Impact of Renal Allograft Nephrectomy on Graft and Patient Survival Following Retransplantation: a Systematic Review and Meta-Analysis

Abstract:Background:It is not clear whether renal allograft removal affects the outcome of renal retransplantation. This study aimed to determine the effect of allograft nephrectomy (AN) and no-AN (No AN) on renal retransplantation.Methods:A systematic review and meta-analysis were conducted using MEDLINE, Embase and the Cochrane Library. Observational studies or randomized controlled trials including renal retransplantation recipients with AN or No-AN were included. The primary outcomes were graft survival, patient survival, acute rejection (AR) and delayed graft dysfunction; the secondary outcomes were positive panel reactive antibody rate and serum creatinine level at 1 year after retransplantation, cold ischemia time and time of hemodialysis before recent transplantation. Pooled estimates of odds ratios (ORs) and the weighted mean difference for outcomes were calculated.Results:A total of 13 studies divided into 20 trials including 1923 patients were analyzed. The No-AN group had a significantly higher 3-year graft survival rate {OR 0.48 [95% confidence interval (CI) 0.34-0.69], 10 studies, n = 1030} and 5-year graft survival rate [OR 0.65 (95% CI 0.44-0.97), 16 studies, n = 1878] than the AN group. The rates of 5-year patient survival [OR 1.82 (95% CI 1.14-2.90), 5 studies, n = 749], positive panel reactive antibody [OR 3.08 (95% CI 2.08-4.56), 12 studies, n = 1225], AR [OR 1.59 (95% CI 1.21-2.09), 15 studies, n = 1388] and delayed graft dysfunction [OR 1.66 (95% CI 1.20-2.03), 8 studies, n = 879] were all significantly higher in the AN group. Compared with the No-AN group, cold ischemia time was longer in the AN group [weighted mean difference 1.84 (95% CI 0.90-2.79), 7 studies, n = 919]. The rate of 1-year graft survival and 10-year graft survival, serum creatinine levels at 1 year after retransplantation and the time of hemodialysis before recent transplantation were similar between the AN and No-AN groups.Conclusions:We recommend allowing the failed graft to remain unless symptoms dictate the need for surgery. We also suggest donor-specific antibody dynamic monitoring and better human leukocyte antigen matching for improved long-term outcome of retransplantation.
Relationship between blood pressure and renin, angiotensin II and atrial natriuretic factor after renal transplantation

Abstract:The objective of the study was to assess the evolution of renin, angiotensin II, atrial natriuretic factor (ANF) and blood pressure (BP) in the first trimester following renal transplantation in man Thirty-two recipients were investigated for 3 months post-transplantation. Twenty had a history of hypertension with moderate cardiac hypertrophy. Thirty-one retained their native kidneys. Five kidney donors had a history of mild hypertension. The recipients were perioperatively volume-expanded with 0.9% saline and diuresis was maintained for 48 h with furosemide and dopamine. The sodium intake was 25 mEq/24 hours. Prophylactic immunosuppressive therapy was antilymphocyteglobulins (25 cases), or anti-LFA1 (7 cases) and maintenance therapy was cyclosporine-prednisone (8 cases), or cyclosporine-prednisone-azathioprine (24 cases). Mean BP, serum creatinine, urinary sodium excretion (UNA) and hormonal (renin, angiotensin II and ANF) parameters were collected every other day for the first week after transplantation and then twice monthly. Twenty (62.5%) patients developed hypertension and hypertension was more frequent in patients with a delayed graft function, than in patients with immediate good graft function (10/20 vs. 4/12, p < 0.05%). Both hypertensive (group HBP) and normotensive (group NBP) patients had similar very low renin and angiotensin II plasma levels, after an initial early peak. Analysis of covariance with multiple regression analysis showed that in the HBP patients, BP was negatively correlated with UNA (p = 0.02) and positively with plasma ANF (p < 0.01). The normal BP patients also showed a correlation between BP and UNA, although it was limit of statistical significance (p = 0.05); there was no correlation between ANF and BP. We conclude that the RAS is rapidly depressed after renal transplantation and does not interfere with BP regulation. The hypertension in the early stage of post-transplantation varies inversely with the urinary sodium excretion. The defective sodium excretion, which dominates the effect of the low sodium diet, results in volume overload, increased ANF and volume-dependent hypertension.
Blood Pressure, Chronic Kidney Disease Progression, and Kidney Allograft Failure in Kidney Transplant Recipients: A Secondary Analysis of the FAVORIT Trial

