Abstract:Objective To determine the value of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) for staging of lung cancer. Methods The study was retrospective, a total of 52 patients underwent EBUSTBNA for known or suspected lung cancer. All patients were detected enlarged mediastinal lymph nodes on CT scan ( ≥ 1.0cm). Results Of the 52 patients, 41 patients were found with N2 or N3 disease on EBUS-TBNA. 11 patients with negative EBUS-TBNA underwent thoracoscopy or thoracotomy for pulmonary resection and mediastinal lymph node dissection, 9 patients were confirmed N0 by pathology, whereas 2 patients had metastatic lymph node. The diagnostic sensitivity, specificity, accuracy, positive predictive value and negative predictive value of EBUS-TBNA for the mediastinal staging of lung cancer were 95.3%, 100%, 96.2%, 100%, and 81.8%, respectively. The procedure was uneventful, and there were no postoperative complications. Conclusion EBUS-TBNA is an effective and safe technique for mediastinal staging in lung cancer patients.

Abstract:Background Recently EBUS-TBNA, which has a sensitivity of 94.6%, specificity of 100% and diagnostic accuracy rate of 96.3% as previously reported, has been widely used for patients with mediastinal and hilar lymphadenopathy or suspected lung cancer to get accurate diagnosis. The purpose of the current study was to evaluate the usefulness of EBUS-TBNA in obtaining cytological and histological diagnosis of mediastinal and hilar lymph nodes compared to the results obtained with conventional mediastinoscopy as previously reported, and to assess the relationship of diagnostic accuracy and number of passes and size of lymph nodes. Methods 101 patients with mediastinal and hilar lymphadenopathy or suspected lung cancer in our institution were included in this prospective study. EBUS-TBNA was performed in all cases. The final diagnosis was confirmed by cytology, surgical results, and/or clinical follow-up for at least 6 months. Sensitivity, specificity, accuracy, and positive and negative predictive values were calculated using standard formulas. Results In 101 patients, EBUS-TBNA was successfully performed to obtain samples from 225 lymph nodes, 7 lung masses, 1 mediastinal mass and 2 esophageal masses. 63 malignant tumors and 38 benign diseases were confirmed. Epidermal growth factor receptor mutation was detected in 10 biopsy samples, and epidermal growth factor receptor mutation was detected in 4 cases. With respect to the correct diagnosis of mediastinal and hilar lymphadenopathy, EBUS-TBNA had a sensitivity of 95.08%, specificity of 100%, positive predictive value of 100%, negative predictive value of 93.02%, and overall accuracy of 97.02%. The relationship of diagnostic accuracy and number of lymph node passes or size of lymph nodes was both insignificant (p = 0.27; p = 0.23). The procedure was uneventful without complications. Conclusions EBUS-TBNA is an accurate and safe tool in diagnosis of mediastinal and hilar lymphadenopathy. It cannot completely replace mediastinoscopy, it may indeed reduce the number of mediastinoscopy procedures. In some cases, it can necessarily be the first-line procedure before mediastinoscopy.

Abstract:STUDY OBJECTIVES:This systematic review and meta-analysis was conducted to evaluate the accuracy of the combined endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) techniques and clarify its current role for the mediastinal lymph node staging of lung cancer. METHODS:Medline, Web of Science, Elsevier and Ovid were searched to identify suitable studies up to 15th July 2012. Two investigators independently reviewed articles and extracted data. All EBUS-TBNA plus EUS-FNA studies for the mediastinal node staging of lung cancer were systematically reviewed. Sensitivity, specificity and other accuracy measures were pooled using random-effect models. Summary receiver operating characteristic curves were used to summarise overall test performance. RESULTS:Eight studies met our inclusion criteria. The estimated summary measures for quantitative analysis of EBUS-TBNA plus EUS-FNA for mediastinal nodal staging of lung cancer were sensitivity, 0.86 (95% confidence interval [CI], 0.82-0.90); specificity, 1.00 (95% CI, 0.99-1.00); positive likelihood ratio, 51.77 (95% CI, 22.53-118.94); negative likelihood ratio, 0.15 (95% CI, 0.09-0.25); diagnostic odds ratio, 416.83 (95% CI, 140.08-1240.31); and area under the curve (AUC), 0.99. CONCLUSIONS:The current evidence suggests that the combined technique is more sensitive than EBUS-TBNA or EUS-FNA alone. The diagnostic power of this combined technique is accurate. As an almost completely minimally-invasive examination, EUS-FNA plus EBUS-TBNA may replace more invasive methods for evaluating mediastinal node staging of lung cancer.

