Abstract:It has been extensively investigated that the chewing of smokeless tobacco (SLT) products may enhance the inflammation of the oral cavity. The aim of the present study is to evaluate the relationship between nickel (Ni) exposure via different SLT products with oral cancer (different sites) incidence in the population of Sindh, Pakistan. The different brands of SLT products (mainpuri, gutkha, and moist snuff) commonly consumed by the studied population were analyzed for Ni contents. The biological samples of oral cancer patients and noncancerous control subjects of both genders, who have or have not consumed SLT products, were collected. The concentration of Ni in biological samples and SLT products were measured by electrothermal atomic absorption spectrophotometer after microwave-assisted acid digestion. The validity and accuracy of the methodology were checked by using certified reference materials. The results of this study showed that the Ni level was significantly higher in scalp hair and blood samples of oral cancer patients compared to controls (P < 0.01). The study suggested that exposure of Ni as a result of chewing different SLT products may be synergistic with risk factors associated with oral cancer.
Evaluation of cadmium and zinc in biological samples of tobacco and alcohol user male mouth cancer patients

Abstract:There is compelling evidence in support of positive associations between zinc (Zn) deficiency and high accumulation of cadmium (Cd) in biological samples of different cancerous patients. The incidence of mouth cancer has been increased among people who possess habit of drinking alcohol, tobacco smoking and chewing with gradients in Pakistan. In the present study, Cd and Zn contents of blood and scalp hair samples of 92 male mouth cancer patients and 78 referents of same age group (range 35-65 years), socio-economic status, localities and dietary habits. Among referent male subjects, 56 consumed tobacco (smoking/chewing), 22 had none of smoking and chewing tobacco habits, while none of them admitted for alcohol drinking. The scalp hair and whole blood samples were oxidized by 65% nitric acid and 30% hydrogen peroxide (2:1) ratio in microwave oven. The Zn in digests was determined by flame atomic absorption while Cd was determined by electrothermal atomic absorption spectrometry. The average Cd concentration was higher, while level of Zn was lower in the blood and scalp hair samples of mouth cancer patients as compared to referent subjects (p < .001). The male referents consuming tobacco (chewing and smoking) have high level of Cd in both their biological samples as compared to those not smoking or chewing tobacco (p < .012). The Cd/Zn ratio in both biological samples was found to be higher in patients than in referent subjects.

Abstract:A large number of epidemiologic studies have been undertaken to identify potential risk factors for cancer, amongst which the association with cadmium has received considerable attention. There is compelling evidence in support of positive associations between cadmium and risk of lung cancer. In present study we measured the concentration of Cd in whole blood and scalp hair samples of 120 male lung cancer patients (smokers) and 150 controls or referents (smokers and nonsmokers) from different cities of Pakistan. Both referents and patients were of same age group (ranged 40-70 years), socio-economic status, localities and dietary habits. The scalp hair and whole blood samples were oxidized by 65% nitric acid: 30% hydrogen peroxide (2:1) ratio in microwave oven. To check the validity of the proposed method, a conventional wet acid digestion method was used to obtain total Cd concentration in certified samples of human hair BCR 397 and Clincheck control-lyophilized human whole blood. All digests were analyzed for Cd concentration by electrothermal atomic absorption spectrometer (ETAAS). The results of this study showed that the average Cd concentration was higher in the blood and scalp hair of lung cancer patients at different stages as compared to controls (p<001). The smoker referents have high level of Cd in both biological samples as compared to nonsmoker subjects. These results illustrate that the patients who continued smoking after confirmed diagnosis of lung cancer have 34.2-67.26 and 22.4-57.3% more Cd in blood samples and scalp hair than lung cancer patients who cease smoking. This study is compelling evidence in support of positive associations between cadmium, cigarette smoking and lung cancer risk.
Chromium Exposure in the Adult Population, Consuming Different Types of Smokeless Tobacco Products in Pakistan

