Abstract:Young adult female Dutch Belted rabbits were given a single intravenous injection of polymeric 23gPu-nitrate ( 15% ultrafilterable) or of monomeric 239Pu-citrate (90% ultrafilterable) and killed at 3 days. The plutonium content of the intact femur (bone plus marrow) was higher after the polymeric form. Physical removal of marrow from bone in the tibia showed the plutonium content of the bone to be slightly lower and of the marrow to be 17-fold higher after polymeric than after monomeric plutonium. Polymeric plutonium was also more concentrated in spleen, ovaries, intestines (with contents), skeletal muscle and urine but less concentrated in liver. About the same levels of the two forms of plutonium were found in blood, bile and lung. For the first 30 min after injection, the polymeric plutonium was cleared from the circulation more rapidly than the monomeric. After about 60 min, the two forms were cleared at generally comparable, decreasing rates. Polymeric plutonium was as much as 10-30 times more concentrated in predominantly red (erythropoietic) marrow than in fatty marrow. Monomeric plutonium was 1.3-2.8 times more concentrated in the red marrow. Mice injected with the same plutonium solutions as the rabbits showed, in comparison, a higher per cent of the injected monomeric plutonium in the femurs and a lower per cent in the liver; they showed a higher uptake of polymeric plutonium in the marrow, and a higher uptake of both forms in the spleen and lungs than did the rabbits. The marrow contained 0.27 %/g at 3 days, Because of the differences in metabolism of different physical-chemical forms of plutonium in rodents, and the conflicting results reported for rabbits, the tissue uptake 3 days after intravenous injection of two different, carefully characterized forms of plutonium has been determined in the rabbit. METHODS Polymeric plutonium was prepared by selectively removing the near-colloidal supernate about 2 hr after precipitation of an acidic solution of Pu(NO,), by careful dropwise addition of NaOH to pH -7.(17) The larger particulate matter in the supernate was removed by filtration through a Millipore filter of 5 pm nominal pore diameter. The plutonium in this preparation was 15 % ultrafilterable through Visking (cellophane) casing and may be described as “mid-range polyrneric”.(l7) The monomeric plutonium was prepared from the same stock, adjusted to pH -5 in the presence of 1 % sodium citrate. I t was 90 % ultrafilterable after filtration through a 25 nm Millipore filter. Eight young adult (7-8-month old) Dutch Belted female rabbits, weighing an average of 2.9 kg, each reccived a single 0.5-ml injection of either polymeric plutonium (-0.303 pCi/kg) or monomeric plutonium (-0.356 pCi/kg) into an ear vein. A magnetic stirrer was used to keep the polymeric plutonium uniformly suspended in the injection solution. The amounts of plutonium injected were determined from 20-pl. aliquots of the injection solutions taken before and after the injections. A 1-ml blood sample was taken from the ear vein of each rabbit in a heparinized syringe a t 5 , 30 min, 1, 6 hr, 1, and 3 days for plutonium analysis: two 10-ml samples of cardiac blood were also obtained at 3 days, immediately before sacrifice. The animals were caged individually in ordinary stainless steel rabbit cages adapted with a plastic screen to allow separate collection of urine and feces. Wayne rabbit ration pellets and water were fed ad lib. The rabbits were killed at 3 days by sodium pentothal injected intracardiall y. At autopsy, a small sample of liver and spleen from each rabbit was fixed in 10% neutral formalin and autoradiograms were made as described earlier.(ls) All tissues removed for radiochemical analysis were weighed immediately. The bilewas removed from the gall bladder and measured with a I-ml syringe. The urine in the bladder was added to that excreted. The intestinal tract with contents, from pylorus through rectum, was removed and analyzed intact. Larger tissues and excreta were placed in beakers and combusted by a combination of wet ashirig with concentrated FINO, and muffling at 450”. (Gradual attainment of this temperature avoids foaming.) The inorganic residues remaining after ashing were dissolved in a known volume of 0.1 N HCI, from which aliquots were removed for liquid scintillation assay of plutonium.(l9) The smaller tissues, including lungs and measured samples of blood, muscle and marrow, were placed directly into scintillation vials for wet combustion and plutonium assay. Right and left femurs were ashed and analyzed separately. The tibias were separated into several marrow and bone fractions by making cross cuts 1 cm from each end. After weighing the resulting segments, samples of marrow were quickly scooped out and weighed. The rest of the marrow was then removed with small cotton balls and the weight obtained by difference. (The hollow tibia of the adult rabbit permits almost complete, quantitative marrow removal with this technique. A variable but small amount of marrow remains in the deep trabecular recesses of the proximal bone segment and in the small proximal epiphysis.) Loss of marrow and bone resulting from the two cuts averaged 3.26% by weight. With the exception of this loss, the total plutonium content in bone and marrow was measured. The small fibula, which was essentially devoid of marrow, and weighed about 3.4% of the intact tibia plus fibula, was included with the bone fractions of the mid-shaft and proximal segments. To relate this study to data already obtained in mice, and as an additional characterization of the plutonium solutions given the rabbits, we injected the same solutions intravenously into groups of four 85-day old, 28-g, CF#1 female mice. The mice received an average of 5.1 pCi/ kg of polymeric and 3.2,uCilkg of monomeric plutonium, as determined by analysis of replicate injection volumes. They were sacrificed at 3 days and at 90 days (monomeric form only) for radiochemical analyses. Tissue analyses were performed as in the rabbits, except that M. W. ROSENTHAL et al. 233 certain tissues were pooled (as indicated in Table 3). The separated marrow and bone fractions of a standard 0.9-cm mid-shaft segment were analyzed from the mouse tibias.(20)
Effect of change in vaccine schedule on pertussis epidemiology in France: a modelling and serological study

