Abstract:H earing loss is a very common congenital disorder affecting one in 1000 newborns. More than 50% of deafness cases in the paediatric population have a genetic cause with autosomal dominant, autosomal recessive, X-linked, or mitochondrial patterns of inheritance. Mutations in mitochondrial DNA (mtDNA), particularly in the 12S rRNA and tRNA genes, have been found to be one of the most important causes of sensorineural hearing loss (SNHL). 3 The homoplasmic A1555G mutation in the highly conserved decoding site of the mitochondrial 12S rRNA has been found to be associated with both aminoglycoside-induced and non-syndromic SNHL in many families of different ethnic origins. Recently, the homoplasmic C1494T mutation in the same gene has also been found to be associated with aminoglycoside-induced and non-syndromic SNHL in a large Chinese family. In addition, a C-insertion or deletion at position 961 of the 12S rRNA gene has been shown to be associated only with aminoglycoside-induced deafness. 11 Furthermore, the mitochondrial tRNA appears to be another hot spot for mutations associated with hearing impairment, as five deafness-associated mutations have been identified in the mitochondrial tRNA gene: A7445G, 13 7472insC, T7510C, T7511C, and T7512C. However, non-syndromic deafness-associated mtDNA mutations, such as the A1555G or A7445G mutation, are often not sufficient to produce the clinical phenotype since some individuals carrying these mutations have normal hearing. Thus, other factors including other mtDNA mutations/polymorphisms and/or nuclear backgrounds or environmental factors modulate the phenotypic variability and penetrance of deafness associated with these mtDNA mutations. However, little is known about the incidence of these deafness-associated mtDNA mutations. Furthermore, it is anticipated that additional deafness-associated mutations will be found in the 12S rRNA and tRNA genes. A retrospective database review of all children with SNHL and subsequent molecular analysis has been initiated in the clinical population at the Center for Hearing and Deafness Research (CHDR) at the Cincinnati Children’s Hospital Medical Center (CCHMC). In the present study, a mutational analysis of the 12S rRNA and tRNA genes was performed in 164 SNHL subjects from this clinic and 226 unaffected individuals from a comparable ethnic background. This analysis led to the identification of 17 nucleotide changes including the novel G to T transversion at position 961 (T961G) in the 12S rRNA gene in five affected subjects. To elucidate the molecular basis for the T961G mutation, we completed the mitochondrial genome sequence from these five affected subjects. To examine the role of the GJB2 gene in the phenotypic expression of the known or putative deafness-associated mtDNA mutations, we also performed mutational screening of the GJB2 gene in subjects carrying the mtDNA mutations.
Prevalence of Common Genetic Mutations and Clinical Characteristics Analysis in Patients at Different Ages with Nonsyndromic Hearing Impairment

Abstract:To evaluate the correlation between genetic mutations and the age in nonsyndromic hearing impairment (NSHI) and the clinical characteristics of NSHI, 215 patients with NSHI were enrolled between April 2006 and April 2012. All patients were divided into four groups according to ages of hearing loss onset and clinic presentation (0-3, 3-6, 6-18 and 18+ years). The mutations of GJB2 and mitochondria DNA (mtDNA) 1555G/C1494T were screened from peripheral blood samples in each age group. The prevalence of mutations and the age ratio were obtained. The study showed that 18.14% of all patients were found to have GJB2 mutations and 11.16% were found to have mtDNA A1555G/C1494T mutations. The prevalence of GJB2 mutation in adult group (5.26%) was lower than juvenile group who sought medical attention at 0-18 years of age (22.36%), while the prevalence of mtDNA A1555G/C1494T in adult group (31.48%) was higher than juvenile group (4.97%). Significant differences in the prevalence of GJB2 (χ2=7.108, P=0.008) and mtDNA A1555G/C1494T (χ2=20.852, P=0.000) were observed in both of two groups. The prevalence of GJB2 mutations between adult and juvenile groups according to ages of hearing loss onset was statistically significant different (0%, 20.10%, respectively, and P=0.023), while the prevalence of mtDNA A1555G/C1494T mutations was not different (14.29%, 11.34%, respectively, and P=0.698). The onset age of 66.67% of patients with GJB2 mutations was less than 1 year old, while the onset of patients with mtDNA A1555G/C1494T mutations could be found at any age group. Different standardizations of hearing loss could also show different results. These data strongly suggest that most of GJB2 mutations are found in congenital deafness and mtDNA A1555G/C1494T mutations mainly represent acquired deafness, which can be induced or aggravated by aminoglycoside antibiotics in all age groups and should be tested mainly ranging from 4 kHz to 8 kHz. Both newborn hearing screening and genetic testing are important to find early deafness.

