Abstract:To emphasize the causes, clinical features, diagnosis, management, and prognosis of hyperthyroidism in newborns, 3 cases of neonatal hyperthyroidism were analyzed. The cases were all low birth weight babies, and 2 of them were premature, the other was a term infant. Their mothers' histories were all positive for Graves' disease. In all cases, the infants exhibited tachycardia on admission. Case 1 presented mainly as splenohepatomegalia, case 2 presented mainly as heart failure and pneumonedema, whereas case 3 presented as hepatitis syndrome. The test of thyroid function and history of the mother confirmed the diagnosis of hyperthyroidism. Antithyroid drug therapy was administrated for all 3 cases, 2 with propylthiouracil and 1 with tapazole. In cases 1 and 2, thyroid function recovered after 4 to 5 weeks of treatment, and persisted for the following 3 months. Case 3 recovered after 3 months of therapy, which might be associated with a delay of diagnosis and severe symptoms. Neonatal hyperthyroidism is a rare disorder and usually associated with maternal Graves' disease. Although most neonatal hyperthyroidism is temporary, it may affect many organ systems and cause developmental delay in infants. Early diagnosis and prompt treatment consisting of antithyroid drugs in conjunction with iodine, beta-receptor blocking agents, and at times glucocorticoids and digoxin, are the key elements in management of this disease.
Prevalence and course of thyroid dysfunction in neonates at high risk of Graves' disease or with non-autoimmune hyperthyroidism

Abstract:Objective: Neonatal hyperthyroidism may be caused by a permanent non-autoimmune genetic disorder or, more frequently, by maternally transmitted high serum TRAb levels. Variable thyroid dysfunction may be observed in this second context. We aimed to evaluate the prevalence of neonatal non-autoimmune hyperthyroidism and of the different types of thyroid function in neonates with a high risk of hyperthyroidism due to maternal Graves' disease (GD). Design and methods: This observational cohort study included all neonates identified in the database of a single academic pediatric care center, over a period of 13 years, as having non-autoimmune hyperthyroidism or an autoimmune disorder with high TRAb levels (above 6 IU/L) transmitted by their mothers. Patients were classified as having neonatal hyperthyroidism, hypothyroidism, or euthyroidism with a permanent or transient disorder. Results: Two of the 34 consecutive neonates selected (6%) had permanent non-autoimmune hyperthyroidism due to germline (n = 1) or somatic (n = 1) mutations of the TSH receptor gene. The patients with high serum TRAb levels at birth had transient hyperthyroidism (n = 23), hypothyroidism (primary n = 2, central n = 3) or persistent euthyroidism (n = 4). Conclusion: These original findings highlight the need for careful and appropriate monitoring of thyroid function in the long term, not only for the rare patients with non-autoimmune neonatal hyperthyroidism, but also for repeat monitoring during the first month of life in neonates with maternally transmitted high TRAb levels, to ensure the early identification of thyrotoxicosis in more than two thirds of cases and to detect primary or central hypothyroidism, thereby potentially decreasing associated morbidity.
Neurodevelopmental Follow-Up of Children Born to Mothers with Graves' Disease and Neonatal Hyperthyroidism

Abstract:Introduction: Neonatal hyperthyroidism, often caused by maternal Graves' disease (GD), carries potential neurodevelopmental risks for children. Excessive thyroid hormones during fetal development are linked to neurological issues like ADHD and epilepsy. However, the impact of transient neonatal hyperthyroidism is not well understood. Methods: In a retrospective study at the Royal Children's Hospital in Melbourne, 21 neonates with hyperthyroidism from mothers with GD were examined. Of these, the parents of 10 children consented to participate; thus, questionnaires assessing executive functions, behavior, and social communication were completed. The outcomes were compared to those of control subjects recruited from the community using standardized tools (BRIEF, SDQ, SCQ). The results were analyzed against socio-demographic factors, maternal, and neonatal health. Results: No significant demographic or clinical differences were found between study participants (n = 10) and non-participants (n = 11). Participants, compared to controls, showed similar family demographics but a higher proportion of control parents had university-level education (p = 0.003). Patients displayed more social (SCQ scores: 12.1 ± 2.5 vs. 6 ± 1.07, p = 0.008) and behavioral difficulties (SDQ scores: 10.2 ± 2.17 vs. 6.14 ± 1.03, p = 0.03), with increased executive function challenges (BRIEF scores indicating problem-solving and self-regulation difficulties). Significant effects of family living situation and partner education level on neurodevelopmental measures were noted, underscoring the influence of socio-demographic factors. Conclusions: These findings suggest neonatal hyperthyroidism might lead to subtle neurodevelopmental variations, with socio-economic elements and family dynamics possibly intensifying these effects. While most children did not show severe impairments, early detection and intervention are recommended. The research emphasizes the necessity for inclusive care approaches that consider socio-economic factors for children affected by neonatal hyperthyroidism.
Different outcomes of neonatal thyroid function after Graves' disease in pregnancy: patient reports and literature review

