Abstract:Articular cartilage tissue exhibits a spatial dependence in material properties that govern mechanical behaviour. A mathematical model of cartilage tissue under one dimensional confined compression testing is developed for normal tissue that takes account of these variations in material properties. Modifications to the model representative of a selection of mechanisms driving osteoarthritic cartilage are proposed, allowing application of the model to both physiological and pathophysiological, osteoarthritic tissue. Incorporating spatial variations into the model requires the specification of more parameters than are required in the absence of these variations. A global sensitivity analysis of these parameters is implemented to identify the dominant mechanisms of mechanical response, in normal and osteoarthritic cartilage tissue, to both static and dynamic loading. The most sensitive parameters differ between dynamic and static mechanics of the cartilage, and also differ between physiological and osteoarthritic pathophysiological cartilage. As a consequence changes in cartilage mechanics in response to alterations in cartilage structure are predicted to be contingent on the nature of loading and the health, or otherwise, of the cartilage. In particular the mechanical response of cartilage, especially deformation, is predicted to be much more sensitive to cartilage stiffness in the superficial zone given the onset of osteoarthritic changes to material properties, such as superficial zone increases in permeability and reductions in fixed charge. In turn this indicates that any degenerative changes in the stiffness associated with the superficial cartilage collagen mesh are amplified if other elements of osteoarthritic disease are present, which provides a suggested mechanism-based explanation for observations that the range of mechanical parameters representative of normal and osteoarthritic tissue can overlap substantially.

Abstract:Abstract Articular cartilage in knee joint can be anatomically divided into different regions: medial and lateral condyles of femur; patellar groove of femur; medial and lateral plateaus of tibia covered or uncovered by meniscus. The stress–strain curves of cartilage in uniaxially unconfined compression demonstrate strain rate dependency and exhibit distinct topographical variation among these seven regions. The femoral cartilage is stiffer than the tibial cartilage, and the cartilage in femoral groove is stiffest in the knee joint. Compared with the uncovered area, the area covered with meniscus shows the stiffer properties. To investigate the origin of differences in macroscopic mechanical properties, histological analysis of cartilage in seven regions are conducted. The differences are discussed in terms of the cartilage structure, composition content and distribution. Furthermore, the commonly used constitutive models for biological tissues, namely Fung, Ogden and Gent models, are employed to fit the experimental data, and Fung and Ogden models are found to be qualified in representing the stiffening effect of strain rate.

Abstract:Proceedings of the Institution of Mechanical Engineers, Part E: Journal of Process Mechanical Engineering, Ahead of Print. Several changes occur in both the cartilage's material properties and anatomical structure as osteoarthritis progresses. Unlike most numerical studies that solely consider individual changes, our study aimed to understand the impact on cartilage mechanics by considering the combined effect of material properties and cartilage thickness varied with osteoarthritis progression. In total, 3 three-dimensional finite element models, representing the intact, early, and late osteoarthritis conditions, were developed to simulate a load-bearing area in the knee. The articular cartilage was modelled as fluid-saturated linear biphasic poroelastic to incorporate solid-fluid interaction. All models underwent prolonged creep (50 N) and relaxation (0.3 mm) analyses for 600 s. In the early stage of osteoarthritis, the tibial cartilage demonstrated an overall stiffer behaviour attributed to cartilage swelling despite decreased stiffness at the material level. On the other hand, in the late stage of osteoarthritis, the decrease in cartilage thickness led to increased knee deformation. Additionally, increased permeability resulted in accelerated fluid exudation across all osteoarthritis models, and the elevation in void ratio further intensified fluid pressure within the cartilage to a higher magnitude. Furthermore, these changes collectively influenced both the magnitude and distribution of the outcomes. A holistic understanding of the material properties altered in osteoarthritis may contribute to a better understanding of the mechanical performance of cartilage during disease progression.

