Abstract:We have established a method for quantitative analysis of the deuterium contents (D/H) at the phenyl, methine, benzyl, N-methyl and methyl groups of l-ephedrine/HCl, d-pseudoephedrine/HCl and methamphetamine/HCl by 2H NMR spectroscopy. Comparison of the 5 position-specific D/H values of l-ephedrine/HCl and d-pseudoephedrine/HCl prepared by three methods (chemical synthesis, semichemical synthesis, and biosynthesis) showed that chemically synthesized ephedrines and semisynthetic ephedrines have highly specific distributions of deuterium at the methine position and at the benzyl position, compared with the other positions. The classification of several methamphetamine samples seized in Japan in terms of the D/H values at these two positions clearly showed that the methamphetamine samples had been synthesized from ephedrines extracted from Ephedra plants or semisynthetic ephedrines but not from synthetic ephedrine. This isotope ratio analysis method should be useful to trace the origins of seized methamphetamine in Southeast Asia.
Investigation of essential trace and toxic elements in biological samples (blood, serum and scalp hair) of liver cirrhotic/cancer female patients before and after mineral supplementation

Abstract:Background & aims: The aim of present study was to compare the levels of essential trace and toxic elements in biological samples (blood, serum and scalp hair) of female liver cirrhotic/cancer patients (n = 132), of two age groups (20-45 years) and (46-60 years), before and after sixty days treatment with mineral supplementation. For comparison purpose, same biological samples were also collected from healthy female subjects (n = 75) of same age groups. Methods: The biological samples were oxidized by 65% HNO(3): 30% H(2)O(2) (2:1) in microwave oven. The digests of all biological samples were analyzed for arsenic (As), cadmium (Cd), selenium (Se) and zinc (Zn) by electrothermal atomic absorption spectrometry. Results: The levels of Se and Zn were lower in liver cirrhotic/cancer patients as compared to healthy individuals (p < 0.001). The patients of liver cirrhosis/cancer have two folds higher As and Cd levels in biological samples as related to age matched referents. Moreover, negative correlation was observed between essential and toxic elements. Conclusions: The pathogenesis of liver cirrhosis/cancer has been associated with changes in the balance of certain essential trace and toxic elements. It was observed that the status of Se and Zn in addition to some biochemical parameters was improved in biological samples of both groups of patients after sixty days treatment with mineral supplementation.

Abstract:Four newborn infants with megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS) were identified at Children's Hospital of Pittsburgh. These cases provide additional insight into the syndrome and broaden its spectrum. This report includes MMIHS in an infant boy, one long-term survivor, an apparently related complication of neonatal obstructive volvulus, evidence of small intestinal hypoperistalsis, and histopathologic findings as follows: (1) apparently increased numbers of ganglion cells in early biopsies and normal or even decreased numbers of ganglion cells in later biopsies probably due to bowel dilatation; and (2) in two of three infants at autopsy, there were many nerve trunks (a neuromalike layer in one), and there was elastosis of the urinary bladder.

Abstract:INTRODUCTION UK Exercise Referral Systems (ERS) have been developed to encourage physical activity in the general population. This systematic review investigated the effectiveness and cost-effectiveness of ERS. Identification of factors influencing ERS uptake, adherence and success were also investigated. METHODS Studies were identified from Medline, Cochrane and Pascal and bibliographies of relevant papers. Interventions providing access to ERS (randomized controlled trials or controlled trials), experimental or qualitative studies, and meta-analyses were included. RESULTS Twenty six studies met the inclusion criteria. Compared with usual care, ERS showed an increased number of participants who achieved 90-150 minutes of physical activity of at least moderate intensity per week. However, no significant difference in long-term outcomes (e.g., quality of life, body mass index, glycated haemoglobin, anxiety) were identified between ERS and comparator groups. Cost-effectiveness analysis suggested that ERS were more cost-effective for participants with co-morbid medical conditions. A higher adherence rate was associated with better effectiveness of ERS. DISCUSSION Limited evidence supports the efficacy of ERS to increase physical activity or improve health outcomes. This evidence-based analysis could support the development of effective ERS in France.