Abstract:Background: In April-May, 2013, France modified its pertussis vaccination schedule, which uses the acellular pertussis vaccine, from three primary doses at 2, 3, and 4 months of age and a first booster at 16-18 months of age (former schedule) to two primary doses at 2 and 4 months of age and a first booster at 11 months of age (new schedule). We aimed to assess the subsequent effect of the vaccine schedule change on pertussis epidemiology in France. Methods: In this modelling study, using data collected between Jan 1, 2012, and Dec 31, 2019, from French national surveillance sources, we analysed the PCR test results of nasopharyngeal swabs collected from symptomatic outpatients aged 2-20 years with suspected pertussis. We developed a negative binomial regression model for the number of confirmed pertussis cases by year and age to assess the relative risks of pertussis depending on vaccine schedule. The linear predictor included the year, the age group, the population size, and a proxy of waning immunity. We tested different models in which waning immunity could vary with vaccine schedule and type of primary vaccine. The models were fitted to the 2012-18 data via Bayesian Markov chain Monte Carlo sampling, and the 2019 data were left out for external model validation. We also compared the anti-pertussis toxin (PT) antibody concentrations in leftover sera from children not tested for pertussis or recent respiratory tract infection aged 2-5 years born before and after the vaccine schedule change. Findings: We collected data on 7493 confirmed cases of pertussis. The model that best fitted the 2012-18 epidemiological data supported a faster waning of immunity following vaccination with the new vaccine schedule. 3 years after vaccination, the risk of developing pertussis was 1·7 (95% CI 1·4-2·0) times higher for children vaccinated according to the new schedule than those vaccinated according to the former schedule. The model correctly predicted the age distribution of cases in 2019. Geometric mean concentrations (GMC) of anti-PT IgG were 50% lower in children aged 2 years vaccinated with the new schedule (GMC=5·85 IU/mL [95% CI 4·08-8·39]) than in children of the same age vaccinated with the former schedule (GMC=11·62 IU/mL [95% CI 9·05-14·92]; p=0·0016), and 43% lower in children aged 3 years vaccinated with the new schedule (GMC=3·88 IU/mL [95% CI 2·82-5·34]) than those with the former schedule (GMC=6·80 IU/mL [95% CI 4·77-9·70]; p=0·026). Interpretation: A shorter-lived protection induced by the new vaccine schedule recommended in France since 2013 is associated with an increase of pertussis cases in children aged 2-5 years. If similar findings are observed in other countries and clinical trials, these findings should be considered in future pertussis vaccination policies. Abstract:Background: It has been proven that vaccination is effective against Human Papillomavirus (HPV) infections, genital warts, and pre-cancerous and cancerous cervical lesions. Nevertheless, the HPV vaccine coverage of 37.4 % in 2021 in France is one of the lowest in Europe. To explore and understand the reason why French population is so late compared to its neighbours, we carried out focus groups with mothers and with National Education school staff. We aimed to identify knowledge and perceptions of HPV in both study populations, as well as factors influencing HPV vaccination. Methods: Between January 2020 and March 2021, we performed a qualitative study using an inductive approach with a thematic content analysis (TCA). We conducted semi-structured focus groups with 29 people including 15 mothers of adolescents in middle schools and 14 school staff from the national education system. Results: Different factors influenced the decision-making process of parents and school staff: knowledge and perceptions of HPV and its vaccine, sources of information about HPV and vaccination. Mothers' discourses differed from those of school staff. They mentioned the importance of gynaecological monitoring and the negative image of pharmaceutical companies, and questioned internet as a reliable source of information. For their part, school staff mentioned cultural and/or religious affiliation, municipalities' role to inform the population, and ethical dilemma or logistical challenges regarding HPV vaccination in schools. Conclusion: The results of these focus groups provided information on which elements may harm or help HPV vaccination. Identified perceptions, beliefs, knowledge, barriers, and facilitators will help us to build an intervention program focus on general practitioners (GP), school staff, parents, and adolescents.
Pediatric Pneumococcal Conjugate Vaccine and Dosing Schedule Perceptions Among Health Care Providers and Caregivers in Germany, France, Spain, and Greece

