Abstract:Aim To evaluate the efficacy of guided bone regeneration (GBR) for the treatment of peri‐implant dehiscence defects using a synthetic bone substitute (SBS) or a deproteinized bovine bone mineral (DBBM) as a bone substitute. Methods Patients with expected dehiscence defects following implant placement were randomized to use either SBS or DBBM together with a bioabsorbable collagen membrane over dehiscenced implant surfaces aimed for GBR. The changes in the bone defect size were measured before the GBR procedure and 6 months after implant placement at the re‐entry surgery. Secondary outcomes included peri‐implant health outcomes, implant cumulative survival rates, bone level changes, and patient‐reported outcomes (PROMs) at prosthesis delivery and 1‐year follow‐up. Results Of the 49 included patients, 24 were treated with SBS and 25 with DBBM. In the SBS group, the defect height (DH) at implant insertion was 5.1 ± 2.6 mm and was reduced at re‐entry to 1.3 ± 2.0 mm (74.5%). In the DBBM group, the respective changes in DH were 4.1 ± 1.7 mm and 1.5 ± 1.9 mm (63.4%). These differences were not statistically significant (p = 0.216). The complete defect resolution rate was also comparable in both groups without statistical difference (62.5% of patients (15/24) vs. 44% of patients (11/25)). Overall, the marginal bone levels remained stable during the 1‐year follow‐up in both groups. Conclusion The SBS is noninferior to DBBM for simultaneous GBR to implant placement at implant sites with buccal dehiscences in terms of defect resolution and evaluated secondary outcomes (KCT0008393 − this clinical trial was not registered before participant recruitment and randomization).

Acceleration of De Novo Bone Formation with a Novel Bioabsorbable Film: a Histomorphometric Study in Vivo

Abstract:OBJECTIVE:Different types of barrier membranes have been used in periodontal applications for the technology of guided tissue regeneration (GTR). The aim of this study was to characterize the biological effect of novel calcium alginate film (CAF) on bone tissue regeneration by using rabbit mandible defects model.METHODS:A critical size defect (5 mm in diameter) was created in the bilateral corner of mandible of 45 adult rabbits. The defects were covered with CAF served as the experimental group, or conventional collagen membrane (CCM) or left empty as the controls. Animals were killed after 1, 2, 4, 6 and 8 weeks. Morphological and histomorphometric studies were performed to evaluate their bone regeneration pattern and biological effects.RESULTS:Histological sections showed that bone regeneration pattern was centripetal in growth from defect rim. The quantitative histometry analysis revealed a significantly greater percentage of newly generated bone in CAF defects than that in CCM defects and empty defects from 2 to 6 weeks post-operation (P < 0.01). After 6 and 8 weeks, significantly more mature lamella bone had formed with CAF than with CCM. Empty control defects showed bone formation starting from the defect margins and incomplete healing even after 8 weeks.CONCLUSION:The CAF guided early bone growth and appeared more effective as a bioabsorbable GTR membrane than CCM. This study with mandible defect model suggests that bone defects augmented with CAF may offer most promising results from a histological and histomorphometric perspective.

Abstract:Objectives: To evaluate the effect of guided bone regeneration (GBR) in combination with or without deproteinized bovine bone mineral (DBBM) and/or an enamel matrix derivative (EMD) on the healing of critical-size calvarial defects.Material and methods: Forty rats were used. In all animals, a standardized critical-size calvarial defect was created surgically. The animals were randomly allocated into 4 groups of 10 animals each. Group A: One calvarial defect was left untreated, while the galeal and the cerebral aspect of the contralateral defect were covered with a bioresorbable membrane (GBR). Group B: One calvarial defect was filled with EMD, while the contralateral defect was treated with GBR and EMD. Group C: One defect was filled with DBBM, while the contralateral defect was treated with combination of GBR and DBBM. Group D: One defect was filled with DBBM combined with EMD, while the contralateral defect was treated with combination of GBR, DBBM and EMD. The healing period was 4 months. Five specimens from each group were macerated and the length, the width and the vertical dimension (thickness) of the remaining defect were evaluated by a stereomicroscope. The remaining specimens in each group were analyzed histologically.Results: The defects of the macerated specimens that were left untreated or were treated only by EMD, DBBM and combination of EMD and DBBM did not present predictably complete healing of the defects. All the defects where GBR was applied alone or combined with DBBM and/or EMD presented always complete healing (P<0.05). The combined use of GBR with EMD and/or DBBM did not offer any significant advantage above GBR alone in terms of healing of the length and the width of the defect. However, the vertical dimension of the defect was significantly higher (P<0.05) in the GBR-treated specimens of Groups C and D. The histological analysis supported these findings.Conclusion: he predictability of bone formation in critical-size defects depends mainly on the presence or absence of barrier membranes (GBR). The combined use with deproteinized bovine bone mineral and/or enamel matrix proteins did not significantly enhance the potential for complete healing provided by the GBR procedure.

