Abstract:Objective: Evidence shows type 1 diabetes(T1D) leads to vascular damage and neuropathy. The purpose of this study was to perform a systematic review and a meta-analysis to assess the evidence of the effects of T1D on hearing function. Methods: Three electronic databases were used. The articles were independently reviewed by two authors using predefined inclusion criteria to identify eligible studies. They were then classified as high or low methodological quality. Meta-analysis was performed on pooled data of hearing loss(HL) prevalence, pure tone audiometry(PTA), otoacoustic emissions(OAE) and auditory brainstem response(ABR). Results: Twenty-one articles fulfilled the inclusion criteria. In all studies, HL was defined as pure tone greater than 20 dB in at least one frequency. The prevalence of HL ranged between 5.17% and 48% for diabetics, which was higher than in controls which ranged between 0% à 40% (OR = 7.7, 95% CI 3.32-17.98, p < 0.05 and I2 = 40%). The tendency of mean thresholds of PTA was higher in diabetics than in controls, with results being statistically significant at 250, 500 and 1000 Hz. OAE were significantly lower in diabetic patients. ABR latencies were longer in T1D group compared to controls and were statistical significant. Conclusions: Patients with T1D have a significantly greater prevalence of HL compared to the control group. These damages could be compared to other microvascular diseases. Further studies are needed to assess whether hearing testing should be considered as a part of the screening process in T1D patients and therefore, secondary preventive treatment may be warranted as well.
An association of type 1 diabetes mellitus with auditory dysfunction: A systematic review and meta-analysis

Abstract:Objective: To establish the relationship between the presence of type 1 diabetes mellitus (DM) and auditory dysfunction in clinical settings by a systematic review and meta-analysis of currently available published data. Data sources and review methods: The electronic databases PubMed, Embase, and Wanfang Data were searched for eligible relevant studies up to May 2016, and the reference lists of the retrieved articles were used for additional manual search. All the articles included in this pooled analysis were determined according to the preset inclusion and exclusion criteria. Meta-analysis of pooled data was performed using Review Manager 5.3. Results: A total of 15 studies were included for further combined analysis. The results showed that patients with type 1 diabetes had a significantly higher prevalence of hearing loss than controls (odds ratio = 49.08, 95% confidence interval = 12.03-200.31, P < 0.00001); standardized mean of differences (SMD) of pure tone audiometry at 4,000 Hz between diabetes and controls was 0.87 (Z = 2.22, P = 0.03, I2 = 95%); SMD of the latency time was 0.54 (Z = 2.69, P = 0.007, I2 = 78%) for waves III and 0.61 (Z = 2.38, P = 0.02, I2 = 86%) for wave V, respectively; and SMD of the interpeak latency time was 0.41 (Z = 2.84, P = 0.005, I2 = 39%) for waves I to III and 0.61 (Z = 2.67, P = 0.008, I2 = 81%) for waves I to V, respectively, between diabetics and controls. Conclusion: Our study reveals that there is relationship between the presence of type 1 DM and an increased risk for developing mild and subclinical hearing impairment. Level of evidence: NA. A systematic review of the association of Type I diabetes with sensorineural hearing loss

