Abstract:Acute lymphoblastic leukemia with eosinophilia (ALL/E0) is a rare clinical entity. Since Spitzer and Garson's first description, only less than 100 cases have been reported. Here we report a 48-year-old man who was diagnosed with ALL/E0 in our hospital. The patient presented with pitting edema at both his ankles on March 10, 2005. His complete blood cell counts at out patient clinic showed a WBC count of 39800/mm3 with 68% eosinophils. He was admitted. Abdominal examination showed an enlarged liver of 3cm and spleen of 4cm size, but no lymphadenopathy. No rash or purpuric patche was found. Total leucocyte count after admission was 411000/mm3 with 42% eosinophils. Platelet count was 80000/mm3, and hemoglobin was 115g/l. Bone marrow examination showed 70% blasts and 3% eosinophils, and was diagnosed as acute lymphoblastic leukemia of L3 type. Flow cytometric immunophenotyping of these blasts showed expression of CD10 (86.87%), CD22 (96.68%), CD79a (86.82%), and HLA-DR (92.59%). Cytogenetic analysis of the bone marrow showed a karyotype of 46, XY, del (1)(p32. There was no evidence of parasitic, viral or bacterial infection, drug reaction, allergic/immunologic disorder and other neoplastic disease. A diagnosis of ALL-E0 was made and the patient was start on induction chemotherapy with vincristine (4 mg, d 1,8), daunomycin (70 mg, d 1~3), cyclophosphamide (1.0 g d1, 0.6 g d8) and prednisone (60 mg, d1~14) on March 22. The eosinophils in the blood disappeared the same day after chemotherapy and the bone marrow examination on July 5 showed remission and the percentage of eosinophils was normal. Two more causes of VDCP and two additional courses of high dose arabinosyl cytosine were given, and he remains hematological remission. Eosinophilia can present before, concurrent with, or following the diagnosis of ALL. Several hypotheses seek to explain the hypereosinophilia in ALL. Il-3 plays an important role in the differentiation of the progenitor cells differentiating into mature eosinophils. In the specific cytogenetic abnormality t (5; 14)(q31; q32) and t (5; 9)(q31; p24), immunoglobulin heavy gene enhancer region joins to the promoter region of the IL-3 and JAK2 (9p24) gene, respectively, which causes high levels of IL-3 by leukaemic cells. In addition, when the purine and pyrimidine nucleotide content of the eosinophils in ALL-E0 patients was compared with that of eosinophils from healthy donors and from patients with eosinophilia not associated with leukemia, the ratios of purine: pyrimidine and of uracil:cytosine nucleotides were decreased, while the total nucleotide concentration was increased, especially the concentration of UDP-sugars and pyrimidine nucleotides. Similar changes were also detected in leukemic cells of patients with ALL compared to normal lymphocytes. Thus, it has been suggested that the eosinophils in the patients with ALL-E0 has a malignant character, and it might be an intrinsic part of the lymphoproliferative disorder and not a reaction to leukemic process. Patients with these this syndrome characteristically have cytogenetic abnormalities involving t(5;14), whereas our patient was found to have 46, XY, del(1)(p32). The prognosis of ALL- E0 seems to be determined by the effect of the chemotherapy on ALL, complications of chemotherapy and hypereosinophilia. After having been diagnosed, our patient accepted three course of VDCP (vincristine, daunomycin, Cyclophosphamide and prednisone), two courses of high dose arabinosyl cytosine. He is still in hematological remission and is preparing for allogenetic hemopoietic stem cell transplantation.

Abstract:Polymyositis (PM) and dermatomyositis (DM) are inflammatory myopathic diseases that often accompany cancers. However, the relationship between PM/DM and acute myelocytic leukemia (AML) has not been elucidated. We present a case of PM that developed AML 12 months after initial diagnosis. We reviewed the cases in English literature and analyzed the association between PM/DM and AML. We conclude that PM/DM is a paraneoplastic syndrome of AML.
Flame Figures Associated with Eosinophilic Dermatosis of Hematologic Malignancy: is It Possible to Distinguish the Condition from Eosinophilic Cellulitis in Patients with Hematoproliferative Disease?

