Abstract:Background: The purpose of this study was to develop and validate a positioning method with hand-guiding and contact position feedback of robot based on a human-robot collaborative dental implant system (HRCDIS) for robotic guided dental implant surgery. Methods: An HRCDIS was developed based on a light-weight cooperative robot arm, UR5. A three-dimensional (3D) virtual partially edentulous mandibular bone was reconstructed using the cone bone computed tomography images. After designing the preoperative virtual implant planning using the computer software, a fixation guide worn on teeth for linking and fixing positioning marker was fabricated by 3D printing. The fixation guide with the positioning marker and a resin model mimicking the oral tissues were assembled on a head phantom. The planned implant positions were derived by the coordinate information of the positioning marker. The drilling process using the HRCDIS was conducted after mimicking the experimental set-up and planning the drilling trajectory. Deviations between actual and planned implant positions were measured and analyzed. Results: The head phantom experiments results showed that the error value of the central deviation at hex (refers to the center of the platform level of the implant) was 0.79 +/- 0.17 mm, central deviation at the apex was 1.26 +/- 0.27 mm, horizontal deviation at the hex was 0.61 +/- 0.19 mm, horizontal deviation at the apex was 0.91 +/- 0.55 mm, vertical deviation at the hex was 0.38 +/- 0.17 mm, vertical deviation at the apex was 0.37 +/- 0.20 mm, and angular deviation was 3.77 +/- 1.57 degrees. Conclusions: The results from this study preliminarily validate the feasibility of the accurate navigation method of the HRCDIS.
Deformable Image Registration using Unsupervised Deep Learning for CBCT-guided Abdominal Radiotherapy

Abstract:CBCTs in image-guided radiotherapy provide crucial anatomy information for patient setup and plan evaluation. Longitudinal CBCT image registration could quantify the inter-fractional anatomic changes. The purpose of this study is to propose an unsupervised deep learning based CBCT-CBCT deformable image registration. The proposed deformable registration workflow consists of training and inference stages that share the same feed-forward path through a spatial transformation-based network (STN). The STN consists of a global generative adversarial network (GlobalGAN) and a local GAN (LocalGAN) to predict the coarse- and fine-scale motions, respectively. The network was trained by minimizing the image similarity loss and the deformable vector field (DVF) regularization loss without the supervision of ground truth DVFs. During the inference stage, patches of local DVF were predicted by the trained LocalGAN and fused to form a whole-image DVF. The local whole-image DVF was subsequently combined with the GlobalGAN generated DVF to obtain final DVF. The proposed method was evaluated using 100 fractional CBCTs from 20 abdominal cancer patients in the experiments and 105 fractional CBCTs from a cohort of 21 different abdominal cancer patients in a holdout test. Qualitatively, the registration results show great alignment between the deformed CBCT images and the target CBCT image. Quantitatively, the average target registration error (TRE) calculated on the fiducial markers and manually identified landmarks was 1.91+-1.11 mm. The average mean absolute error (MAE), normalized cross correlation (NCC) between the deformed CBCT and target CBCT were 33.42+-7.48 HU, 0.94+-0.04, respectively. This promising registration method could provide fast and accurate longitudinal CBCT alignment to facilitate inter-fractional anatomic changes analysis and prediction.

