Abstract:In four experiments, we measured the gender-specific face-aftereffect following subject's eye movement, head rotation, or head movement toward the display and following movement of the adapting stimulus itself to a new test location. In all experiments, the face aftereffect was strongest at the retinal position, orientation, and size of the adaptor. There was no advantage for the spatiotopic location in any experiment nor was there an advantage for the location newly occupied by the adapting face after it moved in the final experiment. Nevertheless, the aftereffect showed a broad gradient of transfer across location, orientation and size that, although centered on the retinotopic values of the adapting stimulus, covered ranges far exceeding the tuning bandwidths of neurons in early visual cortices. These results are consistent with a high-level site of adaptation (e.g. FFA) where units of face analysis have modest coverage of visual field, centered in retinotopic coordinates, but relatively broad tolerance for variations in size and orientation. Introduction David Marr (Marr, 1982) characterized vision as " knowing what is where. " For much of the visual cortex, " knowing where " appears to be based in a retinotopic coordinate system. With each eye, head and body movement , the image of the world shifts on the retina and across the retinotopic cortical areas. Despite these displacements, our perception of the world is for the most part reassuringly stationary and stable. Evidently, these displacements in retinal coordinates are corrected at some higher level to generate world-based coordinates. Whether these coordinates are explicitly represented in a spatiotopic map is a topic of intense debate. Abstract:AIM: To study the neurotoxicity and toxicokinetics of norfloxacin (NFLX) in freely moving rats. METHODS: Rats were assigned randomly to four treatment groups that received a single iv dose of 50, 100, 200 mg/kg of NFLX and 0.9 % saline, respectively. Electroencephalogram (EEG) was continuously recorded with a computerized system in freely moving rats. Venous blood samples were collected for determination of the NFLX concentration by microbioassay method with Escherichia coli 441102 as the test strain. Toxicokinetic parameters were determined from serum concentration-time data with the 3p97 program. RESULTS: (1) The epileptiform discharges appeared in all NFLX groups with different latent periods, accompanied with limb twitching and clonic-tonic seizures. The relative total power of the EEG increased. (2) Drug serum concentration-time curves of different doses conformed to a two-compartmental model. The values of clearance, volume of distribution, and terminal half-life were dose-independent, while maximum serum concentrations (C-max) and the areas under the concentration-time curve (AUC(0-->infinity)) of NFLX increased with dosage. (3) The relative total powers of EEG were closely correlated with the administered dose, C-max as well as AUC(0-infinity). CONCLUSION: The present study established a suitable approach to quantitatively determine central nervous system (CNS) stimulant effect of NFLX. There is a significant correlation between AUC(0-->infinity) and the changes of relative total power, which may serve as the index for judgement and prediction of the CNS toxic effect induced by NFLX.

Abstract:Levofloxacin (LVFX), a fluoroquinolone agent, has a broad spectrum that covers Gram-positive and -negative bacteria and atypical pathogens. It demonstrates good clinical efficacy in the treatment of various infections, including lower respiratory tract infections (LRTIs) and urinary tract infections (UTIs). To evaluate the efficacy and safety of oral LVFX 500 mg once daily, a large open-label clinical trial was conducted in 1266 patients (899 with LRTIs and 367 with UTIs) at 32 centers in China. In the per-protocol population, the clinical efficacy rate (cure or improvement) at 7 to 14 days after the end of treatment was 96.4% (666/691) for LRTIs and 95.7% (267/279) for UTIs. In 53 patients diagnosed with atypical pneumonia the treatment was effective. The bacteriological efficacy rate was 96.6% (256/265) for LRTIs and 93.3% (126/135) for UTIs. The eradication rate of the causative pathogens was 100% (33/33) for Haemophilus influenzae and 96.0% (24/25) for Streptococcus pneumoniae in LRTIs, and 94.1% (80/85) for Escherichia coli in UTIs. The overall efficacy rates were 89.3% (617/691) for LRTIs and 87.8% (245/279) for UTIs. The incidence of drug-related adverse events (ADRs) was 17.3% (215/1245), and the incidence of drug-related laboratory abnormalities was 15.7% (191/1213). Common ADRs were dizziness, nausea, and insomnia. Common laboratory abnormalities included “WBC decreased”, “alanine aminotransferase (ALT) increased”, “aspartate aminotransferase (AST) increased”, and “lactate dehydrogenase (LDH) increased”. All of these events were mentioned in the package inserts of fluoroquinolones including LVFX, and most events were mild and transient. Thirty-four patients (2.7%) were withdrawn from the study because of the ADRs. No new ADRs were found. This study concluded that the dosage regimen of LVFX 500 mg once daily was effective and tolerable for the treatment of LRTIs and UTIs.

