Abstract:OBJECTIVE:To analyze the clinical and pathological features of patients with liver retransplantation.METHODS:The clinical and pathological data of 22 patients who had liver retransplantation at our center from October 2003 to June 2006 were retrospectively analyzed.RESULTS:Among the 523 patients who underwent liver transplantation, 22 (4.4%) had liver retransplantation. The causes of liver retransplantation were biliary tract complications (13/22), hepatic artery thrombosis (3/22), recurrence of hepatocellular carcinoma (5/22) and nonfunctional primary graft (1/22). The pathological changes in the livers of patients with biliary complications were intrahepatic cholestasis, primary bile duct hyperplasia and neutrophil infiltrations.CONCLUSION:Biliary tract complications are the main cause of liver retransplantation. Differential diagnosis of various complications through early liver puncture biopsy and imaging examination will contribute to guide clinical treatment and may help in avoiding liver retransplantation.
Chronic Bile Duct Hyperplasia Is A Chronic Graft Dysfunction Following Liver Transplantation

Abstract:AIM: To investigate pathological types and influential factors of chronic graft dysfunction (CGD) following liver transplantation (LT) in rats.METHODS: The whole experiment was divided into three groups: (1) normal group (n = 12): normal BN rats without any drug or operation; (2) syngeneic transplant group (SGT of BN-BN, n = 12): both donors and recipients were BN rats; and (3) allogeneic transplant group (AGT of LEW-BN, n = 12): Donors were Lewis and recipients were BN rats. In the AGT group, all recipients were subcutaneously injected by Cyclosporin A after LT. Survival time was observed for 1 year. All the dying rats were sampled, biliary tract tissues were performed bacterial culture and liver tissues for histological study. Twenty-one day after LT, 8 rats were selected randomly in each group for sampling. Blood samples from caudal veins were collected for measurements of plasma endotoxin, cytokines and metabonomic analysis, and faeces were analyzed for intestinal microflora.RESULTS: During the surgery of LT, no complications of blood vessels or bile duct happened, and all rats in each group were still alive in the next 2 wk. The long term observation revealed that a total of 8 rats in the SGT and AGT groups died of hepatic graft diseases, 5 rats in which died of chronic bile duct hyperplasia. Compared to the SGT and normal groups, survival ratio of rats significantly decreased in the AGT group (P < 0.01). Moreover, liver necrosis, liver infection, and severe chronic bile duct hyperplasia were observed in the AGT group by H and E stain. On 21 d after LT, compared with the normal group (25.38 +/- 7.09 ng/L) and SGT group (33.12 +/- 10.26 ng/L), plasma endotoxin in the AGT group was remarkably increased (142.86 +/- 30.85 ng/L) (both P < 0.01). Plasma tumor necrosis factor-alpha and interleukin-6 were also significantly elevated in the AGT group (593.6 +/- 171.67 pg/mL, 323.8 +/- 68.30 pg/mL) vs the normal (225.5 +/- 72.07 pg/mL, 114.6 +/- 36.67 pg/mL) and SGT groups (321.3 +/- 88.47 pg/mL, 205.2 +/- 53.06 pg/mL) (P < 0.01). Furthermore, Bacterial cultures of bile duct tissues revealed that the rats close to death from the SGT and AGT groups were strongly positive, while those from the normal group were negative. The analysis of intestinal microflora was performed. Compared to the normal group (7.98 +/- 0.92, 8.90 +/- 1.44) and SGT group (8.51 +/- 0.46, 9.43 +/- 0.69), the numbers of Enterococcus and Enterobactena in the AGT group (8.76 +/- 1.93, 10.18 +/- 1.64) were significantly increased (both P < 0.01). Meanwhile, compared to the normal group (9.62 +/- 1.60, 9.93 +/- 1.10) and SGT group (8.95 +/- 0.04, 9.02 +/- 1.14), the numbers of Bifidobacterium and Lac tobacillus in the AGT group (7.83 +/- 0.72, 8.87 +/- 0.13) were remarkably reduced (both P < 0.01). In addition, metabonomics analysis showed that metabolic profiles of plasma in rats in the AGT group were severe deviated from the normal and SGT groups.CONCLUSION: Chronic bile duct hyperplasia is a pathological type of CGD following LT in rats. The mechanism of this kind of CGD is associated with the alterations of inflammation, intestinal barrier function and microflora as well as plasma metabolic profiles. (c) 2012 Baishideng. Abstract:Congenital biliary tract malformations are a series of rare but extremely serious diseases that mainly include biliary atresia and biliary hypoplasia (referred to as Alagille syndrome). The rapid progression of biliary atresia and Alagille syndrome results in jaundice, cholestatic liver disease, cirrhosis, and even liver failure. In most cases, supportive or clinically specific therapies cannot achieve satisfactory outcomes. Therefore, liver transplantation (especially living donor liver transplantation) may be required. As many studies have elucidated the role of genetic factors and the molecular mechanism of congenital biliary tract malformations, experimental therapies such as organoid transplantation, cell therapy, and immunotherapy have been proved to be feasible. These advanced methods have shown outstanding advantages, particularly in patients with end-stage biliary tract malformations, surgery failure, and other problems that cannot be solved by conventional therapies. This review article discusses the potential pathogenesis of and promising therapeutic strategies for biliary tract malformations.
Pathologic Diagnosis of Transplantation Liver Puncture Biopsy: Histopathologic Analysis of 98 Cases

