Abstract:Objective:To observe the effects of transurethral split of the prostate (TUSP) on the urodynamics, the sexual function and the quality of life of benign prostatic hyperplasia.Methods:Eighty patients with benign prostatic hyperplasia admitted to the Deqing People′s Hospital of Zhejiang Province from January 2017 to June 2019 were enrolled into the research objects prospectively. According to the random digits table method, they were divided into group A and group B, with 40 cases in each group. The group A was treated with TUSP, while the group B was treated with bipolar transurethral plasma kinetic prostatectomy (TUPKP). The operation time, intraoperative blood loss, bladder irrigation time, urinary catheter indwelling time, hospitalization time of the two groups were compared and surgical effects were evaluated. The changes of hemoglobin (Hb), serum sodium (Na), post-void residual volume (PVR), maximum urinary flow rate (Q max), international prostate symptom score (IPSS) and international index of erectile function score (IIEF-5), the quality of life index (QOL) of 2 groups before and after the operation were measured. Surgical complications were recorded. Results:The operation time, bladder irrigation time, urinary catheter indwelling time and hospital stay in group A were all shorter than that in group B: (15.63 ± 4.17) min vs. (58.79 ± 10.45) min, (6.26 ± 1.17) h vs. (45.51 ± 10.03) h, (3.07 ± 0.68) d vs. (5.67 ± 1.51) d, (3.63 ± 0.43) d vs. (6.08 ± 1.72) d, (18.32 ± 2.79) ml vs. (65.26 ± 20.64) ml, and there were statistical differences ( P<0.05); there were no statistical difference between 2 groups in surgical curative effect grade and Na 1 d after operation ( P>0.05); 1 day after operation, Hb in group A was higher than that in group B: (115.63 ± 9.78) g/L vs. (109.65 ± 8.36) g/L, and there was statistical difference ( P<0.05); PVR, Q max, IPSS, IIEF-5 and QOL 3 and 6 months after surgery were improved in 2 groups ( P<0.05), but the difference between 2 groups was not statistically significant ( P>0.05); the total complication rate in group A was lower than that in group B: 10.00% (4/40) vs. 27.50% (11/40), and there was statistical difference ( P<0.05). Conclusions:TUSP and TUPKP are equally effective in the treatment of benign prostatic hyperplasia, both of which can improve the symptoms of prostatic hyperplasia, improve the quality of life and improve sexual function. But TUSP has less intraoperative bleeding, shorter operation time, faster postoperative recovery, fewer complications, and higher safety.

Abstract:To assess the effect of bladder neck preservation (BNP) on postoperative continence during laparoscopic radical prostatectomy.One hundred and forty-five patients with localized prostate cancer (Tlb-T2c) underwent laparoscopic radical prostatectomy in our center from July 2006 to May 2010, including 59 cases treated with bladder neck preservation (BNP group) and 86 cases with bladder neck resection (non-BNP group). All cases were diagnosed as prostate cancer by transrectal ultrasonography-guided prostate biopsy preoperatively, in which localized tumors were confirmed by CT or MRI and distant metastases were ruled out by ECT bone scan. All patients had no history of incontinence and no radiation therapy preoperatively. All the 145 operations were performed by the same surgeon. The bladder neck preservation was defined as a procedure of direct suturing of the bladder neck on the urethra without repair and reconstruction of the bladder neck. Both procedures of neurovascular bundle preservation and external striated urethral sphincter preservation were carried out on all cases. Urinary continence was evaluated using the International Continence Society questionnaire at 1, 3 and 6 months postoperatively. Positive surgical margins rates were compared between the two groups. Postoperative continence was defined as the absence of need for pads or the use of one pad daily.At 1, 3 and 6 months, the urinary continence rates were 42.4%, 74.6% and 86.4% in BNP group, respectively, while 25.6%, 58.1% and 80.2% in non-BNP group, respectively. There were statistically significant differences in continence at 1 and 3 months between two groups (P <0.05), while no significant differences were observed at 6 months postoperatively (P=0.331). There were no significant differences in overall rate of positive surgical margins between two groups (10.1% Compared with 10.4% P=0.954) and both groups had one case with positive surgical margins at bladder neck.Bladder neck preservation during laparoscopic radical prostatectomy is helpful for postoperative continence without increase of positive surgical margins rate.

