Abstract:Objective To explore the value of MMP-9 and IMA for diagnosis of coronary heart disease and its clinical significance.Methods Totally 50 cases in physical examination(group A),116 patients with coronary heart disease were randomly selected.Then divided patients with coronary heart disease into 40 subjects with SAP(group B),46 subjects with UAP(group C) and 30 subjects with AMI(group D).The concentrations of serum MMP-9 were detected with enzyme-linked immunosorbent assay(ELISA).And concentrations of ischemia-modified albumim(IMA) were detected with albumin cobalt binding(ACB) test.Four groups were separately compared the levels of MMP-9,IMA and to analysis the correlativity between the two.Results The levels of serum MMP-9 and IMA in group D were obviously higher than those of group A and B(P0.01).The levels of serum MMP-9 and IMA in group D were significantly higher than those of group C(P0.05).The levels of serum MMP-9 and IMA in group C were significantly higher than those of group A and B(P0.05).The leves of serum MMP-9 and IMA in group B were higher than those of group A,but it was no significant(P0.05).MMP-9 leves correlated positively with IMA leves in ACS patients(P0.05).Conclusions MMP-9 and IMA were early sensitive maker for ACS.The joint measurement of MMP-9 and IMA may be the important maker to forecast the dangerous grade of CHD.
Levels Of Pregnancy-Associated Plasma Protein A In Patients With Coronary Artery Disease

Abstract:Purpose: To investigate the levels of pregnancy-associated plasma protein A (PAPP-A) or insulin-like growth factor-1 (IGF-1) in patients with acute coronary syndrome.Methods: Serum PAPP-A and IGF-1 was measured with biotin-tyramide-amplified enzyme immunoassay and Enzyme Linked Immuoserbent Assay, respectively, in patients with ST elevation acute myocardial infarction (STEMI, n=12), unstable angina (UAP, n=15), and stable angina (n=15). PAPP- A and IGF-1 was also measured in 16 healthy subjects ( control group).Results: The serum levels of PAPP-A in the STEMI (16.9 +/- 10.3 mIU/L) and UAP group (15.2 +/- 10.5 mIU/L) were higher than in the stable angina (8.5 +/- 3.1 mIU/L) or control group (8.4 +/- 2.0 mIU/L, P<0.01). The serum levels of IGF-1 in the STEMI (132.3 +/- 40.9 mu g/L) and UAP group (127.3 +/- 36.0 mu g/L) were also higher than in the stable angina (44.9 +/- 18.5 mu g/L) or control group (67.7 +/- 24.5 mu g/L, P<0.01). There were no differences in serum levels of PAPP-A or IGF- 1 among the single, double and three vessel lesion groups. The serum levels of PAPP- A (19.9 +/- 10.1 mIU/L) and IGF- 1 (153.2 +/- 52.4 mu g/L) after PCI were higher than those before PCI (15.1 +/- 10.0 mIU/ L and 91.4 +/- 51.0 mu g/L, respectively, P< 0.01). A positive correlation was found between PAPP- A and IGF- 1 levels in the STEMI and UAP group before PCI (r=0.48, P<0.01).Conclusion: PAPP-A and IGF-1 are elevated in patients with acute coronary syndrome. They may be used as bio-markers for vulnerable plaques in patients with coronary artery disease. Whether post-PCI elevation of IGF-1 can be used to predict restenosis of coronary arteries remains to be seen.

Abstract:Background Recent studies have shown Growth differentiation factor–15 (GDF-15) that is a member of the transforming growth factor β (TGF-β) superfamily might be a potential predictive cytokine for the prognosis of Acute coronary syndrome (ACS). However, there are discrepancies in these studies. Methods Publication searches of the PubMed/Medline and EMBASE databases were performed without any time or ethnicity restrictions. The inclusion and exclusion criteria, when clear, were addressed. Random effects models were used for all analyses. Publication bias was tested using funnel plots and the Egger test. Results We identified eight eligible studies that provided mortality data. Five of these studies provided recurrent myocardial infarction (MI) data. The maximal duration of follow-up ranged from 6 months to 6 years. A significant association was found between the patients with the highest and lowest GDF-15 levels (overall analyses) in terms of mortality ( p < 0.00001; RR = 6.08; 95 % CI = 4.79–7.71) and recurrent MI ( p < 0.00001; RR = 1.76; 95 % CI = 1.49–2.07). We also found significant associations between the subgroup analyses stratified by ACS types, cutoff points and follow-up durations ( p < 0.001). The combined hazard ratio was high for GDF-15 to ACS (HR = 1.656, 95 % CI = 1.467–1.871). Conclusion High plasma GDF-15 levels are associated with an increased risk of mortality and recurrent MI in patients with ACS.

