Abstract:We recently developed the Genomics Research Integration System (GRIS) to help NIAID investigators at the NIH leverage both phenotypic and genotypic patient data to identify causal variants for rare diseases. The project is a bioinformatics compliment to an initiative to sequence exomes for all NIAID patients visiting the NIH Clinical Center. The system is designed to serve as a valuable resource for clinical genomic data annotated with standardized phenotypic terms using the Human Phenotype Ontology \citeKohler2013. GRIS uses PhenoTips® \citeGirdea2013 to capture clinical records and family pedigrees which are linked to genomic records stored in a genetic analysis tool,seqr, developed at the Broad Institute (\urlseqr.broadinstitute.org ) to enable causal variant identification. We have customized both programs in novel ways to meet NIH encryption requirements, to link patient records across programs in a controlled manner, and to provide "tiers" of access so that individual research groups can customize users' ability to edit their patient records and view personally identifiable information (PII). A challenge faced by shared clinical data repositories is to facilitate maximal collective research value of data through open sharing, while respecting the needs of researchers to adjust access to patient data in accordance with research goals and subject to clinical sharing guidelines. We devised a technical approach to meet the needs of sharing policies, formulated collectively by researchers and clinicians, to promote wider acceptance and usage of the system. Accordingly, we implemented a patient identifier mapping system in conjunction with automated notifications to enable transparent sharing. Our approach may prove helpful to other hospital or clinical support systems seeking to respect the confidentiality of patient PII and early findings of individual researchers, while recognizing that data repositories are most primed for discovery (and can significantly increase return on investment) if they are open and accessible to a larger research community.

Abstract:As the Brain Research through Advancing Neurotechnologyies, better known as the BRAIN Initiative moves forward at a rapid pace increasing attention has focused on neuroethics. The National Institutes of Health recently mandated a review of the progress of the BRAIN Initiative progress with the goal of fine-tuning its future directions. The BRAIN Working Group 2 focused its discussion on science while the BRAIN Neuroethics Subgroup focused on neuroethics. The Brain Neuroethics Subgroup deliberated for over a year collecting information and recommendations through in person and video meetings, a public one-day neuroethics symposium and soliciting input from neuroscientists and neuroethicist's. The resulting report entitled "The BRAIN Initiative and Neuroethics: Enabling and Enhancing Neuroscience Advances for Society" was accepted by Director of NIH in October of 2019. The recommendations span many BRAIN research neuroethics concerns including privacy considerations, the use of nonhuman primate model systems, neural modulation and enhancement, subject participation in BRAIN research and equity in neuroscience research. Further the group recommended a transformative project whose goal of detailing the scientific mechanisms and ethical underpinnings of consciousness is one of the most daunting issues that impact our perceptions of ourselves. It is anticipated that the report?s recommendations will provide a foundation or "roadmap" for ensuring that neuroethics and BRAIN research move forward as an integrated effort thereby insuring that BRAIN research is of the highest quality.

Abstract:The founding mission of the National Institute on Aging (NIA) was visionary and broad—from understanding the basic biology of aging to enhancing care, quality, and outcomes for older adults. Their investment and vision have led to innovative research in biological sciences exemplified by identification of about a dozen cellular and molecular mechanisms that drive both aging and several chronic diseases. 1 These advances have given rise to the geroscience hypothesis and the field of geroscience. The geroscience hypothesis states that interventions modifying biological mechanisms of aging (gerotherapeutics) might prevent, delay, or alleviate several age-related diseases simultaneously, lengthening the lifespan and, more importantly, increasing the health span, defined as the length of life without illness or disability. 1 ,2 Thus, geroscience focuses on translational applications that hold the potential to alter the course of aging and chronic diseases (Box).