Abstract:Imbalances in the concentrations of trace metals have become an increasingly recognized source of infirmity worldwide particularly in the development of ischemia heart disease (IHD). Present study is intended to analyze the concentrations of Ca, Cd, Co, Cr, Cu, Fe, K, Li, Mg, Mn, Na, Pb, Sr, and Zn in the blood and scalp hair of the patients and counterpart controls by flame atomic absorption spectrometry after wet-acid digestion. On the average, Cd, Co, Cr, Fe, K, Li, Mn, Na, and Pb revealed significantly elevated concentrations in the blood of the patients compared with the controls (p < 0.05), whereas mean levels of Ca, Cd, Fe, K, Li, Pb, and Sr in the scalp hair were significantly higher in the patients than the controls (p < 0.05). Most of the metals exhibited noticeable disparities in their concentrations based on gender, abode, dietary/smoking habits, and occupations of both donor groups. The correlation study and multivariate statistical analyses revealed some significantly divergent associations and apportionment of the metals in both donor groups. Overall, comparative variations of the metal contents in blood/scalp hair of the patients were significantly different than the controls; thus, evaluation of trace metals status may be indicative of pathological disorders, such as IHD.

Abstract:Illicit drug use has become a global issue, posing significant health, socioeconomic, and cultural risks. The study examined essential trace metals: selenium, zinc, and copper in blood concentrations, as well as in serum and scalp hair samples, from 240 male drug-abuse subjects/patients aged 18–45, categorized into three age groups. The study compared 45 healthy subjects of the same age group using an acid digestion method supported by a microwave oven during sample preparation. The technique of atomic absorption spectrometry was employed to identify essential and toxic elements, utilizing certified reference materials for accuracy. According to a recent study, plasma zinc and selenium concentrations in drug abusers are lower than those in referent subjects, potentially increasing vulnerability to infection due to poor nutritional status or other contaminants.

Abstract:Selenium (Se) has an antagonistic effect on the toxicity of mercury (Hg). This study aimed to investigate the influence of Se supplementation on Hg levels in long-term chronically exposed metropolitan residents in China. The levels of Se and Hg in human hair samples from 1373 metropolitan residents in China were analyzed and differences in sex, age, and region were evaluated. Higher concentrations of Hg were found in the hair of males, 60–74 years of age, and South and East Chinese residents. Participants with Se supplementation exhibited statistical lower hair Hg levels ( p < 0.01) compared to those without additional Se supplementation. The Se:Hg molar ratio was used to evaluate the detoxification ability of Se. The proportion of Se:Hg molar ratios of < 1 were 25.40% and 3.83% for participants belong to G1 (without additional Se supplementation) and G5 (Se supplementation for more than 24 months). These results indicate that long-term Se supplementation may enhance the body's ability to bind to or detoxify Hg, leading to a lower proportion of participants with imbalanced Se:Hg ratios. It is essential to consider targeted Se supplementation or dietary adjustments to optimize the Se:Hg balance and mitigate the risks associated with Hg exposure in Chinese residents.

Abstract:Wandering spleen is a rare condition in which the spleen is attached by a long, vascular pedicle and lacks its usual peritoneal attachments and supporting ligaments. This condition predisposes the spleen to torsion and infarction. We report the case of a 2-year-old boy with a history of intermittent abdominal pain and early satiety who presented with abdominal pain and severe gastric distention. A work-up including computed tomographic and ultrasonographic imaging suggested a wandering spleen. The diagnosis was confirmed during laparoscopy, and splenopexy was performed by anchoring the spleen in a retroperitoneal pocket in the left upper quadrant at the level of the tenth rib. To our knowledge, this is the first reported case of minimally invasive splenopexy for wandering spleen that involves the creation of a retroperitoneal pocket without the use of mesh. The authors believe that this is a safe and effective method that takes advantage of laparoscopy and avoids the risk of infection and complications associated with the use of synthetic material.
Evaluation of chromium, cobalt and manganese in biological samples (scalp hair, blood, and urine) of Pakistani viral hepatitis (A-E) patients and controls

Abstract:Background: The aim of the present study was to compare the level of chromium (Cr), cobalt (Co), and manganese (Mn) in biological samples (blood, urine, and scalp hair) of patients suffering from different types of viral hepatitis (A, B, C, D, and E; n = 521) of both genders, ages ranging from 31 - 45 years. For comparative study, 255 age-matched control subjects of both genders residing in the same city were selected as referents. Methods: The digests of all biological samples were analysed for Cr, Co, and Mn by electrothermal atomic absorption spectrometry (ETAAS). The validity and accuracy of the methodology was checked by using certified reference materials (CRMs) and compared with those values obtained by conventional wet acid digestion method on same CRMs. Results: The results of this study showed that the mean values of Cr, Co, and Mn were higher in blood and scalp hair samples of hepatitis patients than in age-matched control subjects. The urinary levels of these elements were found to be higher in the hepatitis patients than in the age-matched healthy controls (p <0.001). Conclusions: These results are consistent with literature-reported data, confirming that the overload of these trace elements can directly cause lipid peroxidation and eventually hepatic damage.