Abstract:The pervasive smokeless tobacco (SLT) consumption and diseases related to its use is a hot topic for the public discussion. In this study, concentrations of chromium (Cr) were measured in different SLT products [snuff (dry and moist), mainpuri, and gutkha] offered and used in Pakistan. The current study was also designed to assess the Cr levels in the biological (scalp hair and blood) samples of male and female subjects, age ranged from 25 to 60 years, chewing different SLT products. For comparative purpose, the healthy persons of the same age group, who did not consume any SLT products, were selected as referents. The concentrations of Cr in SLT products and biological samples were measured by electrothermal atomic absorption spectrophotometer after microwave-assisted acid digestion. The validity and accuracy of the methodology were checked by certified reference materials (CRMs). The resulted data indicated that the adult persons, who consumed different SLT products, have 2-3 fold higher levels of Cr in biological samples as compared to referent subjects (p < 0.01). The persons, who chew/sniff different SLT products, have 50-80 and 42-82 % higher levels of Cr in their scalp hair and blood samples as related to referents. The daily intake of Cr is lower as compared to the recommended value of 50-200 μg/day. It was expected that 10 g consumption of various kinds of SLT products (snuff, mainpuri, and gutkha) may subsidize 21.2-220, 17.7-122, and 18.4-273 % of the recommended daily intake of Cr, respectively.

Abstract:Background Smokeless tobacco (SLT) is a tobacco-based product consumed without burning or smoking. Snuff, commonly known as naswar, is a widely used SLT product in Pakistan. This study is designed to investigate the concentrations of lead (Pb) and cadmium (Cd) in snuff and their associated health risks to consumers. Methods Blood samples from snuff consumers and non-consumers were analyzed for Pb and Cd concentrations. Additionally, samples of drinking water and wheat flour were also analyzed for Pb and Cd. Health risk assessments were calculated using hazard quotients (HQ) and incremental lifetime cancer risks (ILCR) for Pb and Cd. Results Snuff consumers had significantly higher blood-Pb and blood-Cd levels compared to non-consumers. Approximately 38.5% of snuff consumers were found to exceed blood-Pb levels from the safe limits (10 μg dL−1) of the World Health Organization (WHO), while 48.9% of snuff consumers were found to exceed blood-Cd levels from the WHO safe range of 0.03–0.12 μg dL−1. Snuff samples contained Pb and Cd concentrations ranging from 5.3 to 22.4 μg g−1 and 0.5 to 2.2 μg g−1, respectively. Drinking water and wheat flour samples were found within the safe limits for Pb and Cd concentration, suggesting snuff as the primary source of exposure. Health risk assessments indicated elevated HQ values, for Pb (38.9%) and Cd (14.3%) detected in snuff, surpassing the safe HQ threshold of 1. Concerning ILCR from snuff, values for Pb were within the acceptable range, but alarmingly 100% of samples for Cd exceeded the safe limits of USEPA (1.0×10−4), indicating a significant cancer risk in snuff users. Conclusion This study highlights the exceeded Pb and Cd concentrations in snuff as well as also reporting the significant carcinogenic and non-carcinogenic health risks to consumers. These findings emphasized the urgent need for regulatory bodies to ensure the safety of snuff before selling in the market. Public Health Professionals should also initiate educational campaigns to raise awareness among consumers and manufacturers about various health risks associated with the snuff.

Periodontitis, oral hygiene habits, and risk of upper aerodigestive tract cancers: a case-control study in Maharashtra, India

