Abstract:Background and ObjectiveTransoral laser surgery is useful for treating early glottic carcinoma. Wound areas after carbon dioxide (CO2) laser surgery are not covered by flaps, delaying healing. The healing process following laser cordectomy is not well understood. In this study, laryngoscopes were used to monitor the normal healing process after laser cordectomy and to determine if there was residual or recurrent disease. Materials and MethodsThis prospective case study included 36 patients who underwent CO2 laser surgery for early glottis carcinoma between January 2011 and June 2014 at a university tertiary referral center. Postoperative complications, oncologic results, and mucosal stabilization time were analyzed. Vocal fold stabilization was defined as an absence of gross changes during two consecutive follow-up examinations. ResultsThe 3-year overall survival rate was 100% and the 3-year local control rate was 94%. Vocal cord stabilization was observed in 29 patients at a mean 88.1 days, with stabilization associated with the type of cordectomy (P<0.05). During the wound-healing process, two patients experienced local recurrences. One had a persistent white patch for over 4 months and the other presented with a new mass in a non-operated area. Biopsies of both lesions showed recurrent disease. Postoperative complications included granulation in two patients and anterior commissure adhesion in one. ConclusionCO(2) laser microsurgery is an effective treatment for early glottis carcinoma. Close attention should be paid to delays in healing process after laser surgery. Improving patient care and quality of life after laryngectomy/glossectomy

Abstract:Patients undergoing total laryngectomy/glossectomy are left without a voice at least temporarily in the early postoperative stage, since their larynx and/or tongue has been removed. Those patients rendered permanently speechless usually acquire a form of mechanical speech through electronic devices. However, the life-changing nature of this surgery should not be underrated as effectively these people have lost their normal voice. It can be argued that this patient group is also without a voice in a political sense - theirs seems to be a forgotten cancer, as other forms of cancer appear to attract greater attention for research priorities and funding. In some ways this research attempts to redress the balance through exploring the patient experience postoperatively, in particular the issues related to communication for this patient group. The results also highlight the emotional trauma experienced by patients and the mechanisms healthcare staff employ to support these patients. This research is multidisciplinary, involving patients and all those health workers that come into contact with them on the ward. The patient experience forms a major part of this article, allowing their stories to form much of the content. The results draw attention to deficiencies in service delivery that, as a direct result of this research, have been and are being addressed so that patients' care is improved and their quality of life is restored. Such change has been driven by patient comment such as: 'But then when you wake up after that operation that's the pits when you are laying there and you think "I'm dead". And then when you wake up properly that's the bit I always say you wish you were dead. That's when you wish you had died.'

Abstract:Background and aim of the study: Therapeutic education helps patients with a chronic disease to acquire and maintain the ability to live their life while handling their illness. Patients with temporary medium-term tracheotomy or permanent tracheostomy need to acquire skills to be able to handle the stoma, tracheal tube, related issues, and other apparatuses. This was the purpose of our therapeutic education program, which was aimed to take patients and caregiver to an efficient level of self-care. Methods: In 2018, was created a CME-accredited (Continuing Medical Education) "Workplace-based Learning Project" involving all the nurses in the Otolaryngology Head and Neck Operational Unit, along different specialists on the Disease Management Team, thereby forming an "Improvement Group". We established parallel workgroups for bibliography research on data-based like PubMed, Cinahl, Cochrane, Google scholar, in order to obtain the information to write up a shared document. Results: We wrote out an Operational Protocol which lined up nursing skills - when handling patients with medium-term tracheotomy or tracheostomy - with scientific evidence. Our standard educational plan (customizable, based on each patient's characteristics) promote the patient's learning with respect to self-care. Conclusions: This project has set the basis for the improvement of the quality of assistance given to the patients and therapeutic education provided them. It has encouraged the development of the skills of the nurses involved, along with their motivation, and their integration on the Disease Management Team. But, it will be necessary in the future to further evaluate the effectiveness of the program in terms of self-care.
The impact of total laryngectomy: the patient's perspective

