Abstract:Eight cases of congenital lobar emphysema have been treated in this hospital since 1982. There were 5 males:and 3 females with age ranging from 8 days to 20 months. All presented with aggravating respiratory distress and cyanosis. The diagnosis was confirmed by chest radiography. Seven cases were performed on with lobectomy and all survived. The resected lung showed massive and uniform distention incapable of deflating. Pathological findings were increased alveolar size and cartilage deficiency.
Congenital Rare Abnormal Shapes of Lungs in Relation to Abnormal Lobes and Fissures and Its Clinical Implications

Abstract:Background: Detailed anatomical knowledge of congenital abnormalities of the lungs is essential for proper evaluation and treatment of respiratory diseases, identification of bronchopulmonary segments during lobectomies and surgical resection, and identification and interpretation of their variable radiological imaging. Aim: Accordingly, the aim of this study was designed to evaluate the prevalence of congenital abnormal shapes of lungs in relation to abnormal lobes and fissures. Materials and methods: This study was carried out on 270 isolated lung specimens (right lungs - 135 and left lungs - 135) from 135 formalin-embalmed middle-aged human cadavers irrespective of gender. Results: Out of 270 lung specimens following congenital abnormalities were reported - (1) three lungs (1.1%) had uunusual different abnormal shapes, such as typical snail shell-shaped right lung with single abnormal circular fissure, bearded axe-shaped left lung with huge cardiac notch, and single-lobed classical pointed hat-shaped right lung, (2) one right lung (0.37%) and 9 left lungs (6.67%) showed complete absence of fissures, (3) among 9 single-lobed left lungs, one had split apex separated by a left subclavian groove with the absence of lingula and cardiac notch, (4) cardiac notch and ligula were noted absently in 3 left lungs (2.36%), and (5) one right lung (0.37%) showed five lobes with complete one oblique and three incomplete horizontal fissures. Conclusion: To the best of our knowledge, such abnormalities of the lungs have not been cited in the recent medical literature. Awareness of these abnormalities is necessary to avoid complications during various radiodiagnostic procedures or cardiothoracic surgeries.

Abstract:The pulmonary alveolar proteinosis (PAP) is a kind of causally unknown and infrequent pulmonary diseases. The pathologic feature is that the phospholipoprotein like surfactant, positive while dyed with periodic acid schiff has excessively accumulated in the alveoli cavity and the terminus of the bronchus. In this article, we selected five patients who have probably caught PAP. Vitally examed with the micro bronchoscope,their pulmonary tissue were taken,the ultra thin sections were observed by TEM. Two of these five patients could be excluded from catching PAP and the cellular ultrastructural alteration of the rest was fundamentally the same and corresponded with the pathologic alteration of the pulmonary tissue of PAP. The cellular ultrastructural feature of PAP is that of the typeⅡovergrown of alveolar epithelial cell and the abundence of that cellular chips, pulmonary surfactant, protein like structures and a number of spumous alveolar macrophages in the alveoli cavity and the terminus of the bronchus. The aboundance of these substances lead to the depression of the alveolar function and render the patients dyspnoea. The alteration of these ultrastructures sometimes is difficult to find with the LM. The use of the EM can help greatly in the clinical diagnosis.

Abstract:Key clinical message: Clinicians should think beyond pneumonia and left-to-right shunts when young children have persistent respiratory distress. Congenital lung anomalies, including congenital lobar emphysema, should be considered differential diagnoses. Chest X-ray and CT imaging should be conducted to gain insight regarding and establish diagnosis, respectively. Abstract: Congenital lobar emphysema (CLE) is a rare and life-threatening congenital lung anomaly that often poses a diagnostic dilemma. The issue can be further confused with coexistent congenital heart defects (CHDs) with left-to-right shunts. The clinical presentation of CLE during infancy is similar to that of CHD, with CHD being more common entity, gets detected early. The presence of underlying CLE may only be suspected after CHD repair. In our case, a 2-month-old infant presented with respiratory distress. On evaluation, a large ventricular septal defect and patent ductus arteriosus were detected. After successful cardiac defect repair, the infant continued to experience cough, respiratory distress, and failure to thrive. Furthermore, on radiological reanalysis, CLE was suspected on X-ray and confirmed via computed tomography. CLE can be detected on chest X-rays; however, at an early stage, it is often confusing and misleading.
High comorbidity rates in congenital lobar emphysema and the effect on clinical presentation