Abstract:Graves' disease in pregnancy is a rare condition that directly affects neonatal thyroid function. We describe three newborns born to mothers with Graves' disease and discuss differences in outcomes and management. The first infant presented with a goiter at birth but was euthyroid and did not require therapy. The second infant presented with thyroid storm and the third infant present with neonatal hyperthyroidism, and both required treatment with antithyroid drugs. There was documented elevation of maternal and infant thyroid stimulating hormone immunoglobulin (TSI) levels in all three infants. Management of an infant born to a mother with Graves' disease should include monitoring of both maternal and neonatal thyroid function, and maternal TSI levels during pregnancy. Treatment may be needed if the newborn is symptomatic. With clearance of maternal antibodies and antithyroid drugs, manifestations of abnormal thyroid function in the neonate gradually regress, including eventual resolution of a goiter, if initially present.

Abstract:Background and Objectives: Graves disease complicates 0.2% of pregnancies. Maternal thyrotoxicosis increases morbidity in both fetus and mother. The main objective of this study was to evaluate the clinical outcomes in neonates born to mother with Graves disease at birth and first postnatal month and correlate maternal and neonatal Thyrotropin receptor antibody (TRAb) levels and outcomes. This is the first study in India studying the neonatal outcomes in large cohort of pregnant women with Graves disease. Methods: The study was conducted in a 3000 bedded tertiary care centre in South Tamilnadu over a period of two years. 46 pregnant women with Graves disease who were TRAb positive at third trimester were evaluated with serial thyroid function tests (TFTs). Neonates born to these mothers were evaluated for signs of hyperthyroidism, hypothyroidism, and adverse effect of antithyroid drugs. TFTs and TRAb levels were measured at day 3 of postnatal life and after one month. TFTS and TRAb levels were measured using electro chemiluminescenceimmunoassay (ECLIA) by Roche cobas e411. Results: Out of 46 TRAb positive pregnant women 22 (47%) had TRAb levels more than 3 times in the 3rd trimester and all neonates born to these mothers were TRAb positive (100%). Five women had very high TRAb levels (>10 times), among which 4 (80%) presented with thyroid storm andwereassociatedadverse pregnancy outcomes, 3 had intrauterine death, 1 had abortionand1 with still birth. Of the 41 (89%) women who delivered, 6 were preterm delivery (14%). Out of 41 neonates, 19 (46%) were TRAb positive at day3 of life. TRAb levels normalized by 3-6 months (Mean -150 days) in all neonates. One fetus hadfetal thyrotoxicosis diagnosed by goitre and fetal tachycardiain antenatal ultrasound which improved with treatment in the mother. Out of 19 Trab positive neonates, 4 neonates (21%) had transient neonatal thyrotoxicosis. However, all were asymptomatic, did not require treatment and spontaneously became euthyroid with negative TRAbtitres by3-6 months.5 neonates (26%) had transient hypothyroidismpossibly because of TRAb induced blocking antibodies requiring thyroxine therapyfor 6 months. Thyroxine was stopped onceTRAbtitres became negative. 2 neonates (10%) had transient central hypothyroidism because of transient suppression of pituitary axis by high maternal thyroid hormone levels requiring thyroxine therapyand the axis recovered by 6 months. 1 neonate developed bilateral cleft lip and palate probably due to inadvertent use of high dose of antithyroid drugs in I trimester. Also 5 neonates developed transient drug induced hypothyroidism which resolved by 15 days of life. Conclusion: More than 3 times the TRAb levels in mother wastransmitted to the infants, which was transient and spontaneously resolved by 3-6 months. Few TRAb positive infants (57%) was associated with transient neonatal thyrotoxicosis, transient neonatal hypothyroidism and transient central hypothyroisim which may require followup and close observation. Uncontrolled hyperthyroidism with high TRAb (>10 times)) were associated with poor pregnancy outcomes and all had fetal demise. Thus TRAb levels in mother may predict pregnancy outcomes and in newborn help in vigilant interpretation of abnormal TFTs and helpto avoid inadvertent Thyroxine/ antithyroid drugs.
Neonatal Thyrotoxicosis in Infants of Mothers with Graves' Disease Treated for Radioiodine-Induced Hypothyroidism: A Literature Review