Abstract:Introduction: Most European infant national immunization programs (NIPs) recommend pneumococcal conjugate vaccines (PCVs), which currently cover 10-15 serotypes administered in a three-dose schedule (two primary plus one booster). Recently, a PCV covering 20 serotypes that is administered in a four-dose schedule (three primary plus one booster) was licensed. Methods: An online survey was administered to collect data from health care providers (HCPs) and caregivers of children aged 0-5 (including expectant mothers) in four European countries (Germany, France, Spain, and Greece). All caregiver respondents had a shared or full responsibility to make health decisions for their child. Data on opinions, perceptions, and openness to a change in childhood vaccination dosing schedules were collected, along with demographic information for HCPs as well as caregivers. Results: A total of 601 HCPs and 1954 caregivers were recruited across the four countries. Nearly all HCPs (93%) agreed that broader serotype coverage against pneumococcal disease for children is a significant unmet need, and 92% had a "sense of urgency" to vaccinate children. Both HCPs and caregivers were supportive of an additional PCV dose and doctor visit, assuming it provided at least 20% more serotype coverage than what is currently available. Caregivers strongly agreed on the importance of full vaccination for pneumococcal disease, even if an extra dose and visit to the doctor was required. Conclusions: HCPs and caregivers were virtually unanimous in their support for a PCV with broader serotype coverage and showed a subsequent willingness to include an extra infant dose/visit. These results can help guide broader discussions regarding public health policy and vaccine administration in the context of important efforts to reduce the global disease burden associated with pneumococcal disease.
[Consequences of opposition to vaccination in France and Europe. How to maintain effective vaccine coverage in 2010?]

Abstract:Refusal of vaccination can result in inadequate vaccine coverage. The collective benefit of immunisation depends on a sufficient and sustained level of vaccine coverage. Low vaccine coverage can lead to the persistence of preventable diseases and, in some cases, to a dangerous shift in the age of pathogen encounter towards adulthood. This is the case of measles in Europe, where some countries, including France, have not reached the effective vaccine coverage rate of 95%. Outbreaks are occurring, leading to complications (encephalitis and pneumonia) in adolescents and adults, necessitating hospitalization in nearly one-third of cases. The French population is also under-vaccinated against hepatitis B, due to fears of a risk of demyelinating disorders: the coverage rate is currently only about 30% in infants and 10% in adolescents. These difficulties are due to negligence and to vaccine refusal by parents. Refusal of immunisation has a long history in Europe, and explains for example why pertussis remained endemic in many countries until 1995, and also the resurgence of diphtheria in the Russian federation during the 1990s. Sections of Western society are now questioning the need for some routine vaccines, overlooking the fact that they have eradicated some diseases (polio, diphtheria, etc.) and protect effectively against lesser-known pathogens such as hepatitis B virus and HPV. In France, it will be necessary to restructure healthcare professional training programs in vaccinology and to provide the public with more thorough information on the risk-benefit ratio of vaccination. The recent controversy surrounding pandemic H1N1 influenza vaccination demonstrates that the public and the media tend to focus more on the potential risks of vaccination than on its benefits. A vigorous ethical and political debate is needed to shape an effective and acceptable vaccine policy for the 21st century.

