Abstract:Background People with symptomatic chronic obstructive pulmonary disease (COPD) benefit from pulmonary rehabilitation programs (PRPs), but program attrition is common. Methods For people with COPD who presented to tertiary care and appeared appropriate for a PRP, we prospectively mapped their PRP journey, explored factors influencing attendance to pre-program assessment and captured program attrition. Results Of the 391 participants, 31% (95% CI 27 to 36) were referred to a PRP ( n = 123; age 68 ± 10years, 62 males [50%], FEV 1 45 ± 19%predicted). Of those referred, 94 (76% [69 to 84]) attended a pre-program assessment. Ex-smokers and those who had a healthcare professional (HCP) explain they would be referred were more likely to attend a pre-program assessment (odds ratio [95%CI]; 2.6 [1.1 to 6.1]; and 4.7 [1.9 to 11.7], respectively). Of the 94 who attended, 63 (67% [58 to 77]) commenced; and of those who commenced, 35 (56% [43 to 68]) completed a PRP. All who completed ( n = 35, 100%) were provided at least one strategy to maintain training-related gains. Conclusion Attrition occurs throughout the PRP journey. Interactions with HCPs about PRPs positively influenced attendance. Understanding how HCPs can best contextualise PRPs to encourage referral acceptance and uptake is an important area for further work.

Acceptability and feasibility of pulmonary rehabilitation for COPD: a community qualitative study

Abstract:Background: Pulmonary rehabilitation (PR) is second in importance to smoking cessation treatment in the management of chronic obstructive pulmonary disease (COPD). Access to the service is limited and less than half of those referred complete the treatment. Aims: We assessed the obstacles to participation in PR among COPD patients in a community-based PR programme and associated general practices. Methods: A qualitative interview study was conducted among COPD patients who completed the PR treatment, those who did not complete or declined treatment, and patients never referred. Participants were invited by letter from their own general practitioners or from the PR service. Views on exercise, disease education, social contact, group activity, accessibility, location, role of referrer, and support for participation were assessed. Data were analysed using the framework approach. Results: Twenty-four patients (28%, 13 male, 12 not referred) were interviewed. The acceptability of PR was the major concern. Feasibility of attending was an issue for some. Perceptions of PR and of exercise were highlighted. How a smoker might be seen, the suitability of group activity, and the views of professionals were influential, as were positive and negative recommendations. The location of the centre was important. Participants' willingness or reluctance to take on something new was a central element of the decision. Many would welcome the role of experienced patients in introducing the treatment. Conclusions: For patients who refused referral to PR, had not completed a course, or had yet to be referred, the way the service was introduced was an important determinant of willingness to participate.
An observational cohort study of exercise and education for people with chronic obstructive pulmonary disease not meeting criteria for formal pulmonary rehabilitation programmes

Abstract:Chronic obstructive pulmonary disease (COPD) is a major cause of morbidity and mortality. Pulmonary rehabilitation (PR) is offered to patients with functional breathlessness. However, access to PR is limited. The objective of this study was to evaluate whether a 4-week education and exercise programme offered to COPD patients with Medical Research Council (MRC) dyspnoea 1–2 improves disease self-management. Patients were recruited by their GP to attend four weekly 2-h sessions provided by a multidisciplinary team. Patients completed outcome measures before and after the program. Forty-two patients entered the programme and 26 out of 42 (61.9%) completed all sessions. The Bristol COPD Knowledge Questionnaire and Patient Activation Measure improved (both p ≤ 0.001). Disease burden was not reduced according to the COPD assessment test. All patients accepted a referral for ongoing exercise. Fourteen current smokers (81.3%) accepted a referral for smoking cessation, three patients with anxiety or depression (37.5%) accepted a psychological therapies referral. The programme improved COPD disease knowledge, patient activation and stimulated referrals to further services supporting disease management. Randomised controlled trials are warranted for similar interventions for COPD patients with early stage disease.