Abstract:Type 1 diabetes (T1D) has been associated with several comorbidities such as ocular, renal, and cardiovascular complications. However, the effect of T1D on the auditory system and sensorineural hearing loss (SNHL) is still not clear. The aim of this study was to conduct a systematic review to evaluate whether T1D is associated with hearing impairment. The databases PubMed, Science Direct, Scopus, and EMBASE were searched in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria. Three reviewers independently screened, selected, and extracted data. The Joanna Briggs Institute (JBI) Critical Appraisal Tools for Analytical cross-sectional and case-control studies were used to perform quality assessment and risk of bias analysis on eligible studies. After screening a total of 463 studies, 11 eligible original articles were included in the review to analyze the effects of T1D on the auditory system. The included studies comprised cross-sectional and case-control investigations. A total of 5,792 patients were evaluated across the 11 articles included. The majority of the studies showed that T1D was associated with hearing impairment compared to controls, including differences in PTAs and OAEs, increased mean hearing thresholds, altered acoustic reflex thresholds, and problems with the medial olivocochlear (MOC) reflex inhibitory effect. Significant risk factors included older age, increased disease duration, and higher HbA1C levels. This systematic review suggests that there is a correlation between T1D and impairment on the auditory system. A multidisciplinary collaboration between endocrinologists, otolaryngologists, and audiologists will lead to early detection of hearing impairment in people with T1D resulting in early intervention and better clinical outcomes in pursuit of improving the quality of life of affected individuals. Abstract:The aim of this study is to compare the hearing thresholds of the diabetic patients with the age- and sex-matched normal control group regarding age, glucose level, duration and complications of diabetes with the degree of hearing impairment. Pure tone audiometry was performed in 58 patients with type 1 and 2 diabetes mellitus and compared with 58 age- and sex-matched controls. The patients were categorized into groups according to age, duration of diabetes, complications and control of diabetes. These observations were compared with those from the control subjects. 15–50 years age group with diabetes showed a significant high frequency hearing loss, as compared to the controls. Complicated and poorly controlled diabetics have significant degree of hearing loss in high frequencies as compared to those who were well controlled and uncomplicated. There was also a correlation between the level of hearing loss and duration of diabetes.

Abstract:Diabetes (type 2) and sensorineural hearing loss are common health problems manifested with ageing. While both type 1 and type 2 diabetes have been associated with hearing loss, a causal link has been difficult to establish. Individuals with diabetes have twice the incidence of hearing loss compared to those without diabetes and those with prediabetes have a 30% higher rate of hearing loss. Whether hearing loss is associated with diabetes independent of glycemic control remains to be determined. Hearing loss has its own set of risk factors and shares others with diabetes. This review will summarize the complex relationship between diabetes and sensorineural hearing loss.

Prevalence of Hearing Loss in Type 2 Diabetes Mellitus and Its Association with Severity of Diabetic Neuropathy

Abstract:Abstract Purpose This study assessed the prevalence of hearing loss (HL) in patients with Type 2 Diabetes (T2DM) and its relationship with the presence and severity of diabetic neuropathy. Methods Patients between the ages of 30 to 60 years (both ages inclusive) with T2DM were recruited and divided into three groups. Group 1 included patients without neuropathy. Group 2 had patients with mild neuropathy. Group 3 had patients with moderate and severe neuropathy. After informed consent hearing threshold was assessed using pure tone audiometry (PTA). Results Of the 200 patients recruited, the prevalence of hearing loss was overall 81%. The prevalence was 66.7% in group 1, 80.9% in group 2 and 87.6% in group 3 (p=0.009). Among patients with moderate to severe neuropathy (group 3) 33.3% had clinically significant hearing loss (p=0.015). Age, gender, presence of neuropathy and severity of neuropathy were associated with increased risk of developing hearing loss. Severity of hearing loss worsened with increase in severity of neuropathy. Conclusions Age, gender and severity of neuropathy were associated with increased risk of developing hearing loss. Screening for hearing loss in patients with moderate to severe diabetic neuropathy using self-report questionnaires can help in timely diagnosis and treatment.

