Abstract:For many years, psychiatrists have tried to understand factors involved in response to medications or psychotherapies, in order to personalize their treatment choices. There is now a broad and growing interest in the idea that we can develop models to personalize treatment decisions using new statistical approaches from the field of machine learning and applying them to larger volumes of data. In this pursuit, there has been a paradigm shift away from experimental studies to confirm or refute specific hypotheses towards a focus on the overall explanatory power of a predictive model when tested on new, unseen datasets. In this paper, we review key studies using machine learning to predict treatment outcomes in psychiatry, ranging from medications and psychotherapies to digital interventions and neurobiological treatments. Next, we focus on some new sources of data that are being used for the development of predictive models based on machine learning, such as electronic health records, smartphone and social media data, and on the potential utility of data from genetics, electrophysiology, neuroimaging and cognitive testing. Finally, we discuss how far the field has come towards implementing prediction tools in real‐world clinical practice. Relatively few retrospective studies to‐date include appropriate external validation procedures, and there are even fewer prospective studies testing the clinical feasibility and effectiveness of predictive models. Applications of machine learning in psychiatry face some of the same ethical challenges posed by these techniques in other areas of medicine or computer science, which we discuss here. In short, machine learning is a nascent but important approach to improve the effectiveness of mental health care, and several prospective clinical studies suggest that it may be working already.

Abstract:Precision psychiatry is an ermerging field that aims to provide individualized approaches to mental health care. Multivariate analysis and machine learning are used to create outcome prediction models based on clinical data such as demographics, symptom assessments, genetic information, and brain imaging. While much emphasis has been placed on technical innovation, the complex and varied nature of mental health presents significant challenges to the successful implementation of these models. From this perspective, I review ten challenges in the field of precision psychiatry, including the need for studies on real-world populations and realistic clinical outcome definitions, consideration of treatment-related factors such as placebo effects and non-adherence to prescriptions. Fairness, prospective validation in comparison to current practice and implementation studies of prediction models are other key issues that are currently understudied. A shift is proposed from retrospective studies based on linear and static concepts of disease towards prospective research that considers the importance of contextual factors and the dynamic and complex nature of mental health.

Abstract:Objective: Early identification of individuals who are at risk for suicide is crucial in supporting suicide prevention. Machine learning is emerging as a promising approach to support this objective. Machine learning is broadly defined as a set of mathematical models and computational algorithms designed to automatically learn complex patterns between predictors and outcomes from example data, without being explicitly programmed to do so. The model's performance continuously improves over time by learning from newly available data. Method: This concept paper explores how machine learning approaches applied to healthcare data obtained from electronic health records, including billing and claims data, can advance our ability to accurately predict future suicidal behavior. Results: We provide a general overview of machine learning concepts, summarize exemplar studies, describe continued challenges, and propose innovative research directions. Conclusion: Machine learning has potential for improving estimation of suicide risk, yet important challenges and opportunities remain. Further research can focus on incorporating evolving methods for addressing data imbalances, understanding factors that affect generalizability across samples and healthcare systems, expanding the richness of the data, leveraging newer machine learning approaches, and developing automatic learning systems.

Abstract:Summary In light of the National Institute of Mental Health (NIMH)'s Research Domain Criteria (RDoC), the advent of functional neuroimaging, novel technologies and methods provide new opportunities to develop precise and personalized prognosis and diagnosis of mental disorders. Machine learning (ML) and artificial intelligence (AI) technologies are playing an increasingly critical role in the new era of precision psychiatry. Combining ML/AI with neuromodulation technologies can potentially provide explainable solutions in clinical practice and effective therapeutic treatment. Advanced wearable and mobile technologies also call for the new role of ML/AI for digital phenotyping in mobile mental health. In this review, we provide a comprehensive review of ML methodologies and applications by combining neuroimaging, neuromodulation, and advanced mobile technologies in psychiatry practice. We further review the role of ML in molecular phenotyping and cross-species biomarker identification in precision psychiatry. We also discuss explainable AI (XAI) and neuromodulation in a closed human-in-the-loop manner and highlight the ML potential in multi-media information extraction and multi-modal data fusion. Finally, we discuss conceptual and practical challenges in precision psychiatry and highlight ML opportunities in future research.

