Abstract:RATIONALE, AIMS AND OBJECTIVES Design and implement a concurrent campaign of influenza immunization and tuberculosis (TB) screening for health care workers (HCWs) that can reduce the number of clinic visits for each HCW. METHOD A discrete-event simulation model was developed to support issues of resource allocation decisions in planning and operations phases. RESULTS The campaign was compressed to100 days in 2010 and further compressed to 75 days in 2012 and 2013. With more than 5000 HCW arrivals in 2011, 2012 and 2013, the 14-day goal of TB results was achieved for each year and reduced to about 4 days in 2012 and 2013. CONCLUSION Implementing a concurrent campaign allows less number of visiting clinics and the compressing of campaign length allows earlier immunization. The support of simulation modelling can provide useful evaluations of different configurations.

Abstract:Mathematical modeling has played a prominent and necessary role in the current coronavirus disease 2019 (COVID-19) pandemic, with an increasing number of models being developed to track and project the spread of the disease, as well as major decisions being made based on the results of these studies. A proliferation of models, often diverging widely in their projections, has been accompanied by criticism of the validity of modeled analyses and uncertainty as to when and to what extent results can be trusted. Drawing on examples from COVID-19 and other infectious diseases of global importance, we review key limitations of mathematical modeling as a tool for interpreting empirical data and informing individual and public decision making. We present several approaches that have been used to strengthen the validity of inferences drawn from these analyses, approaches that will enable better decision making in the current COVID-19 crisis and beyond.

Abstract:Evidence is mounting that influenza virus interacts with other pathogens colonising or infecting the human respiratory tract. Taking into account interactions with other pathogens may be critical to determining the real influenza burden and the full impact of public health policies targeting influenza. This is particularly true for mathematical modelling studies, which have become critical in public health decision-making. Yet models usually focus on influenza virus acquisition and infection alone, thereby making broad oversimplifications of pathogen ecology. Herein, we report evidence of influenza virus interactions with bacteria and viruses and systematically review the modelling studies that have incorporated interactions. Despite the many studies examining possible associations between influenza and Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Neisseria meningitidis, respiratory syncytial virus (RSV), human rhinoviruses, human parainfluenza viruses, etc., very few mathematical models have integrated other pathogens alongside influenza. The notable exception is the pneumococcus-influenza interaction, for which several recent modelling studies demonstrate the power of dynamic modelling as an approach to test biological hypotheses on interaction mechanisms and estimate the strength of those interactions. We explore how different interference mechanisms may lead to unexpected incidence trends and possible misinterpretation, and we illustrate the impact of interactions on public health surveillance using simple transmission models. We demonstrate that the development of multipathogen models is essential to assessing the true public health burden of influenza and that it is needed to help improve planning and evaluation of control measures. Finally, we identify the public health, surveillance, modelling, and biological challenges and propose avenues of research for the coming years.

Abstract:The COronaVIrus Disease 2019 (COVID-19) pandemic that has had the world in its grip from the beginning of 2020, has resulted in an unprecedented level of public interest and media attention on the field of mathematical epidemiology. Ever since the disease came to worldwide attention, numerous models with varying levels of sophistication have been proposed; many of these have tried to predict the course of the disease over different time-scales. Other models have examined the efficacy of various policy measures that have been adopted (including the unparalleled use of "lockdowns") to contain and combat the disease. This multiplicity of models may have led to bewilderment in many quarters about the true capabilities and utility of mathematical modeling. Here we provide a brief guide to epidemiological modeling, focusing on how it has emerged as a tool for informed public-health policy-making and has in turn, influenced the design of interventions aimed at preventing disease outbreaks from turning into raging epidemics. We show that the diversity of models is somewhat illusory, as the bulk of them are rooted in the compartmental modeling framework that we describe here. While their basic structure may appear to be a highly idealized description of the processes at work, we show that features that provide more realism, such as the community organization of populations or strategic decision-making by individuals, can be incorporated into such models. We conclude with the argument that the true value of models lies in their ability to test in silico the consequences of different policy choices in the course of an epidemic, a much superior alternative to trial-and-error approaches that are highly costly in terms of both lives and socio-economic disruption.

The role of the mathematical sciences in supporting the COVID-19 response in Australia and New Zealand

Abstract:Mathematical modelling has been used to support the response to the COVID-19 pandemic in countries around the world including Australia and New Zealand. Both these countries have followed similar pandemic response strategies, using a combination of strict border measures and community interventions to minimise infection rates until high vaccine coverage was achieved. This required a different set of modelling tools to those used in countries that experienced much higher levels of prevalence throughout the pandemic. In this article, we provide an overview of some of the mathematical modelling and data analytics work that has helped to inform the policy response to the pandemic in Australia and New Zealand. This is a reflection on our experiences working at the modelling-policy interface and the impact this has had on the pandemic response. We outline the various types of model outputs, from short-term forecasts to longer-term scenario models, that have been used in different contexts. We discuss issues relating to communication between mathematical modellers and stakeholders such as health officials and policymakers. We conclude with some future challenges and opportunities in this area.

