Abstract:OBJECTIVE:To explore the safe and effective method of hemostasis in laparoscopic hysteromyomectomy (LM).METHODS:Two hundred and eighty women with symptomatic uterine intramural fibroids undergoing LM were assigned to 4 groups, Group A undergoing fibroid pedicle ligation, Group B injected with 12 IU diluted vasopressin around the myoma, Group C injected with 20 IU oxytocin combined with pedicle ligation, and Group D injected with vasopressin combined with pedicle ligation. The operation time, amount of blood loss, operative complications, bowel deflation, post-operative hemoglobin dropping, and length of hospital stay were compared.RESULTS:The amounts of blood loss of Groups A and C were (171 +/- 146) ml and (184 +/- 140) ml, both significantly higher than those of Groups B and D [(115 +/- 70) ml and (106 +/- 73) ml, both P < 0.01]. The length of hospital stay of Group D was (2.9 +/- 0.5) d, significantly shorter than those of Groups A, B, and C [(3.1 +/- 0.7) d, (3.6 +/- 0.8) d, and (3.3 +/- 0.7) d, all P < 0.05]. The bowel deflation time of Group D was (20 +/- 6) h, significantly shorter than those of the Groups A, B, and C [(26 +/-) h, (25 +/- 7) h, and (25 +/- 8) h respectively, all P < 0.05]. The post-operative hemoglobin dropping of group D was (1.1 +/- 0.9) g/L, significantly less than those of Groups A, B, and C [(1.5 +/- 1.0), (1.4 +/- 0.8), and (1.2 +/- 0.7) g/L respectively, all P < 0.05].CONCLUSIONS:Vasopressin (12 IU) injection around the myoma is a simple, effective, and safe homeostatic procedure during LM. Pedicle ligation can reduce advanced post-operative bleeding post-operation.
[Influencing Factors of Reproduction Status of Patients Undergoing Laparoscopic Myomectomy].

Abstract:OBJECTIVE:To explore the influencing factors of reproduction status in women undergoing laparoscopic myomectomy (LM).METHODS:A total of 278 LM patients were recruited.We retrospectively reviewed the reproduction status of 87 pregnant cases after LM. The correlations of their pregnancy outcomes and such clinical profiles as age, operative techniques, biological characteristics of fibroids (number, type, size and location) were analyzed.No uterine rupture occurred during the gestation period.RESULTS:None of them switched to open surgery due to laparoscopic difficulties. However, one patient had a laparoscopic suture for secondary bleeding of uterine incision. At 3 months post-operation, sonography showed no heterogeneous echo, effusion and hematoma in uterine incision.Incision through uterine cavity occurred intraoperatively in 8 cases, but no intrauterine adhesion was found on hysteroscopy 3 months later. And 87 women became pregnant and the postoperative fertilization time was from 2 months to 5 years. Age influenced the postoperative pregnancy rate.Other factors such as location, number and size of fibroid had no impact on fertility.CONCLUSION:For achieving a high conception rate and guaranteeing the safety of pregnant women, a clinician should select reasonable surgical approaches, perform accurate anatomical restoration, apply strict hemostasis and choose a right time of conception.

Abstract:In this issue we publish the revised Consensus Statement (pp 663-671) giving guidelines for advising cardiovascular patients on sexual activity and the treatment of erectile dysfunction. In recognition of the need to provide clear but specialised advice, in March 2001 I established a special clinic at Guy's Hospital and appointed a full time sexual advice nurse. We have now seen and advised more than 300 men using the guidelines and have had no reported adverse events. All forms of therapy have been employed and our success rate in re-establishing a sexual relationship approximates to 90%. Positive patient feedback has been a reward in itself but in turn the nature of the problem and its impact on quality of life has been painfully highlighted. 'Every one who dealt with me was very nice and very helpful. The doctor and the nurse was the best ever just wish that other doctors and nurses was as helpful.' A simple quote identifying the unmet need and the failure of medical professionals to address the issue. ' ... after all, we're all human and have the same desires and thoughts and behaviour .. .' Perhaps we have a different view or are seen to have a different approach to cardiac patients reinforced by: and not being talked down to (treated as an equal) ... ' I have now travelled far and wide lecturing and teaching on the subject of sex, the heart and erectile dysfunction. I am just back from a conference in Pavia, Italy, where I was delighted to see that Professor Paolo Marandola (a urologist) has established a similar clinic with a cardiologist (Dr Raisaro) and has reported excellent results. I have no doubt this is a service that needs to be expanded, yet most cardiologists seem unable to grasp its importance. I am constantly disappointed at the lack of interest in the UK, in contrast to the enthusiasm shown by urologists. Our research abstracts were rejected by the British Cardiac Society, yet accepted and praised at national and international urological meetings. When I gave the British Heart Foundation lectures in Pontefract and Milton Keynes, the cardiologists did not attend. Erectile dysfunction is predominantly a vascular problem (70% of cases), yet cardiologists prescribe for less than 2% of men. Laparoscopic Vs. Open Abdominal Radical Hysterectomy for Cervical Cancer: A Single-Institution, Propensity Score Matching Study in China