Abstract:INTRODUCTION The aim of this study was to evaluate auditory organ function in relatively young type 1 diabetic patients, with short duration of the disease and without overt hearing loss. The impact of age, diabetes duration and metabolic control on hearing function was also analysed. MATERIAL AND METHODS Thirty-one patients with type 1 diabetes, aged below 45 years (mean 29.1 ± 7.1 years), with diabetes duration of less than 120 months (mean 54.7 ± 32.5 months), and no evident hearing impairment, were compared to 26 age-matched (30.3 ± 7.8 years, p = 0.567) healthy volunteers. In all subjects, pure-tone audiometry, transient evoked otoacoustic emissions (TEOAE), and auditory brainstem responses (ABR) were performed, after an ENT examination. RESULTS In diabetic patients, compared to healthy subjects, the mean hearing threshold in the pure-tone audiometry was significantly higher at high frequencies, the mean amplitude of TEOAE was lower (7.75 ± 4.43 dB v. 10.00 ± 4.01 dB, p < 0.001), and latency times of wave V and interval I-V in ABR were longer (5.78 ± 0.25 ms v. 5.69 ± 0.18 ms, p = 0.025 and 4.03 ± 0.20 ms v. 3.95 ± 0.17 ms, p = 0.017 respectively). In the diabetic group, the hearing threshold showed positive linear correlation with age, whereas TEOAE was inversely correlated with this parameter. In ABR latency, times of wave V and interval I-V were negatively correlated with diabetes duration. CONCLUSIONS This study confirms the involvement of the auditory organ in type 1 diabetes mellitus. To determine the prognostic and predictive values of this finding, and methods of possible prevention of hearing loss, further prospective observations are required.

Abstract:ABSTRACT Objective: To systematically review the available evidence on the association of HBA1c levels and development of sensorineural hearing loss and to quantitatively analyze the available data on HBA1c levels in patients with type 2 diabetes mellitus and sensorineural hearing loss to determine an HbA1c level that may be associated with the risk of having sensorineural hearing loss. Methods: Design: Systematic Review and Meta-analysis Eligibility Criteria: Cross-sectional studies, or cohort studies which were limited to English language that investigated the correlation of glycemic index using HBA1c and sensorineural hearing loss among adult type 2 diabetic patients which were done from January 2010 to December 2021. Studies with no published outcome, incomplete data or that were ongoing as of August 1, 2022 were also excluded. Information Sources: MEDLINE (through PubMed), Cochrane Library, Scopus, Embase (through OVID@journal), Directory of Open Access Journals (DOAJ), Google Scholar and HERDIN Plus Risk of Bias: Risk of Bias was assessed using the Guidelines for Cochrane Collaboration Synthesis of Results: Results were presented using forest plots for representation.Results: A total of 8 studies were reviewed with 2,103 participants in all. Six articles compared hearing loss incidence between diabetic and non-diabetic patients. Overall, there were a total of 881 diabetic patients and 1222 non-diabetic patients. There was a significantly lower incidence of sensorineural hearing loss in non-diabetic patients with a risk ratio of 1.89, 95% CI [1.65, 2.16]. Three articles compared the HbA1c levels of diabetic patients with or without sensorineural hearing loss. Diabetic patients without sensorineural hearing loss had significantly lower HbA1c levels compared to those with sensorineural hearing loss with mean difference of 1.04, 95%CI [0.82, 1.25].Conclusion: In conclusion, this meta-analysis showed a higher prevalence rate of sensorineural hearing loss among patients with diabetes mellitus compared to non-diabetic patients. Moreover, poor glycemic control among diabetic patients with a glycemic index based on HbA1c of more than 8.3 (6.97-9.6) is associated with sensorineural hearing loss.
Diabetes and Risk of Hearing Impairment in Adults: A Meta-Analysis

