurological conditions (Nurick classification) and bone graft fusion were evaluated.Results Among them,the procedure was either open surgery (n =51) or percutaneously minimally invasive surgery (n =7).A total of 111 screws were placed satisfactorily.Five patients underwent only unilateral C1-C2 transarticular screw fixation.No severe complications were encountered.The mean operative duration was 202.2 ± 54.2 minutes and the average volume of blood loss 412.6 ± 281.2 ml.And the values were comparable to those for the traditional fluoroscopy-guided free-hand procedures.No statistical significance existed between open and percutaneously minimally invasive procedures in operative duration,blood loss volume and precision of screw placement.The patients with incompletely reduced C1-C2 dislocation had comparable clinical outcomes with those with complete reduction.After a 3-month follow-up,all of them showed significant improvements of local symptoms.VAS score decreased from 4.5 ± 1.3 preoperatively to 0.7 ± 0.8 postoperatively while the Nurick classification improved from 2.4 ± 1.1 to 0.6 ± 1.0.And bone graft fusion was satisfactory.Conclusions Posterior C1-C2 transarticular screw fixation for atlantoaxial instability assisted by intraoperative 3-dimensional fluoroscopy-based navigation is both feasible and safe.Combined with minimally invasive techniques,this procedure may achieve greater precision and minimal invasion.

Abstract:Objectives To explore the applicability and precision of O-arm combined with real-time navigation technique in spinal fixation.Methods The clinical data of 31 inpatients with spinal fixation were retrospectively analyzed at Department of Neurosurgery,Beijing Tsinghua ChangGung Hospital from May 2015 to May 2016.Preoperative,intraoperative and postoperative imaging findings were assessed.The patients were divided into occipitocervical fusion group (19 cases) and thoracic-lumbar-sacral spine fixation group (12 cases).The lesion resection was performed under microscope.O-arm combined with real-time navigation was used in spinal fixation.Surgical efficacy in the patients was evaluated by means of operation time,X-ray times,screw positions and complications.Results During the occipitocervical fusion group,112 screws were implanted in the cervical spine,and 57 in occipital bones.Two screws were found to have penetrated bone cortex and were adjacent to vertebral arteries according to intraoperative 3-D image of O-arm.In the thoracic-lumbar-sacral spine fixation group,72 pedicle screws were implanted,but 2 screws were out of vertebral body and corrected in time.O-arm scan times was 3 per case on average,and the operation time of O-arm and navigation was 20-30 minutes per time.Screw implantation lasted 5-7 minutes per screw in cervical spine,8-10 minutes in thoracic and 6-8 minutes in lumbar spine.Conclusions Through O-arm real-time navigation,the angle and depth of screws could be observed in real during screw implantation,which would greatly improve the implantation accuracy and safety,and,at the same time,reduce the radiation dose and times for the patients.

Abstract:Objective To evaluate the accuracy of transarticular screw fixation using intraoperative three?dimensional fluoroscopy?based navigation (ITFN) and to evaluate the clinical outcomes of this treatment method. Methods Data of 56 patients (26 males and 30 females) with atlantoaxial instability who were treated by C1, 2 transarticular screw fixation using ITFN from No?vember 2005 to October 2015 were retrospectively analyzed. The mean age of the patients was 44.5 years (range, 9-68 years). There were 44 cases with congenital malformation, 4 with old odontoid fracture, 7 with spontaneous dislocation, and 1 with rheuma?toid arthritis. C2 isthmus width and height were measured on preoperatively obtained CT scans, and screw positioning was evaluat?ed on postoperatively obtained CT scans, and classified into three types: ideal position (type I), acceptable position (type II) and un?acceptable position (type III). A novel grading system is proposed based on previous study and grading system, and the difficulty of placing C1, 2 transarticular screw using ITFN was classified into three types: easy (total score 0), median (total score 1) and hard (to?tal score 2, 3). Pain scores were assessed using the visual analogue scale. Myelopathy was assessed using the Nurick scale and Odom’s criteria. Results The isthmus width was 5.46±1.86 mm on the right side and 5.38±1.36 mm on the left side. The isth?mus height was 4.89±1.33 mm on the right side and 4.97±1.17 mm on the left side. According to the grading system, 78, 11, and 23 of the sides were classified into easy, median and hard groups respectively. One hundred and seven transarticular screws were placed in 56 patients, and 71.03% of which were ideal screws, and 28.97% were acceptable screws. Five patients had unilateral screws placed. There was no significant difference in screw positioning among the three groups (χ2=0.46, 0.54, 1.18; P=0.50, 0.46, 0.28). The mean follow?up period was 44.7 months (range, 6-120 months). At the latest follow?up, according to Nurick score, there are 30 patients scoring 0, 25 patients scoring 1, and 1 patient scoring 2. According to Odom’s criteria, outcomes were as follows:excellent, 66.1%; good, 26.8%; fair, 7.1%; and poor, 0%. All patients with preoperative neck pain had symptom relief or improve?ment, with more than 89.33% improvement in visual analogue scale scores. No dural laceration, injury to the vertebral artery, spinal cord, or hypoglossal nerve were noted. Conclusion ITFN is a safe, accurate, and effective tool for transarticular screw placement in pa?tients with atlantoaxial instability.
Application of Intraoperative Computed Tomography with or Without Navigation System in Surgical Correction of Spinal Deformity A Preliminary Result of 59 Consecutive Human Cases

