Abstract:AbstractBackgroundAlzheimer’s disease (AD) is a progressive and debilitating neurological disorder that impacts millions of individuals globally. Prompt and accurate detection and diagnosis of Alzheimer’s disease are pivotal for providing effective care and management to patients. However, early identification of the disease remains challenging due to the complexity of diagnostic procedures and the similarity of symptoms with age‐related cognitive decline. With the world transitioning towards a more digital‐focused environment, there is an increasing potential to utilize human‐computer interaction technologies to aid in the early detection of Alzheimer’s disease.MethodWe recruited 40 healthy individuals, 40 patients with mild cognitive impairment (MCI), and 40 patients with early AD. By incorporating fusion perception‐enhancing technologies into intelligent identification devices, such as eye‐tracking and body motion detection, peripheral modal biometric information can be gathered from subjects.ResultThe study found that in human‐computer interaction experiments using eye‐tracking, the patients with early AD showed significantly longer completion times and free rates of eye motion and movement trajectory than the normal group and those with MCI. The correlation between eye motion trajectory and movement trajectory was notably lower as compared to the normal group and the MCI group. During the human‐computer interaction physical movement experiment, the AD group recorded lower target accuracy rate and screen click rate as compared to the normal group and patients with MCI. Similarly, in the human‐computer interaction clock drawing experiment, the AD group demonstrated longer completion time and pen‐up thinking time, as well as lower clock drawing scores. Furthermore, the total completion time of human‐computer interaction tasks was notably shorter as compared to conventional cognitive screening tests. In conclusion, peripheral biomarkers obtained from the aforementioned human‐computer interaction task were analyzed using machine learning techniques, resulting in the identification of early‐stage Alzheimer’s disease with a classification accuracy of 91.1%.ConclusionHence, this non‐invasive and accurate technique may prove instrumental in identifying potential cognitive deficits associated with this disease while also being widely applicable for screening early AD.

Abstract:An 8-year-old boy suffered from an unconsciousness attack and torsade de pointes arrhythmia during sleep or at rest. His electrocardiogram showed prolonged QT intervals, but the T wave morphology was atypical for type 1, 2 or 3 congenital long-QT syndrome (LQTS). Intravenous epinephrine slightly prolonged the QT interval whereas mexiletine infusion shortened the QT interval. Although these clinical characteristics might suggest type 3 LQTS, a genetic analysis identified the G604S-KCNH2 mutation (type 2 LQTS). Because mismatches between the genotype and phenotype of LQTS are possible, genetic analysis of LQTS is important to identify the most appropriate therapeutic option and risk stratification.
Smart Home Technology: A New Approach for Performance Measurements of Activities of Daily Living and Prediction of Mild Cognitive Impairment in Older Adults

Abstract:Background: Functional assessment is of paramount importance when mild cognitive impairment is suspected, but common assessment tools such as questionnaires lack sensitivity. An alternative and innovative approach consists in using sensor technology in smart apartments during scenario-based assessments of instrumental activities of daily living (IADL). However, studies that investigate this approach are scarce and the technology used is not always transposable in healthcare settings. Objective: To explore whether simple and wireless technology used in two different smart environments could add value to performance and rater-based measures of IADL when it comes to predicting mild cognitive impairment (MCI) in older adults. Methods: Twenty-six (26) cognitively healthy older adults (CH) and 22 older adults with MCI were recruited. Functional performance in a set of five scripted tasks was evaluated with sensor-based observations (motion, contact, and electric sensors) and performance-based measures (rated with videotapes). The five tasks could be performed in any order and were detailed on an instruction sheet given to participants. Results: Sensor-based observations showed that participants with MCI spent more time in the kitchen and looking into the fridge and kitchen cabinets than CH participants. Moreover, these measures were negatively associated with memory and executive performances of participants and significantly contributed to the prediction of MCI. Conclusion: Simple, wireless, and sensor-based technology holds potential for the detection of MCI in older adults as they perform daily tasks. However, some limits are discussed and we offer recommendations to improve the usefulness of this innovative approach.