Abstract:Objectives: Levofloxacin is widely used because of its broad-spectrum antimicrobial activity and convenient dosing schedule. However, the relevance of its use in children remains to be investigated. The purpose of this study is to investigate the efficacy and safety of levofloxacin use in children with severe infections. Methods: We conducted a retrospective observational study of patients <18 years of age who received levofloxacin intravenously in the Pediatric Intensive Care Unit (PICU) of our hospital during the period between 2021 and 2022. Patient demographics, course characteristics, clinical effectiveness, and adverse event correlations were extracted through a retrospective tabular review. Results: We included 25 patients treated with 28 courses of levofloxacin. The mean age of these children treated with levofloxacin was 4.41 years. Conversion of pathogenic microbiological test results to negative after levofloxacin treatment was detected in 11 courses (39.29%). A decrease in inflammatory markers, white blood cell or C-reactive protein counts, was detected in 18 courses (64.29%). A total of 57 adverse events occurred during the treatment period, of which 21 were possibly related to levofloxacin and no adverse events were probably related to levofloxacin. Conclusion: The effectiveness of levofloxacin use in children with serious infections is promising, especially for the treatment of multidrug-resistant bacteria. Adverse events occurring during the initiation of levofloxacin therapy in children are reported to be relatively common, but in this study, only a small percentage of them were possibly related to levofloxacin, and none of them were highly possibly related to levofloxacin.

Abstract:The clinical pharmacokinetics of domestic fleroxacin in healthy volunteers were studied. All drug concentrations in serum and urine were assayed by HPLC and bioassay method. The experimental data obtained in 8 healthy volunteers after a single 400 mg oral dose of fleroxacin tablet and capsule could be fitted to a two compartment model. The mean maximum serum drug concentrations being 6.52 �� 1.20 mg/L (domestic tablet) and 6.74 �� 2.31 mg/L (domestic capsule), reached at 0.91 �� 0.43 and 1.20 �� 0.67 h after the drug administration respectively. Their elimination half-lives (T( 1/4 ��)) were 11.07 �� 1.73 and 10.81 �� 2.00 h; AUC were 82.17 �� 17.79 and 89.08 �� 18.89 h �� mg/L. 68.95% �� 6.29% and 77.23% �� 6.76% of the given dosage were excreted in urine with 72 h respectively. Mean urinary drug concentrations exceeded 140 mg/L within 24 hours after the drug administration. The pharmacokinetic parameters and relative bioavailability of domestic tablet and capsule after a single 400 mg dose were similar with that of imported tablet. According to the results in this paper, a scheme of dosage regimen of fleroxacin was suggested.