Abstract:CONTEXT: Recently, several studies have investigated the relationship between diabetes and hearing impairment, but results were inconsistent.OBJECTIVE: Our objective was to compare the prevalence of hearing impairment between diabetic and nondiabetic adults.DATA SOURCES: We performed a systematic literature search using MEDLINE (1950 to May 30, 2011) and EMBASE (1974 to May 30, 2011).STUDY SELECTION: Cross-sectional studies were included if data on numbers of hearing-impaired and non-hearing-impaired cases with diabetes were presented. Hearing impairment was limited to that assessed by pure-tone audiometry that included at least 2 kHz of frequency range and was defined as progressive, chronic, sensorineural, or without specified cause.DATA EXTRACTION: Two authors independently extracted relevant data. Odd ratios (ORs) of hearing impairment related to diabetes calculated in each study were pooled with the random-effects model.DATA SYNTHESIS: Data were obtained from 13 eligible studies (20,194 participants and 7,377 cases). Overall pooled OR (95% confidence interval) of hearing impairment for diabetic participants compared with nondiabetic participants was 2.15 (1.72-2.68). OR was higher in younger participants (mean age, ≤60 yr) than in those over 60 yr among which the OR remained significant (2.61 and 1.58, P = 0.008). The strength of the association between diabetes and prevalence of hearing impairment was not significantly influenced by whether participants were matched for age and gender (P = 0.68) or whether participants chronically exposed to noisy environments were excluded (P = 0.19).CONCLUSIONS: Current meta-analysis suggests that the higher prevalence of hearing impairment in diabetic patients compared with nondiabetic patients was consistent regardless of age.

Abstract:BACKGROUND: Hearing loss is an underestimated comorbid condition in type 2 diabetes. OBJECTIVES: Investigate hearing loss as a comorbidity associated with type 2 diabetes mellitus and evaluate the factors associated with hearing loss. DESIGN: Cross-sectional. SETTING: Tertiary care center, diabetes clinic. PATIENTS AND METHODS: Patients with type 2 diabetes, aged 30 to 60 years, were randomly selected to participate. All patients underwent clinical ear examinations and were referred for full audiological evaluation. Otoacoustic emission was used to assess inner function, tympanometry to assess middle-ear function, and pure tone air/bone audiometry to assess hearing sensitivity. Risk factors for hearing loss were assessed by multivariate logistic regression. MAIN OUTCOME MEASURE: Frequency, severity and risk factors for hearing loss. SAMPLE SIZE: 157 RESULTS: Of the 157 patients, 77 had hearing loss in both ears (49.0%), 13 in the right ear only (8.3%), 14 in the left ear only (8.9%), and 53 (33.8%) had normal hearing. In the 181 ears with sensorineural hearing loss, 90 had mild loss (49.7%), 69 moderate loss (38.2%), 16 severe loss (8.8%) and 6 had profound loss (3.3%). Disabling hearing loss was observed in 46 (29%) patients. A higher frequency of hearing loss was present in patients with glycated hemoglobin levels ≥8%. In the multivariate logistic regression analysis, the most important factors associated with hearing loss were longer diabetes duration, poor glycemic control and the presence of hypertension. CONCLUSIONS: Hearing loss is an underestimated comorbid condition in type 2 diabetes that warrants frequent hearing assessments and management. Strict glycemic and hypertension control is essential for the minimization of the effects of diabetes on hearing sensitivity. LIMITATIONS: Small sample size, limited age window (30-60 years), which was chosen to eliminate the natural aging effect on hearing. Cross-sectional nature was not ideal for the assessment of causality. CONFLICT OF INTEREST: None.

Evaluation of hearing threshold in type 1 diabetic children

Abstract:Objective: To assess the auditory function in a group of children with diabetes mellitus type 1 and to study the association between hearing loss and disease related factors: duration of the disease, metabolic control and diabetic complications.Patients and Methods: Sixty-two diabetic patients below the age of 16 attending AL- Salam Teaching Hospital, Mosul, Iraq from May 2017 to June 2018 were evaluated and compared with equal number (same gender &age) of non-diabetic controls. Pure-tone audiometric tests were performed. Both air and bone conduction were tested at frequencies between 250-8000 Hz.Results: The hearing level was lower in diabetic children than in the control group in all tested frequencies, but the differences was statistically significant only at mid and high frequencies. The hearing loss was bilateral, symmetrical and affects both sexes. HbA1c concentration, age of diabetes and microangiopathic complications showed positive association with the elevated hearing thresholds, while, age at disease onset, dose of insulin, number of hypoglycemic and DKA episodes and presence of neuropathy were not.Conclusions: Hearing impairment may occurs early in diabetic children and is related to the disease duration and the stage of metabolic control. Careful & accurate glycemic control help prevent or delay this complication.

