Abstract:Introduction: Given the increase in life expectancy in the general population of our country, there is an exponential increase since the last decades of functional older adults who undergo total knee replacement (TKR). There is a direct relationship between the increase in age and the prevalence of both functional and cognitive disabling chronic diseases, however, little we know about whether age is an independent factor in predicting worse functional outcomes and readmissions after TKR. The objective of this study was to evaluate the clinical-functional results and unplanned readmissions within the first 90 postoperative days in patients older than 80 years compared with a control group of patients younger than 80 years. Methods: From our institutional patient database, 450 patients who underwent TKR between 2016 and 2019 were retrospectively analyzed. All patients had the medical assurance of Hospital Italiano (Plan de Salud), for which none of these was lost on the follow-up nor were treated in another hospital. Patients were divided in two groups: Group A with 186 patients over 80 years and a control group B with 264 patients between 70 and 80 years. The fragility of these was defined according to the Charlson Comorbidity Index and the Simple Frail Score. Comorbidities were divided in eight groups to define which were the most influential in the final results. Results: No significant differences were observed in terms of unplanned readmissions, pain or in functional scores within 90 days between both groups. There was a significant difference in the length of postoperative hospital stay in favor of group A (A: 2.56 SD + - 0.76, B: 4.08 SD = - 2.23; p = 0.00001). The Charlson score was higher in the group of patients older than 80 years (p = 0.02) as well as the Simple Frail Score (p = 0.004). The ASA score did not show significant differences between both groups. Conclusion: Age as an independent factor proved not to be a predictor by itself of unplanned readmissions or worse clinical-functional results in a period of 90 postoperative days between both groups. We believe that the preoperative evaluation of octogenarian patients should be multidisciplinary, with special attention to the identification of comorbidities that can influence the fragility of a patient and the optimization of the pathology.

Abstract:Objective To explore the outcomes and risks of unicompartmental knee arthroplasty (UKA) versus total knee arthroplasty (TKA) in the patients over 75 years old of knee osteoarthritis. Methods From Jan 2010 to Dec 2015, 67 consecutive knee osteoarthritis over 75 years old were treated by UKA in the department of joint surgery , ChinaJapan Friendship Hospital , and the other 67 patients were treated by TKA in the same period .All these patients were enrolled and matched with respect to age , gender and radiological grade of knee arthrosis .Major postoperative complications , the length of hospital stay, pain visual analogue score (VAS), range of motion (ROM) and hospital for special surgery (HSS) score were evaluated by paired t test.Results The mean age and follow-up time were comparable between the two groups (P>0.05).Two cases were lost during the follow-up in each group.Two cases of UKA and three cases of TKA died of causes not related to arthroplasty before the last follow-up.The preoperative VAS score, ROM and HSS score were not significant different between the two groups .In the UKA group, postoperative hospital-stay was (7.1 ±1.1) days and in the TKA group it was (9.2 ±2.0) days.There was significant difference between the two groups ( t=-7.603, P<0.05) .The patients of the UKA group showed significantly higher mean postoperative ROM and hemoglobin level than the TKA group : ROM, (123.5 ±9.1)°vs (120.1 ±9.3)°( t =2.113, P <0.05);hemoglobin, (108.8 ±13.1) g/L vs (90.0 ±15.7) g/L (t=7.544, P<0.05).The blood transfusion rate was significantly higher in the TKA group than the UKA group ( 61/67 vs 1/67 ) .The postoperative pain VAS score and HSS score were comparable between the two groups, (P>0.05).One case developed infection and three cases developed incision complications in the TKA group .Cardiovascular complication developed in three cases of the UKA group and in 10 cases of the TKA group .Postoperative acute mental disorder occurred in two cases of the UKA group and 11 cases of the TKA group .Respiratory complication developed in two cases of the UKA group and three cases of the TKA group .Digestive system complication was observed in one case of the UKA group and three cases of the TKA group; urinary system complication developed in two cases of the UKA group and nine cases of the TKA group .Conclusion Compared with TKA, UKA is a more safe method for the patients over 75 years old of knee osteoarthritis with fewer complication , less transfusion and quick recovery .
Should Chronological Age be a Consideration in Patients Undergoing Elective Primary Total Knee Arthroplasty?

