Abstract:A quantitative understanding of the dose dependence of topical delivery is important to cosmetic and dermatological product development and to risk assessment for hazardous chemicals contacting the skin. Despite considerable research, predictive capability in this area remains limited. To this end we conducted an experimental skin absorption study of two closely related skin care agents, niacinamide (nicotinamide, NA) and methyl nicotinate (MN), and analyzed the results quantitatively using a transient diffusion model described separately (Yu et al. submitted for publication). Radiolabeled test compounds were solvent-deposited onto ex vivo human skin mounted in Franz diffusion cells over a dose range exceeding 4.5 orders of magnitude, and permeation was measured over a 1-4 day period. At low doses, the permeation rate of NA was approximately 60-fold lower than that of its lower melting, more lipophilic analog, MN; at high doses an even greater difference was observed. The difference can be qualitatively explained based on higher lipid solubility and lower crystallinity of MN relative to NA. Dissolution-limited mass transfer through a lipid layer at the SC surface is suggested. Relevance of the results to practical skin care formulations was confirmed by a parallel study of NA in an o/w emulsion.
The complexity of prognosis communication in heart failure: Patient and cardiologists' preferences in the outpatient clinical setting

Abstract:Background: The determination of prognosis in heart failure (HF) has focused primarily on the identification of potential biological and physiological markers and not on communication. High morbidity and mortality rates suggest that patients require prognostic information to assist in life planning. This study examined the preferences of both patients with HF and cardiologists for prognosis communication in the outpatient clinical setting, with the aim of guiding practitioners in undertaking prognosis conversations. Methods: Using qualitative descriptive techniques informed by a grounded theory approach, 32 patients with HF and 9 cardiologists from outpatient settings in Ontario, Canada were interviewed to identify convergent preferences for prognosis communication. Strategies to enhance methodological rigor were employed. Results: Individualized, context-specific prognosis communication between patients and cardiologists was preferred. Two main themes and ten related attributes were identified to describe convergent preferences; 1) Set the Stage for Prognosis Communication, and 2) Map the HF route. Attributes reflected the complex, dynamic, interactive and iterative nature of prognosis communication preferences. Conclusions: Prognosis communication occurs within a complex, adaptive healthcare system. While specific preferences exist, changing contextual elements within and outside of the system create conditions that require cardiologists to adjust their approach to individual patients. Patients with HF and cardiologists each have preferences that affect their willingness and ability to engage in dyadic HF-specific prognosis communication. Findings have relevance for the implementation of any efforts, including HF guidelines, aimed at improving prognosis communication. Our findings, informed by a complexity science approach, offer an innovative and robust alternative to traditional linear approaches to prognosis communication.

Abstract:A transient model is developed to predict dermal absorption of gas-phase chemicals via direct air-to-skin-to-blood transport under non-steady-state conditions. It differs from published models in that it considers convective mass-transfer resistance in the boundary layer of air adjacent to the skin. Results calculated with this transient model are in good agreement with the limited experimental results that are available for comparison. The sensitivity of the modeled estimates to key parameters is examined. The model is then used to estimate air-to-skin-to-blood absorption of six phthalate esters for scenarios in which (A) a previously unexposed occupant encounters gas-phase phthalates in three different environments over a single 24-h period; (B) the same as 'A', but the pattern is repeated for seven consecutive days. In the 24-h scenario, the transient model predicts more phthalate absorbed into skin and less absorbed into blood than would a steady-state model. In the 7-day scenario, results calculated by the transient and steady-state models converge over a time period that varies between 3 and 4days for all but the largest phthalate (DEHP). Dermal intake is comparable to or larger than inhalation intake for DEP, DiBP, DnBP, and BBzP in Scenario 'A' and for all six phthalates in Scenario 'B'.

Abstract:We extended a mechanistic, physics-based framework of the dry down process, previously developed for liquids and electrolytes, to solids and coded it into the latest UB/UC/P&G skin permeation model, herein renamed DigiSkin. The framework accounts for the phase change of the permeant from dissolved in a solvent (liquid) to precipitated on the skin surface (solid). The evaporation rate for the solid is reduced due to lower vapor pressure for the solid state versus subcooled liquid. These vapor pressures may differ by two orders of magnitude. The solid may gradually redissolve and penetrate the skin. The framework was tested by simulating the in vitro human skin permeation of the 38 cosmetically relevant solid compounds reported by Hewitt et al., J. Appl. Toxicol. 2019, 1-13. The more detailed handling of the evaporation process greatly improved DigiSkin evaporation predictions (r2 = 0.89). Further, we developed a model reliability prediction score classification using diverse protein reactivity data and identified that 15 of 38 compounds are out of model scope. Dermal delivery predictions for the remaining chemicals have excellent agreement with experimental data. The analysis highlighted the sensitivity of water solubility and equilibrium vapor pressure values on the DigiSkin predictions outcomes influencing agreement with the experimental observations.
Quantitative structure permeability relationships for phenolic compounds applied to human epidermal membranes in various solvents