Abstract:Objective To explore the histological morphological features of complications following orthotopic liver transplantation(OLTx) and improve the level of pathologic diagnosis.Methods All cases of post-transplantation liver biopsies were analyzed by using routine HE staining,and combined with immunohistochemical detection,including HBsAg,HBcAg,HCV,CMV,and CK19.Hepatic allograft rejection was determined according to Banff standard and scored with rejection activity index(RAI).Results A total of 129 times of liver biopsies in 98 cases were performed.Acute cellular rejection was most commonly seen in 42 cases(42.86%),and chronic rejection,ischemia-reperfusion injury,complication of biliary tract,complication of blood vessel,drug-induced damage,recurrence of hepatitis,CMV infection and recurrence of liver cancer were also found in 2(2.04%),8(8.16 %),21(21.43 %),6(6.12%),3(3.06%),8(8.16%),3(3.06 %),and 2(2.04%) cases.Otherwise 3 cases of liver biopsies were unable to be diagnosed exactly by the above methods.Conclusion Percutaneous liver biopsy is valuable for the diagnosis of complications and selection of suitable treatments and should be proposed to use routinely after OLTx.
Systematic sequential therapy for ex vivo liver resection and autotransplantation: A case report and review of literature

Abstract:BACKGROUND Perihilar cholangiocarcinoma (pCCA) is a highly malignant tumor arising from the biliary tree. Radical surgery is the only treatment offering a chance of long-term survival. However, limited by the tumor's anatomic location and peri-vascular invasion, most patients lose the chance for curative treatment. Therefore, more methods to increase the resectability of tumors as well as to improve outcomes are needed.CASE SUMMARY A 68-year-old female patient had a hepatic hilar mass without obvious symptoms. Laboratory results showed hepatitis B positivity. Magnetic resonance imaging indicated that the mass (maximum diameter: 41 mm) invaded the left and right branches of the main portal vein, as well as the middle, left and right hepatic veins; enlarged lymph nodes were also detected in the hilum. The patient was diagnosed with pCCA, and the clinical stage was determined to be T4N1M0 (stage IIIC). Considering the tumor's anatomic location and vascular invasion, systematic conversion therapy followed by ex vivo liver resection and autotransplantation (ELRA) was determined as personalized treatment for this patient. Our original systemic sequential therapeutic strategy (lenvatinib and tislelizumab in combination with gemcitabine and cisplatin) was successfully adopted as conversion therapy because she achieved partial response after three cycles of treatment, without severe toxicity. ELRA, anastomotic reconstruction of the middle hepatic vein, right hepatic vein, root of portal vein, inferior vena cava and right hepatic artery, and lymph node dissection were performed at one month after systemic therapy. Pathological and immunohistochemical examination confirmed the diagnosis of pCCA with lymph node metastasis. Although the middle hepatic vein was partially obstructed four months later, hepatic vein stent implantation successfully addressed this problem. The patient has survived for 22 mo after the diagnosis, with no evidence of recurrence or metastasis.CONCLUSION An effective therapeutic strategy for conversion therapy greatly increases the feasibility and efficiency of ELRA.
Etiological diagnosis and management of chronic graft dysfunction after liver transptantation: an analysis of 18 cases