Abstract:Fetal and neonatal thyrotoxicosis occurs in up to 5% of pregnancies in mothers with Graves' disease (GD). This condition is caused by stimulating antibodies against the thyrotropin receptor (TRAbs) that cross the placenta and may stimulate the fetal thyroid, typically in the second half of pregnancy. GD is often treated with radioiodine, resulting in hypothyroidism in most patients, but TRAbs can persist for several years. Even if a pregnant mother is hypothyroid after radioiodine therapy or surgery, her TRAbs can still, although rarely, induce fetal hyperthyroidism. In this review, we first present two cases of neonatal hyperthyroidism in mothers with GD who became hypothyroid after prior radioiodine therapy, identified through a 10-year analysis of the National Perinatal System in Slovenia. Based on these cases, we provide an overview of existing data on this rare clinical condition in neonates. We also discuss the underlying mechanisms and clinical outcomes based on currently available data. In conclusion, our review highlights the importance of careful monitoring during pregnancy in all women with GD, even in those well managed after radioiodine therapy or surgery.
Topical Localization in Mouse of Radioactive Silver Oxide [(Ag111)2O] Introduced by Various Routes.∗

Abstract:Summary and Conclusions (1) Radioactive silver (Ag111) was injected in the form of the insoluble oxide into subcutaneous tissue and serous cavities of normal and tumor bearing (Sarcoma 180, Carcinoma C3H/BA, Sarcoma McGhee, Carcinoma EO771) mice, in doses of 0.1 or 0.2 millicurie. Twenty-four hours later the animals were sacrificed; in each series some mice were used for autoradiography and others for testing radioactivity of their tissues and organs with Geiger counter. (2) Subcutaneously injected (Ag111)2O is stored in normal mice mainly in the injected area and in smaller proportion by liver and spleen; in subcutaneous tumor bearing mice, the major portion of silver oxide injected in vicinity of the malignant growth is taken up by malignant tissue. (3) (Ag111)2O injected into the normal pleural cavity is stored mainly by lungs and liver, and in tumor bearing cavity—by malignant exudate and implants. Similar difference in distribution was recorded for normal and tumor bearing peritoneal cavities. (4) It was concluded that radioactive (Ag111)2O deposited subcutaneously or inside serous cavities is drained by tissue or exudate fluid into adjacent malignant tissue where it is stored by intercellular fluid and by macrophagic elements.