Abstract:INTRODUCTION AND OBJECTIVE Hearing impairment in diabetic subjects is more prevalent than in the general population. Ageing, noise exposure and smoking are known as risk factors of hearing loss. The aim of this study was to assess whether other factors, such as HbA(1c), blood pressure, serum lipids and BMI have an impact on hearing function among relatively young diabetic subjects. MATERIALS AND METHODS 58 patients, 31 with type 1 diabetes and 27 with type 2 diabetes, aged < 45 years, with diabetes duration < 10 years and without overt hearing impairment were included. In all subjects, vital signs, laboratory tests, pure-tone audiometry and trancient-evoked otoacoustic emissions (TEOAE) were evaluated. RESULTS Hearing impairment was revealed in 20 subjects. This group had a lower HDL-cholesterol level compared with normal hearing patients (44.2 mg/dl vs. 57.6 mg/dl, p=0.007). Absence of otoacoustic emissions was diagnosed in 16 subjects. These patients also had a lower HDL-cholesterol level compared with subjects with TEOAE present (45.4 mg/dl vs. 55.2 mg/dl, p=0.018). Hearing threshold was inversely correlated with HDL-cholesterol level, and positively correlated with triglycerides. Patients with HDL-cholesterol level ≥50 mg/dl had lower hearing threshold at frequencies 0.5-12 kHz, as well as higher TEOAE amplitude. Subjects with triglycerides above median had a higher hearing threshold at frequencies 0.5-12 kHz, as well as lower TEOAE amplitude. Patients with elevated BMI (≥25 kg/m(2)) had a higher hearing threshold at frequencies 2-12 kHz, as well as lower TEOAE amplitude. CONCLUSIONS Hearing impairment is frequent among relatively young diabetic subjects. The preseted study reveals that factors like HDL-cholesterol, triglycerides, and BMI may affect hearing function in this group.

Abstract:Observational studies on diabetes and hearing loss have been controversial, and their potential causal relationship has not been investigated. We conducted Mendelian randomization (MR) analyses to determine whether type 1 diabetes (T1D), type 2 diabetes (T2D), fasting insulin (FI), 2-h glucose (2hG), fasting glucose (FG), and glycated hemoglobin (HbA1c) were associated with hearing loss. Among 497,998 individuals in the UK biobank, T1D and T2D were identified by medical history in questionnaires and clinical records. Hearing loss was primarily defined by speech-in-noise tests in the better ear (SiN hearing loss, SiNHL) and secondarily identified by International Classification of Diseases code (ICD-defined hearing loss, ICDHL) and self-reported data (hearing difficulty, HDiff; hearing difficulty with noise, HDiffn; and hearing aid use, HAid). In observational analyses, odds ratios (ORs) in people with T1D than those without diabetes were 1.27 (95% CI 1.06-1.53) for SiNHL, 1.36 (1.14-1.62) for HAid, and 1.78 (1.49-2.12) for ICDHL. The corresponding ORs in people with T2D were 1.17 (1.09-1.24), 1.16 (1.08-1.23), and 1.25 (1.16-1.34), respectively. In MR analyses, genetically predicted T1D and T2D were associated with a higher risk of SiNHL (OR 1.02 per 1-unit higher log-odds of T1D 1.02 [1.00-1.03]; T2D 1.03 [0.99-1.06]). Further analysis of glycemic traits supported an inverse association of genetically elevated FI on SiNHL (per 1-log unit increase in pmol/L FI 0.71 [0.54-0.94]), and a positive association for genetically elevated 2hG (per 1-log unit increase in mmol/L 2hG 1.18 [1.06-1.31]). Results were largely robust to sensitivity analyses accounting for horizontal pleiotropy, sample overlap, and definitions of hearing loss. Our study suggested a potential causal relationship between diabetes and hearing loss risk. Abstract:Diabetes mellitus (DM) is a chronic degenerative disease that impairs normal insulin production and use. DM chronic auditory complications may include spiral ganglion atrophy, degeneration of the vestibulocochlear nerve myelin sheath, reduction of the number of spiral lamina nerve fibers, and thickening of the capillary walls of the stria vascularis and small arteries. Objective This paper aims to verify the hearing thresholds of individuals with type 1 DM. Materials and Methods Sixty patients were enrolled in this trial and divided into case and control groups featuring diabetic and non-diabetic subjects respectively. All individuals were interviewed and underwent physical examination, ENT examination, and audiometric tests. Results Statistically significant difference was observed in hearing thresholds of case group subjects at 250, 500, 10,000, 11,200, 12,500, 14,000 and 16,000 Hz for both ears and ear average. Case group subjects had higher likelihood of having hypacusis at any frequency regardless of ear than controls. Conclusion Statistically significant differences were seen in the audiological findings of case group subjects when compared to controls. Thorough audiological examination including high frequency audiometry is required for subjects with diabetes mellitus type 1.

