Abstract:Comamonas kerstersii (C. kerstersii) is a Gram-negative bacillus abundant in the environment and rarely implicated in human disease. Previously considered nonpathogenic, its scarcity in literature may be partly due to the unreliability of past phenotypic tests used for its identification. In recent years, the development of matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) mass spectrometry (MS) has enabled fast and accurate laboratory identification of C. kerstersii. Since the first report of human infection in 2013, several others have emerged, with most cases involving peritoneal infection. Here, we present a rare case of C. kerstersii bacteremia in an 82-year-old male patient. With no clear predisposing conditions, the source of his infection is unclear. We accompany this report with a review of C. kerstersii bacteremia cases found in the literature.

Abstract:Comamonas kerstersii (C. kerstersii) is a Gram-negative bacterium that was initially thought to be non-pathogenic to humans and is abundant in the environment. In recent years, with the availability of matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) that enable fast and accurate bacterial identification, there have been increasing number of reports of human infections caused by C. kerstersii, indicating that this organism has emerged as human pathogen. In fact, most clinical isolates of C. kerstersii are recovered from peritoneal liquid, and bacteremia has been infrequently reported. Here, we report a case of bacteremia caused by C. kerstersii in a 28-year-old male patient with acute perforated appendicitis and localized peritonitis and present a comprehensive review of C. kerstersii infections in pathogenic diagnosis and clinical treatment as well as prognosis, thus providing a better understanding of C. kerstersii-related infections.

Abstract:Comamonas spp. are gram-negative rods that can be found throughout the environment, including in water, soil, plants, and animals. [ 1–3 ] Although generally thought to have low virulence, a few Comamonas species (especially C testosterone, C kerstersii ) can still cause opportunistic infections in immunocompetent hosts. [ 2 , 4 ] C kerstersii was first identified in 2003, [ 4 ] and since has been predominantly associated with intra-abdominal infections, including appendicitis, peritonitis, diverticulosis. [ 2 ] 27 C kerstersii strains have also been cultured from human fecal samples, suggesting a possible link with the gastrointestinal tract. [ 5 ] The majority of C kerstersii infections appear to be polymicrobial, with 1 case of urinary tract infection being an exception. [ 6 ] C kerstersii isolates have shown high susceptibility to antibiotics. [ 2 , 7 , 8 ] Consequently, the management of C kerstersii infections with appropriate antimicrobial therapy often results in favorable clinical outcomes, even in cases of mixed infections [ 1 , 5 , 8 ]

Abstract:Rationale: Comamonas species are rarely associated with human infections. Recent reports found that Comamonas kerstersii was associated with severe diseases such as abdominal infection and bacteremia. However, C. kerstersii maybe be confused with Comamonas testosteroni using the automatic bacterial identification systems currently available. Patient concerns: A 31-year-old man who had onset of left upper abdominal pain developed clinical manifestations of right lower abdominal pain and classic migration of pain at the temperature of 39°C. The positive strain of aerobic and anaerobic bottles of blood cultures was identified. Diagnoses: The patient was diagnosed as acute peritonitis and perforated appendix with abdominal abscess. Interventions: The bacterium was identified by routine methods, MALDI-TOF-MS and PCR amplification of the 16S rRNA. The patient was treated with exploratory laparotomy, appendectomy, tube drainage, and prescribing antibiotic treatment. Outcomes: The patients were discharged with complete recovery. The organisms were confirmed as C. kerstersii by MALDI-TOF-MS and a combination of the other results. Lessons: Our findings suggest that C. kerstersii infection occurs most often in association with perforated appendix and bacteremia. We presume that C. kerstersii is an opportunistic pathogen or commensal with the digestive tract and appendix bacteria.

Abstract:Comamonas kerstersii is a non-fermenting Gram-negative bacillus. It has been associated with intra-abdominal infections. We describe a clinical case of bacteremia caused by C. kerstersii in a young man with acute appendicitis. The first identification, using the Vitek2 compact system (BioMerieux) from blood cultures, was Comamonas testosteroni. However, using MALDI-TOF was identified as C. kerstersii. The patient was treated first with ampicillin sulbactam and then piperacillin tazobactam with favorable evolution. Within the genus Comamonas, C. testosteroni has been the most frequently reported species as a cause of infections in humans, only 23 cases of C. kerstersii being described. Given the problems of phenotypic identification of these microorganisms, it is possible that some C. testosteroni reports could have corresponded to C. kerstersii. This case highlights the pathogenic role of C. kerstersii and the importance of using MALDI-TOF as a diagnostic tool for non-fermenting Gram-negative bacilli identification.