Abstract:OBJECTIVE:To explore the association between the levels of serum growth differentiation factor-15(GDF-15) and acute coronary syndrome(ACS) in patients undergoing coronary angiography(CAG).METHODS:In this prospective study, 120 candidates (30 healthy controls and 90 patients with ACS) admitted to our hospital from March 2006 to April 2008 were included. The patients with ACS were divided into 3 groups: (1)Group AMI-emergency (AMI-E), including 30 patients with acute myocardial infarction (AMI) who underwent CAG within 12 hours of the onset of symptoms. (2) Group AMI 7-14 d, including 30 patients with AMI who underwent CAG within 7-14 days of the onset of symptoms. (3)Group UAP including 30 patients with unstable angina pectoris. The serum level of GDF-15 was determined by ELISA.RESULTS:The level of GDF-15 for group ACS was (649.0 ± 224.21) ng/L, which was significantly higher than that of control group [(537.2 ± 262.9)ng/L,P<0.01]. The level of serum GDF-15 significantly increased in group AMI-E [(801.6 ± 218.4)ng/L] compared with group AMI 7-14 d [(656.2 ± 252.4)ng/L,P=0.042, 95% CI 1.001-1.061], group UAP [(586.9 ± 225.6)ng/L, P=0.001, 95% CI 0.809-0.950] and control group(P<0.05, 95% CI 1.000-1.012) after being adjusted for relevant covariates. The elevated level of GDF-15 was related to biomarkers, such as white blood cells count (WBC, r(2)=0.276, P=0.002), lactate dehyerogenase(LDH, r(2)=0.288, P=0.004), creatine kinase 2(CK-MB, r(2)=0.350, P<0.001) and troponin T(TnT, r(2) =0.344, P=0.001).CONCLUSION:GDF-15 protein expression increases in ACS, and the level of GDF-15 rapidly increases and remains elevated in the early period of acute myocardial infarction. The elevated level of GDF-15 is related to biomarkers, such as WBC, LDH, CK-MB, and TnT. GDF-15 could be a new and independent biomarker in evaluating the patients with cardiovascular disease.

Abstract:Background: The predictive value of growth differentiation factor-15 (GDF-15) in coronary microvascular dysfunction (CMD) following primary percutaneous coronary intervention (PPCI) in ST-segment elevation myocardial infarction (STEMI) patients is unclear. Methods: This study continuously recruited STEMI patients treated with PPCI at the Chest Pain Center of Qilu Hospital of Shandong University from April 2023 to December 2023. Blood samples were taken before PPCI and the level of circulating GDF-15 was measured by enzyme-linked immunosorbent assay (ELISA), and the patients were divided into CMD and Control group according to angiographic microvascular resistance (AMR) (cut-off value 2.50 mmHg*s/cm). The differences in GDF-15 expression levels between the two groups were compared, and the predictive value of GDF-15 for CMD was systematically evaluated. Results: A total of 134 patients, with an average age of 59.78 ± 12.69 years and 75.37 % being male, were included in this study. Multivariable logistic regression revealed a significant association between GDF-15 and CMD (adjusted OR = 2.505, 95 % CI: 1.661-3.779, P < 0.001). The area under the curve (AUC) of GDF-15 for CMD was 0.782 (95 % CI: 0.704-0.861), with a sensitivity of 0.795 and specificity of 0.643 in predicting CMD in PPCI. The AUC of the GDF-15 model (Model With GDF-15) was 0.867 (95 % CI: 0.806-0.928), significantly outperforming the clinical baseline model (Model Without GDF-15) (Δ AUC = 0.079, 95 % CI: 0.020-0.138, P = 0.009). Furthermore, the net reclassification improvement (NRI) was 0.854 (95 % CI: 0.543-1.166, P < 0.001), and the integrated discrimination improvement (IDI) was 0.151 (95 % CI: 0.089-0.213, P < 0.001). Conclusions: GDF-15 can serve as a biomarker for predicting the development of CMD in STEMI patients undergoing PPCI.