Abstract:Recent progress in gene identification for non-syndromic deafness demonstrates the efficacy of using a large family approach. To assess if clinical and genetic heterogeneity exists in families with non-syndromic hearing impairment we have identified five large Chinese families suitable for investigating clinical and genetic heterogeneity. Five Chinese families spanning 5–6 generations and comprising 48 to 172 members were identified. The auditory phenotype was investigated with pure tone audiometry and DPOAEs. Age at onset of hearing loss was compared between generations by using life-table analysis and the Wilcoxon test in a family. All statistical analyses were performed using the SAS statistical package. Screening for GJB2 and mitochondrial mutations was performed. Two families showing maternal inheritance were found to have the mitDNA A1555G mutation. Subjects with the A1555G mutation exhibit a normal to profound, progressive deafness. Anticipation of the hearing loss was observed in one of these two families. The other three families are segregating for a late onset form of progressive hearing loss with normal vestibular function. DPOAEs from a group of possible gene carriers with normal audiograms in two families have shown a substantial difference from that of controls. Our data show the existence of clinical and genetic heterogeneity in the five large Chinese deaf families. Marked differences in severity of hearing loss, along with a lack of exposure to aminoglycosides and the presence of anticipation in the mitDNA deafness suggest the presence of a modifier. DPOAEs may be a sensitive test for a subclinical change in carriers. Identification of gene(s) for deafness should allow us to better understand the molecular basis of deafness and to improve patient care.
Resolving the genetic heterogeneity of prelingual hearing loss within one family: Performance comparison and application of two targeted next generation sequencing approaches

Abstract:Here, we report an unconventional Chinese pedigree consisting of three branches all segregating prelingual hearing loss (HL) with unclear inheritance pattern. After identifying the cause of one branch as maternally inherited aminoglycoside-induced HL, targeted next generation sequencing (NGS) was applied to identify the genetic causes for the other two branches. One affected subject from each branch was subject to targeted NGS whose genomic DNA was enriched either by whole-exome capture (Agilent SureSelect All Exon 50 Mb) or by candidate genes capture (Agilent SureSelect custom kit). By NGS analysis, we identified that patients from Branch A were compound heterozygous for p.E1006K and p.D1663V in the CDH23 (DFNB12) gene; and patients from Branch B were homozygous for IVS7-2A>G in the SLC26A4 (DFNB4) gene. Both CDH23 mutations altered conserved calcium binding sites of the extracellular cadherin domains. The co-occurrence of three different genetic causes in this family was exceedingly rare but fully compatible with the mutation spectrum of HL. Our study has also raised several technical and analytical issues when applying the NGS technique to genetic testing.

Abstract:A 13-item Self-critical Cognition scale was developed to measure a self-critical and self-defeating cognitive tendency in processing self-relevant information. The scale, administered to 561 male and female university students, evidenced high internal consistency (α = .89) and test-retest reliability of r138 = .83 over a 6.5-week interval. A factor analysis yielded a most interpretable 2-factor solution, Factor 1: negative self-processing and Factor 2: failure in positive self-processing. The scale's construct validity was supported by meaningful correlations of –.71 with Rosenberg's self-esteem, .43 with Watson and Friend's social anxiety and distress, .62 with Cheek and Buss's shyness, .57 with Watson and Friend's fear of negative evaluation, .42 with Beck and Beamesderfer's depression, and .34 with negative adjective counts in spontaneous adjective listing in a self-descriptive task. Use of the scale for experimental and clinical research is suggested.