Abstract:Purpose/objectives: To describe the experiences of patients who had total laryngectomy from their perspective. Research approach: Descriptive, qualitative study. Setting: Participants' homes or investigator's hospital office. Participants: 10 patients after total laryngectomy. Methodologic approach: Data were collected by semistructured, open-ended interviews during a period of six months, with an interview topic guide built on the framework of the literature review. Data were analyzed with descriptive content analysis. Trustworthiness of the study was enhanced through the use of verbatim quotations, audible data analysis trail, and a reflexive approach. Main research variables: Patients' experiences of undergoing total laryngectomy. Findings: Patients who have undergone a total laryngectomy report difficulties and concerns that are largely functional and psychological. The functional difficulties reported included descriptions of altered swallow, excess phlegm, speech difficulties, weak neck muscles, and altered energy levels. The psychological concerns reported included descriptions of depression, regrets, and personal resolve. Conclusions: As a group, patients experience a broad range of problems well after completion of treatment, reinforcing the need for rehabilitation management for prolonged periods after surgery. Interpretation: Nurses are suitably positioned to support this group of patients across the disease management trajectory, from the initial preoperative period to the postoperative period and through to the rehabilitative period and beyond.
Rehabilitation Program for Postlaryngectomy Patients Following Ileocolon Flap Transfer for Voice Reconstruction: An Essential Part of Success

Abstract:Background Speech restoration is important for communication and social activities after pharyngolaryngectomy in head and neck cancer or corrosive injury. Several techniques of voice restoration have been developed to improve life quality. The aim of this paper was to focus on the microsurgical transfer of ileocolon flap and outcome of further voice rehabilitation. Patients and Methods From 2010 to 2022, 69 patients had ileocolon flap at our hospital with postoperative speech training and regular follow-up for over 1 year. The patients received deglutition training first, followed by voice rehabilitation. Voice outcomes were evaluated at an interval of 3 months and finally at 12 months of voice training rehabilitation. Among other examinations, the speech function was evaluated using a 4-point Likert scale and senior surgeon (H-c.C.) scoring system. Results The results showed that speech function reached 13.1% of excellent voice, 65.1% of good voice, 13.1% of fair result, and 8.7% of poor result by Likert scales. Meanwhile, the senior surgeon (H-c.C.) score showed 17.4% of excellent, 63.8% of moderate, and 18.8% of poor results. About voice laboratory results, maximal phonation time was 11.0 seconds, and the average number counted in one breath was 15. Loudness and frequency showed 56.0 dB and 105.0 Hz, respectively. Conclusion The study showed that after voice reconstruction with ileocolon flap followed by the voice rehabilitation program, the patients would have a better understanding of the altered anatomical structures and practice in a more efficient way. Adequate recommendation by the therapists to plastic surgeons for revision surgeries optimized voice function of the patients. Abstract:Abstract Background Surgical treatment for airway stenosis necessitates personalized techniques based on the stenosis location and length, leading to favorable surgical outcomes. However, there is limited literature on functional outcomes following laryngotracheal surgery with an adequate number of patients. Material and Methods We conducted a retrospective analysis of patients who underwent laryngotracheal surgery at the Department of Thoracic Surgery, Medical University of Vienna, from January 2017 to June 2021. The study included standardized functional assessments before and after surgery, encompassing spirometry, voice measurements, swallowing evaluation, and subjective patient perception. Results The study comprised 45 patients with an average age of 51.9 ±15.9 years, of whom 89% were female, with idiopathic being the most common etiology (67%). Procedures included standard cricotracheal resection (CTR) in 11%, CTR with dorsal mucosal flap in 49%, CTR with dorsal mucosal flap and lateral cricoplasty in 24%, and single-stage laryngotracheal reconstruction in 16%. There were no in-hospital mortalities or restenosis cases during the mean follow-up period of 20.8±13.2 months. Swallowing function remained intact in all patients. Voice evaluations showed a decrease in fundamental vocal pitch (203 (81-290) Hz vs. 150 (73-364) Hz, p < 0.001) and dynamic voice range (23.5 ±5.8 semitones vs. 17.8 ±6.7 semitones, p < 0.001). However, no differences in voice volume were observed (60.0 ±4.1 dB vs. 60.2 ±4.8 dB, p = 0.788). The overall predicted voice profile changed from R0B0H0 to R1B0H1. Conclusion Laryngotracheal surgery proves effective in fully restoring breathing capacity while preserving vocal function. Even in cases of high-grade and complex airway stenosis necessitating laryngotracheal reconstruction, favorable functional outcomes can be achieved.