Abstract:Introduction: Assessing functional decline related to activities of daily living (ADLs) is deemed significant for the early diagnosis of dementia. As current assessment methods for ADLs often lack the ability to capture subtle changes, technology-based approaches are perceived as advantageous. Specifically, digital biomarkers are emerging, offering a promising avenue for research, as they allow unobtrusive and objective monitoring. Methods: A study was conducted with the involvement of 36 participants assigned to three known groups (Healthy Controls, participants with Subjective Cognitive Decline and participants with Mild Cognitive Impairment). Participants visited the CERTH-IT Smart Home, an environment that simulates a fully functional residence, and were asked to follow a protocol describing different ADL Tasks (namely Task 1 – Meal, Task 2 – Beverage and Task 3 – Snack Preparation). By utilizing data from fixed in-home sensors installed in the Smart Home, the identification of the performed Tasks and their derived features was explored through the developed CARL platform. Furthermore, differences between groups were investigated. Finally, overall feasibility and study satisfaction were evaluated. Results: The composition of the ADLs was attainable, and differentiation among the HC group compared to the SCD and the MCI groups considering the feature "Activity Duration" in Task 1 – Meal Preparation was possible, while no difference could be noted between the SCD and the MCI groups. Discussion: This ecologically valid study was determined as feasible, with participants expressing positive feedback. The findings additionally reinforce the interest and need to include people in preclinical stages of dementia in research to further evolve and develop clinically relevant digital biomarkers.

Abstract:Abstract An 8-yr-old male leopard (Panthera pardus) was presented with a 4-day history of lethargy, vomiting, and anorexia. Thoracic and abdominal radiographs revealed a soft-tissue mass cranial to the diaphragm and atypical appearance of the gastric fundus. Esophagoscopy revealed gastric mucosa in the lumen of the esophagus, which confirmed gastroesophageal intussusception. An exploratory celiotomy with manual reduction of the intussusception was performed. Reduction was verified by intraoperative esophagoscopy and gastroscopy. An incisional fundic gastropexy to the left abdominal wall was performed to reduce the chance of a recurrence of the intussusception. No postoperative complications related to the surgery were observed, and the animal resumed eating within 48 hr of surgery. A subsequent recurrence of clinical signs was not noted by the owner.
Automatically detecting activities of daily living from in-home sensors as indicators of routine behaviour in an older population

Abstract:Objective: The NEX project has developed an integrated Internet of Things (IoT) system coupled with data analytics to offer unobtrusive health and wellness monitoring supporting older adults living independently at home. Monitoring {currently} involves visualising a set of automatically detected activities of daily living (ADLs) for each participant. The detection of ADLs is achieved {} to allow the incorporation of additional participants whose ADLs are detected without re-training the system. Methods: Following an extensive User Needs and Requirements study involving 426 participants, a pilot trial and a friendly trial of the deployment, an Action Research Cycle (ARC) trial was completed. This involved 23 participants over a 10-week period each with c.20 IoT sensors in their homes. During the ARC trial, participants each took part in two data-informed briefings which presented visualisations of their own in-home activities. The briefings also gathered training data on the accuracy of detected activities. Association rule mining was then used on the combination of data from sensors and participant feedback to improve the automatic detection of ADLs. Results: Association rule mining was used to detect a range of ADLs for each participant independently of others and was then used to detect ADLs across participants using a single set of rules {for each ADL}. This allows additional participants to be added without the necessity of them providing training data. Conclusions: Additional participants can be added to the NEX system without the necessity to re-train the system for automatic detection of the set of their activities of daily living.

