Abstract:Summary: Parathyroid extract (PTE), dibutyrylcyclic AMP (dbcAMP), adenosine cyclic 3':5'-monophosphate (cAMP), calcitonin (CT), and calcium chloride were infused separately into anesthetized, sham-operated, or TPTX vitamin D-fed adult rats to examine the effect of these calciotropic agents on fractional excretion (FE) of α-aminoisobutyric acid (AIB), and phosphate anion (Pi). AIB is a nonmetabolizable amino acid.Inulin clearance, FEAIB, and FEPi were stable in the intact (n = 10) and TPTX rat (n = 10). TPTX decreased FEaib, and FEPi significantly (P < 0.001 for both). PTE and dbcAMP both increased FEaib in the intact rat (P < 0.001); failure to obtain this response in the TPTX animal was a key finding. PTE and dbcAMP increased FEPi (P < 0.001) in both the intact and TPTX animal. CT was the only agent (versus PTE, dbcAMP, adenosine cyclic 3‘:5’-monophosphate, and CaCl2) to increase FEAIB (P < 0.001) in the TPTX rat; furthermore, it was the only agent that did not increase FEPi in the TPTX rat although it had hypocalcemic and hypophosphatemic effects. Changes in inulin clearance or plasma concentration of AIB, following infusion of calciotropic agents, do not explain the unique responses in FEAIB in the TPTX rat.Our findings suggest that hyperaminoaciduria induced by parathyroid hormone and cyclic nucleotide in the intact animal may be mediated by CT. Hyperphosphaturia is not a necessary response to small-dose (25 milliunits/kg.hr) infusions of CT.Speculation: Intracellular calcium in renal epithelium may be the final determinant of hyperaminoaciduria in hyperparathyroidism.
Abdominal Obesity and Cardiometabolic Risk Markers: A Comparative Analysis of Waist Circumference, Dual-energy X-Ray Absorptiometry, and Magnetic Resonance Imaging Techniques

Abstract:Background and aims This study compares three methods to determine central adiposity (waist circumference –WC - and visceral adipose tissue – VAT- estimated by dual-energy x-ray absorptiometry – DXA, and by magnetic resonance imaging - MRI) in their ability to predict increases in cardiometabolic risk (CMR) markers in young individuals. We examined their associations with CMR in 47 men and women aged 25-40. Methods and Results VAT mass was assessed using DXA and MRI. Blood samples were analyzed for CMR markers. Associations between central adiposity measurements and CMR factors were analyzed using Spearman's correlation coefficient, and the ability of these three central adiposity measurements to detect increased CMR was compared using receiver operating characteristic (ROC) curves. Similar to what was observed for the MRI-DXA and VAT-DXA, WC showed strong correlations with LDL-c and triglycerides (TG) and an inverse correlation with HDL-c (rho=-0.657 MRI, rho=-0.628 DXA, and rho=-0.604 WC, p<0.01).On the other hand, only MRI-VAT and WC were associated with insulin and HOMA-IR (rho=0.341 MRI and rho=0.421 WC, p<0.01). Central adiposity measurements were negatively associated with cold-induced 18 F-FDG uptake in subcutaneous adipose tissue and positively associated with VAT TG content. No significant differences were observed when comparing the three central adiposity measurements in ROC curve analysis, and all measurements could predict increases in CMR markers and the combined CMR index. Conclusions This study reinforces the importance of using WC to assess increases in CMR markers among young adults. Given its practicality and efficacy, WC should be recommended in health centers to assess CMR risk.