Abstract:This study assessed the prevalence of genetic variants associated with hearing loss in a large cohort of children. A total of 485 children underwent genetic testing, which identified 923 variants, most of which were of uncertain significance. A genetic cause of hearing loss was found in 15% of children which is lower than typically reported. Objective The objective of this study was to assess the prevalence of genetic variants associated with hearing loss in a large cohort of children in Canada using high throughput next generation sequencing (NGS). Methods A total of 485 children with hearing loss underwent NGS testing with an 80 gene panel of syndromic and non‐syndromic variants known to be associated with hearing loss. Genetic variants were classified as pathogenic, likely pathogenic, likely benign, benign, or variants of uncertain significance (VUS), according to the American College of Medical Genetics and Genomics guidelines. Results Across the 80 genes tested, 923 variants, predominantly in 28 genes, were identified in 324 children. Pathogenic variants occurred in 19/80 (23.8%) of the hearing loss related genes tested and confirmed the etiology of hearing loss in 73/485 (15.1%) of children. GJB2 was the most prevalent gene, affecting 28/73 (38.4%) children with confirmed genetic hearing loss in our cohort. Most identified variants (748/923, 81.0%, in 76/80 genes) were of uncertain significance. Conclusion Genetic testing using NGS identified the etiology in approximately 15% of childhood hearing loss in a Canadian cohort which is lower than what is typically reported. GJB2 was the most common genetic cause of hearing loss. VUS are commonly identified, presenting clinical challenges for counseling. Level of Evidence Level 4 Laryngoscope, 2024

Abstract:Purpose: Uncorrected hearing loss can result in detrimental sequelae. Research addressing clinical presentation and genetic testing would inform clinical decision making. Method: A retrospective chart review of 96 patients aged 1 month to 46 years (median age = 6 years) diagnosed with hearing loss or deafness and who underwent genetic testing at University of Rochester Medical Center from 2011 to 2021. Chi-square and Fisher's exact tests examined the relationship between a diagnostic positive genetic test result and various characteristics of hearing loss, including congenital ( n = 52), noncongenital ( n = 34), prelingual ( n = 53), postlingual ( n = 33), progressive ( n = 13), not progressive ( n = 47), bilateral ( n = 67), unilateral ( n = 26), sensorineural ( n = 68), conductive ( n = 14), mixed ( n = 5), syndromic ( n = 10), and nonsyndromic ( n = 87) hearing loss. We also examined the number of patients with presence of developmental disabilities ( n = 35), having a first-degree relative with hearing loss ( n = 19), having hearing aids or cochlear implants ( n = 45), and having a multisystem presentation prior to diagnosis ( n = 45). Results: Patients with sensorineural hearing loss (44.1%) had significantly more diagnostic positive results than those with mixed (0%) or conductive hearing loss (21.4%), p = .004. However, significantly fewer patients with disabilities (19.4%) had diagnostic positive tests than those without disabilities (43.3%), p < .05. More patients with a multisystem presentation were also found to have syndromic causes of hearing loss (23.3%) than patients who did not have a multisystem presentation, p < .05. Conclusions: Our study suggests a significant association between sensorineural type of hearing loss and a diagnostic positive genetic test result, while the presence of disabilities was significantly associated with a nondiagnostic genetic test result. Knowledge of these findings is critical for understanding the cause of the hearing loss, identifying other associated symptoms, and determining risk to family members.

Abstract:Introduction: Hearing loss is a common sensory disorder that impacts patients across the lifespan. Many genetic variants have been identified that contribute to non-syndromic hearing loss. Yet, genetic testing is not routinely administered when hearing loss is diagnosed, particularly in adults. In this study, genetic testing was completed in patients with known hearing loss. Methods: 104 patients who were evaluated for hearing loss were enrolled and received genetic testing. Results: Of those 104 patients, 39 had available genetic testing, 20 had one missing allele, and 45 yielded no genetic diagnosis. Of the 39 cases with genetic testing data, 24 were simplex cases and 15 were multiplex cases. A majority of patients presented with an autosomal recessive inheritance pattern (n=32), 26 of whom presented with congenital hearing loss. 38% of cases were positive for GJB2 mutation with c.35delG being the most common pathogenic variant. These findings are consistent with previous literature suggesting GJB2 mutations are the most common causes of non-syndromic hearing loss. Conclusion: Given the frequency of genetic variants in patients with hearing loss, genetic testing should be considered as a routine part of the hearing loss work-up, particularly as gene therapies are studied and become more widely available.