Abstract:Introduction The optimal time for total knee arthroplasty (TKA) requires a balance between patient disability and health state in order to minimize complications. While chronological age has not been shown to be predictive of complications in elective surgical patients, there is a point beyond which even optimized elderly patients would be at increased risk for complications. The purpose of this study was to examine the impact of chronological age on complications following primary TKA. Methods Using an administrative database, the records of 2,129,191 patients undergoing elective unilateral TKA between 2006 and 2021 were reviewed. The primary outcomes of interest were cardiac and pulmonary complications, and their relationship to the Charlson-Deyo Comorbidity Index (CDI) and chronological age. Secondary outcomes included risk of renal, neurologic, infection, and intensive care utilization postoperatively. The results were analyzed using a graphical method. The impact of chronological age as a modifier of overall risk for complications was modeled as a continuous variable. An age cutoff threshold of 80 years was also assigned for clinical convenience. Results The risk of complications correlated more closely to the Charlson-Deyo index (OR [odds ratio] 1.37 to 2.1) than chronological age (OR 1.0 to 1.1) across the various complications [Table-1. However, beyond age 80 years, the risks of cardiac, pulmonary, renal, and cerebrovascular complications were significantly increased for all CDI categories (OR 1.73 to 3.40) compared to patients below age 80 years [Table-2] [Figures 1A and 1B]. Conclusions Chronologic age can impact the risk of complications even in well-optimized elderly patients undergoing primary TKA. As arthroplasty continues to transition to outpatient settings and inpatient denials increase, these results can help patients, physicians, and payors mitigate risk while optimizing the allocation of resources.

Abstract:Background: The choice between unicondylar knee arthroplasty (UKA) and total knee arthroplasty (TKA) is likely to have long-term implications for patient-reported health outcomes. However, high-quality studies that compare the outcomes of TKA and UKA and their effects are still lacking in the literature. Thus, the aim of the present study was to compare the UKA and TKA techniques with regard to functional outcomes and perioperative complications in patients who had isolated medial osteoarthritis. Methods: This was a retrospective, single-center, matched-controlled study performed with approval of our hospital (Kunshan hospital of Traditional Chinese Medicine affiliated to Nanjing University of Traditional Chinese Medicine), with the ethics number KZY2020-37. To reduce the effect of selection bias and potential confounding in this observational study, a 1:1 matching algorithm was applied. The groups were split by sex, age to within 6 years, and body mass index within 5 kg/m(2). Thus, we retrospectively reviewed the records of 240 consecutively enrolled patients who underwent UKA and 240 patients who underwent TKA from January 2013 to June 2015 from the database of our institution. Written informed consent was obtained from all subjects participating in the trial. Clinical outcomes included range of motion, Short Form 12 score, new Knee Society Score, Western Ontario and McMaster Universities Arthritis Index, and the complications. The outcome measures were evaluated by a physiotherapist and were assessed preoperatively and postoperatively at 6 months and 2 years. The mean follow-up time was 3 years. Conclusion: We hypothesized that there was no significant difference between the 2 groups in terms of postoperative outcomes.

Abstract:OBJECTIVE:To evaluate the results of total knee arthroplasty (TKA) in patients with posttraumatic degenerative arthritis due to a previous fracture around the knee.METHODS:We analyzed the results of 15 TKAs, performed from 1997 to 2003, in 15 patients with post-traumatic degenerative arthritis due to a previous fracture around knee. There were 3 women and 12 men with an average age of 58 years (range, 31-76 years). The time from fracture to arthroplasty averaged 8.2 years (range, 2-27 years). Internal fixation had previously been performed in 8 patients resulting in retained hardware. At the time of arthroplasty a femoral fracture malunion was present in two knees. Lateral retinacular release (4 knees), extensor mechanism realignment (1 knee) or medial collateral ligament reconstruction (1 knee) were needed at the time of arthroplasty.RESULTS:Follow-up averaged 35 months (range, 12-73 months). No patient was lost for follow-up. According to the Knee Society Score scale, the mean preoperative knee score was 37 (range, 10-70) and functional score was 41 (range, 0-60). They were improved significantly to a mean of 84 (range, 10-100) and 76 (range, 20-100) points, respectively at the latest follow-up. The mean knee arc of motion were improved from 84 degree preoperation to 94 degree at the latest follow-up. Postoperative manipulation under anesthesia for poor motion was carried out in 4 knees. No knee had aseptic loosening that required subsequent revision. Two knees developed superficial infection and were treated with debridement. It subsequently recovered with the retention of components.CONCLUSIONS:Significant improvement in function and relief of pain has been achieved in patients with previous fractures undergoing subsequent TKA. However, this procedure is technically demanding and patients are at increased risk for restricted motion and need more care following TKA. This study suggests that the outcome of TKA may be improved further by making special efforts to restore limb alignment, to ensure correct component positioning, and to manage soft tissue balance.

