Abstract:Previous studies in cancer biology suggest that chemotherapeutic drug resistance and tumor relapse are driven by cells within a tumor termed 'cancer stem cells'. In the present study, a Hoechst 33342 dye exclusion technique was used to identify cancer stem‑like side population (SP) cells in colon carcinoma, which accounted for 3.4% of the total cell population. Following treatment with verapamil, the population of SP cells was reduced to 0.6%. In addition, the sorted SP cells exhibited marked multidrug resistance and enhanced cell survival rates compared with non‑SP cells. The SP cells were able to generate more tumor spheres and were CD133 positive. Subsequent biochemical analysis revealed that the levels of the adenosine triphosphate‑binding cassette sub‑family G member 2 transporter protein, B‑cell lymphoma anti‑apoptotic factor and autocrine production of interleukin‑4 were significantly enhanced in the colon cancer SP cells, which contributed to drug resistance, protection of the cells from apoptosis and tumor recurrence. Therefore, the findings suggested that treatment failure and colon tumorigenesis is dictated by a small population of SP cells, which indicate a potential target in future therapies.

Abstract:This paper examines the effects of public healthcare spending on wage inequality and social welfare in an economy with rural and urban regions. The objectives are twofold. First, we introduce a simple theoretical framework that captures the link between public healthcare spending and firms' output choice and subsequent wage provisions. We demonstrate that in the short run, a rise in public healthcare services can increase urban output and lower urban unemployment and raise both skilled and unskilled wage rates. In the long run, firm entry produces a business‐dynamism effect that enhances the rate of return on capital, reduces the unskilled wage rate and intensifies wage inequality. Second, we empirically test our theoretical predictions using data from 112 economies in the period 2006–2018. We find evidence supporting our theoretical findings concerning the effects of public healthcare on wage outcomes, especially in the long run. Results are highly consistent across the two groups: advanced economies (mainly OECD/European economies), and emerging and developing economies based on alternative measures for wage inequality, including the GINI coefficient and top income share. The paper provides useful information for the public healthcare policy.

Labor Market Concentration and Worker Contributions to Health Insurance Premiums

Abstract:In concentrated labor markets, where workers have fewer employers to choose from, employers may exploit their monopsony power by contributing less to workers' health benefits. This study examined if labor market concentration was associated with higher worker contributions to health plan premiums. We combined publicly available data from the Census to calculate labor market concentration and the Medical Expenditure Panel Survey Insurance/Employer Component to determine premium contributions from 2010 to 2016 for metropolitan areas. After controlling for year fixed-effects and market characteristics, we found that higher labor market concentration was associated with higher worker contributions to health plan premiums, lower take-home income, and no change in employer contributions to premiums, consistent with the hypothesis that greater labor market concentration is associated with less generous health benefits. When evaluating the effects of mergers and acquisitions on labor markets, regulatory agencies should critically assess worker contributions to health insurance premiums.

Abstract:Background: The emergence of COVID-19 in 2020 has brought dramatic impacts to the global economy. The resulting health crisis and widespread fear have triggered labor shortage problems such as high job vacancy rate and low labor participation rate. Thus, how to increase the labor supply has become a hotspot among scholars. We aimed to analyze the influences of public medical insurance system on labor health status and supply. Method: Using the China Health and Nutrition Survey five-phase panel data, the new rural cooperative medical system was taken as an example to empirically analyze the influences of public medical insurance system on the individual health status and labor supply via the panel Tobit model and panel binary Logit model. The analysis revealed the mediating effect of health status. Results: First, Public medical insurance system could significantly improve individual health status. Second, public medical insurance system will lengthen the labor supply time and elevate the labor supply rate by improving individual health status. Third, the influences of public medical insurance system on labor health status and supply vary with gender and age. Conclusion: Public medical insurance system will generate positive influences on labor health status and labor supply. Hence, perfecting the public medical insurance system is able to mitigate the negative impacts of population aging and pandemic on the labor supply.