Study of Relationships Between Intra-Abdominal Obesity and High Sensitive C-reactive Protein,tissue Inhibitor of Metalloproteinase-1 in Patients with Coronary Heart Disease

Abstract:Objective To investigate the relationship between intra-abdominal obesity and the serum levels of inflammatory factors- high sensitive C-reactive protein ( hs-CRP) and tissue inhibitor of metalloproteinase-1(TIMP-1) in patients with coronary heart disease ( CHD) . Methods A total of 85 persons were divided into two groups, CHD group and control group. The height, body weight, waist circumference (WC), body mass index ( BMI) and waist-to-hip ratio( WHR) were recorded and blood fasting total cholesterol ( TC), triglyceride ( TG), high density lipoprotein cholesterol ( HDL-C), low density lipoprotein cholesterol (LDL-C), high sensitive C-reactive protein (hs-CRP) and tissue inhibitor of metalloproteinase-1 (TIMP-1) were measured for all subjects. All subjects were evaluated with a DEXA total body scan performed using a Lunar Prodigy system ( GE Healthcare) . The body fat composition and distribution,serum levels of inflammatory factors were also analyzed. Results The level of hs-CRP was significantly higher in CHD group than in control group [(3. 41 ± 3. 73) mg/ L vs. (1. 48 ± 1. 28) mg/ L, P < 0. 01]. The level of TIMP-1 had no significant deviation between the two groups [(46. 32 ±17. 86) mg/ L vs. (51. 39 ± 16. 50)mg/ L, P = 0. 21]. In both groups, hs-CRP had positive correlation with WC,BMI, WHR and A/ G(all P < 0. 05), the level of TIMP-1 was not significantly correlated with BMI, WC, WHR, TF% , A/ G, LDL-C and hs-CRP(all P > 0. 05). Conclusions The accumulation of abdominal adipose tissue is closely correlated to higher hs-CRP. An extent of chronic inflammatory stress might exist in obese person. The level of hs-CRP is significantly increased in CHD patients. However,there is no significantly correlation between serum TIMP-1 and the accumulation of abdominal adipose tissue.
Sagittal abdominal diameter as a practical predictor of visceral fat

Abstract:Objective: To evaluate the relationships between the supine sagittal abdominal diameter (SAD) and visceral fat, as well as to evaluate intra- and inter-observer reliability of sagittal diameter measurement. Patients: Twenty-eight women ranging in age from 27-78 y with a body mass index (BMI) ranging from 16.9-48.1 kg/m2 and 23 men ranging in age from 32-75 y with BMI ranging from 20-41.6 kg/m2. Measurement: Body fat distribution was measured by waist circumference, waist to hip ratio (WHR), SAD, anthropometrically assessed and a single slice of computed tomography (CT) at the L4-L5 level. Results: In both genders, a significant association was found between visceral adipose tissue (AT) and SAD, as evaluated by CT (women r = 0.80; men r = 0.83, P < 0.001), and SAD by anthropometry (women r = 0.76; men r = 0.82, P < 0.001), as well as between visceral AT and waist circumference (women r = 0.76, men r = 0.86, P < 0.001) and WHR (women r = 0.57, P < 0.01, men r = 0.80, P < 0.001). A significant association was also found between subcutaneous AT and SAD by anthropometry (women r = 0.79, men r = 0.74, P < 0.001). After adjusting for BMI, the association between subcutaneous AT and SAD was no longer significant in men and only moderately significant in women (r = 0.42, P < 0.05), while the association between visceral AT and SAD by anthropometry remained significant in both genders (women r = 0.63, P < 0.001; men r = 0.66, P < 0.001). When the subjects were divided into two groups according to BMI (lean to moderately overweight women with BMI < 28 and men with BMI < 30 and obese women with BMI > 28 and men with BMI > 30) we found that the relationships between SAD by anthropometry, as well as SAD by CT and visceral AT, were higher in lean to moderately overweight subjects than in those who were obese. High inter-observer correlation was found concerning SAD measurement (r = 0.99, P < 0.001). Intra- and inter-observer precision as evaluated by coefficient of variation and intraclass correlation coefficient for SAD measurement was very high. Conclusion: Our study shows the usefulness of SAD by anthropometry to predict visceral fat and its very high inter- and intra-observer precision.