Abstract:Dense biomaterial plays an important role in bone replacement. However, it fails to induce bone cell migration into graft material. In the present study, a novel bone graft substitute (BGS) consisting of porous gradient hydroxyapatite/zirconia composite (PGHC) and gelatin/chitosan slow-release hydrogel containing bone morphogenetic protein 2 and bone mesenchymal stem cells was designed and prepared to repair lumbar vertebral defects. The morphological characteristics of the BGS evaluated by a scanning electron microscope showed that it had a three-dimensional network structure with uniformly distributed chitosan microspheres on the surfaces of the graft material and the interior of the pores. Then, BGS (Group A), PGHC (Group B), or autologous bone (Group C) was implanted into lumbar vertebral body defects in a total of 24 healthy rhesus monkeys. After 8 and 16weeks, anteroposterior and lateral radiographs of the lumbar spine, microcomputed tomography, histomorphometry, biomechanical testing, and biochemical testing for bone matrix markers, including Type I collagen, osteocalcin, osteopontin, basic fibroblast growth factor, alkaline phosphatase, and vascular endothelial growth factor, were performed to examine the reparative efficacy of the BGS and PGHC. The BGS displayed excellent ability to repair the lumbar vertebral defect in rhesus monkeys. Radiography, microcomputed tomography scanning, and histomorphological characterization showed that the newly formed bone volume in the interior of the pores in the BGS was significantly higher than in the PGHC. The results of biomechanical testing indicated that the vertebral body compression strength of the PGHC implant was lower than the other implants. Reverse-transcription polymerase chain reaction and western blot analyses showed that the expression of bone-related proteins in the BGS implant was significantly higher than in the PGHC implant. The BGS displayed reparative effects similar to autologous bone. Therefore, BGS use in vertebral bone defect repair appears promising.

Abstract:A barrier membrane (BM) is essential for guided bone regeneration (GBR) procedures. Absorbable BMs based on collagen have been widely applied clinically due to their excellent biocompatibility. The extracellular matrix (ECM) provides certain advantages that can compensate for the rapid degradation and insufficient mechanical strength of pure collagen membrane due to the porous scaffold structure. Recently, small intestinal submucosa (SIS), one of the most widely used ECM materials, has drawn much attention in bone tissue engineering. In this study, we adopted multilaminate SIS (mSIS) as a BM and evaluated its in vivo and in vitro properties. mSIS exhibited amultilaminate structure with a smooth upper surface and a significantly coarser bottom layer according to microscopic observation. Tensile strength was 13.10 +/- 2.56 MPa. In in vivo experiments, we selected a rabbit mandibular defect model and subcutaneous implantation to compare osteogenesis and biodegradation properties with one of the most commonly used commercial collagen membranes. mSIS was retained for up to 3 months and demonstrated longer biodegradation time than commercial collagen membrane. Quantification of bone regeneration revealed significant differences in each group. Micro-computed tomography (micro-CT) revealed that the quantity and maturity of bones in the mSIS group were significantly higher than those in the blank control group (P < 0.05) and were similar to those in a commercial collagen membrane group (P > 0.05) at 4 and 12 weeks after surgery. Hematoxylin and eosin staining revealed large amounts of mature lamellar bone at 12 weeks in mSIS and commercial collagen membrane groups. Therefore, we conclude that mSIS has potential as a future biocompatible BM in GBR procedures.
[A Study on the Repair of Bone Defects with Deproteinized Bone Surrounded by Titanium Mesh and Osteoblasts].