Abstract:To understand the complex nature of heterogeneous psychiatric disorders, scientists and clinicians are required to employ a wide range of clinical, endophenotypic, neuroimaging, genomic, and environmental data to understand the biological mechanisms of psychiatric illness before this knowledge is applied into clinical setting. Machine learning (ML) is an automated process that can detect patterns from large multidimensional datasets and can supersede conventional statistical methods as it can detect both linear and non-linear relationships. Due to this advantage, ML has potential to enhance our understanding, improve diagnosis, prognosis and treatment of psychiatric disorders. The current review provides an in-depth examination of, and offers practical guidance for, the challenges encountered in the application of ML models in psychiatric research and clinical practice. These challenges include the curse of dimensionality, data quality, the 'black box' problem, hyperparameter tuning, external validation, class imbalance, and data representativeness. These challenges are particularly critical in the context of psychiatry as it is expected that researchers will encounter them during the stages of ML model development and deployment. We detail practical solutions and best practices to effectively mitigate the outlined challenges. These recommendations have the potential to improve reliability and interpretability of ML models in psychiatry.

Abstract:Informatics paradigms for brain and mental health research have seen significant advances in recent years. These developments can largely be attributed to the emergence of new technologies such as machine learning, deep learning, and artificial intelligence. Data-driven methods have the potential to support mental health care by providing more precise and personalised approaches to detection, diagnosis, and treatment of depression. In particular, precision psychiatry is an emerging field that utilises advanced computational techniques to achieve a more individualised approach to mental health care. This survey provides an overview of the ways in which artificial intelligence is currently being used to support precision psychiatry. Advanced algorithms are being used to support all phases of the treatment cycle. These systems have the potential to identify individuals suffering from mental health conditions, allowing them to receive the care they need and tailor treatments to individual patients who are mostly to benefit. Additionally, unsupervised learning techniques are breaking down existing discrete diagnostic categories and highlighting the vast disease heterogeneity observed within depression diagnoses. Artificial intelligence also provides the opportunity to shift towards evidence-based treatment prescription, moving away from existing methods based on group averages. However, our analysis suggests there are several limitations currently inhibiting the progress of data-driven paradigms in care. Significantly, none of the surveyed articles demonstrate empirically improved patient outcomes over existing methods. Furthermore, greater consideration needs to be given to uncertainty quantification, model validation, constructing interdisciplinary teams of researchers, improved access to diverse data and standardised definitions within the field. Empirical validation of computer algorithms via randomised control trials which demonstrate measurable improvement to patient outcomes are the next step in progressing models to clinical implementation.

Prediction of Clinical Outcomes in Psychotic Disorders Using Artificial Intelligence Methods: A Scoping Review

Abstract:Background: Psychotic disorders are major psychiatric disorders that can impact multiple domains including physical, social, and psychological functioning within individuals with these conditions. Being able to better predict the outcomes of psychotic disorders will allow clinicians to identify illness subgroups and optimize treatment strategies in a timely manner. Objective: In this scoping review, we aimed to examine the accuracy of the use of artificial intelligence (AI) methods in predicting the clinical outcomes of patients with psychotic disorders as well as determine the relevant predictors of these outcomes. Methods: This review was guided by the PRISMA Guidelines for Scoping Reviews. Seven electronic databases were searched for relevant published articles in English until 1 February 2024. Results: Thirty articles were included in this review. These studies were mainly conducted in the West (63%) and Asia (37%) and published within the last 5 years (83.3%). The clinical outcomes included symptomatic improvements, illness course, and social functioning. The machine learning models utilized data from various sources including clinical, cognitive, and biological variables such as genetic, neuroimaging measures. In terms of main machine learning models used, the most common approaches were support vector machine, random forest, logistic regression, and linear regression models. No specific machine learning approach outperformed the other approaches consistently across the studies, and an overall range of predictive accuracy was observed with an AUC from 0.58 to 0.95. Specific predictors of clinical outcomes included demographic characteristics (gender, socioeconomic status, accommodation, education, and employment); social factors (activity level and interpersonal relationships); illness features (number of relapses, duration of relapses, hospitalization rates, cognitive impairments, and negative and disorganization symptoms); treatment (prescription of first-generation antipsychotics, high antipsychotic doses, clozapine, use of electroconvulsive therapy, and presence of metabolic syndrome); and structural and functional neuroimaging abnormalities, especially involving the temporal and frontal brain regions. Conclusions: The current review highlights the potential and need to further refine AI and machine learning models in parsing out the complex interplay of specific variables that contribute to the clinical outcome prediction of psychotic disorders.