Abstract:Dear Editor, A 2-year-old girl presented with 5 days of fever of up to 40°C, a diffuse erythematous rash and bright red lips with strawberry tongue. She had mild bilateral conjunctivitis, cervical lymphadenopathy and swollen hands and feet. On presentation, her temperature was 40°C, heart rate 160 beats/min, blood pressure 93/53 mm Hg and respiratory rate 50 breaths/min. Her laboratory data revealed an elevated white blood cell count of 13 300/mL and a C-reactive protein level of 12.3 mg/dL. Based on clinical and laboratory criteria, her symptoms were consistent with a diagnosis of Kawasaki disease (KD).An electrocardiogram did not show any abnormalities and an echocardiogram did not show any evidence of coronary artery lesions (CALs). Intravenous immunoglobulin (IVIg) was administered at 2 g/kg per day on the 6 day of KD onset. However, the patient remained febrile and required a second dose of 1 g/kg per day on the 8 day of KD onset, but her symptoms remained unchanged on physical examination. Under the same diagnosis of KD refractory to IVIg, she was given cyclosporine at 5 mg/kg per day and high-dose acetylsalicylic acid (ASA) at 50 mg/kg per day. Plasma exchange (PE) was performed on the 10 day of KD onset after we obtained written informed parental consent. PE was introduced with 5% albumin per one plasma volume (500 mL/day). Priming volume was 60 mL, the ratio of 5% albumin versus red cell concentrate was 1:1. TR 525 (Toray Medical, Chiba, Japan) was used. During PE, blood was drawn and plasma was infused using low-molecular-weight heparin given at 75 U/kg per day in the radial artery and vein. Her body weight, blood pressure, temperature, pulse rate, respiratory rate, and blood coagulation status were carefully monitored. After the initial PE, her fever gradually deceased and her inflammatory marker levels decreased. PE therapy was performed three times and was effective with no complications throughout her 29 days of hospitalization (Fig. 1). She received ASA and cyclosporine along with exchange or after exchange. Development of CALs is followed up once a month. She has no abnormalities on follow-up echocardiograms. Abstract:Prior research has indicated that as an important biomarker of chronic low-grade inflammation, high-sensitivity C-reactive protein (hs-CRP) can play important roles on the onset of metabolic syndrome and cardiovascular diseases (CVD). We conducted an integrative approach, which combines biological wet-lab experiments, statistical analysis, and semantics-oriented bioinformatics & computational analysis, to investigate the association among hs-CRP, body fat mass (FM) distribution, and other cardiometabolic risk factors in young healthy women. Research outcomes in this study resulted in two novel discoveries. Discovery 1: There are four primary determinants for hs-CRP, i.e., central/abdominal FM (a.k.a. trunk FM) accumulation, leptin, high density lipoprotein cholesterol (HDL-C), and plasminogen activator inhibitior-1 (PAI-1). Discovery 2: Chronic inflammation may involve in adipocyte-cytokine interaction underlying the metabolic derangement in healthy young women.
Reduced fat oxidation and obesity risks among the Buryat of Southern Siberia

Abstract:Over the last 20 years, obesity and associated metabolic diseases have emerged as major global health problems. Among urbanizing populations of developing regions of the world, childhood undernutrition often coexists with adult overnutrition, a phenomenon known as the “dual nutritional burden”. A recent work (Frisancho 2003 : Am J Hum Biol 15:522–532) suggests that linear growth stunting in early childhood may contribute to adult obesity by reducing the body's ability to oxidize fat. We test central aspects of this model drawing on data from 112 adult Buryat herders (53 males; 59 females) from Southern Siberia. The results are consistent with the predictions of the model, but only for women. Shorter Buryat women (height‐for‐age Z‐scores ≤ −1) have significantly lower fasting fat oxidation levels compared to their taller counterparts. Shorter women are also significantly heavier and fatter, and have higher serum lipid levels. Among all Buryat women, reduced fat oxidation is significantly correlated with percent body fatness, serum triglyceride levels, and serum leptin levels, after controlling for relevant covariates. Additionally, Buryat women with high dietary fat intakes and low fat oxidation are significantly fatter and have higher lipid and leptin levels than those with low fat intakes and high fat oxidation. These results suggest that developmental changes in fat oxidation may play a role in the origins of obesity among populations with high rates of linear growth stunting. Further longitudinal research is necessary to elucidate the pathways through which early‐life undernutrition may increase risks for adulthood obesity and cardiovascular disease. Am. J. Hum. Sagittal abdominal diameter compared with other anthropometric measurements in relation to cardiovascular risk