Abstract:Eosinophilic dermatosis of hematologic malignancy is a multifaceted dermatosis with a wide morphological spectrum, presenting as pruritic, erythematous, papular and occasionally vesicular, urticarial, nodular eruptions. Histopathologically eosinophil infiltration in the super and deep dermis was found. We reported a case of eosinophilic dermatosis of hematologic malignancy presented as urticarial and vesicular lesions in a patient with chronic lymphocytic leukemia. A skin biopsy revealed a prominent subepidermal blister and a diffuse infiltrate of eosinophils with flame figures in the dermis and subcutaneous tissue. Although flame figures associated with eosinophilic dermatosis of hematologic malignancy is rarely reported, we believe that it would not seem unusual to find them in this skin disease. Eosinophilic cellulitis, which share clinical and histological features with eosinophilic dermatosis of hematologic malignancy, has also been described as showing an association with hematoproliferative diseases. In order to clearly describe eosinophilic dermatosis in patients with hematologic malignancies, the terminology eosinophilic dermatosis of hematologic malignancy, instead of eosinophilic cellulitis, would be a more suitable term in patients with eosinophilic dermatosis.

Subepidermal Bullous Dermatosis Associated with Iga Multiple Myeloma: A Case Report

Abstract:We describe a case of a 76-year-old man who initially presented with pruritic vesiculobullous eruptions on his trunk and shoulders and was subsequently found to have an immunoglobulin (Ig) A kappa multiple myeloma. Chemotherapy and plasmapheresis led to a dramatic resolution of the skin lesions, which paralleled the fall in serum IgA paraprotein level. However, the myeloma later relapsed, and the resulting paraprotein increase was accompanied by recurrence of vesiculobullous eruptions. The histopathological examinations of both primary and recurrent bullous eruptions demonstrated subepidermal bullae with negative direct immunofluorescence assays. Indirect immunofluorescence test detected neither antibasement membrane nor anti-intercellular circulating antibodies. This is a very rare report of bullous dermatosis with elevated IgA kappa paraprotein that appears before the diagnosis of myeloma, and it is a unique case showing an eosinophil-predominant infiltrate within subepidermal bullae and negative direct and indirect immunofluorescence. As the clinical features and laboratory findings of the bullous eruptions in our patient and the other 2 similar cases were not consistent with the diagnosis of any known bullous disorders, the subepidermal bullous dermatoses might be considered as a novel paraneoplastic entity occurring in association with the underlying IgA multiple myeloma.

Abstract:Cutaneous manifestations of hematologic malignancy represent both a clinical challenge for the treating physician and a pathophysiological model for advancing the knowledge on individual neoplasms. Indeed, a growing body of evidence supports the concept of recurrent molecular defects associating with specific clinical features, as best exemplified by VEXAS. Herein neutrophilic and eosinophilic dermatoses of potential interest for both hematologists and dermatologists will be reviewed, including subcorneal pustular dermatosis-type IgA pemphigus, neutrophilic eccrine hidradenitis, Sweet’s syndrome as well as myelodysplasia cutis and VEXAS, pyoderma gangrenosum, eosinophilic annular erythema, eosinophilic dermatosis of hematological malignancy, Wells syndrome and cutaneous involvement in hypereosinophilic syndromes. Possible management approaches are discussed for each, emphasizing scenarios that require treatment of the underlying condition to achieve remission at the skin level.

Unexpected Multiple Gastrointestinal Cancers in a Patient with Chronic Eosinophilia: A Case Report

