Abstract:Background context: Numerous cases of injury to major abdominal vessels during the excision of a lumbar herniated disc have been reported, but no cases of injury during interbody fusion by a posterior approach have been described. Purpose: To report on an injury to common iliac vessels during a posterior lumbar interbody fusion (PLIF) and discuss the causes and possible preventive measures. Study design: A unique case report and a review of the literature. Methods: The hospital chart and autopsy report of a single patient were analyzed. Results: A 52-year-old woman with L4-L5 disc degeneration underwent PLIF. During scraping of the vertebral end plates, there was a sudden increase in blood flow from the disc space, however not copious, with no changes of vital parameters. When the patient was placed supine, severe hypotension and abdominal distension led to strongly suspect a lesion to abdominal vessels. At laparotomy, carried out by a vascular surgeon, a vast retroperitoneal hematoma was evacuated and the vascular lesions were repaired. Postoperatively, the patient continued to lose blood from the abdominal drains and after 4 hours, she was reoperated by another vascular surgeon, who found a diffuse hemorrhage from the small vessels in the surgical field. Soon after the surgery the patient died. Conclusions: The lesions were produced by a shaver used for scraping the vertebral end plates. The absence of abundant bleeding from the disc space was possibly because of the compression of the iliac vessels by the pads of the frame on which the patient was lying. The causes of the lesions and possible prevention of similar injuries are analyzed.
[Analysis of dynamic changes in glycosylated blood proteins using a computer model].

Abstract:UNLABELLED The authors elaborated a computer model of albumin glycosylation based on the irreversible glycosylation reaction with first order kinetics. The dynamics of changes of glycosylated albumin in relation to the glycaemic profile was confirmed with an older model of haemoglobin glycosylation. By means of regression analysis parameters of the model in three groups of patients were calculated. CONCLUSION 1. Stratification of the red cell pool is the reason why there is a smaller clinical difference between glycosylated protein and haemoglobin than corresponds to their half-times. 2. Glycosylated proteins are probably eliminated more rapidly than non-glycosylated ones. 3. Higher levels of glycosylated proteins sometimes do not correspond to model calculations are probably due to other factors.
Hemorrhage due to a pseudoaneurysm on a dural-pial anastomosis after decompression for Chiari malformation type I: case report

Abstract:While intracranial aneurysms rarely develop after neurosurgical procedures, delayed pseudoaneurysm formation after foramen magnum decompression (FMD) has never been reported. A 52-year-old woman presented with an atypical subarachnoid hemorrhage in the posterior fossa 12 years after a FMD for symptomatic Chiari malformation type I was performed. A pseudoaneurysm on a dural-pial anastomosis was identified as the bleeding source and successfully occluded by endovascular means with full clinical recovery of the patient. Injury to the distal posterior inferior cerebellar artery related to surgery and postoperative infection likely caused formation of a dural-pial anastomosis. Additionally, hemodynamic stress or dissection may have contributed to delayed pseudoaneurysm formation and rupture.

Abstract:Background and Objective: Massive retroperitoneal hematoma caused by lumbar artery rupture is generally associated with trauma or retroperitoneal malignancy. However, despite recent advances in technologies and tools, spontaneous lumbar artery rupture is a very rare disease entity but remains a challenging problem because it is frequently associated with significantly high mortality and morbidity and is very difficult to make a correct diagnosis. Methods: We evaluated the databases of the PubMed, Embase, Cochrane Central Register of Controlled Trial, Google Scholar, the KoreaMed and the Research Information Sharing Service databases, and a detailed systematic review was performed by searching in PubMed. The initial search was performed on 3 February 2018 and a second search conducted in 29 January 2019. Results: A total of 10 case reports on massive hemoperitoneum caused by spontaneous lumbar artery rupture were identified. Of the 10 case reports involving 14 patients, eight were male and six were female under 62.71 ± 13.93. Of the 14 patients, 9 (64.3%) surviving with transcatheter arterial embolization, three (21.4%) died of multi-organ failure or hypovolemia, and two (14.3%) had no definite records on survival or death. Conclusions: A massive retroperitoneal hematoma caused by lumbar artery rupture should be considered in patients with late-onset shock accompanied by blunt abdominal/pelvic trauma. Furthermore, early detection and urgent embolization would prevent further complications and eliminate the need for surgical interventions. Ruptured femoral pseudoaneurysm presenting as a lateral abdominal wall hematoma

Abstract:Lateral abdominal wall hematomas are rare. We describe a patient with a delayed rupture of a femoral artery pseudoaneurysm, who presented with such a hematoma. In contrast to other types of abdominal wall hematomas, which are often managed conservatively, a ruptured femoral artery pseudoaneurysm frequently requires emergent surgical intervention. Rupture of a pseudoaneurysm can be catastrophic. Due to the rising incidence of femoral artery pseudoaneurysms and shorter hospital stays, it is useful for the emergency physician to be familiar with the diagnosis and management of femoral artery pseudoaneurysms and their potentially life-threatening complications.

