Abstract:Cortisol is commonly used as a significant biomarker of psychological or physical stress. With the accelerated pace of life, non-invasive cortisol detection at the point of care (POC) is in high demand for personal health monitoring. In this paper, an ultrasensitive immunosensor using gold nanoparticles/molybdenum disulfide/gold nanoparticles (AuNPs/MoS2/AuNPs) as transducer was explored for non-invasive salivary cortisol monitoring at POC with the miniaturized differential pulse voltammetry (DPV) system based on a smartphone. Covalent binding of cortisol antibody (CORT-Ab) onto the AuNPs/MoS2/AuNPs transducer was achieved through the self-assembled monolayer of specially designed polyethylene glycol (PEG, SH-PEG-COOH). Non-specific binding was avoided by passivating the surface with ethanolamine. The miniaturized portable DPV system was utilized for human salivary cortisol detection. A series current response of different cortisol concentrations decreased and exhibited a linear range of 0.5–200 nM, the detection limit of 0.11 nM, and high sensitivity of 30 μA M−1 with a regression coefficient of 0.9947. Cortisol was also distinguished successfully from the other substances in saliva. The recovery ratio of spiked human salivary cortisol and the variation of salivary cortisol level during one day indicated the practicability of the immunosensor based on the portable system. The results demonstrated the excellent performance of the smartphone-based immunosensor system and its great potential application for non-invasive human salivary cortisol detection at POC.

Abstract:PURPOSE: Cortisol is frequently assayed as a stress-responsive biomarker which changes over the course of minutes to meet the demands of a person's social context. Salivary cortisol is often used as a noninvasive sampling method that possesses important health implications. A critical barrier to psychobiological research that involves salivary cortisol is a time delay of days to months before cortisol results are obtained via immunoassay, long after the person is no longer proximate to the social context in which they provided the sample. The present study was designed to address this critical barrier through creation of a lateral flow test (LFT) cortisol device capable of measuring salivary cortisol within minutes of sample collection. The LFT is frequently used within commercial point-of-care settings to obtain rapid answers to the presence/absence of a biomarker. The present study extends the LFT into the research domain by presenting performance characteristics of a quantitative LFT that measures salivary cortisol within 20 minutes of sample collection.METHODS: Saliva samples from 29 adults (15 men) were obtained in the morning and afternoon by using Passive Drool and then the Super·SAL Extra Collection Device (hereafter Super·SAL) and later assayed with LFT and a commercially available enzyme immunoassay.FINDINGS: Results indicate the LFT correlated well with these collection methods (R = 0.872 with Super · SAL, R = 0.739 with Passive Drool, P < 0.0001) and at comparable levels to correspondence of Super · SAL with Passive Drool (R = 0.798, P < 0.0001) which were measured with the same assay.IMPLICATIONS: These results open an exciting new possibility to integrate this technologic advance into stress research, including knowing and potentially changing the person's social context in a time-sensitive manner. Methodological improvements such as this have the possibility of refining conceptual models of stress reactivity and regulation.

Feasibility of salivary cortisol collection in patients and companions attending dementia diagnostic meetings in memory clinics

Abstract:Abstract Objectives Receiving a diagnosis of dementia is life-changing for the individual and their companion. The aim of the study was to explore the feasibility of collecting salivary cortisol from patients who are informed if they have dementia and their companions. Patients and companions collected nine saliva samples in three batches: 1–2 weeks before, immediately before, and immediately after the diagnostic meeting. Each batch consisted of three samples taken in the evening, after awaking and 30 mins post-waking. Results 22.7% (N = 10) of 44 invited patients and nine companions agreed, with 18.2% patients (N = 8) and 15.9% companions (N = 7) providing samples. Participants found that saliva collection was demanding and disrupted routines. On a purely descriptive level, some indications of an increased cortisol stress response in patients diagnosed with dementia were found in this very small sample. Researchers should expect low recruitment rates in this elderly population. Simpler collection procedures, e.g. pre-labelled packages with date/time, possible omission of morning samples and objective rather than self-report assessment of waking and saliva collection times—using actigraphy wrist-watches bleeps to prompt people at the timepoints and electronic track caps—might improve adherence and improve the accuracy of timepoints when swabs were actually collected.