Abstract:Introduction: A high level of vaccine hesitancy is observed in France; to maintain a high level of vaccine coverage, a policy of mandatory vaccines has been applied since January 2019. As vaccine hesitancy has been described among healthcare workers, we aimed to assess the adhesion to vaccination in students enrolled in the first common year of healthcare studies (PACES), and their perceptions regarding the mandatory vaccine policy. Methods: We elaborated an anonymous online questionnaire and asked the 35 French universities offering PACES studies to diffuse it to their students; 26 accepted. Results: Overall, 4326 participants completed the questionnaire (age 18.7 ± 1.5 years, female 76.1%). They evaluated vaccine usefulness and harmfulness at a median of 90 [IQT:80-100] and 25 [IQT:10-50], respectively; on the same scale, their median trust in vaccines was 82 [IQT:70-94]. Among the five main age classes (17 to 21 years), the positive perceptions of vaccination declined with age. A majority declared that making vaccines mandatory was morally and scientifically justified (65.7% and 84.7%, respectively); in response to the questionnaire showing a blatant anti-mandatory vaccine billboard, 52.3% and 28.6%, respectively, strongly disagreed or rather disagreed with it. Again, these proportions declined between 17 and 21 years. Participants were asked whether the mandatory vaccine policy would improve their perceptions of vaccination; those with poorer perceptions were less likely to answer this question positively. Conclusion: The perceptions of vaccination and the recent mandatory vaccine policy are positive in first-year students but better in younger ones, thus suggesting that vaccination should be taught early, during this study year. The mandatory vaccine policy may not improve the perceptions of those with a high level of vaccine hesitancy.

Abstract:Objective: The vaccination of hospital staff decreases patient mortality and disorganization of services due to sick leave. The main aim of our study was to determine the Clermont-Ferrand University hospital (CHU) personnel's motivations for or against antiflu vaccination to increase the effectiveness of prevention campaigns. Design: An autoquestionnaire with multiple choices was given to the 7601 CHU staff in May 2005. It documented socioprofessional characteristics, vaccinal status, and reasons for vaccination, nonvaccination, or stopping antiflu vaccination. Results: The answer rate was 26.5% (2011 autoquestionnaires returned) and representative of the CHU staff. The rate of vaccination in 2004 was 36.35% with a strong correlation between vaccination and former influenza infection (p<0.001). Five of the six principal reasons for vaccination were altruistic including the first two: avoiding transmission to patients (61.8%) and his family (59.8%). The main reason for stopping was the lack of time. The reasons for nonvaccination were linked to a feeling of invulnerability: conviction of not being at risk, of being too young, or in good health. Conclusions: The Haute Autorité de santé's objective of vaccinal coverage against influenza for 75% of the health professionals requires their active compliance. The effectiveness of future campaigns could aim at improving knowledge by insisting particularly on the young age of the risk populations. The lack of time can be compensated by offering on-site vaccination, including at night and by proposing larger schedules for vaccination.
Pediatricians' self-reported clinical practices and adherence to national immunization guidelines after the introduction of pneumococcal conjugate vaccine