Robust Orthogonal-View 2-D/3-D Rigid Registration for Minimally Invasive Surgery

Abstract:Intra-operative target pose estimation is fundamental in minimally invasive surgery (MIS) to guiding surgical robots. This task can be fulfilled by the 2-D/3-D rigid registration, which aligns the anatomical structures between intra-operative 2-D fluoroscopy and the pre-operative 3-D computed tomography (CT) with annotated target information. Although this technique has been researched for decades, it is still challenging to achieve accuracy, robustness and efficiency simultaneously. In this paper, a novel orthogonal-view 2-D/3-D rigid registration framework is proposed which combines the dense reconstruction based on deep learning and the GPU-accelerated 3-D/3-D rigid registration. First, we employ the X2CT-GAN to reconstruct a target CT from two orthogonal fluoroscopy images. After that, the generated target CT and pre-operative CT are input into the 3-D/3-D rigid registration part, which potentially needs a few iterations to converge the global optima. For further efficiency improvement, we make the 3-D/3-D registration algorithm parallel and apply a GPU to accelerate this part. For evaluation, a novel tool is employed to preprocess the public head CT dataset CQ500 and a CT-DRR dataset is presented as the benchmark. The proposed method achieves 1.65 ± 1.41 mm in mean target registration error(mTRE), 20% in the gross failure rate(GFR) and 1.8 s in running time. Our method outperforms the state-of-the-art methods in most test cases. It is promising to apply the proposed method in localization and nano manipulation of micro surgical robot for highly precise MIS.

Deep learning based CT-to-CBCT deformable image registration for autosegmentation in head and neck adaptive radiation therapy

Abstract:The purpose of this study is to develop a deep learning based method that can automatically generate segmentations on cone-beam CT (CBCT) for head and neck online adaptive radiation therapy (ART), where expert-drawn contours in planning CT (pCT) can serve as prior knowledge. Due to lots of artifacts and truncations on CBCT, we propose to utilize a learning based deformable image registration method and contour propagation to get updated contours on CBCT. Our method takes CBCT and pCT as inputs, and output deformation vector field and synthetic CT (sCT) at the same time by jointly training a CycleGAN model and 5-cascaded Voxelmorph model together.The CycleGAN serves to generate sCT from CBCT, while the 5-cascaded Voxelmorph serves to warp pCT to sCT's anatommy. The segmentation results were compared to Elastix, Voxelmorph and 5-cascaded Voxelmorph on 18 structures including left brachial plexus, right brachial plexus, brainstem, oral cavity, middle pharyngeal constrictor, superior pharyngeal constrictor, inferior pharyngeal constrictor, esophagus, nodal gross tumor volume, larynx, mandible, left masseter, right masseter, left parotid gland, right parotid gland, left submandibular gland, right submandibular gland, and spinal cord. Results show that our proposed method can achieve average Dice similarity coefficients and 95% Hausdorff distance of 0.83 and 2.01mm. As compared to other methods, our method has shown better accuracy to Voxelmorph and 5-cascaded Voxelmorph, and comparable accuracy to Elastix but much higher efficiency. The proposed method can rapidly and simultaneously generate sCT with correct CT numbers and propagate contours from pCT to CBCT for online ART re-planning.

Abstract:Unexpected stability of arterial oxygen and carbon dioxide tensions occurred in infants recovering from respiratory distress syndrome (RDS) on the abrupt withdrawal of CPAP at 6 cmH2O pressure. 30 preterm infants (birthweights 880--3200 g gestational ages 29--38 weeks) were treated at a mean age of 10 hours and for a mean duration of 62 hours. CPAP was stopped when F1O2 requirement fell below 0.3, and stable PO2 was maintained for 4 hours. Discontinuation of CPAP at 6 cmH2O resulted in a mean change in PO2 from 66 to 64 mmHg (8.8 to 8.5 kPa) and a mean change in PCO2 from 41 to 40 mmHg (5.4 to 5.3 kPa). We conclude that this population of infants suffering from moderate RDS tolerated the abrupt withdrawal of CPAP, and that gradual reduction of pressure was unnecessary.

Abstract:Computerized registration between maxillofacial cone-beam computed tomography (CT) images and a scanned dental model is an essential prerequisite in surgical planning for dental implants or orthognathic surgery. We propose a novel method that performs fully automatic registration between a cone-beam CT image and an optically scanned model. To build a robust and automatic initial registration method, our method applies deep-pose regression neural networks in a reduced domain (i.e., 2-dimensional image). Subsequently, fine registration is performed via optimal clusters. Majority voting system achieves globally optimal transformations while each cluster attempts to optimize local transformation parameters. The coherency of clusters determines their candidacy for the optimal cluster set. The outlying regions in the iso-surface are effectively removed based on the consensus among the optimal clusters. The accuracy of registration was evaluated by the Euclidean distance of 10 landmarks on a scanned model which were annotated by the experts in the field. The experiments show that the proposed method's registration accuracy, measured in landmark distance, outperforms other existing methods by 30.77% to 70%. In addition to achieving high accuracy, our proposed method requires neither human-interactions nor priors (e.g., iso-surface extraction). The main significance of our study is twofold: 1) the employment of light-weighted neural networks which indicates the applicability of neural network in extracting pose cues that can be easily obtained and 2) the introduction of an optimal cluster-based registration method that can avoid metal artifacts during the matching procedures.