Abstract:Abstract Background Elevated serum thyrotropin receptor antibody (TRAb) levels often return to baseline one year post RAI therapy but can sometimes take a few years to normalize. Remission rates of TRAb autoimmunity in GD is higher in patients taking antithyroid medications and who have undergone a thyroidectomy compared to RAI(1). The incidence of neonatal hyperthyroidism in mothers treated with RAI is estimated to be 3.6% 18-24 months post RAI.(2) Clinical case A 31-year-old female G4A1L3 diagnosed with GD at 24 years old treated with RAI at age 25 and remained on L-thyroxine replacement for hypothyroidism. First pregnancy (P1) age 24: terminated at 11wks gestation. P2 (age 28): Spontaneous vaginal delivery (SVD) induced at 38+4wks due to vaginal bleeding, birth weight (BW) 3750g, APGARS 9/9. Baby had NG and was admitted to NICU. On day 3, her heart rate (HR) was greater than140bpmin and she has lost 9.5% weight below her BW. Blood work showed TSH <0.01mIU/L (N: 0.71-31.5), FreeT4 >64.0pmol/L (N: 13.5-41.3), FreeT3 40.4pmol/L (no neonatal reference range), TRAb 206U/L (<10). Methimazole (MMZ) (0.5mg/kg/day) and propranolol (1mg/kg/day) were initiated. Therapy was discontinued at 5 months postpartum. Maternal TRAb was 405U/L (N <10) in first trimester (T1) and dropped to 248U/L in third trimester (T3). P3 (age 31): C-section at 36+2wks, dichorionic diamniotic twin pregnancy, serial US showed good growth and normal dopplers. Both twins developed NG and required NICU admission. Baby A, 3100g, APGARS 7/8/8, had TSH <0.02mIU/L(N: 0.71-3,1.5), FT4 at ∼54h of life of >90pmol/L(N: 12.4-72.4), FT3 23.7pmol/L, TRAb 25.9IU/L(N: <=1.8), HR 162bpmin. Started on MMZ 0.75 mg BID. TSH normalized 3 months postpartum 1.30 mIU/L, FT4 11.4pmol/L. Baby B, 2850g, APGARS 9/9, had jaundice with elevated bilirubin 182umol/L(N: 10-180). HR 130-150bpmin, TSH <0.02mIU/L(N: 0.71-3,1.5), FT4 >90 pmol/L(N: 12.4-72.4), FT3 19.5pmol/l. TRAB 21.9IU/L(N<=1.8). Recovered after 3 months of therapy with MMZ. Maternal TRAb was 60.5 IU/L in second trimester (T2) and 43 IU/L in T3. Conclusion Women with GD who were previously treated with RAI need to be closely monitored for TRAb activity throughout pregnancy. When TRAb Ab levels remain elevated in the T3, close monitoring and assessment of the fetus is critical. Obstetricians and neonatologists need to be involved in the management of these women. Abstract:Fetal and neonatal dysfunctions include rare serious disorders involving abnormal thyroid function during the second half of gestation, which may persist throughout life, as for most congenital thyroid disorders, or be transient, resolving in the first few weeks of life, as in autoimmune hyperthyroidism or hypothyroidism and some cases of congenital hypothyroidism (CH) with the thyroid gland in situ. Primary CH is diagnosed by neonatal screening, which has been implemented for 40 years in developed countries and should be introduced worldwide, as early treatment prevents irreversible neurodevelopmental delay. Central CH is a rarer entity occurring mostly in association with multiple pituitary hormone deficiencies. Other rare disorders impair the action of thyroid hormones. Neonatal Graves' disease (GD) results from the passage of thyrotropin receptor antibodies (TRAbs) across the placenta, from mother to fetus. It may affect the fetuses and neonates of mothers with a history of current or past GD, but hyperthyroidism develops only in those with high levels of stimulatory TRAb activity. The presence of antibodies predominantly blocking thyroid-stimulating hormone receptors may result in transient hypothyroidism, possibly followed by neonatal hyperthyroidism, depending on the balance between the antibodies present. Antithyroid drugs taken by the mother cross the placenta, treating potential fetal hyperthyroidism, but they may also cause transient fetal and neonatal hypothyroidism. Early diagnosis and treatment are key to optimizing the child's prognosis. This review focuses on the diagnosis and management of these patients during the fetal and neonatal periods. It includes the description of a case of fetal and neonatal autoimmune hyperthyroidism.
Outcomes in Maternal Graves' Disease: A Population-Based Mother-Infant Dyad Cohort Study

Abstract:Background: Graves' disease has been associated with adverse pregnancy, labor and delivery, and neonatal outcomes. Thyroid function levels, assessed during newborn screening (NBS), can serve as indicators of the adaptation in the hypothalamic-pituitary-thyroid axis. We utilized data from the national thyroid NBS program to investigate the characteristics of the mother-infant dyad of term infants born to mothers with past or active Graves' disease. Methods: The dataset of the Israeli NBS for thyroid function was linked with the electronic records of a tertiary medical center to generate a unified database of mothers and their term infants born between 2011 and 2021. The MDClone big data platform extracted maternal, pregnancy, disease course, labor and delivery, and neonatal characteristics of the mother-infant dyads. Results: Out of 103,899 registered mother-infant dyads, 292 (0.3%) mothers had past or active Graves' disease. A forward multivariate linear regression demonstrated that Graves' disease did not significantly affect NBS total thyroxine (tT4) levels (p = 0.252). NBS tT4 levels in infants born to mothers with active Graves' disease were higher than those observed in the general Israeli population (p < 0.001). Mothers with Graves' disease more frequently used assisted reproductive technology (12.7% vs. 9.0%, respectively, p = 0.012; odds ratio [OR] = 1.46 [CI 1.03-2.07], p = 0.031), and had more gestational hypertension (3.9% vs. 1.1%, p < 0.001; OR = 3.53 [CI 1.92-6.47], p < 0.001), proteinuria (2.5% vs. 0.9%, p < 0.001; OR = 3.03 [CI 1.43-6.45], p = 0.004), cesarean sections (26.4% vs. 19.7%, p = 0.029; OR = 1.46 [CI 1.13-1.90], p = 0.004), prelabor rupture of membranes (15.4% vs. 4.1%, p < 0.001; OR = 4.3 [CI 3.13-5.91], p < 0.001), and placental abnormalities (5.1% vs. 2.0%, p < 0.001; OR = 2.64 [CI 1.57-4.44]; p < 0.001). Their infants had lower adjusted birthweight z-scores (-0.18 ± 0.94 vs. -0.03 ± 0.90, p = 0.007) and were more likely to be small for gestational age (12.0% vs. 8.1%, p = 0.005; OR = 1.54 [CI 1.08-2.19], p = 0.018). Conclusions: Neonatal thyroid function levels were affected by maternal Graves' disease only when the disease was active during gestation. Moreover, maternal Graves' disease was also associated with an increased risk of adverse outcomes for the mother-infant dyad.
Different Forms of Hypothyroidism in Infants with Maternal Graves' Disease: A Case Series

