Abstract:Purpose The diagnostic performance of the Prostate Imaging Reporting and Data System version 2 (PI-RADS v2) has been challenged due to its lower diagnostic accuracy and higher false-positive rates for prostate cancer detection. This study aimed to analyze the diagnostic performance of PI-RADS v2 in combination with clinical parameters in patients with suspected prostate cancer. Material and Methods A total of 424 men with suspicion of prostate cancer were retrospectively analyzed. Logistic regression analyses were performed to identify predictors of clinically significant prostate cancer defined as a Gleason score of 3 + 4 or greater. The prediction performance was compared with prostate specific antigen (PSA), free/total PSA ratio (f/t PSA), PSA density (PSAD), PI-RADS v2 alone, and PI-RADS v2 plus PSAD using receiver operating characteristics (ROCs). Results In total, 231 out of 424 cases (54.48%) were pathologically diagnosed as prostate cancer. The percentage of clinically significant prostate cancer was higher in patients with PI-RADS v2 score of 4 or greater compared to PI-RADS v2 score of less than 4 (90.86% vs. 55.88%, P < 0.001). Age, PSA level, f/t PSA, PSAD, and PI-RADS v2 were significant independent predictors of clinically significant prostate cancer. The ROC area under the curve of PI-RADS v2 plus PSAD (0.952) was larger compared with PSA (0.845), f/t PSA (0.719), PSAD (0.920), and PI-RADS v2 alone (0.885). Conclusion PI-RADS v2 in combination with PSAD may help detect clinically significant prostate cancer and provide benefit in making the decision to biopsy men at suspicion of prostate cancer.

Abstract:Background: Prostate cancer is one of the most prevalent cancers in the male population. To determine the aggressiveness of suspected lesions precisely, predictive models are increasingly being developed using quantitative MRI measurements, and particularly the ADC value. This study aimed to determine whether ADC values could be used to establish the aggressiveness of prostate cancer. Methods: A retrospective single-center study included 398 patients with prostate cancer who underwent a multiparametric MRI prior to radical prostatectomy. DWI ADC values were measured (μm2/s) using b values of 50 and 1000. The dominant lesion best visualized on MRI was analyzed. The ADC values of the index lesion and reference tissue were compared to tumor aggressivity according to the Gleason grade groups based on radical prostatectomy results. Statistical analysis was performed using the Mann–Whitney U test, Kruskal–Wallis H test, Spearman's rank correlation, and ROC curves. Results: A very strong negative correlation (rs = −0.846, p < 0.001) between ADC and GS was found. ROC analysis revealed an AUC of 0.958 and an ADC threshold value of 758 μm2/s in clinically significant prostate cancer diagnoses using the absolute ADC value, with no advantage of using the ADC ratio over the absolute ADC value being identified. Conclusion: DWI ADC values and the calculated ADC ratio have a significant inverse correlation with GS. The findings indicate a strong capability in determining prostate cancer aggressiveness, as well as the possibility of assisting with assigning PI-RADS categories using ADC as quantitative metrics.