Abstract:The presence of comorbidities (CM) in congenital lobar emphysema (CLE) has been previously described with varying rates. However, the clinical implication of CM on the clinical presentation and patient outcome in CLE is unclear. This was a retrospective cohort study between 2000 and 2022 in a single institution. The study included pediatric patients diagnosed with CLE. During the study period, 36 patients were identified with CLE. The presence of respiratory symptoms at diagnosis was documented in 69% (24/35) of the patients with 48% admitted to intensive care units. The presence of CM was documented in 14/36 (39%) of the patients, with cardiovascular anomalies as the most common (22%), followed by genitourinary anomalies (11%) and large airway anomalies (8%). When comparing the CM group ( n = 14) vs the non-CM group ( N = 22), there was no significant difference regarding gender, term birth, age of diagnosis, and the location of the lesion. The CM group had significantly higher rates of intensive care unit (ICU) admissions (75% vs 33%, p = 0.02), longer duration of admission (median 46 days vs 9, p = 0.02), and need for pre-operative non-invasive respiratory support (50% vs 5%, p = 0.002).

Abstract:Congenital lung anomaly (CLA) refers to a rare group of malformations that are typically identified prenatally or in early childhood. However, a significant proportion of cases evade detection until adulthood and either are incidentally discovered or manifest with symptoms of recurrent respiratory infection or pulmonary hemorrhage. While most CLAs have characteristic imaging findings at CT and MRI, they remain a diagnostic challenge due to the infrequency with which they are encountered in adults. Radiologists frequently play a pivotal role in suggesting the diagnosis and guiding appropriate management strategies, and recognition of characteristic imaging patterns is crucial for accurate diagnosis. The authors examine the imaging appearances and clinical manifestations in adult patients with CLA, with a focus on patients who have bronchopulmonary involvement and those with combined bronchopulmonary and vascular anomalies. Entities discussed include bronchogenic cyst, bronchial atresia, congenital lobar overinflation, congenital pulmonary airway malformation, proximal interruption of the pulmonary artery, bronchopulmonary sequestration, hypogenetic lung syndrome, placental transmogrification of the lung, and hybrid lesions. Abstract:Congenital pulmonary airway malformation is a rare congenital abnormality of the lungs. It canmanifest at any age and can lead to significant morbidity and mortality in infants and children. Some individuals with congenital lung malformations may present with respiratory symptoms right after birth, while others may remain asymptomatic for extended periods. We present a case of a 4-year-old female child who experienced recurrent chest infections. Imaging revealed type I congenital pulmonary airway malformation with an underlying infection. Despite the increased risks associated with surgery and the complexity of the disease, the patient underwent a posterolateral thoracotomy with resection of the right lower lobe. The patient achieved successful outcomes and was able to recover successfully following the surgery. This case study holds significance because several studies have focused on the timing and outcomes of surgical intervention in asymptomatic cases, thereremains a lack of consensus regarding symptomatic patients and their outcomes after surgery.

LSC Abstract – Systemic and Local Metabolomics Profiling Reveals Novel Insights into the Progression of Emphysema

Abstract:COPD, the third leading cause of death worldwide, is characterized by small airway remodeling, chronic bronchitis and emphysema, which is the destruction of alveolar structures impairing the ability for gaseous exchange. Metabolomics, a rapidly expanding field, offers a powerful tool for assessing the physiological state of an individual yielding a snapshot of cellular activities closer to the phenotype. To further understand the pathological mechanisms underlying progressive emphysema we used mass spectrometry–based targeted metabolomics approaches to quantitate the lung tissue, BALF and serum metabolome during emphysema progression in the established murine elastase (PPE) model. A single oropharyngeal application of PPE (80U/kg) to B6 mice resulted in a progressive severe emphysema as demonstrated by declining lung function and increased airspace enlargement. PLS analysis revealed greater changes in the metabolome of lung tissue followed by BALF rather than serum during emphysema progression. Furthermore, we demonstrate for the first time that emphysema progression is associated with a reduction in lung specific L-carnitine (1.75 fold), a metabolite critical for energy production and protector against apoptosis. Supplementation of PPE-treated mice with L-carnitine improved lung function compared to PPE only-treated animals (Compliance 0.051±0.006 vs 0.067±0.008 ml/cmH20, p<0.01). Emphysema progression is thus associated with a reduction in the global metabolomic profile, which is compartment specific, with most changes being local to the lung tissue. Importantly, L-carnitine might represent a new significant therapeutic potential for emphysema.
Congenital Cystic Lung Lesions in 52 Children