Economic growth effects of public health expenditure in OECD countries: An empirical study using the dynamic panel threshold model

Abstract:Background: Member countries of the Organization for Economic Co-operation and Development (OECD) have experienced a downward trend in capital and macro investment returns. Countries rely on investments in healthcare and health-related sectors to ensure continuous input and an efficient labor market. Public healthcare coverage is a social welfare policy implemented by governments, which contributes to improving human capital quality and ensuring stable economic growth. Methods: This study employs a method that combines theoretical modeling and empirical testing examining 33 OECD countries from 2001 to 2017. We first construct a nonlinear dynamic panel threshold model to examine the impact of public health spending on economic growth. We then employ the model to empirically analyze the threshold effect of public health expenditure on economic growth, validating the effectiveness of our theoretical model. Results: When the level of household consumption is below the 9.63 threshold, the effect of public health expenditure on economic growth is significantly negative (p < 0.1); however, when consumption exceeds the 9.63 threshold, the effect becomes significantly positive (p < 0.05). Similarly, when employee wages are below the 10.57 threshold, the effect of public health expenditure on economic growth is significantly negative (p > 0.1), and it becomes significantly positive when it exceeds 10.57 (p < 0.5). Finally, when the level of physical capital investment falls below the 8.73 threshold, the effect of public health expenditure on economic growth is significantly negative (p < 0.5), and it becomes significantly positive when it exceeds 8.73 (p < 0.5). The positive impact of public health expenditure on economic growth gradually strengthens as household consumption, employee wages, and per capita physical capital investment continue to rise. Conclusions: In an economic environment with higher household consumption, employee wages, and physical capital investment, public health expenditure will significantly promote economic growth.

Abstract:Objective Economic measures such as unemployment and gross domestic product are correlated with changes in health outcomes. We aimed to examine the effects of changes in government healthcare spending, an increasingly important measure given constrained government budgets in several European Union countries. Design Multivariate regression analysis was used to assess the effect of changes in healthcare spending as a proportion of total government expenditure, government healthcare spending as a proportion of gross domestic product and government healthcare spending measured in purchasing power parity per capita, on five mortality indicators. Additional variables were controlled for to ensure robustness of data. One to five year lag analyses were conducted. Setting and Participants European Union countries 1995–2010. Main outcome measures Neonatal mortality, postneonatal mortality, one to five years of age mortality, under five years of age mortality, adult male mortality, adult female mortality. Results A 1% decrease in government healthcare spending was associated with significant increase in all mortality metrics: neonatal mortality (coefficient −0.1217, p = 0.0001), postneonatal mortality (coefficient −0.0499, p = 0.0018), one to five years of age mortality (coefficient −0.0185, p = 0.0002), under five years of age mortality (coefficient −0.1897, p = 0.0003), adult male mortality (coefficient −2.5398, p = 0.0000) and adult female mortality (coefficient −1.4492, p = 0.0000). One per cent decrease in healthcare spending, measured as a proportion of gross domestic product and in purchasing power parity, was both associated with significant increases ( p < 0.05) in all metrics. Five years after the 1% decrease in healthcare spending, significant increases ( p < 0.05) continued to be observed in all mortality metrics. Conclusions Decreased government healthcare spending is associated with increased population mortality in the short and long term. Policy interventions implemented in response to the financial crisis may be associated with worsening population health.

Abstract:This paper uses eight waves of Australia Household, Income and Labour Dynamics data to study the issues of state dependence and the short‐run and long‐run response to health shocks on the labour market. We consider six alternative panel data binary dependent variable models with different ways of modelling labour market dynamics and individual heterogeneity. We find that the key results with regard to labour market dependence and the impacts of health shocks are sensitive to model specification and pooling of male and female samples with differences as large as sixfold. Specification analysis is conducted and favours the dynamic fixed effects logit model for separate male and female samples. Methods for evaluating dynamic response paths to a one‐time health shock for binary outcomes are also suggested and results are presented.
Health & access to care among working-age lower income adults in the Great Recession: Disparities across race and ethnicity and geospatial factors