Abstract:Purpose: This study aimed to design and evaluate a novel method for the registration of 2D lateral cephalograms and 3D craniofacial cone-beam computed tomography (CBCT) images, providing patient-specific 3D structures from a 2D lateral cephalogram without additional radiation exposure. Methods: We developed a cross-modal deformable registration model based on a deep convolutional neural network. Our approach took advantage of a low-dimensional deformation field encoding and an iterative feedback scheme to infer coarse-to-fine volumetric deformations. In particular, we constructed a statistical subspace of deformation fields and parameterized the nonlinear mapping function from an image pair, consisting of the target 2D lateral cephalogram and the reference volumetric CBCT, to a latent encoding of the deformation field. Instead of the one-shot registration by the learned mapping function, a feedback scheme was introduced to progressively update the reference volumetric image and to infer coarse-to-fine deformations fields, accounting for the shape variations of anatomical structures. A total of 220 clinically obtained CBCTs were used to train and validate the proposed model, among which 120 CBCTs were used to generate a training dataset with 24k paired synthetic lateral cephalograms and CBCTs. The proposed approach was evaluated on the deformable 2D-3D registration of clinically obtained lateral cephalograms and CBCTs from growing and adult orthodontic patients. Results: Strong structural consistencies were observed between the deformed CBCT and the target lateral cephalogram in all criteria. The proposed method achieved state-of-the-art performances with the mean contour deviation of 0.41 +/- 0.12 mm on the anterior cranial base, 0.48 +/- 0.17 mm on the mandible, and 0.35 +/- 0.08 mm on the maxilla, respectively. The mean surface mesh ranged from 0.78 to 0.97 mm on various craniofacial structures, and the LREs ranged from 0.83 to 1.24 mm on the growing datasets regarding 14 landmarks. The proposed iterative feedback scheme handled the structural details and improved the registration. The resultant deformed volumetric image was consistent with the target lateral cephalogram in both 2D projective planes and 3D volumetric space regarding the multicategory craniofacial structures. Conclusions: The results suggest that the deep learning-based 2D-3D registration model enables the deformable alignment of 2D lateral cephalograms and CBCTs and estimates patient-specific 3D craniofacial structures.

Abstract:Traditional navigational bronchoscopy procedures rely on preprocedural computed tomography (CT) and intraoperative chest radiography and cone-beam CT (CBCT) to biopsy peripheral lung lesions. This navigational approach is challenging due to the projective nature of radiography, and the high radiation dose, long imaging time, and large footprints of CBCT. Digital tomosynthesis (DTS) is considered an attractive alternative combining the advantages of radiography and CBCT. Only the depth resolution cannot match a full CBCT image due to the limited angle acquisition. To address this issue, preoperative CT is a good auxiliary in guiding bronchoscopy interventions. Nevertheless, CT-to-body divergence caused by anatomic changes and respiratory motion, hinders the effective use of CT imaging. To mitigate CT-to-body divergence, we propose a novel deformable 3D/3D CT-to-DTS registration algorithm employing a multistage, multiresolution approach and using affine and elastic B-spline transformation models with bone and lung mask images. A multiresolution strategy with a Gaussian image pyramid and a multigrid strategy within the B-spline model are applied. The normalized correlation coefficient is included in the cost function for the affine model and a multimetric weighted cost function is used for the B-spline model, with weights determined heuristically. Tested on simulated and real patient bronchoscopy data, the algorithm yields promising results. Assessed qualitatively by visual inspection and quantitatively by computing the Dice coefficient (DC) and the average symmetric surface distance (ASSD), the algorithm achieves mean DC of 0.82±0.05 and 0.74±0.05 , and mean ASSD of 0.65±0.29mm and 0.93±0.43mm for simulated and real data, respectively. This algorithm lays the groundwork for CT-aided intraoperative DTS imaging in image-guided bronchoscopy interventions with future studies focusing on automated metric weight setting.