Abstract:BACKGROUND:It is important for the patients to reconstruct the voice phonic function by surgery after total laryngectomy in the developing countries.AIMS/OBJECTIVES:To investigate the clinical outcomes of voice reconstruction using an infrahyoid musculocutaneous flap for patients after total laryngectomies.MATERIALS AND METHODS:Eighteen male patients recruited were laryngectomized. The infrahyoid musculocutaneous flap was designed. After total laryngectomy, the lower edge of the flap was sewed with the upper edge of the tracheostomy opening. Next, the lateral and medial edges of the flap were anastomosed to create a pronunciation tube. Finally, the remaining opening of the tube was sutured with the anterolateral wall of the hypopharynx to establish a communication with the pharyngeal cavity.RESULTS:A total of 17 cases of flaps were survived and only 1 necrosed. There were 17 patients without serious complications, except that 6 cases had mild irritable cough when gulping water. However, it could be relieved through blocking tracheostoma. One year after operation, all patients could more remarkably articulate clear, powerful, and consistent words. The articulatory configuration was existed under rigid laryngoscope and CT.CONCLUSIONS AND SIGNIFICANCE:The use of an infrahyoid myocutaneous flap is feasible for the voice restoration in the patients undergoing total laryngectomy.
Communication competence and disability secondary to laryngectomy and tracheoesophageal puncture voice restoration

Abstract:Background: There is a large body of research showing the negative impact of a total laryngectomy on the resultant voice signal across multiple outcomes (e.g., speech intelligibility/acceptability, listener comprehension, voice quality). However, there is limited information on the effects of laryngectomy on broader communication acts, particularly in the area of pragmatics, commonly referred to as the social use of language. Considering that individuals with a laryngectomy (IWL) frequently report reduced quality of life as well as reduced communicative competency, expanding the current research to include pragmatics is warranted. Aim: To explore the pragmatic changes in communication experienced by tracheoesophageal speakers. Methods & procedures: The current study adapted an existing 17-item measure to assess verbal, non-verbal, paralinguistic and receptive communication competence via self-report. This adapted measure was then distributed to 65 tracheoesophageal speakers (52 males, 13 females, mean age = 63.4 ± 8.5 years). Outcomes & results: Both negative and positive pragmatic changes to communication were reported by participants secondary to laryngectomy. This included changes in verbal (negative -37.5%, positive -15.15%); non-verbal (negative -9.54%, positive -35.45%; and paralinguistic acts (negative -29.55%, positive -34.09%). Changes to receptive communication were also noted (negative -14.78%, positive -43.19%). Conclusions & implications: The overall results suggest that communication changes post-laryngectomy exist well beyond the paralinguistic areas (e.g., intelligibility, voice quality) and that males and females may approach or respond to changes in communication differently. Results are discussed specific to clinical intervention and the importance of including assessment of pragmatic function post-laryngectomy. What this paper adds: What is already known on this subject While there is a large body of research on the changes to the speaker and listener following laryngectomy, there is minimal information on how the use of alaryngeal speech affects overall communication, specifically in the area of pragmatics or the social use of language. What this paper adds to existing knowledge This study used an adapted version of the widely used Pragmatic Protocol to delineate changes in pragmatic components of communication for tracheoesophageal speakers. What are the potential or actual clinical implications of this work? Clinically, this information can be used by healthcare professionals to educate and prepare IWL on potential changes in the underrecognized area of pragmatics.
Validating the Changes to Self-identity After Total Laryngectomy

