Biomechanical and Morphometric Evaluation of a Strontium-Incorporated Implant and Four Commercially Available Implants: A Study in Rabbits

Abstract:Purpose: Strontium has shown a positive effect on osseointegration in experiments. This study compared surface characterization and osseointegration of a strontium-incorporated implant with four commercial implants with different surface treatments. Materials and Methods: A strontium-oxide layer was created by hydrothermal treatment on the surface of the implant (SLA-Sr). Surface characterizations were observed using a scanning electron microscope, three-dimensional (3D) optical microscope, and x-ray energy-dispersive spectrometry. Implants of different surface treatments including resorbable blasting media (RBM), sandblasting with large grit and acid etching (SLA-1, SLA-2), sandblasting and thermal acid etching (STA), and SLA-Sr were implanted into the proximal tibiae and femoral condyles of rabbits. Biologic effects were evaluated by removal torque testing and histomorphometric analysis after 3, 6, and 12 weeks of implantation. Results: Nanostructures were observed on the surface of SLA-Sr and STA. Calcium (Ca) was detected on the surface of RBM. Sr was detected on the surface of SLA-Sr. SLA-1 and STA had greater surface roughness than SLA-2, SLA-Sr, and RBM (P < .05). In vivo, SLA-Sr achieved better removal torque value (RTV) than that of RBM and SLA-2 at 3 weeks (P < .05), as well as increased bone area ratio (BA%) in cortical bone compared with RBM at 3 weeks (P < .05). STA showed higher bone-to-implant contact ratio (BIC%) in cortical bone than RBM at 3 and 6 weeks (P < .05). Compared with RBM, SLA-1 had better RTV at 6 weeks and higher BIC% in cortical bone at 12 weeks (P < .05). Conclusion: In vivo, compared with SLA-2 and RBM, the implant with the strontium-oxide layer displayed slight advantages in new bone formation and osseointegration in the early healing stage. In the later osseointegration stage, the results of SLA-Sr were comparable with other implants.

Abstract:OBJECTIVES: The purpose of the present study was to investigate the osseointegration of microstructured zirconia implants in comparison with sandblasted and acid-etched (SLA) titanium implants in a biomechanical study.MATERIALS: Zirconia implants (4.1 mm in diameter, 10 mm in length) were produced using a new low pressure injection molding technique. After that the implants were acid-etched with hydrofluoric acid. Standard Ti-SLA implants of the exact same shape served as controls. Six months after extraction of incisors 2 and 3, 16 adult pigs received a total of 64 implants in the maxillae. After 4, 8, and 12 weeks the animals were sacrificed, and 59 implants could be analyzed to removal torque (RTQ) testing.RESULTS: The mean RTQ values for zirconia implants were 42.4 Ncm at 4 weeks, 69.6 Ncm at 8 weeks, and 69.3 Ncm at 12 weeks of healing, whereas RTQ values for the Ti-SLA implants were 42.1 Ncm, 75.0 Ncm, and 73.1 Ncm at corresponding time intervals. There is no statistical difference in RTQ values between Ti-SLA and zirconia implants at 8 weeks.CONCLUSIONS: Within the limits of the present study it was concluded that acid-etching of zirconia implants enhances bone apposition resulting in RTQ values which were equivalent to that of Ti-SLA.

Biomechanical and histomorphometric comparison between zirconia implants with varying surface textures and a titanium implant in the maxilla of miniature pigs