Abstract:Background: In chronic kidney disease, intensive systolic blood pressure (SBP) control reduces mortality at a cost of greater acute kidney injury risk. Kidney transplantation involves implantation of denervated kidneys and immunosuppressive medications that increase acute kidney injury risk. The optimal blood pressure (BP) target in kidney transplant recipients (KTRs) is uncertain. Prior observational studies from the Folic Acid for Vascular Outcome Reduction in Transplantation (FAVORIT) trial demonstrate associations of lower SBP levels and reduced mortality risk, but the relationship of BP with kidney allograft function remains unknown. Thus, in FAVORIT, we investigated the relationship of SBP and diastolic blood pressure (DBP) with risk of kidney allograft failure and estimated glomerular filtration rate (eGFR) slope among stable KTRs. Methods: Cox proportional hazards and multivariable linear regression models adjusted for demographics, transplant characteristics, comorbidities, baseline eGFR, and urine albumin-to-creatinine ratio were used to determine associations of SBP and DBP with time to a composite kidney outcome of ≥50% eGFR decline or dialysis dependence, and with annualized eGFR change, respectively. Multivariable restricted cubic spline plots were developed to evaluate the functional form of the relationships. Results: Among 3,598 KTRs, mean age was 52 ± 9 years, SBP was 136 ± 20 mm Hg, DBP was 79 ± 12 mm Hg, and eGFR was 49 ± 18 ml/minute/1.73 m2. There were 369 events of ≥50% eGFR decline or dialysis dependence during a mean follow-up of 4.0 ± 1.5 years. There was no association of either SBP (compared with SBP 120 to <130 mm Hg, hazard ratio (HR) for the SBP < 110 was 1.01 (95% confidence interval (CI) 0.60 to 1.70) and 130 to <140 was 0.89 (0.64 to 1.24)) or DBP (compared with DBP 70 to <80 mm Hg, HR for the DBP 60 to <70 was 1.00 (95% CI 0.74 to 1.34) and 80 to <90 was 0.90 (0.68 to 1.18)) with the kidney failure outcome or annualized eGFR slope, and, when examined using restricted cubic splines, there was no evidence of "J"- or "U"-shaped relationships. Conclusions: In a large sample of stable KTRs, we found no evidence of thresholds at which lower BPs were related to higher risk of allograft failure or eGFR decline. In light of prior findings of mortality benefit at low SBP, these observational findings suggest lower BP may be beneficial in KTRs. This important question requires confirmation in future randomized trials in KTRs.
Longitudinal Assessment of Renal Perfusion and Oxygenation in Transplant Donor-Recipient Pairs Using Arterial Spin Labeling and Blood Oxygen Level-Dependent Magnetic Resonance Imaging

