Abstract:Background Epidemiology is considered to be the fundamental science of public health and plays an important role in clinical competence and professional development. The objective of this study is to evaluate the effectiveness of a short-term course for the teaching of epidemiology, which was designed as a community-based class for medical students. Method This course was designed according to Kern’s six-step approach to curriculum development. A total of 75 undergraduates were recruited. Forty-one students were assigned to an experimental group engaged in theoretical teaching and practical courses, while 34 students were assigned to the control group only taking theoretical courses. All participants were asked to complete a pre- and post-course survey and to take a test after completing the course. The scores between the experimental and control groups were compared using the Wilcoxon test. Result The experimental group showed significantly higher self-assessment scores in course understanding ( p = 0.0126) and clinical practice skills ( p = 0.0005) after completing the course, while no significant difference was observed in the control group. In addition, students in the experimental group reported significantly higher interest ( p = 0.0015), stronger learning motivation ( p = 0.0113) and a better mastery of epidemiology ( p = 0.0167) after completing the course than those in the control group. However, test scores ( p = 0.0859) and pass rates ( p = 0.1755) demonstrated no statistical significance between the two groups. Conclusion The short-term practical course in epidemiology exerted significantly positive effects on the improvement of student learning enthusiasm, course understanding and clinical practice skills. These findings provide new ideas and statistical evidence for the development of epidemiological instruction. Future studies should explore how to more widely and optimally apply community-based courses to the teaching of epidemiology.
Gaining insight into the implementation of an e-learning smoking cessation course in Latin American countries

Abstract:Continuous medical education focused on health problems emerging in low- and middle-income countries (LMICs) is scarce. Although tobacco consumption is increasing in LMICs, there is a lack of tobacco cessation training programs in these countries. To promote smoking cessation interventions in Bolivia, Guatemala and Paraguay, we adapted an e-learning program developed in Catalonia (Spain). This process evaluation study reports on reach, dose and satisfaction of participants with the course, as well as the contextual factors of its application. We conducted a multiple method evaluation, which included a survey and several focus groups, each one specific to the same type of healthcare professional (nurses, doctors, other professionals). Two hundred and ninety-two participants registered into the online course. The motivation for undertaking the course was different between doctors and nurses. The main sources of difficulty in enrolling and finishing the course were the technical problems experienced when accessing the platform, and lack of acquaintance with computers and the Internet in general. Our results show that implementing e-learning education in hospitals from LMICs is feasible, especially when there are similarities between participating countries and the country in which the original program was developed. However, several elements such as strong organizational commitment, technical support and resources and adequate communication channels should be provided to facilitate enrollment and training completion. Efforts to improve Internet access should be made to avoid jeopardizing students' motivation to enroll and complete online training.
Pedagogical experience with Public Health campaigns from the design of socio-educational projects with insertion in the local territory

Abstract:Introduction: In today's society, educational management has become an imperative for the creation of knowledge and innovative educational experiences. In the context of the Bachelor's Degree in Nursing at the University of Buenos Aires, a pedagogical experience focused on a public health campaign on Dengue was carried out. The main objective of this experience was to promote the democratization of scientific and health knowledge in relation to Dengue, guaranteeing the right of access to public health information. We sought to create an environment conducive to learning, active participation of students and interaction with the community.Development: The experience was divided into three stages: initial, progressive and territorial anchoring. In the initial stage, students were prepared through theoretical and technical sessions. The progressive stage involved adjustments and corrections in collaboration with the teaching team. In the territorial anchoring stage, groups of students carried out the public health campaign in sectors close to the university. The evaluation was carried out using group monitoring instruments and a checklist to evaluate the individual performance of the students. A review and feedback meeting was also held at the end of the experience.Conclusions: The proposed pedagogical experience demonstrated that effective educational management can promote the democratization of knowledge in the field of public health. Collaboration among students and the focus on the right of access to public health information are key elements. In addition, the importance of constant adaptation of educational practices to scientific and technological advances is highlighted. On the other hand, it provided an effective framework for the training of nursing students and the promotion of public health, demonstrating the importance of educational management in today's society.

