Abstract:The prevalence of urinary complications after various anorectal operations was studied in a group of 273 patients. The overall prevalence of urinary complications was 26.7%; most of these complications affected men between 41 and 50, mainly after hemorrhoidectomy. In 10.6% of patients, bladder catheterization was needed. These urinary complications result from nervous reflexes originating from the anus and determined by the operative trauma and/or rectal distinction. In the treatment of these urinary complications, the role of the muses is essential for reassuring the patients. Parasympathomimetic drugs are often efficient. Urinary catheterization must be delayed until the 18th hour. Fluid restriction may be useful to prevent urinary retention.

Abstract:A man underwent placement of a lumbar catheter to drain cerebrospinal fluid at the start of a vascular surgery procedure. On the first postoperative day, an anesthesia resident who had never removed this type of drain removed the drain after consulting with his attending physician. The patient was seen on postoperative day 6 in the vascular surgery clinic reporting drainage from the lumbar drain site that had persisted since hospital discharge. That same day, the anesthesia resident removed another lumbar catheter and noted that the tip appeared different than the previous one. Suspecting that a fragment of the catheter tip may have been retained in the patient, he notified the attending anesthesiologist and the vascular surgery clinic. On examination of the patient’s back, there was a raised area of skin with clear fluid at the lumbar drain site. Imaging was obtained, which confirmed the retained lumbar catheter tip.

Postoperative urinary retention after surgery for benign anorectal disease: potential risk factors and strategy for prevention

Abstract:PurposeThis study was undertaken to determine the incidence of and risk factors for urinary retention after surgery for benign anorectal disease.MethodsWe reviewed 2,011 consecutive surgeries performed under spinal anesthesia for benign anorectal disease from January through June 2003 to identify potential risk factors for postoperative urinary retention. In addition, we prospectively investigated the preventive effect of perioperative fluid restriction and pain control by prophylactic analgesics on postoperative urinary retention.ResultsThe number of procedures and the urinary retention rates were as follows: hemorrhoidectomy, 1,243, 21.9%; fistulectomy, 349, 6.3%; incision/drainage, 177, 2.3%; and sliding skin graft/lateral subcutaneous internal sphincterotomy, 64, 17.2%. The overall urinary retention rate was 16.7%. With hemorrhoidectomy, female sex, presence of preoperative urinary symptoms, diabetes mellitus, need for postoperative analgesics, and more than three hemorrhoids resected were independent risk factors for urinary retention as assessed by multivariate analysis. With fistulectomy, female sex, diabetes mellitus, and intravenous fluids >1,000 ml were independent risk factors for urinary retention. Perioperative fluid restriction, including limiting the administration of intravenous fluids, significantly decreased the incidence of urinary retention (7.9 vs 16.7%, P<0.0001). Furthermore, prophylactic analgesic treatment significantly decreased the incidence of urinary retention (7.9 vs 25.6%, P=0.0005).ConclusionsUrinary retention is a common complication after anorectal surgery. It is linked to several risk factors, including increased intravenous fluids and postoperative pain. Perioperative fluid restriction and adequate pain relief appear to be effective in preventing urinary retention in a significant number of patients after anorectal surgery.

Role of warm water bath in inducing micturition in postoperative urinary retention after anorectal operations

Abstract:The relation of micturition to sitting in a warm water bath was studied in 30 healthy volunteers and 21 patients with urinary retention after hemorrhoidectomy. The rectal temperature, vesical and urethral pressures as well as the EMG activity of the external urethral sphincter were recorded before sitting in and immediately after getting out of the sitz bath at 40, 45 and 50 degrees C. In the 40 degrees C bath, 16 of the 30 normal subjects reported spontaneous micturition and 14 voluntarily induced micturition. The number of spontaneous micturitions increased with higher-temperature baths. Spontaneous micturition occurred in 19 patients with urinary retention when they sat in the sitz bath; 2 patients did not micturate and were catheterized. The urethral pressure in both the normal and retention subjects showed significant reduction which increased with higher-temperature baths. There were no significant changes in the vesical pressure or EMG activity of the external urethral sphincter with water baths. Micturition on sitting in a warm water bath seems to be initiated by reflex internal urethral sphincter relaxation. A thermo-sphincter reflex is likely to be involved.