Abstract:Background and aims: The predictive value of growth differentiation factor-15 (GDF-15) for individual cardiovascular outcomes remained controversial in patients with coronary artery disease (CAD). We aimed to investigate the effects of GDF-15 on all-cause death, cardiovascular death, MI and stroke in CAD patients. Methods: We searched PubMed, EMBASE, Cochrane library and Web of Science till 30 December, 2020. Hazard ratios (HRs) were combined with fixed or random effect meta-analyses. Subgroup analyses were performed in different disease types. Sensitivity analyses were used to evaluate the stability of the results. Publication bias was tested using funnel plots. Results: A total of 10 studies with 49,443 patients were included in this meta-analysis. Patients with the highest GDF-15 concentrations had significantly increased risk of all-cause death (HR 2.24; 95% CI: 1.95–2.57), cardiovascular death (HR 2.00; 95% CI: 1.66–2.42), MI (HR 1.42; 95% CI: 1.21–1.66) after adjusting clinical characteristics and prognostic biomarkers (hs-TnT, cystatin C, hs-CRP, and NT-proBNP) but except for stroke (HR 1.43; 95% CI: 1.01–2.03, p = 0.05). For the outcome of all-cause death and cardiovascular death, subgroup analyses revealed consistent results. Sensitivity analyses showed that the results were stable. Funnel plots showed that there was no publication bias. Conclusion: In CAD patients with elevated GDF-15 levels on admission, there were independently significant risks for all-cause death and cardiovascular death. The highest concentrations of GDF-15 had a lower predictive effect on MI than all-cause death and cardiovascular death. The association of GDF-15 with the outcome of stroke needs to be further studied.

Abstract:Pregnancy-associated plasma protein-A (PAPP-A) level is an independent predictor of acute cardiovascular event occurrence. To test the hypothesis that increased PAPP-A levels would be associated with a higher burden of coronary thin-cap fibroatheroma (TCFA) thereby underlying the heightened risk for cardiovascular events in patients with coronary artery disease; 154 patients (462 vessels and 975 plaques) with stable angina or non-ST-segment elevation acute coronary syndrome (NSTE-ACS) referred for percutaneous coronary intervention were assessed using 3-vessel virtual histology (VH)-intravascular ultrasound (IVUS). Thin-cap fibroatheroma virtual histology was defined as focal, necrotic core (NC)-rich (>= 10% of cross-sectional area) plaques in contact with the lumen, and plaque burden >= 40%. Pregnancy-associated plasma protein-A levels were determined by sandwich enzyme-linked immunosorbent assay, and patients were divided into 3 groups based on PAPP-A level tertiles. Although the highest PAPP-A level tertile was not associated with 3-vessel plaque number, it was associated with 3-vessel VH-TCFA number and necrotic core volume. Patients with >= 3 VH-TCFAs had a higher PAPP-A level than patients with 1 to 3 VH-TCFAs or without any VH-TCFA (13.3 +/- 11.8 versus 7.8 +/- 4.7 versus 7.4 +/- 4.7 mIU/L, P < 0.001, respectively). Moreover, PAPP-A level was an independent predictor of higher total number of VH-TCFAs (OR 1.18; 95% CI 1.07-1.29, P = 0.001). This VH-IVUS study demonstrated, for the first time to our knowledge, that higher PAPP-A levels are associated with higher 3-vessel TCFA burden in patients with coronary artery disease. Pregnancy-associated plasma protein-A, therefore, might be a useful serum biomarker to predict increased coronary TCFA burden and plaque instability.