Abstract:AIM:To analyze the etiology,diagnosis and management of chronic graft dysfunction after liver transplantation. METHODS:A total of 291cases underwent orthotopic liver transplantation in our hospital from Janunary 1996 to December 2006.The clinical data of 18 cases with chronic graft dysfunction after liver transplantation were analyzed retrospectively. RESULTS:Of 18 patients with chronic graft dysfunction after liver transplantation,2 were found with recurrent biliary inflammation,2 with biliary sludge,1 with anastomotic biliary stricture, 4 with nonanastomotic biliary stricture,3 with reccurent hepatitis B,1 with recurrent hepatitis C,1 with recurrent primary biliary cirrhosis,1with hepatic artery stricture,1 with cytomegalovirus (CMV)infection and 2 with chronic rejection. After different medical support treatment,intervention or liver re-transplantation,11 patients were improved,and 4 cases were found with recurrent diseases.The rest 3 cases died. CONCLUSION:Different therapeutic approaches may be taken in accordance with different causes of chronic graft dysfunction after liver transplan- tation.For irreversible hepatic allograft failure, liver re-transplantation should be carried out.
Liver Transplantation in China: Problems and Their Solutions.

Abstract:BACKGROUND:The past decade has witnessed the rapid development of liver transplantation in China. The 1-year survival of liver transplant patients comes to 80% in many leading medical centers and the number of liver transplantation is increasing. However, liver transplantation in China is facing several challenges including recipient with hepatocellular carcinoma (HCC), recurrence of HCC and hepatitis B, long-term postoperative care, the bridge to liver transplantation, and shortage of liver donor. This review was to understand the status of and problems in liver transplantation in China.DATA RESOURCES:An English-language literature search using MEDLINE (1990-2003) on liver transplantation and other related reports and review articles in Chinese from major transplant centers in China.RESULTS:HCC is one of the main indications for liver transplantation in China but different centers adopted different criteria for selection of patients. Hepatitis B virus reinfection is a vital problem after liver transplantation in HBV-related patients. More and more attention was focused on long-term postoperative care and donor shortage. Artificial liver support system has been applied in patients waiting for a graft in many centers.CONCLUSIONS:HCC remains to be one of the main indications for liver transplantation in China; combined hepatitis B immune globulin and lamivudine is considered effective to prevent hepatitis B virus reinfection. Apart from long-term postoperative care for the improvement of the survival rate, early steroid withdrawal is feasible in liver transplantation. Living donor liver transplantation, split liver transplantation, and marginal donor transplantation can deal with donor shortage to some extent. Artificial liver assist system serves as a bridge to liver transplantation.
Understanding of the Prevention and Tre Atment of Biliary Complications after Orthotopic Liver Transplantation

Abstract:Objective:To review the experience in biliary complications after liver transplantation, and to discuss the major causes and methods of their prevention and treatment. Methods:Five patients were diagnosed respectively hereditary capillary hyperplasia, alcoholic cirrhosis, primary biliary cirrhosis(2 casses) polycystic liver and kidney. Four received piggyback transplantation, one case received associated hepato-renal transplantation. Results:Cases 1 and 2 surffered from post-operative biliary obstruction, both of them were surgically cured with reversion of choledochocholedochostomy to choledochojejunostomy. Three patients acquired long-term survival with quite good health, and other two died 10 days and 30 days respectively after the operation.Conclusions:Ultrasonography and radiological examinations are the major methods for diagnosing post-operative biliary complications. To as prevention, the blood supply of the biliary duct must be respected. The major objectives of the surgical treatment for biliary complications are: resect the ischemic segment and re-establish the patency of bile flow.
A Comprehensive Review of Liver Allograft Fibrosis and Steatosis: From Cause to Diagnosis

