Abstract:Hepatitis B virus (HBV) infection is a worldwide epidemic,while the mother-to-child transmission is the main route of transmission of HBV infection in China.Women of childbearing age often faced with antiviral therapy problem during pregnancy and lactation.Through the retrieval of domestic and foreign database literature,all the approved drugs for anti-hepatitis B virus infection (including interferon α,lamivudine,telbivudine,tenofovir dipivoxil,entecavir,adefovir dipivoxil And enquitril) in pregnancy and lactation using safety data and experience were summarized,and presented by placental transport,animal reproductive toxicity and teratogenicity studies,experience of using in human pregnancy,drug milk secretion and experience of using in human lactation.Among them,research data of tenofovir and telbivudine is relatively sufficient and can be used as a choice of anti-HBV treatment during pregnancy.No teratogenic of both is found in animal studies,and the incidence of birth defects is not increased while tenofovir and telbivudine were employed in human pregnancy.Lamivudine is widely used during pregnancy in human (HIV treatment),and no increase in the incidence of birth defects,it may also be used as one of the options for anti-HBV treatment during pregnancy.
Evaluation of the Efficacy and Safety of Telbivudin in Preventing Mother-to-infant HBV Transmission

Abstract:OBJECTIVE: To evaluate the efficacy and safety of telbivudine in preventing mother-to-infant HBV transmission in pregnant women who have positive HBsAg and HBeAg during the third trimester of pregnancy. METHODS: A total of 80 pregnant women, who had positive HBsAg and HBeAg with HBV DNA levels of ≥1 × 10 6 copies · mL -1 were divided into two groups: the treatment group and the control group. Each group comprised 40 cases. The subjects in the treatment group received telbivudine 600 mg once daily from the 28th weeks of pregnancy till delivery. The subjects in the control group did not receive telbivudine or other anti-virus drugs. The adverse reactions of the subjects in the treatment group were observed, and the serum HBV DNA levels in the two groups were measured at the 28th weeks of pregnancy, upon delivery and 6 months after delivery, respectively. The infants in the two groups received IM HBIG 200 IU immediately after birth and 1 month after birth. And both of the groups received IM hepatitis B vaccine 10 μg immediately after birth as well as 1 month and 6 months after birth. The serum HBsAg and HBV DNA levels in all infants were measured at birth, 6 months, and 12 months after birth, respectively. RESULTS: There was no significant difference in serum HBV DNA levels between the treatment group and the control group at the 28th weeks of pregnancy. The serum HBV DNA level immediately after delivery in the treatment group [(3.48±0.90) × 10 copies · mL -1] was significantly decreased compared with the control group [(6.96±0.94) × 10 copies · mL -1](P<0.05). There was no significant difference in HBV DNA levels between the treatment group and the control group 6 months after delivery. The rates of mother-to-infant HBV transmission in 12-month-old infants in the treatment group and the control group were 97.5% and 75%, respectively. The difference was statistically significant(P<0.05). No adverse reaction was found in the treatment group, and abnormal development was not found in the infants of the two groups. CONCLUSION: Telbivudine appears to be effective and safe in preventing HBV mother-to-infant transmission and has no influence on infant development.
Effect of Prophylactic Antiviral Intervention on T Cell Immunity in Hepatitis B Virus-Infected Pregnant Women

Abstract:Antiviral intervention in hepatitis B virus (HBV)-infected pregnant women can effectively reduce mother-to-child transmission. However, the immunological characteristics of pregnant women with chronic HBV infection and the effects of antiviral intervention during pregnancy on maternal immune response remain unknown. We aimed to investigate these effects by comparing mothers who received antiviral intervention during pregnancy with those who did not. Pregnant women positive for hepatitis B surface antigen and hepatitis B e-antigen (HBsAg+ HBeAg+) were enrolled at delivery, including 34 received prophylactic antiviral intervention during pregnancy (AVI mothers) and 15 did not (NAVI mothers). T lymphocyte phenotypes and functions were analysed using flow cytometry. At delivery, maternal regulatory T cell (Treg) frequency in AVI mothers was significantly higher than that in NAVI mothers (P < 0.002), and CD4+ T cells in AVI mothers displayed a decreased ability to secrete IFN-γ (P = 0.005) and IL-21 (P = 0.043), but an increased ability to secrete IL-10 and IL-4 (P = 0.040 and P = 0.036), which represented a higher Treg frequency, enhanced Th2 response and suppressed Th1 response. Treg frequency among AVI mothers was correlated negatively with serum HBsAg and HBeAg levels. After delivery, the ability of CD4+ T cells or CD8+ T cells to secrete IFN-γ or IL-10 was similar and no significant difference in Treg frequency was found between the two groups. Prophylactic antiviral intervention during pregnancy has an effect on T cell immunity in pregnant women, which was characterised by increased maternal Treg frequency, enhanced Th2 response and suppressed Th1 response at delivery.
Safety and Efficacy of Telbivudine in Late Pregnancy to Prevent Mother‐to‐child Transmission of Hepatitis B Virus: A Multicenter Prospective Cohort Study

