Abstract:There has been increasing interest in the use of hindfoot tibiotalocalcaneal (TTC) nails to treat ankle and distal tibia fractures in select patient populations who are at increased risk for soft tissue complications after open reduction and internal fixation with traditional plate and screw constructs. We describe a technique which uses a retrograde femoral nail as a custom length TTC nail. By using a simple modification of the insertion jig, we are able to achieve safe screw trajectories that allow for robust distal interlocking fixation. Review of implantation in multiple cadaveric specimens demonstrates safe placement of distal screw fixation in the calcaneus without risking injury to important neurovascular structures. Because of the 2-cm incremental length options of this particular device, we are able to achieve supraisthmal fixation in the tibia which may lessen the risk for fracture that may be more likely to occur at the tip of a short TTC nail option. Furthermore, a custom length TTC nail is more costly and also requires advanced notice to acquire for the case; retrograde femoral nails are readily stocked and accessible at our level 1 trauma center. This TTC technique offers anatomic restoration while also offering convenience, instrument familiarity, cost savings, and increased patient safety.

Abstract:Objective: A biomechanical comparative analysis was conducted to evaluate the retrograde tibial nailing (RTN) and distal tibia plate techniques for the treatment of distal tibia fractures. Methods: Fourteen fresh adult tibia specimens were selected, consisting of seven males and seven females aged 34â€“55Â years. The specimens were randomly divided into two groups (Group A and Group B) using a numerical table method, with seven specimens in each group. Group A underwent internal fixation of distal tibial fractures using RTN, while Group B received internal fixation using a plate. The axial compression properties of the specimens were tested in the neutral positions under pressures of 100, 200, 300, 400, and 500Â N. Additionally, the torsional resistance of the two groups was assessed by subjecting the specimens to torques of 1.0, 2.0, 3.0, 4.0, and 5.0Â NÂ m. Results: At pressures of 400 and 500Â N, the axial compression displacement in Group A (1.11 Â± 0.06, 1.24 Â± 0.05) mm was significantly smaller than that in Group B (1.21 Â± 0.08, 1.37 Â± 0.11) mm ( p = 0.023, 0.019). Moreover, at a pressure of 500Â N, the axial compression stiffness in Group A (389.24 Â± 17.79) N/mm was significantly higher than that of the control group (362.37 Â± 14.44) N/mm ( p = 0.010). When subjected to torques of 4 and 5Â NÂ m, the torsion angle in Group A (2.97Â° Â± 0.23Â°, 3.41Â° Â± 0.17Â°) was significantly smaller compared to Group B (3.31Â° Â± 0.28Â°, 3.76Â° Â± 0.20Â°) ( p = 0.035, 0.004). Furthermore, at a torque of 5Â NÂ m, the torsional stiffness in Group A (1.48 Â± 0.07) NÂ m/Â° was significantly higher than that in Group B (1.36 Â± 0.06) NÂ·m/Â° ( p = 0.003). Conclusion: The results obtained from the study demonstrate that the biomechanical performance of RTN outperforms that of the distal tibial plate, providing valuable biomechanical data to support the clinical implementation of RTN.

Abstract:In factures of the distal tibia with simple articular extension, the optimal surgical treatment remains debatable. In clinical practice, minimally invasive plate osteosynthesis and intramedullary nailing are both routinely performed. Comparative biomechanical studies of different types of osteosynthesis of intraarticular distal tibial fractures are missing due to the lack of an established model. The goal of this study was first to establish a biomechanical model and second to investigate, which are the biomechanical advantages of angle-stable plate osteosynthesis and intramedullary nailing of distal intraarticular tibial fractures. Seven 4(th) generation biomechanical composite tibiae featuring an AO 43-C2 type fracture were implanted with either osteosynthesis technique. After primary lag screw fixation, 4-hole Medial Distal Tibial Plate (MDTP) with triple proximal and quadruple distal screws or intramedullary nailing with double proximal and triple 4.0mm distal interlocking were implanted. The stiffness of the implant-bone constructs and interfragmentary movement were measured under non-destructive axial compression (350 and 600 N) and torsion (1.5 and 3Nm). Destructive axial compression testing was conducted with a maximal load of up to 1,200 N. No overall superior biomechanical results can be proclaimed for either implant type. Intramedullary nailing displays statistically superior results for axial loading in comparison to the MDTP. Torsional loading resulted in non-statistically significant differences for the two-implant types with higher stability in the MDTP group. From a biomechanical view, the load sharing intramedullary nail might be more forgiving and allow for earlier weight bearing in patients with limited compliance.