Abstract:To investigate the ability of composite graft of osteobalsts and deproteinized bone-titanium mesh (DPB-TM) scaffold to repair cranial bone defect. 30 rabbits were randomly divided into 3 groups. The passage 3 fetal rabbit osteoblasts were seeded into porous DPB-TM scaffolds at the density of 5 x 10(6) ml(-1) as the experimental group. The same defects were respectively reconstructed by DPB-TM or osteoblasts as the control groups. After 12 weeks, the result was evaluated by three-dimensional computed tomographic scanning, gross inspection, scanning electron microscopy, histological examination and mechanics test, respectively. In the experimental group, bone trabecula was observed to pass the defect and interface was mixed. No demarcation between the region of the bone defect and the normal bone was observed. There was plenty of new bone on the scaffold. Part of the scaffold was absorbed. In view of mechanics, the intensity of artificial bone (18.93+/-1.12 MPa) was higher than that of normal bone (16.96+/-1.60 MPa) (P<0.05). In the control groups, only fibrous tissue was observed in the defect region, there was no new bone formation. The tissue engineering bone constructed by osteoblasts and DPB-TM scaffold can be applied to the repair of bone defect.
Membrane barriers for guided bone regeneration: An overview of available biomaterials

Abstract:Dental implants revolutionized the treatment options for restoring form, function, and esthetics when one or more teeth are missing. At sites of insufficient bone, guided bone regeneration (GBR) is performed either prior to or in conjunction with implant placement to achieve a three-dimensional prosthetic-driven implant position. To date, GBR is well documented, widely used, and constitutes a predictable and successful approach for lateral and vertical bone augmentation of atrophic ridges. Evidence suggests that the use of barrier membranes maintains the major biological principles of GBR. Since the material used to construct barrier membranes ultimately dictates its characteristics and its ability to maintain the biological principles of GBR, several materials have been used over time. This review, summarizes the evolution of barrier membranes, focusing on the characteristics, advantages, and disadvantages of available occlusive barrier membranes and presents results of updated meta-analyses focusing on the effects of these membranes on the overall outcome.
Bone Regeneration with Bilayer Bone Augmentation Technique for the Treatment of Dehiscence-Type Defects Around Implants: A Preliminary Study in Dogs

Abstract:Purpose: The purpose of this study was to histomorphometrically compare the effects of the bilayer bone augmentation technique for the treatment of dehiscence-type defects around implants and evaluate the role as a membrane of the xenogeneic bone positioned as the outer layer in the bilayer bone augmentation technique. Materials and methods: Four standardized dehiscence defects measuring 5 mm in height from the crestal bone, 3 mm in width mesiodistally, and 4 mm in depth from the surface of the buccal bone were prepared on each mandible unilaterally in three dogs, and one implant was placed per defect, where each defect was treated with autograft, xenograft, the bilayer bone augmentation technique, or negative control without a membrane. The animals were sacrificed after an 8-week healing interval for histomorphometric analyses. The measurements of newly formed bone height, newly formed bone height contacting the implant, newly formed bone area, and the width of newly formed bone were made using incandescent and polarized light microscopy. Results: Bone height and newly formed bone height contacting the implant in the autograft group were higher than in the xenograft, bilayer bone augmentation, and control groups. Newly formed bone area in the bilayer bone augmentation and autograft groups was higher than in the xenograft and control groups. The width of newly formed bone at 4.5 mm apically from the implant shoulder was greater in the xenograft and bilayer bone augmentation groups than in the control and autograft groups. However, the differences between the groups in bone height, newly formed bone height contacting the implant, newly formed bone area, and width of newly formed bone were not statistically significant in the histomorphometric examinations (P < .05). Data were verified with the Kruskal-Wallis test. Conclusion: The results of this study show the osteogenic effect of autogenous bone and the effect of mechanical support for prolonged space maintenance of xenogeneic bone for the treatment of dehiscence-type defects around implants. Further studies with a larger sample size are needed to confirm the efficacy of the bilayer bone augmentation technique.
Comparative In Vivo Analysis of the Integration Behavior and Immune Response of Collagen-Based Dental Barrier Membranes for Guided Bone Regeneration (GBR)