Abstract:Background: Apathy, depression, and anxiety are prevalent neuropsychiatric symptoms experienced by older adults. Early detection, prevention, and intervention may improve outcomes. Objective: We aim to demonstrate the feasibility of deploying web-based weekly questionnaires inquiring about the behavioral symptoms of older adults with normal cognition, mild cognitive impairment, or early-stage dementia and to demonstrate the feasibility of deploying an in-home technology platform for measuring participant behaviors and their environment. Methods: The target population of this study is older adults with normal cognition, mild cognitive impairment, or early-stage dementia. This is an observational, longitudinal study with a study period of up to 9 months. The severity of participant behavioral symptoms (apathy, depression, and anxiety) was self-reported weekly through web-based surveys. Participants' digital biomarkers were continuously collected at their personal residences and through wearables throughout the duration of the study. The indoor physical environment at each residence, such as light level, noise level, temperature, humidity, or air quality, was also measured using indoor environmental sensors. Feasibility was examined, and preliminary correlation analysis between the level of symptoms and the digital biomarkers and between the level of symptoms and the indoor environment was performed. Results: At 13 months after recruitment began, a total of 9 participants had enrolled into this study. The participants showed high adherence rates in completing the weekly questionnaires (response rate: 275/278, 98.9%), and data collection using the digital technology appeared feasible and acceptable to the participants with few exceptions. Participants' severity of behavioral symptoms fluctuated from week to week. Preliminary results show that the duration of sleep onset and noise level are positively correlated with the anxiety level in a subset of our participants. Conclusions: This study is a step toward more frequent assessment of older adults' behavioral symptoms and holistic in situ monitoring of older adults' behaviors and their living environment. The goal of this study is to facilitate the development of objective digital biomarkers of neuropsychiatric symptoms and to identify in-home environmental factors that contribute to these symptoms.

Abstract:The ubiquity of sensors in smart-homes facilitates the support of independent living for older adults and enables cognitive assessment. Notably, there has been a growing interest in utilizing movement traces for identifying signs of cognitive impairment in recent years. In this study, we introduce an innovative approach to identify abnormal indoor movement patterns that may signal cognitive decline. This is achieved through the non-intrusive integration of smart-home sensors, including passive infrared sensors and sensors embedded in everyday objects. The methodology involves visualizing user locomotion traces and discerning interactions with objects on a floor plan representation of the smart-home, and employing different image descriptor features designed for image analysis tasks and synthetic minority oversampling techniques to enhance the methodology. This approach distinguishes itself by its flexibility in effortlessly incorporating additional features through sensor data. A comprehensive analysis, conducted with a substantial dataset obtained from a real smart-home, involving 99 seniors, including those with cognitive diseases, reveals the effectiveness of the proposed functional prototype of the system architecture. The results validate the system's efficacy in accurately discerning the cognitive status of seniors, achieving a macro-averaged F1-score of 72.22% for the two targeted categories: cognitively healthy and people with dementia. Furthermore, through experimental comparison, our system demonstrates superior performance compared with state-of-the-art methods.

Abstract:A challenge associated with an ageing population is increased demand on health and social care, creating a greater need to enable persons to live independently in their own homes. Ambient assistant living technology aims to address this by monitoring occupants' 'activities of daily living' using smart home sensors to alert caregivers to abnormalities in routine tasks and deteriorations in a person's ability to care for themselves. However, there has been less focus on using sensing technology to monitor a broader scope of so-called 'meaningful activities', which promote a person's emotional, creative, intellectual, and spiritual needs. In this paper, we describe the development of a toolkit comprised of off-the-shelf, affordable sensors to allow persons with dementia and Parkinson's disease to monitor meaningful activities as well as activities of daily living in order to self-manage their life and well-being. We describe two evaluations of the toolkit, firstly a lab-based study to test the installation of the system including the acuity and placement of sensors and secondly, an in-the-wild study where subjects who were not target users of the toolkit, but who identified as technology enthusiasts evaluated the feasibility of the toolkit to monitor activities in and around real homes. Subjects from the in-the-wild study reported minimal obstructions to installation and were able to carry out and enjoy activities without obstruction from the sensors, revealing that meaningful activities may be monitored remotely using affordable, passive sensors. We propose that our toolkit may enhance assistive living systems by monitoring a wider range of activities than activities of daily living.

Smart Home Sensing and Monitoring in Households With Dementia: User-Centered Design Approach