Abstract:【Objective】To identify the genetic causes for hearing loss in children and present a genetic counseling. 【Methods】Thirty-one children were performed audiological examination before genetic testing.Blood samples were obtained and DNA was extracted from the leukocytes.The coding region of GJB2 gene,SLC26A4 and mitochondrial DNA target fragments were amplified by polymerase chain reaction(PCR).Mutations in GJB2 gene,SLC26A4IVS7-2A>G,mtDNA 1494C>T and mtDNA1555A>G were identified by sequencing analysis. 【Results】The carrier frequency of mutations associated with hearing loss in children was 32%(10/31).of 31 patients,7 had GJB2 mutations,2 had SLC26A4IVS7-2A>G mutation and 1 had mtDNA1555A>G mutation. 【Conclusions】Detection of pathogenic mutations for hearing loss is bringing the possibility to identify children with non-syndromic deafness at an early stage.As a consequence,it will improve the current diagnosis and therapeutical option.

Abstract:Objective:To analyze genetic factors and phenotype characteristics in pediatric population with slight-to-moderate sensorineural hearing loss. Methods:Children with slight-to-moderate sensorineural hearing loss of and their parents, enrolled from the Chinese Deafness Genome Project, were studied. Hearing levels were assessed using pure tone audiometry, behavioral audiometry, auditory steady state response（ASSR）, auditory brainstem response（ABR） thresholds, and deformed partial otoacoustic emission（DPOAE）. Classification of hearing loss is according to the 2022 American College of Medical Genetics and Genomics（ACMG） Clinical Practice Guidelines for Hearing Loss. Whole exome sequencing（WES） and deafness gene Panel testing were performed on peripheral venous blood from probands and validations were performed on their parents by Sanger sequencing. Results:All 134 patients had childhood onset, exhibiting bilateral symmetrical slight-to-moderate sensorineural hearing loss, as indicated by audiological examinations. Of the 134 patients, 29（21.6%） had a family history of hearing loss, and the rest were sporadic patients. Genetic causative genes were identified in 66（49.3%） patients. A total of 11 causative genes were detected, of which GJB2 was causative in 34 cases（51.5%）, STRC in 10 cases（15.1%）, MPZL2 gene in six cases（9.1%）, and USH2A in five cases（7.6%）.The most common gene detected in slight-to-moderate hearing loss was GJB2, with c. 109G>A homozygous mutation found in 16 cases（47.1%） and c. 109G>A compound heterozygous mutation in 9 cases（26.5%）. Conclusion:This study provides a crucial genetic theory reference for early screening and detection of mild to moderate hearing loss in children, highlighting the predominance of recessive inheritance and the significance of gene like GJB2, STRC, MPZL2, USH2A.

Abstract:Abstract Hearing loss is one of the most common birth disorders in humans, with an estimated prevalence of 1–3 in every 1000 newborns. This study investigates the molecular etiology of a hearing loss cohort using a stepwise strategy to effectively diagnose patients and address the challenges posed by the genetic heterogeneity and variable mutation spectrum of hearing loss. In order to target known pathogenic variants, multiplex PCR plus next-generation sequencing was applied in the first step; patients which did not receive a diagnosis from this were further referred for exome sequencing. A total of 92 unrelated patients with nonsyndromic hearing loss were enrolled in the study. In total, 64% (59/92) of the patients were molecularly diagnosed, 44 of them in the first step by multiplex PCR plus sequencing. Exome sequencing resulted in eleven diagnoses (23%, 11/48) and four probable diagnoses (8%, 4/48) among the 48 patients who were not diagnosed in the first step. The rate of secondary findings from exome sequencing in our cohort was 3% (2/58). This research presents a molecular diagnosis spectrum of 92 non-syndromic hearing loss patients and demonstrates the benefits of using a stepwise diagnostic approach in the genetic testing of nonsyndromic hearing loss.

Molecular Etiology of Hearing Impairment in Inner Mongolia: Mutations in SLC26A4 Gene and Relevant Phenotype Analysis