Abstract:In our previous RCT, short-term effects of pulmonary rehabilitation (PR) for chronic obstructive pulmonary disease (COPD) were associated with attendance rate in a dose-response pattern, regardless of whether PR contained conventional physical exercise training or Singing for Lung Health. In this 56 months' follow-up study, we aimed to investigate if current self-reported maintenance training was correlated with initial PR adherence. We hypothesised that maintenance training was related to PR adherence in the past. In 2023, RCT participants, who were still alive, were contacted (n=196; 73% of 270), included (n=159; 81% of 196), and interviewed via telephone. We explored data using stratified analyses and logistic regression. In total, 30 participants had dropped out during the initial RCT. At long-term follow-up, these non-completers were currently more frequently smokers (p=0.04) and less active than completers (38% vs 60%; p=0.03), regardless of initial RCT randomisation. Moreover, they had decreased QoL (St George's Respiratory Questionnaire): mean change from since RCT: Non-completers: 4.0 (SD 15.6); Completers: -3.5 (SD 13.9). Current maintenance training was related to maintained QoL (p=0.04; OR: 2.4, 95% CI [1.0; 5.4]). Our study suggests that long-term maintenance training and maintenance of PR effects are positively associated with completion of an initial PR programme – irrespectively of randomisation to SLH or PExT. Therefore, we see a need for future PR programmes to include support of maintenance training after PR, and additional attention – including smoking cessation support - should be given to activate those who do not complete PR.

Abstract:PURPOSE: Cardiac rehabilitation (CR) is a program of structured exercise and interventions for coronary risk factor reduction that reduces morbidity and mortality rates following a major cardiac event. Although a dose-response relationship between the number of CR sessions completed and health outcomes has been demonstrated, adherence with CR is not high. In this study, we examined associations between the number of sessions completed within CR and patient demographics, clinical characteristics, smoking status, and socioeconomic status (SES). METHODS: Multiple logistic regression and classification and regression tree (CART) modeling were used to examine associations between participant characteristics measured at CR intake and the number of sessions completed in a prospectively collected CR clinical database (n = 1658). RESULTS: Current smoking, lower SES, nonsurgical diagnosis, exercise-limiting comorbidities, and lower age independently predicted fewer sessions completed. The CART analysis illustrates how combinations of these characteristics (ie, risk profiles) predict the number of sessions completed. Those with the highest-risk profile for nonadherence ( 72 years old, not current smoker, higher SES, surgical diagnosis) completed 27 sessions on average. CONCLUSIONS: Younger individuals, as well as those who report smoking or economic challenges or have a nonsurgical diagnosis, may require additional support to maintain CR session attendance.

Expectation, satisfaction, and predictors of dropout in cardiac rehabilitation

Abstract:Background: Despite the evidence of benefit many patients fail to adhere to cardiac rehabilitation (CR). This study aims at describing the rate, reasons, and predictors of dropout from CR, as well as expectations und satisfaction among participants. Methods: Prospective analysis of demographic, medical, and psychosocial data in a large cohort of consecutive participants of outpatient CR. Results: Between 1999 and 2008, 2521 consecutive patients were enrolled in CR and complete data for analysis were available in 2371 patients. Diagnoses were coronary heart disease in 85%, valvular heart disease in 15%, and other in 10%. Mean age was 59.7 ± 11.4 years; 85% were male. Specific expectations towards CR were identified in a subgroup of participants and satisfaction was high in 75% of patients. Early dropout was noted in 305 patients (12.9%); reasons for dropout were cardiac in 39 (1.7%) and non-cardiac in 266 (11.2%). Significant differences between dropouts vs. completers were: peak exercise capacity (116 ± 41 vs. 123 ± 39 Watt, p = 0.003); body mass index (28 ± 7 vs. 27 ± 4 kg/m(2), p = 0.04); prevalence of diabetes (18 vs. 13%, p = 0.011); smoking (32 vs. 16%, p < 0.001); being professionally active (24 vs. 34%, p = 0.007); widowhood (8 vs. 3%, p = 0.04); higher education (37.5 vs. 46.4%, p = 0.003); white collar work (21 vs. 27%, p = 0.012); and scales of a quality-of-life score (p < 0.02). Multivariate analysis revealed low exercise capacity, high body mass index, smoking, diabetes, and widowhood to be significant independent predictors for early dropout. Conclusion: In the present study we demonstrated a low rate of mainly non-cardiac dropout from outpatient CR in motivated and largely satisfied participants. In order to improve adherence, early attention and close supportive counselling is recommended in patients with low exercise capacity and those who are obese, diabetic, smokers, or bereaved of a partner.