Abstract:OBJECTIVE: The objective of our study was to evaluate apparent diffusion coefficient (ADC) and various ADC ratios in determining aggressiveness of prostate cancer.MATERIALS AND METHODS: One hundred sixty-five patients with biopsy-proven prostate cancer underwent 3-T MRI followed by radical prostatectomy. ADC and ADC ratios were calculated using the peripheral zone, transition zone, same zone as the tumor, and urinary bladder as references. ADC and ADC ratios were correlated with Gleason score using the Spearman correlation coefficient (ρ) and were compared between low-grade (Gleason score = 6) and high-grade (Gleason score ≥ 7) prostate cancer using the unpaired t test. ROC curves were used to compare diagnostic accuracies of ADC and ADC ratios in determining high-grade prostate cancer.RESULTS: Fifty-six (33.9%) and 109 (66.1%) patients had low- and high-grade prostate cancer, respectively. ADC (ρ = -0.476) and all ADC ratios (ρ = -0.397, -0.412, -0.381, and -0.474, respectively) correlated significantly with Gleason score (p < 0.001) and were significantly lower in patients with high-grade prostate cancer (p < 0.001). For predicting high-grade prostate cancer, tumor ADC and tumor-to-urinary bladder ADC ratio showed the highest AUC (0.794 and 0.790, respectively) but without statistically significant difference (p = 0.803). AUC of tumor ADC (0.794) was statistically significantly higher than those of the tumor-to-peripheral zone and tumor-to-transition zone ADC ratios (0.746, p = 0.039; 0.751, p = 0.027; respectively). AUC of tumor ADC was not statistically significantly higher than that of the tumor-to-tumor zone ADC ratio (0.763, p = 0.193). AUC calculated using the tumor-to-urinary bladder ADC ratio was statistically significantly higher than that using the tumor-to-transition zone ADC ratio (p = 0.028) and marginally higher than that from tumor-to-peripheral zone ADC ratio (p = 0.080). Otherwise, no significant differences were seen in the AUCs (p = 0.193-0.828).CONCLUSION: Both ADC and various ADC ratios correlated significantly with Gleason score and were significant predictors of high-grade prostate cancer. However, no benefit was found in using ADC ratio over ADC.
Assessment of a fully-automated diagnostic AI software in prostate MRI: Clinical evaluation and histopathological correlation

Abstract:Objective: This study aims to evaluate the diagnostic performance of a commercial, fully-automated, artificial intelligence (AI) driven software tool in identifying and grading prostate lesions in prostate MRI, using histopathological findings as the reference standard, while contextualizing its performance within the framework of PI-RADS v2.1 criteria. Material and methods: This retrospective study analyzed 123 patients who underwent multiparametric prostate MRI followed by systematic and targeted biopsies. MRI protocols adhered to international guidelines and included T2-weighted, diffusion-weighted, T1-weighted, and dynamic contrast-enhanced imaging. The AI software tool mdprostate was integrated into the Picture Archiving and Communication System to automatically segment the prostate, calculate prostate volume, and classify lesions according to PI-RADS scores using biparametric T2-weighted and diffusion-weighted imaging. Histopathological analysis of biopsy cores served as the reference standard. Diagnostic performance metrics including sensitivity, specificity, positive and negative predictive value (PPV, NPV), and area under the ROC curve (AUC) were calculated. Results: mdprostate demonstrated 100 % sensitivity at a PI-RADS ≥ 2 cutoff, effectively ruling out both clinically significant and non-significant prostate cancers for lesions remaining below this threshold. For detecting clinically significant prostate cancer (csPCa) using a PI-RADS ≥ 4 cutoff, mdprostate achieved a sensitivity of 85.5 % and a specificity of 63.2 %. The AUC for detecting cancers of any grade was 0.803. The performance metrics of mdprostate were comparable to those reported in two meta-analyses of PI-RADS v2.1, with no significant differences in sensitivity and specificity (p > 0.05). Conclusion: The evaluated AI tool demonstrated high diagnostic performance in identifying and grading prostate lesions, with results comparable to those reported in meta-analyses of expert readers using PI-RADS v2.1. Its ability to standardize evaluations and potentially reduce variability underscores its potential as a valuable adjunct in the prostate cancer diagnostic pathway. The high accuracy of mdprostate, particularly in ruling out prostate cancers, highlights its clinical utility by reducing workload and enhancing patient outcomes.