Abstract:Objective: Chronic destructive periodontitis, a cause of systemic inflammation, affects some 10% to 15% of adults across the globe, with severity of disease increasing with age. The aim of this study was to explore the relationship between periodontitis and oral hygiene habits and upper aerodigestive tract (UADT) cancers. Study design: We conducted a case-control study, which included 240 UADT cancer cases and 240 controls matched by gender and age (±5 years) from 2 different hospitals in Pune, India. In-person interviews and intraoral examinations were conducted for all patients. Results: Severe periodontitis and greater than 5 missing teeth were associated with a significant risk of UADT cancers (adjusted odds ratio [OR] 2.25, 95% confidence interval [CI] 1.12-4.91; adjusted OR 3.28, 95% CI 1.95-5.49). Among the self-reported oral hygiene habits, dental checkups only at the time of pain was associated with an elevated risk for UADT cancers (adjusted OR 4.12; 95% CI 2.63-6.47). Topical application of mishri (black powder obtained by roasting and grinding tobacco) on gums (adjusted OR 3.06; 95% CI 1.75-5.35) and toothbrushing frequency less than once daily; (adjusted OR 2.09; 95% CI 1.27-3.45) were also associated with an elevated risk. Furthermore, the habit of ever chewing tobacco was associated with an elevated risk of severe periodontitis. Conclusions: Severe periodontitis is associated with an elevated risk for UADT cancers, and tobacco chewing strengthens this association in this population.

Abstract:Oral cancer is a globally widespread cancer that features among the three most prevalent cancers in India. The risk of oral cancer is elevated by factors such as tobacco consumption, betel-quid chewing, excessive alcohol consumption, unhygienic oral condition, sustained viral infections, and also due to dysbiosis in microbiome composition of the oral cavity. Here, we performed an oral microbiome study of healthy and oral cancer patients to decipher the microbial dysbiosis due to the consumption of smokeless-tobacco-based products and also revealed the tobacco-associated microbiome. The analysis of 196 oral microbiome samples from three different oral sites of 32 healthy and 34 oral squamous cell carcinoma (OSCC) patients indicated health status, site of sampling, and smokeless tobacco consumption as significant covariates associated with oral microbiome composition. Significant similarity in oral microbiome composition of smokeless-tobacco-consuming healthy samples and OSCC samples inferred the possible role of smokeless tobacco consumption in increasing inflammation-associated species in oral microbiome. Significantly higher abundance of Streptococcus was found to adequately discriminate smokeless-tobacco-non-consuming healthy samples from smokeless-tobacco-consuming healthy samples and contralateral healthy site of OSCC samples from the tumor site of OSCC samples. Comparative analysis of oral microbiome from another OSCC cohort also confirmed Streptococcus as a potential marker for healthy oral microbiome. Gram-negative microbial genera such as Prevotella , Capnocytophaga , and Fusobacterium were found to be differentially abundant in OSCC-associated microbiomes and can be considered as potential microbiome marker genera for oral cancer. Association with lipopolysaccharide (LPS) biosynthesis pathway further confirms the differential abundance of Gram-negative marker genera in OSCC microbiomes.

Abstract:Background: This study delves into the intricate landscape of oral cancer, a global concern with a high incidence in Asian countries. We focus on oral squamous cell carcinoma (OSCC), primarily driven by the consumption of betel nut and its derivatives. OSCC often arises from premalignant lesions like oral submucous fibrosis (OSF). In Pakistan, OSCC is prevalent among men due to various addictive substances, including smokeless tobacco and chewing materials. Mutations in tumor suppressor genes, such as TP53 and p21, play crucial roles in this malignancy's development. We also explore the involvement of TUSC3 gene deletion in OSCC and OSF. Methods: In this study we investigated demographics, TUSC3 gene expression, deletion analysis, and TP53 and p21 genetic alterations in OSCC and OSF patients (blood and tissue of 50 samples in each condition) who had tobacco derivates usage history. The association analysis was carried out mainly through PCR based genotyping. Results: The study's patient cohort (OSCC and OSF) displayed a wide age range from 13 to 65 years (Mean = 32.96 years). Both conditions were more prevalent in males, with a male-female ratio of approximately 2.5:1. Chewing habits analysis revealed high frequencies of gutka use in both OSF and OSCC patients. TUSC3 expression analysis in OSCC cell lines indicated significant downregulation. Genotyping showed no TUSC3 deletion in OSF cases, but a deletion rate of over 22% in OSCC tissue samples. Analysis supported a significant association of TUSC3 deletion with OSCC development but not with OSF. Polymorphism in p53 exon 4 and p21 (rs1801270) were significantly associated with both OSCC and OSF, adding to their pathogenesis. Our findings further revealed a strong correlation between TUSC3 deletion and the excessive use of tobacco and related products, shedding light on the genetic underpinnings of OSCC development. Conclusions: Notably, our study provides a crucial insight into genetic aspects underlying OSCC and OSF in response of addictive consumption of areca nut, betel quid, and tobacco derivatives. A significant association between TUSC3 deletion and OSCC development, along with polymorphisms in TP53 and p21, underscores the importance of further research into the molecular mechanisms driving oral cancer progression for improved diagnosis and treatment outcomes.

