Music Listening Among Postoperative Patients in the Intensive Care Unit: A Randomized Controlled Trial with Mixed-Methods Analysis

Abstract:Background: Music listening may reduce the physiological, emotional, and mental effects of distress and anxiety. It is unclear whether music listening may reduce the amount of opioids used for pain management in critical care, postoperative patients or whether music may improve patient experience in the intensive care unit (ICU). Methods: A total of 41 surgical patients were randomized to either music listening or controlled non-music listening groups on ICU admission. Approximately 50-minute music listening interventions were offered 4 times per day (every 4-6 hours) during the 48 hours of patients' ICU stays. Pain, distress, and anxiety scores were measured immediately before and after music listening or controlled resting periods. Total opioid intake was recorded every 24 hours and during each intervention. Results: There was no significant difference in pain, opioid intake, distress, or anxiety scores between the control and music listening groups during the first 4 time points of the study. However, a mixed modeling analysis examining the pre- and post-intervention scores at the first time point revealed a significant interaction in the Numeric Rating Scale (NRS) for pain between the music and the control groups (P = .037). The Numeric Rating Score decreased in the music group but remained stable in the control group. Following discharge from the ICU, the music group's interviews were analyzed for themes. Conclusions: Despite the limited sample size, this study identified music listening as an appropriate intervention that improved patients' post-intervention experience, according to patients' self-report. Future mixed methods studies are needed to examine both qualitative patient perspectives and methodology to improve music listening in critical care units.

Abstract:Background: Evidence for music's beneficial effect on physical and mental disorders is mounting. Intensive care unit (ICU) patients experience multiple uncomfortable symptoms, which may be alleviated using a music-based intervention. Few studies have examined the experience and the physical impact of patient-tailored live music offered by trained health musicians in an adult ICU. Aims: This study aimed to explore the experience of live music in adult patients admitted to an ICU, focusing on its effects on relaxation, stress, and pain. Study design: A pilot study with a convergent mixed-methods design. A total of 27 intensive care patients at a Danish University Hospital were offered patient-tailored live music by trained musicians in a single-session design. We performed participant observation and conducted patient interviews using an observational and semistructured interview guide. These data were supported by quantitative pre-post measurements of heart rate, respiration rate, mean arterial blood pressure, subjective pain experience, and heart-rate variability. The study was conducted from February 2020 to December 2021. Findings: Using a Ricoeur-inspired analysis of observations and interviews, we elicited four themes: 'A break where you can swim away and relax', 'The living presence makes the play unique', 'Happy memories of the past and longing for home', and 'An intense and meaningful experience'. The quantitative analysis showed a significant decrease in heart rate (4.33 bpm, p < 0.02), respiration rate (2.93, p < 0.001), and blood pressure (3.30, p < 0.05) and a significant increase in heart-rate variation (-0.22, p < 0.01). Seven patients reported pain reduction after the music intervention, corresponding to a 24% reduction. Conclusion: Live music contributes to meaningful moments by bringing elements of everyday life into the ICU and may promote relaxation and reduce stress and pain. Relevance to clinical practice: Our findings indicate that live music is a nonpharmacological experience that creates meaningful moments for critically ill patients.

Abstract:Postpartum is a condition in which the reproductive organs return to normal which takes 6 weeks. The feeling of discomfort in postpartum mothers occurs because of the sensation of pain that is often complained of. One way to increase comfort for postpartum mothers is by providing non-pharmacological therapies, namely massage and listening to music. Music can provide impulses or stimuli that can override the pain signals. The literature review aims to describe the type of music that can increase the comfort of postpartum mothers. The method used is a literature review study. The database used from several journals and articles with a search process through Pubmed, Proquest, Springlink, Scopus, Google Scholar. The criteria for inclusion in this search were the year the articles were published, starting from 2010 to 2020. The type of classical music that has a medical connection is Mozart's music. Mozart music with a frequency of 40-60 Hz, tempo 60-80 bpm. Music therapy is given for approximately 20-30 minutes to produce a therapeutic effect with a period of twice a day for 2 days using headphones. Music used to increase comfort and reduce the anxiety that has a slow tempo, notes that are not too high or too low, has a low volume and is rhythmic with a simple arrangement, and a stable melody. The provision of music therapy, especially Mozart adagio classical music, affects reducing anxiety levels and reducing pain intensity so that it can provide a sense of comfort to postpartum mothers.