Abstract:Purpose: Laparoscopic Madigan prostatectomy have not been reported yet. We modified the Madigan prostatectomy to make it suitable for laparoscopically enucleating hyperplastic glands larger than 100 g. Patients and Methods: Between May 2007 and Oct 2008, extraperitoneal laparoscopic prostatectomy with maintenance of the intact urethra had been performed on 16 patients with benign prostatic hyperplasia (BPH) and glands than 100 mg. To make it suitable for laparoscopic use, two major modifications had been made: (1) Open the prostate capsule near the bladder neck without sutures along the opening; (2) identify the bladder neck mucosa before recognizing the urethra. All patients were evaluated preoperatively and postoperatively. Data were compared with those from open surgeries. Results: All laparoscopic procedures were successful with the total operative time of 111.8 +/- 28.6 minutes, which had no significant difference compared with open surgeries. Estimated blood loss of laparoscopic procedures (112.5 +/- 47.8 mL) was significantly lower than that of open surgery. The catheterization time and hospital stay time was significantly shorter than open surgery. The improvement of the International Prostate Symptom Score, maximum flow rate, and quality-of-life score were not different between the comparing groups. Conclusions: The laparoscopic Madigan prostatectomy is a safe and feasible approach for large glands (BPH). Furthermore, its advantages include shorter learning curve, reduced blood loss, less retroejaculation rate, shorter catheterization time, and shorter hospital stay.
Comparison of transperitoneal and extraperitoneal laparoscopic radical prostatectomy

Abstract:Objective To compare the clinical features and results between transperitoneal laparoscopic radical prostatectomy and extraperitoneal laparoscopic radical prostatectomy.Methods Thirty-three prostate cancer patients treated with laparoscopic radical prostatectomy. Among them,21 cases had been done transperitoneally and 12 cases had been done extroperitoneally. The two different approaches were evaluated and compared in respects of operating time, estimated blood loss, complications during surgery, postoperative complications, intestinal function recovery time, catheterization time and length of hospital stay.Results All the surgeries had completed successfully without conversion to open surgery. For transperitoneal approach and extraperitoneal approach, the operating time was (299±46)min and (309±64)min, blood loss was (618±448)ml and (677±469)ml. There were 3 cases with severe blood loss, 2 cases with bladder injuries and 1 case with ureteral injury in transperitoneal approach group. There were 1 case with severe blood loss, 1 case with obturator never injury, 1 case with cysto-ureteral injury and 1 case with peritoneum injury in extraperitoneal approach group. For transperitoneal approach and extraperitoneal approach,the catheterization time was(14.6±3.8)d and (12.3±2.9)d, intestinal function recovery time was (2.7±0.7)d and (2.1±0.5)d, length of hospital stay was (17.0±3.6)d and (11.2±3.5)d, respectively.Conclusions Laparoscopic radical prostatectomy is feasible and safe in clinical practice. Extraperitoneal approach has better vision, less impact on abdominal organs, faster recovery and shorter hospital stay comparing to transperitoneal approach.
Comparison between a transurethral prostate split and transurethral prostate resection for benign prostatic hyperplasia treatment in a small prostate volume: a prospective controlled study