Abstract:Objective To investigate the diagnosis,operation and outcomes of congenital cystic lung lesions in children.Methods Fifty-two children with congenital cystic lung lesions were collected.The clinical data s were analyzed including manifestations,lesion characteristics,imaging,histopathology,diagnosis,surgical treatment methods and short-term pulmonary functions.Results There were 33 male and 19 female,aged from 1 day to 159 months,their median age was 3.67 months.Five cases were neonates.There were 42 cases of bronchogenic cyst,6 cases of pulmonary sequestration,3 cases of congenital lobar emphysema and 1 case of congenital cystic adenomatoid malformation.The cases commonly manifested by lung infections.Of 52 cases,61.5％ were diagnosed after the first onset of lung infection,21.1％ for repeated lung infections and 17.3％ with no symptom.Right lower lung was the most common lesion site and the cases accounted about for 36.5％.Multiple gas cysts were the most common imaging findings accounting about for 42.3％.The pathologic examination of all cases showed bronchogenic cysts.The missed diagnosis rate was 17.3％.The misdiagnosis rate was 24.9％,which of bronchogenic cyst cases was 19.2％.All cases received the lobectomy without death.Conclusions The preoperative diagnosis of congenital cystic lung lesions is mostly based on imaging findings.Clinical manifestations are useful for identifying early the lesions.The preoperative diagnosis may be easily missed or incorrectly done.Pathological findings contribute to make a definite diagnosis.The surgery effect is satisfactory and the outcome is good.

Abstract:The lungs are paired, soft and spongy organs of respiration present in the greater part of thoracic cavity on either side of mediastinum resting on diaphragm. The right lung is typically divided into three lobes namely superior, middle and inferior by the oblique and horizontal ssures. The left lung is divided into superior and inferior lobes by a single deep oblique ssure. The ssures act as a plane of cleavage covered by extensions of pulmonary pleura that helps in expansion of lungs during respiration. During the routine dissection of thoracic region of human cadaver, right lung with accessory ssures and distinctive lobar pattern was observed. The right lung was divided into ve disproportionate lobes by four ssures. The left lung had normal anatomy without any variation in this case. The variations in the ssures and lobar pattern of lungs are not uncommon and are due to altered developmental process during intrauterine life. The anatomical knowledge of these variations is important for clinicians prior to performing any pulmonary surgical procedures and to prevent postoperative complications.

Abstract:Abstract Background and Aims Congenital heart disease (CHD) can cause several abnormalities of the lungs, such as pulmonary overflow (PO), pulmonary hypertension and chest infection like pneumonia. We aimed to assess pulmonary abnormalities using lung ultrasound (LUS) in children with congenital heart diseases. Patients and Methods This is a cross-sectional study, which included (33) CHD patients aged between 0.5 to 3.5 years who were admitted to the Cardiology Unit,Ain Shams University, Children’s Hospital. The patients were subjected to detecting any consolidation and measuring B vertical lines in all lung zones. LUS score was recorded at baseline and after course of management. Results There was statistically reduction in the after management group compared to the before management group according to LUS score with p-value (p < 0.001). There was a difference between types of management according to LUS score and its severity in the before management group, with p-value (p < 0.001) whereas, surgical type of management had the highest mean value of the LUS score compared to other types of management among our studied patients before management with a highly statistically significant difference, with p-value (p < 0.001). Conclusion Lung ultrasound is an ideal tool for the diagnosis and follow-up of pulmonary complications such as lung congestion, pulmonary edema and pneumonia. Also, there was improvement in pulmonary congestion after management among CHD children.
Congenital cystic lung lesions: diagnosis and surgical management