Abstract:Abstract Introduction The dim light melatonin onset (DLMO) is the gold-standard for estimating sleep timing and is recognized as a reliable diagnostic tool for circadian rhythm sleep-wake disorders (CRSWD). Its assessment may help optimize treatment of various sleep disorders. The DLMO is usually assessed in a research laboratory or clinic, which presents significant barriers. Thus, there is a need to further evaluate the ecological validity of self-directed collection at home. Salimetrics, Inc. recently introduced a circadian phase assessment kit that allows for self-collection of saliva at home accompanied by laboratory assessed DLMOs. This is a preliminary report of a limited dataset identifying normal (typical) and abnormal (atypical) melatonin profiles. Methods Sleep clinic patients (N = 197) utilized one of four different protocols that varied in the number and timing of saliva collection based on their provider’s suggestion. Despite different protocols, the actual sampling protocol was performed under the same guidance for all assessments. A typical profile was determined as an expected onset (estimated ~2.5-1.5 hours before bedtime) with a baseline of < 10 pg/ml and a sleep onset of >10 pg/ml. An atypical profile fell outside these parameters. Routine medication usage was also analyzed to determine if there was a correlation between melatonin modulating medications and resulting profiles. Only sex and age were recorded as the participants collected samples as part of the clinical care protocol. Results Findings indicate 22% typical profiles vs. 78% atypical profiles. With regards to medication use: 26% of typical profiles vs 19% of atypical profiles had no medications listed, 53% vs 68% used a melatonin modulator, respectively, 14% vs 54% used a psychiatric medication, 21% vs 25% used 2+ psychiatric medications, and 9% vs 14% used a melatonin supplement within 10 days of assessment. Conclusion These data corroborate earlier investigations by Burgess and colleagues and suggest that the use of a self-directed, at-home DLMO assessment is feasible and accurate. It further suggests that medication use, or an underlying psychiatric disorder can modulate melatonin secretion exaggerating the CRSWD. Data indicates that this diagnostic tool can accurately identify many clinical sleep patients with an existing CRSWD or significant sleep wake misalignment. Support (if any) Salimetrics

Abstract:Abstract: Breast cancer is one of the leading causes of death from cancer among women in western countries. The different types of breast cancer are grouped into invasive and noninvasive forms. Among the invasive types, ductal infiltrating carcinoma (DIC) is the most common and aggressive form. Using an in vitro model consisting of a DIC‐derived cell line (8701‐BC) and a nontumoral mammary epithelial cell line (HB2), we used the proteomics approach to search for homology and differences in protein expression patterns between tumoral and nontumoral phenotypes. Within an analysis window comprising 1,750 discernible spots we have currently catalogued 140 protein spots of potential interest. Fifty‐eight of them were identified by gel matching with reference maps, immunodetection, or N‐terminal microsequencing and classified into four functional groups. Twelve proteins were found differentially expressed in two cell lines: four were uniquely present in the neoplastic cell proteome and eight in epithelial cells. In addition, 53 proteins displayed different relative expression levels between the two cell lines, that is, 44 were more elevated in cancer cells and 9 in HB2 cells. Among proteins with greater relative abundance in cancer cells we identified glycolytic enzymes (or their isoforms), which may indicate that the known metabolic dysregulation in cancer can reflect oncogenic‐related defects of glycolytic gene expression.

Abstract:Increasing the sampling resolution when examining plasma hormone and metabolite profiles will more accurately describe diurnal/circadian rhythms and expose previously undiscovered ultradian rhythms that underpin metabolic physiology (1) . In human research studies this must be tempered by the practical, ethical and safety concerns of repeated cannulation/blood draws. Here we examine the correlation between hormone and metabolite profiles present in interstitial fluid microdialysate (2) , and plasma to help solve this dilemma. Hormonal markers of circadian phase (melatonin, cortisol) and metabolites will show similar profiles in plasma and interstitial fluid. Compare time-series hormone and metabolite profiles collected in interstitial fluid using a novel ambulatory microdialysis collection device (U-RHYTHM) with simultaneously drawn plasma samples. All study protocols were reviewed by Health Sciences Faculty Research Ethics Committee, University of Bristol. Fasted healthy male volunteers aged 18-35 (n=3) were fed a standardised breakfast (08:00), lunch (13:00), dinner (19:00) and snack (22:00) (2225kCal [83g protein, 273g carbohydrate, 83g fat, 27g fibre]). Participants remained on the study bed, lights off/sleep occurred between 23:00-07:00 (<4 lux). A 20kDa cutoff 30 mm linear microdialysis membrane was placed in periumbilical subcutaneous tissue, perpendicular to the midline. The membrane was perfused at 1μl/min and the microdialysate collected every 20 minutes for 25 hours into discrete samples using a portable fraction collector (URHYTHM) worn around the waist. Time-matched blood samples were obtained from an antecubital fossa canula. All samples were stored at -80°C prior to analysis. Hormones and metabolites were measured via targeted UPLC-MS/MS metabolomics, for the metabolites the AbsoluteIDQ® p180 kit (Biocrates Life Sciences AG)) was used with 10μL of plasma (3) and 15μL of microdialysate. Simultaneous time-courses of n = 24 metabolites were detected in plasma and interstitial microdialysate: alanine, arginine, asparagine, citrulline, glutamine, glutamate, glycine, histidine, isoleucine, leucine, lysine, methionine, ornithine, phenylalanine, proline, serine, threonine, tryptophan, tyrosine, valine, trans-4-Hydroxyproline, taurine, carnitine and acetylcarnitine. Spearman rank cross correlation identified the time shift that was required to reach highest correlation between the plasma and microdialysate profiles. Excluding taurine, microdialysate profiles were delayed by 32.3 minutes (range 0-130) and 12.9 minutes (range 0-50) for participant 1 and 2, respectively (p <0.05), only 3/20 metabolite profiles were correlated for participant 3. Trans4-hydroxyproline had the strongest correlation (r s 0.890, 0.814 and 0.808), with only participant 3 displaying a time shift (50 minutes). Taurine had the weakest correlation in all 3 participants. (r s 0.096, 0.182, 0.325). These data demonstrate that our novel approach can be used to comprehensively detect hormone and metabolite profiles in interstitial fluid. Thus, the U-RHYTHM would serve as a useful tool, not only to increase sampling resolution in controlled laboratory studies but would prove particularly useful for examining circadian/ultradian profiles for chronobiology and nutrition studies in free-living individuals.