Abstract:PURPOSE: This study has focused on investigating the relationship between the exponential apparent diffusion coefficient (exp-ADC), selective apparent diffusion coefficient (sel-ADC) values, the ADC ratio (ADCr), and prostate cancer aggressiveness with transrectal ultrasound-guided prostate biopsy in patients with prostate cancer.MATERIAL AND METHODS: All patients underwent a multiparametric magnetic resonance imaging (mpMRI) including tri-planar T2-weighted (T2W), dynamic contrast-enhanced (DCE), diffusion-weighted sequences using a 3.0-Tesla MR scanner (Skyra, Siemens Medical Systems, Germany) with a dedicated 18-channel body coil and a spine coil underneath the pelvis, with the patient in the supine position. Exp-ADC, sel-ADC, and ADCr of defined lesions were evaluated using region-of-interest-based measurements. Exp-ADC, sel-ADC, and ADCr were correlated with the Gleason score obtained through transrectal ultrasound-guided biopsy.RESULTS: Patients were divided into 2 groups. Group I is Gleason score ≥ 3 + 4, group II is Gleason score = 6. Sel-ADC and exp-ADC were statistically significant between 2 groups (0.014 and 0.012, respectively). However, the ADCr difference between nonclinical significant prostate cancer from clinically significant prostate cancer was not significant (<i>p</i> = 0.09).CONCLUSIONS: This study is the first to evaluate exp-ADC and sel-ADC values of prostate carcinoma with ADCr. One limitation of this study might be the limited number of patients. Exp-ADC and sel-ADC values in prostate MRI imaging improved the specificity, accuracy, and area under the curve (AUC) for detecting clinically relevant prostate carcinoma. Adding exp-ADC and sel-ADC values to ADCr can be used to increase the diagnostic accuracy of DWI.

Diagnostic Performance of Multiparametric MRI Parameters for Gleason Score and Cellularity Metrics of Prostate Cancer in Different Zones: a Quantitative Comparison

Abstract:AIM: To compare the diagnostic performance of multiparametric MRI (mpMRI) parameters for Gleason score (GS) and cellularity metrics of prostate cancer (PCa) in the peripheral zone (PZ) and transition zone (TZ) separately. MATERIALS AND METHODS: In total, 225 PCa patients with preoperative mpMRI and whole-mount pathological sections were enrolled retrospectively. Detection rates of index lesions (highest GS or largest dimension) and clinically significant PCa (csPCa) were evaluated. Tumour-to-muscle ratio and skewness of T2 signal intensity, average apparent diffusion coefficient (ADC(mean)) and 10th percentile ADC (ADC(1)(0%)) were derived and correlation with GS was performed with Spearman's correlation coefficient (rho), while effectiveness in differentiating GS 6 from GS >= 7 was compared with receiver operating characteristic (ROC) analysis. Moreover, correlation of cellularity metrics with mpMRI parameters was evaluated with Pearson's correlation coefficient (r). RESULTS: In total, 398 lesions were identified, with 87.1% (196/225) index lesions and 86.8% (249/287) csPCa detected. Compared to T2 parameters, ADC parameters, especially ADC(mean), correlated better with GS (maximal rho: -0.58 versus -0.33, p=0.011) and yielded significantly higher area under the curve (AUC) in differentiating GS 6 from GS >= 7 (maximal AUC: 0.854 versus 0.731, p=0.020) among PZ lesions. Moreover, ADC(mean) demonstrated significantly moderate correlation with the nuclear-to-cytoplasmic ratio and nuclear fraction (r=-0.403 and -0.514, p<0.001); however, for TZ lesions, all parameters demonstrated poor correlation with GS and cellularity metrics. CONCLUSION: mpMRI could effectively detect index and csPCa lesions. ADC parameters, especially ADC(mean), correlated better with GS and cellularity metrics than T2 in PZ, while all parameters demonstrated poor performance within TZ lesions. Abstract:Aim. Accurate diagnosis of prostate cancer (PCa) has a fundamental role in clinical and patient care. Recent advances in diagnostic testing and marker lead to standardized interpretation and increased prescription by clinicians to improve the detection of clinically significant PCa and select patients who strictly require targeted biopsies. Methods. In this study, we present a systematic review of the overall diagnostic accuracy of each testing panel regarding the panel details. In this meta-analysis, using a structured search, Web of Science and PubMed databases were searched up to 23 September 2019 with no restrictions and filters. The study's outcome was the AUC and 95% confidence interval of prediction models. This index was reported as an overall and based on the WHO region and models with/without MRI. Results. The thirteen final articles included 25,691 people. The overall AUC and 95% CI in thirteen studies were 0.78 and 95% CI: 0.73–0.82. The weighted average AUC in the countries of the Americas region was 0.73 (95% CI: 0.70–0.75), and in European countries, it was 0.80 (95% CI: 0.72–0.88). In four studies with MRI, the average weighted AUC was 0.88 (95% CI: 0.86–0.90), while in other articles where MRI was not a parameter in the diagnostic model, the mean AUC was 0.73 (95% CI: 0.70–0.76). Conclusions. The present study's findings showed that MRI significantly improved the detection accuracy of prostate cancer and had the highest discrimination to distinguish candidates for biopsy.
Accuracy of Dynamic Contrast-Enhanced Magnetic Resonance Imaging in the Diagnosis of Prostate Cancer: Systematic Review and Meta-Analysis