Abstract:Study Objective: To compare the surgical and oncologic outcomes between open abdomen radical hysterectomy (ARH) and laparoscopic radical hysterectomy (LRH) for cervical cancer. Methods: Retrospective observational study with propensity score matching was used to ensure balanced groups for ARH and LRH. One-hundred-and-ninety-eight women with cervical cancer, 99 treated using ARH and 99 using LRH, between January 2012 and December 2014. Outcomes included disease-free survival (DFS), overall survival (OS), intra-operative factors, post-operator recovery, urinary retention, and adverse events. Moreover, the inverse probability of the treatment weighting (IPTW) method was also used. Main Results: Compared with ARH, LRH was associated with a lower volume of blood loss (P < 0.001) and transfusion rate (P < 0.001), with a broader resection of the parametrium (P < 0.001). Post-operatively, the time to first flatus was shorter for LRH than ARH (P < 0.001) but the rate of urinary retention was higher for LRH (22.2%) than ARH (8.1%; P = 0.009). DFS and OS were similar between groups. By IPTW, laparoscopy was also not associated with poorer survival in terms of DFS (HR 1.52, CI 0.799-2.891, P = 0.202) or OS (HR 0.942, HR 0.425-2.09, P = 0.883). Conclusion: Compared with ARH, LRH provided better intra-operative and post-operative outcomes, with no significant difference in oncologic outcomes and survival. Urinary retention remains a clinical issue to improve with LRH. The technology of LRH has been improved in China to address the inconsistent results of oncologic outcomes in previous studies. Whether these improvements could be effective needs to be investigated in the future.

Abstract:Objective: To determine the differences in myoma characteristics before laparoscopic myomectomy (LM) and abdominal myomectomy (AM) and to compare outcomes in current practice. Study design: We analysed retrospectively the data for 126 LMs and 176 AMs performed between January 1996 and January 2000 in 11 centres. Myomas were examined by transvaginal B-mode, colour and pulsed energy Doppler ultrasonography. The following data were collated: (i) pre-operative: myoma size, number, type and location and medical treatment, (ii) per-operative: operator, blood loss, surgical procedure and operative time, and (iii) post-operative: myoma weight and number, haemoglobin decrease, complications and length of hospital stay. Results: Compared to women undergoing LM, women undergoing AM had more myomas that were larger and that were generally interstitial and anterior. More of them received GnRH analogues. Excised myomas weighed four times more, the decrease in haemoglobin was greater (1g/dl), fever was more frequent, and nine patients needed transfusions (compared to none for LM). There were 37 laparoconversions (29%) after LM. The conversion rate was high for inexperienced surgeons. Length of hospital stay was reduced by half for LM (without laparoconversion). Recurrence rate at 2 years was 2.5% for LM versus 3.6% for AM (P = 0.506). Conclusion: Pre-operative evaluation by ultrasound is essential to establish myoma number, size, type and location in order to choose the most appropriate surgical procedure.
Minimally invasive myomectomy: practice trends and differences between Black and non-Black women within a large integrated healthcare system