Abstract:Infants of mothers with Graves' disease (GD) may develop central hypothyroidism (CH) due to exposure of the foetal hypothalamic-pituitary-thyroid axis to higher-than-normal thyroid hormone concentrations, primary hypothyroidism (PH) due to transplacental passage of maternal thyroid stimulating hormone receptor antibody (TRAb), antithyroid drugs (ATD) or thyroid dysgenesis secondary to maternal uncontrolled hyperthyroidism. We describe two infants with PH and four infants with CH born to mothers with poorly controlled Graves' disease. All infants required levothyroxine and had normal developmental milestones. While national guideline consensus for high thyroid stimulating hormone (TSH) on neonatal screening is well-established, thyroid function tests (TFTs) should be serially monitored in infants with low TSH on screening, as not all mothers with Graves' disease are diagnosed antenatally.
Thyroid Hormones and Carnitine in the Second Trimester Negatively Affect Neonate Birth Weight: A Prospective Cohort Study.

Abstract:BackgroundMaternal thyroid hormones and carnitine are reported to affect neonate birth weight during the second trimester, which is one of the most important markers for fetal growth and perinatal mortality and morbidity. Nevertheless, the effect of thyroid hormone and carnitine in the second trimester on birth weight has yet to be understood.MethodThis was a prospective cohort study with 844 subjects enrolled during the first trimester. Thyroid hormones, free carnitine (C0), neonate birth weight, as well as other related clinical and metabolic data were collected and assessed.ResultsPre-pregnancy weight and body mass index (BMI) as well as neonate birth weight were significantly different among different free thyroxine (FT4) level groups. Maternal weight gain and neonate birth weight varied significantly when grouped by different thyroid-stimulating hormone (TSH) levels. There was a significantly positive correlation between C0 and TSH (r = 0.31), free triiodothyronine (FT3) (r = 0.37), and FT4 (r = 0.59) (all P < 0.001). In addition, a significantly negative influence was found between birth weight and TSH (r = −0.48, P = 0.028), so as C0 (r = −0.55, P < 0.001) and FT4 (r = −0.64, P < 0.001). Further assessment detected a stronger combined effect of C0 and FT4 (P < 0.001) and of C0 and FT3 (P = 0.022) on birth weight.ConclusionMaternal C0 and thyroid hormones are of great importance in neonate birth weight, and routine examination of C0 and thyroid hormones during the second trimester has a positive effect on the intervention of birth weight.

Abstract:Objective: Gestational transient thyrotoxicosis (GTT) and Graves' disease (GD) are the most common causes of hyperthyroidism during pregnancy. However, few studies have compared pregnancy outcomes of patients who had GTT with those who had GD in the first trimester of pregnancy. Methods: We conducted a prospective multicenter cohort study in China. Participants received questionnaires, physical examinations, and underwent measurements of thyrotropin (TSH), free thyroxine (fT4), thyroid peroxidase antibody (TPOAb), TSH receptor antibody (TRAb), and urinary iodine in the first trimester. The patients diagnosed with either GTT or GD and normal thyroid function (NTF) group were followed until delivery. The thyroid function and pregnancy outcomes were reported. Results: A total of 125 pregnant women with thyrotoxicosis and 246 age-matched pregnant women with NTF were included. (1) The thyroid function of the GTT group returned to normal range in the third trimester, but was consistently abnormal in the GD group. (2) The incidence of gestational diabetes mellitus (GDM) in the GTT group (11.5%, 9/78) was significantly higher than that in NTF group (4.9%, 12/246) (p = 0.037). The incidence of premature delivery in the GD untreated (30.8%, 8/26, p = 0.002) and treated groups (28.6%, 6/21, p = 0.008) was both, respectively, higher than that in the NTF group (7.7%, 19/246). Miscarriage (15.4%, 4/26 vs. 3.7%, 9/246, p = 0.026) and gestational hypertension (19.2%, 5/26 vs. 3.3%, 8/246, p = 0.004) were more prevalent in the GD untreated group than in the NTF group. (3) The presence of positive TRAb and positive TPOAb in the first trimester were independent risk factors for miscarriage (odds ratio [OR] = 5.23, confidence interval [CI] = 1.11-24.78, p = 0.037) and low birth weight infants (OR = 7.76, CI = 1.23-48.86, p = 0.029), respectively. Conclusion: In conclusion, pregnancy outcomes appear variable, according to the etiology of first trimester thyrotoxicosis. GTT appears to be associated with GDM. GD appears to be associated with an increased risk of premature delivery, gestational hypertension, and miscarriage. The diagnosis of GTT and GD patients during early pregnancy and appropriate treatment of GD patients may be associated with improved pregnancy outcomes.