Abstract:The goals of this meta-analysis were to assess the effectiveness of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in patients with prostate carcinoma (PCa) and to explore the risk profiles with the highest benefit. Systematic electronic searched were conducted in database. We used patient-based and biopsy-based pooled weighted estimates of the sensitivity, specificity, and a summary receiver operating characteristic (SROC) curve for assessing the diagnostic performance of DCE. We performed direct and indirect comparisons of DCE and other methods of imaging. A total of 26 articles met the inclusion criteria for the analysis. DCE-MRI pooled sensitivity was 0.53 (95% CI 0.39 to 0.67), with a specificity of 0.88 (95% CI 0.83 to 0.92) on whole gland. The peripheral zone pooled sensitivity was 0.70 (95% CI 0.46 to 0.86), with a specificity of 0.88 (95% CI 0.76 to 0.94). Compared with T2-weighted imaging (T2WI), DCE was statistically superior to T2. In conclusion, DCE had relatively high specificity in detecting PCa but relatively low sensitivity as a complementary functional method. DCE-MRI might help clinicians exclude cases of normal tissue and serve as an adjunct to conventional imaging when seeking to identify tumor foci in patients with PCa.
Quantitative Analysis of Multiparametric Prostate Mr Images: Differentiation Between Prostate Cancer and Normal Tissue and Correlation with Gleason Score-A Computer-Aided Diagnosis Development Study

Abstract:PURPOSE To evaluate the potential utility of a number of parameters obtained at T2-weighted, diffusion-weighted, and dynamic contrast material-enhanced multiparametric magnetic resonance (MR) imaging for computer-aided diagnosis (CAD) of prostate cancer and assessment of cancer aggressiveness. MATERIALS AND METHODS In this institutional review board-approved HIPAA-compliant study, multiparametric MR images were acquired with an endorectal coil in 48 patients with prostate cancer (median age, 62.5 years; age range, 44-73 years) who subsequently underwent prostatectomy. A radiologist and a pathologist identified 104 regions of interest (ROIs) (61 cancer ROIs, 43 normal ROIs) based on correlation of histologic and MR findings. The 10th percentile and average apparent diffusion coefficient (ADC) values, T2-weighted signal intensity histogram skewness, and Tofts K(trans) were analyzed, both individually and combined, via linear discriminant analysis, with receiver operating characteristic curve analysis with area under the curve (AUC) as figure of merit, to distinguish cancer foci from normal foci. Spearman rank-order correlation (ρ) was calculated between cancer foci Gleason score (GS) and image features. RESULTS AUC (maximum likelihood estimate ± standard error) values in the differentiation of prostate cancer from normal foci of 10th percentile ADC, average ADC, T2-weighted skewness, and K(trans) were 0.92 ± 0.03, 0.89 ± 0.03, 0.86 ± 0.04, and 0.69 ± 0.04, respectively. The combination of 10th percentile ADC, average ADC, and T2-weighted skewness yielded an AUC value for the same task of 0.95 ± 0.02. GS correlated moderately with 10th percentile ADC (ρ = -0.34, P = .008), average ADC (ρ = -0.30, P = .02), and K(trans) (ρ = 0.38, P = .004). CONCLUSION The combination of 10th percentile ADC, average ADC, and T2-weighted skewness with CAD is promising in the differentiation of prostate cancer from normal tissue. ADC image features and K(trans) moderately correlate with GS.