Abstract:Abstract Purpose The primary goal of this study is to compare clinical outcomes, complication rate, and survivorship in octogenarians who underwent total knee arthroplasty (TKA) or unicompartmental knee arthroplasty (UKA) by performing a matched cohort analysis. Methods We analyzed 75 medial UKAs performed by a single experienced surgeon. The included cases were matched with 75 TKAs performed during the same study period. Potential TKA matches used identical exclusion criteria. UKAs were age-, gender-, and body mass index (BMI)-matched at the rate of 1 UKA to 1 TKA from our departmental database. Clinical evaluation included the visual analog scale for pain, range of motion (ROM—flexion and extension), Knee Society Score (KSS), and Oxford Knee Score (OKS). Each patient was clinically evaluated on the day before the surgery ( T 0 ) and at two follow-ups at least 12 months ( T 1 ) and 24 months ( T 2 ) after the surgery. For the survivorship, revision was defined as failure of the implant (periprosthetic joint infection, periprosthetic fracture, or aseptic loosening), and survival was based on implant revision or patient death. Undesirable clinical developments that were not present at baseline or that increased in severity after treatment were classified as adverse events. Results The mean age at the time of the surgery was 82.1 ± 1.9 years for UKA and 81.5 ± 1.8 years for TKA ( p = 0.06). The two groups differed in regard to surgical time (UKA 44.9 ± 7.2 min; TKA 54.4 ± 11.3 min; p 0.05). The UKA group reported 7 (9.3%) failures, while TKA reported 6 failures. There were no survival differences between the groups ( T 1 : p = 0.2; T 2 : p = 0.5). Overall complication rate was 6% in the UKA group versus 9.75% in TKA ( p = 0.2). Conclusion The UKA and TKA patients had similar clinical outcomes, post-operative range of motion, and survivorship in octogenarians with medial knee osteoarthritis, with comparable complication rate. Both the surgical procedures may be considered in this patient population, but further long-term follow-up is needed. Level of evidence Level III.

Perioperative complications following total kneel replacement in the elderly

Abstract:BACKGROUND: Critical organ hypofunction and complications are common in elderly patients, so perioperative treatment becomes important for the success of total knee replacement (TKR).OBJECTIVE: To explore clinical perioperative complications of TKR in the patients over 70 years old. DESIGN, TIME AND SETTING: Retrospective analysis of case data was performed at First Hospital of Gannan Medical University and People's Hospital of Peking University from January to December 2002.PARTICIPANTS: 109 patients (168 knees), including 29 males and 80 females, underwent TKR. Of them, 50 underwent single knee surgery, aged (74.2±15.1) years (range 70-85 years), and 59 underwent bilateral knee surgery, aged (73.4±13.2) years (range 70-85 years). In addition, 92 cases (84.4%) were obesity, and 88 were complicated by internal diseases. METHODS: The surgery was performed by the same operator. All patients underwent patellar replacement with Scorpio posterior stable knee prosthesis. Knee anterior median incision and medial patellar approach was applied, and anterior and posterior cruciate ligaments were excised during the surgery, osteophyma and corpus liberum of posterior articular capsule were cleared. Patellofemoral joint track was tested until meeting the requirements. The prosthesis was fixed using antibiotics mixed with bone cement, and the incision was sutured at flexed position.MAIN OUTCOME MEASURES: Early complications following replacement; knee joint and functional evaluation.RESULTS: During surgery and 24 hours after replacement, 8 cases developed hypertension, 7 cases hypotension, and 6 cases arrhythmia. All patients safely passed the perioperative period under treatment of related departments. One case developed pulmonary embolism, 1 case deep infection, 3 cases pulmonary infection, 5 cases urinary system infection, 1 case rapid reduction of platelet caused by Subining, 1 case cognitive disturbance, and 1 case dislocation of knee joint (Charcot arthritis). According to standards of HSS, the knee joint scores were significantly improved from 26.1 prior to replacement to 82.0 at discharge, and function scores were significantly improved from 32.1 prior to replacement to 89.1 at discharge. During 12.4-month follow-up (range 3-22 months), 18 cases lost the follow up; the retention rate was 83.5%. Of 91 retention patients, knee pain disappeared or relieved, restored self-care ability, and no prosthesis loosening or infection was found. At the final follow up, the HSS knee joint scores were significantly improved from 82.0 at discharge to 85.4, and function scores were improved from 89.1 at discharge to 92.3 at discharge.CONCLUSION: Skilled operative technique and positive treatment of complication can effectively prevent perioperative infection, dislocation and other complications following total knee replacement.
Complication Analysis of the Total Kneel Replacement in 70-Year-old and over Elderly