Abstract:In the United States (US) and elsewhere, residents of low resource areas face health-related disparities, and may experience different outcomes throughout times of severe economic flux. We aimed to identify individual (e.g. sociodemographic) and environmental (e.g. region, rurality) factors associated with self-reported health and forgone medical care due to the cost of treatment in the US across the Great Recession (2008-2009). We analyzed nationally representative data (2004-2010) using the Behavioral Risk Factor Surveillance System in the US. Individual and geospatial factors (rurality, census region) were used to identify differences in self-reported health and forgone medical care due to the cost. Adjusted-analyses taking into account individual and geospatial factors among those with incomes <$50,000 identified multiple differences across time, sex, education, disability, rurality and Census Region for health. Similar analyses for forgone medical care found that those in the Recovery and the Recession were more likely to report forgone care than before the Recession. Having insurance and/or being employed (versus unemployed) was a protective factor in terms of reporting fair/poor health and having to forgo health care due to cost. Policies affecting improvements in health and access for vulnerable populations (e.g., low-income minority adults) are critical. Monitoring trends related to Social Determinants of Health, including the relationship between health and place (e.g. Census region, rurality), is necessary in efforts targeted towards ameliorating disparities.

Abstract:For the past 5 years, cultures and subcultures of excised lemon fruit tissue have been maintained in Kimble screw-cap vials (60957, no. 2-2k4), the vials being hermetically sealed with rubber-lined plastic screw caps, size 24 (Kordan, Nature 198: 867, 1963). Under these conditions, the only oxygen available to the excised tissue was that present within the vials at the time of tissue implantation just prior to being sealed with the rubber-lined caps. The culture vessels were hermetically sealed to prevent evaporation of water from the small volumes of nutrient solution employed for growing freshly excised tissue (0.15 ml/vial) and established cultures and subcultures (0.5 ml/vial). A relatively simple tissue-culture vessel is described which possesses the following characteristics: it allows for gas exchange between the interior of the vessel and the external atmosphere; it has an exceedingly low rate of water loss; and it forms a barrier against the entrance of microorganisms. The basic units comprising the modified tissueculture vessel are a Kimble (60957, no. 2-234) screw-cap vial, a size 24 plastic screw cap, a rubber liner, and a polyethylene disc. Size 24 plastic screw caps with paper liners are soaked in water until the liners can be easily removed from the caps. Any adhesive left on the caps by the liners is removed and a hole, l 6 in. (0.8 cm) in diameter, is made in the center of each cap. A Yl6-in. hole is also made in the center of a rubber liner (Aloe no. 17324, white rubber, or Aloe no. 17322, pink rubber). Discs 13f6 in. (2.06 cm) in diameter are punched out from Du Pont no. 1000 natural polyethylene film, 10 mils (0.254 mm) thick, with an arch punch. The plastic caps and rubber liners with holes in their centers are sterilized by autoclaving in petri dishes and are dried before use. The polyethylene discs are surface-sterilized by immersion in full-strength commercial bleach (4.25% NaClO) for 1 hr with frequent agitation. The sterilized discs are rinsed thoroughly with sterile distilled water, and are dried between layers of sterile filter paper. Rubber-lined solid-top plastic screw caps (Aloe no. 17260 or Arthur H. Thomas G.M.C.I. no. 24) normally employed for making the vials air-tight serve as temporary covers during autoclaving of the vials, after the addition of sterile-filtered nutrient solution, and after inoculation of the tissue onto the nutrient solution. The final step consists of replacing the solid-top plastic screw caps with the open-top polyethylenelined caps as described below.
Equilibrium Labor Market Search and Health Insurance Reform, Second Version

Abstract:We present and empirically implement an equilibrium labor market search model where risk averse workers facing medical expenditure shocks are matched with firms making health insurance coverage decisions. Our model delivers a rich set of predictions that can account for a wide variety of phenomenon observed in the data including the correlations among firm sizes, wages, health insurance offering rates, turnover rates and workers' health compositions. We estimate our model by Generalized Method of Moments using a combination of micro datasets including Survey of Income and Program Participation, Medical Expenditure Panel Survey and Robert Wood Johnson Foundation Employer Health Insurance Survey. We use our estimated model to evaluate the equilibrium impact of the 2010 Affordable Care Act (ACA) and find that it would reduce the uninsured rate among the workers in our estimation sample from about 22% in the pre-ACA benchmark economy to less than 4%. We also find that income-based premium subsidies for health insurance purchases from the exchange play an important role for the sustainability of the ACA; without the premium subsidies, the uninsured rate would be around 18%. In contrast, as long as premium subsidies and health insurance exchanges with community ratings stay intact, ACA without the individual mandate, or without the employer mandate, or without both mandates, could still succeed in reducing the uninsured rates to 7.34%, 4.63% and 9.22% respectively. Employment status, unemployment duration, and health-related metrics among US adults of prime working age: Behavioral Risk Factor Surveillance System, 2018-2019