Abstract:SummaryInfection of hepatitis B virus (HBV) occurs in ~10% of infants of HBV‐infected mothers with positive hepatitis B e antigen (HBeAg) after immunoprophylaxis. We aimed to evaluate the safety and efficacy of telbivudine used during late pregnancy for preventing mother‐to‐child transmission of HBV. We conducted a multicenter prospective cohort study in 5 hospitals from 2012 to 2014, which enrolled HBV‐infected singleton pregnant women with positive HBeAg. By their choice, women were divided into therapy (telbivudine 600 mg/day, from gestation 28‐32 weeks to 3‐4 weeks postpartum) and control (no antiviral agent) groups. Infants received passive‐active immunoprophylaxis and follow‐up at the age of 7‐14 months. Totally, 328 pregnant women were included: 149 in the telbivudine group and 179 in the control group. Baseline HBV DNA levels were similar in the 2 groups (7.43 vs 7.37 log10 IU/mL, P = .711). At delivery, HBV DNA levels in the telbivudine and control groups were 3.80 and 7.26 log10 IU/mL, respectively (P < .0001). Of the infants, 128 (85.9%) in the telbivudine group and 156 (87.2%) in the control group were followed up. No infant in the telbivudine group had chronic infection, while 2 (1.28%) infants in the control group did (P = .503). Three (2.34%) infants in the telbivudine group, but none in the control group, had severe congenital or developmental abnormalities (P = .090). The data indicate that telbivudine may block perinatal HBV transmission. However, larger studies are required to clarify whether anti‐HBV therapy in pregnancy is associated with severe adverse effects in the foetuses and infants.
Telbivudine Can Safely Reduce Mother-to-child Transmission in Chronic Hepatitis B Women after 12 Weeks of Gestation

Abstract:To evaluate the efficacy and safety of telbivudine in chronic hepatitis B women during the second and third trimesters of pregnancy. The week 12–34 of pregnant women were screened in this prospective non-intervention study, with HBV DNA > 106 IU/mL and alanine aminotransferase > 50 IU/L. The patients were received telbivudine treatment as a treatment group or without antiviral treatment as a control group. All infants were received recombinant hepatitis B vaccine 10 μg within 12 h of birth, at week 4 and week 24, immunoglobulin G within 12 h of birth and were detected HBV markers at the range from 7 to 12 months after delivery. A total of 241 patients were finally enrolled, 139 patients in telbivudine group and 102 patients in control group. HBsAg negative rate of infants was 99.3% (135/136) in telbivudine group and was 91.9% (91/99) in control group after 7 months (P = 0.005), respectively. The incidence of undetectable HBV DNA levels (47.5%) was significantly lower in telbivudine-treated mothers than that in the controls (0%), and 75.5% patients alanine aminotransferase returned to normal in telbivudine group, and 51% in control group at delivery (P < 0.001), respectively. Telbivudine can safely reduce mother-to-child transmission in chronic hepatitis B women after 12 weeks of gestation.