Abstract:Objectives: The aims of this study were to assess renal function in kidney transplant recipients and their respective donors over 2 years using arterial spin labeling (ASL) and blood oxygen level-dependent (BOLD) magnetic resonance imaging (MRI) and to prospectively evaluate the effect of losartan on functional MRI measures in recipients. Materials and methods: The study included 15 matched pairs of renal transplant donors and recipients. Arterial spin labeling and BOLD MRI of the kidneys were performed on donors before transplant surgery (baseline) and on both donors and recipients at 3 months, 1 year, and 2 years after transplant. After 3 months, 7 of the 15 recipients were prescribed 25 to 50 mg/d losartan for the remainder of the study. A linear mixed-effects model was used to evaluate perfusion, R2*, estimated glomerular filtration rate, and fractional excretion of sodium for changes across time or associated with losartan treatment. Results: In donors, cortical perfusion in the remaining kidney decreased by 50 ± 19 mL/min per 100 g (11.8%) between baseline and 2 years (P < 0.05), while cortical R2* declined modestly by 0.7 ± 0.3 s-1 (5.6%; P < 0.05). In transplanted kidneys, cortical perfusion decreased markedly by 141 ± 21 mL/min per 100 g (34.2%) between baseline and 2 years (P < 0.001), while medullary R2* declined by 1.5 ± 0.8 s-1 (8.3%; P = 0.06). Single-kidney estimated glomerular filtration rate increased between baseline and 2 years by 17.7 ± 2.7 mL/min per 1.73 m (40.3%; P < 0.0001) in donors and to 14.6 ± 4.3 mL/min per 1.73 m (33.3%; P < 0.01) in recipients. Cortical perfusion at 1 and 2 years in recipients receiving 25 to 50 mg/d losartan was 62 ± 24 mL/min per 100 g higher than recipients not receiving the drug (P < 0.05). No significant effects of losartan were observed for any other markers of renal function. Conclusions: The results suggest an important role for noninvasive functional monitoring with ASL and BOLD MRI in kidney transplant recipients and donors, and they indicate a potentially beneficial effect of losartan in recipients.
Impact of Renin-Angiotensin System Blockade on Mortality and Allograft Loss among Renal Transplant Recipients: A Systematic Review and Meta-Analysis

Abstract:Introduction:The blockade of the renin-angiotensin system (RAS) has a beneficial effect on reducing the levels of proteinuria and blood pressure in patients with chronic kidney disease (CKD) and reduces the risk of developing end-stage kidney disease (ESKD) in CKD patients. Nonetheless, a debate persists regarding the impact of RAS inhibitors on outcomes such as mortality and graft survival in renal transplant patients. To assess the effect of RAS inhibitors on graft recipients in the past decade, we conducted a systematic review and meta-analysis. Methods: We searched Embase, PubMed, and the Cochrane Central Register of Clinical Trials from January 1, 2012, to August 1, 2022. We included 14 articles, comprising 5 randomized controlled trials (RCTs) and 9 cohort studies, including 45,377 patients. These studies compared patient or graft survival between an RAS inhibitors treatment arm and a control arm. Results: The meta-analysis revealed that RAS blockade was significantly associated with lower mortality in cohort studies (risk ratio [RR] = 0.66, 95% confidence interval [CI]: 0.55-0.79), reduced allograft loss in cohort studies (RR = 0.62, 95% CI: 0.54-0.71), and significant changes in systolic blood pressure in RCTs. Subgroup analysis of the groups of interest (interventions involving RAS blockade, follow-up period of ≥ 5 years) showed consistently reduced mortality (RR = 0.67, 95% CI: 0.56-0.81) and reduced allograft loss (RR = 0.61, 95% CI: 0.54-0.70). Conclusions: Our results demonstrated that the application of RAS blockade among renal transplant recipients was associated with lower mortality and allograft loss in cohort studies but not in RCTs. More powered clinical trials are needed to evaluate the effects of RAS blockade in renal transplant recipients.

Protocols to eliminate tuning penalties for packet-switched WDM star networks with large tuning latency

Abstract:15 Abstract. The use of low-speed tunable devices may lead to throughput and delay penalties for packet-switched WDM star networks. When the tuning latency is significant, conventional cost-effective solutions result in significant throughput and delay penalties. Our approach: (i) combines and unifies existing approaches to improve performance by pipelining multiple tunable filters per node and making a TDM schedule with maximum time gap for reception; (ii) introduces 'superpackets' (multiple packets with the same source and destination), thereby relaxing the impact of large tuning latency. Since the superpackets increase throughput at the cost of increased delay, we determine the minimum delay, under a constraint of no throughput penalty. A discrete-time queueing model of the protocol shows that a system with two tunable filters per node has low delay and is wavelength-efficient for uniform traffic.