Abstract:Study Design. A retrospective analysis of patients undergoing spinal deformity correction surgery by the assistance of intraoperative computed tomography (iCT) with or without navigation system.Objective. To share our preliminary experience and analysis of the iCT navigation system applied to spinal deformity surgery.Summary of Background Data. The iCT navigation system has been shown to improve accuracy and safety in posterior instrumentation. It not only decreased the operation time but also prevented excessive radiation exposure to the medical staff. To date, there are only few reports about the application of the iCT navigation system in spinal deformity surgery.Methods. From April 2009 to September 2010, 59 patients who had a diagnosis of scoliosis, kyphosis, or scoliokyphosis and underwent iCT-assisted surgical correction were included. Without randomization, 28 patients were operated with the iCT-navigation system, and the other 31 patients were operated with standard procedure under iCT assistance. The detailed procedures, preoperative and intraoperative images were illustrated. The accuracy of screw placement, time for screw insertion, postoperative correction rate, and iCT scanning data were analyzed.Results. There were significant differences between 2 groups in (1) the preoperative Cobb angle (76.2 degrees and 62.6 degrees in the navigation and non-navigation groups), (2) the accuracy and the revision rate of thoracic pedicle screws and total pedicle screws, and (3) the average screw insertion time. The breach rate and the revision rate of thoracic pedicle screws and total pedicle screws were significantly lower and the average screw insertion time was significantly lesser in the navigation group than in the non-navigation group. There were no statistically significant difference in (1) the breach rate and the revision rate of lumbar pedicle screws, (2) the mean iCT scanning time and time-out, (3) the mean number of fusion segments, (4) the mean number of iCT scans, and (5) the postoperative correction rate. Complications were encountered in 2 patients in the non-navigation group but none in the navigation group. There was no reoperation due to implant malposition in both groups.Conclusion. The iCT navigation system provides desirable accuracy of posterior spinal instrumentation for patients during surgical correction of spinal deformity without radiation exposure to the medical staff, especially in thoracic spine instrumentation. Meanwhile, the iCT in itself is an effective means of assessing complex instrumentation of the spinal deformity.
Thoracolumbar instrumentation with CT-guided navigation (O-arm) in 270 consecutive patients: accuracy rates and lessons learned