Abstract:Background: The molecular etiology of hearing impairment in Chinese has not been thoroughly investigated. Study of GJB2 gene revealed that 30.4% of the patients with hearing loss in Inner Mongolia carried GJB2 mutations. The SLC26A4 gene mutations and relevant phenotype are analyzed in this study.Methods: One hundred and thirty-five deaf patients were included. The coding exons of SLC26A4 gene were sequence analyzed in 111 patients, not including 22 patients carrying bi-allelic GJB2 mutations or one patient carrying a known GJB2 dominant mutation as well as one patient with mtDNA 1555A>G mutation. All patients with SLC26A4 mutations or variants were subjected to high resolution temporal bone CT scan and those with confirmed enlarged vestibular aqueduct and/or other inner ear malformation were then given further ultrasound scan of thyroid and thyroid hormone assays.Results: Twenty-six patients (19.26%, 26/135) were found carrying SLC26A4 mutation. Among them, 17 patients with bi-allelic SLC26A4 mutations were all confirmed to have EVA or other inner ear malformation by CT scan. Nine patients were heterozygous for one SLC26A4 mutation, including 3 confirmed to be EVA or EVA and Mondini dysplasia by CT scan. The most common mutation, IVS7-2A>G, accounted for 58.14% (25/43) of all SLC26A4 mutant alleles. The shape and function of thyroid were confirmed to be normal by thyroid ultrasound scan and thyroid hormone assays in 19 of the 20 patients with EVA or other inner ear malformation except one who had cystoid change in the right side of thyroid. No Pendred syndrome was diagnosed.Conclusion: In Inner Mongolia, China, mutations in SLC26A4 gene account for about 12.6% (17/135) of the patients with hearing loss. Together with GJB2 (23/135), SLC26A4 are the two most commonly mutated genes causing deafness in this region. Pendred syndrome is not detected in this deaf population. We established a new strategy that detects SLC26A4 mutations prior to the temporal bone CT scan to find EVA and inner ear malformation patients. This model has a unique advantage in epidemiologic study of large deaf population.
The congenital hearing phenotype in GJB2 in Queensland, Australia: V37I and mild hearing loss predominates

Abstract:GJB2 was originally identified in severe, non-syndromic sensorineural hearing loss (SNHL), but was subsequently associated with mild and moderate SNHL. Given the increasing utilisation of genetic testing pre-conceptually, prenatally, and neonatally, it is crucial to understand genotype-phenotype correlations. This study evaluated the nature and frequency of GJB 2 variants in an Australian paediatric population with varying degrees of SNHL ascertained through newborn hearing screening. Audiograms from individuals with GJB2 variants and/or a GJB6 deletion ( GJB6 -D13S11830) were retrospectively reviewed ( n = 127). Two-thirds were biallelic (homozygous/compound heterozygous) for pathogenic/likely pathogenic variants of GJB2 and/or GJB6 ( n = 80). The most frequent variant was c.109 G > A, followed by c.35delG and c.101 T > C. Compared to biallelic carriage of other GJB2 variants, c.109 G > A positive individuals (homozygous/compound heterozygous) were more likely to have mild HL at their initial and latest audiograms ( p = 0.0004). Biallelic carriage of c.35delG was associated with moderately-severe or greater SNHL at both initial and latest audiograms ( p = 0.007). The c.101 T > C variant presented with milder SNHL and U-shaped audiograms ( p = 0.02). In this agnostically identified cohort, mild SNHL predominated in GJB2/GJB6 carriers in contrast to previous studies targeting individuals with significant loss. Consequently, c.109 G > A, associated with milder phenotypes, was the most frequent. This study provides valuable data to support prognostic confidence in genetic counselling.