Abstract:【Objective】 To determine the value of endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA) in the diagnosis of lung cancer.【Methods】 A total of 56 patients who underwent EBUS-TBNA to diagnose radiologically highly suspected lung cancer between September 2009 and June 2010 was retrospectively reviewed.Conventional bronchoscopic biopsy before EBUSTBNA was non-diagnostic in all patients.【Results】 Of the 56 patients,45 patients were detected lung mass and enlarged mediastinal/hilar lymph nodes on radiographic examination of the chest(≥1.0 cm),EBUS-TBNA was performed to obtain samples from mediastinal or hilar enlarged lymph nodes,36 cases were confirmed lung cancers,nine cases were diagnosed by other methods(3 lung cancer,1 metastatic lung tumor,and 5 benign cases).11 patients with pulmonary masses located close to the central airways were accessed by EBUS-TBNA,lung cancer was confirmed in 9 cases,pneumonia in one.One case that was not confirmed by EBUS-TBNA was diagnosed lung cancer through thoracoscopic biopsy.The sensitivity and diagnostic accuracy of EBUS-TBNA in the diagnosis of lung cancer were 91.8%(45/49) and 92.9%(52/56),respectively.The procedure was uneventful without complications.【Conclusions】 EBUS-TBNA targeting lymph nodes or masses highly suspicious for malignancy demonstrated high diagnostic value in the diagnosis of lung cancer.EBUS-TBNA is recommended for these cases,especially when other diagnostic methods have failed or are difficult.

Abstract:Background Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is considered to have high value in the staging of mediastinal lymph nodes in lung cancer. The current study was conducted to investigate the diagnostic value of EBUS-TBNA in intrapulmonary lesions located near the central airway. Methods From September 2009 to March 2013, 66 patients with pulmonary masses located close to the central airways suspected to be lung cancer were accessed by EBUS-TBNA. Conventional bronchoscopic biopsy before EBUS-TBNA was nondiagnostic in all cases. If EBUS-TBNA did not result in a formal pathological diagnosis of malignancy, patients were subsequently referred for a surgical procedure. Results Among the 66 cases, 59 were confirmed as pulmonary malignancies by EBUS-TBNA, of which 48 cases were non-small cell lung cancer, nine were small cell lung cancer, and two were metastatic lung tumors. No evidence of malignancy was found by biopsy and histopathological examination in the other seven cases. Thoracoscopy or thoracotomy was subsequently undergone for them. Postoperative pathological examinations confirmed three cases of squamous cell carcinoma of the lung, one case of lymphoma, two cases of sclerosing hemangioma, and one case of pulmonary tuberculoma. The definitive diagnosis rate of EBUS-TBNA for intrapulmonary lesions near the central airway was 89.4%. The sensitivity, specificity, and accuracy of EBUS-TBNA in distinguishing benign from malignant intrapulmonary lesions were 93.7%, 100.0%, and 93.9%, respectively. The positive and negative predictive values were 100.0% and 42.9%, respectively. The EBUS-TBNA procedures were well-tolerated by all patients. No associated complications were observed. Conclusions For intrapulmonary lesions near the central airway highly suspected of cancer, EBUS-TBNA has satisfactory diagnostic value. However, the negative predictive value of this technique is low, so negative results obtained by EBUS-TBNA should be confirmed by other methods.