Abstract:Back ground: Smokeless Tobacco (SLT) can cause oral mucosal lesions such as keratosis, gingival inflammation, periodontal inflammation, alveolar bone damage, dental caries, tooth abrasion, staining of tooth structure, dysplasia, and oral cancer. Tobacco has been linked to severe consequences for overall health. Objective: To identify the oral and periodontal manifestation of smokeless tobacco (SLT) users in Khartoum locality. Methodology: A descriptive cross-sectional hospital based study was done among 80current smokeless tobacco patients attending the outpatients and periodontology clinic in ACTH (Academy Charity Teaching Hospita), KDTH(Khartoum Dental Teaching Hospital), and alhekmah health center from January to March 2023, patients undergo oral examination after obtaining an informed consent, data collected using online Google form, pretested questionnaire and interviews. Data collected using Microsoft Excel spreadsheets and analyzed by SPSS version 23. Results: The study included mostly male participants (97.5%), aged 18-29 years old (31.3%). With lower education levels (31.3%). and working as unskilled workers (47.5%). Most participants put the snuff at buccal mucosa (42.5%), used for less than 10 years (41.3%), for 1-5 minutes (52.5%), and 1 pack per day (56.3%). The majority had dental caries (66.3%) with class I predominance (53.8%), inflammation (97.5%), recession (67.5%), staining (78.8%), and halitosis (73.8%). Mucosal hypertrophy was the most common mucosal change (71.3%). There was a statistical association between start time and plaque index (P value = 0.008), and quantity in packs and pocket depth (P value = 0.013). Conclusions: The study revealed that SLT has sever and considerable adverse effects on oral health and periodontal tissues.

Abstract:Oral Pre-Cancer Lesions (OPLs) including leukoplakia, erythroplakia, and submucous fibrosis denote biochemical and histopathologically altered changes in the oral mucosa marked by subcellular and structural anomalies evocating of potential for a malignant transformation, which is primarily caused by tobacco exposure. Early diagnosis is of paramount importance to halt the progression of premalignant lesions to high-grade dysplasia and even oral cancer. Objective: To find the diagnostic and prognostic potential of biochemical and haematological markers in Tobacco Users (TU) with OPL. Methods: PRISMA guidelines were followed to perform this systematic review. After retrieving 170 epidemiological studies published from 2013 to 2023, through multiple databases (PubMed, Google Scholar, Sci-hub, and Science Direct), 21 were included to determine the potential of biochemical and haematological markers in risk stratification and early detection of OPL. Results: According to the following systematic review, extracted data showed specific biochemical and haematological indicators that could serve as markers in risk stratification and early detection of OPL. The OPL group exhibited significantly higher levels of biochemical markers IL-6, IL-8, TNF-α, HCC-1, PF-4, FRR, TP, MDA, MMP-12, and Ceruloplasmin and hematological markers NLR, PLR, CRP, ESR, WBC, and low Hb as compared to the control group. Following risk stratification, a group with older age, tobacco association with OPL, and elevated levels of markers were categorised as a higher-risk group. Conclusions: The biochemical and haematological markers are potential promising markers in the early detection of OPL from malignant lesions with diagnostic and prognostic significance.