Abstract:Collagen-based resorbable barrier membranes have been increasingly utilized for Guided Bone Regeneration (GBR), as an alternative to non-resorbable synthetic membranes that require a second surgical intervention for removal. One of the most important characteristics of a resorbable barrier membrane is its mechanical integrity that is required for space maintenance and its tissue integration that plays a crucial role in wound healing and bone augmentation. This study compares a commercially available porcine-derived sugar-crosslinked collagen membrane with two non-crosslinked collagen barrier membranes. The material analysis provides an insight into the influence of manufacturing on the microstructure. In vivo subcutaneous implantation model provides further information on the host tissue reaction of the barrier membranes, as well as their tissue integration patterns that involve cellular infiltration, vascularization, and degradation. The obtained histochemical and immunohistochemical results over three time points (10, 30, and 60 days) showed that the tissue response to the sugar crosslinked collagen membrane involves inflammatory macrophages in a comparable manner to the macrophages observed in the surrounding tissue of the control collagen-based membranes, which were proven as biocompatible. The tissue reactions to the barrier membranes were additionally compared to wounds from a sham operation. Results suggest wound healing properties of all the investigated barrier membranes. However, the sugar-crosslinked membrane lacked in cellular infiltration and transmembraneous vascularization, providing an exclusive barrier function in GBR. Moreover, this membrane maintained a similar swelling ratio over examined timepoints, which suggests a very slow degradation pattern and supports its barrier function. Based on the study results, which showed biocompatibility of the sugar crosslinked membrane and its stability up to 60 days post-implantation, it can be concluded that this membrane may be suitable for application in GBR as a biomaterial with exclusive barrier functionality, similar to non-resorbable options.
Effects of a mechanical barrier on the integration of cortical onlay bone grafts placed simultaneously with endosseous implant

Abstract:Background: Previous experimental studies on onlay bone graft integration have shown either advantages or disadvantages to the use of mechanical barriers. This indicates that the role played by the biologic properties of transplanted bone and membrane in graft revascularization and bone remodeling has not yet been established. The outcomes regarding osseointegration of titanium dental implants applied in such a condition are still contradictory. Purpose: The rabbit's radius model that is grafted onto the mandibular lower border and covered by membrane can reproduce a challenging experimental situation to preliminarily study the factors involved in osseointegration under deprived blood vessels source. Materials and methods: Fourteen New Zealand White rabbits had a 2.5-cm segment of the right radius osteoectomized and fixed onto the right mandibular lower border using titanium screws. Two screw-shaped titanium implants (2.5 mm wide yen 2.5 mm long) were installed 7 mm apart in the mid length of the grafted bone. In experimental sites, the graft with the implants and graft-host bone junction were covered by expanded polytetrafluoroethylene (e-PTFE) membrane; control sites were left uncovered. Eight animals from the experimental group and six animals from the control group were sacrificed at 6 and 24 weeks after surgery. Ground sections obtained from en bloc tissues containing graft, implants, and recipient bone were subjected to histologic evaluation and histomorphometric analysis (area occupied by the graft and bone-to-implant contact). Results: The graft showed significantly more resorption after 24 weeks than at 6 weeks (p < or =.05) irrespective of the treatment (with or without membrane), although the amount of new bone was greater at 24 weeks in sites where a membrane was covering the graft. Compared with 6 weeks postoperatively, the bone-to-implant contact was considerably improved at 24 weeks (p < or =.05), and the membrane seemed beneficial for implant osseointegration when compared with unprotected sites (p .05). As a result of graft resorption, the amount of soft tissue was considerably expanded in sites beneath membrane, accompanied by a sustained process of trabecular bone deposition close to the barrier. Conclusions: Cortical onlay grafts covered by membrane demonstrated delayed remodeling, probably as a consequence of a hindered process of graft revascularization. Grafts covered by membrane might rely on previous host bone resorption both to become revascularized and to remodel. The findings that the membrane-protected grafts were most resorbed at 24 weeks might be attributable to better implant osseointegration, because the fixtures were exposed to greater mechanical stimulation in these sites. key words: bone regeneration, implant osseointegration, onlay bone-graft
Application of A Novel Resorbable Membrane in the Treatment of Calvarial Defects in Rats