Abstract:Purpose: To review the thirteen-year-experience of diagnosis and surgical management for congenital cystic lung disease in children. Methods: From January 1993 to May 2006, 52 patients with congenital cystic lung disease underwent surgical treatment, including lung cyst in 37 patients, congenital lobar emphysema in 6 and pulmonary sequestration in 10 (intralobar in 3 and extralobar in 7). There were 26 males and 26 females. Mean age at operation was 3.5 years and 16 patients were under one month of age. The preoperative diagnosis was on the basis of history, clinical presentation, chest X-ray and/or computerized tomography. Among the 37 patients with lung cyst, 27 underwent lobectomy, 2 had wedge resection and 8 patients required excision of the cyst. Five of six patients with congenital lobar emphysema presented with respiratory distress preoperatively thus required emergent operation. For patients with intralobar and extralobar sequestration, lobectomy and sequestrectomy were performed respectively. Results: There were no operation deaths. Early postoperatively, one patient presented with recurrent pneumothorax, one had mild pleural effusion and two had pneumonia. All the patients recovered well. During follow-up, there was recurrent pulmonary cyst in one patient, who underwent left lower lobectomy three years after the previous operation. Others were doing well and maintained good lung function. Conclusions: Early diagnosis and prompt management are required for congenital cystic lung disease in children. Surgical resection, commonly lobectomy, is the main treatment for this lesion. Urgent or emergent operation is needed in symptomatic patients with respiratory distress. An excellent lung function following surgery can be expected.

Abstract:Congenital pulmonary airway malformations (CPAM) compose the major part of congenital lung malformations (CLM) and have traditionally been treated by pulmonary lobectomy. In terms of surgical strategy, lobectomy has conventionally been the preferred treatment for CPAM localized to a single lobe. More recently, alternative approaches including lung-sparing resections (LSR), such as wedge or non-anatomic resections and segmentectomy, have been suggested. In asymptomatic CPAM early surgical resection is often shown to reduce infection and malignancy development. We describe two patients who were diagnosed with CPAM when being evaluated for respiratory tract infection. Patient 1 (P1) was a two-month-old infant weighing 4 kg with glucose-6-phosphate dehydrogenase (G6PD) deficiency and Patient 2 (P2) was a toddler aged one year, nine months weighing 9 kg. P1 underwent LSR for the CPAM diagnosed in the left upper lobe of the lung with conventional mechanical ventilation whilst right upper lobectomy was performed in P2 using one/single lung ventilation. In both cases, LSR and right upper lobectomy led to an uneventful postoperative recovery with no complications reported.

Abstract:Purpose: To study the biological relationship between congenital lung malformations (CLMs) and malignancy. Methods: Biopsies of 12 CPAMs, 6 intralobar sequestrations and 2 extralobar sequestrations were analyzed through whole-genome sequencing. Blood samples from 10 patients were used to confirm or exclude somatic mosaicism. Putative somatic Single Nucleotide Variants (SNVs) were called for each malformed sample with a Panel of Normals built with control DNA samples extracted from blood. The variants were subsequently confirmed by Sanger sequencing and searched, whenever possible, in the blood samples of patients. Results: All CLMs but one presented a signature of genomic instability by means of multiple clusters of cells with gene mutations. Seven tumor transformation-related SNVs were detected in 6/20 congenital lung malformations. Four very rare in the general population SNVs were found in a region previously linked to lung cancer in 5p15.33, upstream of TERT oncogene. Furthermore, we identified missense genetic variants, whose tumorigenic role is well known, in the RET, FANCA and MET genes. Conclusions: Genomic instability in 95% of CLMs and genetic variants linked to tumor development in 30% of them, regardless of histopathology, are predisposing factors to malignancy, that combined with exposure to carcinogens, might trigger the development of malignancy and explain the association between CLMs and lung cancer.
A case of congenital bronchial atresia with tracheobronchial stenosis caused by emphysema: Successful management with thoracoscopic surgery

Abstract:Introduction: Congenital bronchial atresia (CBA), as a rare developmental abnormality of the lung, is usually asymptomatic and is accidently discovered in most cases. Currently, no standardized guidelines for the treatment or management of CBA have been established. Case presentation: A 22-year-old male soldier was referred to Shanghai Changhai Hospital, The First Affiliated Hospital of Naval Medical University due to chest tightness and shortness of breath after repeated strenuous activities. Contrast-enhanced computed tomography (CT) revealed an 18mm × 11mm solitary, well-circumscribed, and solid nodule with no enhancement in the right upper lobe (RUL), and emphysematous changes distributed throughout the RUL. A flexible bronchoscopic examination showed extrinsic compression stenosis in the bronchial opening of the right middle lobe (RML). After three-dimensional (3D) reconstruction CT and a multidisciplinary consultation, a diagnosis of CBA in the anterior segment (B3) of RUL was established. Subsequently, thoracoscopic right upper lobectomy was performed and resulted in an improved respiratory capacity 6 months after surgery. To date, the patient has good quality of life without any complication. Conclusion: This study underscores the role of bronchoscopy, 3D reconstruction CT, and a multidisciplinary consultation in the diagnosis of CBA, and highlights that a thoracoscopic intervention should be considered in such case.