Abstract:Background: Little is known about whether pneumococcal conjugate vaccine (PCV) has altered pediatricians' practices regarding well-child and acute care. Objectives: To (1) describe whether PCV caused pediatricians to move other routine infant vaccines and/or add routine visits; (2) characterize adherence to national immunization recommendations; and (3) determine whether PCV altered pediatricians' planned clinical approach to well-appearing febrile infants. Design and methods: One year after PCV was added to the pediatric immunization schedule, we mailed a 23-item survey to 691 randomly selected pediatricians in Massachusetts. The adjusted response rate was 77%. Results: After PCV introduction, 39% of pediatricians moved other routine infant vaccines to different visits and 15% added routine visits to the infant schedule. The self-reported immunization schedules of 36% were nonadherent to national immunization guidelines for at least 1 vaccine. Nonadherence rates were significantly higher among pediatricians who had been in practice longer, moved another vaccine because of PCV introduction, and/or offered to give shots later when multiple injections were due. For a hypothetical febrile 8-month-old girl who had received 3 doses of PCV, pediatricians reported they were significantly less likely to (1) perform both blood and urine testing and (2) prescribe antibiotics than in the pre-PCV era. Conclusions: The introduction of PCV may have had unintended effects on pediatric primary care, including decreased adherence to national recommendations for the timing of immunizations and decreased urine testing for well-appearing febrile infants. Special efforts may be warranted to ensure that pediatricians remain current with changing recommendations.

Abstract:The evolution of the epidemiology of pertussis, new licensed macrolides and vaccines, new recommendations for vaccination among adolescents and adults need an update of the French guidelines for prevention of the disease around one or grouped cases of the disease. A particular attention should be raised to the diagnosis of whooping cough in adults who are presently the main reservoir of Bordetella pertussis. Whooping cough in adults presents as an unexplained prolonged cough with nocturnal exacerbation witch accounts for most of the contaminations of young infants. A bacteriological confirmation of pertussis should be provided before implementation of preventive measures: culture and PCR are presently the gold standard for the diagnosis of pertussis in infants, children and even adults who have been coughing for less than 20 days. Later on, serology (Elisa, immuno-empreinte) is the only technique available, but cannot be interpreted if the patient has been vaccinated less than one year ago. Infants under three months should be admitted to hospital and every case submitted to respiratory isolation. Eviction from the community should be pronounced within the five first days following the onset of an effective antibiotic treatment. New macrolides should be favoured: clarithromycin for seven days or azithromycin for five days. Household contacts should be given the same prophylactic antibiotic treatment: children and adolescent not correctly immunized, parents of the index case as adults parents of not or not completely immunized infants. The vaccination program of the household should be updated. The same measures should be applied in case of grouped cases (at least two contemporary or consecutive cases in the same area). In that case, the Public Health System doctors should be involved in the investigation and the classification of the cases. The close contacts not or not completely immunized should be prescribed and antibiotic prophylaxis and an update of their vaccination program. Among the occasional contacts, high-risk people only should be treated. In the day care centres an antibiotic prophylaxis should be given to children who have received less than four vaccine shots against pertussis and to the personnel contact to the cases as well. In the schools, the antibiotic prophylaxis should be prescribed to all children of the classroom(s) not completely vaccinated and to the teacher(s) as well. In the boarding schools and institutions with handicapped children, antibiotic prophylaxis could be applied to every member if the pertussis vaccine coverage is at less than 50%. In hospitals, coughing personnel should wear masks and investigations towards pertussis should be performed in people with a more than seven day unexplained cough. Preventive measures should be applied in case of confirmation of pertussis.

Abstract:Growth curves of the yeast form of Paracoccidioides brasiliensis B-339 based on total and viable cell counts were determined. Crude culture filtrate antigens were obtained after 7, 10, 15, 20, 25, and 30 days of incubation. Different patterns of proteins were obtained by affinity chromatography on Sepharose 4B-immunoglobulin G complex made with immunoglobulin G from patients with paracoccidioidomycosis, with subsequent analyses by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and scanning densitometry. Three major proteins were excreted during the time course of a 30-day culture: a doublet at 20 to 21 kilodaltons (kDa) and molecules of 43 and 52 kDa. The 43-kDa antigen was present throughout the growth period, and its level reached a peak on days 15 to 20 and then decreased considerably toward day 30. The antigenic preparations collected on days 7, 10, 15, and 20 gave better reactions in immunodiffusion tests than those collected on days 25 and 30. The 7-day exoantigen gave a sensitivity of 97.1% and specificity of 100% on immunodiffusion. The main line of precipitation had a very high intensity, showing a total identity with that of a previously purified glycoprotein of 43 kDa. A 7-day crude exoantigen displayed a high level of sensitivity and specificity, being reproducible from batch to batch and retaining its activity for years when kept lyophilized. A protocol is recommended for the production of a stable diagnostic antigen to be used in immunodiffusion tests for paracoccidioidomycosis.
Parental acceptability of vaccinating young children against influenza and COVID-19