Abstract:Diplen-Gam (DG) is a novel absorbable guided bone regeneration (GBR) membrane. This study was designed to evaluate the capacity of bone repair of DG compared with that of Bio-Gide (BG). Critical size defects were created in both sides of the calcarium of 36 Sprague-Dawley rats. Defects were assigned to six groups and each group was subjected to one of the following treatments: (A1) unfilled defects, (A2) Bio-Oss (BO) grafts, (B1) DG membrane, (B2) BG membrane, (C1) DG membrane + BO grafts and (C2) BG membrane + BO grafts. The animals were killed at 2, 4, 8 and 12 weeks after the operation. The defects and surrounding tissues were examined by gross observation and X-ray examination. The paraffin sections were subjected to HE (hematoxylin and eosin) staining and IHC (immunohistochemistry) for bone morphogenetic protein-2 (BMP-2). The X-rays showed that, at 12 weeks, the DG and BG group exhibited more new bone formation than CSD blank group did; the BG group exhibited more new bone formation than the DG group did (t = 5.240, P = 0.035), the BG + BO group showed no significant differences in bone formation compared with the DG + BO group (t = 1.246, P = 0.339). By IHC staining, BMP-2-positive results could be seen inside the DG membrane, on the surface of the new bone, and inside the new bone. It can be suggested that BG membrane achieved better effects in guided bone regeneration compared with DG membrane. No significant differences were found between the two membranes in their bone healing ability when they are used with BO. Deproteinized Bovine Bone Mineral or Autologous Bone at Dehiscence Type Defects at Implants Installed Immediately into Extraction Sockets: An Experimental Study in Dogs

Abstract:Purpose: The aim of this study was to evaluate bone regeneration at surgically created dehiscence buccal defects at implants placed immediately into extraction sockets (IPIES) of small dimensions filled with autogenous bone or deproteinized bovine bone mineral (DBBM) associated with a collagen membrane. Materials and methods: Eight Labrador dogs were used and implants were placed immediately into the extraction sockets of the second premolar. The buccal wall was subsequently removed to create a standardized defect, 4 mm wide coronally, 2 mm wide apically, and 6 mm high. Autogenous bone particles (AB) or DBBM granules were used to fill the defects. All surgical sites were subsequently covered by a resorbable collagen membrane and a non-submerged healing was allowed. After 4 months, the animals were euthanized and bone blocks harvested and processed for histomorphometric analysis. Results: The bony crest at the buccal aspect (C) was located 2.3 ± 0.8 mm and 1.7 ± 0.7 mm apically to the implant shoulder (IS) at the AB and DBBM sites, respectively. The coronal levels of osseointegration at the buccal aspect (B) were located 2.7 ± 0.7 mm and 2.2 ± 1.0 mm apically to IS at the AB and DBBM sites, respectively. At the AB sites, the peri-implant mucosa was located 4.3 ± 0.9 mm, 4.7 ± 0.9 mm, and 2.0 ± 1.6 mm coronally to C, B, and IS, respectively. The corresponding values at the DBBM sites were 4.3 ± 0.6 mm, 4.8 ± 0.6 mm, and 2.5 ± 0.8 mm, respectively. No statistically significant differences were found. Conclusions: The treatment of surgically created buccal defects at IPIES sites using Bio-Oss® (Geistlich Biomaterials, Wolhusen, LU, Switzerland) or autogenous bone, concomitantly with a collagen membrane, engenders bone regeneration to a similar extent after 4 months of healing.

Abstract:The aim of this study is to evaluate treatment effects of novel GBR membranes (PDLLA membranes) applied to bony defects around dental implants on new bone regeneration alone or in combination with bioceramic bone graft (BIO-OSS®) on the mongrel dog model in comparison with collagen membrane (BIO-GIDE®) and therefore to assess the clinical values of the novel GBR membranes and bioceramic bone graft material in dental implantology. 6 implants were placed in bilateral mandibular of each mongrel dog after preparation of mid- crestal defects. The defects on one side were covered with PDLLA membranes alone or with bone graft, while the contra-lateral sides received collagen membranes alone or with bone graft. These animals were sacrificed at 2, 4, 8,12 weeks post the GBR-operation. And gross sample examination, standardized radiographs, 99mTc-MDP SPECT and histomorphometric analysis were taken. All examinations showed similar amounts of newly formed bone beneath both types of barriers. The PDLLA or collagen membrane with bone graft groups showed better results than the membrane used alone groups. The results indicated that PDLLA membranes especially with bioceramic bone graft presented an improved response in dealing with bony defects around implants.
Immunohistochemical characteristics of lateral bone augmentation using different biomaterials around chronic peri-implant dehiscence defects: An experimental in vivo study

