Abstract:Surgical complications after the transthoracic operation for esophageal cancer mainly include anastomatic fistula, thoraco-stomach fistula, stenosis of anastomosis, gastric perforation, gastric volvulus, diaphragmatic hernia, wound infection, and some other pulmonary complications. Unfortunately, there are few reports about the complications caused by position change of the chest tube until now. We presented an unusual case of a patient who underwent a transthoracic operation for esophageal cancer in our department on August 17, 2006, and a lot of intragastric material was found in his chest tube 17 days later, endoscopic examination suggested that the chest tube had inserted into the stomach. We tried to discuss the etiology and clinical management for this case as well. CASE REPORT A 56-year-old Chinese man without any history of esophageal or gastric disease was newly admitted as esophageal carcinoma to our department on August 14, 2006. His complaint only was retrosternal burning for over 20 days. An endoscopic report from his local hospital suggested that a cauliform mass, of which the biopsy result was high-differentiated squamous carcinoma of esophagus, was found at the very place of 30 cm away from the incisor teeth. The patient thereafter underwent a transthoracic operation of esophagectomy and esophago-gastrostomy with the stapler manufactured by Shanghai Johnson & Johnson Corporation on August 17, 2006, and was treated with fasting, antibiotics and total parenteral nutrition (TPN) after the operation. Everything went well in the first 7 days after the operation, his temperature returned to 36.5–37.5°C, and his condition got better day by day, so we attempted to stop his gastric decompression and allow him to take some fluidity on day 7 after the operation. But soon his wife came to us recounting that she saw some white floc in the water-sealed bottle right after the patient had taken a little of soybean milk. We suspected that the patient had presented anastomatic fistula, and inserted gastric tube for decompression again immediately. On 22 day after the operation, we tried to stop the patient from gastric drainage again as his condition was under control. Two days later, the patient got a developing abdominal distension at the upper abdomen. Although the distension was quickly released by a belch, a lot of dark green juice substitutively appeared in his chest tube. Without any sign of toxic symptom such as fever or empyema, the patient received the following examinations: blood routine test showed white blood cell (WBC): 8.9×109, N: 77.5%. Esophagographic examination indicated anastomatic fistula, while endoscopic examination suggested a fistula at the edge of esophagogastric anastomosis and the chest tube was visible in the stomach (Fig.). Then we chose a kind of conservative therapy of 3 tubes: we remained the chest tube in the stomach for gastric drainage; kept gastric tube for decompression; and placed duodenal feeding tube for life support. Ten days later, through an examination of fistulography with meglucamine diatrizoate we discovered that the fistula had recovered, so we tried to pull the chest tube out gradually as the drainage volume remained below 50 ml/d. On day 39 and day 41 after the operation, the patient received a test of oral methylence blue, as no abnormal drainage in his chest tube, we pulled his chest tube out thoroughly and then stopped his gastric tube. Seven days later, the patient was discharged healthily.Fig.: The chest tube inserted into the stomachDISCUSSION The chest tube was found to touch the stomach so closely that we had to consider that the insertion was initiative. The ways to invade the stomach were three reasons as follows: First is operation skills. Anastomatic fistula is a very common complication after the operation for esophageal cancer.1,2 One review of surgical treatment for esophageal cancer indicated that many anastomatic fistula diagnosed were actually rupture of proximal esophagus or local perforation of thoraco-stomach.3 The patient with severe anastomatic fistula may present fever, empyema, and some other toxic symptoms. This complication is changeable and troublesome with highmortality. Considering these differentiative characteristics, we believed the complication our patient suffered was only a small fistula so that there was no sign of danger. On review of the patient's history of operation, we found some potential risks: (1) During the operation the stomach was dragged and kneaded too excessively to recover its blood-supply, and this may cause extensive capillary thromboembolism and lead to gastric perforation.4,5 (2) Some small eyelets left when the needle penetrated the flimsy gastric wall may be enlarged by tying, severe cough, gastric dilatation, and some other stimulations. (3) The suture was too close to the anamostmatic nail, which may cause ischemic necrosis. (4) The sutures were made too dense, leading to edema of the anastomosis. (5) The stapler was pressed so heavily that the borderline membrane and muscle of anastomosis were all cut off. Second is position change of the chest tube. We exactly placed the chest tube at the place of 3 cm away from the anastomosis, while finally the endoscopic examination showed clearly that the chest tube was in the stomach. The reason was that we restored two-lung breathing mode after the closure of the incision during the operation, and the re-expanded left lung distorted the tube. On the other hand, we made the anastomosis above the aortic arch, and then fastened the stomach into the esophageal bed by hand-sutured gastric chorion with pleura to lessen the tension. The fastened stomach provided a fastened point to suffer the stimulation from the distorted chest tube. Finally, it seemed too early for the patient to take fluidity, when his gastrointestinal function had not recovered, gastric filling and expanding reduced the intra-thoracic space. Meanwhile, a misleading education made the patient cough incorrectly, persistent and severe cough might be the original motivation of chest tube insertion. As noted above, inexperienced operation skill was a basic factor, the variated chest tube was a key factor, and lasting cough was a motivational factor, these three factors influenced each other and occurred together to lead to this rare complication. Therefore, we should take preventions from three aspects: First of all, surgeons should be cautious when taking some actions during the operation, for instance, perform more gently during the operation, and do not suture too close to the gastric wall to avoid mechanical injury of the stomach; make sure that the acus has penetrated the entire layer when taking anastomosis; do not introduce the anvil of the stapler into the lumen of esophagus roughly; keep moderate pressure for about 8–10 seconds before fire the stapler; do not suture too closely to the anastomatic nails; pay attention to the strength when tying sutures over the anastomosis so as to avoid enlarging the eyelets or worsening the gastric blood-supply; fasten the stomach into the esophageal bed when make a supra-aortic arch anastomosis; restore two-lung breathing mode before close the incision to prevent the placed chest tube from position changing. Additionally, for those patients with poor nutrition, providing life support before operation and taking the management of gastric tube, chest tube, duodenal feeding tube therapy after operation are necessary,6 and patient who has been just stopped gastric decompression, should not be allowed to take food immediately so as to avoid complications caused by gastric expanding. Furthermore, surgeons should educate their patients about the correct way of cough, and use some medicine if necessary, to avoid lasting and severe cough postoperatively. Once the chest tube inserted into the stomach, intragastric content would be found in the water-sealed bottle, so we could diagnose easily through an endoscopic examination. In this case, the patient's symptom was not very emergent and severe so that it seemed would bring severe trauma if a re-operation was applied immediately, moreover, pus and digestive juice was all assembling at the place of fistula, undoubtedly re-operation would confront many difficulties. Consequently, we chose a conservative therapy: remained the chest tubes stay in the stomach, and added vacuum pressure for drainage so as to re-expand the lung, form conglutination, eliminate dead cavities, and placed gastric tube all along for gastric decompression, kept duodenal feeding tube for life support. We examined the condition of fistula by antegrade esophagography and retrograde fistulography of meglucamine diatrizoate, and then pulled out the chest tube from the stomach little by little according to the drainage volume when confirmed that the fistula had recovered.