Abstract:PurposeTrans-oral robotic surgery (TORS) using the da Vinci surgical robot is a new minimally-invasive surgery method to treat oropharyngeal tumors, but it is a challenging operation. Augmented reality (AR) based on intra-operative ultrasound (US) has the potential to enhance the visualization of the anatomy and cancerous tumors to provide additional tools for decision-making in surgery.MethodsWe propose a US-guided AR system for TORS, with the transducer placed on the neck for a transcervical view. Firstly, we perform a novel MRI-to-transcervical 3D US registration study, comprising (i) preoperative MRI to preoperative US registration, and (ii) preoperative to intraoperative US registration to account for tissue deformation due to retraction. Secondly, we develop a US-robot calibration method with an optical tracker and demonstrate its use in an AR system that displays anatomy models in the surgeon’s console in real-time.ResultsOur AR system achieves a projection error from the US to the stereo cameras of 27.14 and 26.03 pixels (image is 540×documentclass[12pt]{minimal}usepackage{amsmath}usepackage{wasysym}usepackage{amsfonts}usepackage{amssymb}usepackage{amsbsy}usepackage{mathrsfs}usepackage{upgreek}setlength{oddsidemargin}{-69pt}egin{document}$$ imes $$end{document}960) in a water bath experiment. The average target registration error (TRE) for MRI to 3D US is 8.90 mm for the 3D US transducer and 5.85 mm for freehand 3D US, and the TRE for pre-intra operative US registration is 7.90 mm.ConclusionWe demonstrate the feasibility of each component of the first complete pipeline for MRI-US-robot-patient registration for a proof-of-concept transcervical US-guided AR system for TORS. Our results show that trans-cervical 3D US is a promising technique for TORS image guidance.

Abstract:Purpose: Trans-oral surgery provides a less invasive means for the surgical management of upper aerodigestive tract malignancies but is limited in its ability to readily assess submucosal tumor extent and location of critical structures intraoperatively. We sought to determine surgeons' baseline target localization accuracy during operative laryngoscopy with preoperative imaging alone and then assess for improvement in localization accuracy when presented with intraoperative CT imaging capturing soft tissue deformation. Methods: Fiducial beads were placed submucosally in four cadaver heads. "Preoperative" (PO) and "intraoperative" (IO) neck CTs were acquired before and during suspension laryngoscopy using a CT-compatible laryngoscopy system. Surgeons attempted to localize submucosal fiducials beads using pins based on sequential review of PO and IO images. Results: Mean total error (TE) decreased from 12.8 ± 9.9 to 10 ± 7.5 mm from PO to IO (P < 0.001), respectively. TE for base of tongue and vallecula decreased by 1.7 ± 6.7 mm (P = 0.015). Right-sided structures were most exposed by scope positioning and experienced a TE reduction of 4.8 ± 9.3 mm (P < 0.001). Task completion time decreased from PO to IO by 26% (P < 0.001). Conclusions: Intraoperative imaging significantly improves localization accuracy and task efficiency when targeting submucosal beads in cadaver heads during operative laryngoscopy.
SU‐E‐J‐100: A Contour‐Guided Deformable Image Registration Algorithm for Adaptive Radiotherapy