Outcomes of Children with Hearing Loss: Data Collection and Methods

Abstract:Objectives: The primary objective of this article was to describe recruitment, data collection, and methods for a longitudinal, multicenter study involving children with bilateral mild to severe hearing loss. The goals of this research program were to characterize the developmental outcomes of children with mild to severe bilateral hearing loss during infancy and the preschool years. Furthermore, the researchers examined how these outcomes were associated with the child's hearing loss and how home background and clinical interventions mediated and moderated these outcomes. Design: The participants in this study were children who are hard of hearing (CHH) and children with normal hearing (CNH) who provided comparison data. CHH were eligible for participation if (1) their chronological age was between 6 months and 7 years of age at the time of recruitment, (2) they had a better-ear pure-tone average of 25 to 75 dB HL, (3) they had not received a cochlear implant, (4) they were from homes where English was the primary language, and (5) they did not demonstrate significant cognitive or motor delays. Across the time span of recruitment, 430 parents of potential children with hearing loss made contact with the research group. This resulted in 317 CHH who qualified for enrollment. In addition, 117 CNH qualified for enrollment. An accelerated longitudinal design was used, in which multiple age cohorts were followed long enough to provide overlap. Specifically, children were recruited and enrolled continuously across an age span of 6.5 years and were followed for at least 3 years. This design allowed for tests of time (period) versus cohort age effects that could arise by changes in services and technology over time, yet still allowed for examination of important developmental relationships. Results: The distribution of degree of hearing loss for the CHH showed that the majority of CHH had moderate or moderate-to-severe hearing losses, indicating that the sample undersampled children with mild HL. For mothers of both CHH and CNH, the distribution of maternal education level showed that few mothers lacked at least a high school education and a slight majority had completed a bachelor's degree, suggesting that this sample of research volunteers was more advantaged than the United States population. The test battery consisted of a variety of measures concerning participants' hearing and behavioral development. These data were gathered in sessions during which the child was examined by an audiologist and a speech-language examiner. In addition, questionnaires concerning the child's behavior and development were completed by the parents. Conclusion: The Outcomes of Children with Hearing Loss study was intended to examine the relationship between variation in hearing ability across children with normal and mild to severe hearing loss and variation in their outcomes across several domains of development. In addition, the research team sought to document important mediators and moderators that act between the hearing loss and the outcomes. Because the study design provided for the examination of outcomes throughout infancy and early childhood, it was necessary to employ a number of different measures of the same construct to accommodate changes in developmental performance across age. This resulted in a large matrix of measures across variable types and developmental levels, as described in this manuscript.

Abstract:Stably transfected human kidney 293 cells expressing the wild type rat LH/CG receptor (rLHR) or receptors with C-terminal tails truncated at residues 653, 631, or 628 (designated rLHR-t653, rLHR-t631, and rLHR-t628) were used to probe the importance of this region on the regulation of hormonal responsiveness. The chosen cells line express comparable densities of cell surface rLHR, bind human CG (hCG) with high affinity, and respond to hCG with increases in cAMP and inositol phosphate accumulation. Cells expressing rLHR-wt or rLHR-t653 responded to hCG, or phorbol 12-myristate-13-acetate (PMA) stimulation with a similar increase in rLHR phosphorylation. Neither of these two stimuli increased rLHR phosphorylation in cells expressing rLHR-t631 or rLHR-t628, however. The cell line expressing rLHR-t653, the phosphorylation-positive receptor mutant, desensitized normally in response to PMA or hCG stimulation. This truncated form of rLHR also was down-regulated normally in response to hCG stimulation. In contrast, the cell lines expressing rLHR-t631 or rLHR-t628, the two phosphorylation-negative receptor mutants, showed a delay in the early phase of hCG-induced desensitization, a complete loss of PMA-induced desensitization, and an increase in the rate of hCG-induced receptor down-regulation. These results clearly show that residues 632-653 in the C-terminal tail of the rLHR are involved in PMA-induced desensitization, hCG-induced de-sensitization, and hCG-induced down-regulation. These results also establish a positive correlation between rLHR phosphorylation and desensitization and a negative correlation between rLHR phosphorylation and down-regulation.
Prognosis of Perceptive Hearing Loss in Children with Respect to Genesis and Use of Hearing Aid

Abstract:Eighty-four children with bilateral perceptive hearing loss have been followed up during a period of up to 15 years from the first reliable hearing test. The material has been grouped according to the probable causes of hearing defect. The cases with exogenous (acquired) hearing loss include three different groups: (A) Maternal rubella; (B) Perinatal accident; (C) Meningitis with or without dihydrostreptomycin treatment. The other main group includes cases in which the hearing loss is assumed to be of endogenous (hereditary) genesis. During the time of observation the majority of children had been using hearing aids regularly. None of the children with hearing impairment due to exogenous factors caused by maternal rubella or acquired perinatally showed any signs of progressivity despite the constant use of a hearing aid. All cases of hearing impairments caused by meningitis with or without dihydrostreptomycin treatment, however, showed progressivity of the hearing impairment. Whether the use of a hearing aid contributes to this progressivity or whether it is spontaneous cannot be definitely established. Spontaneous progressivity seems most likely however. More than half of the reported hereditary cases with hearing impairments showed spontaneous progressivity. This progressivity had been characterized by a certain irregularity. During long periods of time the hearing impairment seems to be stationary, but suddenly progresses rapidly. The progress can either be bilaterally symmetric or asymmetric. Progressivity is probably not due to the use of a hearing aid, but spontaneous and due to hereditary factors.