Abstract:Background: Abdominal adiposity has been described as an independent risk factor for coronary heart disease. Sagittal abdominal diameter has been found to be closely related to the amount of visceral adipose tissue. Aim: To compare the sagittal abdominal diameter with other anthropometric measures regarding their relationships to risk factors for coronary heart disease (CHD). Design: A study of 885 men and women participating in a health survey. Measurements: Sagittal abdominal diameter, body mass index (BMI), waist and hip circumferences, waist-to-hip ratio, serum concentrations of risk factors for CHD, blood pressure. Results: In men the sagittal abdominal diameter showed stronger correlations to the CHD risk factors serum cholesterol, low-density lipoprotein (LDL) cholesterol, triglycerides, glucose, insulin, apolipoprotein B (apoB), plasminogen activator inhibitor tissue-type plasminogen activator (t-PA) and lipid-corrected alpha tocopherol, and to systolic and diastolic blood pressures than the other anthropometric measurements. In women, compared with the other anthropometric measurements the sagittal abdominal diameter was more strongly correlated to serum cholesterol, LDL cholesterol, LDL/HDL (high-density lipoprotein), apo B and t-PA, and to systolic and diastolic blood pressure. The sagittal abdominal diameter showed a stronger correlation to 'total risk' for cardiovascular disease (+ 0.66 for men, 0.62 for women), than waist circumference (+ 0.63 for men, + 0.57 for women) and waist-to-hip ratio (+ 0.61 for men and +0.48 for women; P <0.0001 for all correlations). This diameter was also more strongly correlated to 'metabolic risk' (+ 0.64 for men, + 0.59 for women) than waist circumference (+ 0.60 for men, + 0.59 for women) and waist-to-hip ratio (+ 0.58 for men, + 0.52 for women)(P < 0.0001 for all correlations). In a regression analysis including the anthropometric measurements and the risk values, the sagittal diameter was the strongest measure of cardiovascular risk in both men and women. Conclusions: Among both men and women in this study the sagittal abdominal diameter showed stronger correlations to cardiovascular risk and to other risk factors in the metabolic syndrome than other anthropometric variables such as waist circumference, waist-to hip ratio and BMI.

Abstract:Two-route chemotherapy of CDDP and STS was performed in 16 postoperative patients with ovarian cancer under the hypertensive condition of 20 minutes induced with angiotensin II (AT-II). Plasma total CDDP and filtrable CDDP levels serially determined at 15 and 30 minutes after intraperitoneal administration of CDDP (150 mg/body) were significantly higher than under the same condition except for using AT-II. The extreme vasoconstriction in the kidney and other organs during AT-II induced hypertension would protect these organs from CDDP induced toxicities. Furthermore, STS infused immediately after the cessation of AT-II could neutralize the CDDP preventing the occurrence of toxicities. The elevating plasma levels of CDDP might be beneficial to enhance the anticancer effects of CDDP, since the blood flow in cancer tissue was increased during AT-II induced hypertension.