Abstract:SummaryWomen with chronic hepatitis B should maintain nucleotide analogue treatment to prevent disease progression during pregnancy. The aim of this study was to prospectively evaluate the efficacy and safety of telbivudine used throughout pregnancy for preventing hepatitis B virus (HBV) mother‐to‐child transmission (MTCT). From January 2012 to June 2014, women who were receiving telbivudine therapy and became pregnant were enrolled in group A at 28 weeks of gestation. Pregnant women with an HBV DNA level >106 IU/mL were enrolled in either group B (telbivudine started at 28 weeks of gestation) or group C (control group without treatment). MTCT was defined as infants who were positive for serum hepatitis B surface antigen at 7 months after birth. There were 41, 179 and 177 pregnant women (397 infants) enrolled in groups A, B and C, respectively. The HBV DNA load at 28 weeks of gestation and delivery was 1.50 ± 0.62 vs 1.45 ± 0.61, 8.05 ± 0.37 vs 4.24 ± 0.89 and 7.94 ± 0.62 vs 7.86 ± 0.73 log10 IU/mL in groups A, B and C, respectively. The rate of MTCT in group C was 4.60%, which was significantly higher than the rates in groups A and B (0% and 0.6%, respectively) (P = .043). The difference between group A and group B was not significant. The rates of neonatal congenital abnormalities were 2.4%, 0.6% and 2.3% in groups A, B and C, respectively, and there were no significant differences (P = .140). Telbivudine used throughout pregnancy may be safe and effective for mothers and infants, but it may not enhance the efficacy of an HBV MTCT block compared with treatment starting at 28 weeks of gestation (NCT02253485).

Abstract:Background: Hepatitis B virus (HBV) infection is a severe health problem, especially in developing countries. Almost 45% of the population lives in highly endemic areas, where the most common form of transmission is mother to child transmission (MTCT). Administration of antiviral therapy has been established. Nevertheless, its efficacy still remains controversial. Methods: We conducted the current study to fully evaluate the effectiveness of lamivudine in preventing the MTCT of HBV based on randomized controlled trials (RCTs). Four English electronic databases and four Chinese electronic databases were searched from the inception of each database to 26 September 2017. Studies were included if they (1) were human RCT studies, (2) indicated exposure to lamivudine, (3) explicitly indicated control to placebo or no treatment, (4) indicated the participants were pregnant women infected with HBVand (5) compared the outcome of interest as the MTCT. Extracted data were tabulated and analyzed using Review Manager. Results: Eleven RCTs were included and analyzed. Compared with controls (placebo or no treatment), lamivudine significantly reduced the probability of MTCT, as indicated by newborn HBsAg seropositivity (RR=0.44, 95% CI 0.26 to 0.74, I-2=41%), HBeAg seropositivity (RR=0.66, 95% CI 0.36 to 1.19, I-2=0%) and HBV DNA seropositivity (RR=0.29, 95% CI 0.18 to 0.4'6, I-2=0%) within 24 h after birth. Similar results were noted pertaining to infant HBsAg seropositivity and HBV DNA seropositivity within 6-7 and 12 mo. Conclusions: Lamivudine can significantly reduce the MTCT of HBsAg and HBV DNA of neonates during the third trimester of pregnancy without severe adverse events.
The efficacy of immune interruptions on blocking mother-toinfant transmission of hepatitis B virus

Abstract:To investigate the efficacy and the roles of immune interruptions on blocking mother-to-infant transmission of hepatitis B virus (HBV),667 pregnant women who were seropositive for hepatitis B surface antigen (HBsAg) were enrolled. According to the lab test results of HBV e antigen (HBeAg) and HBV DNA in the mothers, the mothers were divided into either HBeAg-positive group or HBeAg-negative group, and either HBV DNA-positive group or HBV DNA-negative group. Depending on whether the mothers were receiving hepatitis B immunoglobulin (HBIG) once a month during the third trimester of pregnancy, they were divided into either the HBIG-receiving group or the non-HBIG-receiving group. All infants received regular passive and active HBV immunization after birth. Infant serum HBV markers were determined 8-12 months after birth. Twenty infants born to HBsAg-positive mothers were infected with HBV and the failure rate of immune interruptions was 3.0%. The failure rate of immune interruptions in the HBeAg-positive group was 8.7%, which was significantly higher than that observed in HBeAg-negative group 0.2% (P<0.001). However, the HBV surface antibody (HBsAb) positive rates of the infants between the two groups were not statistically different (P=0.988). The overall failure rate of immune interruptions of infants born to the HBV DNA-positive group was 8.1%, and the infants infected with HBV were all born to the mothers whose serum HBV DNA levels were above 6 log10 copies/ml. There were no statistical differences in the incidence of perinatal HBV infection between mothers receiving HBIG and those not receiving HBIG (3.7% vs 2.7%, P=0.479). No statistical differences were seen in HBsAb positive rates versus vaccination of the infants between the two groups (84.4% vs 82.4%, P=0.519). Among HBeAg-positive pregnant women receiving HBIG, the failure rate of immune interruptions in their infants was 8.4%, compared with 8.9% in those who did not receive HBIG, with no statistical differences (P=0.892). Importantly, 10 out of 11 infants who were infected with HBV were already serum HBsAg-positive immediately after birth, suggesting intrauterine infection either via venous or cord blood. These results indicate that preventative vaccine immunization plus HBIG administration does not block intrauterine transmission of HBV. Approaches to decrease the serum HBV DNA load in pregnant women by antiviral treatment may efficiently block mother-to-infant transmission.