Abstract:Purpose: This study examined how solvent-skin-solute interactions influenced the human epidermal permeation of three similar-sized phenolic compounds applied in a series of different solvents. Methods: Human epidermal permeation fluxes and lag times of three phenolic compounds were assessed in Franz cells for a range of solvents varying in molecular size and solubility parameters. In order to develop a mechanistic understanding of the determinants of the permeation findings, the solubility of the compounds in solvents and stratum corneum, the extent of solvent uptake by the stratum corneum and the impact of the solvents on skin hydration and transepidermal water loss were also measured. Results: Maximum epidermal fluxes and lag times varied greatly with the various solvent used. Markedly enhanced epidermal permeability fluxes, prolonged lag times and reduced diffusivities of the compounds were evident for many of the solvents. A solvent induced increase in stratum corneum solubility was associated with the uptake of solvent containing dissolved compound into the stratum corneum intercellular lipids, corneodesmosomes, lacunae and corneocytes. This uptake was dependent on both the solvent molecular size and the solubility of the compounds in the solvents. The imbibed solvent led to variable compound uptake, increased thermodynamic activity and modulated flux with, in some cases, reduced diffusion and a prolonged lag time. Conclusion: The solubility, permeation and lag times of compounds in the stratum corneum can be modulated by solvent uptake. Whilst a solvent-induced stratum corneum reservoir effect for a compound may prolong its lag time for a compound before steady state permeation is reached, it does not affect its overall steady state transport defined by variations in SC solubility and diffusion of its free form after solvent exposure.

Abstract:The influence of ethanol on the permeation of 17β-estradiol (estradiol) across viable human skin in vivo was investigated with the human skin sandwich flap model. Maintaining continuous delivery of a constant concentration of the solute in phosphate-buffered saline, pH 7.4 (PBS), or mixtures of ethanol in PBS to the skin surface revealed that steady-state flux of estradiol was achieved within 30–60 min and maintained throughout 4 hr. The 10-fold decrease in in vivo flux and permeability coefficient (Kp) of tracer estradiol solutions in ethanol or ethanol solutions compared with PBS vehicle reflected the 10-fold difference in the apparent partition coefficients (Km) of estradiol from the respective vehicles into isolated human stratum corneum. Neither the stratum corneum thickness nor the diffusion coefficient of estradiol was significantly different among the vehicles tested. In vivo flux of estradiol in ethanol or ethanol solutions across viable human skin was increased with saturated solutions of estradiol. Further, in vivo flux of estradiol from vehicles such as PBS, ethanol, and ethanol mixtures, which minimally alter the rate-limiting barrier, can be successfully predicted with knowledge of only two physicochemical parameters, the estradiol concentration in the vehicle and the Km of estradiol from the vehicle into isolated human stratum corneum.

Abstract:Human skin has continual exposure to chemicals due to various occupational activities. Chemicals that get on skin have the potential to be absorbed. Hence, the potential human health hazards of a chemical must include an estimate for percutaneous absorption. An inexpensive, easy, and adequate model for the quantitative measurement of skin penetration of chemicals from JP-8 is absent. Cutaneous penetration studies in vitro through human skin are severely limited due to the lack of availability of the human skin. In this study, we have shown that pig ear skin can be used as a model for risk assessment from the percutaneous absorption of chemicals. We determined flux and permeability coefficient (Kp) of three chemicals--heptane, hexadecane, and xylene--from their permeation profile through porcine and human skin. Binding of these chemicals to porcine stratum corneum (SC) and human SC were also determined. Factors of difference (FOD) in the permeability of pig and human skin were 1.71, 1.28, and 1.16, respectively, for heptane, hexadecane, and xylene. FOD in binding of heptane, hexadecane, and xylene to pig and human SC were found to be 1.04, 0.76, and 1.31, respectively. Since, FOD for permeability and binding parameters were less than 2, hence, we conclude that pig ear skin can be used as model for humans for risk assessment from percutaneous absorption of chemicals.