Abstract:Background: Mechanical properties and biocompatibility make zirconia ceramics suitable implant material. The characteristics of tooth-color like, the ability to be machined and the low plaque affinity make zirconia especially suitable as a dental implant material. The influence of surface modification on the osseointegration of this material has not been extensively investigated. Purpose: Long-term investigations with titanium implants have shown superior biomechanical results with the sandblasted acid-etched (SLA) surface, demonstrating a high bone-implant interaction. The objective of this study was to compare two different zirconia surface topographies biomechanically and histologically with the well-documented titanium SLA surface. Material and methods: Zirconia implants with either a machined (ZrO2m) or a sandblasted (rough, ZrO2r) surface were manufactured with the exact same cylindrical shape with a standard ITI thread configuration as the SLA titanium implants. The incisors 2 and 3 were removed from both sides of the maxillae of 13 adult miniature pigs and the tissues left to heal for 6 months. After this time period the animals received a total of 78 implants using a randomized scheme, with the titanium SLA implant used as an only individual reference. After healing periods of 4, 8, and 12 weeks 20, 24, and 25 implants, respectively, were subjected to removal torque tests (RTQ) as the main biomechanical analysis of the of the study. A fewer number was resected on bloc, embedded in methylmethacrylat and analyzed for their direct bone apposition under a light microscope. Results: Surface analysis revealed the highest surface roughness for the SLA-implant, followed by ZrO2r and ZrO2m. The turned ZrO2m implants showed statistically significant lower RTQ values than the other two implants types after 8 and 12 weeks, while the SLA implant showed significantly higher RTQs values than ZrO2r surface after 8 weeks. Differences in the bone apposition were observed in the histomorphometric analysis using light microscopy for all surfaces at any time point. Conclusion: The findings suggest that ZrO2r implants can achieve a higher stability in bone than ZrO2m implants. Roughening the turned zirconia implants enhances bone apposition and has a beneficial effect on the interfacial shear strength.
Osseointegration of zirconia implants with different surface characteristics: an evaluation in rabbits

Abstract:Purpose: Zirconia ceramics are a viable alternative to titanium for use as dental implants. However, the smooth surface of zirconia means that longer healing periods are needed to accomplish osseointegration compared to roughened titanium surfaces. Surface modifications can be used to increase the roughness of zirconia. The aim of this study was to assess histologically and compare the degree of early bone apposition around zirconia dental implants with sandblasted, sintered, or laser-modified surfaces to that seen around surface-modified titanium implants. Removal torque was also measured and compared. Materials and methods: Ninety-six implants--24 each of four types (sintered zirconia, laser-modified zirconia, sandblasted zirconia, and acid-etched titanium)--were placed in 48 New Zealand White female rabbits. One implant was inserted in each distal femur. Half of the specimens were harvested at 6 or 12 weeks and processed for light microscopic analysis; the area of bone-to-implant contact was measured morphometrically. The other half were evaluated for removal torque at 6 and 12 weeks. Results: No statistically significant differences existed in bone apposition between the different surfaces at either time point. Differences in removal torque were significantly different between titanium and sandblasted zirconia and between sintered zirconia and sandblasted zirconia, with the first mentioned demonstrating a higher torque value at 6 weeks. At 12 weeks, the only significant difference in removal torque was between titanium and sandblasted zirconia, with titanium demonstrating the higher value. Conclusion: Comparable rates of bone apposition in the zirconia and titanium implant surfaces at 6 and 12 weeks of healing were observed. Removal torque values were similar for all implants with a roughened surface.
Zirconia Implants with Laser Surface Treatment: Peri-Implant Bone Response and Enhancement of Osseointegration