Abstract:While the relationship between eosinophilia and tumors has been extensively studied, eosinophilia is primarily observed clinically after tumor development, with no prior reports of eosinophilia occurring before tumor onset. This case report presents a unique instance of eosinophilia preceding the development of gastric cancer in an old male patient. The patient was evaluated after being diagnosed with gastric cancer. Investigations included physical examinations, hematological evaluations, molecular assays for genetic variations associated with eosinophilia, tumor marker assessments, staging using the AJCC system, histopathological examinations, and comprehensive sequencing of tumor tissue. The patient exhibited disseminated erythematous macules and had a history of eosinophilia. Genetic testing for germline variations associated with eosinophilia was negative, and molecular assays did not detect significant genetic alterations linked to clonal eosinophilia. Staging revealed T1N0MX gastric cancer, with histopathology showing high-grade intraepithelial neoplasia and positive PD-L1 expression. Notably, sequencing identified mutations in the NOTCH1 and NOTCH2 genes, which are known to regulate eosinophil migration. After surgical intervention, pathological examinations confirmed high-grade squamous cell carcinoma in the esophagus and high-grade intraepithelial neoplasia in the stomach, without eosinophil infiltration. Surprisingly, the peripheral blood eosinophilia dramatically decreased following tumor resection. This case underscores the potential for eosinophilia to occur prior to tumor development, challenging the current understanding of the relationship between eosinophilia and cancer. Further research is warranted to explore the implications of eosinophilia in cancer pathogenesis and its clinical significance.

Abstract:A case of metastatic cutaneous plasmacytoma is reported.A 50-year-old male presented with multiple red nodules on the left chest for 1 month.He was diagnosed as multiple myeloma in department of hematology half a year ago.Physical examination showed multiple round nodules with clear bouudaries and smooth surface on the left chest.Pathological examination showed nodular infiltration of plasma cell-like cells.Immunohistochemical staining showed positive for CD38,CD138 and λchain.The diagnosis of metastatic cutaneous plasmacytoma was made.The skin lesions resolved after CMDT chemotherapy.

Abstract:Dermatomyositis (DM) is well-known to be associated with several types of malignancy. This case emphasizes the importance of a thorough examination for an underlying cancer, in patients with the symptoms of dermatomyositis. We report the case of a 62-year-old Chinese man who presented with a two-month history of edema of face and neck, together with erythema of the eyelids diagnosed of small cell lung cancer. Initially, it was thought to be single connective tissue disease such as DM. This study highlights the importance of a thorough physical examination when visiting a patient.

Abstract:Abstract Eosinophilic dermatosis of haematological malignancy (EDHM) is a rare condition first reported in 1965. Although associated with a myriad of haematoproliferative conditions, it is most commonly linked with chronic lymphocytic leukaemia (CLL). With its typical lesions being intensely pruritic erythematous papules, nodules and bullae, it was initially thought to be a hypersensitivity reaction to insect bites. A 70-year-old man was referred for an 11-year history of an overexuberant response to insect bites. He gave a history of an intermittent blistering rash associated with intense generalized pruritus. He noted lesions occurring on both exposed and nonexposed body sites, and throughout the year. His background was significant for CLL, under surveillance, which was diagnosed a decade ago. Physical examination revealed erythematous papules and nodules on the neck, forehead and limbs, with an intact tense bulla at the left ankle. The morphology observed raised suspicion for Sweet syndrome, bullous pemphigoid, cutaneous T-cell lymphoma and EDHM. Laboratory investigations revealed an elevated lymphocyte count in keeping with his known diagnosis of CLL. Other results were within normal limits. Culture biopsy, and direct and indirect immunofluorescence studies were negative. Biopsies illustrated a dense pandermal perivascular lymphocytic inflammation, with an associated lymphocytic vasculitis. Numerous eosinophils were seen. Immunohistochemistry highlighted no convincing involvement by CLL. No clonal T-cell receptor gene rearrangements were identified. Given both the clinical and pathological features, a diagnosis of EDHM was made. Our patient responded to a course of weaning oral steroids commenced synchronously for a respiratory tract infection, alongside moderate-potency topical steroids. In the event of a future flare, steroid-sparing agents to be considered include dapsone and doxycycline, which have shown efficacy in the literature. EDHM can precede, antedate or occur concurrently with a CLL diagnosis. Although exaggerated arthropod responses are associated with CLL, EDHM is believed to be a separate process. Its pathogenesis remains unknown, but hypotheses include underlying malignancy-related immune dysregulation, with a shift towards a T helper type 2 response and excess interleukin-4 and interleukin-5 production, which drive eosinophil recruitment and stimulation. This chronic relapsing dermatosis is characterized by clinical polymorphism and suggestive, but nonspecific, histopathological traits. Careful clinicopathological correlation is essential for diagnosis. Management options depend on the severity and frequency of flares. Varying responses to treatment are reported, but there is no gold-standard treatment. It is important that dermatologists are aware of EDHM as its diagnosis carries a potential negative prognostic outlook on the underlying haematological malignancy.