Abstract:Introduction: Associated congenital anomalies, operative structural injury, and postoperative neurovesical dysfunction combine to cause urinary retention in children with a history of surgery for anorectal malformation (ARM). Aim: To study the presentation and management of urinary retention in patients with a history of ARM surgery. Methodology: Retrospective study. Results and Discussion: Twenty-five children presented with urinary retention with a history of ARM surgery performed elsewhere. There were 17 males (high – 14, intermediate – 2, and low – 1) and 8 females (cloaca). Sixteen children had an abnormal spine. Eight children had urethral injury (total transection – 3) and three had a large residual rectal stump. 41/48 renal units were dilated and 27 refluxing. Seventeen children had abnormal estimated glomerular filtration rate and five had undergone urinary diversion. Management: Definitive surgical management was individualized, the most pertinent consideration being whether normal voiding would be feasible or whether a continent low pressure urinary reservoir with clean intermittent catheterization (CIC) would be a safer option. Operative management included excision of the rectal stump (3), urethral reconstruction (2), bladder augmentation (17), Mitrofanoff port (22), bladder neck closure (2), and antireflux surgery (13). Follow-up estimated glomerular filtration rate had improved/normalized in all but two patients. HUN resolved/improved in all and 25/27 refluxing units ceased refluxing. All are socially continent with ten voiding normally and the rest on CIC. Conclusion: Urinary retention after ARM surgery is multifactorial and requires prompt recognition and possibly urinary diversion. Final reconstruction aims at achieving continence with safe upper tracts. Urethral voiding is possible in the selected cases.

Annals of Surgery, 216, 673-676. 68 HPB INTERNATIONAL Cavernous hemangiomas are the most common benign tumors of the liver. Giant cavernous hernangiomas, defined as those larger than 4crn in diameter, can reach enormous proportions. Newer imaging rnodalities, although often demonstrating characteristic features that strongly suggest the diagnosis, should not be augmented by biopsy because of the risk of hemorrhage. Elective surgical resection may be indicated for symptomatic giant lesions and for those with an atypical appearance where the diagnosis is in doubt. Between October 1986 and May 1991, we treated 10 patients with giant hernangiomas by enucleation or enucleation plus resection. Median operative blood loss was 800 rnL (range 200 to 3000 mL). One patient required reoperation for control of postoperative hemorrhage. Detailed pathologic examination has demonstrated an interface between hemangiomas and the normal liver tissue that allows enucleation. Enucleation is an underused procedure that if carefully performed allows resection of giant hernangiomas with a reduced blood loss and the preservation of virtually all normal hepatic parenchyma.

Abstract:Anorectal surgery is well tolerated. Rates of minor complications are relatively high, but major postoperative complications are uncommon. Prompt identification of postoperative complications is necessary to avoid significant patient morbidity. The most common acute complications include bleeding, infection, and urinary retention. Pelvic sepsis, while may result in dramatic morbidity and even mortality, is relatively rare. The most feared long-term complications include fecal incontinence, anal stenosis, and chronic pelvic pain.
Postoperative urinary retention in general surgical patients

Abstract:A prospective study has been performed to measure the incidence of postoperative urinary retention in 280 general surgical patients. Eighteen patients (6 per cent) were catheterized after a range of operations under general anaesthesia. There was no statistical difference in the retention rate between men and women. Increasing age did not predispose to retention of urine. Of 72 patients who had either herniorrhaphy, haemorrhoidectomy or testicular operations, only 1 was catheterized, whereas 16 (23 per cent) of 70 patients undergoing laparotomy were catheterized. Most of these laparotomy patients had upper abdominal procedures carried out. Those having lower abdominal operations were usually catheterized pre-operatively and were excluded from the study. There was a significantly higher incidence of retention among patients who had long anaesthetics (greater than 60 min). There was also a significantly higher incidence of retention among those who were ventilated, relaxed and reversed by atropine and neostigmine. Of those 193 patients who had opiate analgesia, 15 (8 per cent) developed retention of urine, compared with 3 (3 per cent) of the 87 patients who had non-opiate analgesia. There was a significantly higher retention rate among the patients who had opiate analgesia by intravenous infusion compared with those who had opiates by intramuscular bolus injection; 7 patients had painful and 11 had painless retention. This study suggests that postoperative retention of urine is a result more commonly of diminished awareness of bladder sensation, than of factors such as anxiety or local pain.

Abstract:Anorectal surgery is associated with significant postoperative pain. Pudendal nerve blocks, formerly performed by surgeons, provides effective postoperative analgesia and allow a quicker recovery, which is quite important in the current concept of fast-track postoperative care. However, even for benign surgery with a safe anaesthetic technique, serious adverse events may occur. We report a case of acute renal failure related to urinary retention. Hidden by a prior history of urination difficulties, recognition of symptoms, following haemorrhoidectomy performed with bilateral pudendal block, was late. After complete recovery, electrophysiologic investigations found hypotonic, hypocontractile bladder.Therefore, before performing haemorrhoidectomy with regional anaesthesia, prior history of urination difficulties should be searched. The risk of urinary retention due to surgery and anaesthesia may be increased, as observed in this original case report.
Congenital total absence of the quadriceps muscle and patella: a case report with computed tomography scan and three-dimensional reconstructions