Abstract:Background As life expectancy grows, so do the challenges of caring for an aging population. Older adults, including people with dementia, want to live independently and feel in control of their lives for as long as possible. Assistive technologies powered by artificial intelligence and internet of things devices are being proposed to provide living environments that support the users’ safety, psychological, and medical needs through remote monitoring and interventions. Objective This study investigates the functional, psychosocial, and environmental needs of people living with dementia, their caregivers, clinicians, and health and social care service providers toward the design and implementation of smart home systems. Methods We used an iterative user-centered design approach comprising 9 substudies. First, semistructured interviews (9 people with dementia, 9 caregivers, and 10 academic and clinical staff) and workshops (35 pairs of people with dementia and caregivers, and 12 health and social care clinicians) were conducted to define the needs of people with dementia, home caregivers, and professional stakeholders in both daily activities and technology-specific interactions. Then, the spectrum of needs identified was represented via patient–caregiver personas and discussed with stakeholders in a workshop (14 occupational therapists; 4 National Health Service pathway directors; and 6 researchers in occupational therapy, neuropsychiatry, and engineering) and 2 focus groups with managers of health care services (n=8), eliciting opportunities for innovative care technologies and public health strategies. Finally, these design opportunities were discussed in semistructured interviews with participants of a smart home trial involving environmental sensors, physiological measurement devices, smartwatches, and tablet-based chatbots and cognitive assessment puzzles (10 caregivers and 2 people with dementia). A thematic analysis revealed factors that motivate household members to use these technologies. Results Outcomes of these activities include a qualitative and quantitative analysis of patient, caregiver, and clinician needs and the identification of challenges and opportunities for the design and implementation of remote monitoring systems in public health pathways. Conclusions Participatory design methods supported the triangulation of stakeholder perspectives to aid the development of more patient-centered interventions and their translation to clinical practice and public health strategy. We discuss the implications and limitations of our findings, the value and the applicability of our methodology, and directions for future research.
Smart Aging System: Uncovering the Hidden Wellness Parameter for Well-Being Monitoring and Anomaly Detection

Abstract:Background: Ambiguities and anomalies in the Activity of Daily Living (ADL) patterns indicate deviations from Wellness. The monitoring of lifestyles could facilitate remote physicians or caregivers to give insight into symptoms of the disease and provide health improvement advice to residents; Objective: This research work aims to apply lifestyle monitoring in an ambient assisted living (AAL) system by diagnosing conduct and distinguishing variation from the norm with the slightest conceivable fake alert. In pursuing this aim, the main objective is to fill the knowledge gap of two contextual observations (i.e., day and time) in the frequent behavior modeling for an individual in AAL. Each sensing category has its advantages and restrictions. Only a single type of sensing unit may not manage composite states in practice and lose the activity of daily living. To boost the efficiency of the system, we offer an exceptional sensor data fusion technique through different sensing modalities; Methods: As behaviors may also change according to other contextual observations, including seasonal, weather (or temperature), and social interaction, we propose the design of a novel activity learning model by adding behavioral observations, which we name as the Wellness indices analysis model; Results: The ground-truth data are collected from four elderly houses, including daily activities, with a sample size of three hundred days plus sensor activation. The investigation results validate the success of our method. The new feature set from sensor data fusion enhances the system accuracy to (98.17% ± 0.95) from (80.81% ± 0.68). The performance evaluation parameters of the proposed model for ADL recognition are recorded for the 14 selected activities. These parameters are Sensitivity (0.9852), Specificity (0.9988), Accuracy (0.9974), F1 score (0.9851), False Negative Rate (0.0130).

Abstract:Cognitive impairment has become epidemic in older adult population. The recent advent of tiny wearable and ambient devices, a.k.a Internet of Things (IoT) provides ample platforms for continuous functional and cognitive health assessment of older adults. In this paper, we design, implement and evaluate AutoCogniSys, a context-aware automated cognitive health assessment system, combining the sensing powers of wearable physiological (Electrodermal Activity, Photoplethysmography) and physical (Accelerometer, Object) sensors in conjunction with ambient sensors. We design appropriate signal processing and machine learning techniques, and develop an automatic cognitive health assessment system in a natural older adults living environment. We validate our approaches using two datasets: (i) a naturalistic sensor data streams related to Activities of Daily Living and mental arousal of 22 older adults recruited in a retirement community center, individually living in their own apartments using a customized inexpensive IoT system (IRB #HP-00064387) and (ii) a publicly available dataset for emotion detection. The performance of AutoCogniSys attests max. 93\% of accuracy in assessing cognitive health of older adults.