Abstract:BACKGROUND: The increase in obesity, especially the increase of visceral fat, has been directly associated with health risk. Additionally, a waist-to-height ratio (WHtR) (0.5) has been proposed as an indicator of early health risk.AIMS: To test the association between anthropometric measurements and inflammatory mRNA expression and to evaluate the effectiveness of WHtR to predict inflammatory risk in children.SUBJECTS AND METHODS: Anthropometric measurements and fasting blood samples were collected in 94 Brazilian children (46 boys) aged 9-11 years. Children were identified as being at inflammatory risk based on cytokine mRNA expression levels at and above the 75th percentile. Inflammatory cytokine mRNA expression levels were determined through buff-coat using real-time polymerase chain reaction.RESULTS: Body mass index (BMI), waist circumference (WC) and WHtR showed moderate-to-strong association with inflammatory cytokine mRNA gene expression. WHtR = 0.51 was the most accurate cut-off point in distinguishing children at inflammatory risk. Compared with children with WHtR <0.51, children with a WHtR ≥0.51 presented an odds ratio of 2.643 (95% CI = 1.749-3.994) for both diagnosed IL-6 and MCP-1 above the 75th percentile and 2.796 (95% CI = 1.802-4.338) for TNF-α above the 75th percentile. WHtR = 0.50 presented the same values of sensitivity and odds ratio and ∼7% lower specificity compared to WHtR = 0.51.CONCLUSION: Anthropometric measurements have shown a moderate-to-strong association with the mRNA expression of inflammatory genes and a WHtR of 0.5 could serve as a cut-off point, suggesting increased risk of inflammatory responses.

Exploring the Link between Inflammatory Biomarkers and Adipometrics in Healthy Young Adults Aged 20–35 Years

Abstract:Obesity and aging are associated with an inflammatory state, which represents the common background for a wide range of diseases. This study aims to explore the correlation between hsCRP, IL-1β, IL-6, TNF-α, IFN-γ, and white blood cell count (WBC) and adipometrics (arm, waist, and hip circumferences: AC, WC, HC; total body fat mass: TBFM, visceral fat level: VFL, body mass index: BMI; waist/hip ratio: WHR; waist/height ratio: WHtR) in young and healthy adults aged 20–35 years old. The subjects were divided by BMI into the overweight/obesity (OW/OB) group and normal weight (NW) group, and by hsCRP level into Group 1 (<1 mg/L), Group 2 (≥1–2.99 mg/L), and Group 3 (≥3 mg/L). The concentration of all inflammatory biomarkers was significantly higher in the OW/OB group compared to the NW group, with the exception of IL-1β. Significant positive correlations were found between hsCRP, TNF-α, WBC, and all adipometrics; between IL-6 and WHR, WHtR, BMI, TBFM, and VFL; and between IFN-γ and HC, BMI, and TBFM. IL-1β correlates positively with WHR and VFL. In Groups 1–3, all the differences between the adipometrics showed significant differences. Subclinical inflammation persists in association with being overweight and obese in healthy young adults aged 20–35 years old.

Effect of mutations in the PCSK9 gene on the cell surface LDL receptors.

Abstract:The proprotein convertase subtilisin/kexin type 9 (PCSK9) gene is involved in the post-transcriptional regulation of the low-density lipoprotein (LDL) receptors (LDLR). Mutations in the PCSK9 gene have been associated with both hypocholesterolemia and hypercholesterolemia through 'loss-of-function' and 'gain-of-function' mechanisms, respectively. We have studied the effect of the four loss-of-function mutations R46L, G106R, N157K and R237W and the two gain-of-function mutations S127R and D374Y on the autocatalytic activity of PCSK9, as well as on the amount of the cell surface LDLR and internalization of LDL in transiently transfected HepG2 cells. The two groups of mutations did not differ with respect to autocatalytic activity of PCSK9, but they did differ with respect to the amount of cell surface LDLR and internalization of LDL. The four loss-of-function mutations had a 16% increased level of cell surface LDLR and a 35% increased level of internalization of LDL as compared with WT-PCSK9. The two gain-of-function mutations had a 23% decreased level of cell surface LDLR and a 38% decreased level of internalization of LDL as compared with WT-PCSK9. Our studies have also shown that transfer of media from transiently transfected HepG2 cells to untransfected HepG2 cells, reduces the amount of cell surface LDLR and internalization of LDL in the untransfected cells within 20 min of media transfer. Thus, PCSK9 or a factor acted upon by PCSK9, is secreted from the transfected cells and degrades LDLR both in transfected and untransfected cells.