Abstract:In this study, we present three-dimensional computed tomography (CT) scan image findings of a case with congenital total absence of the quadriceps muscle and patella and partial absence of the sartorius muscle, which caused fixed flexion contracture of the knee joint. Physical examination, radiography, three-dimensional CT scan, and chromosome analysis of the patient were done. On physical examination, the knee joint was at 85 degree fixed flexion contracture. Radiographic views demonstrated soft tissue defects around left femur. Chromosome analysis was normal. CT scan showed total absence of the quadriceps femoris muscle and patella. Congenital absence of the extensor mechanism and patella are very rare anomalies. This case differed from other patellar aplasia or dysplasia syndromes with the presence of fixed flexion knee contracture and aplasia of the extensor mechanism.
[Influence of Different Sphincter-Preserving Surgeries on Postoperative Defecation Function].

Abstract:Advances in surgical techniques and treatment concept have allowed more patients with low rectal cancer to preserve sphincter without sacrificing survival benefit. However, postoperative dysfunctions such as fecal incontinence, frequency, urgency, and clustering often occur in patients with low rectal cancer. The main surgical procedures for low rectal cancer include low anterior rectum resection (LAR), intersphincteric resection (ISR), coloanal anastomosis (Parks) and so on. The incidence of major LARS after LAR is up to 84.6%. The postoperative function of ISR is even worse than LAR. Moreover, the greater the extent of resection ISR surgery, the worse the postoperative function. There are few studies on the function of Parks procedure. Current evidence suggests that the short-term function of Parks procedure is inferior to LAR, but function can gradually recovered over time. Colorectal surgeons have attempted to improve postoperative defecation by modifying bowel reconstructions. Current evidence suggests that J pouch or end-to-side anastomosis during LAR does not reduce the incidence of defecation disorders. Pouch reconstruction during ISR cannot reduce the incidence of severe LARS either. In general, the protection of postoperative defecation function in patients with low rectal cancer still has a long way to go.

Abstract:AimAcute urinary retention (AUR) is a well‐known complication after rectal surgery. It can be associated with additional morbidity. Causes of postoperative AUR are often multi‐factorial – involving patient, pathology and treatment related factors. A proportion of males undergoing TME have pre‐existing urinary dysfunction. This may predispose to AUR. This study prospectively assesses the influence of preoperative urinary function on postoperative AUR in males undergoing TME. MethodsA prospective multicenter cohort study was conducted. All adult males undergoing rectal resection between June 2016 and August 2017 were recruited. Combined pelvic resections, inability to void per urethra and emergency surgery were excluded. Preoperative urinary function was assessed with uroflowmetry, prostate ultrasound and International Prostate Symptom Score (IPSS). Incidence of postoperative AUR, urinary tract infection (UTI) and length of hospital stay (LOS) were measured. ResultsSeventy‐seven patients (mean age 61) were recruited. The overall incidence of AUR was 21%. Neither preoperative urinary function, IPSS, nor past urologic history was predictive for postoperative AUR. AUR was not associated with UTI and did not affect LOS. Patients with UTI had a higher intravesical protrusion of prostate. ConclusionsPreoperative urinary dysfunction in males is not predictive of postoperative AUR after TME. It should not preclude early TOV after TME. AUR did not predispose to UTI, nor did it prolong LOS.

Bethanechol Chloride for the Prevention of Postoperative Urinary Retention After Anal Surgery Under Spinal Anesthesia