Abstract:BACKGROUNDEndobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) for the diagnosis of mediastinal and hilar lymph is poorly studied in patients with extrathoracic malignancies.AIMTo evaluate the value of EBUS-TBNA for the diagnosis of enlarged intrathoracic lymph nodes in patients with extrathoracic malignancies.METHODSThis was a retrospective study of patients with extrathoracic malignancies who were referred to Peking University Cancer Hospital from January 2013 to December 2018 for EBUS-TBNA due to intrathoracic lymphadenopathy. The specimens were defined as positive for malignancy, negative for non-malignancy (tuberculosis, sarcoidosis, etc.), and without a definitive diagnosis. Sensitivity, negative predictive value (NPV) for malignancy, and overall accuracy were calculated. Complications were recorded.RESULTSA total of 80 patients underwent EBUS-TBNA and had a final diagnosis, among which 50 (62.5%) were diagnosed with extrathoracic malignancy with intrathoracic lymph nodes metastasis, 14 (17.5%) were diagnosed with primary lung cancer with nodal involvement, and 16 (20.0%) exhibited benign behavior including tuberculosis, sarcoidosis and reactive lymphadenitis or who had benign follow-up. The diagnostic sensitivity, NPV, and accuracy of EBUS-TBNA for intrathoracic lymphadenopathy in patients with extrathoracic malignancy were 93.8% (n = 60/64), 80.0% (n = 16/20), and 95.0% (n = 76/80), respectively. In the multivariate analysis, longer short axis of the lymph node (OR: 1.200, 95%CI: 1.024-1.407; P = 0.024) and synchronous lung lesion (OR: 19.449, 95%CI: 1.875-201.753; P = 0.013) were independently associated with malignant intrathoracic lymphadenopathy. No characteristics of the lymph nodes and EBUS-TBNA were associated with the location of malignant intrathoracic lymphadenopathy, and no major complication was observed.CONCLUSIONEBUS-TBNA is a simple and accurate procedure for the diagnosis of intrathoracic lymphadenopathy with extrathoracic malignancy.
Evaluation of Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration in the Diagnosis of Hilar and Mediastinal Masses of Unknown Etiologies

Abstract:OBJECTIVE:To explore the diagnostic values of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the patients with hilar and mediastinal masses. METHODS:A total of 91 patients with mediastinal/hilar masses undergoing EBUS-TBNA in Zhongshan Hospital between September 2009 and March 2011 were retrospectively enrolled. Their unknown etiologies were difficult to be assessed by a traditional biopsy. The association of pathologic examinations and clinical data were analyzed. RESULTS:Among them, 61 patients had malignant lesions while 30 patients were diagnosed with benign diseases. In the cases with malignant lesions, the diagnostic sensitivity, specificity and accordance rate were 91.67%, 100% and 91.80% respectively. The diagnostic accordance rate of benign lesions was 60%. And 36.9% (24/65) of the samples were small-sized so as to improve the diagnostic accuracy. The combination of cytology and histology significantly increased the diagnostic accordance versus cytology alone in all cases (P < 0.01). But there was no statistically significant difference in the malignant lesion subgroup with a better tendency (P > 0.05). No severe complication occurred. CONCLUSION:With a high diagnostic accuracy and a low complication rate for the patients with hilar and mediastinal masses of unknown etiologies, EBUS-TBNA has different values for diagnosing malignant and benign lesions.

Abstract:Objective To investigate the application, diagnostic value and safety of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in hilar and mediastinal lesions. Methods A total of 246 patients with thoracic computed tomography (CT) examinations showed mediastinal, hilar lymphadenopathy and/or pleural masses, which were examined for EBUS-TBNA in the department of respiratory medicine, Second Affiliated Hospital of Chongqing Medical University from October 2014 to September 2016.The results of cytology or pathology, the positive rate of diagnosis and the incidence of complications were analyzed. Results In the 246 patients, there were 130 cases of malignant tumors, 72 cases of benigh diseases and 26 cases of suspicious cancer, but 18 cases′ pathological diagnosis couldn′t be definite.In malignant tumors cases, 45 cases were small cell lung cancer and 39 non-small cell lung cancer, 42 cases of non-specific type, 1 case of metastatic neuroendocrine carcinoma, 2 cases of type B lymphoma and 1 case of primary mediastinal sarcoma.In benign disease, there were 22 cases of non-specific inflammation, 2 cases of mediastatic cyst, 1 case of pulmonary abscess, 1 case of pulmonary cyst with actinomycete infection, 8 cases of pneumoconiosis, 27 cases of mediastinal lymph node tuberculosis, and 11 cases of sarcoidosis. Conclusions EBUS-TBNA is of great value in the diagnosis of unexplained hilar, mediastinal lymph node enlargement or hilar and mediastinal epiphyseal lesions, especially for the diagnosis of sarcoidosis.It is also an important auxiliary examination to diagnosis of the rare hilar and mediastinal dieases. Abstract:Objective To discuss the clinical value and safety of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in screening mediastinal and hilar lymphadenectasis;to summarize similarities and differences of clinical features between benign and malignant diseases,so as to provide theoretical basis for clinical di agnosis and treatment of such diseases.Methods EBUS TBNA was performed on 47 hospitalized patients,who had been diagnosed with mediastinal lymphadenectasis through HRCT in respiratory medicine department of Peking University Third Hospital from January 1st,2014 to December 31st,2014,and statistical analysis on clinical data was conducted based on the results of EBUS-TBNA.Results 47 patients were included in this study,and 85 lymph nodes were performed biopsy.The sensitivity,accuracy and specificity of diagnosis through EBUS-TBNA were 91.5％,85.1 ％ and 97.9％,respectively,with no complications.19 (40.4 ％) cases were eventually diagnosed as malignant diseases through EBUS-TBNA,while 28 were benign diseases.The average age of subjects was (53.83 ± 15.05) years old,and patients in malignant group were significantly elder than those in benign group (P＜0.05),since their average ages were (61.68±13.76) and (48.50 ±15.48) years old,respectively.The ratio of major axis to minor axis of the largest lymph node was found significant lower in patients in malignant group than those in benign group through HRCT,with the value as (1.28 ± 0.20) and (1.63± 0.57),respectively.Conclusions EBUS-TBNA has excellent sensitivity,accuracy,specificity and safety in diagnosing mediastinal and hilar lymphadenectasis.Older male,positive CEA,almost circular lymph node showed in HECT were factors requiring particular attention.It is advised that EBUS-TBNA should be conducted for definite diagnosis as early as possible,so as to achieve early treatment.
Evaluation of the Value of EBUS-TBNA in Diagnosis of Mediastinal Lesions

