Abstract:Analysis of lifetime data from epidemiological studies or destructive testing often involves current status censoring, wherein individuals are examined only once and their event status is recorded only at that specific time point. In practice, some of these individuals may never experience the event of interest, leading to current status data with a cured fraction. Cure models are used to estimate the proportion of non-susceptible individuals, the distribution of susceptible ones, and covariate effects. Motivated from a biological interpretation of cancer metastasis, promotion time cure model is a popular alternative to the mixture cure rate model for analysing such data. The current study is the first to put forth a Bayesian inference procedure for analysing current status data with a cure fraction, resorting to a promotion time cure model. An adaptive Metropolis-Hastings algorithm is utilised for posterior computation. Simulation studies prove our approach's efficiency, while analyses of lung tumor and breast cancer data illustrate its practical utility. This approach has the potential to improve clinical cure rates through the incorporation of prior knowledge regarding the disease dynamics and therapeutic options.

Abstract:We consider the problem of estimating the distribution of time-to-event data that are subject to censoring and for which the event of interest might never occur, i.e., some subjects are cured. To model this kind of data in the presence of covariates, one of the leading semiparametric models is the promotion time cure model \citep{yakovlev1996}, which adapts the Cox model to the presence of cured subjects. Estimating the conditional distribution results in a complicated constrained optimization problem, and inference is difficult as no closed-formula for the variance is available. We propose a new model, inspired by the Cox model, that leads to a simple estimation procedure and that presents a closed formula for the variance. We derive some asymptotic properties of the estimators and we show the practical behaviour of our procedure by means of simulations. We also apply our model and estimation method to a breast cancer data set.

Abstract:We propose a novel method for predicting time-to-event in the presence of cure fractions based on flexible survivals models integrated into a deep neural network framework. Our approach allows for non-linear relationships and high-dimensional interactions between covariates and survival and is suitable for large-scale applications. Furthermore, we allow the method to incorporate an identified predictor formed of an additive decomposition of interpretable linear and non-linear effects and add an orthogonalization layer to capture potential higher dimensional interactions. We demonstrate the usefulness and computational efficiency of our method via simulations and apply it to a large portfolio of US mortgage loans. Here, we find not only a better predictive performance of our framework but also a more realistic picture of covariate effects.

Abstract:A tumor is a complex tissue comprised of heterogeneous cell subpopulations which exhibit substantial diversity at morphological, genetic and epigenetic levels. Under the selective pressure of cancer therapies, a minor treatment-resistant subpopulation could survive and repopulate. Therefore, the intra-tumor heterogeneity is recognized as a major obstacle to effective treatment. In this paper, we propose a stochastic clonal expansion model to simulate the dynamic evolution of tumor subpopulations and the therapeutic effect at different times during tumor progression. The model is incorporated in the CES webserver, for the convenience of simulation according to initial user input. Based on this model, we investigate the influence of various factors on tumor progression and treatment consequences and present conclusions drawn from observations, highlighting the importance of treatment timing. The model provides an intuitive illustration to deepen the understanding of temporal intra-tumor heterogeneity dynamics and treatment responses, thus helping the improvement of personalized diagnostic and therapeutic strategies.

Abstract:Cure models have been widely developed to estimate the cure fraction when some subjects never experience the event of interest. However these models were rarely focused on the estimation of the time-to-cure i.e. the delay elapsed between the diagnosis and "the time from which cure is reached", an important indicator, for instance to address the question of access to insurance or loans for subjects with personal history of cancer. We propose a new excess hazard regression model that includes the time-to-cure as a covariate dependent parameter to be estimated. The model is written similarly to a Beta probability distribution function and is shown to be a particular case of the non-mixture cure models. Parameters are estimated through a maximum likelihood approach and simulation studies demonstrate good performance of the model. Illustrative applications to two cancer data sets are provided and some limitations as well as possible extensions of the model are discussed. The proposed model offers a simple and comprehensive way to estimate more accurately the time-to-cure. Key words: Cancer; Cure model; Cure time; Net survival; Right to be forgotten.

Abstract:Statistical Methods in Medical Research, Ahead of Print. We propose a class of cure rate models motivated by analysis of colon cancer and triple-negative breast cancer survival data. This class is indexed by an adaptive activation parameter and a function. We establish that the class is stochastically ordered in the activation parameter and also establish two identifiability results for this class. The first- and last-activation models are members of this class whereas many cure rate models proposed in the literature are also part of this class. We illustrate that while first- and last-activation models may perform poorly under model misspecifications, the proposed model with adaptive activation provides appropriate inference in these cases. We apply the proposed approach to assess treatment-sex interaction on cure rate in a colon cancer study and to assess role of tumor heterogeneity and ethnic disparity in breast cancer.