Abstract:Background Prior research has shown that retrospectively measured apparent diffusion coefficient (ADC) of prostate magnetic resonance imaging (MRI) lesions is associated with clinically significant prostate cancer (csPCa) on targeted biopsy suggesting that ADC should be measured and reported prospectively. Purpose To assess the impact of mandatory prospective measurement of ADC on the rates of positivity across PI-RADS scores for csPCa. Material and Methods Consecutive patients who underwent ultrasound (US)-MRI fusion prostate biopsy from August 2018 to July 2019 and who had prospectively reported ADC were compared to control patients who did not. Rates of positivity by PI-RADS category were computed and compared using Chi-square. Multivariable regression was performed. Results In total, 126 patients (median age 65 years) with 165 prostate lesions (19, 51, 70, and 25 PI-RADS 2, 3, 4, and 5, respectively) and prospectively reported ADC values were compared to 113 control patients (median age 66 years) with 157 prostate lesions (17, 42, 64, and 34 PI-RADS 2, 3, 4, and 5, respectively). Rates of positivity across PI-RADS scores were similar between the two cohorts; 11%, 25%, 55%, and 76% and 0%, 21%, 56%, and 62% for PI-RADS 2, 3, 4, and 5 in the test and control cohorts, respectively (Chi-square P = 0.78). Multivariate logistic regression showed no significant association between the presence of prospectively measured ADC and csPCa (odds ratio 1.1, 95% confidence interval 0.7–1.7, P = 0.82). Conclusion Prospective ADC measurement may not impact PI-RADS category assignments or positivity rates for csPCa under current guidelines. Future versions of PI-RADS may need to incorporate ADC into scoring rules to realize their potential.

Abstract:Abstract Background Multiparametric MRI localizes cancer in the prostate, allowing for MRI guided biopsy (MRI-GB) 43 alongside transrectal ultrasound-guided systematic biopsy (TRUS-GB). Three MRI-GB approaches exist; visual estimation (COG-TB); fusion software-assisted (FUS-TB) and MRI ‘in-bore’ biopsy (IB-TB). It is unknown whether any of these are superior. We conducted a systematic review and meta-analysis to address three questions. First, whether MRI-GB is superior to TRUS-GB at detecting clinically significant PCa (csPCa). Second, whether MRI-GB is superior to TRUS-GB at avoiding detection of insignificant PCa. Third, whether any MRI-GB strategy is superior at detecting csPCa. Methods A systematic literature review from 2015 to 2019 was performed in accordance with the START recommendations. Studies reporting PCa detection rates, employing MRI-GB and TRUS-GB were included and evaluated using the QUADAS-2 checklist. 1553 studies were found, of which 43 were included in the meta-analysis. Results For csPCa, MRI-GB was superior in detection to TRUS-GB (0.83 vs. 0.63 [ p = 0.02]). MRI-GB was superior in detection to TRUS-GB at avoiding detection of insignificant PCa. No MRI-GB technique was superior at detecting csPCa (IB-TB 0.87; COG TB 0.81; FUS-TB 0.81, [ p = 0.55]). There was significant heterogeneity observed between the included studies. Conclusions In patients with suspected PCa on MRI, MRI-GB offers superior rates of csPCa detection and reduces detection of insignificant PCa compared to TRUS-GB. No individual MRI-GB technique was found to be better in csPCa detection. Prospective adequately powered randomized controlled trials are required.