Abstract:Machine learning (ML) has emerged as a promising tool in psychiatry, revolutionising diagnostic processes and patient outcomes. In this paper, I argue that while ML studies show promising initial results, their application in mimicking clinician-based judgements presents inherent limitations (Shatte et al. in Psychol Med 49:1426–1448. https://doi.org/10.1017/S0033291719000151, 2019). Most models still rely on DSM (the Diagnostic and Statistical Manual of Mental Disorders) categories, known for their heterogeneity and low predictive value. DSM's descriptive nature limits the validity of psychiatric diagnoses, which leads to overdiagnosis, comorbidity, and low remission rates. The application in psychiatry highlights the limitations of supervised ML techniques. Supervised ML models inherit the validity issues of their training data set. When the model's outcome is a DSM classification, this can never be more valid or predictive than the clinician's judgement. Therefore, I argue that these models have little added value to the patient. Moreover, the lack of known underlying causal pathways in psychiatric disorders prevents validating ML models based on such classifications. As such, I argue that high accuracy in these models is misleading when it is understood as validating the classification. In conclusion, these models will not will not offer any real benefit to patient outcomes. I propose a shift in focus, advocating for ML models to prioritise improving the predictability of prognosis, treatment selection, and prevention. Therefore, data selection and outcome variables should be geared towards this transdiagnostic goal. This way, ML can be leveraged to better support clinicians in personalised treatment strategies for mental health patients.

Abstract:Applications of machine learning in the biomedical sciences are growing rapidly. This growth has been spurred by diverse cross-institutional and interdisciplinary collaborations, public availability of large datasets, an increase in the accessibility of analytic routines, and the availability of powerful computing resources. With this increased access and exposure to machine learning comes a responsibility for education and a deeper understanding of its bases and bounds, borne equally by data scientists seeking to ply their analytic wares in medical research and by biomedical scientists seeking to harness such methods to glean knowledge from data. This article provides an accessible and critical review of machine learning for a biomedically informed audience, as well as its applications in psychiatry. The review covers definitions and expositions of commonly used machine learning methods, and historical trends of their use in psychiatry. We also provide a set of standards, namely G uidelines for RE porting M achine L earning I nvestigations in N europsychiatry ( GREMLIN ), for designing and reporting studies that use machine learning as a primary data-analysis approach. Lastly, we propose the establishment of the Machine Learning in Psychiatry ( MLPsych ) Consortium, enumerate its objectives, and identify areas of opportunity for future applications of machine learning in biological psychiatry. This review serves as a cautiously optimistic primer on machine learning for those on the precipice as they prepare to dive into the field, either as methodological practitioners or well-informed consumers.

Abstract:Machine learning (ML) may help to predict successful psychotherapy outcomes and to identify relevant predictors of success. So far, ML applications are scant in psychotherapy research and they are typically based on small samples or focused on specific diagnoses. In this study, we predict successful therapy outcomes with ML in a heterogeneous sample in routine outpatient care. We trained established ML models (decision trees and ensembles of them) with routinely collected clinical baseline information from n = 685 outpatients to predict a successful outcome of cognitive behavioral therapy. Treatment success was defined as clinically significant change (CSC) on the Brief-Symptom-Checklist (reached by 326 patients; 48%). The best performing model (Gradient Boosting Machines) achieved a balanced accuracy of 69% (p < .001) on unseen validation data. Out of 383 variables, we identified the 16 most important predictors, which were still able to predict CSC with 67% balanced accuracy. Our study demonstrates that ML models built on data, which is typically available at the outset of therapy, can predict whether an individual will substantially benefit from the intervention. Some of the predictors were theoretically expected (e.g., level of functioning), but others need further validation (e.g., somatization). From a theoretical and practical perspective, ML is clearly an attractive addition to more established psychotherapy research methodology.