Abstract:Aim: People with chronic obstructive pulmonary disease (COPD) can be reluctant to engage in a pulmonary rehabilitation program (PRP) when offered a referral. This study explored how the interaction between healthcare professionals (HCPs) and people with COPD can influence uptake of a PRP. Methods: We purposefully sampled Western Australians with COPD who expressed interest in sharing their experience of being recommended a PRP, and HCPs who refer to a PRP from tertiary care. Semi-structured interviews were recorded and transcribed verbatim. We conducted thematic analysis to deconstruct determinants influencing uptake of a referral to a PRP. Results: We conducted 56 interviews involving 17 people with COPD (p) and 37 HCPs (h) from March to July 2021. Themes generated include: (i) persona of the referrer "It's the attitude of the way they say it. It makes quite a bit of difference, whether they really believe it. I think that's important because then they pass that belief onto the patient."(p12) (ii) unpacking unhelpful emotions/beliefs "Put hope into people and take away their fear. Sometimes we have perceptions, before we take up something, we think that's like that. But it's not. Sometimes if we clear that up, they think 'oh actually, I can give it a go'."(h1) "I judge myself. It's because I'm a smoker. And because of that, then you don't deserve treatment."(p4) Conclusion: Interactions where HCPs demonstrate how invested they are in optimising outcomes meaningful to each patient are important when recommending people with COPD to a PRP. Precise conversations where fear of symptoms or exercise, and feelings of unworthiness are carefully acknowledged and challenged may facilitate greater PRP uptake.

Abstract:Introduction: After completing Pulmonary Rehabilitation (PR), there is a new challenge that involves maintaining the benefits and activity levels achieved during the program. The GOLD guidelines state that PR's aim is to translate physical conditions into an increase in levels of physical activity (PA). Objectives: The aim of this study was to analyse the impact of an outpatient PR on the PA and functional capacity of individuals with COPD. Materials and Methods: A quasi-experimental study was implemented using a convenience sample of individuals with COPD who attended a 12-week PR program (20 sessions). A component that supported educational and behavioural change was included in the PR, which included endurance and muscle strengthening sessions. A pedometer (Yamax EX 510) was used to assess PA by monitoring the number of steps. The 1-minute Sit-to-Stand Test (1MSTS) was used to assess functional capacity. Both measures were evaluated at the beginning and end of the PR. Results: The number of repetitions in the 1MSTS revealed statistically significant improvements in functional capacity (p-value 0.03). Despite not being statistically significant (pre-RR – 3690, and post-RR – 3079, p-value 0.06), there was a decrease in steps number for PA levels, as shown in the data. Conclusions: This study showed that the PR did not result in gains in PA levels despite improving functional capacity (assessed by the 1MSTS). In order to increase PA levels, it seems necessary for the PR program to be accompanied by follow-up strategies and support for an extended period.

Abstract:Introduction: Few studies have examined patterns of physical activity (PA) during a PR programme in people with COPD. Aims: To compare the patterns of PA in: 1) the week prior to commencing PR (pre-PR), a week during PR (week 7), and the week after PR programme completion (post PR); and 2) supervised PR training days (PR days) and non-supervised days (non-PR days) during a PR week. Methods: A multicentre, prospective cohort study. Participants attended twice weekly supervised PR. Daily step count (primary outcome), time in light activities, time in moderate to vigorous PA (MVPA), time in sedentary behaviour and sit-to-stand (STS) transitions were measured using a thigh worn accelerometer (activPAL3 micro) for seven days at each assessment timepoint: pre-PR, week 7 and post PR. Minimum wear time for data inclusion was four days. Results: 29 participants, mean age (SD) 69 years (7), FEV1 53%pred (16). At week 7 compared to pre-PR, there were daily increases in, mean difference (95%CI): step count, 941 steps (388 to 1494); and MVPA, 11 mins (6 to 15), with no difference in: time in light activities, -1 min (–6 to 5); total sedentary time, 7 mins (–21 to 36); or STS transitions, 0 (–5 to 6). Comparison of PR days to non-PR days found increases in daily: step count, 2810 steps (1706 to 3913); time in light activities 11 mins (1 to 20); time in MVPA, 27 mins (17 to 35) and STS transitions, 8 (4 to 12), with no difference in total sedentary time: -33 mins (–80 to 15). PA post-PR was no different to pre-PR (all p<0.05). Conclusion: Daily step count and time spent in MVPA increased significantly during participation in a PR programme. Differences in PA arose solely due to increased PA on days participants attended PR.