A new approach to modeling transdermal ethanol kinetics

Abstract:Measurement of ethanol above the skin surface (supradermal) is used to monitor blood alcohol concentrations (BAC) in both legal and consumer settings. Previously, the relationship between supradermal alcohol concentration (SAC) and BAC was described using partial and ordinary differential equations (PDE model: J. Appl. Physiol. 100: 649-55, 2006). Using a range of BAC profiles by varying absorption times and peak concentrations, the PDE model accurately predicted experimental measures of SAC. Recently, other mathematical models have relied on the PDE model. This paper proposes a new approach to modeling transdermal ethanol kinetics using a mass transfer coefficient and only ordinary differential equations (ODE model). Using a range of BAC profiles, the ODE model performed very similarly to the PDE model. The ODE model had slightly slower washout rates and slightly slower times to peak SAC and to zero SAC. Similar to the PDE model, a sensitivity analysis on the ODE model showed changes in solubility and diffusivity within the stratum corneum, stratum corneum thickness, and the volume of gas above the skin affected model performance. This new model will streamline integration into larger physiologic models, reduce computation time, and decrease the time to transform skin alcohol measurements to blood alcohol concentrations.

Abstract:The purpose of study is to quantify and compare diffusion of propylene glycol, glucose, glycerol in the human skin in vivo noninvasively. Optical coherence tomography (OCT) was utilized in the functional imaging of optical cleaning agents for monitoring and quantifying the permeability coefficients (PCs) of them. Our experiments showed that the permeability coefficient of 40% propylene glycol from different subjects was averaged and found to be (2.52 ± 0.02) × 10 −6 cm/s, the permeability coefficient of 40% glucose was (1.94 ± 0.05) × 10 −6 cm/s, and the permeability coefficient of 40% glycerol was (1.82 ± 0.04) × 10 −6 cm/s. The results indicated that the diffusion of propylene glycol solutions was faster than that of glucose solution, and the diffusion of glucose solutions was faster than that of glycerol solutions. The dependence of the permeability on the different hyperosmotic analytes could potentially be used in various basic science and clinical fields, such as optical clearing of tissues and cells as well as in clinical pharmacology.

Abstract:Dermal exposure to indoor gas-phase semi-volatile organic compounds (SVOCs) has recently received a great deal of attention, and this has included evaluating the role of clothing in this process. Several models have been developed to assess dermal exposure to SVOCs, based on the transient mass transfer of SVOCs from air to dermal capillaries. Assumptions of these models are either that clothing completely retards SVOC transport, or that there is an air gap of constant thickness between the clothing and the surface of the skin, which may lead to errors in the model calculations. To solve this problem, we tried to describe SVOC transport between clothing and epidermis by considering two parallel processes: partitioning of SVOCs by direct contact (ignored in existing models), and Fickian diffusion through the air gap. Predictions from the present model agree well with the experimental data found in the literature (dermal uptake of diethyl phthalate (DEP) and di-n-butyl phthalate (DnBP) of a clothed participant). This study provides a useful tool to accurately assess dermal exposure to indoor SVOCs, especially for evaluating the effects of clothing on dermal exposure.
An Experimental Method for Measuring VOC Emissions from Individual Human Whole-Body Skin under Controlled Conditions

Abstract:The human body is an important source of volatile organic compounds (VOCs), especially in densely occupied environments. This research provides an experimental method for separately measuring the whole-body skin emissions of an individual person under controlled conditions. The designed experimental device consisted of a 1.5 m(3) stainless steel test chamber and a transition chamber surrounding it. The environmental parameters such as temperature and background concentrations were controlled in both the test chamber and the transition chamber. During measurement, a human subject with standard clothing sat calmly in the test chamber operating in an airtight mode, and breathed through the respiratory channel with a mask tightly covering the face. A male subject was tested to verify the feasibility and repeatability of this experimental method. The results showed that VOCs emitted from the whole-body skin had accumulated in the test chamber air and changed linearly, indicating that the whole-body skin was a nearly stable source of VOCs during the experimental period. The 37 VOCs emitted from this subject's skin showed clear qualitative results, including alcohols, aldehydes, ketones, alkanes, aromatics, esters, alkenes, and acids, and the first three kinds contributed the most to the emissions. The reported VOC emission rates ranged from 0.8 to 86.3 mu g/h, and were highly consistent in magnitude with those of the same compounds in the literature. The high repeatability of this experimental method suggested its applicability to explore human skin emissions of more subjects and under various conditions in the future.

Abstract:Gas chromatography with mass spectrometric detection (GC-MS) coupled with solid phase micro-extraction as pre-concentration method (SPME) was applied to identify and quantify volatile organic compounds (VOCs) emitted by human skin. A total of 64 C4-C10 compounds were quantified in skin emanation of 31 healthy volunteers. Amongst them aldehydes and hydrocarbons were the predominant chemical families with eighteen and seventeen species, respectively. Apart from these, there were eight ketones, six heterocyclic compounds, six terpenes, four esters, two alcohols, two volatile sulphur compounds, and one nitrile. The observed median emission rates ranged from 0.55 to 4,790 fmol cm(-2)min(-1). Within this set of analytes three volatiles; acetone, 6-methyl-5-hepten-2-one, and acetaldehyde exhibited especially high emission rates exceeding 100 fmol cm(-2)min(-1). Thirty-three volatiles were highly present in skin emanation with incidence rates over 80%. These species can be considered as potential markers of human presence, which could be used for early location of entrapped victims during Urban Search and Rescue Operations (USaR).