Abstract:Background: Comamonas kerstersii is rarely associated with infections in humans and has never been reported in animals until now. Case presentation: Herein, we describe a case of urinary tract infection caused by C. kerstersii in a young goat. A seven-month-old male goat showed lethargy, generalised weakness and anorexia and in the last hours before its death, severe depression, slight abdominal distention, ruminal stasis, and sternal recumbency. Grossly, multifocal haemorrhages in different organs and tissues, subcutaneous oedema and hydrocele, serous fluid with scattered fibrin deposition on the serosa of the abdominal organs and severe pyelonephritis with multifocal renal infarction were detected. Histopathological examination confirmed severe chronic active pyelonephritis with renal infarcts, multi-organ vasculitis and thrombosis suggestive of an infectious diseases of bacterial origin. The bacterium was identified using routine methods, matrix assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF-MS), and sequencing of the gyrB gene. Conclusions: To the best of our knowledge, this is the first report of C. kerstersii infection in animals (goat). Our findings support the possibility of C. kerstersii isolation from extraintestinal sites and suggest this organism as a possible cause of urinary tract infection.

Abstract:Background: Comamonas kerstersii (C. kerstersii) infections have considered as non-pathogenic to humans, however due to new techniques such as matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF-MS), more cases have been identified. Case summary: This is the first report of a maternal patient with a C. kerstersii bacteremia following caesarean section. Due to the severity of the patient's condition; high fever and rapidly progressing organ damage, the patient was transferred to the intensive care unit. C. kerstersii was detected by metagenomic next-generation sequencing testing. Based on the drug sensitivity test, appropriate antibiotic treatment was given and the patient recovered fully. Conclusion: This case report confirms that the detection via MALDI-TOF-MS and metagenomic next-generation sequencing testing provides a reliable basis for the diagnosis of this rare bacterial infection.

Abstract:We report the first identification in Ecuador of the microorganism Comamonas kerstersii, a strain isolates from a sample of fluid from the abdominal cavity and identified by the technique of mass spectrometry MALDITOF. It was obtained during the surgical act of a patient with the diagnosis of acute peritonitis, secondary to a perforated appendicitis. This microorganism is considered a rare pathogen. Even though the genus to which it belongs has a wide geographical distribution, this pathogen is considered an opportunistic agent.

Abstract:Objective: To report a series of four uncommon cases of bacteremia due to Parabacteroides distasonis. Methods: We have analyzed the epidemiological characteristics, clinical features, diagnostic methods, treatment and outcome of these patients. Results: Two sets of blood cultures of each patient yielded a pure culture of an anaerobic microorganism identified as P. distasonis by MALDI-TOF MS, and confirmed by 16S rRNA gene sequencing. All patients were male and they had risk factors for anaerobic bacteremia. All isolates were susceptible to metronidazole and the outcome was successful in three patients. Conclusions: Bloodstream infections due to P. distasonis are still rare. MALDI-TOF MS appear to be an excellent tool for the correct identification.

Abstract:We present the first described case of bacteremia due to Butyricimonas paravirosa, a commensal gram-negative anaerobic bacterium identified by whole-genome sequencing in an elderly patient with acute terminal ileitis, who was successfully treated with ceftriaxone and metronidazole. We reviewed eleven previous cases of infection due to other Butyricimonas spp, which can cause a range of diseases but may be treated conservatively with a short antimicrobial course in the appropriate clinical setting. Additionally, while most Butyricimonas spp are susceptible to empiric anaerobic therapy, drug resistance has been reported in some cases.