Abstract:Background: Transurethral resection of the prostate (TURP) was considered the golden standard to treat benign prostatic hyperplasia (BPH) for decades. However, TURP was associated with low efficiency to alleviate the lower urinary tract symptoms (LUTS) and a significantly higher risk of bladder neck contracture (BNC) for patients with small-volume BPH. Our study aims to compare the therapeutic effect of a transurethral split of the prostate (TUSP) with TURP for patients with small-volume BPH (<30 mL). Methods: In this study, 101 small-volume BPH patients were randomly divided into two groups (TUSP and TURP group). The patient's baseline characteristics and perioperative outcomes were recorded. The follow-up was done at six months, one year and two years after surgical treatment. Results: No significant differences were observed between the two groups for the baseline characteristics, including age, prostate volume, prostate-specific antigen (PSA) level, concurrent disease, post-void residual (PVR), maximum urinary flow rate (Qmax), international prostate symptoms score (IPSS), and quality of life (QoL) score. The operative time and hemoglobin decrease were significantly lower in the TUSP group compared to the TURP group. However, no significant differences were observed between both groups for catheterization time, postoperative hospital stay, and incidence of transurethral resection syndrome (TURS). However, of the late complications, the incidence of BNC in the TUSP group was significantly lower than the TURP group. No significant differences were found between both groups for other complications, including postoperative bleeding, micturition urgency, micturition frequency, micturition pain, urinary tract infection, recatheterization, transient incontinence, and continuous incontinence. Follow-up results showed that the IPSS of the TUSP group was significantly lower than the TURP group, while the Qmax of the TUSP group was significantly higher than the TURP group. Conclusions: This study shows that TUSP may be an efficient and safe treatment for small-volume BPH (<30 mL) with a lower incidence of postoperative BNC and better longtime clinical outcomes than TURP. It suggested that TUSP could be an ideal treatment choice for small-volume BPH.
Diode Laser Enucleation vs. Bipolar Transurethral Enucleation of Prostate for Benign Prostatic Hyperplasia: A Retrospective Comparative Study with Three-Year Follow Up

Abstract:Objective: To assess the clinical outcome of 1470 nm diode laser enucleation of the prostate (DiLEP) vs. bipolar transurethral enucleation of prostate (TUEP) for benign prostatic hyperplasia (BPH). Methods: One hundred and fifty-seven patients were retrospectively involved in this study. Eighty-two patients underwent DiLEP while 75 underwent bipolar TUEP. Seventy-three patients in DiLEP and sixty-nine in bipolar TUEP completed the 3-year follow-up, respectively. The baseline properties, perioperative data, and postsurgical outcomes were evaluated. Results: No statistically significant differences were found between DiLEP and bipolar TUEP in preoperative parameters. Significantly shorter operating time was noted in DiLEP group (p = 0.000). No patient suffered dangerous complications, and none in either group required a blood transfusion. No statistically significant differences were found between DiLEP and bipolar TUEP in the decrease in hemoglobin or sodium. During the 3-year postoperative follow-up, ongoing and significant improvements were found in both groups without any difference. Conclusions: Both DiLEP and bipolar TUEP can improve low urinary tract symptoms (LUTS) secondary to BPH in a comparable way with high efficacy. Compared with bipolar TUEP, DiLEP with a morcellator required a shorter operative time.
Analysis of clinical effect of transperitoneal approach and extraperitoneal approach for laparoscopic radical prostatectomy

Abstract:Objective To evaluate the efficiency of transperitoneal laparoscopic radical prostatectomy (T-LRP) and extraperitoneal laparoscopic radical prostatectomy (E-LRP). Methods Clinical data 130 patients with prostate cancer who underwent laparoscopic radical prostatectomy from January 2010 and December 2012 were retrospectively analyzed. Of these patients, 72 were done by transperitoneal style and 58 by extraperitoneal style. The BMI, preoperative PSA, preoperative Gleason grade, c-TNM, operating time, bleeding volume during operation, blood transfusions rate, postoperative pain, the recovery time of intestinal function, the duration of catheterization, hospitalization stay after operation, complication, p-TNM, continence rate were comparatively analyzed. Results Operations of two groups were all successfully performed. One case of T-LRP group and 1 case of E-LRP were converted to open surgery. The mean operating time of T-LRP group was longer than that of the E-LRP group, (120.6±92.6)min vs.(110.8±100.5)min ,and there was statistical difference between these two groups(P<0.001). The mean bleeding volume during operation of T-LRP group was more than that of the E-LRP group , (110.9±50.6)ml vs.(95.8±123.5)ml, there was a statistical difference between these two groups(P<0.05). The duration of catheterization of T-LRP group was longer than that of the E-LRP group, (13.42±4.64)days vs.(12.33±4.82)days, there was a statistical difference between these two groups (P<0.05). The hospitalization of T-LRP group was longer than that of the E-LRP group, (14.54±5.25)days vs.(11.63±5.82)days, there was a statistical difference between these two groups(P<0.05). 120 patients were followed up for 6~42months, and the median time was 32 months. In the T-LRP group and E-LRP group, the continence rate at the time of pulling the urine tube was 48.6%(35/72)vs34.5%(20/58); the six-month continence rate was 63.8%(46/72)vs. 89.6%(52/58), there was statistical difference between these two groups(P<0.05, P<0.001,separately).But there was no statistical difference between these two groups in the one-year continence rate (P=0.324). Conclusion E-LRP and T-LRP have the equivalent oncological and functional outcomes. ELRP is superior to T-LRP with respect to faster recovery of continence. E-LRP shows some advantage over T-LRP including the small of intraoperative trauma ,short operation time, fewer abdominal complications, quicker recovery, etc.
Bipolar Transurethral Enucleation and Resection of the Prostate Versus Bipolar Resection of the Prostate for Prostates Larger Than 60gr: A Retrospective Study at a Single Academic Tertiary Care Center