Abstract:Background: A total laryngectomy often prolongs life but results in long-term disablement, disfigurement, and complex care needs. Current clinical practice addresses the surgical options, procedures, and immediate recovery. Less support is available longer-term despite significant changes to aspects of personhood and ongoing medical needs. Objective: The aim of this study was to explore the experience of living with and/or supporting individuals with a laryngectomy at least 1 year after surgery. Methods: Constructivist grounded theory methods and symbolic interactionism were used to guide collection and analysis of interview data from 28 participants (12 individuals with a laryngectomy, 9 primary supporters, and 7 health professionals). Results: The phenomena of "validating the altered self after total laryngectomy" highlighted how individuals, postlaryngectomy, navigate and negotiate interactions due to the disruption of their self-expression, related competencies, and roles. Several reframing patterns representing validation of the self emerged from the narratives. They were as follows: destabilized, resigned, resolute, and transformed. The data describe the influence of the processes of developing competence and building resilience, combined with contextual factors, for example, timing and turning points; being supported; and personal factors on these reframing patterns. Conclusion: The findings further our understanding of the long-term subjective experience of identity change after laryngectomy and call attention to the persisting need for psychosocial support. Implications for practice: This research provides important evidence for evaluating and strengthening the continuum of services (specialist to community) and supporting social participation, regardless of communication method, and for competency training for all involved to optimize person-centered practices.
Surgical Rehabilitation of Voice and Swallowing after Jugular Foramen Surgery

Abstract:Objectives We sought to determine the patient population that will benefit from surgical rehabilitation of voice and swallowing after jugular foramen tumor (JFT) resection. Methods We performed a retrospective case study of patients with a history of JFT resection. The patients' files were reviewed for data on preoperative and postoperative function of cranial nerves VII and IX through XII, voice and swallowing function, and surgical procedures for voice and swallowing rehabilitation and their timing. Results Twenty-one patients underwent JFT resection. Thirty-eight percent presented with deficits of cranial nerves VII and IX through XII, and 61% developed new postoperative deficits. Three patients recovered glossopharyngeal nerve function, 2 recovered vagus nerve function, and 1 recovered facial nerve function. Surgical rehabilitation procedures were undertaken in 8 patients. Patients who eventually underwent surgical rehabilitation procedures for voice and swallowing tended to have larger tumors, tumors within the nerve bundle in the jugular foramen, and multiple nerve deficits. Conclusions Most patients with multiple deficits of cranial nerves VII and IX through XII after JFT resection are unlikely to regain spontaneous nerve function, will experience long-term dysphonia and dysphagia, and will elect to undergo corrective surgery to improve voice and swallowing. Preoperative evaluation and close postoperative follow-up can identify patients who would benefit from early surgical rehabilitation.
Effectiveness of voice rehabilitation on vocalisation in postlaryngectomy patients: a systematic review