Abstract:The mechanical response of articular cartilage (AC) under compression is anisotropic and depth-dependent. AC is osmotically active, and its intrinsic osmotic swelling pressure is balanced by its collagen fibril network. This mechanism requires the collagen fibers to be under a state of tensile pre-strain. A simple mathematical model is used to explain the depth-dependent strain calculations observed in articular cartilage under 1D axial compression (perpendicular to the articular surface). The collagen fibers are under pre-strain, influenced by proteoglycan concentration (fixed charged density, FCD) and collagen stiffness against swelling stress. The stiffness is introduced in our model as an anisotropic modulus that varies with fibril orientation through tissue depth. The collagen fibers are stiffer to stretching parallel to their length than perpendicular to it; when combined with depth-varying FCD, the model successfully predicts how tissue strains decrease with depth during compression. In summary, this model highlights that the mechanical properties of cartilage depend not only on proteoglycan concentration but also on the intrinsic properties of the pre-strained collagen network. These properties are essential for the proper functioning of articular cartilage.

Abstract:Abstract Knee osteoarthritis (OA), characterized by multiple joint tissue degenerations, remains a significant clinical challenge. Recent evidence suggests that crosstalk within the osteochondral unit may drive OA progression. Although structural-biomechanical properties of bone and cartilage have been studied, potential interaction within the osteochondral unit in the context of OA has yet to be investigated. We performed comprehensive structural and biomechanical quantification of the cartilage, subchondral bone plate (SBP), and subchondral trabecular bone (STB) using 101 osteochondral cores collected from tibial plateaus of 12 control human cadavers (CT, 5 male/7 female) and 19 patients undergoing total knee replacement (OA, 6 male/13 female). For each sample, we quantified SBP microstructure, plate-and-rod morphological properties of the STB using individual trabecula segmentation, and morphological and compositional properties of the articular cartilage. We also performed indentation testing on each compartment of the osteochondral unit to extract the respective structural-mechanical properties. Cartilage thickness was lower in moderate and severe OA regions, while Osteoarthritis Research Society International score was higher only in severe OA regions. GAG content did not change in any OA region. Aggregate and shear moduli were lower only in severe OA regions, while permeability was lower only in moderate OA regions. In the SBP, thickness and tissue mineral density were higher in moderate and severe OA regions. Tissue modulus of STB was lower in moderate OA regions despite a thicker and more mineralized SBP; this deterioration was not observed in severe OA regions. Regression analysis revealed strong correlations between cartilage and STB properties in CT; these correlations were also found in moderate OA regions but were not observed in severe OA regions. In summary, our findings comprehensively characterize the human OA osteochondral unit. Importantly, uncoupling cartilage and subchondral bone structural-mechanical properties may be a hallmark of OA.

Abstract:The articular cartilage of a knee joint has a variety of functions including dispersing stress and absorbing shock in the tissue and lubricating the surface region of cartilage. The metabolic activity of chondrocytes under the cyclic mechanical stimulations regenerates the morphology and function of tissues. Hence, the stress evaluation of the chondrocyte is a vital subject to assess the regeneration cycle in the normal walking condition and predict the injury occurrence in the accidents. Further, the threshold determination of stress for the chondrocytes activation is valuable for development of regenerative bioreactor of articular cartilage. In this study, in both macroscale and microscale analyses, the dynamic explicit finite element (FE) method was used for the solid phase and the smoothed particle hydrodynamics (SPH) method was used for the fluid phase. In the homogenization procedure, the representative volume element for the microscale finite element model was derived by using the multiphoton microscope measured 3D structure comprising 3 different layers: surface, middle, and deep layers. The layers had different anisotropic structural and rigidity characteristics because of the collagen fiber orientation. In both macroscale and microscale FE analyses, the visco-anisotropic hyperelastic constitutive law was used. Material properties were identified by experimentally determined stress-strain relationships of 3 layers. With respect to the macroscale and microscale SPH models for non-Newtonian viscous fluid, the previous observation results of interstitial fluid and proteoglycan were used to perform parameter identifications. Biphasic multiscale FE and SPH analyses were conducted under normal walking conditions. Therefore, the hydrostatic and shear stresses occurring in the chondrocytes caused by the compressive load and shear viscous flow were evaluated. These stresses will be used to design an ex-vivo bioreactor to regenerate the damaged articular cartilage, where chondrocytes are seeded in the culture chamber. To know the stress occurred on and in the chondrocytes is vitally important not only to understand the normal metabolic activity of the chondrocyte but also to develop a bioreactor of articular cartilage regeneration as the knee joint disease treatment.
Mechanical effects of ionic replacements in articular cartilage. Part I: The constitutive model