Abstract:OBJECTIVE:Subclinical atherosclerosis can predict the occurrence of cardiovascular events. This paper aimed to estimate predictive effect of adiposity measurement parameters,including body mass index(BMI),waist-to-hip ratio(WHR) or body fat mass(BFM) on the occurrence of subclinical atherosclerosis. METHODS:The cross-sectional study was performed at the Center of Medicine Preventive Cardiovascular in Paris from March 2005 to November 2006. A total of 649 consecutive cases aged 22-79 years,without cardiovascular disease history was investigated. Parameter of subclinical atherosclerosis,including carotid intima-media thickness and plaque number was measured by high resolution ultrasonography. BMI,WHR and BFM were detected by physiological impedance method. The association between subclinical atherosclerosis,high coronary risk and low-grade inflammation was analyzed. RESULTS:BMI,WHR and BFM were strongly positive associated with the occurrence of subclinical atherosclerosis(P < 0.01). BFM had the closest relation both in the occurrence of subclinical atherosclerosis and high coronary risk. WHR had the strongest effect on low degree inflammation. CONCLUSIONS:BFM shows the best association with subclinical atherosclerosis. The results suggest that BFM can be used as a complementary measure to identify the occurrence of heart disease in adult subjects.

Abstract:Microwave imaging of weak scattering objects within a strong multi-path environment is a challenging task. Differential imaging is used when the wanted targets are subjected to induced or inherent changes while the surrounding keeps stable. The paper illustrates the approach of differential imaging, discusses the performance limitations by random signal perturbations, and shows some examples from medical engineering.

Abstract:Recent experiments based on cryogenic microwave oscillators have tested the isotropy of the speed of light (Michelson-Morley experiment) at sensitivities of the order of a part in 10/sup 15/, which is a similar sensitivity to other best tests. Further improvements of the accuracy in this type of experiment are not expected due to the already long data set and the systematic error limit. We have constructed a new rotating Michelson-Morley experiment consisting of two cylindrical cryogenic sapphire resonators. The temperature of the dual cavity is controlled at approximately 6 K where the beat frequency between the two oscillators is independent of temperature. By rotating the experiment, an improvement of several orders of magnitude in our sensitivity to light speed anisotropy is expected, as the relevant time variations will now be at a rotation frequency where the frequency stability of the cryogenic oscillators is at its best.

Abstract:Both cardiometabolic and chronic inflammatory diseases pose a significant challenge to global public health, particularly among older adults. Here, we investigated the interplay between systemic inflammatory status and the cardiometabolic index (CMI) in older men with adequate weight or obesity. In this observational cross-sectional study, older men (71.79 ± 7.35 years) were separated into groups with normal weight (NW, n = 34) and obesity (O, n = 32) to assess circulating levels of pro- and anti-inflammatory cytokines and CMI. Overall, the O group showed not only a higher inflammatory status but also increased CMI (p < 0.0001) compared with the NW group. Interestingly, only positive correlations were found between pro- and anti-inflammatory cytokines in both groups. Through multivariate regression analysis, IL-6 (β = -0.2276, p = 0.0003) and IL-10 (β = 0.2023, p = 0.0030) significantly influenced CMI in the NW group. No significant results were found in the O group. Our findings reinforce the effects of obesity in inflammaging, as well as suggesting that the influence of cytokines in CMI occurs in older men with normal weight, since the elevated pro-inflammatory profile observed in older men with obesity can interfere in this effect.
A Cloud Point Extraction-Spectrofluorimetric Method for Determination of Thiamine in Urine