Abstract:Objective: Recent studies have used Bayesian methods to predict timing of influenza epidemics many weeks in advance, but there is no documented evaluation of how such forecasts might support the day-to-day operations of public health staff. Methods: During the 2015 influenza season in Melbourne, Australia, weekly forecasts were presented at Health Department surveillance unit meetings, where they were evaluated and updated in light of expert opinion to improve their accuracy and usefulness. Results: Predictive capacity of the model was substantially limited by delays in reporting and processing arising from an unprecedented number of notifications, disproportionate to seasonal intensity. Adjustment of the predictive algorithm to account for these delays and increased reporting propensity improved both current situational awareness and forecasting accuracy. Conclusions: Collaborative engagement with public health practitioners in model development improved understanding of the context and limitations of emerging surveillance data. Incorporation of these insights in a quantitative model resulted in more robust estimates of disease activity for public health use. Implications for public health: In addition to predicting future disease trends, forecasting methods can quantify the impact of delays in data availability and variable reporting practice on the accuracy of current epidemic assessment. Such evidence supports investment in systems capacity.

Abstract:Mathematical models played an important role in understanding and combatting the pandemic of novel coronaviruses. The advantages and further tasks of these activities are discussed using the example of the extended Susceptible, Exposed, Infected and Removed model combined with economic macro model. The article advocates for a broader outlook to tackle future pandemics and related challenges, underscoring the importance of ongoing academic cooperation and global governance to effectively address emerging infectious diseases and their far‐reaching implications. This article reviews the essential role of mathematical models in understanding and combatting the pandemic of novel coronaviruses, in particular focusing the advance in the use of mathematical models in disease control in Japan. Highlighting the integral role of mathematical models in public health, the article introduces a model that factors in the heterogeneity of infectious contacts, concentrating on the effectiveness of testing and isolation, alongside a model that involves economic losses. The models exhibit how, given such heterogeneity, milder behavioral restrictions can still achieve suppression, rigorous testing and isolation can effectively curb the spread, and containment measures can mitigate economic losses. These models aid in grasping the complicated dynamics of disease transmission and optimizing interventions. The knowledge of population ecology is also considered effective for public health in statistical analysis, organizing concepts using dynamic mathematical models, which lead to policy proposals and deepen understanding. Evolution theory may help the understanding of virulence subject to change. However, effective prevention necessitates not only models but also the practical implementation of efficacious measures. The cooperation of various disciplines is particularly crucial in achieving a balance between health measures, economic interests, and human rights. Moreover, the article acknowledges the limitations of models and underscores the significance of real‐world execution. Overall, the article advocates for a broader outlook to tackle future pandemics and related challenges, underscoring the importance of ongoing academic cooperation and global governance to effectively address emerging infectious diseases and their far‐reaching implications.

Abstract:During the coronavirus disease 2019 pandemic, mathematical modeling has been widely used to understand epidemiological burden, trends, and transmission dynamics, to facilitate policy decisions, and, to a lesser extent, to evaluate infection prevention and control (IPC) measures. This review highlights the added value of using conventional epidemiology and modeling approaches to address the complexity of healthcare-associated infections (HAI) and antimicrobial resistance. It demonstrates how epidemiological surveillance data and modeling can be used to infer transmission dynamics in healthcare settings and to forecast healthcare impact, how modeling can be used to improve the validity of interpretation of epidemiological surveillance data, how modeling can be used to estimate the impact of IPC interventions, and how modeling can be used to guide IPC and antimicrobial treatment and stewardship decision-making. There are several priority areas for expanding the use of modeling in healthcare epidemiology and IPC. Importantly, modeling should be viewed as complementary to conventional healthcare epidemiological approaches, and this requires collaboration and active coordination between IPC, healthcare epidemiology, and mathematical modeling groups.