Abstract:Background: Postoperative urinary retention is a commonly encountered problem after anal surgery particularly under spinal anesthesia. Bethanechol chloride, a muscarinic cholinergic receptor agonist was used to determine whether it could prevent this problem. Methods: One hundred six patients with mean age of 37 ± 9 who underwent anal surgery under spinal anesthesia from January to August 2007 were included in this nonblinded randomized prospective experimental study. Forty-six patients were given bethanechol chloride 25mg/tab 1 hour post-op then another dose after 4-6 hours. Intravenous fluid was limited to 8) were immediately catheterized and the amount drained recorded. Results: The study showed that the number of patients catheterized in the bethanechol group was significantly lower (6.52% vs 23.33%, P=0.038) and the mean amount of urine drained was less than the control (<400cc, P=0.001). Using Z test, the proportion of patients catheterized in the bethanechol group was significantly lower than the control (P=0.0097). No significant relationship was noted between the patient's age ( 50) and gender with regards to urinary retention (P=0.254 and P=0.187, respectively). The number needed to treat (NNT) showed that six patients treated with bethanechol are needed to prevent one patient from experiencing urinary retention. Conclusion: Although bethanechol chloride did not completely prevent the development of postoperative urinary retention, its use was associated with reduced need for catheterization. Abstract:Background Despite the high efficiency of surgical methods in correction of bladder outlet obstruction (BOO) caused by benign prostatic hyperplasia (BPH), a significant proportion of patients (up to 35%) remains after surgery with impaired contractile function of the bladder and lower urinary tract symptoms (LUTS). Both are consequences of structural and functional changes of the detrusor due to long-term exposure to obstructive factor. Today there is a lack of systematic reviews that provide a specialist with an integrative conception of pathological changes in the bladder wall caused by BPH associated prolonged BOO and evidence based methods of detrusor rehabilitation. The objective: systematization of modern conceptions about structural and functional changes in the bladder of patients with BPH caused by prolonged BOO, and methods of their correction. Materials and methods. An analytical review of the literature covering pathogenesis of bladder remodeling due to BPH associated long-term BOO and methods of bladder rehabilitation was performed. Literature sources were searched in PubMed, Google Scholar, Scopus and Web of Science databases by keywords. The search depth was 40 years. 74 relevant publications were selected for analysis. Results. Bladder remodeling due to obstructive factor is a complex staged process that involves all layers of the wall at the tissue, cellular and subcellular levels, affecting not only the executive structures (urothelium, smooth-muscle syncytium, fibrous connective tissue matrix), but also the system metabolism. Depending on the nature of the changes, there are 3 stages of this process: compensation, subcompensation and decompensation. In the compensatory stage, an increase of the load on the bladder leads to hypertrophy of smooth muscle fibers. At the same time, there is a reorganization of the vascular bed – neoangiogenesis, which should provide increased energy needs of muscles. In the stage of subcompensation, vicarious hypertrophy and neoangiogenesis cease. The most prominent disorders of the structure and function of the bladder are observed in the stage of decompensation. It includes a number of pathological processes: hypoxia, anaerobic metabolism, oxidative stress, inflammation, changes in the paracrine environment (increased levels of HIF-1α, vascular endothelial growth factor (VEGF) and angiopoietin-1). It is characterized by progressive loss of contractile function of the detrusor due to the death of smooth muscle cells and neurons, deterioration of viscoelastic characteristics of the bladder wall due to excessive collagen synthesis by fibroblasts, loss of barrier properties of the mucous membrane due to dystrophy. It was found that the severity of these pathological changes correlates with the severity of LUTS in patients who underwent surgery for BPH. The current arsenal of measures for bladder rehabilitation is quite diverse and includes periodic sterile catheterization, pharmacotherapy (cholinesterase inhibitors, antioxidants), physiotherapy (electrical stimulation, muscles training) and plastic surgery. However, there is still a lack of high-level studies to prove their effectiveness in patients who have undergone prostate surgery for BOO caused by BPH. Conclusions. Persistence of LUTS in patients who have undergone prostate surgery due to BOO may be caused by bladder decompensation. Long-term bladder decompensation due to prolonged exposure to obstructive factor is a complex process that includes decreased contractile activity, deterioration of the viscoelastic characteristics of the bladder, and impaired mucosal barrier function. Further research is needed to develop an effective bladder rehabilitation protocol.

Abstract:American Journal of Men's Health, Volume 18, Issue 3, May-June 2024. Benign anorectal diseases such as hemorrhoidal disease, anal fissure, anal pruritus, perianal abscess, and fistula are the most common ones. The aim of this study was to assess sexual function in patients after surgery for benign anorectal diseases. Sixty-one male patients with perianal fistulas, operated on at Department of General Surgery, Faculty of Medicine, completed a self-administered questionnaire including the International Index of Erectile Function (IIEF) score. The median IIEF score of the postoperative patients was significantly higher (24, range [10–25]) than that of preoperative patients (22, range [5–25]), p < .0001. Sexual function is significantly influenced by surgery for benign anorectal diseases.

Abstract:Low anterior resection syndrome (LARS) refers to a disturbance of bowel function that commonly manifests within 1 month after rectal cancer surgery. A low level of anastomosis and chemoradiotherapy have been consistently found to be risk factors for developing LARS. Thorough history taking and physical examination with adjunctive procedures are essential when evaluating patients with LARS. Anorectal manometry, fecoflowmetry, and validated questionnaires are important tools for assessing the quality of life of patients with LARS. Conservative management (medical, physiotherapy, transanal irrigation), invasive procedures (neuromodulation), and multimodal therapy are the mainstay of treatment for patients with LARS. A stoma could be considered when other treatment modalities have failed. An initial meticulous surgical procedure for rectal cancer, creation of a neorectal reservoir during anastomosis, and proper exercise of the anal sphincter muscle (Kegel's maneuver) are essential to combat LARS. Pretreatment counseling is a crucial step for patients who have risk factors for developing LARS.