Abstract:A three-phase multi-species electro-chemo-mechanical model of articular cartilage is developed that accounts for the effect of two water compartments, namely intra-fibrillar water stored in between collagen fibrils and extra-fibrillar water covering proteoglycans. The collagen fibers constitute the solid phase while intra-fibrillar water and dissolved NaCl and CaCl(2) on one hand and extra-fibrillar water, ions Na(+), Ca(2+) and Cl(-) and proteoglycans on the other hand, form the two fluid phases. The complete picture that includes time-dependent mass transfers between the two fluid phases, diffusion of water and ions and electrical flow emerges from the Clausius-Duhem inequality but it is deferred to further study. The analysis is restricted to equilibrium states. The present work complements the mechanical model developed in Loret and Simões (Mech Material 36(5-6): 515-541, 2004a) where the presence of the sole NaCl was considered. In its current version, the model can handle mechanical and chemical loadings and unloadings involving the two salts, NaCl and CaCl(2). In order to reproduce experimental data, the shielding effects are made cation-dependent. Strong orientation of collagen fibers parallel to the joint surface implies anisotropic mechanical properties. Electro-chemo-mechanical couplings result in a chemistry-dependent apparent tensile Poisson's ratio, that increases to large values as the solution gets fresher. The model captures these aspects as well. The features of the model are first exposed in an infinitesimal strain context. Subsequently, large strains that typically occur in uniaxial traction under deionized water are accounted for, and a nonlinear anisotropic hyper-elastic behavior is developed. Parametric identification and simulations of actual loading processes are described in a companion paper, Loret and Simões (Biomech Model Mechanobiol, in press, DOI 10.1007/s10237-004-0063-6).
Measuring the depth-dependent fracture toughness of articular cartilage based on CTOD method

Abstract:Studying the anisotropic mechanical properties of cartilage induced by inhomogeneous structure is significant to prevent cartilage damage induced by overloading or fatigue and to develop biomaterials for tissue engineering. The depth-dependent fracture behavior of knee articular cartilage was studied by conducting the fracture tests on the clamped single-edge notched tensile (SENT) samples. The articular cartilage exhibited excellent load bearing capacity and ductility due to its unique depth-dependent matrix structure. The necking length of cartilage decreases through the depth of cartilage, while the peak tensile load first increases, then decreases in depth. The grow of crack in full-thickness cartilage starts in superficial layer at about peak load and the cartilage fails gradually once the crack begins to grow. The fibril network experiences irreversible damage with large area of collagen fibrils yielding before the fracture of full-thickness cartilage. Three fracture parameters, namely the stress intensity factor (K), the critical total work (CTW) and the crack tip opening displacement (CTOD) are calculated to study the fracture properties of cartilage, and CTODΔa = 0.2 is employed to evaluate the depth-dependent initiation fracture toughness of cartilage. The fracture toughness of cartilage decreases along depth direction and the fracture toughness of full-thickness cartilage is close to that of deep layer. In addition, the superficial layer with external layer exhibits larger ductility and larger crack-tip blunting degree than the middle and deep layer of cartilage, and plays a vital role in fracture of cartilage. In addition, the fracture toughness of cartilage increases slightly with the increase of loading rate. The study could provide mechanical reference for repairing cartilage defects with artificial cartilage produced by tissue engineering.