Abstract:Background: Although multiple professional organizations encourage minimally invasive surgical approaches whenever feasible, nationally, fewer than half of myomectomies are performed via minimally invasive routes. Black women are less likely than their non-Black counterparts to have minimally invasive surgery. Objective: This study aimed to assess the trends in surgical approach among women who underwent minimally invasive myomectomies for uterine leiomyomas within a large integrated healthcare system as initiatives were implemented to encourage minimally invasive surgery, particularly evaluating differences in the proportion of minimally invasive surgery performed in Black vs non-Black women. Study design: We conducted a retrospective cohort study of women, aged ≥18 years, who underwent a myomectomy for a uterine leiomyoma within Kaiser Permanente Northern California between 2009 and 2019. Generalized estimating equations and Cochran-Armitage testing were used to assess myomectomy incidence and linear trend in the proportions of myomectomy by surgical route-abdominal myomectomy and minimally invasive myomectomy. Multivariable logistic regression analyses were used to assess the associations between surgical route and (1) race and ethnicity and (2) complications, controlling for patient demographic, clinical, and surgical characteristics. Results: A total of 4033 adult women underwent a myomectomy during the study period. Myomectomy incidence doubled from 0.12 (95% confidence interval, 0.12-0.13) per 1000 women in 2009 to 0.25 (95% confidence interval, 0.24-0.25) per 1000 women in 2019 (P<.001). During the 11-year study period, the proportion of minimally invasive myomectomy increased from 6.0% to 89.5% (a 15-fold increase). The proportion of minimally invasive myomectomy in Black women remained lower than in non-Black women (54.5% vs 64.7%; P<.001). Black women undergoing myomectomy were younger (36.4±5.6 vs 37.4±5.8 years; P<.001), had a higher mean fibroid weight (436.0±505.0 vs 324.7±346.1 g; P<.001), and had a higher mean body mass index (30.8±7.3 vs 26.6±5.9 kg/m2; P<.001) than their non-Black counterparts. In addition to patient race, surgery performed between 2016 and 2019 compared with surgery performed between 2009 and 2012 and higher surgeon volume compared with low surgeon volume were associated with an increased proportion of minimally invasive myomectomy (adjusted relative risks, 12.58 [95% confidence interval, 9.96-15.90] and 6.63 [95% confidence interval, 5.35-8.21], respectively). Black race and fibroid weight of >500 g each independently conferred lower rates of minimally invasive myomectomy. In addition, there was an interaction between race and fibroid weight such that Black women with a fibroid weight of ≤500 g or >500 g were both less likely to have minimally invasive myomectomy than non-Black women with a fibroid weight of ≤500 g (adjusted relative risks, 0.74 [95% confidence interval, 0.58-0.95] and 0.26 [95% confidence interval, 0.18-0.36], respectively). Operative, perioperative, and medical complications were low during the 11-year study period. In regression analyses, after controlling for race, age, fibroid weight, parity, low-income residence, body mass index, surgeon volume, and year of myomectomy, the risk of complications was not markedly different comparing abdominal myomectomy with minimally invasive myomectomy. Similar results were found comparing laparoscopic minimally invasive myomectomy with robotic-assisted minimally invasive myomectomy except for women who underwent laparoscopic minimally invasive myomectomy had a lower risk of experiencing any medical complications than those who underwent robotic-assisted minimally invasive myomectomy (adjusted relative risk, 0.27; 95% confidence interval, 0.09-0.83; P=.02). Conclusion: Within an integrated healthcare delivery system, although initiatives to encourage minimally invasive surgery were associated with a marked increase in the proportion of minimally invasive myomectomy, Black women continued to be less likely to undergo minimally invasive myomectomy than their non-Black counterparts. Race and fibroid weight alone did not explain the disparities in minimally invasive myomectomy.

Abstract:Myomectomy is one of the most common surgical procedure in the field of gynecology. However, the role of laparoscopic myomectomy is still debated for many factors, including surgical considerations, safety and fertility concerns, long-term outcomes, and cost-related issues. The aim of this study is to evaluate the surgical peri- and post-operative outcomes of laparoscopic and abdominal myomectomy. A systematic search for studies was performed up to June 2023 through MEDLINE, Pubmed, Embase. Studies reporting the comparison of surgical and obstetrical outcomes in laparoscopic versus laparotomic myomectomy were included for the following outcomes: time of surgery, estimated blood loss, decrease of postoperative hemoglobin, hospital stay, intra-operative complication rates, postoperative complications rates, postoperative analgesic use, postoperative pain at 24 h and pregnancy rate. The meta -analysis was performed using the Cochrane Review software. Fifty-six relevant articles were retrieved through the process of evidence acquisition. Eleven articles met inclusion criteria, for a total of 2,133 patients undergoing laparoscopic or laparotomic myomectomy. The estimated blood loss [standard mean differences (SMD) 0.72, IC 95 % 0.22 to 1.22], the hospital stays [SMD 3.12, IC 95 % 0.57 to 4.28], were significantly lower in laparoscopic than in open group. No statistically significant difference in intra-operative and post-operative complication rates, in pregnancy rate and others obstetrical outcomes between two surgical approaches were found. The findings of present metanalysis suggest that laparoscopic myomectomy offers multiple benefits, including reduced blood loss, shorter hospital stays, and less postoperative analgesic need, without a significant increase in complication rates and similar results in obstetrical outcomes when compared to abdominal myomectomy. However, the presence of few randomized studies on selected population may limit the generalizability of the findings to the entire population. Therefore, more well-designed studies or large population program data to draw definitive conclusions are therefore warranted.