Abstract:Congenital lung malformation (CLM) is a leading cause of infant mortality. Clinical methods for diagnosing CLM mainly rely on computed tomography, magnetic resonance imaging, ultrasonography, and Doppler. However, forensic identification of the cause of death in neonates is challenging. Unequivocal classification criteria for CLM are missing as its forensic identification is ambiguous. Therefore, we aimed to analyze neonatal death cases at our center to assist in identifying those with congenital lung malformation. This retrospective study identified and classified the causes of deaths of neonates autopsied between January 2008 and April 2023. All cases born alive and died within 28 days with a clear time of death were selected, and forensic experts reviewed their records. The manner, cause of death, and other characteristics were noted and discussed. This retrospective study reveals a steady increase in autopsy cases from 2008 to 2015, attributed to improved parental consent, heightened awareness of autopsy importance, and enhanced medical resources. However, a subsequent decline post-2015 is observed, potentially influenced by advancements in medical technology and prenatal examination protocols. The top causes of neonatal mortality include respiratory diseases, asphyxia, congenital dysplasia, and fetal distress. Congenital lung malformations, particularly bronchopulmonary malformations, constitute a significant portion of congenital anomalies. This study underscores the importance of standardized autopsies and histopathological examinations in diagnosing and understanding CLM. Future research should focus on expanding case collections and elucidating the genetic basis of CLM to improve forensic management and outcomes.

Congenital interstitial lung diseases: What the anesthesiologist needs to know

Abstract:Congenital interstitial lung diseases can affect both adults and children. Pediatric congenital interstitial lung diseases generally carry high risk for morbidly and mortality and include congenital alveolar capillary dysplasia with misalignment of pulmonary veins, congenital alveolar dysplasia, acinar dysplasia, congenital pulmonary lymphangiectasis, diffuse pulmonary lymphangiomatosis, neuroendocrine cell hyperplasia of infancy, pulmonary hemosiderosis, pulmonary alveolar proteinosis, and pulmonary interstitial glycogenosis. Given their usual non-specific clinical presentation, they are frequently misdiagnosed and recognized late, particularly in children who have been apparently healthy for several years (eg, diffuse pulmonary lymphangiomatosis). Some diseases have a very poor prognosis, whereas others have a benign course with appropriate treatment. The current manuscript reviews congenital interstitial lung diseases that typically affect neonates and young children and may be encountered by the pediatric anesthesiologist.

Abstract:BACKGROUND: The major sites of obstruction in chronic obstructive pulmonary disease (COPD) are small airways (<2 mm in diameter). We wanted to determine whether there was a relationship between small-airway obstruction and emphysematous destruction in COPD.METHODS: We used multidetector computed tomography (CT) to compare the number of airways measuring 2.0 to 2.5 mm in 78 patients who had various stages of COPD, as judged by scoring on the Global Initiative for Chronic Obstructive Lung Disease (GOLD) scale, in isolated lungs removed from patients with COPD who underwent lung transplantation, and in donor (control) lungs. MicroCT was used to measure the extent of emphysema (mean linear intercept), the number of terminal bronchioles per milliliter of lung volume, and the minimum diameters and cross-sectional areas of terminal bronchioles.RESULTS: On multidetector CT, in samples from patients with COPD, as compared with control samples, the number of airways measuring 2.0 to 2.5 mm in diameter was reduced in patients with GOLD stage 1 disease (P=0.001), GOLD stage 2 disease (P=0.02), and GOLD stage 3 or 4 disease (P<0.001). MicroCT of isolated samples of lungs removed from patients with GOLD stage 4 disease showed a reduction of 81 to 99.7% in the total cross-sectional area of terminal bronchioles and a reduction of 72 to 89% in the number of terminal bronchioles (P<0.001). A comparison of the number of terminal bronchioles and dimensions at different levels of emphysematous destruction (i.e., an increasing value for the mean linear intercept) showed that the narrowing and loss of terminal bronchioles preceded emphysematous destruction in COPD (P<0.001).CONCLUSIONS: These results show that narrowing and disappearance of small conducting airways before the onset of emphysematous destruction can explain the increased peripheral airway resistance reported in COPD.