Complex mechanical behavior of human articular cartilage and hydrogels for cartilage repair

Abstract:The mechanical behavior of cartilage tissue plays a crucial role in physiological mechanotransduction processes of chondrocytes and pathological changes like osteoarthritis. Therefore, intensive research activities focus on the identification of implant substitute materials that mechanically mimic the cartilage extracellular matrix. This, however, requires a thorough understanding of the complex mechanical behavior of both native cartilage and potential substitute materials to treat cartilage lesions. Here, we perform complex multi-modal mechanical analyses of human articular cartilage and two surrogate materials, commercially available ChondroFillerliquid, and oxidized alginate-gelatin (ADA-GEL) hydrogels. We show that all materials exhibit nonlinearity and compression-tension asymmetry. However, while hyaline cartilage yields higher stresses in tension than in compression, ChondroFillerliquid and ADA-GEL exhibit the opposite trend. These characteristics can be attributed to the materials' underlying microstructure: Both cartilage and ChondroFillerliquid contain fibrillar components, but the latter constitutes a bi-phasic structure, where the 60% nonfibrillar hydrogel proportion dominates the mechanical response. Of all materials, ChondroFillerliquid shows the most pronounced viscous effects. The present study provides important insights into the microstructure-property relationship of cartilage substitute materials, with vital implications for mechanically-driven material design in cartilage engineering. In addition, we provide a data set to create mechanical simulation models in the future.
Application of multiphysics models to efficient design of experiments of solute transport across articular cartilage

Abstract:Transport of solutes helps to regulate normal physiology and proper function of cartilage in diarthrodial joints. Multiple studies have shown the effects of characteristic parameters such as concentration of proteoglycans and collagens and the orientation of collagen fibrils on the diffusion process. However, not much quantitative information and accurate models are available to help understand how the characteristics of the fluid surrounding articular cartilage influence the diffusion process. In this study, we used a combination of micro-computed tomography experiments and biphasic-solute finite element models to study the effects of three parameters of the overlying bath on the diffusion of neutral solutes across cartilage zones. Those parameters include bath size, degree of stirring of the bath, and the size and concentration of the stagnant layer that forms at the interface of cartilage and bath. Parametric studies determined the minimum of the finite bath size for which the diffusion behavior reduces to that of an infinite bath. Stirring of the bath proved to remarkably influence neutral solute transport across cartilage zones. The well-stirred condition was achieved only when the ratio of the diffusivity of bath to that of cartilage was greater than ≈1000. While the thickness of the stagnant layer at the cartilage-bath interface did not significantly influence the diffusion behavior, increase in its concentration substantially elevated solute concentration in cartilage. Sufficient stirring attenuated the effects of the stagnant layer. Our findings could be used for efficient design of experimental protocols aimed at understanding the transport of molecules across articular cartilage.
Site-specific elastic and viscoelastic biomechanical properties of healthy and osteoarthritic human knee joint articular cartilage

Abstract:Articular cartilage exhibits site-specific biomechanical properties. However, no study has comprehensively characterized site-specific cartilage properties from the same knee joints at different stages of osteoarthritis (OA). Cylindrical osteochondral explants ( n = 381) were harvested from donor-matched lateral and medial tibia, lateral and medial femur, patella, and trochlea of cadaveric knees ( N = 17). Indentation test was used to measure the elastic and viscoelastic mechanical properties of the samples, and Osteoarthritis Research Society International (OARSI) grading system was used to categorize the samples into normal (OARSI 0–1), early OA (OARSI 2–3), and advanced OA (OARSI 4–5) groups. OA-related changes in cartilage mechanical properties were site-specific. In the lateral and medial tibia and trochlea sites, equilibrium, instantaneous and dynamic moduli were higher ( p < 0.001) in normal tissue than in early and advanced OA tissue. In lateral and medial femur, equilibrium, instantaneous and dynamic moduli were smaller in advanced OA, but not in early OA, than in normal tissue. The phase difference (0.1–––0.25 Hz) between stress and strain was significantly smaller ( p < 0.05) in advanced OA than in normal tissue across all sites except medial tibia. Our results indicated that in contrast to femoral and patellar cartilage, equilibrium, instantaneous and dynamic moduli of the tibia and trochlear cartilage decreased in early OA. These may suggest that the tibia and trochlear cartilage degrades faster than the femoral and patellar cartilage. The information is relevant for developing site-specific computational models and engineered cartilage constructs.