Abstract:Objective In order to study the clinical complications during peri-operation period of total kneel replacement (TKR) in patients with age of 70 years old and over and improve the effectiveness of the treatment Methods One hundred and nine patents with 168 knees treated by TKR from January to December 2002 were reviewed. Results All patients were safe during peri-operation period. The rate of early-stage postoperation complications was up to 31.2%. Hospital for special surgery (HSS) knee score and function score was risen respectively from 26.1 to 82.0 and form 32.1 to 89.1 pre-operation and postoperation. Conclusions Although the high risk and more complications of TKR in patients over 70 years old, the TKR operation is secure and satisfactory results can also be achieved if we do the operation skillfully and reduce the unnecessary injury by paying enough attention.

Abstract:Background: for kinematic alignment (KA) total knee arthroplasty (TKA), it was unknown whether 'the pace of recovery' at six weeks was different for patients with ages ranging between 50–59, 60–69, 70–79, and 80–89 years who were discharged on the surgery day and self-administered their rehabilitation. Methods: a single surgeon treated 206 consecutive patients with a KA-designed femoral component and an insert with a medial ball-in-socket, lateral flat articulation, and PCL retention. Each filled out preoperative and six-week Oxford Knee Score (OKS), Knee Society Score (KSS), Knee Function Score (KFS), and Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS, JR) questionnaires. The six-week minus preoperative value indicated improvement. Results: between age cohorts, the improvement was not different (p = 0.2319 to 0.9888). The mean improvement/six-week postoperative value was 6°/−2° for knee extension, 0°/119° for knee flexion, 7/31 for the OKS, 39/96 for the KSS, 7/64 for the KFS, and 13/62 for the KOOS. The 30-day hospital readmission rate was 1%. Conclusion: surgeons who perform KA TKA can counsel 50 to 89-year-old patients that they can be safely discharged home on the surgery day with a low risk of readmission and can achieve better function at six weeks than preoperatively when performing exercises without a physical therapist.

Abstract:Aims This study aims to determine the rate of and risk factors for total knee arthroplasty (TKA) after operative management of tibial plateau fractures (TPFs) in older adults. Methods This is a retrospective cohort study of 182 displaced TPFs in 180 patients aged ≥ 60 years, over a 12-year period with a minimum follow-up of one year. The mean age was 70.7 years (SD 7.7; 60 to 89), and 139/180 patients (77.2%) were female. Radiological assessment consisted of fracture classification; pre-existing knee osteoarthritis (OA); reduction quality; loss of reduction; and post-traumatic OA. Fracture depression was measured on CT, and the volume of defect estimated as half an oblate spheroid. Operative management, complications, reoperations, and mortality were recorded. Results Nearly half of the fractures were Schatzker II AO B3.1 fractures (n = 85; 47%). Radiological knee OA was present at fracture in 59/182 TPFs (32.6%). Primary management was fixation in 174 (95.6%) and acute TKA in eight (4.4%). A total of 13 patients underwent late TKA (7.5%), most often within two years. By five years, 21/182 12% (95% confidence interval (CI) 6.0 to 16.7) had required TKA. Larger volume defects of greater depth on CT (median 15.9 mm vs 9.4 mm; p 12.8 mm was associated with TKA requirement (area under the curve (AUC) 0.766; p = 0.001). Severe joint depression of > 15.5 mm (hazard ratio (HR) 6.15 (95% CI 2.60 to 14.55); p 15.5 mm were managed with fixation, 11/25 ultimately required TKA. Conclusion Overall, 12% of patients aged ≥ 60 years underwent TKA within five years of TPF. Severe joint depression and pre-existing knee arthritis were independent risk factors for both post-traumatic OA and TKA. Effect of Patient Age on Total Hip Arthroplasty Outcomes in Patients Who Have Osteonecrosis of the Femoral Head Compared to Patients Who Have Hip Osteoarthritis