Abstract:Background: Laryngeal cancer is one of the most common malignant neoplasia of the head and neck. Its incidence has been increasing steadily all over the world. Many patients will undergo total laryngectomy with or without radical neck dissection after being diagnosed. After this surgery, normal speech is lost, and a permanent stoma in the middle of the neck is left. Therefore, voice rehabilitation is one of the most difficult challenges that these patients must overcome. In order to support the patients, otolaryngologists, nursing specialists and speech pathologists have explored several different methods for voice rehabilitation. Variations exist on the approaches of rehabilitation and indicators selected to measure the effectiveness. There is a need to undertake a systematic review to provide a plan of care and ascertain the effectiveness regarding different voice rehabilitation programs for postlaryngectomy patients. REVIEW OBJECTIVE: To critically analyse the literature and present the best available evidence related to the effectiveness of voice rehabilitation program on postlaryngectomy patients. Search strategy: A three-step search strategy was utilised. An initial limited search of MEDLINE and CINAHL databases was undertaken followed by an analysis of the text words contained in the title and abstract to identify the optimal index terms. A second extensive search using all identified key words and index terms was then undertaken. Third, the reference list and bibliographies of all identified reports and articles were searched for additional studies. The measurement index included voice intelligibility, volume, clarity, quality of voice, patients' satisfaction, quality of life, etc. The search included reports in English and Chinese. Selection criteria: The review considered any randomised controlled trials that addressed voice rehabilitation methods in postlaryngectomy patients. In the absence of randomised controlled trials, other quantitative research designs, such as non-randomised controlled trials, cohort studies and case-controlled studies, were considered for inclusion. Data collection and analysis: Full copies of articles considered to meet the inclusion criteria were obtained for critical appraisal by two reviewers using the CASP (Critical Appraisal Skills Program) and McMaster scales. We utilised the 60% fulfilling of the evaluation scale items as the cut-off point and articles with a quality score less than 60% were excluded. Details of eligible trials were extracted and summarised by two reviewers independently using standardised data extraction tools developed by the Joanna Briggs Institute. Results: Twenty-two articles were included in the review (Appendix I). Different voice rehabilitation methods for laryngectomees were investigated in the included studies including oesophageal voice, electrolarynx voice and surgical voice restoration. One cohort studies, five prospective studies, five retrospective studies and 11 descriptive studies were included. Because of the heterogeneity of included studies, meta-analysis was not possible. Therefore, the results were presented in narrative summary. The following main findings were identified: 1 At present, oesophageal speech, electrolarynx and tracheoesophageal were the commonly used voice rehabilitation methods with total laryngectomy patients. 2 Among these three methods, the success rate of electrolarynx and tracheoesophageal is much higher than oesophageal speech. 3 The intelligibility and speech quality of electrolarynx was lower than tracheoesophageal. 4 Patient satisfaction and self-assessed quality of life was better in tracheoesophageal group. 5 The objective index was similar between excellent tracheoesophageal and oesophageal speech patients. Conclusion 1 Electrolarynx is the easiest vocal rehabilitation method for total laryngectomy patients to use as it requires little training and does not limit the patients. But patients' satisfaction was lower because of the mechanical voice and noise. 2 Oesophageal speech is the hardest vocal rehabilitation method to learn. It needs a long period of time to practise and requires the patient to be in good physical condition and to be relatively young. The success rate was relatively lower; however, it is the most commonly used rehabilitation method in developing countries because of low cost. 3 Tracheoesophageal is the most commonly used voice rehabilitation method in developed countries. It is a surgical method that could be performed as either a primary procedure or secondary procedure. Reported patient quality of life and satisfaction following tracheoesophageal were the best; however, there are complications and the frequent replacement of the prostheses is an important problem yet to be solved. Implication for practice 1 Voice rehabilitation after total laryngectomy is an immediate and long-term problem that patients and health worker must face. 2 Healthcare workers should understand the advantages and disadvantages of each voice rehabilitation method in detail to assist people with total laryngectomy to make the most appropriate decision in regard to rehabilitation method taking into consideration their age, sex, physical condition, job, economic status and other context factors. Implication for research 1 Further high-quality studies comparing the effectiveness of oesophageal speech, electrolarynx and tracheoesophageal vocal rehabilitation methods are needed, especially with the subjective and objective outcome index concurrently. 2 Further investigation is required to identify strategies to decrease the complications of tracheoesophageal and reduce frequency of required tracheoesophageal replacement. 3 More research is needed in the context of developing countries where healthcare resources may be limited.
Rehabilitation Practice of Esophageal Speech After Total Laryngectomy

