Abstract:Epidermoid cyst is commonly known as sebaceous cyst. It is the most commonly encountered cyst of the skin. Epidermoid cyst over the breast is uncommon. Punctum is the hallmark for clinical diagnosis. Local excision with primary closure is the treatment of choice. Biopsy is mandatory in giant cyst to exclude malignancy.

Abstract:Introduction: Epidermoid cyst is a common clinical entity and it can occur anywhere in the body. But its occurrence and huge size in the breast is very rare and more liable to develop complications, including malignant transformation. Presentation of case: We present here an unusual case of a giant epidermoid cyst in the breast, which is about 7cm in greatest dimension. After proper preoperative diagnosis by clinical, imaging and histopathological findings, it was managed by total excision. Discussion: Imaging and fine needle aspiration cytology is essential for accurate preoperative diagnosis. However, it is often very difficult to differentiate it from other benign and malignant conditions of breast. Infection and malignant transformation are its potential complications. Total excision along with its capsule is the treatment of an epidermoid cyst. Conclusion: Epidermoid cyst is an important differential diagnosis while managing benign breast disease.

Abstract:The epidermal cysts are benign cysts most commonly found in the head and neck region and trunk. Epidermal cysts in breast are very rare with approximately 50 cases described in literature. It is important to diagnose these correctly as they may be misdiagnosed as benign or malignant mammary gland tumor both clinically and radiologically. We hereby report a rare case of epidermal cyst of breast clinically mistaken for a fibroadenoma in a 35 year old female, which was diagnosed on fine needle aspiration cytology as epidermal cyst and later confirmed on histopathology. This case is being reported to create awareness among clinicians as well as pathologists of this unusual entity and to reemphasize the importance of FNAC and core biopsy in diagnosis of breast lumps.

Abstract:Epidermal cyst [10] and panniculitis are relatively uncommon conditions [2]. A combination of these arising in the breast is extremely rare. We report a case of epidermal cyst with panniculitis in the breast. A patient presented with a large breast lump of 5 cm in the right breast. Sonography showed a well-defined, central hyperechoic mass. A giant fibroadenoma was suspected clinically and based on imaging. Fine needle aspiration revealed a benign breast lump. The lump was excised, and the specimen revealed an epidermal cyst with panniculitis.

Abstract:An epidermal inclusion cyst can be seen at any location. Epidermal cysts are commonly found on the scalp, face, trunk, neck, and extremities. They are rarely seen in the breast parenchyma. These benign lesions are important in that they may undergo neoplastic differentiation, although very rarely. Epidermoid cysts usually develop as a result of the implantation of superficial epidermal tissue into the dermis or subcutaneous tissue after trauma or surgical procedures. In this study, a 37-year-old female patient who underwent a histopathological examination that showed a 10-cm epidermal cyst without a history of trauma or a surgical procedure was discussed.

Abstract:Epidermal inclusion cysts are formed by inclusion of keratinizing squamous epithelium within the dermis, resulting in a cyst filled with lamellated keratin. These benign cysts are usually very small and intradermal subcutaneous lesions. They can occur anywhere in the body although they are more common on the face, trunk, neck, extremities and scalp. Abstract:Sebaceous cysts are a prevalent form of benign cutaneous cysts that often manifest as epidermal cysts when their size exceeds 5 cm in diameter. Despite their infrequent occurrence, cases of massive epidermal cysts measuring 5 cm or larger have been documented. Malignant transformations within epidermal cysts are exceedingly rare but not impossible. Although their development into malignancies is a rare event, it is essential to acknowledge the potential for benign epidermal cysts to undergo such transformation. In clinical terminology, these cysts predominantly affect the facial, cervical, and dorsal regions. They usually remain asymptomatic, with their formation attributed to the sequestration of epithelial remnants during embryonic fusion or trauma. In this report, we present a remarkable case of a colossal epidermal cyst found on the upper back of a 75-year-old male individual, highlighting the uncommon nature of such occurrences within the medical domain. This case adds to the literature by presenting a rare case report highlighting the clinical presentation, probable consequences, and surgical therapy of larger epidermal cysts.

Abstract:BACKGROUND:Dermoid cyst is a rare benign tumor exhibiting a typical radiological pattern and most commonly located along the midline. Laboratory examination was always normal. However, the features of some rare cases are atypical that can be easily misdiagnosed as other tumors.OBSERVATIONS:A 58-year-old patient presented with tinnitus, dizziness, blurred vision, and gait unsteadiness. Laboratory examination showed the serum levels of carbohydrate antigen 19-9 (CA19-9) were significantly increased (186 U/mL). A computed tomography (CT) scan revealed a predominant hypodense lesion in the left frontotemporal region with a hyperdense mural nodule. The lesion appeared as an intracranial extradural mass with a mural nodule on the sagittal image, displaying mixed signal on T1- and T2-weighted imaging. A left frontotemporal craniotomy was performed for cyst resection. Histological results confirmed a diagnosis of dermoid cyst. No tumor recurrences were observed at the 9-month follow-up.LESSONS:Extradural dermoid cyst with a mural nodule is extremely rare. When a hypodense lesion on CT shows mixed signal on T1- and T2-weighted imaging with a mural nodule, even if it is located in the extradural areas, it is important to consider a dermoid cyst. Serum CA19-9 combined with atypical imaging features may contribute to the diagnosis of dermoid cysts. Only recognition of atypical radiological features can avoid misdiagnosis.