Abstract:Background: While unemployment has been associated with poor health, few recent studies in the United States have comprehensively assessed associations between employment status (including duration unemployed) and healthcare access, health-related behaviors, and specific health outcomes. Methods: We assessed health-related metrics by employment status in 2018-2019 Behavioral Risk Factor Surveillance System respondents ages 25-54. We calculated the unadjusted prevalence and adjusted prevalence ratios to compare employed workers to respondents who were self-employed, short-term (<12 months) unemployed, long-term unemployed, and unable to work. Results: Adverse health outcomes increased with unemployment duration and were highest for those unable to work. Non-Hispanic Blacks were most likely to be unemployed or unable to work. Short-term unemployment and self-employment were associated with poor healthcare access. Health behaviors and outcomes declined with increasing duration of unemployment and were worst for those unable to work. Conclusions: In the United States, access to affordable healthcare is problematic for both the self-employed and the short-term unemployed. Short-term unemployment is a particularly important locus for intervention and provision of resources to prevent health declines that may hinder re-employment.
Public Health Insurance, Labor Supply, and Employment Lock: Effects or Data Artifacts?

Abstract:Using Social Security Administration (SSA) data on employment and Census population estimates, we re-examine the labor supply effect of a public health insurance disenrollment that took place in Tennessee in 2005. The disenrollment left about 4 percent of Tennessee's non-elderly, adult population without public coverage and was previously reported to have caused large increases in labor supply. These labor supply increases were interpreted as evidence of "employment lock." In contrast to the previous research conducted with the Current Population Survey (CPS), we find no such labor supply increases in SSA/Census data. Furthermore, our findings using CPS data contradict the "employment lock" interpretation. We find, for example, that Tennessee's employment rates increases are driven by decreases in population rather than increases in total employment. Our results narrow the range of existing estimates of the effect of Medicaid on labor supply and highlight importance of considering data error when analyzing survey data.
The influence of labor market changes on first-time medical school applicant pools

Abstract:Purpose: To explore whether the number and composition of first-time applicants to U.S. MD-granting medical schools, which have fluctuated over the past 30 years, are related to changes in labor market strength, specifically the unemployment rate and wages. Method: The authors merged time series data from 1980 through 2010 (inclusive) from five sources and used multivariate time series models to determine whether changes in labor market strength (and several other macro-level factors) were related to the number of the medical school applicants as reported by the American Medical College Application Service. Analyses were replicated across specific sex and race/ethnicity applicant pools. Results: Two results surfaced in the analyses. First, the strength of the labor market was not influential in explaining changes in applicant pool sizes for all applicants, but was strongly influential in explaining changes for black and Hispanic males. Increases of $1,000 in prevailing median wages produced a 1.6% decrease in the white male applicant pool, while 1% increases in the unemployment rate were associated with 4.5% and 3.1% increases in, respectively, the black and Hispanic male applicant pools. Second, labor market strength was a more important determinant in applications from males than in applications from females. Conclusions: Although stakeholders cannot directly influence the overall economic market, they can plan and prepare for fewer applications from males, especially those who are black and Hispanic, when the labor market is strong.

Abstract:Objective To understand trends in the long‐term services and supports (LTSS) workforce and assess workforce data as a measure of progress in shifting LTSS resources from institutional to community‐based settings. Data Sources/Study Setting Workforce data from the American Community Survey from 2008 to 2022. Study Design Measures of LTSS rebalancing and institutional and community workforce supply per 1000 persons with LTSS needs were constructed. After showing national trends over the study period, state fixed effects regressions were used to evaluate the within‐state relationship of these measures with existing measures of LTSS utilization. Workforce supply measures were compared to the percentage of state Medicaid LTSS spending spent in the community to assess their utility for across state comparisons. Each state's progress in LTSS rebalancing over the study period was then shown using workforce data. Data Collection/Extraction Methods A sample of 336,316 LTSS workers and 3,015,284 people with LTSS needs over the study period was derived from American Community Survey data. Principal Findings From 2008 to 2022, the percentage of the LTSS workforce employed in the community rose from 44% to 58%. Thirty states saw more than a 10 percentage point increase. From 2008 to 2013, the size of the community workforce expanded dramatically but has since stagnated. In contrast, the institutional workforce entered a long‐term decline beginning in 2015 that accelerated during the COVID‐19 pandemic. State fixed effects regressions showed that measures of workforce supply have a strong relationship with LTSS utilization measures for older adults, but not for younger people with disabilities. Conclusions Workforce data can serve as an effective measure of changes in LTSS utilization for older adults. This offers researchers and policymakers a useful alternative to administrative claims, bypassing threats to comparability from coding changes and the shift to managed care. Additional data is needed on workforce trends in services for younger LTSS consumers.