Abstract:Objective To enhance the living quality of laryngectomees through the training of esophageal phonation in total laryngectomy patients.Methods 105 laryngectomees had received the intact esophageal phonation training for four weeks since June 2004.Among the 105 patients,61 patients received total laryngectomy,21 patients received total laryngectomy and neck dissection,23 patients received operation and radiotherapy.There were 48 patients within 38～58 years old,40 patients within 59～69 years old and 17 patients within 70～79 years old.We investigated the effect of esophageal phonation and speech according to different ages and therapy modes when the training was finished.Results 84 patients(80.0%) regained the phonation function to different extents and the effective rate was different with the difference of ages or therapy modes.The effective rate of the patients received total laryngectomy was 80.3%(49/61),the patients received total laryngectomy and neck dissection was 80.9%(17/21),the patients received operation and radiotherapy was 78.3%(18/23),38～58 years old was 89.5%(43/48),59～69 years old 87.5%(35/40) and 70～79 years old was 35.3%(6/17).Conclusion The training of esophageal phonation is the relatively perfect speech rehabilitation for laryngectomees and the esophageal speech is similar to the normal laryngeal speech.There is no significant difference between the different kinds of therapy modes,but the effect of the esophageal phonation training is worse when the patient's age is older than 70.

Abstract:BACKGROUND: Most patients perceive total laryngectomy as a mutilation carrying with it a loss of physical and psychological integrity. Thus, an artificial larynx system that can replace the laryngeal functions would significantly improve the quality of life for the afflicted patients.METHODS: This report, with accompanying video, presents the first case in an ongoing clinical trial of laryngeal rehabilitation using an artificial larynx after total laryngectomy for squamous cell carcinoma, for an 8-month follow-up period. We depict the prosthesis' features, our 2-step surgical procedure, and the outcome. The prosthesis is formed of 2 parts: (1) a tracheal prosthesis with a porous titanium junction with trachea, which was implanted in the first step to ensure its colonization, and (2) a removable part composed of concentric valves that enable inhalation and exhalation. The second part was implanted endoscopically. The implant was monitored with a retrograde nasofibroscopy of the tracheal prosthesis lumen and CT scans over a course of 8 months.RESULTS: The patient's functioning in the relevant postoperative problem areas, such as swallowing, breathing, and smelling, has significantly improved. The patient was able to talk in a whispering fashion while the tracheostomy was temporarily closed. The implant's porous part was in the process of being colonized by the surrounding tissue and no fistulas were observed as evidenced by barium swallow.CONCLUSION: As the current case shows, tracheotomy closure can be performed, and laryngeal functions are restored, by means of an implant. With further improvements, this system can alleviate the need for a permanent tracheostomy after total laryngectomy, while maintaining important larynx functions intact.

Abstract:Objective: Speech rehabilitation significantly contributes to the enhanced quality of life for patients who have undergone laryngectomy due to head and neck cancer. The initiation of tracheoesophageal speech early in the rehabilitation process has been associated with improved speech quality. Despite this, voice changes can carry a stigma due to communication challenges, even after successful voice restoration, potentially limiting the overall improvement in quality of life. Thus, our aim was to gain a profound understanding of the transition process from the selection to the adaptation of tracheoesophageal speech in patients with head and neck cancer. Methods: Participants, who had undergone laryngectomy for head and neck cancer, were recruited from peer support groups in Japan, resulting in the identification of thirteen tracheoesophageal speakers. The data were analyzed using grounded theory methodology, incorporating open, axial, and selective coding. Semi-structured interviews delved into tracheoesophageal speakers' perceptions of their voices and the adaptive strategies employed for their preferred alternative communication methods. Results: During the adaptation process, participants underscored the importance of communicating with their own voices. However, they also experienced anxiety about losing their natural voice, confronted the reality of living without a voice, and coped with their new voice. Eventually, participants came to recognize the changed voice as their own. Conclusions: The personal conviction associated with the notion of "communicating with one's own voice" is integral to the adaptation process for patients with head and neck cancer who undergo laryngectomies and utilize tracheoesophageal speech. Some aspects of tracheoesophageal speech can evoke feelings of hopelessness, emphasizing the importance of comprehensive support for professional speech rehabilitation.
A qualitative study of the experience of returning to family life and the coping styles of patients after total laryngectomy