Abstract:Study Design. Case report. Objective. To describe a unique case of iliac pseudoaneurysm-complicated lumbar disc surgery performed 13 years ago. Summary of Background Data. Major vascular injury is an unusual but life-threatening complication of lumbar disc surgery. If a major vascular injury is unnoticed, the late complications, pseudoaneurysm, may be diagnosed years after primary lumbar disc surgery. Pseudoaneurysm can lead to compression of adjacent neurovascular structures; however, erosion into the adjacent vertebras and presentation with symptoms of nerve root irritation are rare. Methods. The patient was a 54-year-old man who presented with pain and numbness in right hip and leg. Examination was normal. A plain radiograph showed destruction of the fourth and fifth lumbar vertebras. Magnetic resonance image showed a retroperitoneal mass eroding into the adjacent vertebras. The mass was primarily misdiagnosed as a neoplasm and a percutaneous needle biopsy was recommended. Before puncture, the computed tomographic angiogram confirmed it to be a pseudoaneurysm of left common iliac artery. Then, implantation of a covered stent was undergone. Results. After implantation of a covered stent, the symptoms were relieved immediately. At the 12-month follow-up, the patient remained asymptomatic, and a computed tomographic angiogram revealed progressive shrinkage of the mass and significant healing of the eroded vertebras. Conclusion. As an unusual complication of lumbar disc surgery, iliac pseudoaneurysm can present with radiating pain even after 13 years and erode into the adjacent vertebras producing appearances mimicking a tumor. For mass located anterior to vertebras, the percutaneous biopsy must be cautiously performed, and a contrast-enhanced computed tomographic scan is necessary.

Abstract:The present study reported the case of a patient with degenerative lumbar scoliosis who suffered from post?operative hemorrhage due to lumbar artery injury in corrective osteotomy. The patient presented with decline of blood pressure, extremely low hemoglobin, even after blood transfusion and fluid infusion. Digital subtraction angiography showed lumbar ar?tery injury. Successful control of bleeding and gradually stable vital signs were observed after vascular embolisation. Severe and complex spinal deformity, severe osteophytes in the anterior edge of vertebral body, hypertension, and angiosclerosis were the risk factors of large vascular injuries in spinal osteotomy. Lumbar artery injury should be highly suspected when unknown and progressively aggravating abdominal or back pain, abdominal distention, neurological symptoms of lower limbs, decline of blood pressure, increase of heart rate, decrease of hemoglobin level, continuously bloody drainage or incision bleeding oc?curred. Vascular embolisation was effective and safe in the management of iatrogenic lumbar artery injury.
Isolated vertebral bone infarction following lumbar artery embolization- a case report

Abstract:Purpose: To report a rare case of isolated lumbar vertebral body infarction following lumbar artery embolization for suspected retroperitoneal haemorrhage. Methods: We present the case of a 75-year-old male who underwent lumbar artery embolization due to a suspected retroperitoneal haemorrhage post-surgery. Magnetic resonance imaging (MRI) was performed to monitor post-embolization severe lumbar pain. Results: Initial MRI two days post-embolization showed no osseous signal changes or epidural abscess. A subsequent MRI 26 days post-embolization was performed because of increasing lumbar back pain and it revealed a new bone infarction at the L3 vertebral body. CT showed embolization deposits within the vertebral body, not present before the procedure. Conclusion: This unique case underscores the importance of considering vertebral body infarction as a potential complication following lumbar artery embolization. MRI was critical in early detection of the bone infarction, while CT confirmed the presence of embolization material. Awareness of this rare complication is crucial for prompt diagnosis and management.
Delayed Epidural Pseudoaneurysm Following Cervical Laminectomy and Instrumentation in a Patient with Canal Stenosis Secondary to Skeletal Fluorosis