ObjectiveTo evaluate the effectiveness of retrograde tibial intramedullary nail (RTN) in addressing nonunion in the distal one-fourth of the tibia.MethodsThis retrospective study included consecutive patients who were treated with RTN for nonunion in the distal one-fourth of the tibia between December 2020 and August 2023. Data regarding age, sex, injury mechanism, fracture type, initial fixation method, nonunion duration and type, risk factors, surgical duration, hospital stay, time to bone union, ankle function at final follow-up, and any complications were extracted from hospital records and analysed.ResultsFive patients in total were included, with previous treatments comprising locking plates and/or external fixation. The mean duration of RTN surgery was 94.0 ± 13.7 min, and mean duration of hospital stay was 9.8 ± 1.9 days. Patients were monitored for 10–18 months post RTN, achieving complete bone healing within a mean of 4.8 months. At the latest follow-up, the mean American Orthopedic Foot and Ankle Society (AOFAS) score was 84.4 ± 6.8 (range, 77–95). No complications, such as infection, reoperation, implant issues, rotational deformity, or shortening were reported.ConclusionRTN emerges as a dependable, minimally invasive, and safe treatment modality for managing nonunion in the distal one-fourth of the tibia.

Abstract:Aim: Distal tibial injuries combining bone loss, articular destruction and infection can be treated through distraction osteogenesis combined with ankle fusion. Bone transport is not without complications. This study investigates our preliminary results using a retrograde prefabricated gentamicin-coated nail (ETN PROtect®) to treat complications after infected bone defects of the distal tibial were managed by ankle arthrodesis and distraction osteogenesis.Materials and methods: This is a retrospective case series study. All consecutive patients with bone transport complications after ankle arthrodesis and distraction osteogenesis who were subsequently operated on using a retrograde ETN PROtect® nail were analysed. The cases occurred between 2017 and 2020. The primary objective was to report on the resolution of the clinical problem and the risk of deep infection after nail implantation.Results: Five patients have included: two docking site non-unions, two regenerated bone fractures and one hypotrophic regenerated bone. These complications were resolved in all patients (5/5, 100%). A painless, stable and plantigrade ankle arthrodesis was achieved in all cases. No patient developed a local infection or required nail removal (mean follow-up: 35.2 months). The mean LEFS score was 46.8 ± 13.8 and the mean knee ROM was 112 ± 12.7°. All patients tolerated full weight-bearing. All patients were very satisfied with the procedure (mean SAPS score was 93.8 points).Conclusion: The staged retrograde nailing technique using the ETN PROtect® nail may represent an effective and safe treatment for bone transport complications in high-infection-risk patients. Furthermore, the technique allows simultaneous achievement of ankle arthrodesis. The patients had good functional outcomes and were satisfied with the procedure.Clinical significance: This strategy of using retrograde gentamicin-coated tibial nails offers a solution to resolve bone transport complications while simultaneously achieving functional ankle arthrodesis.How to cite this article: Pujol O, Vicente M, Castellanos S, et al. Abstract:IntroductionWith development in the techniques of reduction and fixation, there has been a controversy in comparison between intramedullary nailing (IMN) and plating for the treatment of distal tibial metaphyseal fracture (DTF). The study aimed to investigate: (1) which fixation, IMN or plating, was better in the clinical outcomes and in the complications for the treatment of DTF and (2) which modifying variables affected the comparative results between the two modalities.MethodsPubMed, EMBASE, OVID, Scopus, ISI Web of Science, the Cochrane Library, Google Scholar and specific orthopaedic journals were searched from inception to July 2013, using the search strategy of '('Fracture Fixation, Intramedullary' [MeSH]) AND ('Tibial Fractures' [MeSH]) AND (plate OR plating)'. All prospective and retrospective controlled trials comparing function, pain, bone union and complications between IMN and plating for DTF were identified. Our analysis had no limitation of the language or the publication year. The primary outcome measurements were complication rate, union time, operation time and hospital stays, while the secondary outcome measurements were functional score and pain score.ResultFourteen of 6620 studies with 842 patients were included. IMN was probably preferential to plating for DTF given its higher functional score (p = 0.01), lower risk of infection (p = 0.02) and comparable pain score (p = 0.33), total complication rate (p = 0.53) and time to union (p = 0.86). However, plating had a lower malunion rate than IMN (p < 0.0001). All the results were based on the Grading of Recommendations Assessment, Development and Evaluation (GRADE) evidence of moderate quality.ConclusionsWith a satisfying alignment obtained, IMN may be preferential to plating for fixation of DTF with better function and lower risk of infection. However, IMN showed higher malunion rate for fixation of DTF. With the biases in our meta-analysis, it will ultimately require a rigorous and adequately powered randomised controlled trial (RCT) to prove.Level of evidenceLevel III, therapeutic study (systematic review).
Early complications of a novel retrograde intramedullary femoral nail in the treatment of femur fractures