New development of cardiac tamponade on underlying effusive–constrictive pericarditis: an uncommon initial presentation of scleroderma

Abstract:A 40-year-old man with a medical history of hypertension was admitted for weight loss, generalised weakness, joint pains and mottling of fingertips. The initial laboratory data revealed microangiopathic haemolytic anaemia, thrombocytopenia and acute renal failure. Intravenous steroids were started for possible diagnosis of systemic lupus erythematosus based on admission assessment. Intravenous immunoglobulin and plasmapharesis were subsequently added to the treatment plan to cover thrombotic thrombocytopenic purpura while his autoimmune panel was pending. The echocardiogram study on day 2 revealed cardiac tamponade for which he underwent pericardiocentesis and right heart catheterisation. The atrial waveforms postpericardiocentesis demonstrated effusive–constrictive pericarditis. His clinical condition kept on deteriorating with reaccumulation of pericardial effusion and further complicated by hemoperitoneum and colonic obstruction. He had cardiorespiratory arrest on his fourth admission day and was not revived. Anti-Scl-70 antibody came back positive. Autopsy findings confirmed the presence of fibrinous pericarditis and hemoperitoneum.
Induction of radio-adaptive response by low-dose X-irradiation on chromosome aberrations in human embryonic fibroblasts.

Abstract:Inonizing radiation induces mutation and chromosome aberrations in the irradiated cells. However, the frequency of chromosome aberrations induced by high-dose X-ray irradiation is diminished in cells exposed to low-dose pre-irradiation. The radio-adaptive response induced by low-dose radiation has been previously reported. These studies usually choose V79 cells or human peripheral lymphocytes as test materials. However, V79 cells are no longer normal diploid cells and in the case of human peripheral lymphocytes, artificial blastoid transformation by a mitogen is necessary for the experiments. In order to observe the radio-adaptive response of normal cells under natural conditions, we elected to study human embryonic fibroblasts as cells in the normal cell cycle under natural conditions. We investigated the dose dependency of the frequency of chromosome aberrations induced by radiation, conditions required to induce the radio-adaptive response, and the relationship between radio-adaptive response and protein production system. As a result, the number of chromosome aberrations significantly increased with high dose irradiation as a function of dose. When a conditioning irradiation of 10 cGy was given to cells 4 hours before the irradiation of 150 cGy, the number of chromosome aberrations significantly decreased. However, the induction of this radio-adaptive response was inhibited by treatment with 1 microgram/ml of cycloheximide during the 2 to 4 hour period after conditioning irradiation. These findings possibly suggest that the radio-adaptive response can be induced by pre-irradiation with about 10 cGy and that X-ray induced protein influences the induction of radio-adaptive response.
Evaluation of a Spanish Health Topics Course for Undergraduate Pre-health Latino Students

Abstract:Introduction: Language concordance between doctors and patients and increased matriculation of underrepresented minorities in medical school are commonly cited goals of medical centers and medical schools in addressing health disparities for individuals with limited English proficiency. Pre-medical education may represent a high-yield untapped opportunity to address both through a Spanish Health Topics, or Temas de Salud, course for Latino pre-health students. Methods: The authors implemented a longitudinal course for pre-health, Latino, undergraduate students to prepare them for success in bilingual health careers. The course included educational components of health knowledge, Spanish terminology, Hispanic/Latino sociocultural context, and exposure to medical learning formats presented during monthly sessions spread out over two years. A post-course survey with comfort and knowledge assessments was administered after each one-year cycle of the program. Results: One hundred and sixteen students (57%) out of 203 course-participants responded to the Spanish Health Topics course survey. The student comfort level and self-perceived knowledge about specific health issues increased for both students of native-level Spanish and less advanced fluency, though a larger improvement was noted in several health topics for native speakers. Eighty-five percent of students reported perceiving the class to be useful for their future careers, and 92% of respondents indicated having applied learned concepts in social and/or academic settings outside of class. Discussion: Most students reported benefits of the course. Future studies should focus on a more detailed evaluation of enrolled students' knowledge, attitudes, confidence, and long-term retention compared to students in a standard premedical path. Temas de Salud may enhance the bilingual, bicultural skillset of Latino underrepresented minorities in medicine, and can be replicated at other institutions.
Continuing Professional Development at Two Rural Hospitals in Ecuador

