Abstract:Objectives: Extracting PICO (Populations, Interventions, Comparison, and Outcomes) entities is fundamental to evidence retrieval. We present a novel method, PICOX, to extract overlapping PICO entities. Materials and methods: PICOX first identifies entities by assessing whether a word marks the beginning or conclusion of an entity. Then, it uses a multi-label classifier to assign one or more PICO labels to a span candidate. PICOX was evaluated using 1 of the best-performing baselines, EBM-NLP, and 3 more datasets, ie, PICO-Corpus and randomized controlled trial publications on Alzheimer's Disease (AD) or COVID-19, using entity-level precision, recall, and F1 scores. Results: PICOX achieved superior precision, recall, and F1 scores across the board, with the micro F1 score improving from 45.05 to 50.87 (P ≪.01). On the PICO-Corpus, PICOX obtained higher recall and F1 scores than the baseline and improved the micro recall score from 56.66 to 67.33. On the COVID-19 dataset, PICOX also outperformed the baseline and improved the micro F1 score from 77.10 to 80.32. On the AD dataset, PICOX demonstrated comparable F1 scores with higher precision when compared to the baseline. Conclusion: PICOX excels in identifying overlapping entities and consistently surpasses a leading baseline across multiple datasets. Ablation studies reveal that its data augmentation strategy effectively minimizes false positives and improves precision.

Abstract:Abstract Motivation Automated extraction of population, intervention, comparison/control, and outcome (PICO) from the randomized controlled trial (RCT) abstracts is important for evidence synthesis. Previous studies have demonstrated the feasibility of applying natural language processing (NLP) for PICO extraction. However, the performance is not optimal due to the complexity of PICO information in RCT abstracts and the challenges involved in their annotation. Results We propose a two-step NLP pipeline to extract PICO elements from RCT abstracts: (i) sentence classification using a prompt-based learning model and (ii) PICO extraction using a named entity recognition (NER) model. First, the sentences in abstracts were categorized into four sections namely background, methods, results, and conclusions. Next, the NER model was applied to extract the PICO elements from the sentences within the title and methods sections that include >96% of PICO information. We evaluated our proposed NLP pipeline on three datasets, the EBM-NLPmod dataset, a randomly selected and re-annotated dataset of 500 RCT abstracts from the EBM-NLP corpus, a dataset of 150 Coronavirus Disease 2019 (COVID-19) RCT abstracts, and a dataset of 150 Alzheimer’s disease (AD) RCT abstracts. The end-to-end evaluation reveals that our proposed approach achieved an overall micro F1 score of 0.833 on the EBM-NLPmod dataset, 0.928 on the COVID-19 dataset, and 0.899 on the AD dataset when measured at the token-level and an overall micro F1 score of 0.712 on EBM-NLPmod dataset, 0.850 on the COVID-19 dataset, and 0.805 on the AD dataset when measured at the entity-level. Advancing PICO Element Detection in Biomedical Text via Deep Neural Networks

Abstract:In evidence-based medicine (EBM), defining a clinical question in terms of the specific patient problem aids the physicians to efficiently identify appropriate resources and search for the best available evidence for medical treatment. In order to formulate a well-defined, focused clinical question, a framework called PICO is widely used, which identifies the sentences in a given medical text that belong to the four components typically reported in clinical trials: Participants/Problem (P), Intervention (I), Comparison (C) and Outcome (O). In this work, we propose a novel deep learning model for recognizing PICO elements in biomedical abstracts. Based on the previous state-of-the-art bidirectional long-short term memory (biLSTM) plus conditional random field (CRF) architecture, we add another layer of biLSTM upon the sentence representation vectors so that the contextual information from surrounding sentences can be gathered to help infer the interpretation of the current one. In addition, we propose two methods to further generalize and improve the model: adversarial training and unsupervised pre-training over large corpora. We tested our proposed approach over two benchmark datasets. One is the PubMed-PICO dataset, where our best results outperform the previous best by 5.5%, 7.9%, and 5.8% for P, I, and O elements in terms of F1 score, respectively. And for the other dataset named NICTA-PIBOSO, the improvements for P/I/O elements are 2.4%, 13.6%, and 1.0% in F1 score, respectively. Overall, our proposed deep learning model can obtain unprecedented PICO element detection accuracy while avoiding the need for any manual feature selection.