Abstract:Lipoprotein-associated phospholipase A2 (Lp-PLA2) is a newly emerging biomarker with strong pro-inflammatory effects, and is an independent risk predictor of atherosclerotic plaque rupture and thrombosis. In addition, ischemic modified albumin (IMA) is another important marker for the evaluation of myocardial ischemia, and has been approved by the U.S. Food and Drug Administration. The objective of this study was to investigate serum Lp-PLA2 and IMA in the early diagnosis, progression and prognosis of acute coronary syndrome (ACS). Serum Lp-PLA2 and IMA were detected using an AU5800 automatic biochemical analyzer in samples from 180 patients with ACS [n = 60 with unstable angina pectoris (UA), n = 56 with non-ST segment elevation myocardial infarction (NSTEMI), and n = 64 with ST segment elevation myocardial infarction (STEMI)] and 60 healthy control subjects. The relationship between Lp-PLA2 and IMA with Gensini score and the number of coronary artery lesions was explored, and logistic regression was conducted to identify risk factors for major adverse cardiovascular events (MACE). Serum Lp-PLA2 and IMA were significantly higher in all ACS subgroups compared to the control group (P < 0.05), were positively associated with the severity of ACS based on the Gensini score (P < 0.05), and were significantly higher in patients with double- and triple-vessel lesions compared to those with single-vessel lesions and healthy controls (P < 0.05). Logistic regression identified Lp-PLA2, IMA, and troponin I levels as independent risk factors for MACE. Lp-PLA2 and IMA were predictive of the degree of myocardial ischemia in patients with ACS, and may provide important clinical significance for the early diagnosis of ACS and the choice of treatment strategy.

Abstract:Growth differentiation factor-15 (GDF-15) has recently emerged as a risk predictor in patients with cardiovascular diseases. We therefore aimed to investigate the role of GDF-15 in the occurrence of cardiac injury during off-pump coronary artery bypass grafting (OPCAB). 55 consecutive patients with coronary artery diseases were recruited in this prospective, observational study. All patients were operated for OPCAB surgery. Serial blood samples were collected preoperatively, 12 hours and 36 hours after surgery. GDF-15, together with C-reactive protein, cardiac troponin I, creatine kinase MB and N-terminal pro B-type natriuretic peptide levels in plasma were measured at each time-point. GDF-15 levels increased significantly at 12 hours after surgery, attaining nearly 2.5 times the baseline levels (p < 0.001). Postoperative GDF-15 levels correlated positively with cTnI (p = 0.003) and EuroSCORE II (p = 0.013). According to the ROC curves, postoperative plasma GDF-15 was found to be the best biomarker to predict perioperative cardiac injury, compared with cTnI, CK-MB and EuroSCORE II. Circulating GDF-15 is a promising novel biomarker for identifying perioperative myocardial injury in patients undergoing OPCAB.
Ischemia-modified albumin and C reactive protein in diagnosis of acute coronary syndromes

Abstract:Objective To investigate the values of ischemia-modified albumin(IMA) and C reactive protein (CRP) in early diagnosis of acute coronary syndromes(ACS). Methods Levels of IMA and CRP were detected in 113 patients with ACS or stable angina pectoris (SA) right after admission as were in 76 healthy controls. Receiver operator characteristic curve (ROC curve) analysis was used to determine the optimal cutoff of identifying ACS from SA. Results There were statistical differences in IMA and CRP levels between the patients with ACS and those with SA or the controls. The area beneath the ROC curve of IMA was 0.948. Sensitivity and specificity of IMA at a cutoff value of 70.5 U/mL were 94.4% and 82.6%. Positive predictive value of IMA for ischemia origin was 95.5% and negative was 79.2%. The area beneath the curves of CRP was 0.746. Sensitivity and specificity of CRP at a cutoff value of 4.905 mg/L were 55.6% and 87.0%. Positive predictive value of CRP for ACS was 78.1% and negative was 18.4%. Conclusions Early diagnosis of acute myocardial ischemia can be made by detection of IMA. IMA can significantly enhance the diagnostic sensitivity of ACS.
Growth differentiation factor-15 combined with N-terminal prohormone of brain natriuretic peptide increase 1-year prognosis prediction value for patients with acute heart failure: a prospective cohort study.