A systematic review of the management of eosinophilic dermatosis of hematological malignancy

Abstract:Eosinophilic dermatosis of hematologic malignancy (EDHM) is a cutaneous manifestation seen in patients with hematoproliferative and lymphoproliferative disorders, most commonly chronic lymphocytic leukemia. This systematic review aimed to summarize the therapeutic interventions of EDHM. A comprehensive search yielded 71 studies, predominantly case reports and series. The most frequently reported modalities were systemic and topical corticosteroids, as well as treatment of the underlying malignancy. Responses to these treatments varied. Targeted therapies, including dupilumab and omalizumab, showed promise, as did other modalities such as montelukast, dapsone, doxycycline, and phototherapy. Higher-quality studies should be conducted to facilitate higher-quality management recommendations for EDHM.
Overexpression of helper T cell type 2-related molecules in the skin of patients with eosinophilic dermatosis of hematologic malignancy

Abstract:Background: Eosinophilic dermatosis of hematologic malignancy (EDHM) is a rare dermatosis associated with blood tumors. Objective: To characterize the expression of T-cell and B-cell markers and pruritogenic mediators in EDHM skin. Methods: Immunohistochemical and immunofluorescence analysis were performed in 12 skin samples of EDHM, 11 samples of bullous pemphigoid (BP), and 5 samples from healthy controls (HC). Serum levels of interleukin (IL) 4 were analyzed in 11 patients with EDHM, 11 BP patients, and 5 HC by enzyme-linked immunosorbent assay. Results: T-cell markers, including clusters of differentiation (CD) 3, CD4, CD8, and CD5 were significantly overexpressed in EDHM and BP skin compared to HC. A predominance of CD4+ over CD8+ cells and GATA3+ (helper T cell type 2 [Th2] marker) over T-bet+ (Th1 marker) cells were observed. FOXP3 expression was increased but the FOXP3/CD4 ratio was low. B-cell markers were under-represented, without significant differences between the 3 groups. IL-4 and IL-31 were significantly overexpressed in EDHM and BP compared to HC and colocalized with the Th2-associated marker GATA3. Eotaxin-1 was significantly overexpressed in EDHM compared to BP and HC. IL-4 serum concentration was significantly increased in EDHM and BP compared to HC. Limitations: Small sample size; retrospective design. Conclusions: Targeting Th2-related molecules, in particular IL-4, holds promise for EDHM management.
Acute Lymphocytic Leukemia with Eosinophilia: a Case Report and Review of the Literature

Abstract:We present a 61-year-old man with marked peripheral blood eosinophilia, feature of hypereosinophilic syndrome, that later evolved into acute lymphocytic leukemia (ALL-L2). Initially the patient suffered from significant complications related to eosinophilic toxicity, including large urticarial hyperpigmented plaques, myocardial infarction, and eosinophilic pneumonia. He was treated with high dose of steroids resulting in a rapid suppression of the eosinophilia. Two weeks later, the eosinophilia had relapsed, so a bone marrow aspiration was performed. Cytomorphological examination of the bone marrow showed typical ALL features, while flow cytometric analysis revealed an My+pre-B-ALL immunophenotype, and chromosome analysis of bone marrow showed a normal karyotype. He received chemotherapy according to the standard protocol for ALL and died from refractory respiratory failure and congestive heart failure immediately after antileukemic therapy. We review the literature and compare the demographics, clinical features, and outcomes of several cases and reported studies.