We created a Medicaid generosity index by simulating the share of a nationally representative sample of adults that would be eligible for Medicaid under each state's 2007 Medicaid guidelines using data from the 2007 Current Population Survey's Annual Social and Economic Supplement.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Principal Findings</h3><p>A percentage point (PPT) increase in the county unemployment rate was associated with a 1.3 PPT (95% CI: 0.9‐1.6, <i>P</i> < .01) increase in the likelihood of being uninsured and a 0.86 PPT (95% CI: 0.6‐1.1, <i>P</i> < .01) increase in unmet medical needs due to cost in states with restrictive Medicaid eligibility guidelines. Conversely, a one PPT increase in unemployment was associated with only a 0.64 PPT (<i>P</i> < .01) increase in uninsurance among states with the most generous eligibility guidelines. Among states in the fourth quartile of generosity (ie, most generous), rises in county‐level unemployment were associated with a 0.68 PPT (<i>P</i> < .10) increase in unmet medical needs due to cost—a 21% smaller decrease relative to states with the most restrictive Medicaid eligibility guidelines. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Increased access to Medicaid during the Great Recession mitigated the effects of increased unemployment on the rate of unmet medical need, particularly for adults with limited income.</p></section>

Abstract:Based on the panel data of the China Health and Retirement Longitudinal Study (CHARLS) collected from 2011 to 2018, this paper establishes a model using the instrumental variables method to investigate the effects of poor health on labor participation, labor hours, agricultural labor participation, agricultural labor hours, off-farm labor participation, and off-farm labor hours of older adults with different economic statuses. This paper conducts an empirical analysis to examine how subjective, self-rated poor health and objective poor health measured using the number of chronic diseases can affect the labor supply of older adults. The study of its influence on labor supply from the perspective of health can help to maintain the labor supply of the aged from the perspective of improving the health of the aged, provide a certain reference for the labor shortage caused by China’s aging society, and enrich the content of health economics. According to the research findings, subjective, self-rated poor health significantly reduces the labor participation of older adults. Although self-rated poor health does not affect the off-farm labor participation of older adults, it significantly reduces the likelihood of older adults engaging in agricultural labor. In addition, self-rated poor health also reduces the overall labor hours and off-farm labor hours of older adults, although no effects were observed on their agricultural labor hours. On the other hand, chronic diseases also reduce the overall likelihood of labor participation for older adults, resulting in significantly lower off-farm labor participation, although no effects were observed on their agricultural labor participation. The number of chronic diseases found in older adults does not affect their off-farm labor hours, but it does increase the hours they invest in agricultural labor.

Abstract:The authors study the distributional consequences of the COVID-19 pandemic’s impact on employment, both during the onset of the pandemic and over subsequent months. Using cross-sectional and matched longitudinal data from the Current Population Survey, they show that the pandemic has exacerbated pre-existing inequalities. Although employment losses have been widespread, they have been substantially larger—and more persistent—in lower-paying occupations and industries. Hispanics and non-White workers suffered larger increases in job losses, not only because of their over-representation in lower-paying jobs but also because of a disproportionate increase in their job displacement probability relative to non-Hispanic White workers with the same job background. Gaps in year-on-year job displacement probabilities between Black and White workers have widened over the course of the pandemic recession, both overall and conditional on pre-displacement occupation and industry. These gaps are not explained by state-level differences in the severity of the pandemic nor by the associated response in terms of mitigation policies. In addition, evidence suggests that older workers have been retiring at faster rates.