Abstract:BACKGROUND: With the continuous advancement of medical technology, the survival rate of preterm infants is gradually improving, However, due to the underdeveloped function of various organs and systems, preterm infants are often exposed to light, noise, medical as well as nursing operations and other stimuli during their hospitalization in neonatal intensive care unit (NICU); it is highly susceptible to a number of problems, such as pain, unstable vital signs, growth retardation, and sleep disruption. OBJECTIVE: This article reviews the research progress of music intervention in nursing of premature infants in NICU with both traditional and conventional care. METHODS: This article reviews the research background, methodology/design, and measurement/application effects of music interventions, including Chinese and Western traditional music, in the care of preterm infants in NICU. All scholarly literature retrieved from MEDLINE\PubMed, Science Citation Index Expanded, Google scholar, CNKI scholar, Scopus and PubsHub. RESULTS: As a simple and effective non-pharmacological intervention, music intervention can effectively alleviate neonatal surgical pain, increase heart rate and oxygen saturation, promote sleep, growth and development, and improve neurological development. CONCLUSIONS: This review provide theoretical references for clinical practice.

Abstract:Aim To gain more insight into how nurses experience a participatory live music practice in relation to their ability to deliver compassionate care to medically hospitalised patients. Design Qualitative interpretive design. Methods Sixteen nurses participating in a live music practice with patients were interviewed using in‐depth interviews with open‐ended questions. Audio recordings were transcribed verbatim and subsequently coded. Theory‐driven inductive and deductive approaches were applied in thematic data analysis. Results We identified four themes: (1) Nurses' empathy and compassion; (2) The caring nurse–patient relationship; (3) Person‐centred approaches to care and (4) Nurses' subjective wellbeing. By observing patients' reactions to the music, nurses described that they obtained a deeper insight and understanding of patients' emotional wellbeing. These observations led to increased feelings of compassion in patient contact and stimulated informal communication between nurses and patients through a sense of shared humanity. According to nurses, these aspects positively affected collaboration with patients in delivering care and stimulated them to pursue person‐centred approaches to care. Participating in the live music practice also positively affected nurses' wellbeing, enhanced relaxation and created an ambiance in which compassion could be expressed. Conclusion A live music practice can positively contribute to the delivery of compassionate care by providing meaningful shared moments that increase feelings of empathy and compassion and strengthen the caring relationship. Implications for the profession Offering a live music practice at the ward and bedside offers a unique possibility to enhance engagement in person‐centred, compassionate care. Impact While compassion and compassionate care are essential component of nursing, nurses often experience multiple barriers to its provision in daily practice. An innovative way to stimulate compassionate care is through the participation of nurses and patients in a live music practice, providing a meaningful moment shared between them. Soothing the heart with music: A feasibility study of a bedside music therapy intervention for critically ill patients in an urban hospital setting

Abstract:Objectives: Music therapy has been shown to be effective for reducing anxiety and pain in people with a serious illness. Few studies have investigated the feasibility of integrating music therapy into general inpatient care of the seriously ill, including the care of diverse, multiethnic patients. This leaves a deficit in knowledge for intervention planning. This study investigated the feasibility and effectiveness of introducing music therapy for patients on 4 inpatient units in a large urban medical center. Capacitated and incapacitated patients on palliative care, transplantation, medical intensive care, and general medicine units received a single bedside session led by a music therapist. Methods: A mixed-methods, pre-post design was used to assess clinical indicators and the acceptability and feasibility of the intervention. Multiple regression modeling was used to evaluate the effect of music therapy on anxiety, pain, pulse, and respiratory rate. Process evaluation data and qualitative analysis of observational data recorded by the music therapists were used to assess the feasibility of providing music therapy on the units and patients' interest, receptivity, and satisfaction. Results: Music therapy was delivered to 150 patients over a 6-month period. Controlling for gender, age, and session length, regression modeling showed that patients reported reduced anxiety post-session. Music therapy was found to be an accessible and adaptable intervention, with patients expressing high interest, receptivity, and satisfaction. Significance of results: This study found it feasible and effective to introduce bedside music therapy for seriously ill patients in a large urban medical center. Lessons learned and recommendations for future investigation are discussed.