Abstract:Background: E-learning Continuing Professional Development (CPD) is an activity demonstrated to improve the quality of healthcare delivery. The CPD of medical and nursing staff in high income countries (HICs) is commonplace. CPD of administrative staff is less common, but increasingly frequent. In low- and middle-income countries (LMICs), CPD of any kind is infrequent, particularly in rural and remote areas.Objective: The aim of this study was to describe a hospital-based e-learning CPD program for clinical and non-clinical personnel as a unique example of a successful, ongoing educational pilot, quality improvement program involving a broad cohort of employees, in a country that does not require such activities.Methods: Using the online educational platform Chamilo, e-learning modules were created for eight groups including clinical and non-clinical employees. Upon completion of each module, one to two paragraph discussions were provided for each incorrect answer submitted. Two additional chances were offered for the employee to achieve a passing score of 70%. This study reports on the first 10-month period of the program.Findings: All participants achieved the 70% passing threshold after the first or second attempt. There was 100% participation by the employees required to complete the e-learning modules. Employee feedback suggested the modules were good for continuing education, but some felt the CPD was imposed on them.Conclusion: E-learning CPD is an important and emerging element for CPD and may provide opportunities for healthcare service quality improvement as part of broader pedagogical modalities, such as conferences and directed readings, in rural and remote areas of LMICs. These pilot programs could provide important information to develop Spanish-language e-learning CPD programs across a broader region, promote collaboration with regional professional societies, and possibly contribute to the establishment of national health program CPD standards.

Participation and satisfaction with thee-learning course “Mental health promotion in school health care”: a mixed-method study

Abstract:Purpose This study aims to evaluate the participation and satisfaction of learners with the brief “Mental health promotion in school health care” e-learning course, and to describe factors related to their participation and satisfaction. Design/methodology/approach A convergent mixed-method study with a descriptive, posttest-only design was conducted in Finland. Quantitative data was collected from the learning portal data and via an electronic feedback questionnaire and qualitative data in four focus group discussions. Findings The three modules of the course were opened altogether 12,922 times during the 21 months period. The identified factors influencing participation within the course were: favorite methods attracting attention, the relevance of additional information and postprocessing boosting diligence. The learners’ satisfaction with the course was high. The factors identified for improving satisfaction were: filling gaps in earlier education, clear and concise structure and content, inspiring and interesting design and suitability for clinical use. In addition to the primary target group (school nurses), the course was well-participated and evaluated as satisfying among other health and social care professionals as well as undergraduate students. Originality/value This study demonstrates demand for brief, fully online mental health trainings among school nurses, but also among other health and social care professionals and students. The results indicate that learners perceive such training as beneficial. This paper also presents a novel training intervention and its pedagogical base.

Abstract:Background Massive open online courses have the potential to enable dissemination of essential components of quality improvement learning. Subsequent to conducting the massive open online course ‘Take the Lead on Healthcare Quality Improvement’, this paper is a report of the evaluation of the course’s effectiveness in increasing healthcare professionals’ quality improvement knowledge, attitudes, self-efficacy and systems thinking. Methods Using the Kirkpatrick model for evaluation, a pretest–posttest design was employed to measure quality improvement knowledge, attitude, self-efficacy and systems thinking. Interprofessional learners across the globe enrolled in the 5-week online course that consisted of 10 modules (short theory bursts, assignments and assessments). The objective of the course was to facilitate learners’ completion of a personal or clinical project. Of the 5751 learners enrolled, 1415 completed the demographic survey, and 88 completed all the surveys, assignments and assessments. This paper focuses on the 88 who completed the course. Results There was a significant 14% increase in knowledge, a 3.5% increase in positive attitude, a 3.9% increase in systems thinking and a 21% increase in self-efficacy. Learners were very satisfied with the course (8.9/10). Conclusions Learners who completed the course ‘Take the Lead on Healthcare Quality Improvement’ had significant gains in learner outcomes: quality improvement knowledge, attitude, self-efficacy and systems thinking supporting this course format’s efficacy in improving key components of students’ quality improvement capabilities.