Abstract:Dermatomyositis represents an autoimmune disorder characterized by notable skin and muscular manifestations. The annual incidence of dermatomyositis stands at approximately (5~10)/1 million individuals. Notably, patients with malignant tumors exhibit an elevated risk of developing dermatomyositis compared to the general population. However, in cases where dermatomyositis co-occurs with malignancy, the efficacy of hormone therapy alone tends to be suboptimal. Moreover, reports addressing the correlation between tumor treatment and the management of dermatomyositis are scarce. A 60-year-old male patient presented with dermatomyositis, initially manifesting through symptoms such as rash, muscle weakness, and dysphagia. Despite undergoing standard hormone therapy, there was no discernible improvement in the dermatomyositis symptoms. Considering the patient's concomitant troublesome cough, further investigations were conducted, including CT, PET-CT, and pathological biopsy. These assessments confirmed the diagnosis of limited-stage small cell lung cancer (T1cN3M0 IIIB). Notably, in this patient, dermatomyositis was suspected to be a paraneoplastic syndrome associated with small cell lung cancer. Standard chemotherapy and radiotherapy were employed to treat the small cell lung cancer, resulting in partial remission after two treatment cycles. As the malignancy regressed, a notable improvement in dermatomyositis symptoms was observed, subsequently leading to a gradual reduction in the prescribed hormone dosage. In conclusion, we present a comprehensive case study of dermatomyositis as a paraneoplastic syndrome throughout the treatment process. The response to tumor therapy coincided with the amelioration of dermatomyositis symptoms. Therefore, diligent malignancy screening is imperative for patients diagnosed with dermatomyositis.

Abstract:Peripheral eosinophilia can be the presenting sign in many cutaneous diseases but is often missed as a marker of a serious undiagnosed underlying disease such as atopic dermatitis, urticaria, drug eruption, bullous pemphigoid, inflammatory bowel disease, helminthic infection, Churg-Strauss syndrome, rheumatoid arthritis, or lymphoma. We report a case of non-Hodgkin's lymphoma presenting as persistent eosinophilia with a diffuse nodular cutaneous eruption.

Abstract:Abstract We present a case of a patient presenting with severe insect bite-like reactions in her skin with underlying chronic lymphocytic leukaemia (CLL). A 71-year-old woman was diagnosed with CLL in 2017 after presenting with a low-level lymphocytosis. She has never required treatment for CLL and remains under active observation in the haematology clinic. She was initially referred to dermatology 5 years ago with a nonresolving insect bite-like rash on her chest. The biopsy of the lesion was thought to be reactive with CLL infiltration. Over the years she has had a number of flare-ups of differing severity; the rash has always been itchy, and she has often presented with tense vesiculobullous lesions, occasionally haemorrhagic in appearance. Multiple biopsies were taken over a period of 5 years due to the diagnostic challenge, with the main differentials being urticarial vasculitis, insect bite-like reactions, Wells syndrome/dermal hypersensitivity reaction and Churg–Strauss syndrome. One of the common trends with the patient’s rash and blistering lesions is that they have often flared up during the summer, during gardening or travelling, and she has also reported them after receiving the COVID-19 vaccine. There was no reaction following the flu vaccine. Biopsies were always negative for direct immunofluorescence, ruling out immunobullous reactions. There was often the presence of a heavy neutrophilic infiltrate, vascular inflammation, and vague granulomata with absence of frank vasculitis. Following multidisciplinary team discussion and close clinicopathological correlation, we have concluded that this could be eosinophilic dermatosis of haematological malignancy mimicking insect bites, of which only a few reported cases have been documented. This case report reviews the association between CLL and insect bite-like reaction and highlights the challenges associated with diagnosing and managing such dermatological manifestations. Previous literature showed no clear association with regards to treatment of CLL causing regression of the dermatological manifestation. The treatment is difficult and recurrences are common (Rodríguez-Lojo R, Almagro M, Piñeyro F et al. Eosinophilic panniculitis and insect bite-like eruption in a patient with chronic lymphocytic leukaemia: a spectrum of the same entity. Dermatol Res Pract 2010; 2010: 263827). In our case, the plan is to actively treat the underlying haematological malignancy and to assess the possibility of bringing the cutaneous manifestation under control. This case also highlights the lack of tailored therapeutic strategies for patients with CLL, in the context of its diverse and complex dermatological presentation.