Abstract:An epidermoid cyst is a common benign skin lesion that can occur in any part of the body. However, epidermoid cysts occurring in the breast are extremely rare, rarely reported, and more prone to complications such as infection, rupture, and malignant alteration. Here, we report a case of a 50-year-old woman with a mammary epidermoid cyst. The patient noticed a mass of the left nipple during bathing. A puncture test was performed considering the epidermoid cyst. Moreover, local extended resection of the nipple and areola was performed. Postoperative pathological findings confirmed the diagnosis of a mammary epidermoid cyst. After 2 years of postoperative follow-up, the patient had no surgery-related complications and no signs of tumor recurrence.

Abstract:Epidermal cysts are common cysts located cutaneously or subcutaneously in the head, neck, and trunk. However, deep epidermal cysts of the breast are very rare, and are frequently associated with traumatic implantation. We present the case of a 62-year-old woman with a palpable mass in the right breast. The patient was evaluated using mammography, ultrasound, and MRI, which uniquely characterized the mass and revealed a second mass. Histological analysis revealed fragments of an epidermoid cyst. The origin of the cysts and location deep within the breast tissue likely were due to a previous bilateral-reduction mammoplasty.

Abstract:Background: Epidermal cyst of the breast is a rare benign intramammary lesion; however, a common finding in other parts of the body and most often located in the scalp, back, and neck. Only a few cases of epidermal cysts of the breast have been reported in literature. Case report: The patient presented with a small movable left breast lump. Mammography and ultrasonography did not show features of a benign lesion. Ultrasound-guided core biopsy was undertaken. Histology showed an epidermal cyst. The patient returned with an inflammation at the puncture site and a small abscess seen on ultrasound. The abscess was removed surgically. Conclusion: Mammographic and sonographic features of an epidermal cyst may mimick a malignant lesion. Biopsy can result in complications, such as inflammation. In addition, an association between epidermal cyst and squamous carcinoma has been reported. Therefore, it is recommended that these lesions are resected.

Abstract:Epidermal cysts are the most commonly occurring cysts in the subcutaneous plane and are usually asymptomatic, small, and slow growing. When the size of an epidermal cyst is greater than 5 cm, it is considered a giant epidermal cyst. Common etiologies include sun-damaged skin and acne vulgaris, and they can appear anywhere in the body, but preferably over the face, neck, and trunk. Unusual sites include the breast, penis, spleen, bones, subungual region, palms, soles, and buttocks. In this report, we presented the case of a 31-year-old female having a large, painless, gradually progressing swelling for two years in the left gluteal region, which was insidious in onset and slow-growing. The patient eventually described a discomfort that made it impossible for her to sit for long hours or sleep in a supine position. Clinical examination revealed circumscribed mass over the left gluteal region from which diagnosis of giant lipoma was confirmed, but due to its large size and involvement of the whole left buttock, we considered it important to perform an ultrasound examination to reinforce our diagnosis, which revealed a large cystic mass in the left gluteal subcutaneous plane that was excised. As a definitive management, surgery was performed with excision of the swelling, which was extracted in toto and was identified as a cyst, and on histopathological examination, the cyst wall was lined by stratified squamous epithelium. Hence, this case report highlights a rare case of a giant epidermal cyst in the gluteal region.
Epidermal inclusion cyst of the breast: a rare benign entity

Abstract:Epidermal inclusion cyst (EIC) arising from the breast is an uncommon benign condition. We report two cases of enlarging EIC of the breast in two women in their forties. The diagnosis of this condition may not be straightforward with imaging alone if an EIC presents as an enlarging lump in the breast and mimics a benign breast lesion, most commonly a fibroadenoma or malignant lesion with benign imaging features. Excision is usually recommended for a definite histopathological diagnosis and for the prevention of potential risks of malignant transformation. Asymptomatic stable lesions do not require treatment; biopsy is unnecessary, and follow-up imaging suffices if typical sonographic and clinical findings are found.

Abstract:Male patients presenting to the mammography department for workup of breast disease is an overall rare event compared to females. Gynecomastia is statistically the most common breast condition afflicting male patients, with ductal carcinoma an uncommon secondary diagnostic consideration. Secondary benign breast conditions or mimickers of breast disease in men are even rarer than primary carcinoma and can pose a significant challenge in breast imaging workups. We present a challenging workup of a superficial epidermoid cyst (EC) in a male patient with significant subcutaneous growth and indeterminate imaging characteristics, mimicking a breast neoplasm. Furthermore, we present undesirable consequences of performing a biopsy of an EC including rupture and abscess formation.