Abstract:Natural language processing could assist multiple tasks in systematic reviews to reduce workflow, including the extraction of PICO elements such as study populations, interventions, comparators and outcomes. The PICO framework provides a basis for the retrieval and selection for inclusion of evidence relevant to a specific systematic review question, and automatic approaches to PICO extraction have been developed particularly for reviews of clinical trial findings. Considering the difference between preclinical animal studies and clinical trials, developing separate approaches is necessary. Facilitating preclinical systematic reviews will inform the translation from preclinical to clinical research.

Abstract:The rapid growth in published clinical trials makes it difficult to maintain up-to-date systematic reviews, which requires finding all relevant trials. This leads to policy and practice decisions based on out-of-date, incomplete, and biased subsets of available clinical evidence. Extracting and then normalising Population, Intervention, Comparator, and Outcome (PICO) information from clinical trial articles may be an effective way to automatically assign trials to systematic reviews and avoid searching and screening - the two most time-consuming systematic review processes. We propose and test a novel approach to PICO span detection. The major difference between our proposed method and previous approaches comes from detecting spans without needing annotated span data and using only crowdsourced sentence-level annotations. Experiments on two datasets show that PICO span detection results achieve much higher results for recall when compared to fully supervised methods with PICO sentence detection at least as good as human annotations. By removing the reliance on expert annotations for span detection, this work could be used in human-machine pipeline for turning low-quality crowdsourced, and sentence-level PICO annotations into structured information that can be used to quickly assign trials to relevant systematic reviews.

Abstract:The PICO framework (Population, Intervention, Comparison, and Outcome) is usually used to formulate evidence in the medical domain. The major task of PICO extraction is to extract sentences from medical literature and classify them into each class. However, in most circumstances, there will be more than one evidences in an extracted sentence even it has been categorized to a certain class. In order to address this problem, we propose a step-wise disease Named Entity Recognition (DNER) extraction and PICO identification method. With our method, sentences in paper title and abstract are first classified into different classes of PICO, and medical entities are then identified and classified into P and O. Different kinds of deep learning frameworks are used and experimental results show that our method will achieve high performance and fine-grained extraction results comparing with conventional PICO extraction works.

Abstract:The core of evidence-based medicine is to read and analyze numerous papers in the medical literature on a specific clinical problem and summarize the authoritative answers to that problem. Currently, to formulate a clear and focused clinical problem, the popular PICO framework is usually adopted, in which each clinical problem is considered to consist of four parts: patient/problem (P), intervention (I), comparison (C) and outcome (O). In this study, we compared several classification models that are commonly used in traditional machine learning. Next, we developed a multitask classification model based on a soft-margin SVM with a specialized feature engineering method that combines 1-2gram analysis with TF-IDF analysis. Finally, we trained and tested several generic models on an open-source data set from BioNLP 2018. The results show that the proposed multitask SVM classification model based on 1-2gram TF-IDF features exhibits the best performance among the tested models.

Abstract:Hand-labelling clinical corpora can be costly and inflexible, requiring re-annotation every time new classes need to be extracted. PICO (Participant, Intervention, Comparator, Outcome) information extraction can expedite conducting systematic reviews to answer clinical questions. However, PICO frequently extends to other entities such as Study type and design, trial context, and timeframe, requiring manual re-annotation of existing corpora. In this paper, we adapt Snorkel's weak supervision methodology to extend clinical corpora to new entities without extensive hand labelling. Specifically, we enrich the EBM-PICO corpus with new entities through an example of "Study type and design" extraction. Using weak supervision, we obtain programmatic labels on 4,081 EBM-PICO documents, achieving an F1-score of 85.02% on the test set.