Abstract:PURPOSE: Microvascular involvement in patients with diabetes mellitus is one of the causes of retinopathy, nephropathy, and neuropathy. The same pathologic processes may occur in the inner ear structures. This case-control study aimed to evaluate the hearing thresholds in type 2 diabetic patients with different severity of diabetic retinopathy (DR) and to compare these findings with controls.MATERIALS AND METHODS: We evaluated the hearing threshold in four groups of eligible subjects aged 20-70 years. These groups were controls, diabetic patients with no-DR, with mild-moderate non-proliferative DR (NPDR), and with severe NPDR/proliferative DR (PDR). Each group consisted of 105 subjects. Speech-frequency and high-frequency hearing levels (SFHL and HFHL, respectively) were measured and log-transformed. Analysis of covariance was used. The prevalence rate of moderate or more hearing loss in the groups was estimated.RESULTS: In total, 194 men and 226 women participated. The ratio of means of SFHL and HFHL between PDR and controls was 0.18 and 0.20, respectively. Hearing loss was prevalent in severe NPDR/PDR (adjusted prevalence ratio 3.36 for SFHL and 1.51 for HFHL) compared to controls. Also, the prevalence of high-frequency hearing loss was more in mild-moderate NPDR (adjusted prevalence ratio 1.33).CONCLUSIONS: The magnitude of the increase in hearing impairment prevalence between the severe NPDR/PDR patients and controls was about 24% for both SFHL and HFHL. We recommend hearing assessment in the screening of the DR patients.

Abstract:In order to determine the mechanisms which could be responsible for the hypertrophy of vascular wall associated with primary hypertension, we have investigated the mechanisms involved in the proliferation of aortic cells from spontaneously hypertensive rats (SHR) and normotensive Wistar-Kyoto (WKY) controls. In this study we have examined the role of phospholipase C which is responsible for the formation of second messengers, the function of protein kinase C and that of GTP-binding proteins (G proteins) which couple membrane receptors to phospholipase C. The adventitial fibroblasts from thoracic aorta were chosen as cell model in culture. The capacity of proliferation in response to 10% serum was analyzed by cell number determination and by measuring the incorporation of 3H-thymidine into newly synthesized DNA. Our results showed that SHR-derived fibroblasts proliferated more rapidly and incorporated more 3H-thymidine than WKY-derived fibroblasts. However, the phospholipase C activity, measured by the serum-stimulated production of 3H-inositol phosphates in cells prelabeled with 3H-inositol, did not differ between SHR- and WKY-derived cells. The desensitization of protein kinase C, by long term (2 d) treatment with high dose (300 nM) of phorbol 12-myristate, 13-acetate (TPA), markedly augmented, but to the same extent, the 3H-thymidine incorporation into DNA of SHR- and WKY-derived fibroblasts. Moreover, protein kinase C activation by TPA caused a parallel reduction (25-30%) of the incorporation of 3H-thymidine into DNA of SHR- and WKY-derived cells. Under the same experimental conditions, the growth of SHR- and WKY-derived fibroblasts was reduced by TPA in a dose-dependent manner (up to 20%) to the same extent.(ABSTRACT TRUNCATED AT 250 WORDS)
The Impact of Diabetes Mellitus on Sensorineural Hearing Loss: A Cross-Sectional Study in Eastern India