Abstract:Objectives. To evaluate the effectiveness of an online chronic disease self-management program for South Australia residents. Method. Data were collected online at baseline, 6 months, and 12 months. The intervention was an asynchronous 6-week chronic disease self-management program offered online. The authors measured eight health status measures, seven behaviors, and four utilization measures; self-efficacy; and health care satisfaction. Results. Two hundred fifty-four South Australian adults with one or more chronic conditions completed baseline data. One hundred forty-four completed 6 months and 194 completed 1 year. Significant improvements ( p < .05) were found at 6 months for four health status measures, six health behaviors, self-efficacy, and visits to emergency departments. At 12 months, five health status indicators, six health behaviors, self-efficacy, and visits to emergency departments remained significant. Satisfaction with health care trended toward significance. Discussion. The peer-led online program was both acceptable and useful for this population. It appeared to decrease symptoms, improve health behaviors, self-efficacy, and reduce health care utilization up to 1 year. This intervention also has large potential implications for the use of a public health education model for reaching large numbers of people. It demonstrates that an Internet self-management program, which includes social media, can reach rural and underserved people as well as be effective and reduce health care costs. If this intervention can be brought to scale, it has the potential for improving the lives of large numbers of people with chronic illness. It represents a way the medical care and public health sectors can interact.

Effects of Online Health Promotion Program to Improve the Health and Wellbeing of Healthcare Students: A Feasibility Study

Abstract:Background: Healthcare students' health and wellbeing have been seriously affected worldwide. Research studies highlighted the need to establish health promotion strategies to improve them. Methods: A mixed method feasibility with a randomized controlled trial study followed by qualitative focus-group interviews to evaluate the effect of a 24 h online health promotion program improving healthcare students' health and wellbeing was performed in mid-2022. The study also illustrated the program's effect, contents, and activity arrangements. Healthcare students from two large tertiary institutions were randomly assigned to intervention and waitlist-control groups. Outcomes were measured by self-completed online questionnaires at three-time points (baseline, week four, and week eight), and in-depth focus-group interviews followed. Results: Among 70 enrolled and 60 eligible students, 54 completed the study, with a 10% attrition rate. Results demonstrated a significant difference between groups at week eight. Within the intervention group, there were significant differences were found from baseline to week eight for depression (p = 0.001), anxiety (p = 0.004), and stress (p < 0.001). The program also improved certain domains of personal wellbeing and quality of life. Qualitative findings further illustrated the program contents and activities' feasibility, acceptability, and suitability. Most participants welcomed the online mode's flexibility and convenience. They enjoyed diversified and complementary content and activities. They had increased self-awareness of health and wellbeing. Besides, mental health knowledge enables them to 'self-care' and help those in need in the future. Conclusions: The results indicate the feasibility of performing full-scale research in the future and may provide more support for the students of higher education institutions.

Effectiveness of an Interpersonal Influence Intervention to Increase Commitment to Adopt Health-Promoting Behavior in Nursing Students

Abstract:Purpose: To determine the efficacy of the "interpersonal influence intervention" conducted by health professionals to increase the commitment to adopt health-promoting behavior in nursing students compared to the usual care of a university wellness program. Patients and methods: A quasi-experimental study was performed. The study included 114 nursing students from a university in Cali, Colombia, who were divided into a control group (n = 57) that received usual care and an experimental group (n = 57) that received the intervention designed and validated according to Nola Pender's Health Promotion Model. The main outcome was the lifestyle measured before and after the test using the Health-Promoting Lifestyle Profile II Spanish version. The effect of the intervention was carried out through the average comparison, effect size measures were calculated using Cohen's d and analysis of the effect of possible confounding variables on the intervention (ANCOVA). Results: A statistically significant difference was observed between the experimental group and the control group (p = 0.015; 95% CI -0.42, -0.05). The effect size of the intervention was 0.49. The interpersonal influences exhibited by health professionals can increase the commitment to adopt health-promoting behaviors (Hypothesis 1), and the greater the commitment to a specific action plan, the more likely it is that health-promoting behaviors will be maintained over time (Hypothesis 2). Conclusion: The effectiveness of the intervention interpersonal influences exerted by health professionals to increase the commitment to adopt health-promoting behavior is proven. Evidence demonstrates the practical utility of the Health Promotion Model.