Abstract:BackgroundGraves’ hyperthyroidism affects 0.2% of pregnant women. Establishing the correct diagnosis and effectively managing Graves’ hyperthyroidism in pregnancy remains a challenge for physicians.MainThe goal of this paper is to review the diagnosis and management of Graves’ hyperthyroidism in pregnancy. The paper will discuss preconception counseling, etiologies of hyperthyroidism, thyroid function testing, pregnancy-related complications, maternal management, including thyroid storm, anti-thyroid drugs and the complications for mother and fetus, fetal and neonatal thyroid function, neonatal management, and maternal post-partum management.ConclusionEstablishing the diagnosis of Graves’ hyperthyroidism early, maintaining euthyroidism, and achieving a serum total T4 in the upper limit of normal throughout pregnancy is key to reducing the risk of maternal, fetal, and newborn complications. The key to a successful pregnancy begins with preconception counseling.

Abstract:Purpose: The aim is to validate the third generation Thyrotropin receptor antibody (TRAb) assay for predicting neonatal thyroid dysfunction and adverse pregnancy outcomes in pregnant women with Graves' disease. Methods: This prospective cohort study was conducted in TRAb positive pregnant women with Graves' disease and their off springs. The primary outcome was to assess different forms of neonatal thyroid dysfunction in relation to maternal and neonatal TRAb levels. The secondary outcome was to predict adverse pregnancy outcomes by using maternal TRAb levels. Serum T3, FT4, TSH, TRAb levels were measured using electrochemiluminescence immunoassay. Results: 51 pregnant women were included. Five women had adverse pregnancy outcomes, TRAb levels of > 19.06 IU/L (10.9 times the upper limit of normal (ULN)) predicted adverse pregnancy outcomes with 100% sensitivity and 93.5% specificity. Among the 46 successful live births, 13 (28.3%) had neonatal thyroid dysfunction. Out of 13 neonates, 7 (32%) had neonatal thyrotoxicosis, 4 (18%) had primary hypothyroidism, and 2 (9%) had central hypothyroidism. Third trimester maternal TRAb levels of > 7.99 IU/L (4.6 times the ULN)and day three neonatal TRAb levels of > 5.03 IU/L (2.9 times the ULN), predicted the neonatal thyrotoxicosis with 100% sensitivity and 97.4% specificity. Conclusion: Very high maternal third generation TRAb levels strongly predicted the adverse pregnancy outcomes and neonatal thyroid dysfunction in pregnant women with Graves' disease. Neonatal thyroid function test along with the TRAb levels strongly correlated with different forms of neonatal thyroid dysfunction and is very useful in avoiding inadvertent treatment to neonates.