Abstract:Computational mechanics has been advanced in every area of orthopedic biomechanics. The objective of this paper is to provide a general review of the computational models used in the analysis of the mechanical function of the knee joint in different loading and pathological conditions. Major review articles published in related areas are summarized first. The constitutive models for soft tissues of the knee are briefly discussed to facilitate understanding the joint modeling. A detailed review of the tibiofemoral joint models is presented thereafter. The geometry reconstruction procedures as well as some critical issues in finite element modeling are also discussed. Computational modeling can be a reliable and effective method for the study of mechanical behavior of the knee joint, if the model is constructed correctly. Single-phase material models have been used to predict the instantaneous load response for the healthy knees and repaired joints, such as total and partial meniscectomies, ACL and PCL reconstructions, and joint replacements. Recently, poromechanical models accounting for fluid pressurization in soft tissues have been proposed to study the viscoelastic response of the healthy and impaired knee joints. While the constitutive modeling has been considerably advanced at the tissue level, many challenges still exist in applying a good material model to three-dimensional joint simulations. A complete model validation at the joint level seems impossible presently, because only simple data can be obtained experimentally. Therefore, model validation may be concentrated on the constitutive laws using multiple mechanical tests of the tissues. Extensive model verifications at the joint level are still crucial for the accuracy of the modeling.

Abstract:Pancreases were obtained from five human fetuses 12 to 16 weeks old. The islets of Langerhans were isolated with collagenase, and then incubated with buffer, glucose, tolbutamide, or glucagon added to the medium. The insulin released into the medium was measured by immunoassay. Glucagon produced the only significant increase above base line; glucose and tolbutamide failed to enhance secretion of insulin. The data suggest that isolated human fetal islets of this gestational age develop responsiveness to glucagon earlier than to glucose or tolbutamide.
Application of confocal, SHG and atomic force microscopy for characterizing the structure of the most superficial layer of articular cartilage

Abstract:The surface of articular cartilage plays a crucial role in attenuating and transmitting mechanical loads in synovial joints to facilitate painless locomotion. Disruption to the surface of articular cartilage causes changes to its frictional properties instigating the deterioration of the tissue. In this study, we physically peeled the most superficial layer, a transparent membrane of 20.0 ± 4.7 µm thick, from the central loading region of femoral condyles of sheep. The ultrastructure of this layer without interference from the underlying cartilage was independently investigated using confocal, second harmonic generation and atomic force microscopy. We found that the most superficial layer contains chondrocytes, densely packed collagen, coarse elastic fibres and a fine elastic network. The elastic fibres are most prevalent at the surface of the layer, where collagen and chondrocyte densities are lowest. At the interface of this most superficial layer with the underlying bulk cartilage, a dense fibrillar network exists, formed mainly by collagen fibrils and elastin microfibrils. By contrast, the interface of the underlying cartilage with the most superficial layer contains collagen fibrils, fine microfibrils and microfibrils distinctively laced on one side. The findings of this study will play an important role in understanding the mechanical function and wear resistance of articular cartilage, and in developing more promising tissue engineering techniques to treat cartilage defects and osteoarthritis. LAY DESCRIPTION: The chronic pain and dysfuction in synovial joints caused by osteoarthritis can have a debilitating impact on daily activities for sufferers. Osteoarthritis is characterised by the deterioration of the articular cartilage. Despite intensive research, the wear mechanism of articular cartilage and the progression of osteoarthritis remain unclear in the literature. Articular cartilage is a resilient tissue that provides a low friction surface to facilitate painless locomotion. The coarse elastic fibres are most prevalent at the surface of the layer where collagen and chondrocyte densities are lowest. Furthermore, we investigated the surfaces at the interface of the most superficial layer with the underlying articular cartilage. At the interface of this most superficial layer with the underlying bulk cartilage, a dense fibrillar network exists, formed mainly by collagen fibrils and elastin microfibrils. In contrast, the interface of the underlying cartilage with the most superficial layer contains collagen fibrils, fine microfibrils and microfibrils distinctively laced on one side. The findings of this study have confirmed that there is a most superficial layer that is able to be removed using a tangential force. Through the application of advanced imaging technologies, we have shown that this most superficial layer is cellular and have detailed its composition and ultrastructure. Due to the close association between the form and function of tissues, the findings of this study will play an important role in understanding the mechanical function and wear mechanism of articular cartilage. This may lead to the development of more promising tissue engineering techniques to treat cartilage defects and osteoarthritis.
Influence of applied stress on energy dissipation and crack growth in articular cartilage