Abstract:Background: Biparametric MRI (bpMRI) has an important role in the diagnosis of prostate cancer (PCa), by reducing the cost and duration of the procedure and adverse reactions. We assess the additional benefit of the ADC map in detecting prostate cancer (PCa). Additionally, we examine whether the ADC value correlates with the presence of clinically significant tumors (csPCa). Methods: 104 peripheral lesions classified as PI-RADS v2.1 score 3 or 3+1 at the mpMRI underwent transperineal MRI/US fusion-guided targeted biopsy. Results: The lesions were classified as PI-RADS 3 or 3+1; at histopathology, 30 were adenocarcinomas, 21 of which were classified as csPCa. The ADC threshold that maximized the Youden index in order to predict the presence of a tumor was 1103 (95% CI (990, 1243)), with a sensitivity of 0.8 and a specificity of 0.59; both values were greater than those found using the contrast medium, which were 0.5 and 0.54, respectively. Similar results were also found with csPCa, where the optimal ADC threshold was 1096 (95% CI (988, 1096)), with a sensitivity of 0.86 and specificity of 0.59, compared to 0.49 and 0.59 observed in the mpMRI. Conclusions: Our study confirms the possible use of a quantitative parameter (ADC value) in the risk stratification of csPCa, by reducing the number of biopsies and, therefore, the number of unwarranted diagnoses of PCa and the risk of overtreatment.

Comparison in prostate cancer diagnosis with PSA 4-10 ng/mL: radiomics-based model VS. PI-RADS v2.1

Abstract:Background: To evaluate accuracy of MRI-based radiomics in diagnosing prostate cancer (PCa) in patients with PSA levels between 4 and 10 ng/mL and compare it with the latest Prostate Imaging Reporting and Data System (PI-RADS v2.1) score. Methods: 221 patients with prostate lesions and PSA levels in 4-10 ng/mL, including 154 and 67 cases in the training and validation groups. Pathological confirmation of all patients was accomplished by the use of MRI-TRUS fusion targeted biopsy or systematic transrectal ultrasound (TRUS) guided biopsy. 851 radiomic features were extracted from each lesion of ADC and T2WI images. The least absolute shrinkage and selection operator (LASSO) regression algorithm and logistic regression were employed to select features and build the ADC and T2WI model. The combined model was obtained based on the ADC and T2WI features. The clinical benefit and diagnostic accuracy of the three radiomics models and PI-RADS v2.1 score were evaluated. Results: 10 radiomic features were ultimately selected from the ADC images, 13 from the T2WI images and 7 from the combined models. The ADC, T2WI and combined models achieved satisfactory diagnostic accuracy in the training [AUC:0.945 (ADC), 0.939 (T2WI), 0.979 (combined)] and validation groups [AUC: 0.942 (ADC), 0.943 (T2WI), 0.959 (combined)], which was significantly higher than those in PI-RADS v2.1 model (0.825 for training cohort and 0.853 for validation cohort). Compared with the PI-RADS v2.1 score, the three radiomics models generated superior PCa diagnostic performance in both the training (p = 0.002, p = 0.005, p < 0.001) and validation groups (p = 0.045, p = 0.035, p = 0.015). Conclusion: Radiomics based on ADC and T2WI images can better identify PCa in patients with PSA 4-10 ng/mL, and MRI-based radiomics significantly outperforms the PI-RADS v2.1 score. Abstract:Prostate MRI has traditionally relied on qualitative interpretation. However, quantitative components hold the potential to markedly improve performance. The ADC from DWI is probably the most widely recognized quantitative MRI biomarker and has shown strong discriminatory value for clinically significant prostate cancer (csPCa) as well as for recurrent cancer after treatment. Advanced diffusion techniques, including intravoxel incoherent motion, diffusion kurtosis, diffusion tensor imaging, and specific implementations such as restriction spectrum imaging, purport even better discrimination, but are more technically challenging. The inherent T1 and T2 of tissue also provide diagnostic value, with more advanced techniques deriving luminal water imaging and hybrid-multidimensional MRI. Dynamic contrast-enhanced imaging, primarily using a modified Tofts model, also shows independent discriminatory value. Finally, quantitative size and shape features can be combined with the aforementioned techniques and be further refined using radiomics, texture analysis, and artificial intelligence. Which technique will ultimately find widespread clinical use will depend on validation across a myriad of platforms use-cases.
The diagnostic value of synthetic MRI quantitative parameters for prostate cancer