Abstract:BACKGROUND:Clinical assessment and treatment guidance for heart failure depends on a variety of biomarkers. The objective of this study was to investigate the prognostic predictive value of growth differentiation factor-15 (GDF-15) and N-terminal prohormone of brain natriuretic peptide (NT-proBNP) in assessing hospitalized patients with acute heart failure (AHF). METHODS:In total, 260 patients who were admitted for AHF in the First Affiliated Hospital of Nanjing Medical University were enrolled from April 2012 to May 2016. Medical history and blood samples were collected within 24 h after the admission. The primary endpoint was the all-cause mortality within 1 year. The patients were divided into survival group and death group based on the endpoint. With established mortality risk factors and serum GDF-15 level, receiver-operator characteristic (ROC) analyses were performed. Cox regression analyses were used to further analyze the combination values of NT-proBNP and GDF-15. RESULTS:Baseline GDF-15 and NT-proBNP were significantly higher amongst deceased than those in survivors (P < 0.001). In ROC analyses, area under curve (AUC) for GDF-15 to predict 1-year mortality was 0.707 (95% confidence interval [CI]: 0.648-0.762, P < 0.001), and for NT-proBNP was 0.682 (95% CI: 0.622-0.738, P < 0.001). No statistically significant difference was found between the two markers (P = 0.650). Based on the optimal cut-offs (GDF-15: 4526.0 ng/L; NT-proBNP: 1978.0 ng/L), the combination of GDF-15 and NT-proBNP increased AUC for 1-year mortality prediction (AUC = 0.743, 95% CI: 0.685-0.795, P < 0.001). CONCLUSIONS:GDF-15, as a prognostic marker in patients with AHF, is not inferior to NT-proBNP. Combining the two markers could provide an early recognition of high-risk patients and improve the prediction values of AHF long-term prognosis. Susceptibility to killing by bcg‐activated macrophages associated with ‘spontaneous’ neoplastic transformation in culture

Abstract:Non‐tumorigenic and ‘spontaneously’ transformed tumorigenic paired cell lines derived from mouse, rat, or hamster embryonic tissues were examined for their susceptibility to killing by mouse peritoneal macrophages activated in vivo by infection with Mycobacterium bovis, strain Bacillus Calmette‐Guérin (BCG). Susceptibility to killing was measured by the extent of clearing in a confluent layer of target cells overlying an area of adherent macrophages or by the release of 3H counts from target cells prelabelled with [3H]‐thymidine. Only tumorigenic lines showed complete clearing by activated macrophages; the reaction occurred regardless of species differences between macrophages and target cells. The degree of clearing increased with increasing ratio of macrophages to target cells. Normal macrophages from uninfected mice did not produce a clearing reaction. The macrophage reaction discriminated between non‐tumorigenic and tumorigenic cells with short tumor latent periods (<100 days) even when neoplastic and non‐neoplastic cells were derived in culture from the same cell. The more tumorigenic lines (i.e., shortest latent period) tended to be more susceptible to killing. Thus, cells spontaneously transformed in culture are more susceptible to killing by activated macrophages than are their non‐neoplastic counterparts. In two cloned lines the enhanced susceptibility to macrophage killing appeared to be a late manifestation of the neoplastic change.

Abstract:Objectives Ischemia-modified albumin (IMA), a new serum biomarker of myocardial ischemia, has been introduced in the diagnosis of acute coronary syndrome. However, few studies have been carried out in patients with stable coronary atherosclerotic heart disease (CHD). We aimed to evaluate the value of IMA in the diagnosis of stable CHD and to determine the correlation between IMA levels and the severity of myocardial ischemia. Methods A total of 129 patients with diagnosed CHD who had signed an informed consent form were enrolled. Patients were willing to undergo coronary angiography, which was the gold standard for the diagnosis of CHD. IMA was detected by the albumin cobalt binding test before coronary angiography. Results The mean IMA level of the non-CHD group was 67.68+/-29.61 U/ml and that of the CHD group was 90.24+/-29.61 U/ml. In the CHD group, the mean IMA levels of the one-branch lesion group, the two-branch lesion group, and the three-branch lesion group were 79.87+/-18.37, 98.10+/-23.39, and 98.06+/-33.47 U/ml, respectively. IMA levels and the number of diseased coronary arteries were significantly correlated (P<0.01, r=0.299). When the cut-off threshold derived from the receiver operating characteristics curve was determined to be 93.07 U/ml, the sensitivity of admission IMA for a final diagnosis of CHD was 0.49, the specificity was 0.90, the negative predictive value was 0.45, and the positive predictive value was 0.91. Logistic regression analysis showed that IMA (P=0.000, b=2.056, odds ratio=7.815) was an independent predictor of CHD. Conclusion IMA showed a high specificity and predictive value in the diagnosis of CHD. In addition, IMA was a strong and independent predictor of CHD. Diagnostic Value of Circulating Progranulin and Its Receptor EphA2 in Predicting the Atheroma Burden in Patients with Coronary Artery Disease.