Abstract:Objective To compare the efficacy and safety of telbivudine (LdT), tenofovir alafenamide fumarate (TAF), and tenofovir disoproxil fumarate (TDF) for preventing hepatitis B transmission in immune‐tolerant pregnant women with HBV infection. Methods We conducted a retrospective cohort study involving women who had hepatitis B virus deoxyribonucleic acid (HBV DNA) ≥ 2 × 105 IU/mL and initiated LdT, TDF, or TAF to prevent mother‐to‐child transmission (MTCT). The primary endpoint was the safety of mothers and infants. The secondary endpoints were maternal HBV DNA reduction at delivery and MTCT rate. Results A total of 96 patients were enrolled in the study (LdT group, n = 36; TDF group, n = 35; TAF group, n = 25). All infants received hepatitis B virus immunoprophylaxis. The MTCT rate was 0%([0 of 25] vs. [0 of 35] vs. [0 of 36], p > .05). No severe liver function damage occurred in any of the mothers. Babies delivered in all groups had prenatal ultrasound screening abnormalities, but abnormality rates were not statistically significant between groups. Conclusion The application of TDF, TAF, or LdT to immune‐tolerant HBV‐infected pregnant women in middle–late pregnancy can successfully interrupt MTCT of the HBV virus. However, for all three groups of pregnant women who delivered babies with abnormal prenatal ultrasound screening, an expanded sample size may be needed for further observation.

Abstract:Background Few data exist regarding use of nucleos(t)ide analogs started in early pregnancy for mothers with active chronic hepatitis B (CHB). We assessed the safety and efficacy of lamivudine/telbivudine initiated in the first trimester versus no treatment in mothers with active CHB. Methods We retrospectively enrolled 94 mothers newly diagnosed with active CHB in the first trimester of pregnancy. Patients with or without antiviral therapy were followed until postpartum week 28. All newborns received immunoprophylaxis. The primary endpoint was the safety of mothers and infants. The secondary endpoints were hepatitis B virus (HBV) DNA suppression and mother-to-child transmission (MTCT) rate. Results Fifty-nine of the 94 mothers initiated lamivudine/telbivudine (27/32) in the first trimester of pregnancy; 35 received no treatment. At delivery, the viral load reduction was similar between lamivudine and telbivudine. Early initiation of lamivudine/telbivudine significantly increased the proportion of mothers achieving HBV DNA <10 6 copies/ml compared with those with no treatment (100 versus 42.42 %, p < 0.001). At postpartum week 28, the MTCT rate was significant lower in the treated group than in the control group (0/61 or 0 versus 4/34 or 11.76 %, p = 0.028). Lamivudine and telbivudine were well tolerated in the mothers except mild creatine kinase (CK) elevation. There existed no differences in gestational age, infant length and weight, Apgar score, adverse events, or birth defect rates between infants from treated and untreated mothers. Conclusions Treatment with lamivudine or telbivudine for active CHB in early pregnancy appears to be safe and effective for controlling maternal disease as well as interrupting MTCT.

Abstract:Background There are few large sample studies evaluating the safety and efficacy of lamivudine (LAM) or telbivudine (LdT) in preventing hepatitis B mother-to-child transmission (MTCT) in highly viremic mothers in the third trimester of pregnancy in real-world settings. The purpose of this study was to analyze a large sample size of HBV-infected mothers to better understand the safety and efficacy of LAM and LdT under the aforementioned criteria. Methods During the period of November 2008 to November 2017, we retrospectively enrolled mothers with HBV DNA > 1 × 106 IU/mL who received LAM or LdT during the third trimester of pregnancy and compared them to untreated mothers. All mothers were divided into the three following groups: the LAM group, the LdT group, and the control group. Results A total of 2624 HBV-infected mothers were enrolled in the study, with 363 in the LAM group, 1283 in the LdT group, and 978 in the control group. The MTCT rates were significantly lower in the LAM or LdT group than that in the control group (0.4% or 0.3% versus 9.0%, P < 0.001). Infants born to untreated mothers had a significantly higher risk of HBV infection (OR = 28.6, 95% CI: 10.4–78.7, P < 0.001). There were no significant differences in perinatal complications between the three groups (P > 0.05). There were also no differences for gestational age or infants' height, weight, Apgar scores, or birth defect rates. Postpartum discontinuation of antiviral therapy did not seem to increase the risk of postpartum alanine aminotransferase (ALT) flare. Conclusion LAM or LdT treatment initiated in the third trimester for mothers with HBV DNA > 1 × 106 IU/mL was equally safe and effective in preventing MTCT.

