Abstract:Malignant tumours of myofibroblasts are rare lesions. Post radiation sarcoma is a rare potential late sequel of ionizing radiation. We present the first case of myofibrosarcoma of the hypopharynx in a 67-year-old man, occurring 16 years after radiotherapy for laryngeal carcinoma.

Abstract:We herein present the first-reported case in the English-language literature of a spindle cell rhabdomyosarcoma in the hypopharynx of an adult. A 58-year-old male presented with a progressive, 7-month history of discomfort during swallowing, accompanied by hoarseness for 1 month. Computed tomography and magnetic resonance imaging revealed a tumor in the left pyriform sinus; frozen section evaluation suggested that it was rhabdomyosarcoma. The tumor was removed completely via left lateral cervical incision. Section margins were negative, and laryngeal function was preserved. The postoperative pathological results indicated spindle cell rhabdomyosarcoma. The patient received postoperative radiotherapy (6,000 cGy in 200 cGy fractions, delivered over 30 days). However, he was also required to undergo urgent tracheostomy due to severe laryngeal obstruction, precipitated by laryngeal edema, 3 months subsequent to radiotherapy. The patient was free of disease 25 months postoperatively but is still unable to close his tracheostomy.

Abstract:IntroductionPrimary thyroid leiomyosarcoma is an extremely rare soft tissue sarcoma, characterized by high malignancy and poor prognosis. Currently, only 13 cases of primary thyroid leiomyosarcoma have been described in the medical literature (limited to English).Case presentationA 76-year-old female presented with a giant neck mass. Physical examination revealed a large, firm mass in the left thyroid gland. No symptoms such as hoarseness or dysphagia were noted at the time of presentation. The patient underwent unilateral thyroidectomy and cervical lymph node dissection.Clinical discussionPathologic findings revealed a low-grade sarcoma with spindle-shaped tumor cells in an interwoven, sheet-like distribution. Immunohistochemistry showed positivity for desmin, SMMHC, STAT6, CK19, and Galectin3, but negativity for S-100, MyoD1, CD34, CK (AE1/AE3), CD117, and CD56. The findings were consistent with thyroid leiomyosarcoma.ConclusionThe treatment of primary thyroid LMS presents challenges due to its atypical symptoms and high malignance, highlighting the imperative for further exploration of therapeutic strategies to improve the outcomes.

Abstract:Objective Carcinosarcoma consists of carcinomatous and sarcomatous tissues and is an aggressive malignant tumor. It is rarely reported in the hypopharynx. Methods A 72-year-old man presented with dysphagia and dyspnea. Laryngoscopy, computed tomography (CT), and F-18-fluorodeoxyglucose positron emission tomography/CT (F-18-FDG PET/CT) showed a neoplasm on the left posterior hypopharyngeal wall. The patient underwent bilateral neck dissection and excision of the hypopharyngeal cancer followed by postoperative radiation therapy. Results Immunohistochemistry revealed carcinomatous cells with membrane positivity for cytokeratin, glucose transporter-1 (GLUT-1), phosphoinositide-3 kinase (PI3K), hypoxia-inducible factor-1 (HIF-1), and hexokinase-II as well as sarcomatous cells with membrane positivity for smooth muscle actin, GLUT-1, HIF-1, and PI3K. Histopathology and immunohistochemistry revealed a true carcinosarcoma of the hypopharynx (pT3N0M0, Stage III). Conclusions Thorough immunohistochemistry is required for a correct diagnosis of hypopharyngeal carcinosarcoma. F-18-FDG PET/CT may help to distinguish hypopharyngeal carcinosarcoma from benign tumors.
Analysis of Clinicopathologic Features and Prognosis in 10 Patients with Non-Epithelial Malignant Neoplasmas of the Larynx and Hypopharynx