Abstract:Objective:To investigate the diagnostic value of quantitative parameters of synthetic MRI in patients with prostate cancer (PCa).Methods:From April 2018 to April 2019, 25 PCa patients confirmed by whole-mount pathology were prospectively enrolled in Beijing Hospital. All patients underwent prostate MRI examinations including DWI and synthetic MRI. Four kinds of tissue types were identified in pathology, including PCa, stromal hyperplasia (SH), glandular hyperplasia (GH) and prostatitis. According to the pathological results, quantitative parameters including ADC, T 1, T 2 and proton density (PD) values were measured on DWI and synthetic MRI. One-way analysis of variance or Kruskal-Wallis H test was performed to compare the difference of each parameter among PCa, SH or GH in the transitional zone (TZ). The difference of each parameter between PCa and prostatitis in the peripheral zone (PZ) was evaluated by using independent sample t test or Mann-Whitney U test. The diagnostic performance of each parameter in discriminating PCa from other benign conditions was evaluated by using ROC curve. Area under the curve (AUC) of each parameter was compared by using DeLong test. Results:The overall differences of T 1, T 2 PD and ADC values were statistically significant among PCa and other benign conditions. The T 1, T 2 and ADC values of PCa in the TZ were lower than those in the SH ( P=0.041, 0.030,<0.001) and GH (all P<0.001). The PD value of PCa in the TZ was lower than that in the GH ( P=0.040). The differences of T 1, T 2, PD and ADC values between PCa and prostatitis in the PZ were significant ( P<0.05). The AUC of ADC value (0.943) was higher than that of T 1 (0.691, P=0.001), PD (0.555, P=0.002) and T 2 (0.754, P=0.016) values in differentiating PCa from SH. For discriminating PCa and GH, T 1, T 2 and ADC showed similar AUC ( P>0.05), which were higher than AUC of PD value ( P=0.001). For discriminating PCa and prostatitis, T 1, T 2, PD and ADC values showed similar AUC ( P>0.05). Conclusions:Quantitative parameters of synthetic MRI were practical tools for discriminating PCa from other benign pathologies.
Artificial Intelligence Compared to Radiologists for the Initial Diagnosis of Prostate Cancer on Magnetic Resonance Imaging: A Systematic Review and Recommendations for Future Studies

Abstract:Computer-aided diagnosis (CAD) of prostate cancer on multiparametric magnetic resonance imaging (mpMRI), using artificial intelligence (AI), may reduce missed cancers and unnecessary biopsies, increase inter-observer agreement between radiologists, and alleviate pressures caused by rising case incidence and a shortage of specialist radiologists to read prostate mpMRI. However, well-designed evaluation studies are required to prove efficacy above current clinical practice. A systematic search of the MEDLINE, EMBASE, and arXiv electronic databases was conducted for studies that compared CAD for prostate cancer detection or classification on MRI against radiologist interpretation and a histopathological reference standard, in treatment-naïve men with a clinical suspicion of prostate cancer. Twenty-seven studies were included in the final analysis. Due to substantial heterogeneities in the included studies, a narrative synthesis is presented. Several studies reported superior diagnostic accuracy for CAD over radiologist interpretation on small, internal patient datasets, though this was not observed in the few studies that performed evaluation using external patient data. Our review found insufficient evidence to suggest the clinical deployment of artificial intelligence algorithms at present. Further work is needed to develop and enforce methodological standards, promote access to large diverse datasets, and conduct prospective evaluations before clinical adoption can be considered.