Abstract:Despite advances in posttransplant care, long-term outcomes for liver transplant recipients remain unchanged. Approximately 25% of recipients will advance to graft cirrhosis and require retransplantation. Graft fibrosis progresses in the context of de novo or recurrent disease. Recurrent hepatitis C virus infection was previously the most important cause of graft failure but is now curable in the majority of patients. However, with an increasing prevalence of obesity and diabetes and nonalcoholic fatty liver disease as the most rapidly increasing indication for liver transplantation, metabolic dysfunction-associated liver injury is anticipated to become an important cause of graft fibrosis alongside alloimmune hepatitis and alcoholic liver disease. To better understand the landscape of the graft fibrosis literature, we summarize the associated epidemiology, cause, potential mechanisms, diagnosis, and complications. We additionally highlight the need for better noninvasive methods to ameliorate the management of graft fibrosis. Some examples include leveraging the microbiome, genetic, and machine learning methods to address these limitations. Overall, graft fibrosis is routinely seen by transplant clinicians, but it requires a better understanding of its underlying biology and contributors that can help inform diagnostic and therapeutic practices.

Abstract:Liver transplantation is known to generate significant inflammation in the entire organ based on the metabolic profile and the tissue's ability to recover from the ischemia-reperfusion injury (IRI). This cascade contributes to post-transplant complications, affecting both the synthetic liver function (immediate) and the scar development in the biliary tree. The new occurrence of biliary strictures, and the recurrence of malignant and benign liver diseases, such as cholangiocarcinoma (CCA) and primary sclerosing cholangitis (PSC), are direct consequences linked to this inflammation. The accumulation of toxic metabolites, such as succinate, causes undirected electron flows, triggering the releases of reactive oxygen species (ROS) from a severely dysfunctional mitochondrial complex 1. This initiates the inflammatory IRI cascade, with subsequent ischemic biliary stricturing, and the upregulation of pro-tumorigenic signaling. Such inflammation is both local and systemic, promoting an immunocompromised status that can lead to the recurrence of underlying liver disease, both malignant and benign in nature. The traditional treatment for CCA was resection, when possible, followed by cytotoxic chemotherapy. Liver transplant oncology is increasingly recognized as a potentially curative approach for patients with intrahepatic (iCCA) and perihilar (pCCA) cholangiocarcinoma. The link between IRI and disease recurrence is increasingly recognized in transplant oncology for hepatocellular carcinoma. However, smaller numbers have prevented similar analyses for CCA. The mechanistic link may be even more critical in this disease, as IRI causes the most profound damage to the intrahepatic bile ducts. This article reviews the underlying mechanisms associated with biliary inflammation and biliary pathology after liver transplantation. One main focus is on the link between transplant-related IRI-associated inflammation and the recurrence of cholangiocarcinoma and benign liver diseases of the biliary tree. Risk factors and protective strategies are highlighted.

Abstract:Liver transplantation is the current standard of care for end-stage liver disease and an accepted therapeutic option for acute liver failure and primary liver tumors. Despite the remarkable advances in the surgical techniques and immunosuppressive therapy, the postoperative morbidity and mortality still remain high and the leading causes are biliary complications, which affect up to one quarter of recipients. The most common biliary complications are anastomotic and non-anastomotic biliary strictures, leaks, bile duct stones, sludge and casts. Despite the absence of a recommended treatment algorithm many options are available, such as surgery, percutaneous techniques and interventional endoscopy. In the last few years, endoscopic techniques have widely replaced the more aggressive percutaneous and surgical approaches. Endoscopic retrograde cholangiography is the preferred technique when duct-to-duct anastomosis has been performed. Recently, new devices and techniques have been developed and this has led to a remarkable increase in the success rate of minimally invasive procedures. Understanding the mechanisms of biliary complications helps in their early recognition which is the prerequisite for successful treatment. Aggressive endoscopic therapy is essential for the reduction of morbidity and mortality in these cases. This article focuses on the common post-transplant biliary complications and the available interventional treatment modalities.
[Analysis and Management of the Biliary Complications after Liver Transplantations].