Abstract:Object: Thoracolumbar instrumentation has experienced a dramatic increase in utilization over the last 2 decades. However, pedicle screw fixation remains a challenging undertaking, with suboptimal placement contributing to postoperative pain, neurological deficit, vascular complications, and return to the operating suite. Image-guided spinal surgery has substantially improved the accuracy rates for these procedures. However, it is not without technical challenges and a learning curve for novice operators. The authors present their experience with the O-arm intraoperative imaging system and share the lessons they learned over nearly 5 years. Methods: The authors performed a retrospective chart review of 270 consecutive patients who underwent thoracolumbar pedicle screw fixation utilizing the O-arm imaging system in conjunction with StealthStation navigation between April 2009 and September 2013 at a single tertiary care center; 266 of the patients underwent CT scanning on postoperative Day 1 to evaluate hardware placement. The CT scans were interpreted prospectively by 3 neuroradiologists as part of standard work flow and retrospectively by 2 neurosurgeons and a senior resident. Pedicle screws were evaluated for breaches according to the 3-tier classification proposed by Mirza et al. Results: Of 270 patients, 266 (98.5%) were included in the final analysis based on the presence of a postoperative CT scan. Overall, 1651 pedicle screws were placed in 266 patients and yielded a 5.3% breach rate; 213 thoracic and 1438 lumbosacral pedicle screws were inserted with 6.6% and 5.1% breach rates, respectively. Of the 87 suboptimally placed screws, there were 13 Grade 1, 16 Grade 2, and 12 Grade 3 misses as well as 46 anterolateral or "tip-out" perforations at L-5. Four patients (1.5%) required a return to the operating room for pedicle screw revision, 2 of whom experienced transient radicular symptoms and 2 remained asymptomatic. Interestingly, the pedicle breach rate was higher than anticipated at 13.21% for the 30 patients over the initial 6-month period with the O-arm. After certain modifications to the authors' technique, the subsequent 30 patients experienced a statistically significant decrease in breach rate at 5.6% (p = 0.014). Conclusions: Image-guided spinal surgery can be a great option in the operating room and provides high pedicle screw accuracy rates. With numerous systems commercially available, it is important to develop a systematic approach regardless of the technology in question. There is a learning curve for surgeons unfamiliar with image guidance that should be recognized and appreciated when transitioning to navigation-assisted spinal surgery. In fact, the authors' experience with a large patient cohort suggests that this learning curve may be more significant than previously reported.

Summary of background data: Computer assisted stereotactic navigation for placement of spinal instrumentation has been widely studied and implemented in the thoracic and lumbar spine. However less literature exists regarding the use of computer assisted navigation for posterior cervical instrumentation, particularly with lateral mass fixation. Here we present the first prospective study of navigated cervical lateral mass screw placement for cervicothoracic fusion. Methods: Patients who met indications for posterior cervical fusion were screened, consented, and enrolled preoperatively for instrumentation with Medtronic Infinity Occipital-Cervical-Thoracic implants, with use of intraoperative O-arm and stereotactic Stealth navigation. Postoperative CTs of the instrumented levels were obtained during the same hospital admission. Primary outcome of the trial was safety. Secondary outcomes were screw accuracy assessed by Gertzbein-Robbins grade, neurologic exams, and patient reported outcomes on the PROMIS 29 questionnaire. Results: A total of 50 patients underwent surgery, and 557 screws were placed. There were no adverse events related to the use of navigation or screw malposition. Gertzbein-Robbins grade A or B placement comprised 95% of navigated screws. There was a decrease in positive Hoffmann sign rate postoperatively, and sensory and motor exams remained stable. There was improvement in patient reported pain and sleep domains. Conclusions: Navigation for cervicothoracic instrumentation is safe overall and leads to high rates of accurately placed screws. Longer term follow up could provide more insight to whether the use of this technology results in durable improvement in spinal alignment parameters and patient reported outcomes. Intraoperative, full-rotation, three-dimensional image (O-arm)-based navigation system for cervical pedicle screw insertion

Abstract:Object: The aim of this study was to retrospectively evaluate the reliability and accuracy of cervical pedicle screw (CPS) placement using an intraoperative, full-rotation, 3D image (O-arm)-based navigation system and to assess the advantages and disadvantages of the system. Methods: The study involved 21 consecutive patients undergoing posterior stabilization surgery of the cervical spine between April and December 2009. The patients, in whom 108 CPSs had been inserted, underwent screw placement based on intraoperative 3D imaging and navigation using the O-arm system. Cervical pedicle screw positions were classified into 4 grades, according to pedicle-wall perforations, by using postoperative CT. Results: Of the 108 CPSs, 96 (88.9%) were classified as Grade 0 (no perforation), 9 (8.3%) as Grade 1 (perforations < 2 mm, CPS exposed, and < 50% of screw diameter outside the pedicle), and 3 (2.8%) as Grade 2 (perforations between ≥ 2 and < 4 mm, CPS breached the pedicle wall, and > 50% of screw diameter outside the pedicle). No screw was classified as Grade 3 (perforation > 4 mm, complete perforation). No neurovascular complications occurred because of CPS placement. Conclusions: The O-arm offers high-resolution 2D or 3D images, facilitates accurate and safe CPS insertion with high-quality navigation, and provides other substantial benefits for cervical spinal instrumentation. Even with current optimized technology, however, CPS perforation cannot be completely prevented, with 8.3% instances of minor violations, which do not cause significant complications, and 2.8% instances of major pedicle violations, which may cause catastrophic complications. Therefore, a combination of intraoperative 3D image-based navigation with other techniques may result in more accurate CPS placement.
Navigated percutaneous screw fixation of the pelvis with O-arm 2: two years' experience