Abstract:AIM:To evaluate the efficacy and safety of lamivudine treatment of chronic hepatitis B disease in pregnancy.METHODS:The study group was comprised of 38 chronic HBV patients who were diagnosed pregnant during lamivudine treatment and voluntary to continue the same therapy. The control group was from documented patient data in the literatures. We compared the following parameters with those of a control group: anti-HBV efficacy, complications of pregnancy (abortion, preterm birth, neonatal asphyxia, fetal death, and congenital anomaly), incidence of HBV-positive babies and developmental anomalies in pregnant women treated with lamivudine.RESULTS:The blocking rate of lamivudine treatment was significantly higher than that of active vaccine immunization for babies with double-positive (HBsAg/HBeAg) mothers with 30-30-10 mug doses of vaccine (74.07%) and with 30-20-10 mug (64.87%). The natural vertical HBV transmission from mother to infant of "double-positive" mothers was 100% (10/10). No pregnancy complication was noted during the observation period, but in the control group the incidences of pregnancy complication were 16.67% (abortion), 43.02%(preterm), 15.62% (neonatal asphyxia), and 4.49% (fetal death), 10.0% (congenital anomaly). No HBV-positive newborn was detected and no developmental anomaly was found in the study group.CONCLUSION:Lamivudine is helpful to prevent maternal-infant HBV transmission and may reduce the complications of HBV-infected pregnant patients.

Abstract:AIM To evaluate the safety of lamivudine (LAM) treatment for chronic hepatitis B in early pregnancy. METHODS A total of 92 pregnant women who received LAM treatment either before pregnancy or in early pregnancy were enrolled in this study. All of the pregnant women volunteered to take lamivudine during pregnancy and were not co-infected with hepatitis C virus, human immunodeﬁciency virus, cytomegalovirus, or other viruses. All infants received passive-active immunoprophylaxis with 200 IU hepatitis B immunoglobulin and three doses of 10 μg hepatitis B vaccines (0-1-6 mo) according to the guidelines for the prevention and treatment of chronic hepatitis B. Adverse events were observed throughout the entire pregnancy and perinatal period, and the effectiveness of lamivudine treatment for blocking mother-to-infant transmission of hepatitis B virus (HBV) was evaluated. All adverse events in mothers and infants during pregnancy and the perinatal period and the HBV mother-to-infant transmission blocking rate were compared with the literature. RESULTS Among the 92 pregnant women, spontaneous abortions occurred in 11 cases, while 3 mothers had a second pregnancy after the initial abortion; 72 mothers delivered 73 live infants, of whom 68 infants were followed up for no less than 6 mo, and 12 mothers were still pregnant. During pregnancy, the main maternal adverse events were vaginitis (12/72, 16.7%), spontaneous abortion (11/95, 11.6%), and gestational diabetes (6/72, 8.3%); only one case had 1-2 degree elevation of the creatine kinase level (195 U/L). During the perinatal period, the main maternal adverse events were premature rupture of the membranes (8/72, 11.1%), preterm delivery (5/72, 6.9%), and meconium staining of the amniotic fluid (4/72, 5.6%). In addition, 2 infants were found to have congenital abnormalities; 1 had a scalp hemangioma that did not change in size until 7 mo, and the other had early cerebral palsy, but with rehabilitation training, the infant's motor functions became totally normal at 2 years of age. The incidence of adverse events among the mothers or abnormalities in the infants was not higher than that of normal mothers or HBV-infected mothers who did not receive lamivudine treatment. In only 2 cases, mother-to-infant transmission blocking failed; the blocking rate was 97.1% (66/68), which was higher than has been previously reported. CONCLUSION Lamivudine treatment is safe for chronic HBV-infected pregnant mothers and their fetuses with a gestational age of less than 12 wk or throughout the entire pregnancy.
Comparative efficacy of antiviral therapy in preventing vertical transmission of hepatitis B: a network meta-analysis