Abstract:Background: Osteonecrosis of the femoral head (ONFH) affects both young and old patients. However, outcomes following total hip arthroplasty (THA) for these patients may vary with age. This study aimed to examine the effect of age on THA outcomes for non-traumatic ONFH patients, an area currently lacking research. Methods: Patients who had non-traumatic ONFH undergoing THA with at least 2 years of follow-up were identified using a database and divided into four groups by age. Then, 4 matched control groups of patients who had hip osteoarthritis (OA) were created. Multivariate logistic regression analyses were used to evaluate the rates of medical and surgical complications. Additionally, cohorts with a minimum 5-year follow-up were filtered to obtain further data on surgical outcomes. The study analyzed 85,462 non-traumatic ONFH and 80,120 hip OA patients undergoing THA. Results: Multiple medical complications in ONFH patients increased with age. Periprosthetic fracture within 2 years increased with age, while 90-day wound complications, 2-year periprosthetic joint infections, dislocations, and revisions decreased. The trends for complications continued at the 5-year follow-up. Compared to OA patients, those who had ONFH had higher risks of most complications, but this discrepancy decreased with age. Conclusions: In ONFH patients undergoing THA, medical complications generally rise with age, while most surgical complications, including revisions, decrease. It is notable that ONFH patients experienced more complications than those who had OA, but this difference diminished with age.

Abstract:PURPOSE: To identify the modes of failure after total knee arthroplasty (TKA) in patients >55 years of age and to compare with those >55 years of age in patients who underwent revision TKA.MATERIALS AND METHODS: We retrospectively reviewed 256 revision TKAs among patients who underwent TKA for knee osteoarthritis between January 1992 and December 2012. The causes of TKA failure were analyzed and compared between the groups.RESULTS: Thirty-one revision TKAs were performed in patients ≤55 years of age at the time of primary TKA, whereas 225 cases were in those >55 years of age at primary TKA. In the ≤55 years of age group, the most common cause of TKA failure was polyethylene wear (45%) followed by infection (26%) and loosening (17%). The interval from primary TKA to revision was 8.6 years (range, 1 to 17 years). There were relatively lower infection rate and higher loosening rate in patients ≤55 years of age, but the difference was not statistically significant.CONCLUSIONS: The main causes of failure after TKA in patients ≤55 years of age were polyethylene wear, infection and loosening, and there was no significant difference in the modes of failure after TKA between the two groups.

Abstract:Total knee arthroplasty (TKA) is a widely performed surgical intervention designed to relieve pain and restore function in patients with severe knee joint degeneration, most commonly due to osteoarthritis. This procedure involves replacing the damaged knee joint with a prosthetic implant, providing significant improvements in mobility and quality of life. While TKA is beneficial for many, the individuals who stand to gain the most are those with advanced knee arthritis unresponsive to conservative treatments, typically aged 60 and above, experiencing activity‐related pain and functional limitations. Identifying patients with the highest potential for positive outcomes is critical to maximizing the benefits of this transformative surgery. Goal of this editorial is to give a critical view on today's common practice of arthroplasty surgery.

Functional Outcomes and Satisfaction Rates in Patients Aged 80 Years or Older are Not Clinically Different From Their Younger (65 to 75 Years) Counterparts Following Total Hip Arthroplasty