Abstract:Aim To evaluate the case series of the patients operated with percutaneous fixation by the navigation system based on 3D fluoroscopic images, to assess the precision of a surgical implant and functional outcome of patients. Methods A retrospective study of pelvic ring fractures in a 2-year period included those treated with the use of the O-Arm 2 in combination with the Stealth Station 8. Pelvic fractures were classified according to the Tile and the Young-Burgess classification. All patients were examined before surgery, with X-rays and CT scans, and three days after surgery with additional CT scan. The positioning of the screws was evaluated according to the Smith score, the outcome with the SF-36. Results Among 24 patients 18 were with B and six with C type fracture according to Tile, while eight were with APC, 10 LC, and six with VS type according to Young-Burgess classification. All patients were treated in the supine position, except two. A total of 41 iliosacral or transsacral screws and five anterior pelvic ring screws were implanted. The medium surgical time per screw was 41 minutes. There was a perfect correspondence of screw scores value from post-operative CT and intraoperative fluoroscopy. The mean screw score value was 0.92. There were no cases of poor positioning. The median follow-up was 17.5 months. The patients were satisfied with their health condition on SF-36. Conclusion The use of the O-arm guarantees great precision in the positioning of the screws and reduced surgical times with excellent clinical results in patients.
EVALUATION OF CLINICAL OUTCOMES AND ACCURACY IN TRANSPEDICULAR SCREW FIXATION USING INTRAOPERATIVE CT-GUIDED NAVIGATION FOR LUMBAR SPONDYLOLISTHESIS

Abstract:The use of intraoperative navigation and robotic surgery for minimally invasive lumbar fusion has been increasing over the past decade. The aim of this study is to evaluate postoperative clinical outcomes, intraoperative parameters, and accuracy of pedicle screw insertion guided by intraoperative navigation in patients undergoing lumbar interbody fusion for spondylolisthesis. Patients who underwent posterior lumbar fusion interbody using intraoperative 3D navigation since December 2021 were included. Visual Analogue Scale (VAS), Oswestry Disability Index (ODI), and Short Form Health Survey-36 (SF-36) were assessed preoperatively and postoperatively at 1, 3, and 6 months. Screw placement accuracy, measured by Gertzbein and Robbins classification, and facet joint infringement, measured by Yson classification, were assessed by intraoperative Cone Beam CT scans performed at the end of instrumentation. Finally, operation time, intraoperative blood loss, hospital stay, and screw insertion time were evaluated. This study involved 50 patients with a mean age of 63.7 years. VAS decreased from 65.8±23 to 20±22 (p<.01). ODI decreased from 35.4%±15 to 11.8%±14 (p<.01). An increase of SF-36 from 51.5±14 to 76±13 (p<.01) was demonstrated. The accuracy of “perfect” and “clinically acceptable” pedicle screw fixation was 89.5% and 98.4%, respectively. Regarding facet violation, 96.8% of the screws were at grade 0. Finally, the average screw insertion time was 4.3±2 min, hospital stay was 4.2±0.8 days, operation time was 205±53 min, and blood loss was 169±107 ml. Finally, a statistically significant correlation of operation time with hospital stay, blood loss and placement time per screw was found. We demonstrated excellent results for accuracy of pedicle screw fixation and violation of facet joints. VAS, ODI and SF-36 showed statistically significant improvements from the control at one month after surgery. Navigation with intraoperative 3D images represents an effective system to improve operative performance in the surgical treatment of spondylolisthesis.