Abstract:Activity of daily living (ADL) is a significant predictor of the independence and functional capabilities of an individual. Measurements of ADLs help to indicate one's health status and capabilities of quality living. Recently, the most common ways to capture ADL data are far from automation, including a costly 24/7 observation by a designated caregiver, self-reporting by the user laboriously, or filling out a written ADL survey. Fortunately, ubiquitous sensors exist in our surroundings and on electronic devices in the Internet of Things (IoT) era. We proposed the ADL Recognition System that utilizes the sensor data from a single point of contact, such as smartphones, and conducts time-series sensor fusion processing. Raw data is collected from the ADL Recorder App constantly running on a user's smartphone with multiple embedded sensors, including the microphone, Wi-Fi scan module, heading orientation of the device, light proximity, step detector, accelerometer, gyroscope, magnetometer, etc. Key technologies in this research cover audio processing, Wi-Fi indoor positioning, proximity sensing localization, and time-series sensor data fusion. By merging the information of multiple sensors, with a time-series error correction technique, the ADL Recognition System is able to accurately profile a person's ADLs and discover his life patterns. This paper is particularly concerned with the care for the older adults who live independently.
Feasibility and usability of remote monitoring in Alzheimer's disease

Abstract:Introduction Remote monitoring technologies (RMTs) can measure cognitive and functional decline objectively at-home, and offer opportunities to measure passively and continuously, possibly improving sensitivity and reducing participant burden in clinical trials. However, there is skepticism that age and cognitive or functional impairment may render participants unable or unwilling to comply with complex RMT protocols. We therefore assessed the feasibility and usability of a complex RMT protocol in all syndromic stages of Alzheimer's disease and in healthy control participants. Methods For 8 weeks, participants (N = 229) used two activity trackers, two interactive apps with either daily or weekly cognitive tasks, and optionally a wearable camera. A subset of participants participated in a 4-week sub-study (N = 45) using fixed at-home sensors, a wearable EEG sleep headband and a driving performance device. Feasibility was assessed by evaluating compliance and drop-out rates. Usability was assessed by problem rates (e.g., understanding instructions, discomfort, forgetting to use the RMT or technical problems) as discussed during bi-weekly semi-structured interviews. Results Most problems were found for the active apps and EEG sleep headband. Problem rates increased and compliance rates decreased with disease severity, but the study remained feasible. Conclusions This study shows that a highly complex RMT protocol is feasible, even in a mild-to-moderate AD population, encouraging other researchers to use RMTs in their study designs. We recommend evaluating the design of individual devices carefully before finalizing study protocols, considering RMTs which allow for real-time compliance monitoring, and engaging the partners of study participants in the research.

A Social Robot-facilitated Performance Assessment of Self-care Skills for People with Alzheimer's: A Preliminary Study

Abstract:Worldwide, there are approximately 10 million new cases of dementia reported each year. Due to impairments in cognitive functioning such as loss of memory, linguistic abilities, and problem-solving skills, persons living with dementia (PLWDs) may face challenges in accomplishing activities of daily living (ADLs). Socially assistive robotics (SAR) holds promise as an effective tool to assist PLWDs with their ADLs; however, developing a powerful, reliable SAR requires a robust dataset of PLWDs performing ADLs to ensure the efficacy and reliability of the SAR. In this paper, we present the development of a teleoperated robotic platform using a humanoid social robot. This platform provided instructions to perform ten Performance Assessment of Self-care Skills (PASS) tasks, while multimodal sensors were employed to collect the individual's physiological signals, behaviors, and interactions with the robot. We conducted a preliminary study to investigate the acceptability, user experience, and feasibility of the platform in a simulated home environment with five young cognitively normal individuals. Participants' experiences and satisfaction with the platform were evaluated through a questionnaire. The results demonstrate the capability of the robot administering PASS tasks. However, one limitation of the current platform is the low efficiency of controlling the robot to move in the Smart Home. Our next step is to implement navigational abilities in the robot and then conduct the experiments with a large cohort of PLWDs to generate our multimodal dataset. This work aims to contribute significantly towards the development of a powerful, reliable, and trustworthy SAR to help PLWDs with their ADLs.