Abstract:Introduction As the population ages, proportion of elderly patients requiring total hip arthroplasty (THA) increases, but it isn't clear whether older age independently influences outcome. The aim was to assess function, quality of life (QoL), and satisfaction after THA in patients ≥ 80 years compared to between 65 and 75 years when adjusting for confounding factors. Methods A single-center retrospective cohort study was performed between 2010 and 2019. A total 2,367 THAs were performed on patients ≥ 80 years and 5,113 on patients 65 to 75 years. Demographic data and length of stay (LOS) were recorded. Pre-operative and two-year post-operative Oxford Hip Scores (OHS), EuroQol (EQ-5D), and satisfaction scores were collected. Clinically meaningful difference was defined as 5-points in OHS and utility of 0.085 in EQ-5D. Regression analyses were performed to adjust for confounding factors. Results Patients in ≥ 80 years group were more likely women (P < 0.001), have higher American Society of Anesthesiology (ASA) grade (P < 0.001), worse preoperative OHS (mean difference (MD) 2.3, P < 0.001), and EQ-5D (MD 0.087, P < 0.001). Both age groups achieved clinically meaningful and statistically significant (P < 0.001) improvement in OHS and EQ-5D utility at 2 years. When adjusting for confounding variables, the ≥ 80 year old group had significantly (P < 0.001) lower improvement in OHS (MD -1.9 points) and EQ-5D (MD -0.055 utility), which were not clinically meaningful. There was no difference (P = 0.813) in satisfaction between groups. When adjusting for confounding variables, ≥ 80 year-old group had increased risk of longer LOS (odds ratio 1.27, P < 0.001). Implications There was no clinically meaningful differences in hip-specific outcome or health related quality of life according to age group, and both were equally satisfied with their outcome. The older age group did, however, have longer LOS.

Abstract:Background: Understanding the short-term complication profile following unicompartmental knee arthroplasty (UKA) versus total knee arthroplasty (TKA) can improve surgical decision-making and patient outcomes. This study aimed to determine if the difference in risk of 30-day morbidity and mortality between UKA and TKA varied based on patient age. Methods: This retrospective study of a national quality improvement database using data from 2014 to 2020 included 403,342 patients undergoing UKA (n = 12,324) or TKA (n = 391,018). A generalized additive model evaluated nonlinear relationships between primary outcome and predictors (age, procedure, and procedure × age interaction) using a 1:5 UKA to TKA matched sample. Probabilities and odds ratios (95% confidence interval [CI]) estimated the relative risk of complications by age. Results: In the generalized additive model, TKA patients relative to UKA had 1.30 odds (95% CI 1.19 to 1.43, P < 0.001) of 30-day morbidity and mortality. There was a significant nonlinear relationship between age and primary outcome (P = 0.02), such that the odds were lowest at younger ages. They increased slowly until the age of 65 years, when the slope became steeper. The interaction terms for age and procedure were not significant (P = 0.30). The 30-day probability for short-term complications of a 65-, 75-, and 85-year-olds undergoing UKA was 2.1% (95% CI 1.8 to 2.3), 2.4% (95% CI 2.0 to 2.8), and 3.2% (95% CI 2.3 to 4.1), respectively. The probability of a 65-, 75-, and 85-year-old undergoing TKA was 2.9% (95% CI 2.7 to 3.0), 3.6% (95% CI 3.3 to 3.8), and 5.5% (95% CI 4.7 to 6.3), respectively. Conclusions: Patients undergoing UKA had a quantifiable lower likelihood of morbidity or mortality than TKA at all ages. These data can provide individualized risk for UKA and TKA across the age spectrum and could be helpful in counseling patients regarding their perioperative risk. Unicompartmental knee arthroplasty in octogenarians: An analysis of 1,466 patients with 2-year follow-up

Abstract:Introduction: Unicompartmental knee arthroplasty (UKA) has been shown to improve pain and function in appropriately selected patients. Limited data exists regarding outcomes and complication rates following UKA among octogenarians. Methods: The PearlDiver Mariner database was queried for patients undergoing primary UKA between 2010-2022. Patients < 80 years old were matched 4:1 to the octogenarian cohort (≥80 years old) by sex, year, Elixhauser Comorbidity Index (ECI), tobacco use, obesity, and diabetes. A total of 1,334 octogenarians and 5,313 controls were included in our analysis. Multivariate logistic regression was utilized to compare medical complications at 90-days post-operatively and surgical complications at 1- and 2-years post-operatively. Our regression analysis controlled for sex, ECI, tobacco use, obesity, and diabetes. Results: Octogenarians had an increased risk of acute kidney injury (OR: 2.306, 95% CI: 1.393-3.749; p < 0.001), pneumonia (OR: 2.367, 95% CI: 1.301-4.189; p = 0.003), UTI (OR: 1.846, 95% CI: 1.304-2.583; p < 0.001), ED visits (OR: 2.229, 95% CI: 1.586-3.105; p < 0.001), and any complication (OR: 1.575, 95% CI: 1.304-1.895; p < 0.001) at 90-days post-operatively. Octogenarians had lower odds of all-cause revision at 2-years (OR: 0.607, 95% CI: 0.382-0.923; p = 0.026). No differences were demonstrated between cohorts in rates of PJI (OR: 0.832, 95% CI: 0.334-1.796; p = 0.664), periprosthetic fracture (OR: 0.516, 95% CI: 0.120-1.520; p = 0.289), or aseptic loosening (OR: 0.285, 95% CI: 0.045-1.203; p = 0.088) at 2-years. Discussion: These findings suggest that despite an increased risk of certain medical complications within the acute post-operative period, octogenarians undergoing UKA experienced similar rates of surgical complications to younger matched controls at 2-year follow-up.
Total Knee Arthroplasty in Patients Who Have Ankylosed Knees: A Ten-Year Retrospective