Abstract:Objective To analyze the efficacy and safety of quinolones in children with severe infection. Methods Patients with severe infections treated in the Pediatric Intensive Care Unit of Beijing Children's Hospital from November 2016 to January 2018 were enrolled. The clinical data were collected and analyzed. Results Totally 24 children were treated with quinolones, including levofloxacin, ciprofloxacin and moxifloxacin. Patients with poor initial treatment were treated with levofloxacin or ciprofloxacin on the basis of drug sensitivity for bacterial infections. Moxifloxacin was used to treat Mycoplasma pneumoniae infection in patients poorly treated by macrolides antibiotics or macrolide-resistance gene positive. Among the 24 cases, 1 case (4.2 %) was cured, 19 cases (79.1 %) treated effectively, 1 case had no response (4.2 %), and 3 cases (12.5 %) discharged automatically during treatment, with a response rate of 83.3 %. The consciousness, skin, joint and gastrointestinal function were closely observed; blood routine test, liver and kidney function were closely monitored. There were no adverse drug reactions during the period of medication. Conclusions Quinolones can improve efficacy and prognosis for pediatric patients with severe infections. There are no drug related adverse reactions, indicating a safe short-term use. The use of quinolones in children is super-instructive, and clinical pharmacists should assist physicians in doing related work and reduce medical risks. Abstract:Antofloxacin hydrochloride is a newly developed fluoroquinolone antibacterial in China, which has comparable in vitro and animal pharmacological and toxicological properties to levofloxacin and is worthy of further clinical trial. Human tolerance of single escalating doses from 50 to 500mg was shown to be safe. The purpose of the present study was to evaluate the pharmacokinetic characteristics of a single oral dose of antofloxacin hydrochloride in Chinese healthy male volunteers. Twelve subjects were randomized for administration of a single dose of 300, 400 and 500 mg antofloxacin hydrochloride in a 3-way crossover design. High pressure liquid chromatography (HPLC) was used to assay the serum and urine concentrations of antofloxacin in samples collected over a period of 72 h following drug administration. All three dosages were well tolerated. Antofloxacin hydrochloride demonstrated linear pharmacokinetic characteristics with mean pharmacokinetic values varying from 0.09 to 0.101/h/kg for CLIF and from 2.75-3.001/kg for V/F The t(1/2 beta) was around 20 h and the mean fraction of dose excreted in urine varied from 0.40 to 0.46. Based on these data, 300 mg of antofloxacin hydrochloride administered once daily was selected for further investigation in a multiple dose administration study. Abstract:A study was conducted to determine the efficacy and safety of cefpodoxime proxetil (CFP) dispersible tablets in the treatment of respiratory tract infection in children and to analyze the antibacterial effect of cefpodoxime proxetil nanoemulsion (CFP-NE). A retrospective analysis was conducted on 118 children with upper respiratory tract infection who received antibiotic treatment between March 2018 and March 2021. The control group was treated with cefradine and the CFP group was treated with CFP dispersible tablets. The serum indexes and bacteriological test results of the two groups were collected before and after treatment, and the bacterial clearance rate, infection control, and occurrence of adverse reactions were observed and compared between the two groups. Cefpodoxime proxetil nanoemulsion (CFP-NE) was prepared and its surface characteristics were observed. CFP-NE and CFP were used to treat infections by multiple strains of bacteria and the antibacterial effect and minimum inhibitory concentration (MIC) of CFP-NE were observed. The bacterial strain clearance rate of the CFP group was 96.2% and the total infection control rate was 94.92%. The bacterial strain clearance rate of the control group was 88.9% and the total infection control rate was 81.36%. Serum index detection showed that antibiotic treatment could reduce serum indexes of amyloid A, C-reactive protein, procalcitonin, and other indexes and CFP had better efficacy. There were only four cases of adverse reactions after treatment with CFP, a result similar to that of other antibiotics. The prepared CFP-NE system was stable, the particles were uniform, and the diameter of the inhibitory zone against pathogenic Escherichia coli and Salmonella was larger than that of the CFP group. The MIC of CFP-NE was significantly lower than the MIC of the CFP group ( P < 0.05). CFP dispersible tablets are safe and effective for the treatment of respiratory tract infection in children and the antibacterial effect of CFP-NE is greater than that of CFP.

Abstract:Virtually all models for reconstructing ancestral states for discrete characters make the crucial assumption that the trait of interest evolves at a uniform rate across the entire tree. However, this assumption is unlikely to hold in many situations, particularly as ancestral state reconstructions are being performed on increasingly large phylogenies. Here, we show how failure to account for such variable evolutionary rates can cause highly anomalous (and likely incorrect) results, while three methods that accommodate rate variability yield the opposite, more plausible, and more robust reconstructions. The random local clock method, implemented in BEAST, estimates the position and magnitude of rate changes on the tree; split BiSSE estimates separate rate parameters for pre-specified clades; and the hidden rates model partitions each character state into a number of rate categories. Simulations show the inadequacy of traditional models when characters evolve with both asymmetry (different rates of change between states within a character) and heterotachy (different rates of character evolution across different clades). The importance of accounting for rate heterogeneity in ancestral state reconstruction is highlighted empirically with a new analysis of the evolution of viviparity in squamate reptiles, which reveal a predominance of forward (oviparous-viviparous) transitions and very few reversals.
Population Pharmacokinetics of Oral Levofloxacin 500mg Once-Daily Dosage in Community-Acquired Lower Respiratory Tract Infections: Results of a Prospective Multicenter Study in China