Abstract:Pharmacoepidemiology is the study of the use and the effect of drugs in large numbers of people. The work relies largely on standard epidemiological principles, which, applied in a new context, bring particular constraints and opportunities. In January 1994, a cluster of children with cystic fibrosis (CF) who developed strictures in the colon was reported. It was suggested that these may have been associated with high strength pancreatic enzyme preparations which had been introduced into use in these patients approximately one year previously. This hypothesis has been evaluated by a case-control study which confirms the association with high-strength pancreatic enzyme usage. The study is described and is used to illustrate statistical issues to provide a framework for future research in statistical methodology in pharmacoepidemiological case-control studies.
[Completely Reversed Acute Rejection Episodes Do Not Influence the Long-Term Renal Allograft Survival].

Abstract:OBJECTIVE:To assess the influence of times and duration of acute rejection episodes and the effect of antirejection therapy in renal transplantation recipients on the long-term survival of renal allograft.METHODS:The clinical data of 946 patients who received renal transplantation were analyzed to analyze the survival of renal allograts in different conditions: times of rejection episode, time of onset of acute rejection, and effect of antirejection therapy by life table and Wilcoxon test, and to identify the risk factors through Cox regression analysis.RESULTS:During the follow-up with a range of 3 approximately 158 months acute rejection occurred in 172 patients. The 946 cases of kidney transplantation were divided into rejection free group (NAR, n = 774), one time rejection group (1AR, n = 159), and twice and more rejection group (2AR, n = 13) according to the times of rejection. The 1AR group was subdivided into completely reversed group (CAR) and incompletely reversed group (1AR). The 1AR cases were subdivided into early-stage rejection group (EAR, with episode within 90 days after transplantation, n = 112) and late-stage rejection group (LAR, with episode 90 days later transplantation, n = 47) according to the onset time of rejection episode. The five-year survival rate was 70.9% in the AR group (n = 172) and was 93.3% in the NAR group (P < 0.000 1). The ten-year survival rate of renal allografts was 29.8% in the AR group, and was 83.3% in the NAR group (P < 0.000 1). The 5-year survival rate of renal allograft was less than 30% in the 2AR group, significantly lower than those in the NAR and IAR groups (P < 0.000 1 and P< 0.003). The 5-year survival rate of renal allograft was 89.0% in the EAR group, significantly higher than that in the LAR group (48.9%, P < 0.000 1). The 8-year survival rate was 84.3% in the EAR group, significantly higher than that in the LAR group (32.1%, P < 0.000 1). Both the survival rates of renal allograft in the EAR and LAR groups were significantly lower than that in the NAR group (P = 0.025 and P < 0.000 1). The condition had been completely reversed in 95 patients and incompletely reversed in 54 patients, and failed to be improved in 10 patients out of the 159 cases in the IAR group after antirejection therapy. The 5-year survival rate of renal allograft was 93.9% in the CAR group, significantly higher than that in the IAR group (63.1%, P < 0.000 1) but not significantly different from that in the NAR group (P = 0.96). The 8-year survival rate of renal allograft was 89.2% in the CAR group, significantly higher than that in the IAR group (41.4%, P < 0.000 1) but not significantly different from that in the NAR group (P = 0.96). The time of rejection onset was not the main factor effecting survival rate of grafts. The main risk factor influencing the long-term survival of renal allograft was the therapeutic effect after the onset of rejection with a risk rate of 3.14.CONCLUSION:The acutely rejected renal allografts have poor long-term survival. The long-term survival rate of renal allograts with acute rejection that occurs only once and is completely recovered after antirejection therapy is not significantly different from that of the renal allografts in NAR group.
Impact of Acute Rejection Episodes on Long-Term Renal Allograft Survival