Abstract:RATIONALE:The typical intraoperative presentation of vertebral artery injury (VAI) usually involves profuse bleeding and requires immediate treatment. However, an occult VAI may occur intraoperatively and result in delayed life-threatening epidural pseudoaneurysm several days postoperatively.PATIENT CONCERNS:A 21-year-old man with compressive cervical myelopathy resulting from canal stenosis of skeletal fluorosis underwent decompression of C1 to C7 and instrumentation from C2 to C7. No impressive bleeding event occurred during the operation. On postoperative day 40, progressive quadriplegia developed.DIAGNOSES:Pseudoaneurysm of the VA was established by angiography.INTERVENTIONS:After occlusion of the right VA, the patient underwent hematoma clearing.OUTCOMES:Fortunately, the patient experienced significant recovery of neurologic function after the second surgery.LESSONS:From this case, we realize even in the absence of obvious signs of VAI during a cervical operation, postoperative evaluation should be mandatory for suspected bleeding events occurring at VAI-prone sites during surgery. Moreover, the bone morphological abnormality of skeletal fluorosis was determined to be the most important risk contributing to VAI in this case. The safety limits of bone removal should be determined preoperatively to avoid the effects of bone morphological abnormalities.
A Case of Profunda Femoris Artery Pseudoaneurysm Treated With Catheterization and Surgical Hematoma Removal

Abstract:A pseudoaneurysm of the profunda femoris artery (PFA) is a rare disease induced by femoral trauma, proximal femoral fracture, or iatrogenic causes associated with orthopaedic surgery. Recently, transcatheter arterial embolism was reported as an effective treatment for profunda femoris artery pseudoaneurysm. This report presents the case of an 85-year-old male who underwent artificial head replacement for a left femoral neck fracture and was hospitalized with a peri-stem fracture four years later. Conservative treatment was conducted with a brace, though the swelling and pain in his left thigh increased one week after his hospital admission. Contrast-enhanced computed tomography (CT) led to a diagnosis of a left profunda femoris artery pseudoaneurysm, and the patient underwent emergency transcatheter arterial embolism and surgical hematoma removal. Since the emergency surgery, the patient's course has been good, and he has been undergoing rehabilitation.
Anterior Decompression and Reconstruction for Lumbar Burst Fractures

Abstract:Burst fracture which tends to occur between T9 and L5 level is usually caused by a high-energy axial load such as motor vehicle accidents and falls, in which the margins of the vertebral body spread out in all directions making the spine much less stable than a compression fracture. The bone chips which spread out toward the spinal cord could bruise the spinal cord causing neurologic injury. There are more than 25,000 thoracolumbar burst fracture cases every year in the United States. Typical lumbar burst fracture can lead paralysis of the legs and loss of control of the bowel and bladder. In a mild burst fractures only partial paralysis or transient neurologic symptoms may be present. Surgical treatment is required when the burst fracture is unstable. The aim of the surgery is to reach adequate decompression and stabilization. The most popular technique is posterior approach, but there are many complications, including instrumentation failure, pseudarthrosis and so forth1-4. Adequate anterior column support is necessary to prevent instrumentation failure5, 6. An anterior approach is effective for lumbar burst fractures with neural injury because it allows direct decompression and reconstruction7, 8. Anterior technique has been shown to have better reconstruction, better restoration, fewer complications and less operating time9-12. What's more, anterior approach is strongly considered in the setting of unstable burst fracture at L2-L5 level because of the importance of biomechanical integrity and sagittal profile here11, 13. However, the extensive operative trauma and high technical requirements of anterior surgery limit its wide application. From the year 2000, we investigated a series of improvements in this procedure. A 32-year-old man fell from a height of 4 m 3 h prior to presenting with pain in his low back, difficult in urination and defecation and limitations in lower extremity activity. Careful physical examination showed tenderness and sensitivity to percussion at the L3 spinous process. Decreased sensation was found over both lower extremities. The knee jerk and Achilles tendon reflexes were decreased and muscle strength in the two extremities (knee extension, ankle dorsiflexion and plantar flexion) was grade 3+. Routine laboratory investigations were normal. X-ray films revealed a burst fracture of L3. CT and MR scans showed that more than 50% of the vertebral canal was occupied by bone chips at the L3 level. Therefore, the preoperative diagnosis was L3 burst fracture (AO type A3) with cauda equina syndrome and incomplete paralysis of the lower extremities. Anterior corpectomy and reconstruction with an expandable artificial vertebral body (Synthes, Solothurn, Switzerland) was planned. After induction of general anesthesia, the patient was positioned in a right lateral decubitus position. A transverse incision was made at the L3 level between the left anterior and posterior axillary lines and the external oblique, internal oblique and transverse abdominal muscles bluntly separated. Next, the prevertebral fascia was exposed through the retroperitoneal space. After the L3 level had been confirmed fluoroscopically, a Synframe Retractor System (Synthes)14 was installed and four Hoffman retractors placed on the proximal and distal vertebrae to create a stable surgical field15. Before performing corpectomy, the L3 segmental vertebral vessel was carefully ligatured and electrocoagulated. A routine corpectomy was then carried out and the spinal canal carefully decompressed. After both ends of the vertebral endplates had been scraped appropriately, an appropriately sized expandable artificial vertebral body with autogenous bone was inserted into the intervertebral space. The cage was then unfolded to achieve reduction. After the position of the cage had been confirmed fluoroscopically, the cage was fixed with a VentroFix System (Synthes). The operation time was 90 min with an estimated blood loss of 300 mL. The incision length was 6.5 cm and the duration of hospitalization 7 days. Compared with traditional anterior surgery16, only one rather than three levels of segmental vessels are severed in this improved procedure. Two Cobb elevators are used to press the segmental artery before cutting it, after which the two vessel ends are sufficiently dissociated before ligaturing (proximal part) or electrocoagulating (distal part) them. The integrity of the psoas is preserved thanks to the use of Hoffman retractors. The external oblique, internal oblique and transverse abdominal muscles are bluntly separated rather than being severed, thus the improved procedure is less invasive. The Synframe Retractor System14 offers a more stable operative field with a smaller skin incision and thus decreases the risks of vessel and organ injury during the procedure. This technique makes decompression of the spinal canal more direct and complete17. The expandable artificial vertebral body18 makes the reconstruction more effective and offers much stronger and more stable support19, which likely results in higher fusion rates and reduced rate of instrument failure compared with the traditional technique. In our experience, a satisfactory clinical outcome can be achieved by this technique. Additional video images may be found in the online version of this article. Iliac Artery Injury During Lumbar Disc Hernia Surgery