Abstract:Psoriasis and atopic dermatitis (AD) are prevalent inflammatory skin disorders, each stemming from diverse factors, and characterized by recurring episodes. In certain complex cases, the clinical and pathological features exhibit overlapping and atypical characteristics, making accurate clinical diagnosis and targeted treatment a challenge. Psoriasiform dermatitis is the term used to describe such cases. Moreover, when patients have a history of malignancy, the situation becomes even more intricate, resulting in limited treatment options. Biologic therapies have transformed the management of immune-mediated inflammatory diseases, including psoriasis and AD. Meanwhile, the safety of biologics in special populations, especially among patients with a history of malignancy, should be underlined. The selective Janus kinase 1 (JAK1) inhibitor abrocitinib has been approved for the treatment of AD and has showed satisfying efficacy and safety in the treatment of psoriasis in clinical trials. Although unreported, JAK1 inhibitors are thought to have the potential to increase the risk of potential tumors. Apremilast, an oral phosphodiesterase (PDE)-4 inhibitor, is approved for moderate to severe plaque psoriasis. It has been investigated for its efficacy in AD, and is not contraindicated in malignancy. This report presents three cases of psoriasiform dermatitis in patients with a history of malignancy, showcasing significant improvement following treatment with systemic glucocorticoid, abrocitinib, or apremilast.

Cutaneous Leukemic Infiltrates Successfully Treated With Biomodulatory Therapy in a Rare Case of Therapy-Related High Risk MDS/AML

Abstract:Cutaneous manifestations in hematologic malignancies, especially in leukemia, are not common and may be very variable. Here we report a very unusual case of a patient (female, 70 years old) who was admitted to the hospital in 2016 because of skin lesions on the face, the trunk of the body and the extremities. She had a history of breast cancer in the year 2004 (pT1b, pN0, cM0, L0, V0, R0) which had been resected and treated with adjuvant radiation and chemotherapy (cyclophosphamide, methotrexate, 5-fluorouracile) as well as psoriasis treated with methotrexate and cyclosporine. Because of mild cytopenia a bone marrow aspirate/biopsy was performed showing myelodysplastic syndrome (MDS) with multilineage dysplasia. Cytogenetic review revealed a complex aberrant karyotype denoting adverse outcome. Simultaneously, a skin biopsy could confirm leukemic skin infiltration. Consequently, a therapy with azacitidine was started. After the first cycle the patient developed severe pancytopenia with a percentage of 13% peripheral blasts (previously 0-2%) as well as fever without evidence for infection which was interpreted as progressive disease. Therefore, the therapeutic regimen was changed to a biomodulatory therapy consisting of low-dose azacitidine 75 mg/day (given sc d1-7 of 28), pioglitazone 45 mg/day per os, and all-trans-retinoic acid (ATRA) 45 mg/m2/day per os. After cycle 1 of this combined biomodulatory therapy the patient showed hematologic recovery; besides a mild anemia (hemoglobin 11.1 g/dl) she developed a normal blood count. Moreover, the cutaneous leukemic infiltrates which had been unaffected by the azacitidine ameliorated tremendously after 2 cycles resulting in a complete remission of the skin lesions after cycle 6. In conclusion, we report a very unusual case with cutaneous infiltrates being the first clinical manifestation of hematologic disease, preceding the development of acute myeloid leukemia. While azacitidine alone was ineffective, a combined biomodulatory approach resulted in a complete remission of the cutaneous manifestation.

Abstract:Eosinophilia is rare in acute leukemia at presentation. Discrete reports and case studies in recent years have created significant interest in the field of "Acute leukemia with eosinophilia". We herein present two cases of eosinophilia in association with acute lymphoblastic leukemia with brief review of literature in this field. First case is about 21-year-old female who presented with mediastinal mass along with leukocytosis and hypereosinophilia. On evaluation, she was found to have T cell acute lymphoblastic leukemia. After ruling out benign causes of eosinophilia, she was treated with modified BFM-90 protocol. Her eosinophilia resolved after 4 weeks of induction therapy. Second case is about 32-year-old male who was diagnosed as a case of mixed phenotype leukemia (B cell/myeloid type) along with severe eosinophilia. His hypereosinophilia finally resolved by week 16 of modified BFM-90 protocol. Diagnosing ALL is challenging when eosinophilia is the initial presentation. These two cases emphasize on the importance of considering ALL amongst one of the etiological causes of eosinophilia as delay in diagnosis endangers patient's life at risk. Also eosinophilia per se is an independent poor risk factor, hence prompt diagnosis and early treatment is the key in all such cases.