Abstract:Background: Schizophrenia is a severe psychiatric disorder associated with a significant negative impact. Early diagnosis and treatment of schizophrenia has a favorable effect on the clinical outcome and patients quality of life. In this context, machine learning techniques open up new opportunities for a more accurate diagnosis and prediction of the clinical features of this illness. Aim: This literature review is aimed to search for information on the use of machine learning techniques in the prediction and diagnosis of schizophrenia and the determination of its clinical features. Methods: The Google Scholar, PubMed, and eLIBRARY.ru databases were used to search for relevant data. The review included articles that had been published not earlier than January 1, 2010, and not later than March 31, 2023. Combinations of the following keywords were applied for search queries: machine learning, deep learning, schizophrenia, neural network, predictors, artificial intelligence, diagnostics, suicide, depressive, insomnia, and cognitive. Original articles regardless of their design were included in the review. Descriptive analysis was used to summarize the retrieved data. Results: Machine learning techniques are widely used in the functional assessment of patients with schizophrenia. They are used for interpretation of MRI, EEG, and actigraphy findings. Also, models created using machine learning algorithms can analyze speech, behavior, and the creativity of people and these data can be used for the diagnosis of psychiatric disorders. It has been found that different machine learning-based models can help specialists predict and diagnose schizophrenia based on medical history and genetic data, as well as epigenetic information. Machine learning techniques can also be used to build effective models that can help specialists diagnose and predict clinical manifestations and complications of schizophrenia, such as insomnia, depressive symptoms, suicide risk, aggressive behavior, and changes in cognitive functions over time. Conclusion: Machine learning techniques play an important role in psychiatry, as they have been used in models that help specialists in the diagnosis of schizophrenia and determination of its clinical features. The use of machine learning algorithms is one of the most promising direction in psychiatry, and it can significantly improve the effectiveness of the diagnosis and treatment of schizophrenia.

Abstract:Precision medicine has the ambition to improve treatment response and clinical outcomes through patient stratification, and holds great potential in mental disorders. However, several important factors are needed to transform current practice into a "precision psychiatry" framework. Most important are (1) the generation of accessible large real-world training and test data including genomic data integrated from multiple sources, (2) the development and validation of advanced analytical tools for stratification and prediction, and (3) the development of clinically useful management platforms for patient monitoring that can be integrated into healthcare systems in real-life settings. This narrative review summarizes strategies for obtaining the key elements – well-powered samples from large biobanks, integrated with electronic health records and health registry data using novel artificial intelligence algorithms – to predict outcomes in severe mental disorders and translate these models into clinical management and treatment approaches. Key elements are massive mental health data and novel artificial intelligence algorithms. For the clinical translation of these strategies, we discuss a precision medicine platform for improved management of mental disorders. We include use cases to illustrate how precision medicine interventions could be brought into psychiatry to improve the clinical outcomes of mental disorders.

Abstract:Machine and deep learning methods, today’s core of artificial intelligence, have been applied with increasing success and impact in many commercial and research settings. They are powerful tools for large scale data analysis, prediction and classification, especially in very data-rich environments (“big data”), and have started to find their way into medical applications. Here we will first give an overview of machine learning methods, with a focus on deep and recurrent neural networks, their relation to statistics, and the core principles behind them. We will then discuss and review directions along which (deep) neural networks can be, or already have been, applied in the context of psychiatry, and will try to delineate their future potential in this area. We will also comment on an emerging area that so far has been much less well explored: by embedding semantically interpretable computational models of brain dynamics or behavior into a statistical machine learning context, insights into dysfunction beyond mere prediction and classification may be gained. Especially this marriage of computational models with statistical inference may offer insights into neural and behavioral mechanisms that could open completely novel avenues for psychiatric treatment.

Abstract:The use of clinical prediction models to produce individualised risk estimates can facilitate the implementation of precision psychiatry. As a source of data from large, clinically representative patient samples, electronic health records (EHRs) provide a platform to develop and validate clinical prediction models, as well as potentially implementing them in routine clinical care. The present review describes promising use cases for the application of precision psychiatry to EHR data and considers their performance in terms of discrimination (ability to separate individuals with and without the outcome) and calibration (extent to which predicted risk estimates correspond to observed outcomes), as well as their potential clinical utility (weighing benefits and costs associated with the model compared to different approaches across different assumptions of the number-needed-to-test). We review four externally validated clinical prediction models designed to predict, respectively: psychosis onset, psychotic relapse, cardiometabolic morbidity, and suicide risk. We then discuss the prospects for clinically implementing these models, and the potential added value of integrating data from evidence syntheses, standardised psychometric assessments, and biological data into EHRs. Clinical prediction models can utilise routinely collected EHR data in an innovative way, representing a unique opportunity to inform real-world clinical decision making. Combining data from other sources (e.g. meta-analyses) or enhancing EHR data with information from research studies (clinical and biomarker data) may enhance our abilities to improve performance of clinical prediction models.