Abstract:We introduce a new modelling for long-term survival models, assuming that the number of competing causes follows a mixture of Poisson and the Birnbaum-Saunders distribution. In this context, we present some statistical properties of our model and demonstrate that the promotion time model emerges as a limiting case. We delve into detailed discussions of specific models within this class. Notably, we examine the expected number of competing causes, which depends on covariates. This allows for direct modeling of the cure rate as a function of covariates. We present an Expectation-Maximization (EM) algorithm for parameter estimation, to discuss the estimation via maximum likelihood (ML) and provide insights into parameter inference for this model. Additionally, we outline sufficient conditions for ensuring the consistency and asymptotic normal distribution of ML estimators. To evaluate the performance of our estimation method, we conduct a Monte Carlo simulation to provide asymptotic properties and a power study of LR test by contrasting our methodology against the promotion time model. To demonstrate the practical applicability of our model, we apply it to a real medical dataset from a population-based study of incidence of breast cancer in São Paulo, Brazil. Our results illustrate that the proposed model can outperform traditional approaches in terms of model fitting, highlighting its potential utility in real-world scenarios.

Abstract:In modeling survival data with a cure fraction, flexible modeling of covariate effects on the probability of cure has important medical implications, which aids investigators in identifying better treatments to cure. This paper studies a semiparametric form of the Yakovlev promotion time cure model that allows for nonlinear effects of a continuous covariate. We adopt the local polynomial approach and use the local likelihood criterion to derive nonlinear estimates of covariate effects on cure rates, assuming that the baseline distribution function follows a parametric form. This way we adopt a flexible method to estimate the cure rate locally, the important part in cure models, and a convenient way to estimate the baseline function globally. An algorithm is proposed to implement estimation at both the local and global scales. Asymptotic properties of local polynomial estimates, the nonparametric part, are investigated in the presence of both censored and cured data, and the parametric part is shown to be root-n consistent. The proposed methods are illustrated by simulated and real data.

Abstract:We propose a new cure model for survival data with a surviving or cure fraction. The new model is a mixture cure model where the covariate effects on the proportion of cure and the distribution of the failure time of uncured patients are separately modeled. Unlike the existing mixture cure models, the new model allows covariate effects on the failure time distribution of uncured patients to be negligible at time zero and to increase as time goes by. Such a model is particularly useful in some cancer treatments when the treat effect increases gradually from zero, and the existing models usually cannot handle this situation properly. We develop a rank based semiparametric estimation method to obtain the maximum likelihood estimates of the parameters in the model. We compare it with existing models and methods via a simulation study, and apply the model to a breast cancer data set. The numerical studies show that the new model provides a useful addition to the cure model literature.

Abstract:The promotion time cure rate model (PCM) is an extensively studied model for the analysis of time-to-event data in the presence of a cured subgroup. There are several strategies proposed in the literature to model the latency part of PCM. However, there aren't many strategies proposed to investigate the effects of covariates on the incidence part of PCM. In this regard, most existing studies assume the boundary separating the cured and non-cured subjects with respect to the covariates to be linear. As such, they can only capture simple effects of the covariates on the cured/non-cured probability. In this manuscript, we propose a new promotion time cure model that uses the support vector machine (SVM) to model the incidence part. The proposed model inherits the features of the SVM and provides flexibility in capturing non-linearity in the data. To the best of our knowledge, this is the first work that integrates the SVM with PCM model. For the estimation of model parameters, we develop an expectation maximization algorithm where we make use of the sequential minimal optimization technique together with the Platt scaling method to obtain the posterior probabilities of cured/uncured. A detailed simulation study shows that the proposed model outperforms the existing logistic regression-based PCM model as well as the spline regression-based PCM model, which is also known to capture non linearity in the data. This is true in terms of bias and mean square error of different quantities of interest, and also in terms of predictive and classification accuracies of cure. Finally, we illustrate the applicability and superiority of our model using the data from a study on leukemia patients who went through bone marrow transplantation.