Abstract:The aim of this study was to prepare zirconia implants with laser-modified surfaces and to evaluate peri-implant tissue response and osseointegration in an animal study. The experimental zirconia implants received one of the following surface treatments and were placed in the tibiae of SD rats: vertical irradiation with a fiber laser (vertical fiber laser), horizontal irradiation with a fiber laser (horizontal fiber laser), vertical irradiation with a Neodymium-doped yttrium orthovanadate (YVO4) laser (vertical YVO4 laser), and horizontal irradiation with a YVO4 laser (horizontal YVO4 laser). The control implants were smooth surfaced. Tibiae with implant bodies were collected 28 days after implant placement, and removal torque values were measured. Tissue sections were prepared for light microscopy, and the bone-implant contact (BIC) ratio and the peri-implant bone area (BA) were measured. The vertical fiber-laser implants had a mean BIC that was significantly higher than other implants. The mean BIC of the vertical fiber-laser implants was approximately 4.2 folds of the value of the control implants on the cortical bone side and approximately 2.7 folds of the value of the control implants on the bone marrow side. The mean BA was significantly higher in the vertical YVO4-laser implants. The vertical fiber-laser implants had a mean torque removal value that was approximately 2.4 folds of the value of the control implants and approximately double of the value of the vertical or horizontal YVO4-laser implants. The horizontal fiber-laser implants had a mean removal torque value that was approximately double of the value of the control implants and approximately 1.7 folds of the value of the vertical or horizontal YVO4-laser implants (p<0.05). Both types of lasers were useful in implant surface treatment to enhance osseointegration of zirconia implants.

Abstract:Abstract Background Titanium and titanium alloys are widely used for fabrication of dental implants. Since the material composition and the surface topography of a biomaterial play a fundamental role in osseointegration, various chemical and physical surface modifications have been developed to improve osseous healing. Zirconia-based implants were introduced into dental implantology as an altenative to titanium implants. Zirconia seems to be a suitable implant material because of its tooth-like colour, its mechanical properties and its biocompatibility. As the osseointegration of zirconia implants has not been extensively investigated, the aim of this study was to compare the osseous healing of zirconia implants with titanium implants which have a roughened surface but otherwise similar implant geometries. Methods Forty-eight zirconia and titanium implants were introduced into the tibia of 12 minipigs. After 1, 4 or 12 weeks, animals were sacrificed and specimens containing the implants were examined in terms of histological and ultrastructural techniques. Results Histological results showed direct bone contact on the zirconia and titanium surfaces. Bone implant contact as measured by histomorphometry was slightly better on titanium than on zirconia surfaces. However, a statistically significant difference between the two groups was not observed. Conclusion The results demonstrated that zirconia implants with modified surfaces result in an osseointegration which is comparable with that of titanium implants.

Abstract:Purpose: To assess different microstructured surfaces created by sandblasting and acid etching in an effort to optimize the osseointegration performance of dental zirconia implants with an optimized surface. Materials and Methods: Sixty CAD/CAM zirconia implants were divided into four groups. The control group had no surface treatment after sintering. The other groups had three different types of surface modifications: sandblasting; sandblasting and etching with hydrofluoric acid; and sandblasting and etching with an experimental hot etching solution composed of methanol, 37% hydrochloric acid, and ferric chloride, heated to 100 degrees C and applied for 60 minutes. Commercially available titanium implants with sandblasted and etched surfaces and identical dimensions were employed as a positive control. Surface micromorphologies of implants from the five groups were evaluated. The osseointegration performance of all the implants was assessed in adult New Zealand rabbits based on microcomputed tomography (micro-CT) and histologic analysis. Results: Sandblasting and acid etching with hot etching solution or hydrofluoric acid exhibited moderately rough surfaces with microstructures in both microscale and nanoscale. The sandblasting and etching with hydrofluoric acid group showed the highest surface roughness. Micro-CT revealed a significantly lower mean bone volume/total volume for the control group compared with the other four groups (P < .05). Among the groups, the sandblasting and hydrofluoric acid etching group was the highest, significantly higher than the titanium implant and sandblasting groups (P < .05). The sandblasting and etching with hot etching solution (P = .006) group also showed a significantly higher bone volume/total volume value than the titanium implant group. Histologic analysis revealed significantly higher bone-to-implant contact for implants with modified surfaces compared with a sintered surface (P < .05), and no significant difference was found with respect to the sandblasted and etched titanium implants. Conclusion: The microstructured surfaces created by sandblasting and acid etching show osseointegration comparable to that of commonly used titanium implants.
Early bone formation around immediately placed two-piece tissue-level zirconia implants with a modified surface: an experimental study in the miniature pig mandible