Abstract:Background: Pulmonary rehabilitation (PR) is the recognised gold-standard of community-based care for chronic respiratory conditions but long-term gains in patient wellbeing are not sustained. Community-based exercise maintenance programmes have shown promise in addressing this issue but there is an absence of qualitative patient data informing same. Methods: Participants who had completed a programme of PR within the previous 18 months at Irish Health Service Executive Primary Care centres in Limerick and Ennis were invited to participate in focus groups discussing PR maintenance programmes. Results: Seventeen participants (female:n=9 (53%); mean (SD) age:68.8 (10.1) years) took part in four focus groups (three in-person, one online) in July, 2023. All participants reported a positive PR experience, but most failed to maintain exercise habits afterwards. All participants were enthusiastic about maintenance programmes, which they thought should be led by a trained health/fitness professional or a clinician. Six-eight week rolling blocks to be accessed within one-two months post-PR, comprising exercise and social components were preferred. Educational sessions were favoured by some, with others finding these boring and repetitive. Light-touch ongoing assessment to monitor progress was preferred. In-person settings for maintenance classes were overwhelmingly favoured over online offerings. Conclusions: Patient insights about PR maintenance programmes provide useful guidance for policy makers considering provision of this service.

Abstract:Objective: Pulmonary rehabilitation (PR) improves function, reduces symptoms and decreases healthcare usage in people with chronic obstructive pulmonary disease (COPD) following an acute exacerbation (AECOPD). However, rehabilitation uptake rates are low. This study aimed to address barriers to uptake and completion of PR following AECOPD. Methods: An action research approach was used to reflect on study feasibility, and to plan and implement an improved protocol. Phase I tested the feasibility of home-based PR started early after AECOPD. Phase II used qualitative interviews to identified potential barriers to program uptake. Phase III re-tested the program with changes to recruitment and assessment strategies. Results: Phase I: From 97 screened patients, 26 were eligible and 10 (38%) started home-based PR. Eight participants undertook ≥70% of PR sessions, achieving clinically meaningful improvement in 6-minute walk distance (mean (SD) change 76 (60) m) and chronic respiratory disease questionnaire total score (15 (21) units). Phase II: Potential barriers to uptake of home-based PR included access issues, confidence to exercise, and lack of information about PR benefits. Phase III: From 77 screened patients, 23 were eligible and 5 (22%) started the program. Discussion: Home-based PR improved clinical outcomes, but program eligibility and uptake remain challenging. Efforts should be made to ensure PR program eligibility criteria are broad enough to accommodate patient needs, and new ways of engaging patients are needed to improve PR uptake after AECOPD.