Abstract:Background: Vascular injury complications during lumbar discectomy are rare but potentially life-threatening. Therefore, an early diagnosis and effective treatment management is required for these complications. Case description: A 50-year-old female patient was admitted to our outpatient clinic with severe back and right leg pain. She underwent surgery for right L4-5 extruded disc herniation with general anesthesia. Sudden arterial hemorrhage occurred during discectomy performed with straight disc forceps and was controlled using hemostatic materials, with no significant decrease in blood pressure. However, the patient became hypotensive near the end of the operation. The incision was quickly closed, and she was turned to supine position. Emergency abdominal ultrasound, computed tomography, and computed tomography angiography revealed an injury of the left main iliac artery, which was repaired by endovascular stenting. Laparotomy and Bogota bag were applied because of increased intrabdominal pressure at 3 hours postoperative. In addition, a retroperitoneal catheter was placed into the area of the right retroperitoneal hematoma on the first postoperative day. Tissue plasminogen activator was administered through the catheter. On postoperative day 3, the Bogota bag was removed, and the abdomen was closed. The patient was discharged without neurodeficit on day 27. Her abdominal fascial defect was closed with a synthetic graft after 5 months. Conclusions: Although lumbar discectomy is one of the most commonly performed neurosurgical procedures, the routine rules of discectomy should not be neglected. Early detection and a multidisciplinary approach can help prevent mortality in the event of vascular injury.

Surgical Treatment of Post-Traumatic Pseudoaneurysms and Arteriovenous Fistulas

Abstract:OBJECTIVE: To report the experience in the diagn osis and treatment of post-traumatic pseudoaneurysms and arteriovenous fistulas. METHODS: A series of 30 patients (11 women and 19 men) with pos ttraumatic pseudoaneurysms were reviewed retrospectively. Among them 7 patients (5 women and 2 men) were associated with arteriovenous fistula. RESULTS: The causes included sharp penetrating trauma (18 cases ), blunt trauma (6 cases) and iatrogenic arterial injury (6 cases). The main cli nical manifestations consisted of local pulsatile mass (26 cases), vascular brui ts (19 cases), thrill (13 cases), ischemia of distal limb (9 cases), neuropathy (5 cases) and pseudoaneurysm rupture (2 cases). All patients underwent surgery. The operations included: ligation of the vessels (12 cases), surgical resection and primary suture repair of the vascular defect or anastomosis (11 cases), vasc ular reconstruction with autogenous saphenous vein (3 cases) and synthetic vascu lar graft (4 cases). CONCLUSIONS: Because of the imminent clinical course, early op eration is usually indicated. The operative treatment is effective and safe for most of the patients with post-traumatic pseudoaneurysms and arteriovenous fist ulas.
Splenic rupture following prone lateral discectomy and arthrodesis: illustrative case