Abstract:Purpose: To histologically examine early bone formation around transmucosal implants and to evaluate the influence of surface characteristics on early peri-implant bone healing using a miniature pig model. For this, commercially available dental implants with a rough zirconia (YTZP) surface were compared to surface-modified Ti control implants at 4 and 8 weeks after placement. Methods: Immediately following the extraction of six mandibular premolars, 20 two-piece, tissue-level, screw-shaped YTZP implants (Patent™ Standard Zirconia Implant ø4.1 × 11 mm) with a modified rough blasted before sintering surface were inserted in four adult miniature pigs. In addition, four titanium (Ti) tissue-level implants (Straumann® Standard RN ø4.1 × 10 mm Roxolid®) with a moderate surface (SLActive®), one per animal, were placed as control implants. A histological analysis was performed on the hard tissues after 4 and 8 weeks of transmucosal healing. Results: The results show a high rate of osseointegration of the test YTZP dental implants at 4 and 8 weeks following insertion. At 4 weeks, a bone-to-implant contact ratio (BIC) of 73.7% (SD ± 16.8) for the test implants (n = 10) and 58.5% for the first control implant was achieved. The second control implant had to be excluded from analysis. At 8 weeks, a BIC of 82.4% (SD ± 16.9) for the test implants (n = 9) and 93.6% (SD ± 9.1) (n = 2) for the control implant was achieved. No statistical difference was observed comparing 4 and 8 weeks YTZP data (p = 0.126). Conclusions: The results indicate a predictable osseointegration of immediate zirconia implants with a modified YTZP implant surface and a high degree of BIC present at 4 weeks following insertion. After 8 weeks of healing both the zirconia implants and the Ti implants show a BIC indicating full osseointegration. Further studies involving a larger sample size with more time points are needed to confirm these results.
Relevant Aspects of Titanium and Zirconia Dental Implants for Their Fatigue and Osseointegration Behaviors

Abstract:Osseointegration capacity and good mechanical behavior are key to the success of the dental implant. In many investigations, comparisons of properties are made using different dental implant designs and therefore the results can be influenced by the macrodesign of the dental implant. In this work, studies were carried out with the same dental implant model using different roughness and different materials—commercially pure titanium (grade 4) and zirconia. For this purpose, 80 smooth passivated titanium (Ti), 80 smooth zirconia (ZrO2), and 80 rough passivated titanium (Ti-R) dental implants were used. The samples were characterized by their roughness, wettability, surface energy, residual stresses, and fatigue behavior. The implants were implanted in minipigs for 4 and 12 weeks. The animals were sacrificed, and histological studies were carried out to determine the osseointegration parameters for each of the implantation times. Ti and ZrO2 dental implants have very similar wettability and surface energy properties. However, the roughness causes a decrease in the hydrophilic character and a decrease of the total surface energy and especially the dispersive component, while the polar component is higher. Due to the compressive residual stresses of alumina sandblasting, the rough dental implant has the best fatigue behavior, followed by Ti and due to the lack of toughness and rapid crack propagation the ZrO2 implants have the worst fatigue behavior. The bone index contact (BIC) values for 4 weeks were around 25% for Ti, 32% for ZrO2, and 45% for Ti-R. After 12 weeks the Ti dental implants increased to 42%, for Ti, 43% for ZrO2, and an important increase to 76% was observed for Ti-R implants. In vivo results showed that the key factor that improves osseointegration is roughness. There was no significant difference between ZrO2 and Ti implants without sandblasting.