Abstract:OBJECTIVETo assess the impact of the number, and time of acute rejection (AR) and outcome of anti-rejection therapy on the long-term survival of renal allografts and the relative risk factors.METHODSThe Kaplan-Meier analysis and log-rank test were used to calculate the survival rates of patients and grafts in no acute rejection group (NAR, 895 patients), 1 rejection episode group (1AR, 183), 2 and more than 2 rejection episodes group (2AR, 17), acute rejection group [AR (1AR + 2AR), 200], early acute rejection group (within 90 days after transplantation, EAR, 125), late acute rejection group (91 days later, LAR, 58), completely AR reversed group (CAR, 105), and incompletely AR reversed group (IAR, 68). The relative risk factors were analyzed by the Cox proportional hazards regression.RESULTSThe 5- and 10-year survival rates of renal allografts were 75.4% and 17.1% in AR and 93.2% and 86.5% in the NAR group (P < 0.0001). The long-term graft survival was much lower in the 2AR group than in the NAR or 1AR groups (P < 0.0001 and P = 0.002, respectively). It was similar in either the NAR or CAR groups (P = 0.31), but it was significantly lower (P < 0.0001) in the IAR group. Multivariate Cox regression analysis revealed that the outcome of anti-rejection therapy is an important risk factor affecting the long-term survival of allografts.CONCLUSIONSAR is significantly associated with poor long-term survival of renal allografts. But the long-term graft survival of patients with one acute rejection but completely reversed is not significantly different from that of patients without acute rejection.

Abstract:Background: Renal transplant dysfunction has been shown to be independent predictor for premature cardiovascular disease and mortality. Renalase, a flavoprotein secreted by several tissues, including the kidney, has been found to regulate sympathetic tone and blood pressure. The purpose of this secondary analysis was to explore relationships among parameters of endothelial dysfunction, lipids, glomerular filtration rate, and renalase in 2 groups: renal transplant patients with controlled hypertension and healthy volunteers. Methods: In the parent study, 73 renal transplant recipients and 32 age- and gender-matched controls were enrolled. A fasting sample for endothelial, lipid, and renalase values, along with other clinical parameters, was obtained. Results: We found statistically significant inverse correlation between renalase and estimated glomerular filtration rate ( r = −0.552, P < .001), positive correlation between renalase and creatinine ( r = 0.364, P = .003), total cholesterol ( r = 0.578, P < .001), low-density lipoprotein cholesterol ( r = 0.261, P = .046), and non-high-density lipoprotein cholesterol ( r = 0.327, P = .01). Renalase inversely correlated with hemoglobin ( r = −0.232, P = .032) and positively with white blood cells ( r = 0.233, P = .032). There was a significant difference in plasma renalase with regard to chronic kidney disease stages ( F = 13.346, P < .001) but did not correlate with C-reactive protein. Renalase did not correlate with any of parameters of endothelial dysfunction, C-reactive protein, neither with some demographic data (gender, age, time or type of transplantation, risk factors). There were no differences in renalase concentration with regard to antihypertensive therapy. Conclusion: Renalase strongly and inversely correlated with kidney function, positively with creatinine and lipid disturbances. Due to that it is very likely that renalase levels are determined mostly by renal function.

Abstract:The growing role of mineralocorticoid receptor antagonists among kidney transplant recipients. Background The fundamental role of the renin–angiotensin–aldosterone system in the pathophysiology of chronic kidney disease, congestive heart failure, hypertension and proteinuria is well established in pre‐clinical and clinical studies. Mineralocorticoid receptor antagonists are among the primary options for renin–angiotensin–aldosterone system blockage, along with angiotensin‐converting enzyme inhibitors and angiotensin receptor blockers. Methods In this narrative review, we aim to evaluate the efficiency and safety of mineralocorticoid receptor antagonists in kidney transplant recipients, including the potential underlying pathophysiology. Results The efficiency and safety of mineralocorticoid receptor antagonists in managing chronic kidney disease and proteinuria, either non‐nephrotic or nephrotic range, have been demonstrated among nontransplanted patients, though studies investigating the role of mineralocorticoid receptor antagonists among kidney transplant recipients are scarce. Nevertheless, promising results have been reported in pre‐clinical and clinical studies among kidney transplant recipients regarding the role of mineralocorticoid receptor antagonists in terms of ischaemia–reperfusion injury, proteinuria, or calcineurin inhibitor‐mediated nephrotoxicity without considerable adverse events such as hypotension, hyperkalaemia or worsening renal functions. Conclusion Even though initial results regarding the role of mineralocorticoid receptor antagonist therapy for kidney transplant recipients are promising, there is clear need for large‐scale randomized clinical trials with long‐term follow‐up data.