Abstract:OBJECTIVE:To evaluate the efficacy and safety of bipolar transurethral enucleation and resection of the prostate (B-TUERP) versus bipolar transurethral resection of the prostate (B-TURP) in the treatment of prostates larger than 60g.MATERIAL AND METHODS:Clinical data for 270 BPH patients who underwent B-TUERP and 204 patients who underwent B-TURP for BPH from May 2007 to May 2013 at our center were retrospectively analyzed. Outcome measures included operative time, decreased hemoglobin level, total prostate specific antigen (TPSA), International Prostate Symptom Score (IPSS), maximal urinary flow rate (Qmax), quality of life (QoL) score, post void residual urine volume (RUV), bladder irrigation duration, hospital stay, and the weight of resected prostatic tissue. Other measures included perioperative complications including transurethral resection syndrome (TURS), hyponatremia, blood transfusion, bleeding requiring surgery, postoperative acute urinary retention, urine incontinence and urinary sepsis. Patients in both groups were followed for two years.RESULTS:Compared with the B-TURP group, the B-TUERP group had shorter operative time, postoperative bladder irrigation duration and hospital stay, a greater amount of resected prostatic tissue, less postoperative hemoglobin decrease, better postoperative IPSS and Qmax, as well as lower incidences of hyponatremia, urinary sepsis, blood transfusion requirement, urine incontinence and reoperation (P<0.05 for all).CONCLUSIONS:B-TUERP is superior to B-TURP in the management of large volume BPH in terms of efficacy and safety, but this finding needs to be validated in further prospective, randomized, controlled studies.
Laparoscopic Simple Prostatectomy with Prostatic Urethra Preservation for Benign Prostatic Hyperplasia

Abstract:OBJECTIVE:To explore the feasibility and superiority of retropubic extraperitoneal laparoscopic simple prostatectomy with prostatic urethra preservation to treat large volume benign prostatic hyperplasia(BPH) .METHODS:From January 2006 to August 2009, laparoscopic simple prostatectomy with prostatic urethra preservation was performed in 45 patients with symptomatic BPH,and the age of patients was 70.5+/-7.2 (range 47 to 83) years old. The transrectal ultrasound (TRUS) revealed BPH and calculated prostatic gland weight was 126.1+/-52.4 (range 62 to 365) g. There were 3 cases presented with bladder calculus and 1 case presented with bladder diverticulum. The technique included retropubic extraperitoneal space produced by balloon dilation, five trocars in a reverted U shape placed,transverse prostatic capsular incision made, subcapsular plane developed, prostatic adenoma removed while prostatic urethra preserved as well as prostatic capsule sutured. Demographic, perioperative and outcome data were recorded.RESULTS:No patient required conversion to open surgery. The mean operative time was 123.9+/-51.3 (range 37 to 270) minutes and the estimated blood loss was 230.6+/-194.5 (range 50 to 800) mL. Blood transfusion was not necessary in this group of patients. Bladder irrigation was not needed except for the initial 2 cases and the average Foley catheter duration was 7.5+/-3.5 (range 2 to 14) days. Significant improvement was noted in the maximum flow rate, the International Prostate Score Symptoms (IPSS) and the quality of life questionnaires (QOL) three months after surgery. The erectile function was preserved in all patients who were potent before surgery and the ejaculation maintained antegrade. No urinary incontinence was reported by patients.CONCLUSION:Laparoscopic simple prostatectomy with prostatic urethra preservation for large benign prostatic hyperplasia is feasible and reproducible. Postoperative bladder irrigation can be avoided and antegrade ejaculation is preserved. The patients have a shorter hospital stay and early return to normal activity.
Modified Transperitoneal Laparoscopic Radical Prostatectomy:Technique and Clinical Outcomes