Generalized parametric cure models for relative survival

Abstract:Cure models are used in time-to-event analysis when not all individuals are expected to experience the event of interest, or when the survival of the considered individuals reaches the same level as the general population. These scenarios correspond to a plateau in the survival and relative survival function, respectively. The main parameters of interest in cure models are the proportion of individuals who are cured, termed the cure proportion, and the survival function of the uncured individuals. Although numerous cure models have been proposed in the statistical literature, there is no consensus on how to formulate these. We introduce a general parametric formulation of mixture cure models and a new class of cure models, termed latent cure models, together with a general estimation framework and software, which enable fitting of a wide range of different models. Through simulations, we assess the statistical properties of the models with respect to the cure proportion and the survival of the uncured individuals. Finally, we illustrate the models using survival data on colon cancer, which typically display a plateau in the relative survival. As demonstrated in the simulations, mixture cure models which are not guaranteed to be constant after a finite time point, tend to produce accurate estimates of the cure proportion and the survival of the uncured. However, these models are very unstable in certain cases due to identifiability issues, whereas LC models generally provide stable results at the price of more biased estimates.

Abstract:To address an important risk classification issue that arises in clinical practice, we propose a new mixture model via latent cure rate markers for survival data with a cure fraction. In the proposed model, the latent cure rate markers are modeled via a multinomial logistic regression and patients who share the same cure rate are classified into the same risk group. Compared to available cure rate models, the proposed model fits better to data from a prostate cancer clinical trial. In addition, the proposed model can be used to determine the number of risk groups and to develop a predictive classification algorithm.

Abstract:A general class of semiparametric transformation cure models is studied for the analysis of survival data with long-term survivors. It combines a logistic regression for the probability of event occurrence with the class of transformation models for the time of occurrence. Included as special cases are the proportional hazards cure model (Farewell, 1982; Kuk Chen, 1992; Sy Taylor, 2000; Peng & Dear, 2000) and the proportional odds cure model. Generalised estimating equations are proposed for parameter estimation. It is shown that the resulting estimators are asymptotically normal, with variance-covariance matrix that has a closed form and can be consistently estimated by the usual plug-in method. Simulation studies show that the proposed approach is appropriate for practical use. An application to data from a breast cancer study is given to illustrate the methodology.

Abstract:Cure rate models have been widely adopted for characterizing survival data that have long-term survivors. Under a mixture cure rate model where the population is a mixture of cured and susceptible subjects, a primary goal is to study covariate effects on the cure probability and survival function of the susceptible subjects. In this article, we propose a penalization method for estimating the mixture cure rate model where we explicitly consider the structural effects of covariates. The proposed method is more informative than the standard estimations and more flexible than the existing works on structural effects. Depending on data characteristics, we develop different penalties and corresponding computational algorithms. Simulation shows that the proposed method outperforms the alternatives by more accurately estimating parameters and identifying relevant variables. Two breast cancer datasets, one with low-dimensional clinical variables and the other with high-dimensional genetic variables, are analyzed.

Abstract:When studying the treatment effect on time-to-event outcomes, it is common that some individuals never experience failure events, which suggests that they have been cured. However, the cure status may not be observed due to censoring which makes it challenging to define treatment effects. Current methods mainly focus on estimating model parameters in various cure models, ultimately leading to a lack of causal interpretations. To address this issue, we propose 2 causal estimands, the timewise risk difference and mean survival time difference, in the always-uncured based on principal stratification as a complement to the treatment effect on cure rates. These estimands allow us to study the treatment effects on failure times in the always-uncured subpopulation. We show the identifiability using a substitutional variable for the potential cure status under ignorable treatment assignment mechanism, these 2 estimands are identifiable. We also provide estimation methods using mixture cure models. We applied our approach to an observational study that compared the leukemia-free survival rates of different transplantation types to cure acute lymphoblastic leukemia. Our proposed approach yielded insightful results that can be used to inform future treatment decisions.

Abstract:In population-based cancer survival analysis, the net survival is important for government to assess health care programs. For decades, it is observed that the net survival reaches a plateau after long-term follow-up, this is so called ``statistical cure''. Several methods were proposed to address the statistical cure. Besides, the cure time can be used to evaluate the time period of a health care program for a specific patient population, and it also can be helpful for a clinician to explain the prognosis for patients, therefore the cure time is an important health care index. However, those proposed methods assume the cure time to be infinity, thus it is inconvenient to make inference on the cure time. In this dissertation, we define a more general concept of statistical cure via conditional survival. Based on the newly defined statistical cure, the cure time is well defined. We develop cure time model methodologies and show a variety of properties through simulation. In data analysis, cure times are estimated for 22 major cancers in Taiwan, we further use colorectal cancer data as an example to conduct statistical inference via cure time model with covariate sex, age group, and stage. This dissertation provides a methodology to obtain cure time estimate, which can contribute to public health policy making.