Abstract:Background: Graduate training is a high-risk period for worsening mental health. Previous research reported the effectiveness of a classroom-based self-care intervention for graduate students. Purpose: The study evaluated the effectiveness of an online self-care intervention for graduate-level students to prevent worsening mental health. Methods: Participants were 187 students in public health. The intervention consisted of behavior change assignments designed to increase health-promoting behaviors within four domains (nutrition, physical activity, mental health, social support). Students received bonus points for maintaining health behaviors for the duration of the 12-week semester. Outcomes included measures of nutrition, physical activity, depression, anxiety, and perceived stress. The study employed a control group of students not enrolled in the course (n = 29). Abstract:Chile faces the challenge of responding to the complexities in addressing and managing issues arising from the current demographic, epidemiological, social, and cultural profile. The country’s public health policies have followed the guidelines of Alma Ata Primary Health Care (PHC), Renewed PHC, Astana PHC, Universalization of PHC, and have enabled the sustained development of care networks by implementing a model of comprehensive family and community health care, with a focus on promotion, prevention, and anticipatory management of harm within networks. Throughout these processes, in line with the Mission and Social Responsibility of the University and the Global Consensus on the Social Responsibility of Medical Schools, institutions responsible for training health professionals and specialists have played a supportive and facilitating role, with particular emphasis on developing human resources within and through practical experiences and supporting teams in the territorial implementation of health policies. The presented experience aims to share the contributions and lessons learned from an alliance of diverse academic institutions that, through collaboration and the formation of a community of learning and practice, are shaping a process of social innovation, a rhizomatic learning initiative, and a good formative practice: the Chilean University Consortium of Family Health and Medicine from 2004 to 2014. Its development, progress, results and projections are analyzed.
Implementation and lessons learned from 2 online interprofessional faculty development programs for improving educational practice in the health professions in Chile and the United Kingdom from 2018 to 2021

Abstract:This study presents the design, implementation, and lessons learned from 2 fit-for-purpose online interprofessional faculty development programs for educational practice improvement in the health professions in Chile and the United Kingdom from 2018 to 2021. Both programs were designed to enhance teaching and learning practices in an interprofessional environment based on 4 pillars: professional diversity, egalitarianism, blended/online learning, and active learning strategies. A multidisciplinary mix of educators participated, showing similar results. The 3 main lessons learned were that the following factors facilitated an interprofessional environment: a professions-inclusive teaching style, a flexible learning climate, and interprofessional peer work. These lessons may be transferable to other programs seeking to enhance and support interprofessionality. Faculty development initiatives preparing educators for interprofessional practice should be an integral component of health professions education, as delivering these courses within professional silos is no longer justifiable. As the relevance of interprofessional education grows, an effective way of promoting interprofessonal education is to train the trainers in formal interprofessional settings.

Abstract:Background: The training of health students regarding good treatment and humanized care is essential; it is a quality benchmark in health care. The relevance of good treatment and humanized care should be consistent with the treatment students receive from teachers and clinical tutors, modeling humanized cultures. However, research shows the normalization of mistreatment of medical and nursing students. A Humanized Training Unit was implemented in a Chilean nursing school, which included promoting a support Program for students by their teachers between 2020 and 2021. Aim: Explore the Program's impact on the exercise of humanized care from the student's perspective. Material and methods: An intrinsic case study with a qualitative approach, with the objective of 12 semi-structured interviews carried out via Zoom with young people who had been tutored virtually by program teachers for one month and two years during their student period. We performed the data analysis using the constant comparisons method and in compliance with qualitative rigor criteria. Results: The Program positively impacted personal, academic, and professional areas, allowing students to feel cared for by their teachers and recognizing the development of skills transferable to their relationship with users for more humanized care. Conclusions: The Program is replicable to other careers and institutions, requiring time, disposition, and teacher training, which can be a limitation. The benefits evidenced by our study demonstrate the program's usefulness in promoting cultures of more humanized care in educational and health spaces.
Empowering Underserved Communities in Southern Puerto Rico: A Formal Training Program in Community Health Promotion