Abstract:Airborne laser scanning has been used increasingly for extracting and estimating forest parameters. Experiences in Nordic countries and Canada have shown that retrieval of stem volume and mean tree height on a tree or stand level from laser scanner data performs as well as, or better than, photogrammetric methods, and better than other remote sensing methods. The increasing interest in laser data for forestry applications has led to the present research, which quantifies forest growth and detects possible changes over time using repeated multi-temporal laser surveys over boreal forests. For the thesis, methods and techniques were developed for detecting change automatically and estimating forest growth using multi-temporal airborne laser scanning. The performance of these methods was evaluated based on the field measurements consisting of individual trees or sample plots. All the component studies were carried out in boreal forest at a test site in southern Finland. For the detection of change, e.g. harvested or fallen trees, an automatic method was developed based on the image differencing technique applied to digital canopy height models generated from laser data from different dates. New scientific approaches developed for height and volume growth estimation were the individual tree-top differencing method, digital surface differencing and canopy height distribution based analysis. In the individual tree-top differencing method, growth estimation was based on individual tree identification and a tree-to-tree matching algorithm. The digital surface differencing method was based on the difference image of digital surface models. In the analysis based on canopy height distribution, growth was determined as a function of the difference in corresponding percentiles of the canopy height distribution between different laser acquisitions. These methods can be applied at both the individual tree level and the plot/stand level. The findings reported in this thesis indicated that multi-temporal airborne laser scanner data can be used for estimating or predicting growth and detecting harvested area and fallen trees with an acceptable level of accuracy (an RMSE of less than 0.5m for individual tree height growth, a standard deviation of about 6.7 mha (26.8%) for volume growth and 0.15 m for mean height growth, and a detection accuracy of 80% for harvested trees). The methods developed could be used to complement field measurements, to improve predictions from a growth model and to develop newgeneration forest growth models.

Abstract:The mechanisms that appear to account for the effects of ionizing radiation on the growth of plants are reviewed. Preliminary results are reported from a systematic study of the effects of gamma radiation on a terrestrial ecosystem and its components. The ecosystem chosen for this experiment supports a stana of the Long Island pine-oak forest. A 9500 c Cs/sup 137/ source, designed so it can be shielded when desired. was centrally located. Rates of exposure vary from several thousand r/day within a few Meters to about 2 r/day at 130 meters. The source has been exposed 20 hr/day since Nov. 22, 1961 Prior to installation of the source, a detailed survey was made on the species composition of the vegetation and on the size and vigor of individual plants. The early effects of radiation exposure are described. (C.H.)

Abstract:The purpose of this study was to compare three music therapy strategies (music listening, music composition, and Orff-based active engagement) on physiological (heart rate, blood pressure, oxygen saturation, and pain) and psychosocial (anxiety) behaviors of hospitalized children (N=32, 17 females,15 males, ranging in age from 6 to 17). This study was designed and facilitated cooperatively by pediatric nurses and music therapists. Results indicated no clinically significant changes in heart rate, blood pressure, or oxygen saturation (p>.05). Pain and anxiety both decreased significantly (p=.01) but not differentiated among conditions. Videotape analysis determined level of engagement in coping-related behaviors.

Abstract:<div>BackgroundNonpharmacological interventions appear to benefit many patients and do not have the side effects commonly associated with medications. Music-based experiences may benefit critical care patients.ObjectiveTo examine the effect of an active music therapy intervention on physiological parameters and self-reported pain and anxiety levels of patients in the intensive care unit.MethodsA study was conducted using a pretest-posttest, within-subject, single-group design. The study population consisted of a convenience sample of 52 patients. Study participants received a 30-minute music therapy session consisting of either a relaxation intervention or a "song choice" intervention. The music therapist recorded the patients’ vital signs before and after the intervention, and patients completed self-assessments of their pain and anxiety levels before and after the intervention.ResultsAfter the intervention, significant decreases (all P < .001) were found in respiratory rate (mean difference, 3.7 [95% CI, 2.6–4.7] breaths per minute), heart rate (5.9 [4.0–7.8] beats per minute), and self-reported pain (1.2 [0.8–1.6] points) and anxiety levels (2.7 [2.2–3.3] points). No significant change in oxygen saturation level was observed. Outcomes differed between the 2 intervention groups: patients receiving the relaxation intervention often fell asleep.ConclusionsThe results of this study support active music therapy as a nonpharmacological intervention in intensive care units. This study may lay the groundwork for future research on music therapy in critical care units using larger, more diverse samples.</div>