Abstract:Background:In chronic obstructive pulmonary disease (COPD), the problem of poor patient participation in studies of self-management (SM) and pulmonary rehabilitation (PR) programmes (together referred to as COPD support programmes) is established. Understanding this problem beyond the previously reported socio-demographics and clinical factors is critical.Aims:The aim of this study was to explore factors that explain patient participation in studies of COPD support programmes.Methods:Thematic ‘framework’ synthesis was conducted on literature published from 1984 to 1 February 2015. Emergent themes and subthemes were mapped onto the adapted ‘attitude–social influence–external barriers’ and the ‘self-regulation’ models to produce analytical themes.Results:Ten out of 12 studies were included: PR (n=9) and SM (n=1). Three descriptive themes with 38 subthemes were mapped onto the models' constructs, and it generated four analytical themes: ‘attitude’, ‘social influences’ and ‘illness’ and ‘intervention representations’. The following factors influenced (1) attendance—helping oneself through health improvements, perceived control of worsening condition, perceived benefits and positive past experience of the programme, as well as perceived positive influence of professionals; (2) non-attendance—perceived negative effects and negative past experience of the programme, perceived physical/practical concerns related to attendance, perceived severity of condition/symptoms and perceived negative influence of professionals/friends; (3) dropout—no health improvements perceived after attending a few sessions of the programme, perceived severity of the condition and perceived physical/practical concerns related to attendance.Conclusions:Psychosocial factors including perceived practical/physical concerns related to attendance influenced patients’ participation in COPD support programmes. Addressing the negative beliefs/perceptions via behaviour change interventions may help improve participation in COPD support programmes and, ultimately, patient outcomes.

Abstract:Patients who participate in cardiac rehabilitation programs (CRP) experience significant improvements in quality of life, rehospitalization rates, and mortality associated with cardiovascular disease. The potential efficacy of CRP is limited however, by significant program dropout rates and poor patient adherence to prescribed exercise regimens following rehabilitation. Recently, models of motivational readiness for behavior change, such as the Transtheoretical Model, have been applied to understanding the process of exercise adoption and maintenance. Interventions based on this dynamic model of behavior change have produced significant improvement in adherence to exercise in community and worksite populations. This study investigates the applicability of this model to this special population. Sixty-two men and women completed measures of motivational readiness, self-efficacy, and decisional balance for exercise adoption upon entry into a 12-week cardiac rehabilitation program at posttreatment and at a 3-month follow-up. Patients made signi ficant gains during CRP in time spent exercising and self-efficacy, but not in utilization of the cognitive processes or in the perceived benefits of exercising. Exercise maintenance at follow-up was differentially associated with self-efficacy, decisional balance, and use of behavioral processes at posttreatment. Motivation-based models of exercise adoption may provide insights regarding the adoption and maintenance of regular physical activity in cardiac rehabilitation populations.

Abstract:Psychological co-morbidities are common in chronic obstructive pulmonary disease (COPD) but remain overlooked. Psychosocial interventions are deemed to promote mental health and optimize management. This study aimed to determine the role of detailed psychological evaluation and treatment in the comprehensive management of COPD. COPD patients after screening with the general health questionnaire-12 (GHQ-12) for psychological co-morbidity were divided into three groups (26 patients each): i) group A [GHQ-12 score<3, received pulmonary rehabilitation (PR) and standard medical management]; ii and iii) group B and C (GHQ-12 score>3, in addition, received management by a psychiatrist and counseling by a pulmonologist, respectively). At baseline and 8 weeks of follow-up, all participants were evaluated for respiratory [forced expiratory volume in the first second (FEV1), six-minute walk distance (6-MWD), St. George's respiratory questionnaire (SGRQ), modified medical research council (mMRC) dyspnea scale], and psychological [GHQ-12, patient distress thermometer (PDT), coping strategy checklist (CSCL), World Health Organization-quality of life-brief (WHOQOL-Bref-26), and depression anxiety stress scales (DASS)] parameters. Psychological distress (GHQ-12>3) decreased significantly at follow-up, with 11.5% and 53.8% of patients having psychological distress in groups B and C, respectively, versus baseline (p<0.001). mMRC score, SGRQ score, FEV1 and 6-MWD significantly improved in all three groups. Improvement in mMRC and SGRQ was maximal in group B when compared with the other groups. PDT, CSCL, and WHO-QOL-Bref-26 scores improved significantly at follow-up in all three groups, with maximum improvement in group B, followed by group C, and then group A. The DASS score also improved maximally in group B. Patients should be screened for psychological co-morbidities using simple screening tools. PR plays an important role in improving the psychology of COPD patients. However, results are better with directed psycho-educative sessions by non-experts and best with definitive treatment by psychiatrists.
Exercise rehabilitation in COPD and heart failure: comparison of two national audits