Abstract:OBJECTIVE:To evaluate the technique and clinical outcomes of modified transperitoneal laparoscopic radical prostatectomy.METHODS:A total of 105 patients received the operation with age ranging from 51 to 73 years from January 2008 to June 2010. Mean level of serum prostate specific antigen was 13.6 µg/L and mean prostatic volume was 45 ml. Pathological studies of biopsy confirmed the prostate carcinoma with Gleason score 6-8. Radionuclide bone scan revealed no metastasis. Based on previously retroperitoneal radical prostatectomy, modified technique was applied involving surgical approach, bladder neck dissection and vesicourethral anastomosis.RESULTS:Mean operative time was 93 min (65 - 150 min). Intraoperative blood loss was 115 ml (50 - 400 ml). No complication of bowl injury occurred. Positive surgical margin was present in 24 patients. Normal continence were seen in 64 patients after catheter removed. Recovery of incontinence within 3 months was seen in 33 patients and 3 to 12 months in 5 patients respectively. Three patients with incontinence were still in the follow-up.CONCLUSIONS:Transperitoneal laparoscopic radical prostatectomy provides large working space and clear anatomic exposure. Higher efficiency and lower complication rate are obtained through modified laparoscopic technique involving seminal vesicle isolation, bladder neck dissection and vesicourethral anastomosis.
An analog closed-loop SC accelerometer interface for a capacitive high-Q sensor element

Abstract:This paper presents an accelerometer interface circuit using a vacuum packaged MEMS sensor element. In order to damp the sensor element, increase the linearity and improve the input range of the sensor, a closed-loop analog SC interface with a PD-controller is used. The theoretical input range of sensor is ±1.4 g while measured range of the sensor is from −0.7 g to +1.4 g. The interface achieves maximum non-linearity of 0.67% over temperature range of −40 to +85 °C. The sensor has bias stability of 11 µg and the noise density of 2 µg√Hz. The interface has been implemented in a 0.35 µm CMOS process with a total active area of 1.3 mm2, and it consumes 2.4 mA from a 3.6 V supply.