Abstract:Introduction: The struggle against influenza, and more recently against Coronavirus Disease 2019 (COVID-19), relies mostly on protective measures and vaccines. In France, COVID-19 vaccination campaigns include children over 5 years of age, whereas the influenza vaccine is reserved for children and infants older than 6 months with severe chronic disease. The final decision to vaccinate ultimately rests with the parents. This study aimed to explore the opinions of parents regarding COVID-19 and influenza vaccinations, and which factors are likely to influence their decision in order to improve prevention approaches, and thus obtain better vaccination coverage among children. Methods and survey: In this trans-sectional multicenter survey, self-administered questionnaires were sent to parents of children attending 14 childcare institutions over a period of 2 months, December 2021-February 2022. The questionnaire consisted of three sections: the first collected general, social and professional data, the second addressed perceived parental acceptability regarding influenza and COVID-19 vaccinations in healthy children, while the last section explored the main sources of information consulted by the parents regarding vaccinations. Subject's characteristics were described using number and percentages, as they are all categorical variables. Description of answers regarding both influenza and COVID-19 sources of information data were performed. Characteristics of patients answers were described and compared according to intention of vaccine groups. Results: Among the 343 questionnaires collected, 41.7 % of respondents stated they were supportive of vaccination of their children against influenza and 37 % against COVID-19. Media (Internet, TV and radio) were the most cited sources of information. Incentive factors for both vaccination against influenza and COVID-19 were individual, and collective protection. Limiting factors were fear of side effect and Age of the child. Media was the sole source of information, concerning vaccination against influenza for 59 % non-graduates and 28 % of participants having a higher education level, and respectively 49 % and 19 % for vaccination against COVID-19. The acceptability rate was influenced by the level of education and the sources of information. Discussion and conclusion: About 40 % of the parents were supportive of children's vaccinations against COVID-19 and influenza respectively (37 % and 42 %) in order to protect the vulnerable people around them. The media influences the decision to vaccinate especially when the level of education is low.

Abstract:INTRODUCTION Cigar, cigarillo, and little cigar (CCLC) use is prevalent among adolescents, particularly among those who smoke cigarettes. METHODS Using data from a longitudinal study of smoking patterns among adolescents, we examined differences between CCLC users (ever and past 30 days) and nonusers (never and not in the past 30 days) among adolescents who smoked a cigarette in the last month (n = 486). RESULTS In our sample, 76.7% reported ever trying CCLC and 40.7% reported past month CCLC use. Bivariate analyses showed that CCLC users differed from nonusers in terms of demographics, other forms of tobacco use, other substance use, and mental health. Multivariate logistic regression analyses found that both ever and past 30-day CCLC use were strongly associated with being male and concurrent use of hookah. Ever CCLC use was also strongly associated with recent use of alcohol, and past 30-day CCLC use was strongly associated with antisocial behavior. After controlling for the number of days on which cigarettes were smoked in the past 30 days, past 30-day CCLC use was associated with most other forms of tobacco use, other substance use, and mental health, but not with number of cigarettes smoked in the past month and nicotine dependence. CONCLUSIONS Results suggest that CCLC use is high among adolescent cigarette users and is associated with a variety of negative correlates. Importantly, many of these relationships are not accounted for by the adolescent's level of cigarette use. Further characterizing CCLC use will be important for developing more targeted and tailored interventions.