Abstract:OBJECTIVES: To determine the early implant failure rate of a novel retrograde intramedullary femoral nail METHODS: Design : Retrospective matched case control Setting: Academic level 1 trauma center Patients Selection Criteria: Patients 18 years and older with an acute AO/OTA 32-A, 32-B, 32-C, and 33-A fracture or periprosthetic distal femur fracture from April 2018 to April 2022 were included in the study. The two interventions compared were the Synthes Expert Retrograde/antegrade femoral nail (RAFN or control implant) versus the next generation RFN-Advanced retrograde femoral nail (RFNA or experimental implant) (Synthes, West Chester, PA). Outcome Measures and Comparisons : Early implanted-related complications between the experimental and control implants were assessed including locking screw backout, screw breakage, intramedullary nail failure, need for secondary surgery, and loss of fracture reduction RESULTS: 314 patients were identified with a mean age of 31.0 years and 62.4% of the patients being male. Open fractures occurred in 32.5% of patients with 3.8% of injuries being distal femur periprosthetic fractures. 56 patients in the experimental group and 258 patients in the control group. Mean follow-up was 46.8 weeks for the control cohort and 21.0 weeks for the experimental cohort. Distal interlocking screw backout occurred in 23.2% [13 of 56] of experimental group patients and 1.9% [5 of 258] of control group patients (p<0.0001). Initial diagnosis of interlocking screw back out occurred at an average of 3.2 weeks post-operatively (range, 2-12 weeks). 54% of patients that sustained screw back-out underwent a secondary operation to remove the symptomatic screws (12.5% of all patients treated with the experimental implant required an unplanned secondary operation due to screw backout). A logistic regression model was used to predict screw back out and found the experimental implant group was 4.3 times as likely to experience distal locking screw backout compared to the control group (p=0.01). CONCLUSIONS: The RFNA implant was associated with a significantly higher rate of screw back-out with a substantial number of unplanned secondary surgeries compared to the previous generation of this implant. Abstract:Purpose: The objective of this study was to describe the use of retrograde gentamicin-coated tibial intramedullary nail (ETN PROtect™) in patients with tibial defects who required a tibiotalocalcaneal arthrodesis (TTC). Methods: Consecutive series case review of seven men treated with TTC using retrograde PROtect™ between January 2018 and December 2023. The main outcomes evaluated were fracture union, complications, and the health-related quality of life using the EuroQol five-dimension three-level questionnaire (EQ-5D-3L). Results: The mean age was 45.3 ± 8.0 years. Six patients had a clinical history of chronic osteomyelitis, and one case underwent TTC for congenital pseudoarthrosis. Fracture union was achieved in 5 of 7 patients between 4 and 11 months after surgery. Three patients developed complications; two patients had fistulas, and one had persistent pain. At the end of the follow-up, a median of 70 points (interquartile range: 60 to 90) on the EQ-5D-3L was reported. No complications directly attributed to the use of the PROtect™ were reported. Conclusion: TTC with retrograde PROtect™ is a prophylactic treatment option in patients with tibial defects treated with external fixation requiring a tibiotalar and subtalar arthrodesis. This novel use of PROtect™ allows simultaneous fixation of the tibiotalocalcaneal joint and protection of the regenerated bone, facilitating earlier rehabilitation in patients at high risk for postoperative infections.