Abstract:Objective To investigate the clinicopathologic features,diagnosis and treatment of non-epithelial malignant tumors of larynx and hypopharynx. Methods From 1979 to 2002,the clinicopathologic features,diagnosis and treatment of 10 patients with non-epithelial malignant neoplasmas of larynx and hypopharynx were retrospectively analyzed.Primary lesions of 8 cases located at larynx whose pathological types included 5 sarcomas,2 lymphomas and 1 extramedullary plasmacytoma;Lesions of 2 cases located in hypopharynx.whose pathological types included 1 primary carcinosarcoma and 1 metastatic carcinoma diffused from stomach through lymphoma.All patients were treated with surgery combined with radiotherapy and/or chemotherapy. Results 2 patients with free-neoplasm have outlived for 7 years or 8 years respectively;other 7 patients with free-neoplasm had died in 1-6 years after surgery;and 1 case lost follow-up. Conclusion Non-epithelial malignant neoplasm in larynx and hypopharynx is uncommon and has poor prognasis.Immunohistochemistry staining is helpful for the final diagnosis.The therapy must differ from different types of tumors.
[Myofibroblastic sarcoma of the larynx : a case report and review]

Abstract:Background: Myofibroblastic sarcomas or myofibrosarcoma, are extremely rare malignant neoplasms of myofibroblasts. They are characterized by the pattern of cells and special immunohistochemical markers such as vimentin, desmin and alpha-smooth-muscle actin. Patient and method: The case of a patient with a history of frequently relapsing papillomas of the larynx is reported. Chronic laryngitis with focal low-grade dysplasia of the squamous epithelium was diagnosed approximately 1 year after the first treatment of the papillomas. After approximately 2 years the pathologist diagnosed the rare myofibroblastic sarcoma of the larynx. The patient underwent laryngectomy due to the spread of the tumor with a bilateral selective neck dissection. The patient is at present still free of recurrence and metastases. Results and conclusions: There is a great danger of misjudging a myofibroblastic sarcoma as an inflammatory myofibroblastic tumor and consequently to delay the urgently needed treatment. Therefore, an overview of the present state of knowledge about diagnosis and treatment of myofibroblastic sarcomas will be given based on this case report.
Myofibrosarcoma of Maxilla - Case Report and Review of Literature

Abstract:Myofibrosarcoma is a distinct mesenchymal malignancy which commonly occurs in head and neck region. It has a high tendency for local recurrence and distant metastasis. 39-year-old male presented with epistaxis, nasal obstruction and left sided complete loss of vision. He underwent functional endoscopic sinus surgery and guided biopsy. MRI scan showed a lesion epicentred in the left maxillary sinus, superiorly extending into the orbit. He underwent Class 4b maxillectomy with neck dissection, tracheostomy and free flap reconstruction. Histopathological examination yielded final diagnosis as myofibrosarcoma of maxilla. The patient was planned for adjuvant radiotherapy and has been disease free for 3 years.

Abstract:Breast cancer is one of the leading causes of cancer globally. Radiotherapy following breast‐conserving surgery is the standard treatment of breast cancer. Recently, hypofractionated irradiation comprising 42.56 Gy in 16 fractions was selected as a viable radiation therapeutic option. Radiation‐induced sarcoma is the most prevalent secondary malignancy in patients undergoing radiotherapy after breast cancer surgery. Angiosarcomas are the predominant type of radiation‐induced sarcomas, whereas liposarcomas have rarely been reported. The present report details an uncommon instance of radiation‐induced pleomorphic liposarcoma that occurred 8 years after breast‐conserving surgery and hypofractionated radiotherapy. The patient visited the hospital due to hardening of the tissue beneath the skin of the right breast. Ultrasonography revealed a hypoechoic mass in the lower part of the right breast containing internal blood flow. An excisional biopsy revealed that the tumor contained infiltrating spindle‐shaped cells without a capsule containing pleomorphic cells. Lipoblasts were also observed and tended to differentiate into adipose tissue, leading to a diagnosis of pleomorphic liposarcoma. Immunostaining revealed negativity for cytokeratin AE1/AE3, ERG, MDM2 and S‐100 protein; the Ki‐67 index was ~20%. An enlargement resection involving a postoperative bed was performed because of close tumor margins. 18 F‐fluorodeoxyglucose positron emission tomography/computed tomography revealed pale accumulation of 18 F‐fluorodeoxyglucose in the right chest wall, which was interpreted as a postoperative change owing to the resection biopsy. The tumor was observed in the irradiated field with no distant metastases. Following extensive resection, the patient maintained a recurrence‐free survival period of 3 years and 2 months, during which no adjuvant therapy was administered. Therefore, follow‐up is necessary in patients with breast cancer treated with radiotherapy.