Abstract:GRaNIE builds enhancer‐based gene regulatory networks (GRNs) using chromatin accessibility and RNA‐seq data. GRaNPA assesses the biological significance of GRNs and transcription factors. Together, they provide insights into cell‐type‐specific gene regulation. Enhancers play a vital role in gene regulation and are critical in mediating the impact of noncoding genetic variants associated with complex traits. Enhancer activity is a cell‐type‐specific process regulated by transcription factors (TFs), epigenetic mechanisms and genetic variants. Despite the strong mechanistic link between TFs and enhancers, we currently lack a framework for jointly analysing them in cell‐type‐specific gene regulatory networks (GRN). Equally important, we lack an unbiased way of assessing the biological significance of inferred GRNs since no complete ground truth exists. To address these gaps, we present GRaNIE (Gene Regulatory Network Inference including Enhancers) and GRaNPA (Gene Regulatory Network Performance Analysis). GRaNIE (https://git.embl.de/grp‐zaugg/GR aNIE) builds enhancer‐mediated GRNs based on covariation of chromatin accessibility and RNA‐seq across samples (e.g. individuals), while GRaNPA (https://git.embl.de/grp‐zaugg/GRaNPA) assesses the performance of GRNs for predicting cell‐type‐specific differential expression. We demonstrate their power by investigating gene regulatory mechanisms underlying the response of macrophages to infection, cancer and common genetic traits including autoimmune diseases. Finally, our methods identify the TF PURA as a putative regulator of pro‐inflammatory macrophage polarisation.

Abstract:The National Institute on Aging (NIA) conducts and funds research aimed at extending the healthy, active years of life. 1 This mission distinguishes the institute from others that focus primarily on the study of specific organs and diseases. Over its 50 years, the NIA has supported clinical and behavioral research across a wide range of issues. In this Viewpoint we highlight multicomplexity and dementia, 2 areas where the NIA has made integral and distinctive contributions. We also discuss the NIA's role in applying principles from behavioral economics to research designed to improve the health of older persons.

Abstract:Gene therapy has made substantial progress in the treatment of the genetic diseases, focussing on the reduction of characteristics of recessive/dominant disorders, as well as various cancers. Extensive research has been conducted in the past few decades to investigate the application of nanotechnology and CRISPR/Cas technology in gene therapy. Nanotechnology due to attributes such has targeted drug delivery, controlled release, scalability and low toxicity has gained attention of the medical world. CRISPR/Cas9 system is considered as an impactful genome editing tool in the area of next-generation therapeutics and molecular diagnostics. CRISPR technology emphasises on gene editing, gene regulation modulation, and formulation of defined genetic changes. Its applications in treatment of the genetic disorders are extended beyond traditional therapies. These techniques are being explored as treatment of several genetic disorders including Duchenne muscular dystrophy, cystic fibrosis, Alzheimer's disease, Parkinson's disease, and Huntington disease. Despite considerable therapeutic potential of gene therapy, several obstacles must be addressed before it can be widely adopted in clinical practice, particularly in terms of ensuring safety and effectiveness. As research advances in this captivating field, these therapies will become the primary treatments and will have significant beneficial effects on the lives of patients with genetic disorders.

Abstract:In 2016, Nucleic Acids Research ('NAR') assembled a special collection of studies, published in its pages over the preceding five-years, that described the biology and use of CRISPR/Cas systems (1). The 79 manuscripts presented in that collection included 37 studies that described the evolution, genetics, and mechanisms of CRISPR defense systems and an almost equal number of studies describing their development and application for gene modification and genome engineering. Those publications were supplemented with several descriptions of related online computational tools and two review articles that summarized the rapidly growing body of literature surrounding the evolution of CRISPR systems (2) and their application for gene editing and genome engineering (3).

Intact Perception of Biological Motion in the Face of Profound Spatial Deficits: Williams Syndrome

Abstract:Williams syndrome (WS) is a rare genetic disorder that results in profound spatial cognitive deficits. We examined whether individuals with WS have intact perception of biological motion, which requires global spatial integration of local motion signals into a unitary percept of a human form. Children with WS, normal mental-age-matched children, and normal adults viewed point-light-walker (PLW) displays portraying a human figure walking to the left or right. Children with WS were as good as or better than control children in their ability to judge the walker's direction, even when it was masked with dynamic noise that mimicked the local motion of the PLW lights. These results show that mechanisms underlying the perception of at least some kinds of biological motion are unimpaired in children with WS. They provide the first evidence of selective sparing of a specialized spatial system in individuals with a known genetic impairment.

Their complementary foci, structures, and review mechanisms make the extramural and intramural cancer research contributions of the National Institutes of Health the perfect partners in the quest to rid the world of cancer as we know it.