Abstract:Background: The prone lateral approach to lumbar spine surgery is known to have a multitude of potential complications, including damage to neurovascular structures, surrounding viscera, and intra-abdominal structures near the surgical site. However, iatrogenic injury to the spleen following prone lateral lumbar discectomy and arthrodesis as a potential complication has not yet been described in the literature. Observations: The authors present the case of a 71-year-old female with a history of L3-S1 laminectomy and L3-5 arthrodesis who underwent a prone lateral discectomy of L2-3 with arthrodesis of the endplates for chronic lower-back pain. On postoperative day 1, the patient developed hypotension unresponsive to pressor medications, significant abdominal pain, and anemia requiring 2 transfusions. Bedside ultrasound revealed free fluid in the abdomen. She then underwent an exploratory laparotomy for splenic injury. Lessons: Although rare, splenic rupture should be considered as part of the differential diagnosis for patients with hemodynamic instability after lateral surgical approaches to the lumbar spine. Any patient with evidence of hypotension, anemia, and/or abdominal pain following lumbar surgery should be evaluated for splenic injury with an abdominal computed tomography scan and considered for surgical intervention. https://thejns.org/doi/10.3171/CASE23639.

Abstract:Objective To present 3 cases of retroperitoneal hematoma ( RPH ) following transforaminal percutaneous endoscopic lumbar discectomy and to discuss how to prevent the complication of unintended hemorrhage by reviewing literatures. Methods The clinical data of 3 patiets admitted in 2 hospitals from March 2016 to January 2018 were retrospectively studied. The literatures about the RPH following transforaminal percutaneous endoscopic lumbar discectomy before July 2018 were searched in CNKI, Weipu, Wanfang and PubMed databases by using "percutaneous lumbar endoscopic discectomy" and"retroperitoneal hematoma" as key words. Seven cases reported in English and one case reported in Chinese were detected. The clinical and imaging features, prevention and treatment of the complication were observed and analyzed. Results All 3 cases were undertaken transforaminal percutaneous endoscopic lumbar discectomy under local anesthesia. The patient with hypovolemic shock was treated by lumbar vertebral segment arterial embolization. The case with progressive weakness of her right leg underwent open hematoma evacuation via posterior lumbar approach. The last case was treated conservatively. In 6-24 months follow-up, 2 cases recovered without any neurological or vascular sequelae, but 1 case presented residual neurological deficits. Eleven cases of RPH after percutaneous transforaminal lumbar discectomy were involved, including 3 cases in this paper and 8 cases reported in references. Most of them presented with pain and discomfort in the groin area after a short period of asymptomatic operation. Two cases of hypovolemic shock were treated with lumbar vertebral segment arterial embolization, two cases with diffuse hematoma (hematoma volume ≥500 mL) were treated with laparotomy hematoma removal, one case with progressive weakness of right leg underwent open hematoma evacuation via posterior lumbar approach, and the other 6 cases were treated conservatively. With proper treatments, overall prognosis was good. Conclusions RPH is an infrequent but serious complication of transforaminal percutaneous endoscopic lumbar discectomy. Adequate technical and anatomical considerations are important to avoid this unusual hemorrhagic complication. A high index of suspicion and early detection are also important to avoid the progression of the hematoma.
Bilateral dual iliac screw pelvic fixation for adult spinal deformity: a case report of a superior gluteal artery pseudoaneurysm secondary to aberrant iliac screw trajectory

Abstract:Purpose: To present a case of a pseudoaneurysm of a branch of the left superior gluteal artery (SGA) secondary to lateral wall perforation from an iliac screw and its subsequent evaluation and management. Methods: Case report. Results: A 67-year-old female with a history of degenerative flatback and scoliosis and pathological fractures of T12 and L1 secondary to osteodisciitis underwent a single0stage L5-S1 ALIF and T9-pelvis posterior instrumented fusion with bilateral dual iliac screw fixation, revision T11-S1 decompression, and T12 and L1 irrigation and debridement and partial corpectomies. During the operation, non-pulsatile bleeding was encountered after creating an initial trajectory for the more proximal of the two left iliac screws. While the initial post-operative course was benign, the patient was readmitted for hypotension and anemia. Computed tomography of the abdomen/pelvis demonstrated a pseudoaneurysm (2.3 cm × 2.1 cm × 2.3 cm) of a branch of the left SGA. Diagnostic angiogram confirmed a pseudoaneurysm off of one of the branches of the left SGA. Endovascular embolization using multiple coils resulted in a complete cessation of blood flow in the pseudoaneurysm. At 2 years follow-up, no symptoms suggestive of recurrence of the pseudoaneurysm were reported. Conclusions: A pseudoaneurysm of a branch of the left superior gluteal artery as a result of lateral wall perforation from an aberrantly placed iliac screw during an adult spinal deformity operation involving dual screw pelvic fixation is reported. Prompt recognition, multidisciplinary collaboration, and appropriate intervention were key in achieving a successful outcome and preventing further morbidity.
Delayed postpartum haemorrhage secondary to a ruptured uterine artery pseudo-aneurysm, successfully treated by transarterial embolisation