Abstract:We report the case of a patient with a large postauricular keloid treated with excision and immediate post‐operative radiation who developed a high‐grade pleomorphic sarcoma in the scar bed. To our knowledge, this is the first reported case of high‐grade pleomorphic sarcoma arising in a radiated keloid scar bed in the head and neck. This finding demonstrates that it is important to consider risk of radiation‐induced malignancy when prescribing radiotherapy for the post‐excision treatment of keloids. Keloid scars can cause significant morbidity to the patient including substantial cosmetic disfigurement, particularly in the head and neck region. Surgical excision followed by immediate postoperative radiation therapy has been shown to be more effective than single modality treatment. Radiation therapy increases risk for radiation‐induced malignancy, though very few cases have been reported in the treatment of keloids. We report the case of a patient with a large postauricular keloid treated with excision and immediate post‐operative radiation who developed a high‐grade pleomorphic sarcoma in the scar bed. Secondary malignant fibrous histiocytoma of thigh with surface ulceration following radiotherapy for carcinoma cervix

Abstract:Introduction: Radiotherapy is known to cause rarely various malignancies including leukemia, sarcoma, and thyroid and lung carcinoma. Post-radiation sarcomas account for 0.5-5.5% of all sarcomas; and include extra skeletal osteosarcoma, fibrosarcoma leiomyosarcoma, and malignant fibrous histiocytoma. Reports on post radiotherapy malignant fibrous histiocytoma in cervical cancer patients are rare. Case report: We present a case of 45-years-old female diagnosed with squamous cell carcinoma of the uterine cervix stage IIB; she underwent a radical hysterectomy with pelvic lymphadenectomy and postoperative radiotherapy and chemotherapy. Five years later, she presented with ulcerated nodular masses in right and left medial aspect of thigh. Cytological and histological diagnosis of the masses was malignant fibrous histiocytoma which was further confirmed by immunohistochemistry. Conclusion: Post-irradiation sarcoma must be considered in patients of carcinoma cervix treated with radiotherapy, when soft tissue mass is seen in the previously irradiated area. It should be differentiated from metastasis from primary tumor with the help of appropriate markers as it has a grave prognosis.
Myofibroblastic sarcoma of the breast. Report of a case induced by radiotherapys

Abstract:Myofibroblastic sarcoma (MFS) is an uncommon tumor rarely located in the breast. Ionizing radiation is a carcinogen capable of inducing sarcomas through DNA damage. A 42-year-old woman was diagnosed with synchronous bilateral breast infiltrating ductal carcinoma with axillary lymph node metastases on the left side. After modified left radical mastectomy and simple right mastectomy, she underwent postoperative radiation with a total volume dose of 50 Gy that included the thoracic wall and the left axillary-supraclavicular region. After a latency period of 6 years and 4 months, the patient developed an MFS in the area of radiation (mammary upper outer quadrant). To our knowledge, only 11 cases of MFS have been previously published in the breast. The study of the 12 cases including the present one revealed that the ages of the patients ranged from 42 to 86 years (mean 60.3 years). There was a clear difference concerning sex (M:F, 1:5). The average duration of the lesion varied from 1 week to 8 months (mean 3.3 months). The size ranged from 2.2 to 22 cm (average 5.1 cm). The tumors showed frequent mitosis and areas of necrosis. The percentage of recurrences, metastasis, and death due to the tumor was 27.3%, 36.4%, and 27.3% respectively. MFS cases differ from those affecting extramammary regions. They are more common in females and show a greater degree of aggressiveness. Correct diagnosis of mammary MFS requires morphological and immunohistochemical study. We present for the first time a case of MFS of the breast induced by radiotherapy.