Abstract:OBJECTIVE:To evaluate the value of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in diagnosis of mediastinal lesions and to discuss its optimal indication.METHODS:One hundred and twenty three patients with mediastinal lesions who underwent EBUS-TBNA were included in this study. The accuracy, sensitivity, specificity, positive and negative predictive value of EBUS-TBNA in diagnosis of mediastinal lesions were analyzed according to the final diagnosis and evaluate its value and the optimal indication.RESULTS:In the 123 patients, EBUS-TBNA was successfully performed to obtain samples from 286 stations of lymph nodes (2.33 stations/per patient). The puncture success rate was 100%. The procedure was uneventful without complications. Final diagnosis indicated that there were 83 positive and 40 negative patients. EBUS-TBNA had a sensitivity of 95.2%, specificity of 100%, positive predictive value of 100%, negative predictive value of 90.0%, and overall accuracy of 96.8%. For diagnosis of the epithelial cancer, EBUS-TBNA had an accuracy of 98.8%, sensitivity of 98.8%, specificity of 100%, positive predictive value of 100%, and negative predictive value of 100%. EBUS-TBNA failed to reveal three lymphomas. For diagnosis of benign mediastinal diseases, EBUS-TBNA had a diagnosis rate of 47.2% which had a confirmed clinical application value.CONCLUSIONS:EBUS-TBNA may be expected to replace the mediastinoscopy as a superior choice for diagnosis of mediastinal epithelial cancers. EBUS-TBNA can not replace mediastinoscopy but being a promising tool for diagnosis of benign mediastinal lesions including granulomas. For certain special diseases such as lymphoma, mediastinoscopy cannot be replaced. However, EBUS-TBNA can be a potentially favorite choice for early stage screening.