Abstract:We offer evidence that people construe mathematical relations as causal. The studies show that people can select the causal versions of equations, and that their selections predict both what they consider most understandable and their expectations of change. When asked to write down equations, people have a strong preference for the version that matches their causal model. Causal Models Frame 3 Causal Models Frame Interpretation of Mathematical Equations Empirical phenomena afford multiple representations that each highlight a different aspect of empirical structure. Even though mathematical equations can provide a concise and exact representation of how variables relate, they need not represent causal structure. Mathematical relations are symmetric, X = Y is identical to Y = X. Causal relations are not, X causes Y is not the same as Y causes X. Moreover, the representation of causal structure is essential to intervene on the world and influence its dynamics. In these senses, causal structure is more demanding than purely mathematical structure. Nevertheless, or perhaps as a result, people impose causal structure to understand (diSessa, 1993; Hunt & Minstrell, 1994; Reif & Allen, 1992). We propose they impose causal structure even when they need not: in the interpretation of mathematical representations. We hypothesize that people prefer equations whose variables are in an order that matches the causal relations that generate the phenomena represented. People impose a causal frame even when it distorts a representation. For example, people assert a unidirectional causal relation between variables that interact in a more dynamic fashion. The circuit between a battery and a light is often incorrectly understood as unidirectional, with energy flowing from the battery to the light (Andersson, 1986; Driver, Guesne & Tiberghien, 1993; Reiner, Slotta, Chi & Resnick, 2000). Teaching correct causal interpretations often helps form a deeper understanding (White, 1993). Pearl (2000) suggested that not all versions of equations are cognitively equivalent. People will learn and use equations more effectively to the extent that these match their causal model. Supporting evidence is that people consider the left side of an Causal Models Frame 4 equation the outcome of something changing on the right (Sherin, 2001). The hypothesis predicts that, in the case of a three-variable equation matches a single causal model, the variable that is an effect of the other two variables should be written on the left and the two causes on the right. For example, the equation that relates pressure (P), weight (W) and area (A), can be written three different ways: P = W/A, A = W/P and W = AP. However, P = W/A should seem more intuitive, because the most natural naïve causal model represents pressure as the effect of weight and area, not as a cause of them. This is suggested by the intuition that if weight were increased, pressure would increase. The equation that solves for A is not as understandable; the intuition that increasing weight would increase area is weaker. Through a series of studies, we tested whether people have a preference for the version of equations that matches causal understanding. For these studies, we used 16 equations from various fields, such as physics, biology and economics (see Table 1 for full list of equations). They were grouped into 3 classes: The first type (Causal) were the equations with a clear causal model (e.g., P = W/A). These are the equations that represent causally related phenomena that are familiar, so most people understand what causes what. We assumed that the causal models of these simple situations are consistent across our population of Brown University undergraduates. The second type (Unclear) were equations with unclear causal models. These are equations that represent phenomena that are not very familiar, but for which causal models are assumed to exist. Therefore, we expected different people to think that they are related in different ways. One example is the equation that relates thermal efficiency (η), work done during one cycle (W), and heat added during one cycle (Q): η = W/Q. Some people might think that Causal Models Frame 5 increasing the thermal efficiency would increase the work done during one cycle. Conversely, others might think that increasing the work done during one cycle would increase the thermal efficiency. The third type (Noncausal) are equations that have no causal model. These are equations that represent geometric relations or scale transformations with no causal interpretation. An example of this type would be the equation that gives the relation between temperature in Kelvin and Celsius: K = oC + 273.
Clinical features of neonatal hyperthyroidism: a retrospective analysis in southwestern China

Abstract:Purpose: This study aimed to explore the clinical characteristics and evaluate the different types of thyroid dysfunction in babies with neonatal hyperthyroidism. Methods: The clinical data of 19 neonates with hyperthyroidism admitted to the Children's Hospital of Chongqing Medical University between January 2012 and April 2021 were retrospectively analyzed. Results: Fifteen (78.9%) infants were born to mothers with Graves' disease. Eleven (57.9%) infants were premature; two babies were born at small for gestational age. The age at diagnosis ranged from 3 to 34 days, with a mean of 18.53 ± 6.85 days. The majority of the babies presented with goiter (84.2%) and tachycardia (94.7%) after birth. Nine (47.4%) of them presented with abnormal weight gain, seven (36.8%) presented with stare or ocular protrusion, six (31.6%) presented with hyperexcitability, four (21.1%) presented with jaundice and liver dysfunction, two (10.5%) presented with sweating, one (5.3%) presented with fever, and one case presented without any symptoms. Transient hyperthyroidism was the main thyroid dysfunction in our study. Overt hyperthyroidism was diagnosed in 13 (68.4%) neonates. Another three babies (15.8%) presented with hyperthyroidism with slightly elevated free triiodothyronine levels, normal thyroxine (T4) levels, and low thyroid-stimulating hormone (TSH) levels. Normal thyroid hormone levels with low TSH levels were observed in three (15.8%) neonates. Ten children were treated with antithyroid drugs. Eighteen children recovered normal thyroid function at 1–3 months of age; one baby in the study group required further levothyroxine supplementation due to primary hypothyroidism (HT). One child was found to have developmental delay at 2 years of age during follow-up. Conclusions: Our study highlights the need for prolonged monitoring of thyroid function in suspected patients. A single normal screening for hyperthyroidism or the absence of a maternal history of hyperthyroidism cannot exclude this disease.