Surface modification of zirconia dental implants by laser texturing

Abstract:The aim of this work was to perform an integrative literature review on the influence of laser irradiation on zirconia implants to enhance surface topographic aspects and the biological response for osseointegration. An electronic search was carried out on the PubMed database using the following search terms: “zirconia” AND “laser” AND “surface modification” OR “surface treatment” AND “dental implants” OR “bone” OR “osteoblast” OR “osseointegration.” Of the identified articles, 12 studies were selected in this review. Results reported that the laser irradiation was capable of promoting changes on the zirconia surfaces regarding topographic aspects, roughness, and wettability. An increase in roughness was recorded at micro- and nano-scale and it resulted in an enhanced wettability and biological response. Also, adhesion, spreading, proliferation, and differentiation of osteogenic cells were also enhanced after laser irradiation mainly by using a femtosecond laser at 10nJ and 80 MHz. After 3 months of osseointegration, in vivo studies in dogs revealed a similar average percentage of bone-to-implant contact (BIC) on zirconia surfaces (around 47.9 ± 16%) when compared to standard titanium surfaces (61.73 ±16.27%), denoting that there is no significant difference between such different materials. The laser approach revealed several parameters that can be used for zirconia surface modification such as irradiation intensity, time, and frequency. Laser irradiation parameters can be optimized and well-controlled to reach desirable surface morphologic aspects and biological response concerning the osseointegration process.

Femtosecond laser microstructuring of zirconia dental implants

Abstract:This study evaluated the suitability of femtosecond laser for microtexturizing cylindrical zirconia dental implants surface. Sixty-six cylindrical zirconia implants were used and divided into three groups: Control group (with no laser modification), Group A (microgropored texture), and Group B (microgrooved texture). Scanning electron microscopy observation of microgeometries revealed minimal collateral damage of the original surface surrounding the treated areas. Optical interferometric profilometry showed that ultrafast laser ablation increased surface roughness (R(a), R(q), R(z), and R(t)) significantly for both textured patterns from 1.2 x to 6 x-fold when compared with the control group (p < 0.005). With regard to chemical composition, microanalysis revealed a significant decrease of the relative content of contaminants like carbon (Control 19.7% ± 0.8% > Group B 8.4% ± 0.42% > Group A 1.6% ± 0.35%) and aluminum (Control 4.3% ± 0.9% > Group B 2.3% ± 0.3% > Group A 1.16% ± 0.2%) in the laser-treated surfaces (p < 0.005). X-ray diffraction and Raman spectra analysis were carried out to investigate any change in the crystalline structure induced by laser processing. The original predominant tetragonal phase of zirconia was preserved, whereas the traces of monoclinic phase present in the treated surfaces were reduced (Control 4.32% > Group A 1.94% > Group B 1.72%) as the surfaces were processed with ultrashort laser pulses. We concluded that femtosecond laser microstructuring offers an interesting alternative to conventional surface treatments of zirconia implants as a result of its precision and minimal damage of the surrounding areas.

Peri-implantitis induction and resolution around zirconia versus titanium implants

Abstract:Background: This study compared titanium and zirconia implant ligature-induced peri-implant defect progression and response to regenerative surgical intervention. Methods: Eight tissue-level endosseous implants were placed in 6 mixed-breed foxhounds, with 2 zirconia and 2 titanium alternating in each hemimandible. Cotton ligatures were placed subgingivally for 16 weeks followed by 8 weeks of spontaneous progression. Standardized radiographs were captured every 2 weeks to evaluate the rate of bone loss. Regenerative surgery was performed utilizing water-jet decontamination, enamel matrix derivative, and locally harvested autogenous bone. After 16 weeks of healing, final radiographic bone levels as well as probing depths, recession, and clinical attachment levels were assessed. Results: All 48 implants integrated successfully. The final average post-ligature radiographic defects were 2.88 and 3.05 mm for titanium and zirconia implants, respectively. There was no significant difference between materials in the rate of radiographic bone loss (p = 0.09). Following regenerative surgery, the total average amount of radiographic bone gain was 1.41 and 1.20 mm for titanium and zirconia, respectively. The percentage of defect fill was 51.56% and 37.98% (p = 0.03) for titanium and zirconia, respectively. Inter-group differences were minimal for clinical parameters at the time of sacrifice including periodontal pocket depths (p = 0.81), recession (p = 0.98), or clinical attachment levels (p = 0.51). Conclusions: No significant difference was found in the rate of peri-implant defect development between titanium and zirconia implants. Both materials gained significant radiographic bone following regenerative surgery with significantly greater defect percentage fill in titanium implants. The final clinical parameters were similar in both groups.
Osseointegration of zirconia implants: an SEM observation of the bone-implant interface