Abstract:Background & aims: Antiviral drugs are safe and effective in the third trimester to prevent intrauterine transmission of hepatitis B virus, and are recommended for hepatitis B virus (HBV) infected gravid mothers (between weeks 28 and 32) with high viral load, followed by postnatal hepatitis B immunization in the newborn. We estimated the comparative efficacy of antiviral drugs for prevention of vertical transmission of HBV, through a network meta-analysis of clinical trials. Methods: We conducted a comprehensive search of MEDLINE, EMBASE and published proceedings from major liver meetings from January 1980 to November 2014. We conducted pair-wise meta-analyses and Bayesian framework using Markov chain Monte Carlo methods, combining direct and indirect evidence for any given pair of treatments. Results: Seventeen clinical trials involving 2764 newborns of hepatitis B surface antigen seropositive mothers were eligible for analysis. There were no clinical trials involving tenofovir or entecavir. On pair-wise meta-analyses, telbivudine (hazard ratio, HR 0.12, 95% confidence interval (CI) 0.04-0.37; I(2) = 0%), and Lamivudine (HR 0.40, 95% CI 0.24-0.65; I(2) = 0%), were more effective than placebo in reducing vertical transmission of HBV in high viremic hepatitis B e antigen (HBeAg)-positive chronic Hepatitis B Chinese patients. Sensitivity analyses limited to studies with HBeAg seropositive mothers revealed similar results. Conclusions: Based on a Bayesian network meta-analysis of clinical trials, combining direct and indirect treatment comparisons, telbivudine appears to be more effective than Lamivudine for preventing vertical transmission of HBV infection. Trials assessing the efficacy of tenofovir or entecavir compared to placebo or other antiviral drugs are lacking.

Abstract:Objective:To study the safety of Telbivudine for hepatitis B virus (HBV) transgenic mice with pregnancy and the efficacy of preventing HBV mother-to-child. Methods:Thirty-three transgenic mice replicating high levels of HBV were randomly allocated into 3 groups,each group of eleven mice. Pregnant HBV transgenic mice in experimental groups,Group A were given normal saline and Group B were given orally telbivudine (600mg/kg/d) from mating to delivery,Group C were given orally telbivudine (1200mg/ kg/d).The changes of serum HBsAg,HBV DNA before and after the treatment were detected. At the same time,eleven C57BL /6J female mice (D) were given telbivudine (600mg/kg/d).The safety indexes of pregnant mice,fetus and newborns in experimental groups and control groups were observed. Newborns were detected about the changes of serum HBsAg and HBV DNA. Liver tissue was examined by immunohistochemical method. Results:The average serum HBsAg titers from mice in experimental Group A was no significant difference between before and after the treatment (P>0.05),while Group B and Group C were lowered after drug treatment (P<0.05). The serum HBV DNA titers from mice in each experimental groups was no significant difference between before and after the treatment (P> 0.05). No significant abnormalities of live fetus was observed. The postnatal survival and weight of live pups were not different between the groups. The liver tissue of newborn was collected to detect negative HBsAg in the cells by immunohistochemical method.Conclusions:Effective telbivudine treatment during gestation can prevent HBV mother-to-child. It has no significant adverse effect on safety of pregnant mice and their offspring.
Comparison of HBV-DNA and HBeAg As Antiviral Therapeutic Indicators among HBV-infected Pregnant Women: a Systematic Review and Meta-Analysis

Abstract:Background: Hepatitis B virus deoxyribonucleic acid level (HBV-DNA) >= 5.3 log10IU/mL among pregnant women was recommended as an antiviral therapeutic indicator. However, implementation of HBV-DNA testing has varying difficulties in places. In this study, we explored the implementation rate of HBV-DNA testing worldwide, and possibility of HBeAg testing replacing HBV-DNA as an antiviral treatment indicator during pregnancy. Methods: We searched five electronic databases including PubMed, Embase, Cochrane Library, Scopus, and China National Knowledge Infrastructure (CNKI) for studies published between Jan 1, 2000, and Nov 16, 2020. Studies were eligible for inclusion if HBV DNA testing implementation rate is available, or if maternal HBV DNA level could be analyzed by HBeAg status. The rates were pooled after data was made a Freeman-Tukey double arcsine transformation. This study is registered with PROSPERO, CRD42021235711. Results: A total of 9,575 studies were identified, 79 were finally included in this study. The HBV-DNA testing implementation rate was 36.6% (95% CI, 28.3-45.3%) globally. The rate of HBV-DNA >= 5.3 log10IU/mL was 81.51% (95% CI, 71.68-89.74%) among HBeAg positive pregnant women, and was 4.08% (95% CI, 2.14-6.54%) in HBeAg negative pregnant women. Even if infants were immunized with hepatitis B vaccine and hepatitis B immunoglobulin, the rate of mother-to-child transmission was still 4.87% (95% CI, 4.10-5.68%) among HBeAg positive mothers, and was 0 (95% CI, 0-0.07%) among HBeAg negative mothers, with a RR of 30.40 (95% CI, 11.31-81.72). Conclusions: The implementation rate of HBV DNA testing varies from region to region. Limited studies show that HBV DNA testing does not cover all pregnant women with hepatitis B. When HBV-DNA testing is not available, it is worth considering to use HBeAg positivity as an antiviral therapeutic indicator among HBV-infected pregnant women for preventing MTCT.
Safety of Telbivudine Treatment for Chronic Hepatitis B for the Entire Pregnancy