Abstract:Background The complicated anatomy of the cervical spine and the variation among pedicles reduces the accuracy and increases the risk of neurovascular complications associated with screw implantation in this region. In this study, we compared the accuracy of cervical (C2-C7) pedicle screw fixation assisted by X-ray fluoroscopy, computed tomography (CT)-based navigation, or intraoperative three-dimensional (3D) C-arm navigation.Methods This prospective cohort study was performed in 82 consecutive patients who underwent cervical pedicle screw fixation. The accuracy of screw insertion was assessed by postoperative CT scan with 3D reconstruction. The accuracy of screw insertion was assessed as: excellent (screw completely within pedicle); acceptable (5 1 mm screw outside pedicle cortex); poor (>1 mm screw outside pedicle cortex).Results A total of 145 screws were inserted in 24 patients who underwent C-arm fluoroscopy. Of these, 96 screws (66.2%) were excellent, 37 (25.5%) were acceptable, and 12 (8.3%) were poor. One hundred and fifty-nine screws were inserted in 29 patients in the CT-based navigation group. Among these, 141 (88.7%) were excellent, 14 (8.8%) were acceptable, and 4 (2.5%) were poor. A total of 140 screws were inserted in 29 patients in the intraoperative 3D C-arm navigation group, of which 127 (90.7%) were excellent, and 13 (9.3%) were acceptable. No severe or permanent neurovascular complications associated with screw insertion were observed in any patient.Conclusions CT-based and intraoperative 3D C-arm navigation were similarly accurate, and were both significantly more accurate than C-arm fluoroscopy for guiding cervical pedicle screw fixation. They were able to accurately guide the angle and depth of screw placement using visual 3D images. These two techniques are therefore preferable for high-risk cervical pedicle screw fixation. The ease and convenience of intraoperative 3D C-arm navigation suggests that it may replace virtual-fluoroscopy and CT-based navigation systems in future clinical applications. Abstract:OBJECTIVE: This study aimed to evaluate the accuracy of screw placement and clinical outcomes in patients undergoing occipitocervical fusion. METHODS: Patients who underwent occipitocervical fusion with O-arm-based navigation were retrospectively reviewed between January 2015 and December 2017. The patients' characteristics, clinical and radiographic outcomes, and surgical complications were recorded and analyzed. Cervical screw insertion accuracy was evaluated using the Richter scale. RESULTS: Thirty consecutive patients (11 male and 19 female) with an average treatment age of 40.03 +/- 15.19 years were studied. The most common cause was atlantoaxial instability combined with Chiari malformation (63.33%). Weakness (76.67%) and paresthesia (70.00%) were the most common symptoms. Eighty-six occipital and 139 cervical screws were placed using an O-arm-assisted navigation system. In total, 130 (93.53%) cervical screws were graded as group A and 9 (6.47%) as group B. The optimal accuracy rate was 88.41% (61 of 69 screws) in the first 15 patients and 98.57% (69 of 70 screws) in the subsequent 15 patients. The mean follow-up time was 7.50 +/- 5.70 months. The mean Japanese Orthopedic Association scores were 13.30 +/- 2.41 preoperatively and 15.30 +/- 1.60 at final follow-up (P < 0.001), and the mean recovery rate in the Japanese Orthopedic Association score was 53.26 +/- 33.82%. Clinical improvement was seen in 25 patients (83.33%), whereas no change was observed in 5 patients (16.67%). The overall complication rate was 6.67% (2/30), with 1 intraoperative vertebral artery injury and 1 postoperative screw loosening. CONCLUSION: Occipitocervical fusion with O-arm-based navigation is effective and safe for treating instability of the craniovertebral junction. Intraoperative navigation can help surgeons insert screws accurately.
Feasibility of mixed reality–based intraoperative three-dimensional image-guided navigation for atlanto-axial pedicle screw placement

Abstract:This study aimed to evaluate the safety and accuracy of mixed reality–based intraoperative three-dimensional navigated pedicle screws in three-dimensional printed model of fractured upper cervical spine. A total of 27 cervical model from patients of upper cervical spine fractures formed the study group. All the C1 and C2 pedicle screws were inserted under mixed reality–based intraoperative three-dimensional image-guided navigation system. The accuracy and safety of the pedicle screw placement were evaluated on the basis of postoperative computerized tomography scans. A total of 108 pedicle screws were properly inserted into the cervical three-dimensional models under mixed reality–based navigation, including 54 C1 pedicle screws and 54 C2 pedicle screws. Analysis of the dimensional parameters of each pedicle at C1/C2 level showed no statistically significant differences in the ideal and the actual entry points, inclined angles, and tailed angles. No screw was misplaced outside the pedicle of the three-dimensional printed model, and no ionizing X-ray radiation was used during screw placement under navigation. It is easy and safe to place C1/C2 pedicle screws under MR surgical navigation. Mixed reality–based navigation is feasible within upper cervical spinal fractures with improved safety and accuracy of C1/C2 pedicle screw insertion.