Abstract:Background: The relationship between exposure to heavy metals and the development of cancer has been a subject of extensive research. Trace elements, both essential and non-essential, play significant roles in various biological processes, with imbalances potentially contributing to carcinogenesis. This study aims to investigate serum concentrations of selected metals in cancer patients and compare these levels with those of a healthy control group, highlighting the potential link between metal exposure and cancer risk. Objective: To quantify and compare the serum concentrations of sodium (Na), calcium (Ca), iron (Fe), cadmium (Cd), and arsenic (As) in cancer patients and a control group, thereby assessing the potential association between elevated levels of specific metals and cancer incidence. Methods: This cross-sectional study analyzed blood samples from cancer patients (n=30) and a matched control group (n=30) from the Jamshoro and Hyderabad areas. Inductively Coupled Plasma Mass Spectrometry (ICP-MS) was utilized to measure serum concentrations of Na, Ca, Fe, Cd, and As. Participants' demographic data, including age, gender, place of residence, degree of education, cancer type, and water sources, were collected through questionnaires. Statistical analyses were conducted to compare metal concentrations between groups, with P-values 0.05), with an average concentration exceeding 32.04 μg/L. Cd levels were also higher in the cancer group (0.0046 ppm) compared to controls (0.0000 ppm), contrary to expectations. Conversely, no significant difference was found in As levels between groups. Serum Fe concentrations were substantially higher in cancer patients (P > 0.4949), with Ca levels in cancer patients also elevated (3.302 ppm). Conclusion: The study found significant discrepancies in serum metal concentrations between cancer patients and healthy controls, suggesting an association between elevated levels of certain metals and increased cancer risk. These findings emphasize the need for further investigation into the role of metal exposure in carcinogenesis and the potential for incorporating metal concentration assessments into cancer risk evaluation protocols.
The Relationship between Serum Selenium and Zinc with Gastroesophageal Cancers in the Southeast of Iran

Abstract:Background: Selenium (Se) and zinc (Zn) have antioxidant and anticancer properties. Objective: The aim of this study was to evaluate the serum levels of Se and Zn and the correlation between the levels of these two elements with risk of incidence of esophageal cancer (EC) and gastric cancer (GC). Materials and methods: In a case-control study, we selected sixty patients with GC or EC as the intervention group and 120 age-matched individuals as the control group. Exclusion criteria were the individuals with kidney and liver failure and the consumer of dietary supplements such as Se and Zn. Measurement of serum Se was done in a graphite furnace system and atomic absorption device of Varian and of serum Zn was done by a flame photometer system (flame) and atomic absorption device of Varian. Results: In thirty patients of ECs, 90% were squamous cell carcinoma and 10% adenocarcinoma, and out of thirty patients of GCs, 90% were intestinal type and 10% diffuse type. The level of two elements in cancer patients was lower than the control group (P < 0.05). There was no significant different between two cancer groups for level of Se and Zn, but there was a significant difference between the control group with two other groups. Conclusion: Our study confirmed the findings from previous prospective studies and randomized trials that reducing of lower levels of Se and Zn can effect on incidence of cancer.

Abstract:Background: Deficiency or excess of trace elements can induce metabolic disorders and dysregulate cell growth, and even lead to mutations and tumorigenesis. Many reports have indicated a direct association between micronutrient deficiency and cancer mortality. Prostate cancer is the sixth most common cancer among men in Saudi Arabia, yet there are few studies of the association between trace element levels and prostate cancer in the country. Objective: This study aimed to explore the association between concentrations of select hair trace elements, including selenium (Se), zinc (Zn), copper (Cu), manganese (Mn), and iron (Fe), as long-term indicators, and tumorigenesis of prostate cancer in Saudi Arabia. 1.2 Patients and Methods: The study included 58 patients with prostate cancer, 64 with benign prostatic hyperplasia (BPH), and 52 healthy controls. Full history and clinical data were recorded for all subjects. Hair samples were collected from the nape of all subjects, and levels of Se, Zn, Cu, Mn, and Fe were analyzed by inductively-coupled plasma mass spectrometry. Results: Hair Se and Zn levels of prostate cancer patients were significantly lower compared to BPH and healthy groups whereas Cu, Mn, and Fe levels were significantly high. Hair Se and Zn levels were significantly lower in metastatic prostate cancer patients than in localized cancer patients whereas mean hair levels of Cu, Mn, and Fe were not significantly different among these patients. Conclusion: Prostate cancer may be associated with trace element-mediated metabolic disorders. Low levels of Se and Zn and high levels of Cu, Mn, and Fe appear to be associated with its tumorigenesis. Additional prospective studies are warranted to confirm the inverse correlation between Se and Zn levels and prostate cancer.