Abstract:BackgroundTibiotalocalcaneal arthrodesis is commonly considered a salvage option for a variety of hindfoot disorders involving the ankle and subtalar joint mostly with concomitant deformity. Retrograde interlocking nailing represents one of the biomechanically most stable fixation modes after reduction of hindfoot geometry. The considerable complications, non-union and revision rates at a moderate outcome reported even with modern retrograde nails underscore the idea that the development of an optimized retrograde may be warranted.MethodsA novel circular shaped nail was designed with a perfect fit to the reamed canal and the implant geometry which respects physiologic hindfoot anatomy and alignment. The present clinical series reports the experience and the clinical and radiographic outcomes after application of the implant in the first 30 consecutive patients. Assessments included validated versions of the AOFAS Ankle/Hindfoot Score and the Foot Function Index.ResultsAnatomical alignment was achieved and maintained in 29/30 cases with a single case of later loss of reduction due to a technical error which led to surgical reintervention two months after the index procedure. The overall union rate reached 93%. Two non-unions (one ankle, one subtalar joint) were observed without necessitating further surgery. Three superficial surgical site infections were registered which made a local flap coverage necessary in two patients due to local skin break-down. No deep infection occurred. There were 2 implant removals, one was not related to hindfoot nailing. At the time of follow-up, the AOFAS Ankle/Hindfoot Score was 57 (median) from a maximum of 86 points. The self-assessment via the Foot Function Index improved from preoperative 155 points to 62.5 postoperatively (median values, p < 0.001, Wilcoxon test).ConclusionOn the basis of our results, which saw a high rate of successful fusion and patient satisfaction and where the hindfoot reduction was maintained until definite healing in the vast majority of cases, the novel circular arc nail represents a viable and safe option for tibiotalocalcaneal arthrodesis with a low complication rate.
Retrograde talon intramedullary nails versus distal locking plates in the management of extra-articular distal femoral shaft fractures

Abstract:Introduction: Distal femoral shaft fractures are characterized by their increasing incidence and complexity, presenting a significant challenge in management. The objective of this retrospective study was to compare the clinical and radiological results of patients with extraarticular distal third femoral shaft fractures treated using either retrograde Talon Distal Fix nail or a distal femur locking plate. Material and Method: The study comprised 40 patients aged >18 years who presented at our hospital with a distal third femoral shaft fracture between January 2017 and January 2023. The patients were divided into two groups: Group TDN, treated with retrograde Talon Distal Fix nailing (n=18), and Group DLP, treated with a distal locking plate (n=22). Demographic data, follow-up period, operating time, time to union, range of motion (ROM), mechanism of injury (traffic accident, fall from height, workplace accident, gunshot injury), fracture type, complications, and surgical method were retrospectively recorded. Clinical evaluation included deformity, knee ROM, pain, and the knee total score (KSS) for walking and knee stability. Results: The mean age of the patients was 48.03±12.31 (min-max: 23-69) years, and the mean follow-up time for all patients was 15.88±2.32 (12-21) months. The mean time to union was 25.55±1.86 (22-30) weeks. Delayed union and non-union rates were similar between the research groups (P=1.000, P=0.673, respectively). Union time (weeks) and mean ROM were not significantly different between the groups (P=0.881, P=0.892, respectively). The mean operation time of the TDF group (48.78±3.94 minutes) was significantly lower than that of the DLP group (62.45±3.33 minutes) (P<0.001). The mean blood loss values of the TDF group (267.5±32.4) were significantly lower than those of the DLP group (324.1±20.2) (P<0.001). Conclusion: This study demonstrated that both retrograde talon nails and locking plates provided satisfactory clinical and radiological results in the management of distal third femoral shaft fractures. Moreover, the retrograde talon nail offered the advantages of a shorter operating time and less intraoperative blood loss.