Analysis on the Factors Related to the Operative Efficacy of Laparoscopic Myomectomy

Abstract:Objective To evaluate the indications,limitations,technique and the factors related to operative efficacy of laparoscopic myomectomy.Methods From January 2001 to August 2003,167 patients with uterine myoma underwent laparoscopic myomectomy were retrospectively analyzed.Results A total of 293 myomas were removed from 102 patients with single myoma and 65 with multiple myomas,including 92 intramural myomas,50 subserosal and 25 both intramural and subserosal.The number of myoma per patient varied from 1 to 9.The mean diameters of the myomas in three dimensions were (6.30±1.49) cm,(5.62±1.41) cm,and (5.49±1.30) cm respectively.The largest myoma was 11.3 cm×10.0 cm×8.7 cm.Among 167 patients,laparoscopic myomectomy were performed in 157 patients successfully and laparotomy conversion in 10 patients,including one with bowel injure and two with adenomyoma.The mean duration of operation was 114.8 min and mean blood loss was 87.28 ml.The operation lasted longer and the blood loss was more when the myoma was larger than 6 cm (P0.05).The operative duration of intramural myoma was longer and the blood loss was more than that of subserosal one.(P=0.001).The complication rate was 1.19%.Conclusions Our primary results indicate that laparoscopic myomectomy is a less invasive,safe and useful technique.Patient selection as well as laparoscopic suturing skill is the key factors in achieving a successful operation.

Abstract:Background Myomectomy is the preferred treatment for women with uterine fibroids and fertility requirements. There are three modalities are used in clinical practice for myomectomy: abdominal myomectomy (AM), laparoscopic myomectomy (LM), and robot‐assisted laparoscopic myomectomy (RLM). Objectives To compare the perioperative and postoperative outcomes of RLM, AM, and LM. Search Strategy We searched PubMed, Web of Science, Embase, and Clinical Trials for relevant literature published between January 2000 and January 2023. Selection Criteria We included all studies reporting peri‐ and postoperative outcomes of myomectomy in patients with uterine myomas. Surgical treatments were classified as RLM, LM, or AM. Data Collection and Analysis Two or more authors selected studies independently, assessed risk of bias, and extracted data. We derived mean difference (MD) or odds ratio (OR) with 95% confidence intervals (CIs) for each outcome, subgrouping trials by the patient characteristics and myoma characteristics. We used the I2 statistic to quantify heterogeneity and the random‐effects model for meta‐analysis when appropriate. We used the funnel plot to assess the publication bias. Feasibility and Perioperative Outcomes of Minimally Invasive Higher Order Myomectomy

Abstract:Study Objective To investigate perioperative outcomes of minimally invasive higher order myomectomy as defined by removal of ten or more fibroids. Design A retrospective cohort study between January 2018 and December 2022 Setting A tertiary academic medical center. Patients or Participants Women who underwent minimally invasive myomectomy via laparoscopic or robotic approach. Interventions Surgical intervention in the form of minimally invasive myomectomy. Measurements and Main Results A total of 735 women met inclusion criteria of which 578 had fewer than ten fibroids removed, and 157 patients had ten or more removed (average number of fibroids removed 3.8 vs 14.7, p < .001; specimen's weight 317.4 g vs 371.0 g, p = .07). BMI was similar in both groups (p = .66) and patients with higher order myomectomy were more likely to have a history of prior myomectomy (12.0% vs 26.8%, p < .001). The average estimated blood loss (EBL) was 246 mL vs 470 mL in each group (p < .001). There were no significant differences in packed red blood cell transfusion (1.0% vs 0.6%, p = .65), conversion to laparotomy (0.5% vs 0.6%, p = .86), or complications including visceral injury, wound complication, venous thromboembolism, ileus, or readmission (5.9% vs 4.5%, p = .49). The hospital length of stay was similar in both groups (0.5 days vs 0.5 days, p = .63). On linear regression analysis, after adjusting for specimen's weight, operative time, and history of prior myomectomy, EBL remained significantly higher in patients with ten or more fibroids removed (p= .02). Conclusion EBL is increased in higher order myomectomy, however, blood transfusions, conversion to laparotomy, complication rates, and length of hospital stay did not differ compared with patients with fewer than ten fibroids removed, highlighting the feasibility of minimally invasive higher order myomectomy.