Abstract:Aim: To investigate the immunohistochemical characteristics of a highly porous synthetic bone substitute and a cross-linked collagen membrane for guided bone regeneration. Methods: Three experimental groups were randomly allocated at chronic peri-implant dehiscence defect in 8 beagle dogs: (i) biphasic calcium phosphate covered by a cross-linked collagen membrane (test group), (ii) deproteinized bovine bone mineral covered by a natural collagen membrane (positive control) and (iii) no treatment (negative control). After 8 and 16 weeks of submerged healing, dissected tissue blocks were processed for immunohistochemical analysis. Seven antibodies were used to detect the remaining osteogenic and angiogenic potential, and quantitative immunohistochemical analysis was done by software. Results: The antigen reactivity of alkaline phosphatase was significantly higher in the test group compared to the positive and negative controls, and it maintained till 16 weeks. The intensity of osteocalcin was significantly higher in the positive control at 8 weeks than the other groups, but significantly decreased at 16 weeks and no difference was found between the groups. A significant large number of TRAP-positive cells were observed in the test group mainly around the remaining particles at 16 weeks. The angiogenic potential was comparable between the groups showing no difference in the expression of transglutaminase II and vascular endothelial growth factor. Conclusion: Guided bone regeneration combining a highly porous biphasic calcium phosphate synthetic biomaterial with a crosslinked collagen membrane, resulted in extended osteogenic potential when compared to the combination of deproteinized bovine bone mineral and a native collagen membrane.
[Barrier Effect of Improved Porcine Small Intestinal Submucosa Absorbable Membrane on Early Healing of Mandibular Defects in Rabbits].

Abstract:OBJECTIVE To evaluate the barrier effect of an absorbable barrier membrane made by small intestinal submucosa (SIS) compared with Bio-Gide collagen membrane. METHODS 12 healthy New Zealand male white rabbits were randomly assigned. A or B round bone defects with a depth of 2 mm and a diameter of 5 mm or 8 mm was made in each rabbit's mandibular. The following treatments were given respectively: covered with SIS membrane (S), covered with Bio-Gide membrane (G) and blank control (O). Then we got six groups: AS, AG, AO, BS, BG, and BO (n=4). After 4 weeks, the rabbits were sacrificed. The specimens were examined by naked-eye observation, new bone percentage (BV/TV) and bone mineralized density (BMD), which were measured and analyzed by Micro-CT. The data were analyzed with one-way ANOVA. RESULTS After 4 weeks, Bio-Gide membranes were fused with the surrounding tissue while SIS membranes held the form with no significant degradation. In the AS, BS and AG groups, the absorbable membranes smoothly covered on the new bone. While in the BG group, Bio-Gide membranes collapsed to the center of the bone defects. The 3D reconstruction of Micro-CT showed that a large number of newly formed trabeculae were found in the four groups of AS, BS, AG, and BG. In the central subsidence area of the BG group, the newly formed trabeculae were sparse. However only a small amount of new bone trabecula appeared at the bottom of the defects in groups AO and BO. Micro-CT quantitative results showed that BV/TV (39.10%±0.79%) and BMD [(517.73±11.22) mg/cm3] of AS group were significantly higher than those of AO group [26.67%±1.12%, (319.81±8.00) mg/cm3] (P<0.05), and there was no significant difference between AS group and AG group [38.15%±0.91%, (518.65±7.48) mg/cm3] (P>0.05). BV/TV (34.90%±1.35%) and BMD [(409.09±8.14) mg/cm3] of BS group were significantly higher than those of BO group [23.63%±2.07%, (171.00±16.24) mg/cm3] (P < 0.05). Meanwhile, there was no significant difference between BS and BG groups [33.40%±1.06%, (412.70±8.6) mg/cm3] (P>0.05). HE staining analysis revealed that significant bone formation was achieved in the AS, AG, BS and BG groups, and trabecular bone of AS and AG groups were thicker and denser. In AO and BO group, there were scattered new bone tissues in edges of host bone, and no coarse trabecular bone formed. CONCLUSION In the early healing of two sizes bone defects in rabbit mandibular, SIS membrane and Bio-Gide membrane have a similar barrier effect in guided bone regeneration. And SIS membrane's ability to maintain space for bone regeneration seems to be better.