Abstract:Background Pulmonary (PR) and cardiac rehabilitation (CR) are recommended in the management of chronic obstructive pulmonary disease (COPD) and chronic heart failure (CHF); the impact of coexisting COPD and CHF on completion and outcomes of rehabilitation programmes is unknown. We examined enrolment, completion and clinical outcomes of CR and PR in adults with COPD, CHF and coexisting COPD and CHF. Methods The National Audit of CR and National COPD Audit Programme: clinical audits of PR were analysed (211 PR and 237 CR programmes); adults with a diagnosis of CHF, COPD or coexisting COPD and CHF were identified (COPD+CHF or CHF+COPD according to database). Propensity matching was conducted (age, sex, body mass index and functional status) between COPD+CHF and COPD, and CHF+COPD and CHF. Group by time interaction was examined using mixed 2x2 analysis of variance. Results Those with CHF+COPD had lower enrolment and completion of CR compared to those with CHF; there were no differences in PR enrolment or completion between the two groups. Adults with COPD made a significantly larger gain in the incremental shuttle walk test compared to adults with COPD+CHF following PR (59.3 m versus 37.4 m); the improvements following CR were similar (CHF 77.3 m versus CHF+COPD 58.3 m). Similar improvements were made in the 6-min walk test following CR (CHF 45.1 m versus CHF+COPD 38.8 m) and PR (COPD 48.2 m versus COPD+CHF 44.0 m). Comparable improvements in quality of life and mood state were made following CR and PR, regardless of diagnosis. Conclusion We have demonstrated that multi-morbid adults benefit from exercise-based rehabilitation, yet efforts are needed to promote completion. These findings support group-based, tailored, multi-morbid exercise rehabilitation.

Abstract:Background: Chronic diseases lead to both financial and organisational burdens on the health system. One of the possible solutions to keep health care affordable is self-management. However, non-adherence is a recognised problem with regard to self-management, especially in long-term treatment. Once it is clear who is or is not adherent, care can potentially be tailored accordingly. To predict adherence over time, it may be useful to understand how this variable might change over time: It is unclear whether adherence is a constant, or rather increases or decreases or fluctuates. The aim of this study is to determine the course of exercise adherence over a 12-month period in patients with chronic obstructive pulmonary disease (COPD) receiving prolonged rehabilitation. Methods: In this prospective cohort study participated 196 patients with COPD, who were undertaking pulmonary rehabilitation (PR), for at least one month, in 53 primary physiotherapy practices in The Netherlands and Belgium between January 2021 and August 2022. Outcomes were measured at baseline, and at 3, 6 and 12 months. The primary outcome was exercise adherence, and secondary outcomes were exercise capacity and health related quality of life. Multilevel regression analysis with adherence at the first level and patients at the second level were used for analyses of the data. Results: There was no significant change in exercise adherence over time (p = 0.89). Also, exercise capacity (p = 0.59) and health related quality of life (p = 0.24) remained stable. Conclusions: Adherence is constant over a period of 12 months in patients with COPD receiving prolonged pulmonary rehabilitation in a primary physiotherapy practice in The Netherlands and Belgium. Also, measured health outcomes remained constant in this same period. Possibly, this will allow for better tailored care over 12 months and lower health care costs.

Insufficient Pulmonary Rehabilitation Uptake After Severe Exacerbation of COPD: A Multicentre Study in the South West Region of France