Abstract:Objective To review the experience d EBUS-TBNA for staging of lung cancer and the value in diagnosing thoracic diseases in our single center.Methods The data of 343 patients who underwent EBUS-TBNA from September 2009 to August 2011 in our institution were retrospectively reviewed.There were 219 males and 124 females with an average age of (59.4 ± 13.6 ) years.Based on their primary indication,patyients were divided into three categories:group A:with known or strongly suspected lung cancer and enlarged mediastinal lymph nodes on chest radiographic examination ( short axis ≥ 1.0cm) ; group B:with enlarged mediastinal lymph nodes or mediastinal masses of unknown origin; and group C:with pulmonary parenchymal mass located close to the central airways.Results The average short axis diameter of the thoracic lesions was ( 1.94 ± 1.01 ) cm ( range from 0.5 to 8.0cm),and 2.66 punctures were performed per lesion.In group A ( n =208 ),151 patients were confirmed to have mediestinal lymph nodes metastasis while 51 showed negative results.Four patients were diagnosed as tuberculosis and two were confirmed to be stage Ⅱ sarcoidosis.37 in the 51 patients with negative EBUS-TBNA underwent thoracoscopic or thoracotomy for pulmonary resection and mediastinal lymph node dissection.Postoperative pathology confirmed that 32 patients did not have lymph nodes metastases.The diagnostic sensitivity,specificity,accuracy,positive predictive and negative predictive of EBUS-TBNA for the mediastiral staging of lung cancer were 96.8％ (151/156),100.0％ (32/32),97.3％ ( 183/188 ),100％ ( 151/151 ) and 86.5％ (32/37),respectively.In group B ( n =94),22 patients had malignancy and 72 had benign diseases.Thirteen patients received operative validation in the 23 cases which were diagnosed as proliferative lymph nodes by EBUS-TBNA,and by further operation two and three patients were confirmed as malignancy and other benign diseases respectively.The sensitivity,negative predictive value ( NPV ) and accuracy of EBUS-TBNA in distinguishing malignant mediastinal diseases was 88.0％ (22/25)、100％ (73/73) 、95.9％ (70/73)and 97.9％ (92/94),respectively.In group C( n =41 ),malignant diagnosis was achieved in 33 patients,while 4 patients confirmed as malignancy by further operations in the other 8 negative cases.The diagnostic sensitivity and accuracy of EBUS-TBNA for the diagnosis of unknown pulmonary parenchymal mass were 89.2％ (33/37) and 90.2％ (37/41),respectively.All the procedures were uneventful and no complication occurred.Conclusion EBUS-TBNA is a highly effective and safe procedure in the diagnosis of thoracic diseases and staging.of lung cancer.

Abstract:BACKGROUND AND OBJECTIVE:It has been proven that endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) was a new minimally invasive method in the diagnosis for mediastinal lymph nodes. The aim of this study is to evaluate the diagnostic and staging yield of EBUS-TBNA for lung cancer.METHODS:Seventy-five patients with tumors and enlarged mediastinal lymph nodes found by CT underwent the diagnosis by endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) from April 1, 2009 to Febuary 8, 2010. The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of EBUS-TBNA were evaluated.RESULTS:Seventy-five patients with 177 lymph node groups (2.4 groups in average) were studied. Histopathological samples were found in 49.33% patients and in 28.81% groups. Overall sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of EBUS-TBNA for diagnostic were 98.43%, 100.00%, 100.00%, 91.67% and 98.66%, respectively, in per patient analysis and were 95.10%, 100%, 100.00%, 82.93%, and 96.05%, respectively, in per group analysis, higher than CT examination (P < 0.05) expect for sensitivity (P = 0.435). Staging changed in 19 (26.03%) patients after EBUS-TBNA.CONCLUSION:EBUS-TBNA proved to be a safe procedure with a high yield for the diagnosis of lung cancer.

Abstract:Objective: To evaluate the role of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the diagnosis of isolated mediastinal lesions. Methods: A retrospective study was conducted of 73 consecutive patients with isolated mediastinal lesions of unknown origin without parenchymal lung abnormalities, who underwent EBUS-TBNA from September 2009 to April 2011. The patients who were nondiagnostic with EBUS-TBNA subsequently underwent surgical biopsies and a minimum of 6 months'clinical and radiologic follow-up. Results: EBUS-TBNA achieved definitive diagnoses in 60 patients of the 73 patients (82.2%, 60/73). Malignancies were diagnosed in 23 patients and benignancies in 37. The sensitivity, specificity, and accuracy of EBUS-TBNA in distinguishing malignant mediastinal lesions were 95.8%(23/24), 100%(49/49) and 98.6%(72/73), respectively. EBUS was well tolerated by all of the patients with no complications. Conclusion: EBUS-TBNA of isolated mediastinal lesions is a minimally invasive and safe diagnostic technique with high yield.

Abstract:Objective To evaluate the value of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) for CT-positive mediastinal lymph nodes. Methods From September 2009 to December 2009, 28 patients with confirmed or suspected non-small-cell lung cancer with CT scan demonstrating enlarged ( ≥ 1 cm) mediastinal lymph nodes underwent EBUS-TBNA. The diagnostic accuracy, sensitivity, specificity, positive predictive value and negative predictive value were evaluated. Results 28 patients with 40 lymph nodes were studied. 27 patients had been performed successfully with enough specimens. No complications happened in the group. Mediastinal metastases were confirmed by EBUS-TBNA in 20 patients. 8 patients with benign mediastinal nodes as detected by EBUS-TBNA underwent surgery and mediastinal lymph node dissection, which confirmed N2 disease in 2 patients. Overall diagnostic accuracy, sensitivity, specificity, positive predictive value and negative predictive value of EBUS-TBNA in the detection of mediastinal metastasis were 92.9%, 90.9%,100%, 100%, 75% respectively. Conclusion EBUS-TBNA is a safe and effective technique for CT-positive mediastinal lymph nodes on CT scan.