Abstract:Purpose: To develop a contour‐guided deformable image registration algorithm to improve the accuracy of deformation image registration in adaptive radiotherapy, especially for improving the accuracy of low contrast structures deformation. Methods: We developed an efficient algorithm to accurately deform low‐contrast structures in CT images. The new DIR algorithm is based on our original developed Demons algorithm by adding constraints defined by physician drawn contour pairs on images. By iteratively increasing the intensity of ROI enclosed by contour pairs, the optimization problem defined in Demons is ensured to converge to a desirable solution. Results: The new algorithm was validated on both a numerical phantom experiment and a clinical patient case. For the digital phantom experiment, a moving image is defined as two intersected spheres embedded in a cube and the contrast between spheres and cube is low; and a target image is a low‐contrast sphere in a cubic. For the clinical case, two CT images scanned for a head and neck cancer patient in one month apart are treated as a pair of moving and target images. The left parotids with anatomical changes are defined as structures of interest (SOIs). Qualitatively, with visual inspection, the proposed algorithm yields satisfactory results in both numerical and clinical testing cases. Quantitatively, root mean squares error (RMS) and normalized correlation coefficient (NCC) were calculated between the deformed and target SOI. Compared to the original Demons algorithm, the contour‐guided Demons algorithm results the decrease of RMS and the increase of NCC for both cases. Conclusions: The proposed contour‐guided demons algorithm permits us to take advantage of the structures defined by physicians to obtain more accurate image deformations. It provides a useful clinical tool for future interactive adaptive radiotherapy.
Scale-invariant registration of monocular endoscopic images to CT-scans for sinus surgery

Abstract:In this paper, we present a novel method for intra-operative registration directly from monocular endoscopic images. This technique has the potential to provide a more accurate surface registration at the surgical site than existing methods. It can operate autonomously from as few as two images and can be particularly useful in revision cases where surgical landmarks may be absent. A by-product of video registration is an estimate of the local surface structure of the anatomy, thus providing the opportunity to dynamically update anatomical models as the surgery progresses. Our approach is based on a previously presented method [Burschka, D., Hager, G.D., 2004. V-GPS (SLAM):--Vision-based inertial system for mobile robots. In: Proceedings of ICRA, 409-415] for reconstruction of a scaled 3D model of the environment from unknown camera motion. We use this scaled reconstruction as input to a PCA-based algorithm that registers the reconstructed data to the CT data and recovers the scale and pose parameters of the camera in the coordinate frame of the CT scan. The result is used in an ICP registration step to refine the registration estimates. The details of our approach and the experimental results with a phantom of a human skull and a head of a pig cadaver are presented in this paper.

Abstract:In order to use pre-operative images during an intervention for navigation, they must be registered to the patient's co-ordinate system in the operating theatre or to an intra-operative image. For the registration to be valid in the case of patient movements, the registration must be updated or the patient movement must be tracked. One problem in this area is the registration of intra-operatively acquired X-ray fluoroscopies with 3D CT images obtained before the intervention as well as motion tracking for this setup. The result can be used to support the placement of pedicle screws in spine surgery or aortic endoprostheses in transfemoral endovascular aneurysm management (TEAM). The different approaches to 2D/3D registration are discussed and a novel voxel-based method is presented: using a small part of the CT image covering only the vertebra of interest, pseudo-projections are computed and the resulting vertebra template is compared to the X-ray projection using a new similarity measure which is called pattern intensity. Application, performance and registration accuracy are discussed and demonstrated by application to images of a TEAM procedure and of a spine phantom.

Abstract:Throat tumour margin control remains difficult due to the tight, enclosed space of the oral and throat regions and the tissue deformation resulting from placement of retractors and scopes during surgery. Intraoperative imaging can help with better localization but is hindered by non-image-compatible surgical instruments, cost, and unavailability. We propose a novel method of using instrument tracking and FEM-multibody modelling to simulate soft tissue deformation in the intraoperative setting, without requiring intraoperative imaging, to improve surgical guidance accuracy. We report our first empirical study, based on four trials of a cadaveric head specimen with full neck anatomy, yields a mean TLE of 10.8 ± 5.5 mm, demonstrating methodological feasibility.