Abstract:Abstract Community health promotion offers a potential solution to persistent healthcare challenges, with community health workers playing a pivotal role. The Community Training Institute for Health Disparities (CTIHD) implemented a problem-solving curriculum in Community Health Promotion, integrating a competency-based learning model through two courses: Introduction to Community Health Promotion and Design of an Action Plan for the Promotion of Community Health. Each course comprised ten three-hour sessions, featuring pre/post-tests, evaluations, and a cognitive debriefing. Knowledge change was assessed using pre/post-test scores among 27 community leaders from southern Puerto Rico. Cohort 1 and Cohort 2 demonstrated an overall retention rate of 62.6% and 96.7%, respectively. Although differences in knowledge gained between cohorts and courses weren’t statistically significant, a trend toward increased knowledge was noted. Cohort 1 experienced a 22% knowledge increase in Course 1 and a 24% increase in Course 2. Cohort 2 demonstrated a 41% knowledge increase in Course 1 and a 25% increase in Course 2. The CTIHD’s Community Health Promotion Program has made significant strides in elevating awareness and knowledge, marking a positive step toward reducing health disparities and fostering healthier, empowered communities in southern Puerto Rico.

Abstract:Digital health literacy (DHL) is the ability to search, understand and evaluate information from digital media and apply that knowledge to solve health problems. However, currently many citizens have not developed these skills, and this compromises not only the self-management of their health, but the possibility that health services are socially sustainable. The objective of this article was to present the objectives, activities and results of the IC-Health project whose objective was to develop a series of massive open online courses (MOOCs) to improve the DHL skills of European citizens. An exploratory report on DHL’s current evidence was developed. Furthermore, a survey, focus groups and group interviews were conducted to determine DHL levels and the needs of population cohorts (children; adolescents; pregnant and lactating women; the elderly; and people affected by type 1 and type 2 diabetes). A participation strategy with end users was designed through a community of practice for the creation of MOOCs with the seven European countries that participated in the consortium. Thirty-five MOOCs were developed in eight different languages and a descriptive and exploratory assessment of MOOCs was conducted with new participants. This first evaluation indicated that MOOCs can be an effective educational resource for DHL and a facilitator of shared decision-making processes. The process of co-creation of MOOCs, the components, the challenges and the opportunities identified in this European project could be useful for other developers of MOOCs who want to co-create interventions with beneficiaries in similar settings. Further longer-term actions are still needed to improve citizens’ DHL.

Abstract:Introduction In the United States, nearly half of all women will report having at least one type of female sexual dysfunction at any given point in their lives. Female Sexual Dysfunction (FSD) is defined as a disturbance in sexual desire, psychophysiological changes that characterize the sexual response cycle, and marked sexual distress and interpersonal difficulty. Most women feel their healthcare provider should discuss their sexual concerns, yet few report being asked or screened about their sexual health. Women may seek care to address their sexual health concerns because sexual concerns can affect their quality of life. Objective The specific aim of this study was to evaluate the effectiveness of an online educational intervention about female sexual dysfunction on the knowledge and self-efficacy of healthcare providers. Methods The project is a single-arm effectiveness study that used a pre-test/post-test design, aiming to improve self-efficacy of physicians, physician assistants, nurse practitioners, and certified nurse-midwives who provide gynecological care. Participants were recruited via convenience and snowball sampling. The participants provided demographic and pre-test data before viewing the educational intervention and completed the post-test immediately following. Results Forty-three participants completed responses to the entire questionnaire, including pre and post-test. The majority of participants identified as female (91%), Caucasian (51%), nurse practitioners (81%) providing gynecological care for 6-15 years (53%) and live in the South (58%). Following the intervention, participants demonstrated a significant increase in confidence in their ability to communicate about sex with older adults (p < 0.001, d 0.51) and with those who have differing sexuality from their own (p <0.00, d 0.53). There was an increase in confidence in the knowledge to care for patients with religious/spiritual convictions about sexuality (p < 0.006, d 0.40) and sexual problems related to a medical, pharmacological, or surgical treatment (p < 0.009, d 0.38). Participants reported fewer learning needs following the educational module with a moderate to large effect size across different competencies. Conclusions Educating health care providers using an online educational module may lead to more confidence in the healthcare providers' communication with, knowledge of, and attitudes related to female sexual concerns. Healthcare providers can provide the necessary support and care for the women they serve by educating themselves to properly obtain sexual health history screening, physical exams, diagnoses, and treatment options. Online educational modules have enhanced healthcare providers' knowledge on various topics. The results of this study can be useful in creating education for healthcare providers in caring for FSD. Further studies are needed to determine the long-term effect of online education modules on the screening of female sexual dysfunction, attitudes and behaviors, effect on the self-efficacy of healthcare providers.