Abstract:In recent years, there has been a surge in the publication of clinical trial reports, making it challenging to conduct systematic reviews. Automatically extracting Population, Intervention, Comparator, and Outcome (PICO) from clinical trial studies can alleviate the traditionally time-consuming process of manually scrutinizing systematic reviews. Existing approaches of PICO frame extraction involves supervised approach that relies on the existence of manually annotated data points in the form of BIO label tagging. Recent approaches, such as In-Context Learning (ICL), which has been shown to be effective for a number of downstream NLP tasks, require the use of labeled examples. In this work, we adopt ICL strategy by employing the pretrained knowledge of Large Language Models (LLMs), gathered during the pretraining phase of an LLM, to automatically extract the PICO-related terminologies from clinical trial documents in unsupervised set up to bypass the availability of large number of annotated data instances. Additionally, to showcase the highest effectiveness of LLM in oracle scenario where large number of annotated samples are available, we adopt the instruction tuning strategy by employing Low Rank Adaptation (LORA) to conduct the training of gigantic model in low resource environment for the PICO frame extraction task. Our empirical results show that our proposed ICL-based framework produces comparable results on all the version of EBM-NLP datasets and the proposed instruction tuned version of our framework produces state-of-the-art results on all the different EBM-NLP datasets. PRTA:Joint extraction of medical nested entities and overlapping relation via parameter sharing progressive recognition and targeted assignment decoding scheme

Abstract:Nested entities and relationship extraction are two tasks for analysis of electronic medical records. However, most of existing medical information extraction models consider these tasks separately, resulting in a lack of consistency between them. In this paper, we propose a joint medical entity-relation extraction model with progressive recognition and targeted assignment (PRTA). Entities and relations share the information of sequence and word embedding layers in the joint decoding stage. They are trained simultaneously and realize information interaction by updating the shared parameters. Specifically, we design a compound triangle strategy for the nested entity recognition and an adaptive multi-space interactive strategy for relationship extraction. Then, we construct a parameter-shared information space based on semantic continuity to decode entities and relationships. Extensive experiments were conducted on the Private Liver Disease Dataset (PLDD) provided by Beijing Friendship Hospital of Capital Medical University and public datasets (NYT, ACE04 and ACE05). The results show that our method outperforms existing SOTA methods in most indicators, and effectively handles nested entities and overlapping relationships.
SpanRE: Entities and Overlapping Relations Extraction Based on Spans and Entity Attention

Abstract:Extracting entities and relations is an essential task of information extraction. Triplets extracted from a sentence might overlap with each other. Previous methods either did not address the overlapping issues or solved overlapping issues partially. To tackle triplet overlapping problems completely, firstly we extract candidate subjects with a standard span mechanism. Then we present a labeled span mechanism to extract the objects and relations simultaneously, we use the labeled span mechanism to generate labeled spans whose start and end positions indicate the objects, and whose labels correspond to relations of subject and objects. Besides, we design an entity attention mechanism to enhance the information fusion between subject and sentence during extracting objects and relations. We test our method on two public datasets, our method achieves the best performances on these two datasets.

Abstract:The advancement of medical informatization necessitates extracting entities and their relationships from electronic medical records. Presently, research on electronic medical records predominantly concentrates on single-entity relationship extraction. However, clinical electronic medical records frequently exhibit overlapping complex entity relationships, thereby heightening the challenge of information extraction. To rectify the absence of a clinical medical relationship extraction dataset, this study utilizes electronic medical records from 584 patients in a hospital to create a compact clinical medical relationship extraction dataset. To address the pipelined relationship extraction model's limitation in overlooking the one-to-many correlation problem between entities and relationships, this paper introduces a cascading relationship extraction model. This model integrates the MacBERT pre-training model, gated recurrent network, and multi-head self-attention mechanism to enhance the extraction of text features. Simultaneously, adversarial learning is incorporated to bolster the model's robustness. In scenarios involving one-to-many relationships between entities, a two-phase task is employed. Initially, the main entity is predicted, followed by predicting the associated object and their correspondences. Employing this cascade-structured approach enables the model to flexibly manage intricate entity relationships, thereby enhancing extraction accuracy. Experimental results demonstrate the model's efficiency, yielding F1-scores of 82.8%, 76.8%, and 88.2% for fulfilling relational extraction requirements and tasks on DuIE, CHIP-CDEE, and private datasets, respectively. These scores represent improvements over the benchmark model. The findings indicate the model's applicability in practical domains, particularly in tasks such as biomedical information extraction.