Abstract:Objective:The long-term follow-up results of patients with locally advanced prostate cancer (LAPC) treated with laparoscopic extensive pelvic lymph node dissection combined with radical prostatectomy (ePLND+LRP) are still rare. This study reports 246 LAPC patients treated with ePLND+LRP from 2006 to 2020 in the Third Affiliated Hospital of Sun Yat-sen University.Methods:From October 2006 to October 2020, 246 LAPC patients with ePLND+LRP were enrolled in this study. The operation time, blood loss, duration of hospital stay and complications, oncology results, and adjuvant treatments occurred within 30 days after surgery were collected.Results:The median age was 70(65-74) years; the median PSA was 24(10-60) ng/ml. The median operation time, blood loss, and hospital stay were 217 minutes, 170 ml, and 7 days, respectively. In pathological staging, there were 28 cases (11.4%) in pT2, 117 cases (47.6%) in pT3a, 92 cases (37.4%) in pT3b, and 9 cases (3.7%) in pT4. The median number of lymph nodes dissected was 21(13-27), and the median number of positive lymph nodes was 2(1-3). 48(19.5%) and 31(12.6%) patients had positive lymph nodes and positive surgical margins. Thirty-eight(15.4%) patients experienced complications, of which 17 cases (6.9%) were classified as Clavien I, 19 cases (7.7%) were classified as Clavien II, and 2 cases (0.8%) were classified as Clavien III. The median follow-up time was 127.8 months, during the follow-up period, 23(9.3%), 219(89.0%), 59(24.0%), and 7(2.8%) patients received adjuvant radiotherapy (RT), hormone therapy (HT), and salvage lymph node dissection (sLND), salvage local treatment. The 10-year BCR-free survival and metastasis-free survival (MFS) rates were 58.5%(144/246) and 73.6%(181/246), respectively. Cancer-specific survival (CSS) and overall survival (OS) were 91.6% and 85.3%, respectively. The rate of urinary control at one year after operation was 96.7%.Conclusion:The ePLND+LRP treatment of LAPC has fewer complications and a good tumor control effect. Abstract:n Russian and Western debates about developments in the post–Soviet world, the term “reforms” has become a kind of magic fetish. The mere mention of reforms, like a shamanistic incantation, unleashes a storm of passions across the spectrum of public opinion. However, the specific meanings of the term are often as murky and diverse as the interests and goals of those who invoke the word. Profound and substantive differences exist over what is meant by “Russia’s reforms”—differences between Russians and Westerners, as well as between various intellectual and political camps in Russia itself. Thus, in the view of most present-day Western observers, as well as old-style Westernizers inside Russia (including most of the orthodox Marxists in the early twentieth century), reform has been deterministically linked to the idea of modernization and more generally to a belief in the linear progress of civilization. In this conception of history, all nations are perceived as developing, perhaps at different speeds, in the direction of a single universal standard. One of the popularizations of this doctrine was the much advertised essay by Francis Fukuyama on “the end of history.”1 However, it is clear that in late Soviet and post–Soviet Russia the number of proponents of the optimistic view of modernization and a linear perception of history has been shrinking. By contrast, an increasing number of Russian historians and social scientists have embraced variations of the cyclical paradigm of change. The roots of this approach go back to Heraclitus and, in modern times, to Giambatista Vico, whose teaching about the corsi e ricorsi, the ebbs and flows of history, was the first in modern Western thought to challenge the doctrine of universal and irreversible historical progress. Although few serious scholars would interpret the historical cycles as mere repetition without development, establishing parallels between distant periods of Russian history has long been characteristic of Russians’ view of their past, present, and even future. Note the widespread use of such terms as “Bolshevism” and “revolution from above,” which were coined in the twentieth century but are used to describe earlier as well as recent periods. I

Abstract:In this study we evaluate the oncological and functional results of the largest cohort of patients in China treated by laparoscopic radical prostatectomy (LRP) and with at least 3 years of follow-up. 126 inconsecutive patients (range 56-78 years, median 62.5) who had an LRP were retrospectively analyzed. The mean prostate specific antigen level and Gleason score was 13.4 ng mL(-1) and 6.4, respectively. Twenty-seven patients had unilateral or bilateral nerve preservation and 29 had pelvic lymphadenectomy. Multivariate analysis was used to adjust for differences in clinical and pathological features when comparing the risk for biochemical progression-free survival (bPFS). Urinary continence was assessed by incontinence questionnaire and erectile function by the Sexual Health Inventory for Men score. The mean operative duration was 250 min and blood loss 354 mL. Five patients received blood transfusion and nine had complications, including rectal injury (two), ureteral injury (one), active bleeding (one), bladder neck stenosis (two), paralytic ileus (one), subcutaneous hematoma (one) and port-site hernia (one). The overall positive surgical margin rate was 20.6% and correlated with pathological stage and Gleason score respectively (P = 0.03, P < 0.001 respectively). All patients had >or= 3 years of follow-up (range 3-6.75 years, mean 4.6, median 4.75). At 3 years of follow-up, the overall survival rate was 100% and the bPFS was 81.0% in all patients; 124 patients (98.4%) were continent; 22 of 27 patients (81.5%) who underwent nerve preservation retained erectile function. Our series confirms that LRP is an effective, safe and precise technique at Chinese institution.
[Transurethral columnar balloon dilation of the prostate for benign prostatic hyperplasia]