Abstract:ABSTRACTS Introduction: Almost a third of the global load of oral squamous cell carcinoma (OSCC) occurs in India and can be attributed to the widespread use of tobacco and tobacco-related products in this part of the sub-continent. Materials and Methods: Records of 274 patients of OSCC treated between January 2018 and December 2019 in our institute were analyzed for the study for history of tobacco abuse and distribution of associated demographic, clinical, and pathological factors. Results: The age of the patients in the study ranged from 31 to 82 years with a median age of 60 years. The ratio of oral cancer in males: females was 3:1. Exposure to tobacco was seen in the majority of patients (89%) who reported with oral carcinoma. Smokeless tobacco in the form of gutka was the most common abused tobacco, followed by bidi in our study. Tongue and buccal mucosa (38% and 36%, respectively) were the most common sites. Significant statistical correlation of tobacco use was seen with age, gender, clinical, and pathological tumor stages. Conclusion: With tobacco being the main cause of OSCC, further studies with a larger number of patients and preferably with a comparison arm of non-tobacco OSCC would help in elucidating the exact clinical and statistical correlation of tobacco with the clinicopathological factors.

Oral submucous fibrosis (OSMF) is a chronic precancerous lesion which has the highest rates of malignant transformation. Therefore, the present study was conducted among industrial workers to observe the effect of various forms of smokeless tobacco (SLT) usage and development of OSMF. Materials and Methods: The present cross-sectional study was conducted among 3998 industrial after obtaining approval from concerned authorities. A self-administered questionnaire approved by specialists was used to collect data from the study participants. The clinical diagnosis (the presence of fibrous bands and blanching) was performed by trained examiners who were calibrated in the department. Mouth opening was recorded by Vernier caliper. Statistical analysis was done using the Chi-square test and statistical significance was set at P ≤ 0.05. Results: The mean age of the study population was 38.341 ± 9.89 years. OSMF was reported in 50.2% (1149) of participants who were consuming one or other forms of SLT. Fibrous bands were seen in 59.6% of participants chewing areca nut, 56.8% of participants who consumed pan masala, and 50.6% of participants consuming khaini. 48.9% of participants who chewed gutkha developed blanching over their intraoral mucosa. Fibrous bands and blanching of the oral mucosa were observed in 89.4% and 88.3% of participants, respectively, who consumed all four types of SLT. Conclusion: Consumption of SLT has a strong correlation with the development of OSMF, especially with the use of gutkha, pan masala, and khaini. There is a need for public education programs among such vulnerable population groups.

Trace elements, both essential and non-essential, play significant roles in various biological processes, with imbalances potentially contributing to carcinogenesis. This study aims to investigate serum concentrations of selected metals in cancer patients and compare these levels with those of a healthy control group, highlighting the potential link between metal exposure and cancer risk. Objective: To quantify and compare the serum concentrations of sodium (Na), calcium (Ca), iron (Fe), cadmium (Cd), and arsenic (As) in cancer patients and a control group, thereby assessing the potential association between elevated levels of specific metals and cancer incidence. Methods: This cross-sectional study analyzed blood samples from cancer patients (n=30) and a matched control group (n=30) from the Jamshoro and Hyderabad areas. Inductively Coupled Plasma Mass Spectrometry (ICP-MS) was utilized to measure serum concentrations of Na, Ca, Fe, Cd, and As. Participants' demographic data, including age, gender, place of residence, degree of education, cancer type, and water sources, were collected through questionnaires. Statistical analyses were conducted to compare metal concentrations between groups, with P-values 0.05), with an average concentration exceeding 32.04 μg/L. Cd levels were also higher in the cancer group (0.0046 ppm) compared to controls (0.0000 ppm), contrary to expectations. Conversely, no significant difference was found in As levels between groups. Serum Fe concentrations were substantially higher in cancer patients (P > 0.4949), with Ca levels in cancer patients also elevated (3.302 ppm). Conclusion: The study found significant discrepancies in serum metal concentrations between cancer patients and healthy controls, suggesting an association between elevated levels of certain metals and increased cancer risk. These findings emphasize the need for further investigation into the role of metal exposure in carcinogenesis and the potential for incorporating metal concentration assessments into cancer risk evaluation protocols.
Effect of Smoking and Tobacco Chewing on Periodontal Disease and Non-Surgical Treatment Outcome: A Clinical and Biochemical Study