Abstract:Orally administered nucleus(t)ide analogues (NA) have brought about a simple, safe and effective therapeutic approach for chronic hepatitis B (CHB). However, treatment duration is long and some female patients become pregnant during treatment. In recent years, there have been gradually increasing reports on the safety of telbivudine (LdT) treatment for chronic hepatitis B virus (HBV) infection in the third trimester of pregnancy to block mother-to-infant transmission (MTIT) of HBV; however, the safety of LdT treatment for chronic HBV infection for the entire pregnancy has not been reported. The aim of the present study was to evaluate the safety of LdT treatment for chronic HBV infection for the entire pregnancy and provide a reference for HBV-infected fertile women on how to block MTIT of HBV. Eighty-six pregnant women who received LdT treatment either before or in early pregnancy were enrolled in the study. Adverse events were prospectively observed for the entire pregnancy and perinatal period, and short-term and long-term follow-up of infants was conducted, monitoring the abnormalities of infants and blocking rate of MTIT with LdT treatment. Eighty-six pregnant women treated with LdT had a total of 89 pregnancies: 6 (6.7%) had early embryonic death or spontaneous abortion, 1 (1.1%) had ectopic pregnancy and three had a second pregnancy after initial abortion. Fifty-one mothers completed pregnancy: one had induction of labour at 24 weeks of pregnancy for cleft lip and palate of the foetus and 50 delivered 52 full-term live infants. One infant had right ear accessories, and the total occurrence of congenital abnormality was 3.8%. Thirty-nine infants were followed up for more than 6 months and completed all examinations for MTIT. None of the infants were HBsAg positive, resulting in a 100% success rate of blocking MTIT. All mothers maintained good liver function during the third trimester of pregnancy; 86% maintained complete virological response (HBV DNA <500 copies/mL) prior to delivery, and none developed progression of liver disease. Factors leading to increased adverse effects and drug resistance were not found. LdT treatment is safe and effective in chronic HBV-infected pregnant mothers for the entire pregnancy.

Abstract:Objective To investigate the safety and efficacy of telbivudine in treating hepatitis B virus (HBV) infection in pregnant transgenic mice. Methods Thirty-three transgenic mice with HBV DNA positive were evenly divided into 3 groups: HBV Tg-L group, HBV Tg-H group and HBV Tg group, which were treated with 600 mg ·kg~(-1)·d~(-1) telbivudine, 1200 mg·kg~(-1)·d~(-1) telbivudine and 0. 9% sodium chloride intragastrically every day during the period from mating to delivery, respectively. Healthy control group (11 common mice) were treated with 600 mg·kg~(-1) ·d~(-1) telbivudine. The changes of serum HBsAg titers and HBV DNA levels before and after treatment were determined. The titers of HBsAg were measured by enzyme-linked immunosorbent assay (ELISA). The levels of HBV DNA were measured by fluorescent real-time polymerase chain reaction (PCR). The safety indexes of pregnant mice, fetus and newborns in all experimental groups and control group were observed. Newborns were examined for the changes of serum HBsAg and HBV DNA. The data were compared using analysis of variance, t test and χ~2 test. Results The mean serum HBsAg titers in HBV Tg group were not significantly changed before and after treatment (t = 0. 34, P>0. 05). Serum HBsAg titers in HBV Tg-L group and HBV Tg-H group were (187. 39 ± 23. 18) μg/L vs (160. 50±27. 69) μg/L, (190. 48 ± 22. 43) μg/L vs (161. 89 ± 21. 23) μg/L before and after treatment, respectively (t=2.91, t = 3. 16;both P<0.05). The mean serum HBV DNA levels in all experimental groups were not significantly different before and after treatment (P>0. 05). The mean serum HBV DNA levels of the offspring mice in HBV Tg, HBV Tg-L group and HBV Tg-H group were (11. 22±1. 08), (8. 38±1. 80) and (8. 97±1. 86) lg copy/mL, respectively (F=4. 34, P<0. 05). The serum HBsAg titers of offspring mice were (93. 03±18. 37), (9. 56±4. 39) and (9. 27±2. 69) μg/L, respectively (F=18. 11, P<0. 05). No significant abnormalities of live fetus were observed. The postnatal survival and weight of live pups were not different among the three groups. Conclusions Effective telbivudine treatment during gestation has no significant adverse effects on the safety of pregnant mice and their offsprings. HBsAg titers are significantly different before and after treatment, whereas serum HBV DNA levels are similar before and after treatment, which is possibly due to the species of HBV transgenic mice and the relatively short pregnant time of mice.
Infant immune response to hepatitis B vaccine after fetal exposure to telbivudine