A unique presentation of Graves' disease in a pregnant woman with severe hypothyroidism

Abstract:BackgroundGraves' disease occurrence during pregnancy is not a frequent event, showing an incidence of 0.2-0.4% in unselected pregnant women. Depending on their functional properties, TSH-receptor antibodies can induce hypothyroidism or hyperthyroidism. Recognizing the signs of altered thyroid function is essential to prevent possible complications on the fetus.Materials and methodsThe case of a pregnant woman without previous history of thyroid disease presenting with severe overt hypothyroidism during the first trimester is reported. Levothyroxine therapy was started and 6 weeks later overt hyperthyroidism was observed. TRAb were detected at high titers. Levothyroxine was withdrawn and low dose methimazole was started. Serial obstetric ultrasound scans were negative for indirect signs of fetal thyroid dysfunctions and no fetal goiter was visualized throughout pregnancy. Spontaneous delivery occurred without complications at 39 weeks of gestation. In the post-partum, severe overt hypothyroidism recurred, thus methimazole was discontinued and levothyroxine was restarted. TRAb persisted at high levels. The infant experienced a transient thyrotoxicosis, which fully resolved in three months with normalization of thyroid function and negativization of TRAb levels.ResultsThe present case report allows us to overview the challenges related to the management of hypo and hyperthyroidism in patients with high TRAb levels, requiring strict monitoring aimed at early detection of both maternal and fetal consequences.ConclusionsThis case underlines the importance of close follow-up and the need of collaboration in a multidisciplinary team when Graves's disease is diagnosed in a pregnant woman to prevent adverse neonatal outcomes.
Neonatal Screening for Hyperthyroidism Proof of Concept

Abstract:Context: Early treatment is essential to avoid the cardiac complication of neonatal hyperthyroidism (NH). Our results have direct implications for clinical care. Objective: NH can cause potentially fatal neonatal thyrotoxicosis. Here, we have evaluated the feasibility of neonatal hyperthyroidism screening using the thyroid-stimulating hormone value in dried blood collected routinely on filter paper on the third postnatal day of life for congenital hypothyroidism screening. Methods: Retrospective case-control study. Cases were identified using data from our previously published study of 280 000 infants born in 10 maternity units in France in 2007-2014. Controls were identified among the 1 362 564 infants born in the Ile-de-France region during the same period. Results: A screening thyroid-stimulating hormone level below 0.18 mIU/L on the third postnatal day had 71% (95% CI 44-90%) sensitivity, 99% (95% CI 99-100%) specificity, 81% (95% CI 74-86%) positive predictive value, and 98% (95% CI 97-99%) negative predictive value for detecting severe NH. By univariate regression analysis, the screening thyroid-stimulating hormone value was the strongest predictor of NH (P < .00001), with an area under the receiver-operating characteristics curve of 0.98 (95% CI 0.95-1.0). Expected frequencies were not significantly different from observed frequencies (Hosmer-Lemeshow test, P = .99). Conclusion: The screening thyroid-stimulating hormone test can be used to detect severe NH, the optimal cut-off being 0.18 mIU/L. The additional cost compared with screening for congenital hypothyroidism would be small. Infants with neonatal hyperthyroidism would benefit from an earlier diagnosis with treatment initiation at the presymptomatic stage in many cases, ensuring optimal outcomes.

Abstract:The melanocortin-4 receptor (MC4R) was cloned in 1993 by degenerate PCR; however, its function was unknown. Subsequent studies suggest that the MC4R might be involved in regulating energy homeostasis. This hypothesis was confirmed in 1997 by a series of seminal studies in mice. In 1998, human genetic studies demonstrated that mutations in the MC4R gene can cause monogenic obesity. We now know that mutations in the MC4R are the most common monogenic form of obesity, with more than 150 distinct mutations reported thus far. This review will summarize the studies on the MC4R, from its cloning and tissue distribution to its physiological roles in regulating energy homeostasis, cachexia, cardiovascular function, glucose and lipid homeostasis, reproduction and sexual function, drug abuse, pain perception, brain inflammation, and anxiety. I will then review the studies on the pharmacology of the receptor, including ligand binding and receptor activation, signaling pathways, as well as its regulation. Finally, the pathophysiology of the MC4R in obesity pathogenesis will be reviewed. Functional studies of the mutant MC4Rs and the therapeutic implications, including small molecules in correcting binding and signaling defect, and their potential as pharmacological chaperones in rescuing intracellularly retained mutants, will be highlighted.