Abstract:Objective: To evaluate and compare mini-laparotomy (MLPT) with laparoscopic (LPS) myomectomy perioperative and reproductive outcomes. Methods: We systematically searched for related articles in the MEDLINE, Embase, Web of Science and the Cochrane library databases. Nine studies (4 randomized, 3 retrospective, 1 prospective and 1 case-control study) which involved 1723 patients met the inclusion criteria and were considered eligible for inclusion. Results: Demographic characteristics were similar between the two groups. LPS was associated with shorter hospital stay (p = 0.04), lower blood loss (p < 0.00001), shorter duration of median ileus (p < 0.00001) and fewer episodes of postoperative fever (p = 0.04). None of the reproductive factors examined (pregnancy rate, preterm delivery, vaginal delivery and delivery with caesarean section) in women diagnosed with unexplained infertility and/or symptomatic leiomyomas reached statistical significance although the results represent a small size effect. Conclusion: Our analysis demonstrated that LPS seems to be an alternative, safe and reliable surgical procedure for uterine leiomyoma treatment and in everyday practice seems to offer improved outcomes-regarding at least the perioperative period-over MLPT.
An analysis of the surgical outcomes of laparoendoscopic single-site myomectomy and multi-port laparoscopic myomectomy

Abstract:BACKGROUND: This study sought to compare the surgical results of patients undergoing a laparoendoscopic single-site myomectomy (LESS-M) and a conventional laparoscopic myomectomy (CLM) at our hospital.METHODS: The basic data of 233 patients undergoing LESS-M and 233 patients undergoing CLM at the Obstetrics and Gynecology Hospital Affiliated to Fudan University were collected from January 2018 to January 2020, and the results of the operations were compared by evaluating a number of factors, including operation time, intraoperative bleeding, postoperative fever, and postoperative maximum body temperature.RESULTS: The operation times of the LESS-M and CLM groups were 83.9±33.4 and 75.2±26.7 min, respectively; the difference between the groups was statistically significant. The surgical blood loss of the LESS-M group was 86.1±76.9 mL, and that of the CLM group was 83.8±79.9 mL (P>0.05). When the diameter of a fibroid was ≥8 cm, a fibroid was located in the posterior wall or the number of fibroids was ≥4, the operation time of the CLM group was shorter than that of the LESS-M group. When the diameter of a fibroid was ≥8 cm, the blood loss of the CLM group was less than that of the LESS-M group.CONCLUSIONS: LESS-M is safe and feasible. If the diameter of a fibroid is ≥8 cm, the fibroid is located in the posterior wall, or the number of fibroids is ≥4, the utility of single-port surgery should be carefully considered.

A comparative retrospective analysis on robot-assisted laparoscopic surgery compared to conventional laparoscopy in case of myomectomy: experience in a third-level hospital of Southern Italy