Abstract:Background. Progranulin (PGRN) and its potential receptor Eph-receptor tyrosine kinase-type A2 (EphA2) are inflammation-related molecules that present on the atherosclerotic plaques. However, the roles of circulating PGRN and EphA2 in coronary artery disease (CAD) remain unclear. Objective. To study the clinical significance of circulating PGRN and EphA2 levels in Chinese patients undergoing coronary angiography. Methods. Levels of circulating EphA2 fragments and PGRN were examined in 201 consecutive individuals who underwent coronary angiography for suspected CAD in our center from Jan 2020 to Oct 2020. Demographic characteristics, results of biochemical and auxiliary examinations, and other relevant information were collected. The coronary atheroma burden was quantified by the Gensini score and the existence of chronic total occlusion (CTO). Univariate analysis and multivariate logistic regression analysis were used to analyze the risk factors for acute coronary syndrome (ACS). In patients with ACS and SAP, a receiver operating characteristic (ROC) curve was generated to detect the accuracy and discriminative ability of levels of EphA2 and PGRN, the Gensini score, and cardiac injury biomarkers as surrogate endpoints for CTO. Results. Circulating EphA2 levels were significantly higher in patients with ACS than in subjects with stable angina pectoris (SAP) or control subjects ( p < 0.001 ). A positive linear correlation was verified between EphA2 levels and the Gensini score ( r = 0.306 , p < 0.001 ), and negative correlation was detected with the left ventricular ejection fraction (LVEF) ( r = − 0.405 , p < 0.001 ). Both PGRN and EphA2 were positively associated with cardiac injury biomarkers (i.e., NT-proBNP, cTnT, and hs-CRP) ( p < 0.05 ). The area under the ROC curve of PGRN and EphA2 was 0.604 and 0.686, respectively ( p < 0.01 ). Conclusions. Higher circulating EphA2 and PGRN levels were detected in patients with ACS than in patients with SAP. Circulating EphA2 and PGRN levels might be diagnostic factors for predicting the atheroma burden in patients with CAD.

Abstract:Background: Premature coronary artery disease (PCAD) is prevailing. We aimed to investigate the evaluation value of atherogenic index of plasma (AIP) and high-sensitivity C-reactive protein (hs-CRP) for the occurrence and severity of coronary artery lesion in PCAD patients. Methods: PCAD (PACD group)/non-PCAD (control group) patients were enrolled. The coronary artery lesion degree was evaluated using Gensini score (GS). PCAD patients were allocated into the low/medium/high GS groups, with general clinical baseline data analyzed. Plasma hs-CRP/AIP levels were compared in PCAD patients with different disease degree. Correlations between plasma hs-CRP/AIP with Gensini score, independent risk factors affecting the occurrence of PCAD, and the predictive value of hs-CRP/AIP/their combination for the occurrence and degree of PCAD were evaluated by Spearman correlation analysis/Logistic multivariate regression/receiver operating characteristic (ROC) curve. The differences in the area under the curve (AUC) were compared using MedCalc-Comparison of ROC curves. Results: Plasma hs-CRP/AIP levels in the PCAD group were increased. Plasma hs-CRP/AIP levels varied significantly among PCAD patients with different disease degree. Plasma hs-CRP/AIP levels were markedly positively correlated with the Gensini score. Smoking history/homocysteine/fasting blood-glucose/hs-CRP/AIP were all independent risk factors affecting PCAD occurrence. The AUC of hs-CRP and AIP combination predicting the occurrence of PCAD was 0.950 (90.80% sensitivity/93.33% specificity). hs-CRP/AIP combination assisted in predicting the disease degree in PCAD patients. Conclusions: AIP and hs-CRP are independent risk factors for the occurrence of PCAD, and their combination has high predictive value for PCAD occurrence and disease degree, which are both positively correlated with coronary artery lesion degree.