Abstract:The pinhole collimator is currently the collimator of choice in small animal single photon emission computed tomography (SPECT) imaging because it can provide high spatial resolution and reasonable sensitivity when the animal is placed very close to the pinhole. It is well known that if the collimator rotates around the object (e.g., a small animal) in a circular orbit to form a cone-beam imaging geometry with a planar trajectory, the acquired data are not sufficient for an exact artifact-free image reconstruction. In this paper a novel skew-slit collimator is mounted instead of the pinhole collimator in order to significantly reduce the image artifacts caused by the geometry. The skew-slit imaging geometry is a more generalized version of the pinhole imaging geometry. The multiple pinhole geometry can also be extended to the multiple-skew-slit geometry. An analytical algorithm for image reconstruction based on the tilted fan-beam inversion is developed with nonuniform attenuation compensation. Numerical simulation shows that the axial artifacts are evidently suppressed in the skew-slit images compared to the pinhole images and the attenuation correction is effective.
Practices of French General Practitioners Regarding Vaccination of Boys Against Human Papillomavirus (HPV), One Year After the Application of Its Official Recommendation

Abstract:In 2019, French health authorities extended the recommendation for human papillomavirus (HPV) vaccination to include boys aged 11 to 19 years. We describe HPV vaccination practices among French general practitioners (GPs) since this recommendation wasapplied. We also identified factors associated with the propensity to propose HPV vaccination to boys. Cross-sectional study, between May and August 2022, among French GPs using a questionnaire asking about the GPs, their practices, and opinions regarding HPV vaccination, including whether they systematically proposed HPV vaccination to eligible boys or not. We investigated factors associated with systematic proposal of HPV vaccination, using univariate and multivariate logistic regression. In total, 360 GPs participated (76.6% females; mean age 34.7 ± 7.8 years; 22.9% had additional training in gynecology or pediatrics); 5.5% reported that they systematically offered HPV vaccination to boys prior to the recommendation, whereas 61.2% do so systematically since the recommendation. Factors associated with systematic proposal to boys (post recommendation) were female GP sex (78.6% versus 66.2%; OR = 2.0 [95% confidence interval (CI) 1.2–3.3]; p = 0.007) and systematic proposal prior to the recommendation (8.5% versus 0.7%; OR = 13.3 [1.7–101.7]; p = 0.01). Protection against HPV-induced cancer was cited as an argument to vaccinate girls (98.3% versus 89.2%; p < 0.0001); while reducing the risk of transmission was more commonly an argument to vaccinate boys (78.1% versus 51.8%; p < 0.0001). This study underlines the positive impact of the official recommendation for HPV vaccination of boys on the attitude of GPs, with an increase in the systematic proposal of HPV vaccination to boys.

Chickenpox: An update

Abstract:Despite its benign characteristics, chickenpox is a childhood disease responsible for complications and deaths, particularly in the high-risk population. VariZIG®, not commercialized in France, is a good alternative for seronegative individuals exposed to the virus and not eligible for vaccination. The efficacy of routine vaccination has been demonstrated with a decrease in chickenpox incidence and with the development of herd immunity. Over time, the protective antibody titer of vaccinated people decreases and can be maintained by two doses of the vaccine. A tetravalent measles-mumps-rubella-chickenpox vaccine, used in the United States, has a good tolerability in spite of the occurrence of fever and febrile seizures. Routine vaccination would contribute to make savings in France, by reducing direct and indirect costs of chickenpox.

Abstract:The COVID-19 pandemic has highlighted the challenges of vaccination and the infectious risks among the elderlies. However, immunization rates for recommended vaccines in this population are insufficient in France. We aimed in this study to identify the levers and obstacles to vaccination among seniors, and to establish the arguments that could lead to some new vaccination behaviors. A qualitative survey based on 14 semi-structured interviews was conducted with people aged 65 and over, living at home in the Auvergne Rhône-Alpes region (France) from March to May 2022. The interviews were recorded, entirely transcribed, then subjected to a thematic content analysis. The results show that participants are keen to preserve their health by maintaining their physical capacity, taking regular exercise and eating a balanced diet. However, vaccination was not mentioned as a mean of preventing infectious diseases. Infections were not perceived as a cause for concern. Furthermore, the physical and cognitive consequences of infectious diseases, which could result in a loss of autonomy, were not known. These elements could be a good lever to bring hesitant elderly people to reconsider their position toward vaccination. Vaccines were not seen as a strategy to prevent loss of autonomy among elderlies. Since vaccines against influenza, COVID-19, shingles, pneumococcus have proved to be effective in protecting against cardiovascular events, this argument might be the starting point for a prevention campaign for the elderly.