Abstract:Mechanical stress-induced damage to the articular cartilage can result in fracture imitation or significant tissue degeneration leading to osteoarthritis (OA) compromising joint mobility. Despite the clinical significance, a comprehensive understanding of the crack progression in cartilage damage remains elusive due to complex mechanical responses and variable initial crack geometry. This study investigated the impact of physiologically relevant stress levels, ranging from light-weight to high stress activities, on crack propagation in cartilage under compressive cyclic loading. Through empirical analysis, interconnections between localized damage (crack growth) and global damage (changes in material properties) across various levels of applied stress were established. Microscale cracks were nucleated in cylindrical cartilage plugs extracted from porcine patella specimens via microindentation and then subjected to controlled cyclic loading to examine changes in structural integrity, mechanical properties, and crack extension. Results demonstrated a decrease in cartilage thickness and apparent stiffness with increasing stress levels, indicative of bulk material damage. Dynamic mechanical properties such as energy dissipation and phase angle showed an overall reduction after fatigue loading. Crack growth followed an empirical law with increasing applied stress, reflective of greater mechanical damage at higher stress values. The findings of the study showed the interplay between global and local damage in cartilage under cyclic loading, suggesting insights into the fundamental changes occurring within the tissue. The outcomes of this investigation contributed to detailed understanding of the cartilage fatigue behavior through empirical laws and could potentially serve to prevent or delay tissue degeneration in OA.

Abstract:<h2>Abstract</h2><p>Synovial fluid flow in articular joint capsule plays an important role during mixed mode lubrication. However, the actual fluid flow behaviour during cartilage contact has not been fully understood so far. This is due to the difficulties in measuring the gap permeability using conventional experimental techniques. The problem becomes further complicated with consideration of the cartilage surface roughness. Here a validated numerical study was developed to quantify the gap permeability to lateral synovial fluid flow. Both macro and micro scale gap flow models were created based on Darcy's law at the macroscale and the Navier-stokes equation at the microscale. To generate model inputs, the cartilage topography was numerically synthesised based on the experimental measurements of bovine media tibia cartilage using Dektak Stylus Profilers. The experimental results show that the average surface roughness R<sub>a</sub> is 1.97 μm and root-mean-square roughness R<sub>q</sub> is 2.44 μm, while the correlation lengths of the secondary and tertiary undulations are round 100 μm and 20 μm, respectively. The numerical results indicate that the contact gap height and fluid pressure gradient are two critical parameters which significantly affect the gap permeability. As the contact gap closes, there is a decrease in gap permeability, and most importantly, the gap permeability is also very sensitive to the fluid pressure gradient. Furthermore, with gap closure, the permeability of the contact gap gradually approaches that of the cartilage tissue, at which point the contact gap is functional closed. This occurs at a contact gap around 1 μm and for a fluid pressure gradient is below 5 × 10<sup>5</sup> Pa/m.</p>

Abstract:Ultra-sensitivity TSH assays were performed, using Behring's kit (sensitivity: 0.03 microU/ml, in 183 patients operated upon for differentiated thyroid carcinoma and followed up while under treatment with L-thyroxine. Clinically, all patients were euthyroid. TSH was undetectable in 38 patients and within normal limits in 93. A negative correlation was found between the logarithm of thyroxine plasma levels and that of TSH levels, but there were many discrepancies (normal thyroxine level with undetectable TSH, high thyroxine level with normal TSH level). This study shows that the ultra-sensitive TSH assay provides accurate information on the inhibition of TSH secretion. Clinical evaluation and plasma thyroxine assays are needed to make sure that the patient is in a euthyroid state.