Abstract:Objectives Since COVID-19 first emerged in 2019, mathematical models have been developed to predict transmission and provide insight into disease control strategies. A key research need now is for models to inform long-term vaccination policy. We aimed to review the early modelling methods utilised during the pandemic period (2019–2023) in order to identify gaps in the literature and highlight areas for future model development. Study design This study was a systematic review. Methods We searched PubMed, Embase and Scopus from 1 January 2019 to 6 February 2023 for peer-reviewed, English-language articles describing age-structured, dynamic, mathematical models of COVID-19 transmission and vaccination in high-income countries that include waning immunity or reinfection. We extracted details of the structure, features and approach of each model and combined them in a narrative synthesis. Results Of the 1109 articles screened, 47 were included. Most studies used deterministic, compartmental models set in Europe or North America that simulated a time horizon of 3.5 years or less. Common outcomes included cases, hospital utilisation and deaths. Only nine models included long COVID, costs, life years or quality of life-related measures. Two studies explored the potential impact of new variants beyond Omicron. Conclusions This review demonstrates a need for long-term models that focus on outcome measures such as quality-adjusted life years, the population-level effects of long COVID and the cost effectiveness of future policies – all of which are essential considerations in the planning of long-term vaccination strategies.

Abstract:Mathematical models are increasingly adopted for setting disease prevention and control targets. As model-informed policies are implemented, however, the inaccuracies of some forecasts become apparent, for example overprediction of infection burdens and intervention impacts. Here, we attribute these discrepancies to methodological limitations in capturing the heterogeneities of real-world systems. The mechanisms underpinning risk factors of infection and their interactions determine individual propensities to acquire disease. These factors are potentially so numerous and complex that to attain a full mechanistic description is likely unfeasible. To contribute constructively to the development of health policies, model developers either leave factors out (reductionism) or adopt a broader but coarse description (holism). In our view, predictive capacity requires holistic descriptions of heterogeneity which are currently underutilised in infectious disease epidemiology, in comparison to other population disciplines, such as non-communicable disease epidemiology, demography, ecology and evolution.

Abstract:Abstract During emerging disease outbreaks, public health, emergency management officials and decision-makers increasingly rely on epidemiological models to forecast outbreak progression and determine the best response to health crisis needs. Outbreak response strategies derived from such modelling may include pharmaceutical distribution, immunisation campaigns, social distancing, prophylactic pharmaceuticals, medical care, bed surge, security and other requirements. Infectious disease modelling estimates are unavoidably subject to multiple interpretations, and full understanding of a model's limitations may be lost when provided from the disease modeller to public health practitioner to government policymaker. We review epidemiological models created for diseases which are of greatest concern for public health protection. Such diseases, whether transmitted from person-to-person (Ebola, influenza, smallpox), via direct exposure (anthrax), or food and waterborne exposure (cholera, typhoid) may cause severe illness and death in a large population. We examine disease-specific models to determine best practices characterising infectious disease outbreaks and facilitating emergency response and implementation of public health policy and disease control measures.

A case of metaplastic meningioma with extensive xanthomatous change

Abstract:We report a case of metaplastic meningioma with extensive xanthomatous change occurring in a 61‐year‐old woman. Magnetic resonance imaging demonstrated a well‐demarcated, dura‐based mass measuring 7 cm in maximal diameter in the right occipital to parietal area. Under a clinical diagnosis of meningioma, right parietal craniotomy was performed. Histologically, the tumor showed extensive xanthomatous change together with the common features of meningothelial meningioma. Tumor cells showed diffuse xanthomatous change in various quantities, and xanthomatous tumor cells frequently showed a gradual transition to the non‐xanthomatous counterpart. The xanthomatous tumor cells showed immunopositivity for epithelial membrane antigen (EMA), vimentin, fatty acid synthase and several histiocytic markers (CD68, Ki‐M1p, MAC387, lysozyme, alpha 1‐antitrypsin and alpha 1‐antichymotrypsin). In addition, there was patchy infiltration of foamy macrophages. Since these histiocytes did not show immunopositivity for EMA, these cells were distinguished from the xanthomatous tumor cells. These two types of xanthomatous changes comprised approximately two‐thirds of the whole tumor tissue. Any atypical features such as necrosis, frequent mitotic figures or brain parenchymal invasion were not observed. We diagnosed this case as xanthomatous meningioma, WHO grade I.

Abstract:Infectious disease models provide a systematic way to estimate crucial features of epidemic dynamics and explore different transmission and control scenarios. Given the importance of model-based analysis in managing public health crises, there has been an increase in post-pandemic creation of both academia-driven modelling centres, hubs and consortiums and government-driven public health agencies with in-house modelling units or teams ( Centers for Disease Control and Prevention, 2016 , COVID-19 Forecasting Hub, 2020 , COVID-19 Scenario Modelling Hub, 2020 , European Covid-19 Scenario Hub, 2022 , UK Health Security Agency, 2023 , World Health Organization, 2021 ). However, in the past, the delineation of roles and responsibilities between government- and academia-led modelling groups has often been unclear. Who should perform which tasks and when? This ambiguity can increase the risk of duplicated work or unaddressed gaps in analysis. It also raises questions about the sustainability of modelling capacity for addressing routine operational analytical needs while also developing new approaches that can be tailored for emergencies.In the below sections, we discuss factors that could inform decisions about where to locate infectious disease modelling activity. Rather than giving a fixed set of rules, we outline key considerations and trade-offs that could be taken into account to enable academic and government modelling activities to complement each other effectively, which can in turn be refined as new public health crises emerge in future.