Abstract:Purpose: Conventional cephalomedullary nails (CMNs) are commonly employed for internal fixation in the treatment of reverse obliquity intertrochanteric (ROI) fractures. However, the limited effectiveness of conventional CMNs in addressing ROI fractures results in significant implant-related complications. To address challenges associated with internal fixation, a novel Proximal Femoral Bionic Nail (PFBN) has been developed. Methods: In this study, a finite element model was constructed using a normal femoral specimen, and biomechanical verification was conducted using the GOM non-contact optical strain measurement system. Four intramedullary fixation approaches—PFBN, Proximal Femoral Nail Antirotation InterTan nail (ITN), and Gamma nail (Gamma nail)—were employed to address three variations of ROI fractures (AO/OTA 31-A3). The biomechanical stability of the implant models was evaluated through the calculation of the von Mises stress contact pressure and displacement. Results: Compared to conventional CMNs, the PFBN group demonstrated a 9.36%–59.32% reduction in the maximum VMS at the implant. The A3.3 ROI fracture (75% bone density) was the most unstable type of fracture. In comparison to conventional CMNs, PFBN demonstrated more stable data, including VMS values (implant: 506.33 MPa, proximal fracture fragment: 34.41 MPa), contact pressure (13.28 MPa), and displacement (17.59 mm). Conclusion: Compared to the PFNA, ITN, and GN, the PFBN exhibits improvements in stress concentration, stress conduction, and overall model stability in ROI fractures. The double triangle structure aligns better with the tissue structure and biomechanical properties of the proximal femur. Consequently, the PFBN has significant potential as a new fixation strategy for the clinical treatment of ROI fractures.

Novel concepts of reduction like medial cortical support classification and antero-posterior reduction (Subtype classification system) refine the reduction strategies. Despite reporting of the reduction analysis, there has been continuing number of implant failures following proximal femoral nail implantation. Complications like 'Z' effect and reverse 'Z' effect are very well reported with the 2 screw Recon - Proximal femur nails. Analysis of failure and complications have constantly upgraded the designs of proximal femur nails. Design of the new generation intramedullary implants for treatment of intertrochanteric fractures like Gamma nail, PFNA-II (Proximal Femoral Nail Anterotation), Zimmer Natural Nail, Trigen Intertan nail and TFN – Advanced (Trochanteric Femoral Nail Advanced) are described in this review. While the new implant designs offer advancements, fundamental principles of reduction remains crucial for successful outcomes. Design rationale and instrumentation of each implant must be known at the time of application along with techniques of maintaining the reduction for a successful outcome.

Abstract:Objective To discuss the curative effect of AO expert tibial intramedullary nailing(ETN)for minimally invasive treatment of distal metaphyseal tibial fractures.Methods Totally 18 cases of distal tibial fractures located within 50mm of the ankle joint,who were admitted from Nov.2007 to Nov.2009,were retrospectively analyzed.All cases were used ETN to apply minimally invasive fixation under fluoroscopy.Results All patients had been followed-up for 5-22 months,averaging 12 months.All patients were united within an average of 4.5 months(range,3 to 12 months).According to the Johner-Wruhs criteria: excellent in 17 cases,good in 1 case.Complications such as infection,nail breakage,malunion and bone nonunion were not observed.Conclusion ETN has advantages of reasonable design,simple operation,stable fixation,minimally invasive trauma,fewer complications.It is a safe and effective method for the treatment of distal metaphyseal tibial fractures.
Systematic Review of the Literature: Comparison of Open and Minimal Access Surgery (Thoracoscopic Repair) of Esophageal Atresia with Tracheo-Esophageal Fistula (EA-TEF)