An Intelligent Assistive System Based on Augmented Reality and Internet of Things for Patients with Alzheimer's Disease

Abstract:Independent life of the individuals suffering from Alzheimer's disease (AD) is compromised due to their memory loss. As a result, they depend on others to help them lead their daily life. In this situation, either the family members or the caregivers offer their help; they attach notes on every single object or take out the contents of a drawer to make those visible when they leave the patient alone. The aim of this thesis is to provide multi-level support and some helping means for AD patients and their family members through the integration of existing science and methods. This study reports results on an intelligent assistive (IA) system, achieved through the integration of Internet of Things (IoT), augmented reality (AR), and adaptive fuzzy decision-making methods. The proposed system has four main components; (1) a location and heading data stored in the local fog layer, (2) an AR device to make interactions with the AD patient, (3) a supervisory decision-maker to handle the direct and environmental interactions with the patient, (4) and a user interface for family or caregivers to monitor the patient's real-time situation and send reminders once required. The system operates in different modes, including automated and semi-automated. The first one helps the user complete the activities in their daily life by showing AR messages or making automatic changes. The second one allows manual changes after the real-time assessment of the user's cognitive state based on the AR game score. We provide further evidence that the accuracy, reliability and response time of the IA system are appropriate to be implemented in AD patients' homes. Moreover, the system response in the semi-automated mode causes less data loss than the automated mode, as the number of active devices decreases.

Down-Regulation of Monocyte Apoptosis by Phagocytosis of Platelets: Involvement of a Caspase-9, Caspase-3, and Heat Shock Protein 70-Dependent Pathway1

Abstract:Monocytes interact and cross-talk with platelets in many settings including inflammation, hemostasis, or vascular disorders. During inflammatory diseases, there is a rapid targeting of monocytes and platelets to points of inflammation and endothelial injury, where they lie side-by-side. In this in vitro study, we investigated different interactions between monocytes and platelets and elucidated whether platelets might affect monocyte apoptosis. Freshly isolated human monocytes were rendered apoptotic by serum deprivation or CD95 ligation and cocultured with platelets. Monocyte apoptosis was determined by flow cytometry, TUNEL staining, DNA electrophoresis, and transmission electron microscopy imaging. We could show that monocyte apoptosis was highly suppressed when platelets were added to the cultures. Transmission electron microscopy depicted that monocytes completely ingested thrombocytes by phagocytosis. Blocking thrombocyte uptake by the phagocytosis inhibitor cytochalasin D abrogated the enhanced monocyte survival and led to high apoptosis levels. Monocyte survival was paralleled by down-regulation of caspase-9 and -3 and up-regulation of heat shock protein 70 during uptake of platelets. Platelet supernatants and contents of platelet granules were ineffective in altering monocyte senescence. Also, ingestion of latex beads or zymosan by monocytes was ineffective to mimic platelet-dependent rescue from apoptosis. In conclusion, this study shows that platelets can suppress apoptosis of monocytes by a specific phagocytosis-dependent process with further consequences for atherosclerotic or inflammatory conditions.

Abstract:Specialised stockings and bandages are utilised for compression therapy in venous ulcer treatment. A successful therapy depends on an optimal fit of those stockings and bandages. In many cases treatment is not optimal because the patients are not able to properly apply stockings or bandages or because the pressure gradient of the synthetic fibre degrades over time. Currently, afflicted people have no tools to manage their treatment besides regularly visiting dermatology clinics. An approach for a biotelemetry pressure sensing system is presented. It is designed to be an easy-to-use and reliable measurement technique that allows patients to monitor and control their therapy. This will increases their compliance, which is admired by dermatologists.

Abstract:In this article, we presented a multimodal approach to monitor older adults’ activities of daily living (ADLs) using the combination of a wearable device and a companion robot. A dynamic Bayesian network (DBN) model was developed for activity recognition, which fuses different data, including location, object, sound event, body action, and time. The walking action is detected as the transition between consecutive activities, which helps capture the inception of activities and save energy on the wearable device. Three tests were conducted to evaluate the proposed approach. First, multiple daily activities were simulated and evaluated the approach based on a public ADL dataset. Second, the proposed approach was tested based on an offline dataset collected in our smart home testbed, which contains images, sound events, motion, and time data. Third, the proposed approach was tested in real time and a web-based interface was developed, which helps caregivers better monitor the ADLs of older adults and provide further assistance. In the offline test and the real-time test, the results show that the system achieved 91% and 93% activity detection ratio, respectively, which significantly outperformed the baseline periodic sampling methods. In addition, the camera and microphone sensor trigger times were reduced from 1537 to 140 and 78, leading to energy reduction of 36.0% and 37.6% on the wearable device, respectively.