Abstract:Objective: The purpose of this study was to compare effects of smoking and smokeless forms of tobacco consumption (tobacco chewing) on periodontal disease parameters and response of these subjects to non-surgical periodontal therapy. Methods: One hundred sixty-eight patients with chronic periodontitis were screened for the study. Eighteen patients were excluded as they decided to quit the tobacco habit. One hundred fifty patients fulfilling the inclusion and exclusion criteria were grouped as: Group 1, 50 smokers; Group 2, 50 tobacco chewers; and Group 3, 50 non-smokers, non-tobacco chewers (controls). Scaling and root planing was performed at the initial visit as a part of initial therapy. The clinical parameters recorded at baseline, 1 month, 2 months and 3 months were plaque index (PI), gingival index (GI), probing depth (PD), gingival recession (GR), and gingival crevicular fluid (GCF) measurement. Results: With respect to the comparison between smokers and tobacco chewers, smokers had significantly more probing depth at baseline examination, while tobacco chewers had more gingival recession. Gingival inflammation, response to non-surgical treatment and oral hygiene maintenance were more suppressed in smokers as compared to tobacco chewers. Conclusion: Tobacco consumption in both forms affects the severity of periodontal disease. It affects the response of periodontal tissues to non-surgical treatment. In addition it leads to poorer oral hygiene and hampers maintenance of oral hygiene.

Abstract:Abstract The amino acid analysis of pea embryo histone IV showed the presence of 102 residues distributed as follows: Lys10, His2, Arg15, Asp5, Thr7, Ser2, Glu6, Pro1, Gly17, Ala7, Val8, Met1, Ile7, Leu8, Tyr4, and Phe2. The composition of calf thymus histone IV was identical except for the presence of 1 residue of e-N-methyllysine, an additional residue of valine, 1 residue less of arginine and isoleucine, and possibly 1 less residue of glycine. Sequence studies on an 18-residue cyanogen bromide peptide from both histones established the identical COOH-terminal sequence -Met-Asp-Val-Val-Tyr-Ala-Leu-Lys-Arg-Gln-Gly-Arg-Thr-Leu-Tyr-Gly-Phe-Gly-Gly-COOH for both proteins. The identity of this region and the similar amino acid compositions suggest that the remainder of the sequence of this histone from both the animal and the plant must be closely related. The evolutionary and functional implications of this remarkable similarity are discussed.
Assessment of the Influence of Risk Factors on the Incidence of Oral Squamous Cell Carcinoma (OSCC) in the Northern Pakistani Population

Abstract:Background: Oral squamous cell carcinoma (OSCC) is the most common type of oral cancer, accounting for over 94 percent of all cases, with substantial disparities in occurrence among South Asian countries. In Pakistan its occurrence rate rises from last two decade. A combination of risk factor habit, duration and intensity doubles the rate of incidence. Methods: Fully informed consent was taken. Both genders having age 18-55 years and cases confirmed by biopsy reports were included in the study. A thorough history of the disease and risk factors were taken and labelled (smoking, alcohol consumption, and betel nut chewing) accordingly. Data was analysed using SPSS 20. Gender base stratification was done using chi-square test and significance was defined as a P value of ? 0.05. Results: In present study 110 (73.3%) male and 40 (26.7%) female patients. Mean ±SD of age of OSCC patients was 47.00± 9.170 years. Among the participants 9 (6.0%) smokers, 39 (26.0%) consumed alcohol, use betel and 24 (16.0%) areca nut pan. Association of smoking with gender was statistically significant (p=0.013). Similarly, betel nut chewing also showed statistically significant correlation with gender (p=0.044). Both alcohol and snuff did not show any statistically significant correlation with gender. Conclusion: The public should be aware of the association of the risk factors and development of oral cancer. Public health measures should be taken to prevent smoking and chewing tobacco.