Abstract:Introduction Patients with end stage renal disease (ESRD) are largely excluded from clinical trials concerned with heart failure (HF) therapeutics, leading to a dearth of quality evidence of guideline directed medical therapy (GDMT) in this high-risk group. Furthermore, renin angiotensin aldosterone system inhibition (RAASi) is generally avoided in hemodialysis patients due to potential side effects including hypotension and hyperkalemia, possibly negating the known benefits of these medications. We studied the prognostic significance of RAASi in HF patients with ESRD on hemodialysis. Methods In a retrospective study protocol, we studied patients followed in our HF clinic who had concomitant ESRD on hemodialysis. Individual and combined RAASi use was registered, and baseline clinical, hemodynamic, and echocardiographic data were assessed. Patients were followed for a median of 31.8 months for cardiac mortality and HF hospitalizations. Results From 2014 to 2022, 89 patients were included for analysis [age: 62.4±15 years, 43% women, EF: 46±15%, 39 (45%) reduced EF (HFrEF), and 60 (55%) preserved EF (HFpEF)]. During follow-up, 14 (16%) patients died, and 61 (69%) patients had HF hospitalizations. 56 (63%) patients were on RAASi (ACEi, ARBs, ARNI), with no significant differences in baseline characteristics (e.g. age, sex, cause of cardiomyopathy, risk factors, device therapy, AHA/ACC class, BNP, baseline EF) (44.1±14.7 vs. 48.7±15, p=0.167) or follow-up EF (44.7±14 vs. 50±15.7, p=130) compared to those not on RAASi. Although RAASi patients had more hypotension (48% vs. 20%, p=0.014), RAASi-related adverse events (cough, angioedema, hyperkalemia, intradialytic hypotension, intradialytic inotrope) were similar between both groups. Compared to patients not on RAASi, RAASi patients had statistically significant increase in HF hospitalizations [44 (79%) vs. 17 (52%), p=0.004), but were associated with a trend towards fewer deaths [6(11%) vs. 8(24%), p=0.09]. Both HFrEF patients and HFpEF patients had comparable RAASi use (55% vs. 69%, p=0.162), and did not differ in outcomes of mortality (18% vs. 14%, p=0.679) and rehospitalization (73% vs. 65%, p=0.467). Moreover, on survival analysis, RAASi had significantly less risk for mortality (HR: 0.33, 95% CI 0.11 to 0.97, Figure 1A,C) but no differences in hospitalization (HR: 1.72, 95% CI: 0.98 to 3, Figure 1B,C). Similar results were obtained after adjusting for co-variates. Conclusions In our study, although hypotension was more common in patients receiving RAASi, hyperkalemia and intradialytic hypotension were not associated with RAASi use. If tolerated, RAASi was associated with improved mortality, but not hospitalization.

[Surgical therapy of obstructive sleep apnea syndromes: results of the Ulm treatment program].