Abstract:Objective: To study the clinical effect of transurethral columnar balloon dilation of the prostate (TUCBDP) in the treatment of BPH and introduce the experience with the surgical procedure. Methods: We retrospectively analyzed the clinical data on 265 cases of BPH treated by TUCBDP from August 2016 to August 2019. Results: Operations were successfully completed in all the cases, with the mean operation time of (24.67 ± 7.6) min and the average intraoperative blood loss of (26.5 ± 21.4) ml, and all the patients had urinary patency after removal of the catheter. Follow-up examinations every 3 months after surgery showed significant improvement over the baseline in IPSS, quality of life score (QOL), maximum urinary flow rate (Qmax), postvoid residual urine (PVR) and other indicators (P < 0.05). Forty-five of the patients with sexual life exhibited no significant difference from the baseline in IIEF-5 and erectile hardness scale (EHS) scores (P > 0.05). Postoperative complications were observed in 53 cases (20%), including 28 cases of transient urinary incontinence (10.56%), 3 cases of hemorrhage (1.13%), 11 cases of urinary tract infection (4.15%), 1 case of urethral stricture (0.37%), and 8 cases of acute urinary retention (3.01%), which were all improved after regular treatment, with no occurrence of true urinary incontinence. Retrograde ejaculation occurred in 2 (4.45%) of the 45 patients with sexual life. Conclusions: Transurethral columnar balloon dilation of the prostate, with the advantages of short operation time and less intraoperative bleeding, has a significant short-term clinical effect in the treatment of BPH, particularly suitable for the elderly and those who want to retain the sexual function. Intraoperative localization of the protrusion may significantly influence the outcome of surgery, which deserves strengthened studies. Special attention should be paid to the incidence of postoperative transient urinary incontinence.

Abstract:Objective To evaluate the effectiveness of extraperitoneal laparoscopic radical prostatectomy. Methods A total of 53 patients diagnosed with localized prostate carcinoma were admitted from February 2003 to December 2006, whose serum PSA ranged from 7.5-47ng/ml (mean 14ng/ml). The volume of the prostate ranged from 35-75ml (mean 52ml). Biopsy was performed before operation and the pathological results revealed prostate carcinoma with Gleason sore no more than 7. CT, MR and ECT revealed no lymph node involvement or bone metastasis. The procedures were performed with antegrade techniques. Pelvic lymphadectomy was performed in 12 cases and nerve-sparing technique was performed in 5 cases. Results The operation duration ranged from 110-270 min (mean 190 min). Intraoperative blood loss was 120-1200 ml (mean 320 ml). Incontinence occurred in 12 cases in the early stage and recovered to normal within 3 months. Positive margin occurred in 4 cases. There was no complication of urethra stricture or incontinence during 3-30 months of follow-up. No positive lymph nodes were involved in 12 cases with pelvic lymphadectomy. Of the 5 patients who received nerve-sparing prostatectomy, 3 had normal erectile function during follow-up. Conclusions Extraperitoneal laparoscopic radical prostatectomy is a safe, effective and efficient surgical procedure with minimal invasion, less morbidity and rapid recovery. Laparoscopic radical prostatectomy is emerging as an alternative to open radical prostatectomy.