Abstract:The complex mer-[Co(III)(L(1)Npy)(2)](+) (1') where the L(1)Npy(-) is the tridentate 3-[(2-pyridyl)methylimino]butan-2-one oximate ligand, gives alkyl-cobalt derivatives after reduction with NaBH(4)/Pd(2+) to the Co(I) and alkylation. The formation of the cobalt-carbon bond is accompanied by the reduction to the amino form of one or both imino ligands (depending on the experimental conditions) initially present in 1'. In one series of experiments, complexes of the type fac-[RCo(III)(L(1)Npy)(H-L(1)NHpy)](+) (R = Me, i-Pr, CH(2)Cl, CH(2)Br, CH(2)CF(3), and Bz) were obtained, in which only one of the two ligands was reduced to the amino form (H-L(1)NHpy). The saturation of one azomethine group causes the products to assume a fac configuration and induces the formation of one asymmetric carbon and one asymmetric nitrogen center in the chelating system. When an excess of reducing agent is used, both azomethine groups may be saturated, causing the introduction of one pair of chiral carbons and one pair of chiral nitrogens. Two isomers of the methyl derivative [MeCo(III)(L(1)NHpy)(H-L(1)NHpy)](+) were isolated. The X-ray analysis reveals that these isomers differ from one another in configuration of the C and N chiral centers. Possible reaction mechanisms leading to these different types of complexes are proposed.
Mercury (Hg) and Selenium (Se) Distribution in Human Brain Tissue Using Synchrotron Micro-X-ray Fluorescence (μ-XRF)

Abstract:Mercury is a well-recognized environmental contaminant and neurotoxin, having been associated with a number of deleterious neurological conditions including neurodegenerative diseases, such as Alzheimer's Disease. To investigate how mercury and other metals behave in the brain, we used synchrotron micro-x-ray fluorescence to map the distribution pattern and quantify concentrations of metals in human brain. Brain tissue was provided by the Rush Alzheimer's Disease Center and samples originated from individuals diagnosed with Alzheimer's disease and without cognitive impairment. Data were collected at the 2-ID-E beamline at the Advanced Photon Source at Argonne National Laboratory with an incident beam energy of 13 keV. Course scans were performed at low resolution to determine gross tissue features, after which smaller regions were selected to image at higher resolution. The findings revealed (1) the existence of mercury particles in the brain samples of two subjects; (2) co-localization and linear correlation of mercury and selenium in all particles; (3) co-localization of these particles with zinc structures; (4) and association with sulfur in some of these particles. These results suggest that selenium and sulfur may play protective roles against mercury in the brain, potentially binding with the metal to reduce the induced toxicity, although at different affinities. Our findings call for further studies to investigate the relationship between mercury, selenium, and sulfur, as well as the potential implications in Alzheimer's Disease and related dementias. Chronic exposure to mercury increases arrhythmia and mortality post-acute myocardial infarction in rats

Abstract:Introduction: Mercury (Hg) is a heavy metal that causes a variety of toxic effects in eukaryotic cells. Previous studies have reported detrimental effects of mercury toxicity in the cardiovascular system. Given the importance of understanding the relationship between Hg and cardiovascular disease, we sought to investigate if the Hg could worsen the myocardial repercussions following ischemic injury. We demonstrated that once mercury toxicity is established, it can influence the outcome of myocardial infarction (MI). Methods: Male Wistar rats received intramuscular injections of either saline (NaCl 0.9%) or mercuric chloride (HgCl2, first dose of 4.6 μg/kg, and subsequent doses of 0.07 μg/kg/day) for 4 weeks. Three weeks post-exposure, we induced transmural infarction in the left ventricle free wall through coronary artery occlusion surgery. Results: ECG recordings obtained from MI groups demonstrated alterations in the rhythm of the heartbeat/heart electrical activity, as expected, including ventricular extrasystoles and ventricular tachycardia. However, the MI group exposed to Hg (MI-Hg) exhibited augmented ventricular extrasystoles and ventricular tachycardia compared to the MI group. Also, Basckó coefficient revealed that the arrhythmic events-after MI-were aggravated by Hg exposure. Discussion: Our results indicate that the significantly increased mortality in MI-Hg groups when compared to MI (21%, MI vs 32%, MI-Hg) is correlated with greater occurrence of arrhythmias. In conclusion, this study further supports the idea that exposure to mercury (Hg) should be recognized as a significant risk factor that exacerbates the impact of cardiac ischemic injury, potentially leading to an increased mortality rate among patients experiencing acute MI.
Mercury level in biological samples of dentists in Iran: a systematic review and meta-analysis