This systematic review investigates the use of music to manage burnout in nurses.Data SourceMEDLINE (Ovid), MEDLINE InProcess/ePubs, Embase, APA PsycINFO, the Cochrane Central Register of Controlled Trials, and ClinicalTrials.gov databases were searched.Study Inclusion and Exclusion CriteriaFull-text articles were selected if the study assessed the use of music to manage burnout in nurses. Burnout was defined according to the International Classification of Diseases 11th Revision.Data ExtractionData were extracted using an Excel sheet. The second and third authors independently extracted study characteristics, frequency and type of music engagement, measures of burnout, and burnout outcomes (occupational stress, coping with stress, and related symptoms such as anxiety).Data SynthesisStudy and outcome data were summarized.ResultsThe literature search resulted in 2210 articles and 16 articles were included (n = 1205 nurses). All seven cross-sectional studies reported upon nurses' self-facilitated use of music including music listening, playing instruments, and music entertainment for coping or preventing stress, supporting wellbeing, or enhancing work engagement. Externally-facilitated music engagement, including music listening, chanting, percussive improvisation, and song writing, was reported in the four randomized controlled trials and five cohort studies with reductions in burnout outcomes.ConclusionsSelf-facilitated and externally-facilitated music engagement can help to reduce burnout in nurses.

Abstract:Delirium is a neuropsychiatric syndrome represented by an acute disturbance in attention, awareness and cognition, highly prevalent in older, and critically ill patients, and associated with poor outcomes. This review synthesized existing evidence on the effectiveness of music interventions on delirium in adults, and music interventions (MIs), psychometric assessments and outcome measures used. We searched MEDLINE, PsychINFO, SCOPUS, Clinical Trials and CENTRAL for quantitative designs comparing any MIs to standard care or another intervention. From 1150 studies 12 met the inclusion criteria, and 6 were included in the meta-analysis. Narrative synthesis showed that most studies focused on prevention, few assessed delirium severity, with the majority of studies reporting beneficial effects. The summary relative risk for incident delirium comparing music vs. no music in postsurgical and critically ill older patients was 0.52 (95% confidential interval (CI): 0.20–1.35, I2 = 79.1%, heterogeneity <0.0001) for the random effects model and 0.47 (95% CI: 0.34–0.66) using the fixed effects model. Music listening interventions were more commonly applied than music therapy delivered by credentialed music therapists, and delirium assessments methods were heterogeneous, including both standardized tools and systematic observations. Better designed studies are needed addressing effectiveness of MIs in specific patient subgroups, exploring the correlations between intervention-types/dosages and delirium symptoms.

Abstract:Music therapy can improve neonatal function and reduce anxiety in parents during neonatal intensive care unit (NICU) stays. Live music entrained to an infant's observed vital signs, provided by a certified music therapist with First Sounds RBL (rhythm, breath, and lullaby) training, enhanced bonding for infant-parent dyads and triads. The author's song of kin intervention, which employs parent-selected songs, is compared to the presentation of a well-known folk theme ("Twinkle") in 272 neonates. Culturally based, parent-selected, personalized musical tunes provided in song, as a noninvasive intervention, foster optimal, continuous quality of care. Music psychotherapy sessions for parents before working with their infants can instill a potent means of nonconfrontational support, allowing for expression of fear or anxiety related to the premature birth. Although most attention is typically directed to their infant, using music can support the parents' grief and assist in the expression of hope that can instill a sense of security and containment. From the NICU to home, a familiar thread-line theme can be resourced directly from the family and/or parent and applied effortlessly throughout the growing baby's transitional moments.