Abstract:Whether telbivudine (LdT) treatment to pregnant women with hepatitis B surface antigen (HBsAg) affects infant immune response to hepatitis B vaccine (HepB) has not been investigated. A total of 127 HBsAg positive mothers and their neonates were enrolled and followed up at 11–13 months. Mothers took LdT (LdT group) or did not receive antiviral therapy (control group). Infant anti-HBs, immune cells and cytokines were measured after HepB was administered according to 0-1-6 procedure. We performed a 1:3 propensity score matching (PSM). Immune indexes in the two groups were compared. Baseline characteristics of mother-baby pairs were comparable in LdT group and control group. Infant anti-HBs geometric mean concentration (GMC) did not differ significantly between the two groups [767.70 (745.35) vs. 711.90 (819.60), P = .599]. There was no difference between the two groups in infant positive rate of anti-HBs [97.8% (91/93) vs. 97.1% (33/34), P = .999] and strong positive rate of anti-HBs [40.9% (38/93) vs. 44.1% (15/34), P = .742]. Infants with negative, low, medium, and high anti-HBs levels were similarly distributed between the two groups (P = .511). No differences in proportion of helper T cells, cytotoxic T cells, B cells, myeloid dendritic cells, and plasmacytoid dendritic cells of infants (P > .05) were detected between the two groups. Children in the LdT and control group had comparable levels of interleukin-2, interleukin-4, interleukin-6, interleukin-10, interleukin-12, interferon-α, interferon-γ and tumor necrosis factor-α (P > .05). Intrauterine exposure to LdT was safe to infant immune response to HepB after birth.

Abstract:This study sought to assess the antiviral efficacy of lamivudine (LMV) administered during third trimester to reduce maternal viraemia and to identify the emergence of LMV resistance. A prospective observational analysis was performed on 26 mothers with high viral load (>10⁷ IU/mL). Twenty-one women received LMV (treated group) for an average of 53 days (range 22-88 days), and the remaining five formed the untreated control group. Serum samples from two time points were used to measure HBV DNA levels and antiviral drug resistance. The LMV-treated women achieved a median HBV DNA reduction of 2.6-log10 IU/mL. Although end-of-treatment (EOT) HBV DNA in four (18%) LMV-treated women remained at >10(7) IU/mL (± 0.5 log IU/mL), no mother-to-baby transmission was observed. In contrast, a baby from the untreated mother was HBsAg positive at 9 months postpartum. Four technologies were used for drug resistance testing. Only ultra-deep pyrosequencing (UDPS) was sufficiently sensitive to detect minor viral variants down to <1%. UDPS showed that LMV therapy resulted in increased viral quasispecies diversity and positive selection of HBV variants with reverse transcriptase amino acid substitutions at sites associated with primary LMV resistance (rtM204I/V and rtA181T) in four (19%) women. These viral variants were detected mostly at low frequencies (0.63-5.92%) at EOT, but one LMV-treated mother had an rtA181T variant that increased from 2.2% pretherapy to 25.59% at EOT. This mother was also infected with the vaccine escape variant (sG145R), which was inhibited by LMV treatment. LMV therapy during late pregnancy only reduced maternal viraemia moderately, and drug-resistant viral variants emerged.