Abstract:Purpose: To evaluate apparent diffusion coefficient (ADC) value, metabolic ratio ((Cho + Cr)/Cit) and the combination of the two in identifying prostate malignant regions. Materials and Methods: Fifty-six consecutive patients with prostate biopsy results were retrospectively recruited in this study. Transrectal ultrasound-guided (TRUS) systemic prostate biopsies were used as a standard of reference. Mean ADC value and mean metabolic ratio (MMR) were calculated within each benign sextant region or malignant region. The efficiency of these two indices in prostate cancer (PCa) diagnosis is estimated in Fisher linear discriminant analysis (FLDA). The area under the receiver operating characteristic (ROC) curve was used to evaluate the distinguishing capacity of mean ADC, MMR, and the combination of the two in differentiating between noncancerous and cancerous cases. Results: There were significant differences for mean ADC value and MMR between malignant and benign regions. Weights of mean ADC value obtained by FLDA were much higher than those of MMR. In differentiating malignant regions, both ADC alone and combined ADC and metabolic ratio performed significantly better than MMR alone. However, accuracy improvements were not significant by using combined ADC and MMR than ADC alone. Conclusion: DWI is more efficient than MR spectroscopic (MRS) in the detection of PCa in this study. Combined ADC and MMR performed significantly better than MMR alone in distinguishing malignant from benign region in prostate peripheral zone. J. Magn. Reson. Imaging 2013;37:194200. According to the standard reference of the RP histopathologic assessment, the performance of each radiomics models was compared with that of biopsy and Prostate Imaging Reporting and Data System version 2.1 (PI‐RADS) assessment.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Statistical Tests</h3><p>A receiver operating characteristic curve analysis was employed to estimate the area under the curve (AUC) values of the models. The AUCs of the four models, biopsy, and PI‐RADS assessment were compared using the DeLong test.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The four radiomics models yielded AUCs of 0.710, 0.731, 0.726, and 0.709 in the test cohort, respectively; biopsy and PI‐RADS assessment yielded AUCs of 0.793 and 0.680, respectively. 68Ga-Psma-11 PET/CT Combining ADC Value of MRI in the Diagnosis of Naive Prostate Cancer: Perspective of Radiologist.

Abstract:Ga-68-PSMA-11 positron emission computed tomography /computed tomography (PET/CT) is more sensitive than magnetic resonance imaging (MRI) in detecting prostate cancer (PCa). We evaluated the value of(68)Ga-PSMA-11 PET/CT with MRI in treatment-naive PCa. This retrospective study was approved by the hospital ethics committee. The MRI and(68)Ga-PSMA-11 PET/CT imaging data of 63 cases of highly suspected PCa were enrolled in this study. The SUV(max)and apparent diffusion coefficient (ADC), and their ratio, were assessed as diagnostic markers to distinguish PCa from benign disease. There were 107 prostate lesions detected in 63 cases. Forty cases with 64 malignant primary lesions were confirmed PCa, whereas 23 cases had 43 benign lesions. PSMA-avid lesions correlated with hypointense signal on ADC maps and hyperintense signal on diffusion-weighted imaging. The ADC of PCa was lower than that of benign lesions, and SUV(max)and SUVmax/ADC of PCa was higher than that of benign lesions (P < .01). ADC had significant negative correlation with Gleason score (GS) and SUVmax, SUVmax, and SUVmax/ADC positively correlated with GS. From ROC analysis, we established cutoff values of ADC, SUVmax, and SUVmax/ADC at 1.02 x 10(-3)mm(2)/s, 11.72, and 12.35, respectively, to differentiate PCa from benign lesions. The sensitivity, specificity, and AUC were 90.6%, 58.1%, and 0.816 for ADC, 67.2%, 97.7%, and 0.905 for SUVmax, and 81.2%, 88.4%, and 0.929 for SUVmax/ADC, respectively. Ga-68-PSMA-11 PET/CT combined with MRI offers higher diagnostic efficacy in the detection of PCa than either modality alone.