Abstract:The human skin is an important source of volatile organic compounds (VOCs) in indoor environments. In this study, VOC emissions from whole-body skin of 14 healthy young adults were measured using a specially designed environmental chamber under controlled conditions. The number of VOC species in whole-body skin emissions ranged from 38 to 69 and the total emission rate of all VOCs detected in whole-body skin emissions ranged from 164 to 518 mu g/h. The similarities of VOC species among different subjects were more prominent, while the emission rate of the same VOC among different subjects varied significantly. In particular, eight VOCs (acetone, 6-methyl-5-hepten-2-one (6-MHO), geranylacetone, hexanal, heptanal, octanal, nonanal, and decanal) were emitted from the skin of all subjects, most likely originating due to the reaction of ozone with human skin. These VOCs accounted for a relatively large proportion of the total emission rate, indicating that the reaction of skin with ozone was an important source for skin VOC emissions. In addition, siloxanes, aldehydes, ketones, and alcohols contributed remarkably to whole-body skin emission rates. Overall, this study showed information on the apportionments of VOC species and emission rates of whole-body skin emissions, which shed more light on the emission characteristics of human skin as a VOC source.

Abstract:Dermal absorption of weak electrolytes applied to skin from pharmaceutical and cosmetic compositions is an important consideration for both their efficacy and skin safety. We developed a mechanistic, physics-based framework that simulates this process for leave on applications following solvent deposition. We incorporated this framework into our finite dose computational skin permeation model previously tested with nonelectrolytes to generate quantitative predictions for weak electrolytes. To test the model, we analyzed experimental data from an in vitro human skin permeation study of a weak acid (benzoic acid) and a weak base (propranolol) and their sodium and hydrochloride salts from simple, ethanol/water vehicles as a function of dose and ionization state. Key factors controlling absorption are the pH and buffer capacity of the dose solution, the dissolution rate of precipitated solids into a lipid boundary layer and the rate of conversion of the deposited solid to its conjugate form as the nonionized component permeates and (sometimes) evaporates from the skin surface. The resulting framework not only describes the current test data but has the potential to predict the absorption of other weak electrolytes following topical application.

Abstract:This study focuses on the semivolatile organic compound (SVOC) absorption through clothing and the skin. SVOCs are ubiquitous in daily life, in products like personal care items, plastics, and building materials. Understanding their permeation through the skin barrier is crucial for evaluating potential health risks of complete exposure. A PBPK model was developed to comprehend the dynamic interplay between SVOCs and human skin and to estimate tissue distribution throughout the body. The framework incorporated parameters such as skin permeability, physicochemical properties of the chemicals, and the impact of protective clothing and adsorbents. This model predicted the rate and extent of SVOC absorption under diverse scenarios. The PBPK predictions matched the experimental amount of mono-ethyl phthalate (MEP), a phthalate metabolite, when urine samples were collected for bare-skinned and clothed participants. Urine concentrations of MEP during a 6-hr exposure and for the next 48 hr show that clean clothing effectively decreased dermal uptake and the buildup of chemicals in the body. Additional removal of MEP was achieved through adsorption on activated carbon fabric. An increase in the maximum monolayer adsorption capacity or the Langmuir equilibrium constant further reduced the amount of MEP in the urine.

Abstract:The sbr gene of Drosophila melanogaster belongs to the NXF (nuclear export factor) family responsible for the mRNA transport from nucleus to cytoplasm. We have shown that in the heat-exposed (37 degrees C, 1 h) females, the l(1)ts403 (sbr10) mutation leads, in particular, to the high-frequency nondisjunction and loss of sex chromosomes in meiosis. For this trait, the incomplete dominance of the sbr10 mutation is observed. At the same time, the sbr10 mutation is recessive for many other traits of the heat-exposed flies: reduced viability, low fertility, impaired synthesis of the heat shock proteins, etc. The females heterozygous for the null allele (Df(1)vL4, a deletion eliminating gene srb) do not differ from females homozygous for the wild-type allele in frequency of the heat shock-induced nondisjunction and loss of sex chromosomes in meiosis. Because of this, the sbr10 mutation can be assigned to the gain-of-function alleles (those gaining the dominance function). Expression of the mutant sbr10 allele against the background of the wild-type allele suggests that in the heat shock-exposed females, the heat-modified product of this ts allele has a strong effect on sex chromosome disjunction in meiosis.