Abstract:Evidence-Based Medicine (EBM) aims to apply the best available evidence gained from scientific methods to clinical decision making. A generally accepted criterion to formulate evidence is to use the PICO framework, where PICO stands for Problem/Population, Intervention, Comparison, and Outcome. Automatic extraction of PICO-related sentences from medical literature is crucial to the success of many EBM applications. In this work, we present our Aceso1 system, which automatically generates PICO-based evidence summaries from medical literature. In Aceso, we adopt an active learning paradigm, which helps to minimize the cost of manual labeling and to optimize the quality of summarization with limited labeled data. An UMLS2Vec model is proposed to learn a vector representation of medical concepts in UMLS,2 and we fuse the embedding of medical knowledge with textual features in summarization. The evaluation shows that our approach is better on identifying PICO sentences against state-of-the-art studies and outperforms baseline methods on producing high-quality evidence summaries.

Abstract:Research profiling is a large-scale analysis method based on the literature information to depict the state-of-the-art scientific researches wisely. Much research profiling work has been carried out based on formal, structured scientific publications (such as Chemical Abstracts) while there is much information contained in abundant, unstructured scientific web resources. In order to profiling research based on those unstructured scientific web resources, the authors bring forth a solution that tries to extract useful knowledge objects from them. Three kinds of knowledge objects have been extracted: (1) research related objects (research objects), such as institutes, scientists, projects, conferences etc.; (2) the relationships between the research objects which reflected in scientific web resources, such as one scientist worked in one institute; (3) the terms which indicate the topic of the research areas. The authors implemented the knowledge objects extraction system and did some experiments to test and evaluate the effect of this system.
Boundaries and edges rethinking: An end-to-end neural model for overlapping entity relation extraction

Abstract:Overlapping entity relation extraction has received extensive research attention in recent years. However, existing methods suffer from the limitation of long-distance dependencies between entities, and fail to extract the relations when the overlapping situation is relatively complex. This issue limits the performance of the task. In this paper, we propose an end-to-end neural model for overlapping relation extraction by treating the task as a quintuple prediction problem. The proposed method first constructs the entity graphs by enumerating possible candidate spans, then models the relational graphs between entities via a graph attention model. Experimental results on five benchmark datasets show that the proposed model achieves the current best performance, outperforming previous methods and baseline systems by a large margin. Further analysis shows that our model can effectively capture the long-distance dependencies between entities in a long sentence.

Learning entity-oriented representation for biomedical relation extraction

Abstract:Biomedical Relation Extraction (BioRE) aims to automatically extract semantic relations for given entity pairs and is of great significance in biomedical research. Current popular methods often utilize pretrained language models to extract semantic features from individual input instances, which frequently suffer from overlapping semantics. Overlapping semantics refers to the situation in which a sentence contains multiple entity pairs that share the same context, leading to highly similar information between these entity pairs. In this study, we propose a model for learning Entity-oriented Representation (EoR) that aims to improve the performance of the model by enhancing the discriminability between entity pairs. It contains three modules: sentence representation, entity-oriented representation, and output. The first module learns the global semantic information of the input instance; the second module focuses on extracting the semantic information of the sentence from the target entities; and the third module enhances distinguishability among entity pairs and classifies the relation type. We evaluated our approach on four BioRE tasks with eight datasets, and the experiments showed that our EoR achieved state-of-the-art performance for PPI, DDI, CPI, and DPI tasks. Further analysis demonstrated the benefits of entity-oriented semantic information in handling multiple entity pairs in the BioRE task.
A Scoping Review of Adopted Information Extraction Methods for RCTs