Abstract:The article reports on the results obtained by uvulopalatopharyngoplastic surgery (= UPPPS) in 31 patients suffering from obstructive sleep apnoea syndrome (= OSAS) and in 9 patients with habitual or obstructive snoring. All patients were subjected to thorough preoperative examination including rhinometry, nasopharyngeal video-endoscopy, radiocephalometry and polysomnography. In 8 out of 9 (89%) of the habitually snoring patients and in 17 of 31 (55%) of OSAS patients, surgery was successful. In these patients, a postoperative respiratory disturbance index (RDI) of less than 10 was recorded, whereas in 4 patients (13%) the postoperative RDI was between 10 and 15. Ten patients did not respond satisfactorily to UPPPS (32%). RDI was postoperatively still above 15. Analysis of patient data shows that almost all habitually snoring patients and the OSAS patients with slight overweight and low to medium apnoea index were successfully treated with UPPPS. In accordance with these results a treatment concept has been drawn up for OSAS patients that takes stock of all the possibilities and limitations of available conservative and surgical treatment methods.
Alcohol use patterns of first-year medical students: II. Psychosocial characteristics associated with drinking level.

Abstract:Psychosocial characteristics that identified first-year medical students who drank more or less alcohol than their peers were examined. Subjects were 116 freshman medical students (96% of one class) who completed questionnaires about alcohol use, mood, personality, social relationships, and parental alcohol abuse on the first day of school, and 106 (88% of the class) who responded to the same questionnaires in April of the first year. Heavier drinking students (i.e., the upper 25% of the sample) were more likely to be male and single, to have drunk heavily prior to medical school, to come from larger social networks which include fewer relatives, and to demonstrate a lower interpersonal orientation. Mood, academic performance, and parental alcohol abuse were not associated with drinking level. The heaviest drinkers in this sample appear to be precisely those individuals who are most impervious to peer or other social influences.

Abstract:Structured Background The cardiovascular and renal benefits of renin-angiotensin aldosterone system (RAAS) blockade are not well-established in patients with advanced CKD. We conducted a systematic review and meta-analysis to identify potential risks and benefits from RAAS blockade in patients with CKD stage 4-5. Methods A Medline search from inception to November 2022 was conducted to identify randomized controlled trials (RCTs) in patients with CKD stage 4-5 (estimated GFR ≤ 30 mL/min/1.73m 2 ) comparing RAAS blockade against placebo or alternative antihypertensive therapy. Different intervention strategies were assessed (RAAS use vs non-use, initiation vs placebo/alternative therapy or discontinuation vs continuation). The primary outcome was progression to end-stage kidney disease (ESKD). Secondary outcomes were all-cause mortality and major adverse cardiovascular events (MACE). The risk ratio (RR) was estimated using a random-effects model. Results Nine RCTs (1,150 patients) were included. In RCTs, RAAS blockade was associated with a significant reduction in progression to ESKD: RR 0.84 (95% confidence interval [CI] 0.74 – 0.96; p = 0.01). There was no benefit from RAAS blockade on all-cause mortality or MACE: RR 1.02 (95% CI 0.63 – 1.65; p = 0.93) and RR 0.87 (95% CI 0.49– 1.57; p = 0.65), respectively. Conclusions RAAS blockade may be considered in selected patients with CKD stage 4-5 to delay progression to ESKD.

SNR enhancement by the noise elimination technique for decomposed OFDM signals

Abstract:In general, it is difficult to reduce the noise power selectively from the received signals in wireless communication systems. Thus, in order to improve the SNR, there are variety of signal processing techniques to obtain some processing gain; e.g. various kinds of diversity techniques, spectrum spreading/de-spreading technique, and so forth. In this paper, a novel SNR enhancement technique has been proposed and demonstrated. After the multiple of constant-amplitude signals and the residual signal separated from the OFDM signal are transmitted in parallel from antennas of a MIMO scheme, noise added on the on-off-signals can be eliminated taking advantage the knowledge of their constant amplitude at the receiver. It is confirmed that the proposed scheme improves the receiver SNR by 1.1dB for the single-threshold and 2dB for the dual-thresholds at the BER of 10−3, respectively. In addition to the SNR enhancement, the proposed scheme also has the large advantage of PAPR reduction for OFDM signals.