Abstract:<p>Bone resorption can negatively influence the osseointegration of dental implants. Barrier membranes for guided bone regeneration (GBR) are used to exclude nonosteogenic tissues from influencing the bone healing process. In addition to the existing barrier membranes available on the market, a growing variety of membranes for GBR with tailorable physicochemical properties are under preclinical evaluation. Hence, the aim of this review is to provide a comprehensive description of materials used for GBR and to report the main industrial and regulatory aspects allowing the commercialization of these medical devices (MDs). In particular, a summary of the main attributes defining a GBR membrane is reported along with a description of commercially available and under development membranes. Finally, strategies for the scaling‐up of the manufacturing process and the regulatory framework of the main MD producers (USA, EU, Japan, China, and India) are presented. The description of the regulatory approval process of GBR membranes is representative of the typical path that medium‐ to high‐risk MDs have to follow for an effective medical translation, which is of fundamental importance to increase the impact of biomedical research on public health.</p>

Abstract:Abstract Purpose The purpose of this study was to evaluate the influence of biodegradable polycaprolactone membrane on new bone formation and the biodegradation of biphasic alloplastic bone substitutes using animal models. Materials and methods In this study, bony defect was formed at the canine mandible of 8 mm in diameter, and the defects were filled with Osteon II. The experimental groups were covered with Osteoguide as barrier membrane, and the control groups were closed without membrane coverage. The proportion of new bone and residual bone graft material was measured histologically and histomorphometrically at postoperative 4 and 8 weeks. Results At 4 weeks, the new bone proportion was similar between the groups. The proportion of remaining graft volume was 27.58 ± 6.26 and 20.01 ± 4.68% on control and experimental groups, respectively ( P < 0.05). There was no significant difference between the two groups in new bone formation and the amount of residual bone graft material at 8 weeks. Conclusion The biopolymer membrane contributes to early biodegradation of biphasic bone substitutes in the jaw defect but it does not affect the bone formation capacity of the bone graft.

Abstract:Bone defect treatment is still one of the most difficult tasks in reconstructive bone surgery.Guided bone regeneration provides a new option for it to maintain a good biological environment for bone defects,prevents the growth of fibrous tissue and accelerates fracture healing through the application of membrane material.Therefore,this paper reviews membrane guided bone tissue regeneration in orthopaedics,which may provide a new method for the treatment of long bone defects.
Bone Augmentation of Peri-Implant Dehiscence Defects Using Multilaminated Small Intestinal Submucosa As a Barrier Membrane: an Experimental Study in Dogs

Abstract:Objective The aim of the study is to evaluate the effects of multilaminated small intestinal submucosa (mSIS) combined with bone substitute material to repair peri-implant defects during guided bone regeneration procedures. Methods Twelve implants were placed in bilateral lower premolars of three beagle dogs, and a peri-implant buccal bone defect (3 mm width and 4 mm height) was created at each implant site. A total of 12 sites were filled with a particulate bone substitute material and then randomly divided into three treatment groups: covered by mSIS membrane (mSIS group), covered by collagen membrane (BG group), and no treatment (control group), each group of four sites. After 12 weeks of healing, all of the animals were euthanized and dissected blocks were obtained for micro-computed tomography (micro-CT) and histological analyses. Results Micro-CT results revealed similar horizontal width of augmented tissue and new bone formation between mSIS and BG groups (P < 0.05). Histological analyses revealed that the differences in horizontal widths of newly formed bone and bone-to-implant contact between mSIS and BG groups were not significant (P > 0.05). All of these parameters were significantly different from those in the control group (P < 0.05). Conclusions These findings confirmed that mSIS combined with the bone substitute material enhanced bone regeneration in peri-implant defects, in a manner similar to that of a collagen membrane.