Abstract:This study aimed to explore the pharmacokinetic features of levofloxacin (LVFX) in Chinese patients with infections and to confirm oral LVFX 500 mg once daily as an optimal treatment regimen based on pharmacokinetic-pharmacodynamic (PK-PD) analysis. A total of 1052 plasma samples from 164 Chinese adult patients with communityacquired lower respiratory tract infections (CALRTIs) and 18 healthy volunteers were used for population PK analysis. LVFX 500-mg tablets were given once daily. A nonlinear mixed effects model (NONMEM) program was used for population PK model-building and a two-compartment model with first-order absorption process was established. Creatinine clearance (CL cr ) and body weight were identified as intrinsic factors which significantly affected oral clearance (CL t /F) and the apparent volume of distribution of the central compartment (V1/F), respectively. The final model is described as follows: CL t /F (l/h) = (8.97 + 0.917 × (CL cr (ml/min) − 100.92) × 60/1000) × exp (η CLt/F ). V1/F (l) = (85.3 + 1.22 × (weight (kg) − 60.75)) × exp (η V1/F ). Q/F (l/h) = 0.351. V2/F (l) = 6.81. k a (h −1 ) = 1.44 × exp(η ka ). Based on the population PK model, mean C max and AUC 0–24h in CALRTI patients were estimated as 5.13 µg/ml and 58.98 µg·h/ml, respectively. A subgroup analysis showed that patients with mild renal dysfunction (50 ml/min ≤ CL cr < 80 ml/min) had 34% higher AUC 0–24h values compared to patients with normal renal function (CL cr ≥ 80 ml/min). Postmodeling simulation using final population PK estimates also showed that C max and AUC 0–24h increased markedly in patients with severe renal dysfunction. The results indicate that LVFX dosage adjustment should be individualized on the basis of the CL cr , especially in those with CL cr less than 50 ml/min. None of the PK parameters had any correlation with the occurrence of adverse events. PK-PD analysis indicated that, in patients treated with LVFX 500 mg once daily, the AUC 0–24h /MIC ratio exceeded the target for those major CALRTI pathogens isolated. In addition, the C max /MIC ratio reached 5 for Streptococcus pneumoniae , indicating that the emergence of LVFX-resistant S. pneumoniae could be prevented during the therapy with this dosage regimen. These results demonstrate that oral LVFX 500 mg once daily has favorable PK parameters and PK-PD features in patients with CALRTIs, and the results strongly support this dosage regimen for the treatment of CALRTI.

Abstract:Shigella causes shigellosis that requires antibiotic treatment in severe cases. Sublethal antibiotic concentrations can promote resistance, but their effect on antibiotic-sensitive bacteria before resistance development is unclear. This study investigated the effects of sublethal norfloxacin (NOR) challenges on a NOR-sensitive strain, Shigella sonnei UKMCC1015. Firstly, the whole genome of S. sonnei UKMCC1015 was assembled, and 45 antimicrobial resistance (AMR) genes were identified. Interestingly, transcriptomic analysis showed that low NOR levels do not change either the expression of the AMR genes or NOR targets such as gyrA . Instead, multiple ribosomal protein genes were downregulated, which could be attributed to decreased ribosomal protein promoter activity, modulated by elevated guanosine pentaphosphate and tetraphosphate (ppGpp) levels. This alarmone is involved in the bacterial stringent response during environmental stress, and it is mainly produced from the ppGpp synthetase ( relA ). Additionally, we observed that a relA overexpression (prolonged period of elevated ppGpp levels) may negatively affect the NOR tolerance of the bacteria. In conclusion, this study revealed that a NOR-sensitive strain responds differently to sublethal NOR than commonly reported in resistant strains.