Abstract:Aim: This study aims to investigate the value of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the diagnosis and treatment of mediastinal and pulmonary diseases. Methods: The clinical data of 52 patients who were hospitalized in the Department of Respiratory and Critical Care Medicine at Jinling Hospital from July 2014 to February 2015 with documented EBUS-TBNA was obtained. Results: Positive results of EBUS-TBNA biopsy were statistically compared to the final diagnosis. A total of 82 specimens from 52 patients were obtained through EBUS-TBNA biopsy. Out of 52, 45 patients had concurrent surgical pathology, including 33 malignant lesions and 12 benign lesions. There were 7 patients with false-negative results, including 1 case of pulmonary sclerosing hemangioma, 4 cases of lung cancer, and 2 cases of tuberculosis. The sensitivity of EBUS-TBNA in this study was 85.2%, accuracy of 85.4%, positive predictive value of 100%, and negative predictive value of 7.7%. There were no serious complications. Conclusion: EBUS-TBNA is a safe and minimally invasive method for the diagnosis of mediastinal and pulmonary diseases. It may also have utility as a therapeutic option during the diagnostic process of some diseases.

Abstract:Objective To study the diagnostic value and safety of CT-guided needle biopsy of mediastinal and pulmonary hilar masses. Methods 34 cases of mediastinal and hilar masses with negative results of bronchoscopy from April 1998 up to 2001 were retrospectively summarized. This group included 19 cases of mediastinal or near to mediastinal masses, 7 cases of mediastinal lymphadenopathy near to superior vena cava or in the subcarinal region and 8 cases of pulmonary hilar masses, of which there were 25 men, 9 women and aged 16～75 years old with mean 47.Puncture under CT guiding and keep away from the important organ components one to three times was performed using 18～20 Gauge core-needle and aspiration needle.Results The specimens were satisfactory obtained in all cases and pathologic diagnosis was made in 32 cases and the positivity was 94%. Mild pneumothorax and mild pulmonary bleeding due to biopsy were found in 4 cases (12%) and 2 cases( 6%) respectively. This group showed most of unexplained masses in the pulmonary hilum and near to the mediastinum which were negative on bronchoscopy were small cell lung carcinoma(SCLC).Conclusion CT-guided core-needle biopsy is of great value for mediastinal and lung hilar masses which were negative on bronschoscopy. This method is safe and reliable. Additional aspiration biopsy may be helpful to increase positivity for some lesions of thymal regions.

Abstract:BACKGROUND:Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a valuable tool for diagnosing pulmonary disease due to its efficiency and safety. We retrospectively analyzed patients with mediastinal masses who underwent diagnostic EBUS-TBNA at Shanghai Chest Hospital, and evaluated the clinical accuracy of EBUS-TBNA in the diagnosis mediastinal masses.METHOD:From 2009 and 2014, patients who received EBUS-TBNA to diagnose a isolated mediastinal mass were enrolled. Clinical follow-up was performed to ascertain the patient's final diagnosis.RESULTS:Forty-six patients were enrolled in this study. Thirty-seven were diagnosed with an oncologic disease, 3 were diagnosed with a mediastinal infection, and 2 were found to have a mediastinal goiter. The overall sensitivity, specificity, positive predictive value, negative predictive value, diagnostic yield was 63.6%, 100%, 100%, 42.9%, and 71.4%, respectively.CONCLUSION:EBUS-TBNA is a safe and effective means of diagnosing mediastinal masses.
Endobronchial Ultrasound-guided Transbronchial Needle Aspiration for Diagnosis and Differential Diagnosis of Mediastinal Lesions