Abstract:A simple and efficient cloud point extraction-spectrofluorimetric method for the determination of thiamine in human urine is proposed. The procedure is based on the oxidation of thiamine with ferricyanide to form thiochrome, its extraction to Triton X-114 micelles and spectrofluorimetric determination. The variables affecting oxidation of thiamine, extraction and phase separation were studied and optimized. Under the experimental conditions used, the calibration graphs were linear over the range 2.5-1000 ng . The limit of detection was 0.78 ng of thiamine and the relative standard deviation for 5 replicate determinations of thiamine at 400 ng concentration level was 2.42%. Average recoveries between 93-107% were obtained for spiked samples. Association of simple anthropometric measures of obesity with visceral fat and the metabolic syndrome in male Caucasian and Indo-Asian subjects

Abstract:Aims: The aims of this study were first, to investigate the relationship between simple anthropometric measures of obesity with visceral fat as assessed by a single slice magnetic resonance imaging (MRI)-scan in patients attending a hospital clinic. Second, to determine which anthropometric measure best relates to the adverse metabolic profile of the metabolic syndrome. Methods: Forty-one male subjects [body mass index (BMI): 30.2 + 5.8 kg/m(2), age: 50.3 + 13.6 years] were studied by MRI-scan to measure visceral fat at L4/L5 level and to investigate its relationship with simple anthropometric measures. Second, we studied 83 male subjects to determine which anthropometric measure best predicts the metabolic complications (using the ATPIII criteria) of obesity in the setting of a hospital clinic. Results: Waist circumference was the best anthropometric measurement that correlated with MRI-visceral fat mass assessed at L4/L5 in 41 subjects who had an MRI scan (P = 0.0001, r(2) = 0.36, beta = 0.56) amongst variables which also included age, BMI, sagittal diameter, diabetes and ethnicity. Stepwise multiple regression analysis showed sagittal diameter (P = 0.001, r(2) = 0.4, beta = 0.406), age (P = 0.003, beta = 0.271) and waist circumference (P = 0.012, beta = 0.297) were the best predictors of the adverse metabolic profile of the metabolic syndrome in all 83 male subjects amongst BMI, waist-hip ratio (WHR), ethnicity and diabetes-related factors. Conclusions: Waist circumference is a simple anthropometric parameter that best correlates with single slice MRI-scan, but sagittal diameter (measured using abdominal calipers) better predicts the adverse metabolic profile of the metabolic syndrome. Although there is considerable variation in abdominal fat topography between ethnic groups, and also within populations, sagittal diameter assessment is a technique that is simple and best predicts the metabolic syndrome.

Abstract:Objective To explore the best measurement of waist circumference related with intra-abdominal fat volume evaluated by CT scan.Methods Height,weight,and hip circumference were measured among 147 subjects aged over 18 years old.Waist circumference was measured at 3 different levels:the upper brim of the iliac crest ( WC1 ),the midpoint between costal brim and iliac crest ( WC2 ),and the umbilicus ( WC3 ).The intra-abdominal fat volume was evaluated by CT scan.Results Intra-abdominal fat volume was significantly higher in men than in women [ ( 1 236.0±608.4 vs 931.0±665.0)cm3,P＜0.01 ].Correlation analysis showed that WC1 ( r =0.634),WC2( r=0.677),and WC3 (r =0.712)were positively correlated with intra-abdominal fat volume ( all P＜0.01 ).Partial correlation analysis adjusted by gender,weight,or body mass index showed that the correlation of intra-abdominal fat volume with WC3 ( r were 0.488 and 0.432) was better than that with WC1 ( r were 0.347 and 0.293 ) and WC2 ( r were 0.424 and 0.365 ).Multiple linear stepwise regression analysis demonstrated that WC2 and WC3 were independently associated with intra-abdominal fat volume and WC3 was the strongest impact factor (β =0.270,R2c =0.504,P＜0.01 ).Conclusions Waist circumference is a simple anthropometric measurement parameter reflecting the degree of intraabdominal fat accumulation.All three different measurements of waist circumference may reflect intra-abdominal fat volume,while waist circumference at umbilical level is the best among them.