Abstract:Background: Randomized controlled trials (RCTs) provide the strongest evidence for therapeutic interventions and their effects on groups of subjects. However, the large amount of unstructured information in these trials makes it challenging and time-consuming to make decisions and identify important concepts and valid evidence. This study aims to explore methods for automating or semi-automating information extraction from reports of RCT studies. Methods: We conducted a systematic search of PubMed, ACM Digital Library, and Web of Science to identify relevant articles published between January 1, 2010, and 2022. We focused on published Natural Language Processing (NLP), machine learning, and deep learning methods that automate or semi-automate key elements of information extraction in the context of RCTs. Results: A total of 26 publications were included, which discussed the automatic extraction of key characteristics of RCTs using various PICO frameworks (PIBOSO and PECODR). Among these publications, 14 (53.8%) extracted key characteristics based on PICO, PIBOSO, and PECODR, while 12 (46.1%) discussed information extraction methods in RCT studies. Common approaches mentioned included word/phrase matching, machine learning algorithms such as binary classification using the Naïve Bayes algorithm and powerful BERT network for feature extraction, support vector machine for data classification, conditional random field, non-machine-dependent automation, and machine learning or deep learning approaches. Conclusion: The lack of publicly available software and limited access to existing software makes it difficult to determine the most powerful information extraction system. However, deep learning models like Transformers and BERT language models have shown better performance in natural language processing.

Abstract:Extracting entities and relations from the unstructured text has attracted increasing attention in recent years. The existing work has achieved considerable results, yet it is difficult to solve entity overlap and exposure bias. To address cascading errors, exposure bias, and entity overlap in existing entity relation extraction approaches, we propose a joint entity relation extraction model (SMHS) based on a span-level multi-head selection mechanism, transforming entity relation extraction into a span-level multi-head selection problem. Our model uses span-tagger and span-embedding to construct span semantic vectors, utilizes LSTM and multi-head self-attention mechanism for span feature extraction, multi-head selection mechanism for span-level relation decoding, and introduces span classification task for multi-task learning to decode out the relation triad in a single-stage. Experiments on the classic English dataset NYT and the publicly available Chinese relationship extraction dataset DuIE 2.0 show that this method achieves better results than the baseline method, which verifies the effectiveness of this method. End-to-End Models for Chemical-Protein Interaction Extraction: Better Tokenization and Span-Based Pipeline Strategies

Abstract:End-to-end relation extraction (E2ERE) is an important task in information extraction, more so for biomedicine as scientific literature continues to grow exponentially. E2ERE typically involves identifying entities (or named entity recognition (NER)) and associated relations, while most RE tasks simply assume that the entities are provided upfront and end up performing relation classification. E2ERE is inherently more difficult than RE alone given the potential snowball effect of errors from NER leading to more errors in RE. A complex dataset in biomedical E2ERE is the ChemProt dataset (BioCreative VI, 2017) that identifies relations between chemical compounds and genes/proteins in scientific literature. ChemProt is included in all recent biomedical natural language processing benchmarks including BLUE, BLURB, and BigBio. However, its treatment in these benchmarks and in other separate efforts is typically not end-to-end, with few exceptions. In this effort, we employ a span-based pipeline approach to produce a new state-of-the-art E2ERE performance on the ChemProt dataset, resulting in $> 4\%$ improvement in F1-score over the prior best effort. Our results indicate that a straightforward fine-grained tokenization scheme helps span-based approaches excel in E2ERE, especially with regards to handling complex named entities. Our error analysis also identifies a few key failure modes in E2ERE for ChemProt.