Abstract:OBJECTIVE To discuss the relevant factors of biliary complications after liver transplantation and to investigate the value of comprehensive management for the complications. METHODS The data of 366 patients undergoing liver transplantation from October 2000 to March 2012 was analyzed retrospectively, and the risk factors were analyzed by univariate analysis and Stepwise Logistic regression. The cases with biliary leak were administered thorough drainage. The cases with anastomotic biliary stricture were administered sacculus dilatation through percutaneous transhepatic cholangiography (PTC) and endoscopicretrograde cholangiopancreatography (ERCP). If necessary, some cases were placed biliary tract brackets. The patients with nonanastomotic biliary stricture were treated with PTC plus choledochoscope. RESULTS All the 366 patients were followed up for 58.5 (10 to 129) months. Biliary complications after liver transplantation were diagnosed in 42 cases among these patients. The incidence for biliary complications was 11.5%. The univariate analysis and multivariate Logistic regression analysis showed that the second warm ischemia period and the blood loss and the damage of blood supply and the diameter of biliary anastmosis were significantly associated with biliary complications after liver transplantations (Wald = 9.474 to 17.208, P < 0.05). Twelve cases with biliary leak were cured through abdominal and nasobiliary drainage. Twenty-two cases with anastomotic biliary stricture were administered sacculus dilatation through ERCP or PTC and were cured, including 6 cases were placed biliary tract brackets. Among 8 cases with nonanastomotic biliary stricture, 6 cases were cured through PTC associating with choledochoscope. One case was treated second liver transplantation and another case got worse. CONCLUSIONS Ischemic injury and the diameter of anastmosis are risk factors for biliary complications after liver transplantations. The interventional management of biliary stricture and bile leakage after liver transplantation is safe and effective.

BC is one of the major risk factors for the prognosis of liver transplantation. This review summarizes the etiology and the mechanism(s) of biliary complications after liver transplantation.

Abstract:BACKGROUND:Liver transplantation is the therapy of choice for patients with end-stage liver diseases. However, the gap between the low availability of organs and high demand is continuously increasing. Innovative strategies for organ protection are necessary to expand donor pool and to achieve better outcomes for liver transplantation. The present review analyzed and compared various strategies of liver protection.DATA SOURCES:Databases such as PubMed, Embase and Ovid were searched for the literature related to donor liver protection strategies using following key words: "ischemia reperfusion injury", "graft preservation", "liver transplantation", "machine perfusion" and "conditioning". Of the 146 studies identified, only those with cutting edge strategies were analyzed.RESULTS:A variety of therapeutic approaches were proposed to alleviate graft ischemia/reperfusion injury, which included static cold storage, machine perfusion (hypothermic, normothermic and subnormothermic), manual conditioning (pre, post and remote), and pharmacological conditioning. Evidences from animal experiments and clinical trials suggested that all these strategies could potentially protect liver graft; however, their clinical applications are limited partially due to their own disadvantages.CONCLUSIONS:There are a plenty of methods suggested to decrease the degree of donor liver transplantation-related injury. However, none of these approaches is perfect in clinical practice. More translational researches (molecular and clinical studies) are needed to improve the techniques in liver graft protection.
Biliary Complications Following Orthotopic Liver Transplantation and Its Management (report of 150 Cases)

Abstract:Objective To investigate the pathogenesis,diagnosis and treatment of biliary complications following orthotopic liver transplantation.Methods Among 150 OLTs, 35 patients were diagnosed of biliary complications from Apri 2001 to Jun 2004.The clinical data were analysed retrospectively and the causes of biliary complication were discussed with some related literatures.Results There were 12(34.2%) early complications and 23(65.8%%) later complications; including 13(37.5%) with anastomosis strictures, intra- and extra- hepatic multiple biliary strictures and rejection necrosis, hepatic artery thrombosis ,cytomagelovirus infection, and chemotherapy were found in 2,6,1,2(5.7%,17.1%,2.9%,5.7%) patients, respectively. 7(20.0%)had anastomosis bile leakage and 3(8.7%) bile leakage after T tube removed. One(2.9%) patient suffered from cholangitis and cholothiasis caused by Oddi's sphincter dysfunction. Conclusions There are many aspects worth while to be investigated for the pathogenesis of biliary complications following orthotopic liver transplantation. Abstract:End -stage liver disease (ESLD), stemming from a spectrum of chronic liver pathologies including chronic liver failure, acute cirrhosis decompensation and hepatocellular carcinoma, imposes a significant global healthcare burden. Liver transplantation (LT) remains the only treatment for ESLD. However, the escalating mortality on transplant waitlists has prompted the utilization of marginal liver grafts in LT procedures. These grafts primarily encompass elderly livers, steatotic livers, livers from donation after circulatory death, split livers and those infected with the hepatitis virus. While the expansion of the donor pool offers promise, it also introduces concomitant risks. These encompass graft failure, biliary and cardiovascular complications, the recurrence of liver disease and reduced patient and graft survival. Consequently, various established strategies, ranging from improved donorrecipient matching to surgical interventions, have emerged to mitigate these risks. This article undertakes a comprehensive assessment of the current landscape, evaluating the viability of diverse marginal liver grafts. Additionally, it synthesizes approaches aimed at enhancing the quality of such marginal liver grafts. The overarching objective is to augment the donor pool and ameliorate the risk factors associated with the shortage of liver grafts.