Abstract:A simple yet effective way of modeling survival data with cure fraction is by considering Box-Cox transformation cure model (BCTM) that unifies mixture and promotion time cure models. In this article, we numerically study the statistical properties of the BCTM when applied to interval censored data. Time-to-events associated with susceptible subjects are modeled through proportional hazards structure that allows for non-homogeneity across subjects, where the baseline hazard function is estimated by distribution-free piecewise linear function with varied degrees of non-parametricity. Due to missing cured statuses for right censored subjects, maximum likelihood estimates of model parameters are obtained by developing an expectation-maximization (EM) algorithm. Under the EM framework, the conditional expectation of the complete data log-likelihood function is maximized by considering all parameters (including the Box-Cox transformation parameter $\alpha$) simultaneously, in contrast to conventional profile-likelihood technique of estimating $\alpha$. The robustness and accuracy of the model and estimation method are established through a detailed simulation study under various parameter settings, and an analysis of real-life data obtained from a smoking cessation study.

Abstract:Cure rate models are mostly used to study data arising from cancer clinical trials. Its use in the context of infectious diseases has not been explored well. In 2008, Tournoud and Ecochard first proposed a mechanistic formulation of cure rate model in the context of infectious diseases with multiple exposures to infection. However, they assumed a simple Poisson distribution to capture the unobserved pathogens at each exposure time. In this paper, we propose a new cure rate model to study infectious diseases with discrete multiple exposures to infection. Our formulation captures both over-dispersion and under-dispersion with respect to the count on pathogens at each time of exposure. We also propose a new estimation method based on the expectation maximization algorithm to calculate the maximum likelihood estimates of the model parameters. We carry out a detailed Monte Carlo simulation study to demonstrate the performance of the proposed model and estimation algorithm. The flexibility of our proposed model also allows us to carry out a model discrimination. For this purpose, we use both likelihood ratio test and information-based criteria. Finally, we illustrate our proposed model using a recently collected data on COVID-19.

Abstract:A novel mixture cure frailty model is introduced for handling censored survival data. Mixture cure models are preferable when the existence of a cured fraction among patients can be assumed. However, such models are heavily underexplored: frailty structures within cure models remain largely undeveloped, and furthermore, most existing methods do not work for high-dimensional datasets, when the number of predictors is significantly larger than the number of observations. In this study, we introduce a novel extension of the Weibull mixture cure model that incorporates a frailty component, employed to model an underlying latent population heterogeneity with respect to the outcome risk. Additionally, high-dimensional covariates are integrated into both the cure rate and survival part of the model, providing a comprehensive approach to employ the model in the context of high-dimensional omics data. We also perform variable selection via an adaptive elastic-net penalization, and propose a novel approach to inference using the expectation-maximization (EM) algorithm. Extensive simulation studies are conducted across various scenarios to demonstrate the performance of the model, and results indicate that our proposed method outperforms competitor models. We apply the novel approach to analyze RNAseq gene expression data from bulk breast cancer patients included in The Cancer Genome Atlas (TCGA) database. A set of prognostic biomarkers is then derived from selected genes, and subsequently validated via both functional enrichment analysis and comparison to the existing biological literature. Finally, a prognostic risk score index based on the identified biomarkers is proposed and validated by exploring the patients' survival.

Abstract:With advancements in medical research, more and more diseases may be curable, which indicates some patients may not die of the disease of interest. Mixture cure models, which can capture patients being cured, attracts an increasing attention in practice. However, the existing mixture cure models only focused on the major event with a potential cure while ignoring the potential risk from other non-curable competing events, which are commonly seen in the real world. In this paper, we develop a Bayesian approach to estimate a proportional hazards mixture cure (PHMC) model allowing non-curable competing risk. Data augmentation method with latent binary cure indicators and event indicators are adopted to simplify the Markov chain Monte Carlo implementation. The baseline cumulative hazards for the PHMC model are formulated by counting processes with gamma process priors. Its performance is demonstrated through comprehensive simulation studies. Finally, the proposed method is applied to a prostate cancer clinical trial data set.