Abstract:Abstract Kuakini Medical Center (Kuakini) was funded by NIH in late 2019 to create an interdisciplinary Hawai’i-based, Center of Biomedical Research Excellence (COBRE), for translational research on aging. This Center is building upon Kuakini’s five-decades of prior NIH-funded research. These resources include clinical data from the 58-year ongoing Kuakini Honolulu Heart Program (HHP), Honolulu-Asia Aging Study, HHP Offspring Study and a large biorepository with over 500,000 biological samples. The Center’s overarching aim is to increase infrastructure for collaborative aging research in Hawaii. The first step is to grow the Center’s faculty by hiring and mentoring research project leaders (RPLs) from diverse disciplines to become independent, R01-funded, investigators on aging. Our first RPL has graduated after obtaining R01-funded status. His project utilizes novel CRISPR methods to i) improve the safety and efficacy of delivering potentially therapeutic genes (such as FOXO3) to the mouse genome, and ii) test whether temporal enhancement of FOXO3 expression improves healthy aging in this mouse model - both key steps for potential translation to human clinical therapies. This work will be highlighted in the Program Overview session followed by current RPL findings. These findings include novel findings on a potential relation between FOXO3 genotype and stroke dynamics in elderly Japanese-American males; venous congestion in the brain and its potential impact on cognitive function; new findings on the Okinawan longevity phenomenon, among other interesting findings related to healthy aging. An evaluation of programs to support new investigators at the National Institute of Allergy and Infectious Diseases: Striking a balance with funding for established investigators

Abstract:As the largest funder of basic biomedical research in the US, the National Institutes of Health (NIH) has an interest in maintaining a sustainable, productive workforce of investigators. Over the years, NIH has implemented several programs to attract early-stage investigators and other applicants without prior NIH support. The latest of these is the Next Generation Researchers Initiative. These programs have been shown to be successful in meeting NIH-wide goals but their success for any particular NIH institute or center (IC), and in any particular year, is determined by a variety of factors, some extrinsic to an IC's funding decision process. Each IC must balance support for new investigators with funding for productive ongoing programs of research. We examine historical trends in support of new investigators at the National Institute of Allergy and Infectious Diseases (NIAID) over a 22-year period, as well as trends in some major extrinsic influences on that support. The results indicate that NIH's new investigator programs have succeeded in maintaining a balance between the support for new NIAID investigators while also continuing to support an expanded pool of established investigators. The programs have been particularly effective in providing support to early-stage investigators.
The State of National Institute of Health Awards for funding genetic counseling research, resources, and training over the past decade

Abstract:Research related to the practice of genetic counseling has historically been accomplished through student projects, small private grants, or as a part of a larger research study. Yet, recent initiatives supported by the National Society of Genetic Counselors and the National Human Genome Research Institute have recognized and promoted the need for additional genetic counseling research funding and training. In this study, we aimed to characterize awards from the United States' National Institutes of Health (NIH) over the past 10 years that support research related to genetic counseling. A search of the NIH RePORTER database conducted on April 25, 2022, using terms broadly related to genetic counseling, identified 3993 awards from fiscal years 2011 to 2022. After deduplication, 1231 grants were reviewed for screening. The final dataset included 43 awards of various types with the majority being research or R series grants [R01 (n = 17), R21 (n = 7)]. The remaining awards were Cooperative Agreements (U01, n = 6), intramural (Z‐grants, n = 5), Center Core (P30, n = 2), Specialized Center (P50, n = 1), Career Development Award (K01, n = 1), Other Transactions (OT2, n = 1), Resource Program (G13, n = 1), and Research Training and Fellowship (T32, n = 1). Most grants were awarded between 2019 and 2022 (n = 24, 55.8%). The majority of awards were categorized as studies that included a comparison of different genetic counseling models (n = 23), outcomes of implementing genetic counseling (n = 11), core resources for genetic counselors (n = 5), and genetic counseling training programs (n = 4). Thirteen of the awards (30.2%) had a specific aim/goal/focus on underserved or underrepresented populations. The topics were predominantly related to cancer (n = 26). Nine awards were led or co‐led by a genetic counselor (20.9%). Despite significant growth in genetic counseling research support from US‐based funders over the past 3 years, major gaps related to funding exist, including that most award recipients are not genetic counselors.