Abstract:Uterine myomas are the most common gynecological disease in reproductive-aged women, present several symptoms, and require effective medical and/or surgical strategies. This study aimed to compare robotic-assisted laparoscopic myomectomy (RALM) with laparoscopic myomectomy (LM) in terms of operative times, intraoperative estimated blood loss, pre- and post-hemoglobin levels drop, and length of hospital stay. Data from 50 clinical records (25 RALM in Group A and 25 LM in Group B) of patients with uterine fibroids were collected from December 2022 to December 2023 at Gynecological Unit of DAI Materno-Infantile Federico II in Naples, Italy. Patients aged 30–49 years with symptomatic fibroids were included. Data on peri-operative outcomes, including operative time for myomectomy (OTM), overall operative time (OOT), intraoperative estimated blood loss (EBL), pre- and post-operative hemoglobin levels, and length of hospital stay were analyzed. The OTM in the presence of > 5 myomas was 59 [52–65] vs 69 min [61–96] ( p < 0.001) for RALM and LM groups, respectively. Moreover, also in presence of ≤ 5 myomas, a difference was observed in the RALM group 48[43–55] compared to the LM group 53[50–61] min ( p = 0.07). The OOT was also statistically significant for Group A compared to Group B (83[65–93] vs 72[56–110] min, p < 0.001). There were no significant differences between the two groups in terms of pre- and post-operative hemoglobin levels and EBL ( p = 0.178). Group A demonstrated a notably shorter hospital stay 1.2 [1–2] days compared to Group B 2.9[3–3.75] days ( p = 0.007). Our study suggests potential advantages of RALM over LM in terms of reduced operative times and shorter hospital stays. The standardized approach and extensive surgical experience likely contributed to the favorable outcomes of RALM.

Abstract:In the field of benign gynecology, laparoscopic myomectomy has clear advantages, although learning curve is longer. Our study aims to review the factors that improve the technique of laparoscopic myomectomies. For women with symptomatic fibroids who wish to maintain their fertility minimally invasive myomectomy is in most cases the best option. Therefore, laparoscopic approach is associated with shorter hospital stay, faster recovery, less postoperative pain, and reduced adhesion formation. Standardization of laparoscopic techniques could help to reduce the learning curve. The costs are higher compared with open myomectomy, but when we take into account the numerous advantages that laparoscopy has, including shorter operative time, less blood loss, shorter hospital stay and a more rapid recovery time, laparoscopy is more cost-efficient.
Nationwide assessment of practice variability in the utilization of hysteropexy at laparoscopic apical suspension for uterine prolapse

Abstract:Background: Although hysteropexy has been used to preserve the uterus during uterine prolapse surgery for a long time, there is a scarcity of data that describe the nationwide patterns of use of this surgical procedure. Objective: This study aimed to examine the national-level use and characteristics of hysteropexy at the time of laparoscopic apical suspension surgery for uterine prolapse in the United States. Study design: This cross-sectional study used data from the Healthcare Cost and Utilization Project's Nationwide Ambulatory Surgery Sample. The study population included 55,608 patients with a diagnosis of uterine prolapse who underwent laparoscopic apical suspension surgery from 2016 to 2019. Patients who had a hysterectomy were assigned to the hysterectomy group, and those who did not have a hysterectomy were assigned to the hysteropexy group. The main outcome was clinical characteristics associated with hysteropexy, assessed using a multivariable binary logistic regression model. A classification tree was further constructed to assess the use pattern of hysteropexy during laparoscopic apical suspension procedures. The secondary outcome was surgical morbidity, including urinary tract injury, intestinal injury, vascular injury, and hemorrhage. Results: A hysteropexy was performed in 6500 (11.7%) patients. In a multivariable analysis, characteristics associated with increased use of a hysteropexy included (1) patient factors, such as older age, Medicare coverage, private insurance, self-pay, and medical comorbidity; (2) pelvic floor dysfunction factor of complete uterine prolapse; and (3) hospital factors, including medium bed capacity center and location in the Southern United States (all P<.05). Conversely, (1) the patient factor of higher household income; (2) gynecologic factors such as uterine myoma, adenomyosis, and benign ovarian pathology; (3) pelvic floor dysfunction factor with stress urinary incontinence; and (4) hospital factors including Midwest and West United States regions and rural setting center were associated with decreased use of a hysteropexy (all P<.05). A classification tree identified a total of 14 use patterns for hysteropexies during laparoscopic apical suspension procedures. The strongest factor that dictated the use of a hysteropexy was the presence or absence of uterine myomas; the rate of hysteropexy use was decreased to 5.6% if myomas were present in comparison with 15% if there were no myomas (P<.001). Second layer factors were adenomyosis and hospital region. Patients who did not have uterine myomas or adenomyosis and who underwent surgery in the Southern United States had the highest rate of undergoing a hysteropexy (22.6%). Across the 14 use patterns, the percentage rate difference between the highest and lowest uptake patterns was 22.0%. Patients who underwent a hysteropexy were less likely to undergo anteroposterior colporrhaphy, posterior colporrhaphy, and sling procedures (all P<.05). Hysteropexy was associated with a decreased risk for measured surgical morbidity (3.0 vs 5.4 per 1000 procedures; adjusted odds ratio, 0.57; 95% confidence interval, 0.36-0.90). Conclusion: The results of these current, real-world practice data suggest that hysteropexies are being performed at the time of ambulatory laparoscopic apical suspension surgery for uterine prolapse. There is substantial variability in the application of hysteropexy based on patient, gynecologic, pelvic floor dysfunction, and hospital factors. Developing clinical practice guidelines to address this emerging surgical practice may be of use.
A survey of hepatitis C management by Victorian GPs after PBS-listing of direct-acting antiviral therapy.