Abstract:Primary pulmonary chondrosarcoma is a rare neoplasm that usually grows slowly, metastasizes late, and responds well to excision when localized. Herein, we present a 59-year-old man who manifested with hemoptysis, cough and dyspnea with a hemithorax mass. A chest computed tomography scan demonstrated a fast-growing mass in the right upper lobe, and the enhancement was partially heterogeneous. Bronchoscopy revealed a hemorrhagic neoplasm in the right upper bronchus. A bronchoscopy biopsy specimen revealed a variety of tissues, including mucoid cartilage, fibers, respiratory epithelium and squamous epithelium. Because malignancy was suspected, the patient underwent a right upper sleeve lobectomy with mediastinal lymphadenectomy. Macroscopically, we observed a firm white mass 9.5 cm in diameter with a central area of necrosis. Histopathology revealed neoplastic chondrocytes with enlarged and hyperchromatic nuclei in the myxoid matrix. The tumors were positive for S-100. The patient was diagnosed with tracheobronchial myxoid chondrosarcoma. Lymph node dissection indicated no metastasis. The tumor grows slowly in the initial symptom-free phase when localized. Then, a symptomatic phase ensues, during which the tumor progresses rapidly. The patient displayed pulmonary and subcutaneous skull metastases eight months after operation and was treated with adriamycin and ifosfamide chemotherapy.

Abstract:Transoral extrusion of a mucosal‐covered soft tissue mass found to be hypopharyngeal liposarcoma. Liposarcomas are the most common soft tissue sarcoma in adults, whereas liposarcomas of the head and neck, particularly the hypopharynx, are incredibly rare ‐ with approximately 50 cases reported in the literature. We present a case of an otherwise healthy and asymptomatic 42‐year‐old male who presented dramatically after vomiting up a large soft tissue mass. The lesion was surgically removed via transoral approach with blue laser, and diagnosis of well‐differentiated liposarcoma was made via MDM2 gene amplification by FISH. Oral extrusion is a rare feature of this disease. This is the first documented case of an orally extruded liposarcoma to present in an otherwise asymptomatic and healthy patient, demonstrating how this entity may be indolent until initial presentation. Primary non-radiation induced angiosarcoma of the parotid with epithelioid morphology

Abstract:Angiosarcomas are rare, accounting for only 1-2% of sarcomas in the head and neck region. We present an extremely rare case of primary non-radiation-induced angiosarcoma of the parotid with epithelioid morphology which required a tertiary center second opinion for its histological characteristics. We discuss the presentation, imaging, diagnosis, characteristics, and pathway of this case. As parotid lumps are common, a surgical sieve is required to rule out other possible conditions. Early correct diagnosis is crucial in conditions such as angiosarcomas due to their quick metastatic potential.

Abstract:Myofibroblastic sarcoma (MS) is a malignant tumor of soft tissue or bone that can occur in children or adults, with a high rate of recurrence and metastasis. We report a case of low-grade malignant MS of the left shoulder, diagnosed based on pathological examination and immunohistochemical staining. However, the patient had unexplained pleural maculopathy. The patient passed away 6 months after the diagnosis of myofibroblast sarcoma due to multiple metastases throughout the sarcoma. Combined with the patient’s history, ancillary findings, and after MDT discussion, the patient was ultimately considered to have a high probability of myofibroblast sarcoma combined with pleural maculopathy. In conclusion, when a patient is diagnosed with myofibroblast sarcoma in combination with pleural macula, in the absence of other causative factors, a deep tissue biopsy of the pleura should be actively performed to confirm the diagnosis.

Abstract:Radiation-induced sarcomas (RIS) are iatrogenic malignancies that arise following high-dose radiotherapy, posing a significant clinical challenge due to their poor prognosis and resistance to conventional treatments. The incidence of RIS is increasing with advancements in radiotherapy techniques. This report presents a case of a 71-year-old male diagnosed with stage III rectal adenocarcinoma treated with neoadjuvant chemoradiotherapy and curative surgery. Three years postoperatively, he developed a low-grade radiation-induced leiomyosarcoma in the perianal region. Histopathological examination confirmed a spindle cell neoplasm with notable immunohistochemical markers. RIS often presents as aggressive high-grade tumors resistant to radiotherapy and chemotherapy, necessitating surgical resection as the primary treatment. This case underscores the importance of long-term surveillance post-radiotherapy and highlights the need for innovative therapeutic strategies, including immunotherapy. Despite being rare, RIS poses a significant risk following cancer treatment, making early detection through vigilant monitoring and advancements in therapeutic approaches crucial for improving patient outcomes.