Abstract:Introduction Knee ankylosis can result in major functional impairment, and surgical treatment of knee ankylosis is often regarded as a tremendous challenge due to technical difficulties such as difficult joint exposure and a high incidence of complications. The objective of this study was to review the results of preoperative and postoperative functional scores, range of motion (ROM), and complications of total knee arthroplasty (TKA) for the treatment of knee ankylosis. Surgical Methods Between January 2007 and January 2021, 19 patients (17 patients, 11 women, and 6 men) who had knee ankylosis underwent TKA. The mean age of the patients was 52 years (range, 31 to 71), and the mean follow-up period was 10.2 years (range, 3.1 to 13.9). The surgical procedure involved a total knee arthroplasty, performed via a medial parapatellar approach, a quadriceps snip, and a secondary osteotomy with soft-tissue release. The postoperative clinical outcomes and complications were evaluated using a range of methods, including ROM assessment, Hospital for Special Surgery Knee Score (HSS), and visual analogue scale (VAS) scores. Result At final follow-up, the mean HSS score improved significantly from the preoperative score of (33.63 ± 8.69) to (88.05 ± 5.23) ( P < 0.001), ROM improved from (0 ± 0°) to (100.90±14.07°) ( P < 0.001), the VAS score improved from a preoperative score of 0 to (1.00 ± 0.94) ( P < 0.001), and radiographs showed no aseptic loosening of the knee. There were eight knees (42.1%) that had postoperative complications. Conclusion Total knee arthroplasty in patients who have ankylosing knees resulted in significant improvements in flexion and extension, mobility, and quality of life, despite a high incidence of complications such as skin necrosis.

Abstract:Background: Total hip arthroplasty (THA) is increasingly performed in younger patients despite the lack of comprehensive assessment of long-term outcomes. We systematically reviewed the contemporary literature to assess the 1) indications, 2) implant selection and long-term survivorship, 3) complication and reoperation rates and 4) radiographic and functional outcomes of primary THA in patients younger than 55 years.Methods: We searched the Embase and MEDLINE databases for English-language articles published between 2000 and 2018 that reported outcomes of primary THA in patients younger than 55 years with a minimum follow-up duration of 10 years.Results: Thirty-two studies reporting on 3219 THA procedures performed in 2434 patients met our inclusion criteria. The most common preoperative diagnoses were avascular necrosis (1044 [32.4%]), osteoarthritis (870 [27.0%]) and developmental dysplasia of the hip (627 [19.5%]). Modular implants (3001 [93.2%]), cementless fixation (2214 [68.8%]) and metal-on-polyethylene bearings (1792 [55.7%]) were frequently used. The mean 5- and 10-year survival rates were 98.7% and 94.6%, respectively. Data on survival beyond 10 years were heterogeneous, with values of 27%–99.5% at 10–14 years, 59%–84% at 15–19 years, 70%–77% at 20–24 years and 60% at 25–30 years. Rates of dislocation, deep infection and reoperation for any reason were 2.4%, 1.2% and 16.3%, respectively. The mean Harris Hip Score improved from 43.6/100 to 91.0/100.Conclusion: Total hip arthroplasty in patients younger than 55 years provides reliable outcomes at up to 10 years. Future studies should evaluate the outcomes of THA in this population at 15–20 years’ follow-up.