Abstract:A 29-year-old woman (gravida 1, para 1) had an uneventful first pregnancy and a delivery by emergency caesarean section at term. The caesarean section was complicated by a massive obstetric haemorrhage of 5000 ml. After closure, an immediate re-laparotomy was indicated due to heavy vaginal bleeding. The site of bleeding was identified as an extension of the uterine incision, and was sutured. She was stabilised by transfusion of blood and blood products in the intensive therapy unit, and discharged 5 days later. The patient was re-admitted 6 weeks later with brisk, painless vaginal bleeding, passing large clots from a well-contracted uterus. Her haemoglobin decreased from 11.8 to 7.8 g/dl overnight. In view of her history, an urgent CT angiogram was performed, which revealed the presence of a pseudo-aneurysm arising from the left uterine artery. This was successfully occluded by transarterial embolisation, obviating the need for further surgical exploration.

Abstract:beta-Glucosidase (beta-D-glucoside glucohydrolase, EC 3.2.1.21) splitting p-nitrophenyl-beta-D-glucosidase was isolated from cellular preparation of the fungus Geotrichum candidum 3c "cellocandine G10x" and purified 38-fold. The enzyme was homogeneous during ultracentrifugation, gel-filtration, isoelectrofocusing and disc-electrophoresis in polyacrylamide gel and had a pI of 4.2, sedimentation coefficient of 2.6S and molecular weight of 120 000. The enzyme had maximal activity at pH 5.6, 45 degrees and retained up to 45% of its activity under optimal conditions (pH, t degrees) after 48 hr incubation. beta-Glucosidase did not split disaccharides, e. g. lactose cellobiose, laminaribiose, gentibiose as well as o-nitrophenyl-beta-D-galactoside, methyl-beta-D-xyloside. The enzyme hydrolyzed p-nitrophenyl-beta-D-glucopyranoside (Km = 0.18 mM) and some plant beta-glucosides, such as phenylethyl-beta-D-glucopyranoside from rose petals and diosgenine tetrasaccharide from deltoid dioscorea (Km = 0.26 mM).
Delayed Rupture from a Pseudoaneurysm after Mechanical Thrombectomy: A Case Report

Abstract:Pseudoaneurysm following mechanical thrombectomy (MT) is a rare but possible complication associated with endovascular procedures. This report presents a case of delayed rupture of a pseudoaneurysm after MT with a stent retriever, which was confirmed by open surgery. During hospitalization, an 85-year-old woman had right hemiplegia and aphasia. Magnetic resonance imaging and angiography revealed acute ischemic changes in the left middle cerebral artery because of M2 segment occlusion. MT was performed to address persistent M2 occlusion. Retrieving from distal vessels with the fully deployed Solitaire 4 × 20 mm stent retriever was considered dangerous, we resheathed the stent, but the microcatheter jumped distally. Angiography through microcatheter revealed contrast leakage into the subarachnoid space. The diagnosis was vessel perforation caused by the microcatheter. The lesion was treated with temporary balloon occlusion for 5 minutes using a balloon-guiding catheter, combined with the reversal of heparin anticoagulation by protamine, and a systolic blood pressure reduction to below 120 mm Hg. Anticoagulation was initiated after confirming that postprocedural subarachnoid hemorrhage (SAH) decreased 1 day after the procedure. Fourteen days after the procedure, computed tomography and angiography revealed a massive hematoma with a newly formed small pseudoaneurysm at the site of vessel rupture. Open surgery was performed to close the small artery rupture using a clip. Delayed rupture of the pseudoaneurysm occurred after MT using a stent retriever. If SAH is observed after MT, performing follow-up computed tomography angiography or magnetic resonance angiography is recommended to consider pseudoaneurysm formation.