Abstract:BACKGROUND:Transurethral resection of prostate (TURP) has been considered as the standard treatment for benign prostatic hyperplasia (BPH). However, issues that have not yet been overcome for TURP include bleeding and absorption of irrigation fluid. Thus, novel improvement of the surgery is necessary. This study aimed to evaluate the efficacy and safety of bipolar plasma vaporization of the prostate (BPVP) with "button-type" electrode against standard TURP for BPH.METHODS:From January 2009 to January 2012, 30 patients who scheduled for surgical treatment of BPH surgical treatment were enrolled in the trial with provided consent for the study. Patients were prospectively randomized 1:1 to undergo either BPVP or TURP. Participants were blinded to the randomization scheme. All cases were assessed preoperatively and followed at 1, 3, and 6 months postoperatively by indwelling catheter time, blood loss, hospital stays, International Prostate Symptom Score (IPSS), quality of life (QOL), and Qmax.RESULTS:BPVP was significantly superior to TURP in terms of indwelling catheter time ((4.1 ± 4.1) days vs. (6.8 ± 6.8) days, P = 0.000), blood loss ((64.7 ± 103.8) ml vs. (254.7 ± 325.4) ml, P = 0.040), hospital stay ((8.7 ± 1.0) days vs. (11.7 ± 1.5) days, P = 0.000), IPSS ((4.2 ± 8.0) vs. (9.3 ± 3.7), P = 0.049), QOL ((1.5 ± 0.8) vs. (2.6 ± 1.4), P = 0.027), Qmax ((16.3 ± 5.7) ml/s vs. (12.5 ± 3.1) ml/s, P = 0.038), hemoglobin ((130.7 ± 9.4) g/L vs. ((122.1 ± 11.9) g/L, P = 0.047), Na(+) level ((138.6 ± 2.1) mmol/L vs. ((137.2 ± 2.0) mmol/L, P = 0.046) and operation time ((39.0 ± 15.5) minutes vs. ((69.3 ± 24.8) minutes, P = 0.004). And there were no statistical differences between BPVP group and TURP group in preoperatively assessment: patient's age ((70.9 ± 7.1) years vs. (71.9 ± 6.1) years, P = 0.736), IPSS ((24.6 ± 4.7) vs. (27.3 ± 5.9), P = 0.100), QOL ((5.1 ± 0.8) vs. (5.1 ± 1.0), P = 0.940), Qmax ((4.4 ± 2.7) ml/s vs. (5.3 ± 2.6) ml/s, P = 0.314), hemoglobin ((137.4 ± 8.7) g/L vs. (139.2 ± 10.4) g/L, P = 0.623), Na(+) level ((140.5 ± 1.8) mmol/L vs. (141.3 ± 1.4) mmol/L, P = 0.192) and prostate volume ((59.0 ± 17.4) ml vs. (70.1 ± 28.8) ml, P = 0.276).CONCLUSIONS:Compared with TURP, BPVP with "button-type" electrode shows superior efficacy and safety. Therefore, BPVP with "button-type" electrode represents a valuable endoscopic treatment alternative for BPH patients.

Abstract:Objective To investigate the effect and techincal feasibility of laparoscopic radical prostatectomy (LRP) for the treatment of localized prostate cancer.Methods LRP was performed in 126 consecutive patients with localized prostate cancer by the same surgeon at our Institute between October 2000 and May 2004.The average age of the patients was (62.51±6.7)years,and the average preoperative PSA level was (13.36±10.14) ml.The distribution of patients with respect to clinical stage was:17.46%(22/126) of all patients had stage T_(1c),48.41%(61/126) had stage T_(2a),23.81%(30/ 126) had stage T_(2b),10.32% (13/126) had stage T_(2c) disease.In terms of Gleason score evaluation,2-6 points,7 points,and 8-10 points comprised 63.49%,20.63% and 15.87% of the patients, respectively.Results LRP was successfully performed in all 126 cases.The mean operative duration was 250.28min (range,110-660 min).Average blood loss during surgery was 354.05 ml (range,150- 1100 ml),and one case of severe bleeding occurred.No thromboses in the lower limb veins or other serious complications of the heart,brain,and lung were found.The average postoperative positive margin rate was 20.63%,and it was 5.26%(3/57),26.92% (7/26),32.14% (9/28),44.44% (4/9),and 50.00% (3/6),in PT_(2a),PT_(2b),PT_(2c),PT_(3a),and PT_(3b),respectively.M1 126 patients were followed up (55.69+11.81) months on average (range,36-81).The five-year biochemical recurrence-free rate was 71.0%,and it was 75% in PT_2 and 43.0% in PT3,respectively.The PSA level decreased to<0.2 ng/ ml within six months in all patients.The biochemical recurrence rate was 20.63 % (26/126) at a mean of 38.19 months (range,12-60).Ninety-nine patients (78.6%) had satisfactory urinary control after catheter removed.At six months,92.6% of the patients were continent,at one year the continent rate was 97.8%.A11 patients had satisfactory continence at 3-year follow-up.Two patients needed pads when abdominal pressure increased,but they did not required more than 1 pad/24h.Among the patients who had normal erectile function prior to surgery,51.85% (14/27) had satisfactory penile erection twelve months after nerve-sparing radical prostatectomy.Conclusions Our mid-term data is consistent with other published reports,showing that the LRP technique is also safe and effective in China.It can offer equivalent oncological and functional results compared with open radical prostatectomy.