Use of norfloxacin in poultry production in the eastern province of Saudi Arabia and its possible impact on public health

Abstract:Samples of market-ready chicken muscle and liver from 32 local broiler farms were first screened for antibiotic residues by microbiological assay. The antibiotic-residue-positive muscles and livers from 22 farms were further analysed for norfloxacin (NFX) residues by high performance liquid chromatography. NFX was detected in 35.0% and 56.7% of raw antibiotic-residue-positive muscles and livers, respectively. The NFX-positive muscles and livers were respectively obtained from 11 (50.0%) and 14 (63.6%) of the 22 antibiotic-residue-positive farms. Since the maximum residue limit (MRL) for NFX has not yet been fixed, the MRL for enrofloxacin was used in the study. All NFX-positive farms had mean raw tissue levels, which were 2.7- to 34.3-fold higher than the MRL. Although cooking markedly reduced NFX tissue concentrations, mean detectable levels remained above MRL in large proportions of NFX-positive samples and farms. Susceptibility patterns of Enterobacteriaceae isolates from chicken and human patients to NFX showed alarmingly high rates of resistance in chicken isolates especially among Escherichia coli (45.9%) and Pseudomonas spp. (70.6%) compared with patients' isolates (10.5% and 18.2%, respectively). The study reveals widespread misuse of NFX in the local poultry industry, which may pose a major risk to public health including possible stimulation of bacterial resistance and hypersensitivity reactions to fluoroquinolones. More prudent use of fluoroquinolones in food-producing animals is therefore recommended. Further, there is a need to establish MRL values for NFX.

Abstract:Pharmacokinetic, bacteriological and clinical studies on cefpodoxime proxetil (CPDX-PR, CS-807), a newly developed oral cephem, were carried out in the treatment of infectious diseases in the field of pediatrics. 1. Since CPDX demonstrates very powerful antimicrobial actions against such Gram-negative bacilli as Escherichia coli, Salmonella sp., Klebsiella pneumoniae and Serratia sp., such Gram-positive cocci as Streptococcus pyogenes and Streptococcus pneumoniae, and beta-lactamase producing Branhamella catarrhalis and Haemophilus influenzae, this drug was thought to be useful for the treatment of pediatric infectious diseases when main causative bacteria in the field of pediatrics were taken into account. 2. When changes in blood and urine concentrations of CPDX following the administration of this drug at 3.7 mg/kg before meal were determined, Cmax and T1/2 were found to be 2.98 micrograms/ml at 2-hour and 1.73 hours, respectively; an urinary excretion rate in the first 6 hours and a maximum urine concentration were 32.5% and 52 micrograms/ml, respectively. 3. Clinically, 8 of 8 patients with the upper respiratory tract infections (100%), 28 of 29 patients with bronchitis and/or pneumonia (96.6%), 3 of 4 patients with otitis media (75%), 2 of 2 patients with sinusitis (100%), 3 of 3 patients with the skin soft tissue infections (100%), 1 of 1 patient with bacterial enteritis (100%) and 11 of 14 patients with urinary tract infections (78.6%) responded well to the treatment with CPDX-PR, showing a 91.8% efficacy rate in all the patients treated. 4. Bacteriologically, Staphylococcus aureus, Staphylococcus epidermidis, S. pyogenes, S. pneumoniae, E. faecalis, B. catarrhalis, H. influenzae, E. coli and Salmonella typhimurium were all eradicated from 5, 1, 4, 6, 1, 5, 5, 11 and 1 patient, respectively. An eradication rate in all the patients examined was 97.5% (39/40). 5. Gastrointestinal symptoms appeared as side effects in 2 of 71 patients (vomiting in 1 and diarrhea in 1), hence, an incidence of side effects was 2.8% (2/71). As for abnormal laboratory findings, eosinophilia, thrombocytosis and increases in GOT and GPT were observed in 3 of 39 patients examined (7.7%), 1 of 39 patients (2.6%) and 2 of 34 patients (5.9%), respectively. In addition, we also examined the effect of the drug on the hemostatic system, but found no changes upon the treatment. Based on these results, it appeared that CPDX-PR was a useful and safe drug in treatment of infectious diseases in the field of pediatrics when administered 2-3 times a day at a dose of 3-6 mg/kg.
Multi-center Clinical Studies on Pazufloxacin Sodium Chloride Injection in Treatment for Acute Bacterial Infections