Abstract:BACKGROUND To increase access to hepatitis C virus (HCV) treatment, the Pharmaceutical Benefits Scheme (PBS) enabled general practitioners (GPs) to prescribe direct-acting antiviral (DAA) therapy. We conducted a survey to identify GPs' knowledge and management of HCV. METHODS A questionnaire consisting of 20 items about HCV knowledge and management was sent to 1000 GPs. RESULTS One hundred and ninety-one GPs (19.1%) responded; 74% answered correctly that antibody and RNA positivity is diagnostic of HCV. Only 12% could directly request transient elastography. Although 53% of respondents reported interest in prescribing DAAs, 72% continued to refer all patients to specialists. Fifty-five per cent were unsure if people who currently inject drugs were eligible for treatment. DISCUSSION Most respondents were interested in prescribing DAAs, but education, access to transient elastography and clear consultation pathways are required to translate this interest into increased treatment availability. PBS eligibility of current injectors needs promotion.
Laparoscopic Myomectomy for Posterior Myomas: Analysis of 28 Cases

Abstract:Purpose To evaluate the feasibility and safety of laparoscopic myomectomy for posterior myomas.Methods Twenty-eight selected patients with posterior uterine intramural and subserous myomas were subjected to laparoscopic myomectomy.The duration of operation,interoperation blood loss,intra-and post-operation complications were evaluated and compared with forty-one patients with anterior or fundal uterine myoma during the same period.Results There was no difference in the mean diameters between the posterior uterine and anterior or fundal uterine myomas group [(53.32±21.37) mm vs.(55.02±17.99) mm,P0.05].There was no signitiant difference in the mean operation time between the two groups[(105.64±38.18) min vs.(99.41±28.88) min,P0.05].There was no significant difference in the mean blood loss between the two groups[(84.64±39.86) ml vs.(83.17±66.50) mL,P0.05].Conclusions Design the uterine incisions reasonablely and restore the integrity of the uterine anatomy and myometrium effectively will have a good rescontructive outcome for laparoscopic myomectomy of the larger posterior myomas.The indication of laparoscopic myomectomy for larger posterior myomas was expanded.

Abstract:Objective To compare the efficacy of three operative approaches (laparotomy,laparoscopy,vaginal) for hysterectomy in patients with uterine myoma.Methods From September 2009 to January 2015,in Beijing Obstetricsand Gynecology Hospital,Capital Medical University,the clinical data of 725 patients received hysterectomy for uterine myoma were retrospectively analyzed,they were divided into three groups:trans-abdominal hysterectomy group (TAH group,283 cases),trans-laparoscopic hysterectomy group (TLH,251 cases) and trans-vaginal hysterectomy group (TVH,191 cases).The operative time,intraoperative bleeding,postoperative exhaust time,eating time,body temperature,complications,hospitalization time and costs of three groups were compared.Results Three groups were compared,intraoperative bleeding of TAH group was the highest,operative time was the longest,hospitalization costs was the highest,postoperative highest body temperature,exhaust time,eating time,postoperative hospitalization time and total hospitalization time were the lowest,the differences were statistically significant (P ＜ 0.05).The complication rates of three groups were compared,the differences were not statistically significant (P ＞ 0.05).Conclusion TLH is superior to TVH and TAH in terms of intraoperative bleeding,postoperative recovery,hospitalization time and total hospitalization time,except of a longer operative time and higher hospitalization costs.All three hysterectomy approaches have their own advantages and disadvantages,suitable surgical procedure should be chosen according to different condition.