Abstract:Objective To investigate the potential value of growth differentiation factor-15 (GDF-15) in predicting heart failure after coronary intervention for ST-elevation myocardial infarction(STEMI).Methods From June 2008 to January 2009,59 STEMI patients and 30 healthy volunteers were enrolled in Peking University Third Hospital.STEMI patients had emergency percutaneous coronary intervention within 6 h after myocardial infarction onset.Serum biomarkers [GDF-15 and N-terminal pro-brain natriuretic peptide (NT-proBNP)] were measured immediately when STEMI was diagnosed and 24 h later.Killip heart function classification was evaluated and echocardiography was performed to measure pulmonary artery systolic pressure (PASP) 24 h after admission.GDF-15 level was analyzed between STEMI patients and healthy people;correlations of GDF-15 with NT-proBNP,Killip classification and PASP were analyzed.The sensitivity and specificity of serum GDF-15 level in predicting Killip heart function 24 h after STEMI onset were analyzed.Results Serum level of GDF-15 in STEMI patients was significantly higher than that in healthy volunteers [(1 112 ± 543) ng/L vs (309 ± 239) ng/L] (P ＜ 0.01).Serum levels of GDF-15 in Killip Ⅲ,Ⅳ patients were significantly higher than those in Killip Ⅰ,Ⅱ patients [baseline:(1 640 ±483),(1 899 ±214) ng/L vs (781 ±285),(902 ±276) ng/L;24 h after STEMI onset:(1 514 ±599),(2 081 ± 381) ng/L vs (861 ±221),(917 ±243) ng/L] (P ＜0.05).Serum level of NT-proBNP in Killip Ⅳ patients 24 h after STEMI onset was significantly higher than that in Killip Ⅰ,Ⅱ,Ⅲ patients(P ＜0.05).GDF-15 levels(baseline and the level 24 h after onset) were positively correlated with PASP(P ＜ 0.01).Baseline NT-proBNP level had no correlation with PASP (P ＞ 0.05);NT-proBNP level 24 h after onset had correlation with PASP(r =0.335,P ＜ 0.015).In receiver operating characteristic curve analysis,when using 716.6 ng/L as the boundary value of GDF-15,sensitivity and specificity in predicting Killip classification ≥ Ⅱ24 h after STEMI onset were 90.5％ and 56.3％;with 716.6 ng/L as the boundary value,sensitivity and specificity of GDF-15 in predicting Killip classification ≥ Ⅲ were 83.3％ and 72.5％.Conclusion Elevated GDF-15 level may be an earlier predictor of heart failure after coronary intervention for STEMI;the predictive value of GDF-15 is hetter than that of NT-proBNP.

Abstract:Diagnosis of cardiac ischemia in patients coming to emergency departments (ED) with symptoms of acute chest pain is often difficult. Many markers are sensitive and specific for the detection of myocardial necrosis but may not rise during reversible myocardial ischemia. Ischemia-modified albumin (IMA) has recently been shown to be a sensitive and early biochemical marker of ischemia. The variation laws were observed by measuring IMA and C-reactive protein (CRP) of 113 patients in ED within 12 hr after onset of chest pain. In the observation, blood was taken for IMA and CRP. Patients underwent standardized triage, diagnostic procedures, and treatment. Results of IMA and CRP were correlated with final diagnoses of nonischemic chest pain (NICP) and acute coronary syndrome (ACS). There were obvious distinction of IMA and CRP levels between the NICP and ACS groups. Receiver operator characteristic (ROC) curve analysis was used to determine the optimal cutoff of this assay for identifying individuals with ACS patients from NICP. The area under the curves of IMA is 0.948. The sensitivity and specificity of albumin cobalt binding (ACB) at a cutoff value of 70.Ounits/mL were 94.4% and 82.6%, respectively. The area under the curves of CRP is 0.746. Sensitivity and specificity of CRP at a cutoff value of 3.16 mg/L were 70.0% and 73.9%, respectively. Negative predictive value (NPV) of IMA and CRP for ischemia origin was 79.2% and 38.6%, respectively. IMA may make an early diagnosis of acute coronary ischemia, and will improve the early diagnostic sensitivity and specificity of ACS.

IMA level and the number of diseased vessels of coronary were significantly positively correlated (p<0.01, r=0.299). When the cut-off threshold derived from the receiver operating characteristics (ROC) curve (ROC analysis) was determined as 93.07 U/ml, the sensitivity of admission IMA for a final diagnosis of CAD with myocardial ischaemia was 0.49, specificity was 0.90, negative predictive value was 0.45, positive predictive value was 91%. Conclusion IMA showed considerable value on the diagnosis of CAD, which had high specificity and positive predictive value. IMA level and the number of diseased vessels of coronary were significantly positively correlated.