Abstract:Introduction: Clostridium difficile infection always manifests as diarrhea associated with antibiotic use. Nevertheless, extraintestinal infections caused by C. difficile are reported. The current report was on a case of C. difficile bacteremia in a 51-year-old male patient with appendiceal perforation and abscess as well as confirmed cirrhosis. Case Presentation: The patient was admitted to the infectious diseases department with repeated abdominal pain associated with fever. His anaerobic blood culture was positive for C. difficile two days after transarterial chemoembolization. The same toxin strain was isolated from stool four days later. Both of the two isolates were confirmed positive for toxin A (tcd A) and toxin B (tcd B) genes by polymerase chain reaction, and identified as ST3 by multiple locus sequence typing. The two strains showed the same susceptibility to the tested antibiotics. The patient was treated with vancomycin intravenously, and got remitted shortly after that. The patient was discharged with good general health conditions and followed up. Conclusions: A patient with C. difficile bacteremia presenting with appendiceal perforation and abscess as well as confirmed cirrhosis was described, and the microbiological and molecular biological analysis suggested that C. difficile strains isolated from blood came from gut. It is necessary that clinicians detect C. difficile and/or toxins in patients with long terms antibiotic therapy, in case of transformation of intestinal flora, which could cause the infection outside the intestinal tract.

Abstract:The clinical characteristics of bacteremia remain insufficiently described. Therefore, here, we retrospectively analyzed 13 cases of bacteremia at a tertiary care center to evaluate their clinical and microbiological characteristics. Ten patients were aged ≥ 65 years, and seven were female. Nine patients had comorbidities, including seven with solid tumors or hematological malignancies. Eleven patients presented with gastrointestinal symptoms. The isolates included four , four , three , two , and one isolates. Ten available strains exhibited low minimum inhibitory concentrations against β-lactam/β-lactamase inhibitors and metronidazole, whereas they exhibited high minimum inhibitory concentrations for penicillin, ceftriaxone, and minocycline. Several strains harbored antimicrobial resistance genes, including , (Q), , , and (G). Twelve patients received β-lactam/β-lactamase inhibitors, and two elderly patients with solid tumors developed septic shock and died. species can translocate from the gastrointestinal tract into the bloodstream, leading to severe infections.

Abstract:Here we report a case of bacteremia caused by Clostridium paraputrificum in a 64-year-old woman with colon carcinoma and gastrointestinal disease. Using the new EUCAST 2022 clinical breakpoints for Clostridium perfringens, the isolate was susceptible to metronidazole and vancomycin, but resistant to benzylpenicillin, meropenem, and clindamycin. Thus, treatment with metronidazole should be considered in all patients with Clostridium bacteremia until antibiotic susceptibility is determined to minimize the risk of treatment failure.

Abstract:Because of poor differentiation among the members of genus Comamonas using phenotypic methods, human infections caused by C. kerstersii are sporadically reported in the literature. Here, we represent the first complete genome sequence of C. kerstersii 8943, which caused peritonitis in a patient with continuous ambulatory peritoneal dialysis (CAPD). The complete genome with no gaps was obtained using third-generation Pacific Biosciences (PacBio) RSII sequencing system with single-molecule real-time (SMRT) analysis. Protein-coding genes, rRNAs and tRNAs were predicted. Functional annotations of the genome using different databases revealed several genes related to pathogenicity including antibiotic resistance genes and prophages. Our work demonstrates that whole genome sequencing can enhance the resolution of clinical investigations and our data can be used as a reference genome during the rapid diagnosis of C. kerstersii infections in the future.

We report a bacteremia with T. sanguinis and Desulfovibrio desulfuricans in a patient with ulcerative colitis. T. sanguinis grew in thioglycolate media and identification was confirmed with 16S rRNA sequencing.

Abstract:We herein present the first known case of bacteremia caused by Kluyvera georgiana in a 67-year-old female undergoing chemotherapy for recurrent pancreatic cancer. The patient underwent choledochojejunotomy and thereafter developed ascending cholangitis. The diagnosis of K. georgiana was confirmed using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. A literature review of Kluyvera spp. infections indicated potential risk factors including an underlying malignancy and immunosuppression. Although Kluyvera spp. infections are typically sensitive to antibiotics, multidrug resistance is possible. This case highlights the importance of the early diagnosis and treatment of K. georgiana and its associated risk factors.