Abstract:Computer-assisted surgery (CAS) can improve surgical precision in orthopaedic oncology. Accurate alignment of the patient's imaging coordinates with the anatomy, known as registration, is one of the most challenging aspects of CAS and can be associated with substantial error. Using intraoperative, on-the-table, cone-beam computed tomography (CBCT), we performed a pilot clinical study to validate a method for automatic intraoperative registration.

Abstract:PURPOSE: The accuracy of minimally invasive, intracranial neurosurgery can be challenged by deformation of brain tissue - e.g., up to 10 mm due to egress of cerebrospinal fluid during neuroendoscopic approach. We report an unsupervised, deep learning-based registration framework to resolve such deformations between preoperative MR and intraoperative CT with fast runtime for neurosurgical guidance.METHOD: The framework incorporates subnetworks for MR and CT image synthesis with a dual-channel registration subnetwork (with synthesis uncertainty providing spatially varying weights on the dual-channel loss) to estimate a diffeomorphic deformation field from both the MR and CT channels. An end-to-end training is proposed that jointly optimizes both the synthesis and registration subnetworks. The proposed framework was investigated using three datasets: (1) paired MR/CT with simulated deformations; (2) paired MR/CT with real deformations; and (3) a neurosurgery dataset with real deformation. Two state-of-the-art methods (Symmetric Normalization and VoxelMorph) were implemented as a basis of comparison, and variations in the proposed dual-channel network were investigated, including single-channel registration, fusion without uncertainty weighting, and conventional sequential training of the synthesis and registration subnetworks.RESULTS: The proposed method achieved: (1) Dice coefficient = 0.82±0.07 and TRE = 1.2 ± 0.6 mm on paired MR/CT with simulated deformations; (2) Dice coefficient = 0.83 ± 0.07 and TRE = 1.4 ± 0.7 mm on paired MR/CT with real deformations; and (3) Dice = 0.79 ± 0.13 and TRE = 1.6 ± 1.0 mm on the neurosurgery dataset with real deformations. The dual-channel registration with uncertainty weighting demonstrated superior performance (e.g., TRE = 1.2 ± 0.6 mm) compared to single-channel registration (TRE = 1.6 ± 1.0 mm, p < 0.05 for CT channel and TRE = 1.3 ± 0.7 mm for MR channel) and dual-channel registration without uncertainty weighting (TRE = 1.4 ± 0.8 mm, p < 0.05). End-to-end training of the synthesis and registration subnetworks also improved performance compared to the conventional sequential training strategy (TRE = 1.3 ± 0.6 mm). Registration runtime with the proposed network was ∼3 s.CONCLUSION: The deformable registration framework based on dual-channel MR/CT registration with spatially varying weights and end-to-end training achieved geometric accuracy and runtime that was superior to state-of-the-art baseline methods and various ablations of the proposed network. The accuracy and runtime of the method may be compatible with the requirements of high-precision neurosurgery.

Abstract:Many recent reports have indicated interesting structure in the low-dose region of survival curves for mammalian cells (e.g., variations in the effects of oxygen, high-order modulation of the dose-response function, induction of repair, activation of protein-modifying enzymes, influences of cytokines, etc.). Since we have recently identified and corrected several problems causing variability in the results of the alkaline elution methodology, we felt that it was important to investigate the formation and repair of single-strand breaks (SSBs) in this low-dose region. To date we have asked three questions relevant to the structure features noted above: (1) Is the dose response linear at very low radiation doses? (2) Is repair of SSBs complete? (3) Do low "priming" doses of radiation induce subsequent variations in the sensitivity, or rate or extent of repair? To date, we have found no basis in SSB induction and repair for the interesting substructure noted in the survival responses above. At doses from 0.25 Gy to 4 Gy in air, no significant deviations from a linear dose response were observed, and a "priming" dose of 2 Gy has a minimal effect on subsequent sensitivity of SSB formation. Repair of SSBs appears complete at clinical doses of 2 Gy, but substantial unrepaired SSB damage remains 1 h after doses (11 Gy) corresponding to a surviving fraction of 0.01. No significant change in repair of SSBs at 4 h after a 2-Gy priming dose has yet been determined.