Abstract:Objective To investigate the value of endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA) in diagnosis and differential diagnosis of isolated mediastinal lesions.Methods We analyzed 164 cases of simple mediastinal lesions without pulmonary involvement,who received EBUS-TBNA for diagnosis in our hospital from September 2009 to August 2012.Definitive diagnosis was defined as diagnosis confirmed by pathological examinations,and indefinitive diagnosis was defined as missampling or incomplete sampling of tissues,or no definite proofs obtained.For patients with indefinitive diagnosis,a surgical biopsy or at least 6-month follow-up by imaging examination was carried out.Results The procedure was tolerated well in all the patients without causing any complications.Definitive diagnosis was achieved in 139 patients with EBUS-TBNA [68 cases of malignancies and 71 cases of benign lesions;rate of definite diagnosis: 84.8%(139/164)].Indefinitive diagnosis occurred in 25 patients,including missampling or incomplete sampling in 6 cases and atypical lymphnoditis in the other 19.In these 25 cases,a further diagnosis was then achieved in 10 cases(including the 6 cases of missampling or incomplete sampling) by mediastinoscopy,thoracoscopy,or supraclavicular lymph nodes biopsy,which discovered lymphoma in 4 patients,Castleman disease in 2,sarcoidosis in 1,tuberculosis of the mediastinal lymph nodes in 1,mature teratoma in 1,and reactive hyperplasia of the mediastinal lymph nodes in 1;the other 15 patients were followed up for at least 6 months by imaging examinations,which all confirmed the primary diagnosis.The sensitivity,specificity,and accuracy of EBUS-TBNA were 91.9%(68/74),100%(90/90),and 96.3%(158/164),respectively.With the procedure,93.9%(154/164) of the patients avoided surgical biopsy.Conclusion For simple isolated mediastinal lesions,EBUS-TBNA is a safe and effective method for diagnosis and differential diagnosis.

Methods The clinical data of total 52 patients who hospitalized in the department of respiratory and critical care medicine in Nanjing general hospital of Nanjing military command during July 2014 to February 2015 and also taken the examination of EBUS-TBNA were obtained. The positive results of EBUSTBNA biopsy were statistically analyzed with the final diagnosis. Results Total 82 specimens from all 52 patients were obtained via EBUS-TBNA biopsy. Among these,45 cases were diagnosed by pathology,including 33 cases of malignant lesions and 12 cases of benign lesions. There were 7 patients with false-negative results,including 1 cases of pulmonary sclerosing hemangioma,4 cases of lung cancer,2 cases of tuberculosis. The sensitivity of EBUS-TBNA in this study was 85. 2%,specificity of 100%,accuracy of 85. 4%,positive predictive value of 100%,and negative predictive value of 7. 7%. All patients had no serious complications. Conclusion EBUS-TBNA examination is a safe and minimally invasive method for the diagnosis of mediastinal and pulmonary diseases,which can also be used as a further therapy approach during the diagnostic process of some diseases.

Abstract:Background Endobronchial ultrasound-guided transbronchial needle aspiration is a minimally invasive and effective sampling approach for patients with mediastinal or hilar lymphadenopathy. Increased recognition of the ultrasonographic features revealed the value of its images in predicting mediastinal lymph node malignancy. However, its diagnostic validity and the predictive value of its ultrasonographic features have not been demonstrated well in patients after systemic anti-tumor therapy. This study aimed to evaluate the efficiency of endobronchial ultrasound-guided transbronchial needle aspiration in patients with suspicious lymph nodes after anti-tumor therapy. Methods We retrospectively reviewed cases of endobronchial ultrasound-guided transbronchial needle aspiration performed between January 2019 and August 2021 at a single tertiary hospital center. Patients with suspected mediastinal or hilar lymph nodes within 5 years of systemic anti-tumor therapy were enrolled. Final diagnoses were determined by pathologic diagnoses of samples from transbronchial needle aspiration, surgery, or follow-up for at least 6 months. Ultrasonographic features were analyzed to assess the predictive value of malignant lymph nodes after treatment. Results Overall, 168 lymph nodes of 138 patients were analyzed. Among 110 (65.5%) malignant lymph nodes, 75 originated from lung cancers; the other 35 were from other malignancies. No complications related to endobronchial ultrasound-guided transbronchial needle aspiration were observed. Of 58 negative results of transbronchial needle aspiration, 51 were proven to be true negatives; 7 were false. The overall sensitivity and the negative predictive value were 94.02% and 87.93%, respectively. Univariate and multivariate analysis revealed the absence of central hilar structure and short axis > 10 mm as independent predictive factors for malignancy. Conclusions Endobronchial ultrasound-guided transbronchial needle aspiration performs satisfactorily in diagnosing mediastinal and hilar lymphadenopathy even after anti-tumor treatment.