Abstract:Objective To evaluate efficacy and safety of locally-produced pazufloxacin mesilate sodium chloride injection in the treatment of bacterial infections of respiratory and urinary tract.Methods A multi-center double-blind randomized controlled clinical trial was carried out to evaluate efficacy and safety of pazufloxacin mesilate sodium chloride in treatment for acute bacterial infection, as compared to those of levofloxacin hydrochloride and glucose injection as control treatment.A total of 244 patients with acute bacterial infection of respiratory and urinary tract were enrolled in the studies.120 in trial group and 120 in control group, with four withdrawals.Pazufloxacin mesilate and levofloxacin were administered intravenously by drip at a dose of 300 mg and 200 mg, every 12 hours for 7 to 14 days for trial and control groups, respectively.Resuits Overall efficacy of pazufloxacin mesilate was 77.0 percent and 93.5 percent in treatment for acute bacterial infections of respiratory and urinary tract.respectively, and that of levofloxacin was 80.6 percent and 89.6 percent, respectively.Overall bacterial clearance rate WaS 91.5 percent for pazufloxacin mesilate, 89.6 percent for respiratory tract infection and 94.1 percent for urinary tract infection, respectively.and 93.4 percent for levofloxacin, 97.3 percent for respiratory tract infection and 89.7 percent for urinary tract infection, respectively.No significant difference in adverse drug reactions between the two groups(P＞0.05)was found, with 4.88 percent and 7.44 percent for trial and control groups, respectively.Conclusions Pazufloxacin mesilate sodium chloride injection produced locally is a safe and effective antibiotic in treatment for acute infections of respiratory and urinary tract.

Abstract:Background: The safety of fluoroquinolones in the pediatric population has been a matter of concern due to the potential for adverse effects such as growth cartilage toxicity. Despite their broad antimicrobial spectrum and efficacy, the prescription of fluoroquinolones in children and adolescents is limited by the risk of arthralgias and other side effects. This study aims to evaluate the safety of fluoroquinolones in pediatric patients and the impact of regulatory actions on their prescription. Material and methods: A systematic review of studies investigating the safety of fluoroquinolones in the pediatric population was conducted. Studies that met specific inclusion criteria were selected, and relevant information on the decrease in fluoroquinolone prescriptions and their adverse effects was extracted. Results: In the study on the safety and tolerability of quinolones in the pediatric population, 22% (199/904) of patients experienced adverse events following treatment with quinolones. Gastrointestinal disorders, such as nausea and diarrhea, were the most frequent, affecting 12% (108/904) of patients. This was followed by skin rashes and effects on the central nervous system, at 5% (45/904) and 3% (27/904) respectively. Moxifloxacin demonstrated good tolerability across all age cohorts, without significant joint or neuropathic adverse findings. Specific safety studies of moxifloxacin confirmed its safety, showing no significant differences in safety variables compared to other antibiotics. Conclusion: It is crucial to continue monitoring the safety of fluoroquinolones in the pediatric population to ensure an adequate balance between the benefits and risks of their use.
A Phase Ii Study of Antofloxacin Hydrochloride, A Novel Fluoroquinolone, for the Treatment of Acute Bacterial Infections