Abstract:Background: The successful use of zirconia ceramics in orthopedic surgery led to a demand for dental zirconium-based implant systems. Because of its excellent biomechanical characteristics, biocompatibility, and bright tooth-like color, zirconia (zirconium dioxide, ZrO2) has the potential to become a substitute for titanium as dental implant material. The present study aimed at investigating the osseointegration of zirconia implants with modified ablative surface at an ultrastructural level. Methods: A total of 24 zirconia implants with modified ablative surfaces and 24 titanium implants all of similar shape and surface structure were inserted into the tibia of 12 Göttinger minipigs. Block biopsies were harvested 1 week, 4 weeks or 12 weeks (four animals each) after surgery. Scanning electron microscopy (SEM) analysis was performed at the bone implant interface. Results: Remarkable bone attachment was already seen after 1 week which increased further to intimate bone contact after 4 weeks, observed on both zirconia and titanium implant surfaces. After 12 weeks, osseointegration without interposition of an interfacial layer was detected. At the ultrastructural level, there was no obvious difference between the osseointegration of zirconia implants with modified ablative surfaces and titanium implants with a similar surface topography. Bio‐Inspired Micro‐ and Nano‐Scale Surface Features Produced by Femtosecond Laser‐Texturing Enhance TiZr‐Implant Osseointegration

Abstract:Surface design plays a critical role in determining the integration of dental implants with bone tissue. Femtosecond laser‐texturing has emerged as a breakthrough technology offering excellent uniformity and reproducibility in implant surface features. However, when compared to state‐of‐the‐art sandblasted and acid‐etched surfaces, laser‐textured surface designs typically underperform in terms of osseointegration. This study investigated the capacity of a bio‐inspired femtosecond laser‐textured surface design to enhance osseointegration compared to state‐of‐the‐art sandblasted & acid‐etched surfaces. Laser‐texturing facilitates the production of an organized trabeculae‐like microarchitecture with superimposed nano‐scale laser‐induced periodic surface structures on both 2D and 3D samples of titanium‐zirconium‐alloy. Following a boiling treatment to modify the surface chemistry, improving wettability to a contact angle of 10°, laser‐textured surfaces enhance fibrin network formation when in contact with human whole blood, comparable to state‐of‐the‐art surfaces. In vitro experiments demonstrate that laser‐textured surfaces significantly outperform state‐of‐the‐art surfaces with a 2.5‐fold higher level of mineralization by bone progenitor cells after 28 days of culture. Furthermore, in vivo evaluations reveal superior biomechanical integration of laser‐textured surfaces after 28 days of implantation. Notably, during abiological pull‐out tests, laser‐textured surfaces exhibit comparable performance, suggesting that the observed enhanced osseointegration is primarily driven by the biological response to the surface. Abstract:Zirconia implants have emerged as a valuable alternative for clinical scenarios where aesthetic demands are high, as well as in cases of hypersensitivity to titanium or for patients who refuse metallic objects in their bodies due to personal reasons. However, these implants have undergone various changes in geometry, manufacturing techniques, and surface modifications since the introduction of the first zirconia implants. The present study aims to review the current evidence on zirconia implants, considering the changes they have undergone in recent years. Additionally, it aims to analyze the three-dimensional surface characteristics of a failed zirconia implant using scanning electron microscopy and elemental analysis with energy-dispersive X-ray spectrometry (EDX). A zirconia implant lost three weeks after placement was immediately assessed using VP-SEM equipment and chemically analyzed by EDX using a 410-M detector connected to the microscope. Sparse material depositions were found on all parts of the implant, with a notable concentration in the thread grooves. The elements identified in the sample included zirconium, oxygen, carbon, calcium, and phosphorus. This report demonstrates that the surface of zirconia implants can accumulate elements early in the process of bone matrix neoformation, which is consistent with the initial stage of osseointegration.