Abstract:Background: Total laryngectomy is the major treatment for stage III-IV laryngeal cancer, which prolongs patient's life; however, brings significant changes to physical function, mental health, and social life. As a result, patients must adopt various strategies to cope with these changes and challenges. Few studies have investigated the individual experiences of laryngectomized patients returning to home and society in China. Here we explored the living conditions and inner experiences of laryngectomized patients after discharge from the hospital, and identified the main difficulties they encountered and their coping strategies. Methods: This study adopted a qualitative descriptive approach with a semi-structured interview. Purposive sampling helped to recruit 19 patients who had undergone total laryngectomy at a tertiary hospital. Patients were interviewed at their follow-up visits 6-12 months after discharge. The interview content was analyzed according to the Creswell steps. Similar descriptions of the patients' experiences were categorized into specific themes. Results: Three major theme categories were extracted from the real-world experiences of patients following total laryngectomy to return to their former roles in family/social life and work-related activities: (I) impact of surgery on family life, including changes in physical function (e.g., decreased physical activities and functional impairment) and family roles (e.g., becoming a burden to the family and attempts to live as before); (II) impact of surgery on returning to society, including reduced social interaction and difficulty in social reintegration; and (III) the patients' coping styles, including negative coping (e.g., avoidance, compromise, and limited access to social resources) and positive coping (e.g., self-motivation and effective use of social resources). Conclusions: After total laryngectomy, patients face numerous challenges, and adopt different coping styles to deal with stressful situations. Care and support from family can help them adapt to the change in role and regain confidence in life. Actively seeking social support and using social resources effectively can help patients overcome adverse psychological effects and promote self-identity. Medical staff should provide personalized social support (including information, emotional support, and belonging), and strengthen the post-discharge follow-up and continuous management to promote the functional recovery of patients.

Abstract:Introduction: Total laryngectomy is used to cure advanced larynx cancer in many patients. The removal of the larynx requires the rehabilitation of the patient's ability to communicate, and one common method is to place a tracheoesophageal voice prosthesis (TEP) as a secondary procedure after the patient has completed cancer treatment. The traditional technique utilizes a rigid esophagoscope for access, but this can prove difficult in many patients who have kyphosis, scarring of the neck, or trismus. We describe a technique to allow TEP placement in these challenging patients that does not utilize rigid esophagoscopy to access the tracheoesophageal puncture site. Methods: For more than 15 years, the senior authors of this study have used this technique in patients in whom traditional methods of TEP with rigid esophagoscope were unsuccessful or not attempted due to the anticipated high probability of failure. The ease of this technique has prompted its use for all patients undergoing secondary TEP placement in their practice. The technique is described in detail in the Methods section below. Results: The described method has been successfully utilized to place TEPs in many patients with challenging anatomy. There have been no failed placements, including a patient with severe trismus who was able to have a TEP placed by placing the chest tube and flexible endoscope transnasally. Further, because of precise visualization and ease of the technique, there have been no observed complications of injury to the pharyngoesophageal lumen or creation of a false passage. Conclusion: The use of a chest tube and flexible scope allows for the protection of the pharyngoesophageal lumen, precise visualization and placement of the puncture, and avoidance of a false tracheoesophageal passage, all while minimizing the need for extension of the patient's neck. This has proven ideal for patients suffering the consequences of cancer treatment such as cervical scarring, fibrosis, kyphosis, and trismus.

Abstract:Summary Background and purpose Laryngeal transplantation (LT) remains an infrequently performed procedure due to well-defined and limited medical indications. The challenges include a very complicated surgery, continuous immunosuppressive treatment, as well as post-transplant rehabilitation of voice, speech, and swallowing. The aim of this paper is to describe a model of voice and speech rehabilitation after LT, based on our own experience. Study design Observational study. Method Presented here are the rehabilitation processes of two patients who had previously undergone LT as well as their pre- and postrehabilitation voice outcomes. Both patients underwent voice, speech, and swallowing rehabilitation in the fifth month after LT. Results One of the patients had a significant improvement in voice quality. While, the other did not regain his natural, sonorous voice, but he scored very high on a voice self-assessment test. Conclusion Voice rehabilitation after LT is a complex and long-term process that requires the involvement of a team of specialists. The optimal anatomical conditions restored during surgery, the patient's motivation and the support of professionals make it possible to return to normal verbal communication.