Abstract:Abstract Emerging studies show that beyond a phenotype of overt primary aldosteronism, there exists a prevalent phenotype of renin-independent aldosterone production among patients with low-renin hypertension (LRH). Mineralocorticoid receptor antagonist (MRA) therapy to induce an increase in renin is associated with favorable outcomes in primary aldosteronism. Whether a similar approach is beneficial for all patients with low-renin hypertension is not known. We evaluated whether treatment of patients with LRH with MRAs to increase renin is associated with a decrease in blood pressure and/or proteinuria. We have conducted a prospective observational cohort study of consecutive patients from the practice of co-author A.T. between 2005 and 2021. Patients were included if they had hypertension with suppressed (< 1.0 ng/ml/h) plasma renin activity (PRA) and detectable aldosterone levels but favored empiric medical therapy over further diagnostic testing. All patients were subsequently treated with an MRA. MRA was started at 25-50 mg daily of spironolactone (women) or eplerenone (men) and the dose was gradually increased in 25-50 mg increments to target a PRA ≥ 1.0 ng/ml/h or the maximal tolerated dose. Blood pressure, albumin-to-creatinine ratio (ACR), and other patient characteristics at baseline vs. after unsuppressed renin was achieved were compared using a paired-samples t-test. The mean age of 31 study patients who achieved unsuppressed PRA was 67.9 (SD 11.9) and 14 (45.2%) were women. The mean baseline aldosterone was 14.9 ng/dL (SD 9.4); 13 (41.9%) patients had baseline aldosterone < 10.0 ng/dL. Most (21/31; 67.7%) patients had baseline PRA below the assay detection limit. The mean baseline potassium was 4.2 (SD 0.46); one (3.2%) patient had baseline hypokalemia (K < 3.5 mEql/dL). The mean final PRA was 4.78 (SD 5.64) ng/ml/h. The median (IQR) MRA dose required to achieve unsuppressed PRA was 75 (50 to 200) mg. The mean systolic and diastolic blood pressure decreased from 144.9 (SD 14.9) and 79.2 mm Hg (SD 15.0) at baseline to 124.7 (SD 11.1) and 71.5 mm Hg (SD 10.8), respectively, after achievement of unsuppressed PRA (P < 0.001 for both). The mean number of anti-hypertensive medications decreased from 3.4 (SD 1.3) at baseline to 2.5 (SD 1.5) after achievement of unsuppressed PRA (P < 0.001). Among the six (19.4%) patients who had proteinuria, the mean ACR decreased from 179.0 (SD 102.1) at baseline to 36.1 (SD 49.2) mg/g after achievement of unsuppressed PRA (P = 0.007). Three (9.7%) and seven (22.6%) patients had to have their MRA dose decreased due to symptomatic hypotension and hyperkalemia, respectively. These findings suggest that suppressed renin may be sufficient to identify patients who could benefit from MR blockade. Treatment of patients with suppressed renin with sufficient MRA dose to achieve its unsuppression could lower both elevated blood pressure and proteinuria. Abstract:In genetics, major progress was made after pooling of data sets to mega-studies became the norm in the field. In the present commentary, the authors ask whether such an approach would also be worthy of broader application in the field of social epidemiology. Research on job strain and coronary heart disease provides an illustrative example. Over 3 decades, debate has continued as to the relative importance of high psychological demands versus low control—that is, whether one component of job strain is more toxic than the other—and differences by age and sex. Recently, these controversies were largely resolved in an individual-participant meta-analysis of 200,000 participants from 13 cohorts: The combination of both high demands and low control was a greater risk factor than either of the components alone, there were no differences in the associations of job strain with CHD between men and women, between the young and old, or at different levels of socioeconomic position, and the impact was more modest when unpublished data were included but was still robust to all adjustments. The fact that longstanding debates in the job strain literature were resolved by applying an individual-participant data meta-analysis approach suggests that lessons learned in genetics might also apply to social epidemiology.