Abstract:Orthotopic liver transplantation is the definitive treatment for end-stage liver disease and hepatocellular carcinomas. Biliary complications are the most common complications seen after transplantation, with an incidence of 10-25%. These complications are seen both in deceased donor liver transplant and living donor liver transplant. Endoscopic treatment of biliary complications with endoscopic retrograde cholangiopancreatography (commonly known as ERCP) has become a mainstay in the management post-transplantation. The success rate has reached 80% in an experienced endoscopist's hands. If unsuccessful with ERCP, percutaneous transhepatic cholangiography can be an alternative therapy. Early recognition and treatment has been shown to improve morbidity and mortality in post-liver transplant patients. The focus of this review will be a learned discussion on the types, diagnosis, and treatment of biliary complications post-orthotopic liver transplantation.

Abstract:Cholestatic liver injury is characterized by damage induced on the biliary tree and cholangiocytes, the cells lining the biliary tree, thus they are termed "cholangiopathies". Cholangiopathies include diseases such as Primary Biliary Cholangitis, Primary Sclerosing Cholangitis, Biliary Atresia and Cholangiocarcinoma. These pathologies lack viable therapies and most patients are diagnosed during late stage disease progression (with the exception of Biliary Atresia, which is found shortly after birth). The lack of therapies for these diseases has put a significant burden on the need for liver transplantation as this is the only indicative "cure" for cholangiopathies. The molecular mechanisms for cholangiopathies have been extensively studied; however, and unfortunately, the lack of effective biomarkers and therapeutics remains. In this review article we highlight the latest studies to investigate the molecular mechanisms regulating cholangiopathies and the potential therapeutics that might be discovered.
Current concept about postoperative cholangitis in biliary atresia

Abstract:Background Postoperative cholangitis characterized by fever and acholic stool and positive blood culture is a common and serious complication following Kasai’s operation for biliary atresia. The aim of this review was to describe the pathogenesis, clinical manifestations, medical treatment and outcome of postoperative cholangitis. Data sources Articles on biliary atresia retrieved from Pubmed and MEDLINE in the recent 10 years were reviewed. Results The pathogenesis of postoperative cholangitis is still controversial. Recent methods for the diagnosis of postoperative cholangitis include urinary sulfated bile acids (USBA) and magnetic resonance cholangiopancreaticography (MRCP). High-dose steroids and oral antibiotics have been used to reduce the incidence of postoperative cholangitis, and recurrent cholangitis leads to a lower survival rate. Conclusions Cholangitis is one of the most important determinants of long-term survival after the Kasai’s procedure. The knowledge on postoperative cholangitis has been increasing in the past 10 years, showing a lower incidence of the disease and better therapeutic results.

Abstract:To evaluate the value of interventional approaches on the vascular and biliary complications after liver transplantation.Materials and Methods:18patients after orthotopic liver transplantation received interventional approaches.Percutaneous transhepatic artery angiography and thrombolysis were performed in7cases,percutaneous transhepatic artery angiograph and percutaneous transluminal angio-plasty(PTA)therapy in1patient,cholangiography in8cases,inferior vena cava angiography in1case,inferior vena cava and hepatic vein angiography in1case.Results:Eight cases with biliary complication performed PTCD;8patients performed hepatic artery angiography,among them,hepatic artery patency restored after transhepatic artery thrombolysis in6cases,restored partially in1patient,not restored in1case;1patient with IVC stenosis performed stent implantation;1patient with both IVC and hepatic vein stenosis performed IVC stent implantation and hepatic vein percutaneous tranluminal angioplasty(PTA).Conclusion:Interventional approaches including PTCD,balloon angioplasty,stent placement,transartery thrombolysis are useful procedure for the treatment of the patient with vascular and biliary complications after liver transplantation.