Abstract:Hoarding disorder (HD) is characterized by a compulsion to collect belongings, and to experience significant distress when parting from them. HD is often misdiagnosed for several reasons. These include patient and family lack of recognition that it is a psychiatric disorder and professionals' lack of relevant expertise with it. This study evaluates the ability of a supervised machine learning (ML) model to match the diagnostic skills of psychiatrists when presented with equivalent information pertinent to symptoms of HD.

Predicting adolescent psychopathology from early life factors: A machine learning tutorial

Abstract:Objective: The successful implementation and interpretation of machine learning (ML) models in epidemiological studies can be challenging without an extensive programming background. We provide a didactic example of machine learning for risk prediction in this study by determining whether early life factors could be useful for predicting adolescent psychopathology. Methods: In total, 9643 adolescents ages 9-10 from the Adolescent Brain and Cognitive Development (ABCD) Study were included in ML analysis to predict high Child Behavior Checklist (CBCL) scores (i.e., t-scores ≥ 60). ML models were constructed using a series of predictor combinations (prenatal, family history, sociodemographic) across 5 different algorithms. We assessed ML performance through sensitivity, specificity, F1-score, and area under the curve (AUC) metrics. Results: A total of 1267 adolescents (13.1 %) were found to have high CBCL scores. The best performing algorithms were elastic net and gradient boosted trees. The best performing elastic net models included prenatal and family history factors (Sensitivity 0.654, Specificity 0.713; AUC 0.742, F1-score 0.401) and prenatal, family, history, and sociodemographic factors (Sensitivity 0.668, Specificity 0.704; AUC 0.745, F1-score 0.402). Across all 5 ML algorithms, family history factors (e.g., either parent had nervous breakdowns, trouble holding jobs/fights/police encounters, and counseling for mental issues) and sociodemographic covariates (e.g., maternal age, child's sex, caregiver income and caregiver education) tended to be better predictors of adolescent psychopathology. The most important prenatal predictors were unplanned pregnancy, birth complications, and pregnancy complications. Conclusion: Our results suggest that inclusion of prenatal, family history, and sociodemographic factors in ML models can generate moderately accurate predictions of adolescent psychopathology. Issues associated with model overfitting, hyperparameter tuning, and system seed setting should be considered throughout model training, testing, and validation. Future early risk predictions models may improve with the inclusion of additional relevant covariates.

Abstract:Recent research focuses on mental health and brain informatics. Emerging technologies like AI, deep learning, and machine learning drove the advancements. Customizing, diagnosing, and treating depression with data-driven approaches could improve mental health care. A growing field, precision psychiatry uses cutting-edge computer tools to provide tailored mental health care. AI in precision psychiatry is examined in this paper. Complex formulations aid therapy. These tools can identify and treat mental health patients. They can customize therapies for most patients. Unsupervised learning algorithms have shown considerable sadness-related sickness disparities. These methods separate diagnostic categories. Artificial intelligence could help us suggest drugs based on facts, not group averages. Our findings show that data-driven paradigms in healthcare face several challenges. Surprisingly, none of the survey studies reveal how current procedures improve patient outcomes. Standardizing field terminology, forming diverse research teams, evaluating models, identifying flaws, and making datasets accessible are crucial. Randomized controlled trials must show that computer algorithms improve patient outcomes to make models more feasible.

Abstract:A growing body of evidence now suggests that precision psychiatry, an interdisciplinary field of psychiatry, precision medicine, and pharmacogenomics, serves as an indispensable foundation of medical practices by offering the accurate medication with the accurate dose at the accurate time to patients with psychiatric disorders. In light of the latest advancements in artificial intelligence and machine learning techniques, numerous biomarkers and genetic loci associated with psychiatric diseases and relevant treatments are being discovered in precision psychiatry research by employing neuroimaging and multi-omics. In this review, we focus on the latest developments for precision psychiatry research using artificial intelligence and machine learning approaches, such as deep learning and neural network algorithms, together with multi-omics and neuroimaging data. Firstly, we review precision psychiatry and pharmacogenomics studies that leverage various artificial intelligence and machine learning techniques to assess treatment prediction, prognosis prediction, diagnosis prediction, and the detection of potential biomarkers. In addition, we describe potential biomarkers and genetic loci that have been discovered to be associated with psychiatric diseases and relevant treatments. Moreover, we outline the limitations in regard to the previous precision psychiatry and pharmacogenomics studies. Finally, we present a discussion of directions and challenges for future research.