Abstract:Exposure to mercury is an important risk to dentists health. The aim of the present study was to assess the pooled mean mercury level (MML) in the urine, blood, nail, and hair of Iranian dentists (IDs) through the meta-analysis technique. Comprehensive and systematic searches were performed in main local databases including SID, Magiran, Iran medex, and ISC as well as internationally available databases including Embase, PubMed and Scopus for all the relevant studies up to 2018. In order to prevent bias in this study and identify eligible studies, various steps of the study was performed independently by two researchers. Out of 13 studies in the meta-analysis process which included 1499 IDs, the mean of the mercury level in the urine, nail, and blood was estimated to be 6.29 (95% CI: 2.61–9.97, I-square: 62.7%, P: 0.006), 3.54 (95% CI: 2.81–4.28, I-square: 0.0%, P: 0.968), 11.20 (95% CI: 2.28–20.13, I-square: 59.9%, P: 0.082), respectively. The mean mercury level (MML) in the biological samples of IDs was higher than the standard of World Health Organization (WHO). So, in accordance with Article 10 of the European Union Regulations (EUR), in the context of the Minamata Convention (MC) on Dental Amalgam (DA), in order to avoid the dangers of mercury exposure in dentists, it is necessary for Iran and other countries to approve laws and to implement a national plan to reduce mercury levels and replace the appropriate materials.

Abstract:Relationships between Se and Hg in erythrocytes, and between these indices and intakes of fish and other foods, were studied as an adjunct to the British National Diet and Nutrition Survey (NDNS) of young people aged 4–18 years. Hg was measured in 965 packed erythrocyte samples by inductively coupled plasma mass spectrometry. Fe measurements permitted the calculation of whole-blood Hg. Erythrocyte and plasma Se, and 7d weighed dietary intake estimates, were available. Erythrocyte Hg was positively skewed, normalised by log-transformation. It was 20% higher in girls than boys (3·17 v. 2·65nmo/, P=0·004), and increased with age in boys but not girls. It was directly and strongly correlated with erythrocyte or plasma Se. Hg and Se concentrations were directly correlated with fish intake. Certain other food groups were also directly correlated with Se and Hg concentrations, but less strongly than for fish. The strength and consistency of the relationship between erythrocyte Hg and Se suggests an important chemical link. Previous studies suggest that Se protects against the toxicity of Hg, and that fish is an important source of both. No toxic levels of Hg were found, which is reassuring because of the known health benefits of fish consumption, especially oily fish. Hg intakes need to be monitored, especially in women of child-bearing age, to ensure that Food Standards Agency guidelines are met.

Study of Changes in Serum Copper Level in Patients with Acute Myocardial Infarction

Abstract:Acute myocardial infarction (AMI) commonly known as heart attack is defined pathologically as the irreversible death of myocardial cells caused by ischemia. Risk factors include high blood pressure, smoking, diabetes, lack of exercise, obesity, high blood cholesterol and excessive alcohol intake. The major cause of acute MI is coronary atherosclerosis with superimposed luminal thrombus, which accounts for more than 80% of all infarcts. Micronutrients and trace elements are very essential for normal functioning of the body. Even though they are required in very small amount, an alteration in the level of this element may lead to serious diseases like CAD and its consequences. The injury induced by reperfusion of the ischemic myocardium could result partially from the cytotoxic effects of oxygen free radicals. Copper is involved in several of the reactions in the protection from free radical damage. So, this study was designed to evaluate serum cupper level in AMI patients. This case-control study was conducted in the Department of Biochemistry, Mymensingh Medical College, Mymensingh, Bangladesh from July 2016 to June 2017. Total number of subjects was 120 in number. Among them 60 were diagnosed case of acute myocardial infarction and 60 were apparently healthy volunteers. Data were collected using pre-designed data collection sheets. After proper counseling informed written consent was taken from the study population. The study revealed that mean serum copper level was higher in case group as compared to control group. The mean±SD values of copper were 105.44±24.15μg/dl and 146.49±23.52μg/dl in control and case group respectively. The level of significance was 0.001 (p<0.05). After analyzing the results of the study it is concluded that serum copper level was significantly higher in Acute Myocardial Infarction patients than normal individuals. Therefore, estimation of serum copper level in AMI patients might be useful to take appropriate measure to prevent free radical induced reperfusion injury.