Abstract:Radiation-induced carcinosarcoma of the submandibular gland: case report and review of literature. Radiation-induced carcinosarcoma (RICS) of the head and neck is a very rare and extremely aggressive entity. We present the case of a 60-year-old man diagnosed with a T2N2cM0 squamous cell carcinoma at the right base of the tongue (BOT). The patient was treated with neoadjuvant chemotherapy and 3D-conformal radiotherapy (3D-CRT) to the BOT and bilateral neck. He developed left submandibular firmness five years after treatment, initially diagnosed as radiation-induced fibrosis. The increasing size of the left submandibular mass two years later prompted further imaging, which showed a 2.2x1.8 cm mass in the left submandibular region in the previously irradiated field. This was diagnosed as a radiation-induced submandibular gland malignancy. This is the first reported case of a RICS in the submandibular gland and demonstrates the importance of early workup and evaluation of submandibular gland pathology in a patient with a history of radiation for a head and neck malignancy.
Significance of radiation therapy in the myxoid round-cell liposarcoma treatment regimen

Abstract:Because myxoid liposarcomas are more radiosensitive than other soft tissue sarcomas, there have been several reports of 50 Gy preoperative radiation therapy combined with surgery, but the wound complication rate is reportedly high. We have performed preoperative irradiation at a reduced dose of 40 Gy and definitive radiation therapy for unresectable cases. This study aimed to report the tumor reduction rate and oncological results with a reduced dose of preoperative irradiation and the outcome of definitive irradiation for unresectable cases.

Abstract:A patient presented with a 5-year history of slowly progressive dysphagia. He had moderately differentiated squamous cell carcinoma in the middle thoracic esophagus and underwent partial esophagogastrostomy 16 years prior. The patient with postoperative anastomotic stenoses was treated with radiotherapy at a total dose of 60 Gy after esophagectomy. Endoscopic submucosal dissection (ESD) was used to treat the recurrent tumor.Clinical specimens were obtained from the ESD and the tumor was pathologically confirmed to be fibrosarcoma.
Atypical carcinoid tumour of the larynx

Abstract:A man in his 80s with long-standing left-sided throat discomfort presented with hypophonia and odynophagia. A flexible nasoendoscopy revealed a supraglottic mass in the left arytenoid. An urgent microlaryngoscopy and biopsy demonstrated a supraglottic laryngeal tumour consistent with atypical carcinoid on histopathological examination. Head and neck multidisciplinary team discussions led to the selection of ablative carbon dioxide laser therapy as primary treatment. Two years later, there was local recurrence, prompting a second laser resection and selective neck dissection. Further laser resection was performed 3 years later for another recurrence. Despite an initial encouraging response with local control, 4 years after the initial treatment, there was substantial progression with multiple cutaneous and subcutaneous metastases and further local recurrence soon after. Palliative radiotherapy to the cutaneous metastases provided partial benefit, transitioning to best supportive care. He died 10 years after the initial onset of symptoms, primarily due to cancer progression.

To date, only a few cases of malignant hypopharyngeal lymphoma have been reported, and its specific characteristics are unknown. Herein, we report a case of right hypopharyngeal diffuse large B-cell lymphoma (DLBCL) in a 74-year-old man with dysphagia. CASE REPORT A 74-year-old man presented to our hospital with dysphagia. He had no relevant medical history. Endoscopic examination revealed a right hypopharyngeal tumor. The surface of the tumor was smooth, with no evidence of hemorrhage. Computed tomography revealed a 40-mm mass located in the hypopharynx. We performed a tracheotomy and biopsy of the tumor. Histopathological examination revealed a diffuse proliferation of large atypical B cells with negative staining for Epstein-Barr virus by in situ hybridization. Immunohistochemical staining was positive for CD20 but negative for CD3 and CD10. The patient was administered chemotherapy. The tumor reduced in size, and the patient recovered completely. During the two-year follow up, no recurrence of cancer was observed. CONCLUSIONS Although most hypopharyngeal tumors are squamous cell carcinomas (SCCs), the possibility of other types of tumors should also be considered. Malignant lymphoma of the hypopharynx is rare, and more cases need to be studied and reported in the future.