Abstract:Objective Hearing loss as a comorbidity of type 2 diabetes mellitus (type 2 DM) is frequently overlooked by patients and healthcare professionals because of a lack of awareness. This cross-sectional study aims to investigate the impact of DM on sensorineural hearing loss (SNHL) in the population of Eastern India. The primary objectives are to assess the prevalence and severity of SNHL among individuals with DM, explore demographic and clinical factors associated with hearing impairment, and contribute valuable insights to the understanding of this relationship in a specific regional context. Methods An institutional-based cross-sectional study was conducted on 198 patients with type 2 DM. Of these, 46 patients were excluded based on exclusion criteria. All patients underwent detailed demographic and clinical assessments, including glycemic control, DM duration, and associated complications. Pure tone audiometry was used to evaluate hearing thresholds. Otoacoustic emission testing was performed to assess cochlear dysfunction. Results A high prevalence of SNHL (70.4%) was observed among the 152 participants meeting the inclusion criteria. Females exhibited a higher prevalence than males, and most participants experienced mild SNHL. Rural residence, lower socioeconomic status, and poor glycemic control were associated with increased SNHL. Significant associations were found between hearing loss severity and DM duration, glycosylated hemoglobin (HbA1c) levels, and complications. Among complications, a strong association was noted with diabetic neuropathy. No significant association was observed with the presence or absence of otoacoustic-emission. Conclusion This study reveals a substantial impact of DM on SNHL in Eastern India, emphasizing the importance of routine hearing assessments in diabetic populations. The findings contribute to regional understanding and have implications for targeted healthcare interventions and preventive strategies.

Abstract:Objective: Studies have demonstrated that HbA1c is a significant predictor of hearing impairment in type 1 diabetes. We identified additional factors associated with hearing impairment in participants with type 1 diabetes from the Diabetes Control and Complications Trial and its observational follow-up, Epidemiology of Diabetes Interventions and Complications (DCCT/EDIC) study. Methods: 1,150 DCCT/EDIC participants were recruited for the Hearing Study. A medical history, physical measurements, and a self-administered hearing questionnaire were obtained. Audiometry was performed by study-certified personnel and assessed centrally. Logistic regression models assessed the association of risk factors and comorbidities with speech- and high-frequency hearing impairment. Results: Mean age was 55±7 years, duration of diabetes 34±5 years, and DCCT/EDIC HbA1c 7.9±0.9%. In multivariable models, higher odds of speech-frequency impairment were significantly associated with older age, higher HbA1c, history of noise exposure, male sex, and higher triglycerides. Higher odds of high-frequency impairment were associated with older age, male sex, history of noise exposure, higher skin intrinsic florescence (SIF) as a marker of tissue glycation, higher HbA1c, non-professional/non-technical occupations, sedentary activity, and lower LDL cholesterol. Among participants who previously completed computed tomography and carotid ultrasonography, coronary artery calcification (CAC)>0 and carotid intima-medial thickness were significantly associated with high- but not speech-frequency impairment. Conclusion: Consistent with previous reports, male sex, age, several metabolic factors, and noise exposure are independently associated with hearing impairment. The association with SIF further emphasizes the importance of glycemia, as a modifiable risk factor, over time. In addition, the macrovascular contribution of CAC is novel and important.