Abstract:Aims: To evaluate the clinical and bacterial efficacies and the safety of antofloxacin hydrochloride tablets in the treatment of acute exacerbations of chronic bronchitis (AECB) and acute pyelonephritis (AP). Patients and Methods: A randomized, controlled, multicenter, double-blind, double-dummy clinical trial was conducted at 6 hospitals in China. The patients in the investigational group took an oral 400-mg loading dose of antofloxacin on the first day followed by 200 mg daily. Control group patients took 200 mg of levofloxacin orally twice daily for 7-14 days. Results: A total of 284 patients were enrolled into the study, including 140 cases of AECB and 144 cases of AP; 141 patients were in the antofloxacin group and 143 patients were in the levofloxacin group. Two hundred fifty-four patients completed the entire treatment and follow-up and comprised the per-protocol analysis data set; 30 patients were excluded because of informed consent withdrawal, loss to follow-up, protocol violations, or adverse drug events. Treatment was ceased in 3 patients in the antofloxacin group and in 1 patient in the levofloxacin group because of skin rashes, dizziness, or adverse gastrointestinal effects. At the end of the 2-regimen treatment, the observed effectiveness rates for AECB were 90.3 and 87.7% for the treatment and control groups, respectively, and 96.9 and 92.1% for AP. The overall bacterial eradication rates were 95.9 and 92.4%, and drug-related adverse events were observed in 13.1 and 10.1% of patients, respectively. There were no severe adverse effects. No statistical differences in drug efficacy and safety were detected between the 2 treatment groups (p > 0.05). Conclusions: Antofloxacin hydrochloride is an effective and well-tolerated new fluoroquinolone that demonstrates clinical and bacteriological efficacies similar to levofloxacin for the treatment of AECB and AP. Abstract:Norfloxacin (NORF) is a broad-spectrum quinolone that is widely utilized for the treatment of various bacterial infections and is considered one of the most commonly used fluoroquinolone antibiotics. However, NORF's clinical utility is limited by its poor water solubility and relatively low oral bioavailability. This study presents an optimization and synergistic enhancement approach through salt/co-crystal, aiming to maximize the biopharmaceutical properties of NORF with the use of phenolic acid. Following this strategy, two new hydrate salts of NORF with phenolic acid, namely, NORF—3,5-DBA hydrate (salt 1) and NORF—VA hydrate (salt 2), were prepared and systematically confirmed. Two hydrate salts were produced by means of the slow evaporation crystallization method, and the structures were determined through single-crystal X-ray diffraction (SCXRD). Additionally, powder X-ray diffraction (PXRD), Fourier-transform infrared (FT-IR) spectroscopy, differential scanning calorimetry (DSC), thermogravimetric analysis (TGA), and high-performance liquid chromatography (HPLC) were applied to analyze the features of the two salts. The experimental results indicated that the formation of the two salts could enhance the solubility and improve the release behavior of NORF. Interestingly, the physicochemical properties of NORF were significantly improved as a result, leading to an enhancement in its antibacterial activity. This was demonstrated by the enhanced inhibition of bacterial strains and the lower minimum inhibitory concentration values.

Abstract:Elevated expression or activity of the epidermal growth factor receptor (EGFR) is common in ovarian cancer and is associated with poor patient prognosis. A naturally occurring EGFR mutation termed variant III (EGFRvIII) has been detected in many human tumors, including those of the ovary. This mutant receptor does not bind EGF; however, it is constitutively active as detected by receptor dimerization, autophosphorylation, and stimulation of signal transduction cascades. To identify the consequences of EGFRvIII expression in ovarian tumor cells, we introduced EGFRvIII into the epithelial ovarian cancer cell line OVCA 433. The EGFRvIII-transfected cells displayed a motile phenotype, defects in cell spreading, and decreased integrin alpha2 protein expression as detected by Western blot analysis and flow cytometry. Inhibition of EGFRvIII catalytic activity using the EGFR-selective tyrphostin AG1478 restored integrin alpha2 expression within 4 to 8 hours after treatment. The modulation of integrin alpha2 expression corresponded to marked changes in the actin cytoskeleton as detected by redistribution of filamentous-actin. Furthermore, focal adhesions were evident only when EGFRvIII activity was inhibited. Together, these findings suggest that expression of the constitutively active mutant EGFRvIII promotes changes in cell shape and focal adhesion formation, mediated in part through specific modulation of integrin alpha2 expression and function. We conclude that EGFR-activating mutations, such as EGFRvIII, in ovarian cancer may contribute to a more aggressive disease.