Abstract:Purpose: Pulmonary rehabilitation (PR) is a type of multidisciplinary care strongly recommended after severe exacerbation of chronic obstructive pulmonary disease (COPD). Recently, a national French study reported a very low rate of PR uptake (8.6%); however, important clinical data were missing. Here, we aimed to identify the main factors associated with insufficient PR uptake after hospitalisation for COPD exacerbation. Patients and methods: This multicentre retrospective study included patients hospitalised with COPD exacerbation between 1 January 2017 and 31 December 2018, as identified by both coding and a detailed review of medical records. PR was defined as inpatient care in a specialised centre or unit within 90 days of discharge. Multivariate logistic regression was used to identify associations between PR uptake and patient characteristics, such as comorbidities, non-invasive ventilation (NIV), inhaled treatment, and forced expiratory volume in 1 second (FEV1). Results: Among the 325 patients admitted for severe COPD exacerbation, 92 (28.3%) underwent PR within 90 days of discharge. In univariate analysis, relative to those who underwent PR, patients without PR had significantly more comorbidities, were less often treated with triple bronchodilator therapy or NIV, and had a higher FEV1. In multivariate analysis, variables independently associated with the lack of PR uptake were the presence of comorbidities (adjusted odds ratio (aOR) = 1.28 [1.10-1.53], p = 0.003) and a higher FEV1 (aOR = 1.04 [1.02-1.06], p < 0.001). There was no significant correlation between PR uptake and departmental PR centre capacity (notably, some departments had no PR facilities). Conclusion: These data highlight the lack of PR in the early stages of COPD. Collaboration among all healthcare providers involved in patient management is crucial for improved PR uptake.

Abstract:Introduction: Home-based pulmonary rehabilitation (PR) offers potential benefits for COPD patients, but its effectiveness across severity levels remains unclear. PR components are known. Results may be improved if the patient is more engaged with the intervention and receptive to change. This study aims to analyse which patients most benefit from home-based PR programmes regarding COPD severity. Methods: We conducted a quasi-experimental study where an 8-week PR programme was delivered. Regarding functional capacity accessed by the 6mWT, the 1-minute Sit-to-Stand Test, and the physical activity level, we divided the groups according to the cut-offs in the literature for the COPD population indicating poor prognosis. We categorised the patients into severe and non-severe patients. The primary outcome was functional capacity. Secondary outcomes included the impact of COPD and HRQoL. Results: Both groups showed relevant improvements in 6MWD at eight weeks. However, the severe group had a more significant improvement compared to the non-severe. Similar trends were observed for secondary outcomes, with greater improvements in the severe group regarding the impact of COPD and HRQoL. Conclusions: This study suggests that home-based PR can improve outcomes for both severe and non-severe COPD patients, with potentially greater benefits for severe patients. Further research is warranted to confirm these findings and optimise PR programs for different severity levels.

Abstract:Objectives: To characterize patients referred for pulmonary rehabilitation on a large number of psychologic and sociodemographic variables and to determine the contribution of these variables on the response to rehabilitation. Design: Cross-sectional, explorative. Setting: University hospital and outpatient clinic. Participants: Eighty-one consecutive patients with chronic obstructive pulmonary disease (forced expiratory volume in 1 second, 40%+/-16% of predicted) were included in outpatient pulmonary rehabilitation. Intervention: Multidisciplinary rehabilitation program. Main outcome measures: Pulmonary function, exercise capacity (Wmax, 6-minute walk test [6MWT]), Chronic Respiratory Disease Questionnaire (CRDQ), Modified Pulmonary Functional Status and Dyspnea Questionnaire (PFSDQ-M), anxiety and depression (Hospital Anxiety and Depression Scale [HADS]) were assessed before and after 3 months rehabilitation. In addition, psychosocial adjustment, social support, marital status, mode of transportation, education, employment, and smoking status were assessed at the start of the rehabilitation. Results: Rehabilitation improved exercise performance (Wmax, 6+/-12W; P<.01; 6MWT, 41+/-72 m; P<.001), quality of life (CRDQ score, 12+/-13 points; P<.001), functional status (PFSDQ-M activity score, -8+/-11 points; PFSDQ-M dyspnea score, -6+/-12 points; PFSDQ-M fatigue score, -4+/-8 points; all P<.01), HADS anxiety score (-2+/-3 points, P<.01), and HADS depression score (-3+/-3 points, P<.001). In single regression analysis, only baseline depression was weakly negatively correlated with the change in maximal workload. No other relations of initial psychologic or sociodemographic variables with outcome were observed. Conclusions: The effects of rehabilitation are not affected by baseline psychosocial factors. Patients with less favorable psychologic or sociodemographic conditions can also benefit from pulmonary rehabilitation. The multidisciplinary approach of the rehabilitation program might have contributed to this improvement.