Abstract:The aim of this study was to evaluate gingival fibroblasts and human osteoblasts’ response to textured Nd:YAG laser microgrooves, with different dimensions, on zirconia implant surfaces. A total of 60 zirconia disks (8 mm in diameter and 2 mm in thickness) were produced and divided between four study groups (N = 15): three laser-textured (widths between 125.07 ± 5.29 μm and 45.36 ± 2.37 μm and depth values from 50.54 ± 2.48 μm to 23.01 ± 3.79 μm) and a control group without laser treatment. Human osteoblasts and gingival fibroblasts were cultured on these surfaces for 14 days. FEG-SEM (Field Emission Gun–Scanning Electron Microscope) images showed cellular adhesion at 24 h, with comparable morphology in all samples for both cell types. A similar cell spreading within the grooves and in the space between them was observed. Cell viability increased over time in all study groups; however, no differences were found between them. Additionally, proliferation, ALP (Alkaline phosphatase) activity, collagen type I, osteopontin and interleukin levels were not significantly different between any of the study groups for any of the cell types. Analysis of variance to compare parameters effect did not reveal statistically significant differences when comparing all groups in the different tests performed. The results obtained revealed similar cell behavior based on cell viability and differentiation on different microtopographic laser grooves, compared to a microtopography only established by sandblasting and acid-etching protocol, the reference surface treatment on zirconia dental implants.
Zirconia vs. Titanium Dental Implants: Primary Stability In-Vitro Analysis

Abstract:The present experimental trial uses two types of dental implants, one made of titanium (Ti6Al4V) and the other one of zirconia (ZrO2), but both of identical design, to compare their stability and micro-movements values under load. One of each type of implant (n = 42) was placed into 21 cow ribs, recording the insertion torque and the resonance frequency using a specific transducer. Subsequently, a prosthetic crown made of PMMA was screwed onto each of the implants in the sample. They were then subjected to a static compression load on the vestibular cusp of the crown. The resulting micromovements were measured. The zirconia implants obtained a higher mean of both IT and RFA when compared with those of titanium, with statistically significant differences in both cases (p = 0.0483 and p = 0.0296). However, the micromovement values when load was applied were very similar for both types, with the differences between them (p = 0.3867) not found to be statistically significant. The results show that zirconia implants have higher implant stability values than titanium implants. However, the fact that there are no differences in micromobility values implies that caution should be exercised when applying clinical protocols for zirconia based on RFA, which only has evidence for titanium.

Abstract:Standard zirconia implants used in restoration still present problems related to inertness and long-term stability. Various physicochemical approaches have been used to modify the implant surfaces to improve early and late bone-to-implant integration; however, no ideal surface modification has been reported. This study used pulsed laser deposition to deposit a fluorinated hydroxyapatite (FHA) film on a zirconia implant to create a biologically active surface. The film prepared was uniform, dense, and crack-free, and exhibited granular surface droplets; it also presented excellent mechanical strength and favorable biological behavior. The FHA-coated implant was implanted on the femur of Sprague–Dawley rats, and various tests and analyses were performed. Results show that the in vitro initial cell activity on the FHA-coated samples was enhanced. In addition, higher alkaline phosphatase activity and cell mineralization were detected in cells cultured on the FHA-coated groups. Further, the newly formed bone volume of the FHA-coated group was higher than that of the bare micro-adjusted composite nano-zirconia (NANOZR) group. Therefore, the FHA film facilitated osseointegration and may improve the long-term survival rates of dental implants, and could become part of a new treatment technology for implant surfaces, promoting further optimization of NANOZR implant materials.