Abstract:Introduction: Music is an intriguing but relatively under-researched intervention with many potential benefits for mechanically ventilated patients. The review aimed to assess the impact of listening to music as a non-pharmacological intervention on the physiological, psychological, and social responses of patients in an intensive care unit. Methods: The literature review was conducted in the fourth quarter of 2022. The overview included papers found in Science Direct, EBSCO, PubMed, Ovid, Scopus, and original research papers published in English meeting the PICOS criteria. Articles published between 2010 and 2022 meeting the inclusion criteria were included for further analysis. Results: Music significantly affects vital parameters: decreases the heart rate, blood pressure, and breathing; reduces pain intensity. The analyses confirmed that music affects anxiety levels, reduces sleep disturbances and delirium occurrence, and improves cognitive function. The effectiveness of the intervention is influenced by the choice of music. Conclusions: There is evidence of the beneficial effects of music on a patient's physiological, psychological, and social responses. Music therapy is highly effective in reducing anxiety and pain and stabilizes physiological parameters, i.e., the heart rate and respiratory rate, after music sessions in mechanically ventilated patients. Studies show that music reduces agitation in confused patients, improves mood, and facilitates communication.

Abstract:Background: Observation and auditory skills are essential competencies for nursing practice. Research studies reveal that observational abilities are improved with visual training in an art museum and that the standing competence of auscultative skills is inadequate. This study details an innovative strategy to improve nursing students' observational and auscultative abilities. Methods: A pretest–posttest experimental design was used in which 77 students in an accelerated master's entry nursing program were randomized to either receive music auditory training and observation training in a museum and music hall or view a DVD of the music intervention and observation training in a classroom using handheld images and artwork display. Results: Students correctly identified approximately 68% of bowel sounds, 38% of lung sounds and 26% of heart sounds after this 2-h intervention, and significantly improved their observational abilities over time (p < 0.0001) on all measures with few exceptions. In addition, there were no differences between the groups over time for most measures, suggesting that the classroom experience is an effective pedagogy for improving observational skills. Conclusion: The activities of viewing works of art and aural training using music sharpen observational and reasoning skills of nursing students and auscultative interpretive abilities, and hold promise for medical education.

Abstract:Introduction: Music Therapy is a form of therapy that involves the musical sphere to promote the well-being and improvement of patients' vital conditions. Conversely, intensive care units are normally recognized as stressful and threatening environments. Therefore, it can be used to contribute to maintaining the mental health of these patients. Method: A systematic review with meta-analysis of Music Therapy in pediatric and neonatal intensive care units was performed. The methodology followed the PRISMA 2020 guidelines, with 10 articles analyzed, and a total of approximately 737 patients. The vital signs assessed were respiratory rate, heart rate, oxygen saturation, blood pressure, and perceived pain intensity. Additionally, a comparison was made between different types of music for each population studied. Results: It was observed that Music Therapy reduced the respiratory rate and pain sensitivity index in the pediatric ICU by 1.92 bpm (p-value: 0.004) and 2.09 Wong-Baker FACES units (p-value: <0.0001), respectively. Additionally, it has increased oxygen saturation in pediatric and neonatal ICUs by 0.37 (p-value: 0.001) and 0.2 (p-value: 0.04), respectively. The favorite kind of music listened to by the patients presented better results than the others. Conclusion: There is a scarcity of articles that would allow a comprehensive analysis of this theme, specifically studying different types of this therapy with these populations. Additionally, based on the results presented in the study, it is evident that music therapy is a potential treatment that can be utilized in a hospital setting. Few articles facilitate a comprehensive analysis of this theme, particularly regarding various types of therapy for these populations. Furthermore, the study's results have pointed out that music therapy is a viable treatment option in a hospital setting.

Evaluation of positioning performance of a fine-coarse mechanism

Abstract:A new positioning system has been developed to realize both the fine motion and the coarse motion independently and/or simultaneously to reduce the positioning time with guaranteeing positioning accuracy. A conventional speed reducer unit is utilized to cope with this problem in this study. The combination of reducer and overdrive enables to position the end effector with accurate and rapid. This fine-coarse motion of the developed system is realized by only commercial-release parts consisted of two stepping motors and speed reducer aligned in an axis. The main advantage of this mechanism is that there is no stroke limitation in fine and coarse motions compared with the conventional fine-coarse motion stage by usage of PZT device. The basic performances are evaluated by adopting the feedback positioning control in this report. The developed positioning system demonstrates the efficient performance with accurate and rapid positioning.
