Unlocking the Power of Spatial and Temporal Information in Medical Multimodal Pre-training

Abstract:Medical vision-language pre-training methods mainly leverage the correspondence between paired medical images and radiological reports. Although multi-view spatial images and temporal sequences of image-report pairs are available in off-the-shelf multi-modal medical datasets, most existing methods have not thoroughly tapped into such extensive supervision signals. In this paper, we introduce the Med-ST framework for fine-grained spatial and temporal modeling to exploit information from multiple spatial views of chest radiographs and temporal historical records. For spatial modeling, Med-ST employs the Mixture of View Expert (MoVE) architecture to integrate different visual features from both frontal and lateral views. To achieve a more comprehensive alignment, Med-ST not only establishes the global alignment between whole images and texts but also introduces modality-weighted local alignment between text tokens and spatial regions of images. For temporal modeling, we propose a novel cross-modal bidirectional cycle consistency objective by forward mapping classification (FMC) and reverse mapping regression (RMR). By perceiving temporal information from simple to complex, Med-ST can learn temporal semantics. Experimental results across four distinct tasks demonstrate the effectiveness of Med-ST, especially in temporal classification tasks. This paper primarily discusses the potential of unlocking spatial and temporal information in medical multimodal pretraining. Existing medical visual language pretraining methods mainly rely on the correspondence between images and radiology reports, but do not fully utilize the rich supervisory signals in multi-view spatial images and temporal sequences. The paper proposes the Med-ST framework, which extracts multi-view images of chest X-rays and patient medical records by finely modeling spatial and temporal information. In terms of spatial modeling, Med-ST adopts the Mixed-View Experts (MoVE) architecture to integrate visual features from front and lateral views, achieving joint representations of different spatial perspectives. In addition, modal-weighted local alignment is introduced to achieve fine-grained alignment between image regions and text tokens. In terms of temporal modeling, the paper proposes a novel cross-modal bidirectional cyclic consistency objective, which perceives temporal information through forward-mapped classification (FMC) and backward-mapped regression (RMR). This approach allows the model to gradually understand the temporal semantics of sequence context, from simple to complex. Experimental results demonstrate that Med-ST performs well in four different tasks, particularly in time classification tasks. The paper provides code and models to facilitate further research. The main contributions of this study include: comprehensive exploration of information in medical multimodal data, leveraging multi-view spatial data and historical temporal data; processing front and lateral views through the MoVE architecture; proposing a cross-modal bidirectional cyclic consistency objective for temporal modeling.