Abstract:Surgical correction of esophageal atresia with tracheo-esophageal fistula (EA-TEF) has been performed since 1943 (Cameron Haight) via postero-lateral thoracotomy using an extrapleural approach in most cases. This procedure can be considered as the standard treatment of EA. The pitfalls of the operation, the incidence of complications and the outcomes, both short term and long term, have been analysed and reported by many pediatric surgeons around the world. Since 1999, minimal access surgery (MAS) has been practised for the correction of EA 1. The risk of complications and short term outcomes have been reported as equal to the open approach. MAS has been advocated because of a possibly reduced risk of impairment of shoulder function, and a possible reduction in occurrence of postoperative scoliosis. Next to that, it has been postulated the MAS repair might lead to a better cosmesis. Several advantages and disadvantages of both procedures have been described. The open approach is well standardized and is resorted to in difficult cases. Disadvantages of the open approach are the presence of a scar, possible chest wall deformities and rib fusion. The occurrence of scoliosis and possible shoulder function impairment has been related to the open approach as well. The thoracoscopic approach has the advantage of magnification of view. Next to that the chance on a postoperative scoliosis and impaired shoulder function may be reduced due to the small incisions which also might lead to better cosmesis. Technically, the thoracoscopic approach is more demanding than the open approach which has consequences for training and education. So far, it seems that there is no difference between open and MAS approach in the frequency of anastomotic leakages, strictures, recurrent fistulas, tracheomalacie or GERD. Abstract:The study compared the biomechanical performance of retrograde nail used to stabilize supracondylar fracture (three different levels) by means of finite element analysis. Three different nail lengths (200, 260, and 300 mm) of stainless steel and titanium nails were under consideration. Intact femur model was reconstructed from Digital Imaging and Communications in Medicine images of Thai cadaveric femur scanned by computed tomography spiral scanner, whereas geometry of retrograde nail was reconstructed with the data obtained from three-dimensional laser scanner. The retrograde nail was virtually attached to the femur before nodes and elements were generated for finite element model. The finite element models were analyzed in two stages, the early stage of fracture healing and the stage after fracture healing. The finding indicated that purchasing proximal locking screw in the bowing region of the femur may be at risk due to the high stresses at the implant and bone. There were no differences in stress level, elastic strain at a fracture gap, and bone stress between stainless steel and titanium implant. Since the intramedullary canal requires reaming to accommodate the retrograde nail, the length of retrograde nail should be as long as necessary. However, in case that the retrograde nail can be accommodated into the intramedullary canal without reaming, the longer retrograde nail can be used.

Abstract:Background: Biomechanical testing, intramedullary devices have proven advantageous over the extramedullary devices in the management of unstable intertrochanteric fractures. Reverse oblique type of intertrochanteric fractures are highly unstable and intramedullary nails are currently the method of internal fixation. The currently available nails seems to provide rotational, axial and angular stability, but biomechanical analysis of the strain pattern in the bone and implant in this fractures are lacking. The aim of this experimental study was to analyse the strain in three different long femoral nail-bone units under physiological loading when implanted in Saw bone model after creating a reverse oblique intertrochanteric fracture. Methods: A total of 12 sawbones were divided in to 4 equal groups. Group 1 was intact saw bones and were used as controls. Group 2, Group 3 and Group 4 were implanted with Depuy, Stryker and Synthes nails respectively after creating a reverse oblique intertrochanteric fracture. All the four groups were axially loaded with 100 N increments until physiological loads. The strain patterns were measured at the posteromedial cortex and the peak strains were extracted at partial weight bearing i.e. 500 N and full weight bearing physiological loads i.e. 1000 N. Results: There was no significant difference in peak strains among the groups at partial loads. However at 1000 N loads the peak strain in the DePuy nail-bone unit was significantly high compared to the other two nail-bone units and the controls. Conclusions: These results question the safety of immediate full weight bearing following surgery when treating the reverse oblique unstable fractures with DePuy intramedullary nails. A period of partial weight bearing following fixation of reverse oblique fractures would be wise when using DePuy nails .