Abstract:A 22 years old male presented with a mass over the penis since birth, which was painless and gradually increasing in size. The mass was excised and it was found to be penile epidermal cyst. Epidermal cyst is one of the most common benign tumors occurring in the body, which commonly occurs either congenitally or following trauma or surgery, where the epidermal elements get trapped within closed space. But here this case had no trauma and surgeries in the past and the mass present since birth-a case of congenital penile epidermal cyst.

Abstract:BACKGROUND Gigantic epidermal cysts (GECs) are rare benign skin appendicular tumours also known as keratinocysts. GECs have a high incidence and their wall is made up of epidermis. Epidermal cysts can occur in any part of the skin; clinical manifestations include skin colour hemispherical swelling; cystic; mobile; 0.5 cm to several centimetres in diameter; and slow growth. CASE SUMMARY Herein, we report a case involving a 56-year-old female with a GEC in the occipitalia. On July 25, 2023, a patient with a GEC was admitted to the neurosurgery Department of the Second Affiliated Hospital of Xi'an Medical University. The phyma was shown to be a solid mass during the operation and was confirmed to be a GEC based on pathological examination. CONCLUSION Epidermal cysts are common cystic nodules on the surface of the body, the aetiology is unclear, the clinical manifestations can vary, and the misdiagnosis rate is high. However, giant epidermal cysts are rare. In most cases, however, the prognosis is satisfactory. This paper analyses and summarizes the population, location, clinical and pathological characteristics and pathogenesis of the disease to strengthen the understanding of this disease and improve the accuracy of clinical diagnosis.

Abstract:Male breast lesions are relatively less common. The most encountered malignant lesion in the male breast is ductal adenocarcinoma; and benign lesions are gynecomastia, fibrocystic disease, intramammary lymph node, fibroadenoma, lipoma and epidermal inclusion cyst (EIC), respectively [5,6]. To date, there had been published only a few cases of EIC of the male breast in literature [3,5,6]. In this case, we aimed to present a new case of EIC with its clinical, radiological and pathological characteristics in the male breast. It had benign sonographic and magnetic resonance imaging findings but had also malignant imaging findings with diffusion restriction on diffusion-weighted imaging.

Abstract:BACKGROUND: Epidermal inclusion cyst in the breast (EICB) is very rare. It presents as breast lump mainly in peri-areolar region and needs to be differentiated from other breast lesions with predominance of squamous cells.AIM: To analyze the role of fine needle aspiration cytology (FNAC) in patients presenting with breast lump.MATERIALS AND METHODS: The present study is a retrospective study over the period of past 2 years, where the patients presented with firm breast lumps. Six of these cases were diagnosed as EICB on FNAC. Smears from these cases were reviewed for cytomorphological findings and correlated with clinico-radiological details.RESULTS: Patients ranged in age from 25 to 60 years, and female to male sex ratio was 2:1 (female 4, male 2). Mammography was done in women (4), which showed solid, well-defined mass lesions in peri-areolar region and were suggestive of benign breast lesions (3) and infective lesion (1). FNA was performed; Giemsa-stained smears showed predominantly mature benign squamous cell and anucleated squames in all cases, thus one of the cytological diagnoses was EICB. Five of these cases had histological confirmation.CONCLUSION: EIC of breast is not that rare lesion as considered. The patients may not seek medical attention because of small painless swelling; unless the lump increases in size or becomes painful. FNAC is confirmatory in the presence of a typical pultaceous aspirate and cytomorphological features of EIC. Thus, FNAC plays a crucial role in its diagnosis and management. Symptomatic cases should be readily excised and need histological correlation to rule out any potential complications that can arise in these cysts.

Abstract:Epidermal inclusion cyst (EIC) is a benign lesion, which occurs due to proliferation and implantation of epidermal elements within a circumscribed space in dermis. They usually occur in the head and neck region, trunk and extremities. EIC occurring in the breast is very rare. In this study, we are presenting a rare case of EIC occurring in the breast; which was diagnosed by fine-needle aspiration cytology (FNAC). Later on, histopathology confirmed the diagnosis. We are presenting this case not only because of its rare site of presentation, but also to highlight the role of FNAC in diagnosing this benign lesion.

Abstract:Introduction: Epidermoid cyst is very common benign lesion of the skin. And may appear anywhere on the surface of the body. Diagnosis is made in most cases by clinical examination, but histological evaluation may be needed in unusual cases. Presentation of case: We report a rare case of a 44 year-old woman who presented with a rapidly growing inflamed mass localized to the left areola, which grew to 4 cm in diameter within a few months. The diagnosis of malignancy was ruled out following pathology examination consistent with epidermoid cyst. After a course of antibiotics, she underwent surgery with nipple sparing and reconstruction of the areola. Discussion: Preoperative diagnosis based on biopsy enable limited surgical excision and preservation of the nipple. Reconstruction of the areola by local areolar flap enable good aesthetic result. Conclusion: It is important to be aware of this entity of epidermoid cyst mimicking breast cancer, and the treatment options available in these cases.