Abstract:Public health decisions must be made about when and how to implement interventions to control an infectious disease epidemic. These decisions should be informed by data on the epidemic as well as current understanding about the transmission dynamics. Such decisions can be posed as statistical questions about scientifically motivated dynamic models. Thus, we encounter the methodological task of building credible, data-informed decisions based on stochastic, partially observed, nonlinear dynamic models. This necessitates addressing the tradeoff between biological fidelity and model simplicity, and the reality of misspecification for models at all levels of complexity. We assess current methodological approaches to these issues via a case study of the 2010-2019 cholera epidemic in Haiti. We consider three dynamic models developed by expert teams to advise on vaccination policies. We evaluate previous methods used for fitting these models, and we demonstrate modified data analysis strategies leading to improved statistical fit. Specifically, we present approaches for diagnosing model misspecification and the consequent development of improved models. Additionally, we demonstrate the utility of recent advances in likelihood maximization for high-dimensional nonlinear dynamic models, enabling likelihood-based inference for spatiotemporal incidence data using this class of models. Our workflow is reproducible and extendable, facilitating future investigations of this disease system. Quantitative understanding of infectious disease transmission dynamics relies upon mathematical models informed by scientific knowledge and relevant data. The models aim to provide a statistical description of the trajectory of an epidemic and its uncertainty, together with a representation of the underlying biological mechanisms. Evaluation of success at these goals is necessary in order for a model to provide a reliable tool for guiding evidence-based public policy interventions. In this article, we conduct a re-analysis of the 2010–2019 cholera outbreak in Haiti. We use this case study to investigate current procedures for fitting mechanistic models to time series data, while identifying limitations of these methodologies and proposing remedies. Our analysis presents methodology for diagnosing how well a model describes observed data. Using objective measures to assess model fit ensures that our evaluation is based on quantifiable criteria. Incorporating reproducibility into this assessment results in a framework that enables the validation or refinement of model based inferences when revisiting the data, facilitating scientific discovery. Our data analysis workflow is supported by recent advances in algorithms, software and hardware, which facilitate statistical fitting of nonlinear stochastic dynamic models to observed incidence data. However, inference for high-dimensional systems remains a methodological challenge. One of the models under consideration involves spatially coupled stochastic meta-populations, and we demonstrate how a recently developed algorithm permits likelihood-based inference and model diagnostics in this setting. We contend that raising the currently accepted standards of infectious disease modeling will result in a greater ability of scientists and policy makers to understand and respond to future infectious disease outbreaks.

Such models have played a pivotal role in the COVID-19 pandemic, with transmission models—and, by consequence, modelers—guiding global, national, and local responses to SARS-CoV-2. However, these models have largely not accounted for the social and structural factors, which lead to socioeconomic, racial, and geographic health disparities. In this piece, we raise and attempt to clarify several questions relating to this important gap in the research and practice of infectious disease modeling: Why do epidemiologic models of emerging infections typically ignore known structural drivers of disparate health outcomes? What have been the consequences of a framework focused primarily on aggregate outcomes on infection equity? What should be done to develop a more holistic approach to modeling-based decision-making during pandemics? In this review, we evaluate potential historical and political explanations for the exclusion of drivers of disparity in infectious disease models for emerging infections, which have often been characterized as “equal opportunity infectors” despite ample evidence to the contrary. We look to examples from other disease systems (HIV, STIs) and successes in including social inequity in models of acute infection transmission as a blueprint for how social connections, environmental, and structural factors can be integrated into a coherent, rigorous, and interpretable modeling framework. We conclude by outlining principles to guide modeling of emerging infections in ways that represent the causes of inequity in infection as central rather than peripheral mechanisms.

Abstract:During an infectious disease outbreak, biases in the data and complexities of the underlying dynamics pose significant challenges in mathematically modelling the outbreak and designing policy. Motivated by the ongoing response to COVID-19, we provide a toolkit of statistical and mathematical models beyond the simple SIR-type differential equation models for analysing the early stages of an outbreak and assessing interventions. In particular, we focus on parameter estimation in the presence of known biases in the data, and the effect of non-pharmaceutical interventions in enclosed subpopulations, such as households and care homes. We illustrate these methods by applying them to the COVID-19 pandemic.