Abstract:Information on the relationships between the previous night's sleep and the next-day diurnal cortisol rhythm is inconsistent due to confounding factors such as sleep measurements (trait/state sleep and objective/subjective sleep) and cortisol sampling schemes. Therefore, this study aimed to investigate these relationships, considering the confounding factors. College students (n=79) wore actigraphy for 3 days to undergo an evaluation of previous night-time sleep objectively and reported their subjective sleep parameters in a sleep diary. In addition, participants provided six salivary cortisol samples daily. Furthermore, six cortisol sampling schemes were created to reflect diurnal cortisol rhythms, and two different methods were used to calculate the index of diurnal cortisol slope (DCS). A multilevel model was created to examine the impact of both trait and state sleep on next-day diurnal cortisol rhythm. The results revealed that higher objective state sleep efficiency and longer objective state total sleep time were associated with a higher cortisol awakening response (CAR). Moreover, higher objective trait sleep efficiency and longer objective trait total sleep time were associated with higher waking cortisol levels and steeper DCS. In addition, a minimum of four saliva samples were required at different time points, including upon waking, 30 min after waking, 1 h after waking, and at bedtime, to explore the relationship of sleep efficiency/total sleep time with waking cortisol, CAR, and DCS. Furthermore, the index of the peak-to-bed slope was appropriately employed to examine the relationship between sleep efficiency and DCS, whereas the wake-to-bed slope was effective for examining the relationship between total sleep time and DCS. In summary, this study clarified the relationship between sleep and next-day diurnal cortisol rhythm and suggested a cost-effective cortisol sampling schedule and calculation methods.

Abstract:Collection of salivary cortisol has been widely used as a method of investigating an array of health parameters. Monitoring of cortisol levels can help us to understand stress levels and the body’s response to stressors. Traditional methods of measuring cortisol in saliva, however, require costly equipment, trained personnel, and transportation of samples to a centralized laboratory. This creates a barrier to personal monitoring of cortisol. It also adds a level of cost and difficulty to large-scale studies which require participants to store and ship their saliva samples. Here, we present a novel system in which an individual with minimal training may collect their own saliva sample and measure it at home. Our system utilizes a lateral flow assay, a portable imaging device, and a smartphone to give salivary cortisol results in less than 15 min. We also demonstrate the use of our system on samples from a human study and give results from that study, which analyzes the relationship between cortisol levels and alertness across multiple days.

Abstract:Melatonin and cortisol are clinically important for diagnosing sleep and mood disorders. We developed and validated a liquid chromatography-tandem mass spectrometry (LC-MS/MS) assay for simultaneous measurement of salivary melatonin and cortisol concentrations according to the Clinical and Laboratory Standards Institute guidelines. Additionally, we compared the LC-MS/MS assay with immunoassays, ELISA (Direct Salivary Melatonin Elisa EK-DSM, Bühlmann Laboratories AG, Schönenbuch, Switzerland) and electrochemiluminescence immunoassay (Cortisol II, Roche, Mannheim, Germany), using 121 saliva samples. The LC-MS/MS assay exhibited good performance in terms of linearity, precision, accuracy, limit of detection, lower limit of quantification, extraction recovery, carry-over, and matrix effect. The LC-MS/MS assay and immunoassays showed strong correlation (Pearson's r=0.910 for melatonin, r=0.955 for cortisol), but demonstrated a significant mean bias of 23.2% (range 54.0-143.7%) for melatonin and 48.9% (range 59.7-184.7%) for cortisol. Our LC-MS/MS assay provided more sensitive and reliable salivary melatonin and cortisol quantification results compared with immunoassays.

Abstract:Abstract Biochemical function of adrenal masses is currently based on 1mg post-overnight dexamethasone suppression test (pDST). Several approaches are recently developed, in order to reduce false positive/negative samples, only in retrospective series. They are based on the correlation of some different parameters, i.e. late-night salivary cortisol (LNSC) vs serum and salivary cortisol pDST; LNSC vs serum and salivary cortisol and serum dexamethasone pDST; LNSC and cortisone vs serum cortisol and salivary cortisol and cortisone pDST. Although these findings offer a better diagnostic performance, several conditions are still disappointed. No information is traceable about the harvest time of diurnal salivary and serum samples and no study include neither the levels of salivary nor urinary dexamethasone pDST. Aim of our study is to combine all these strategies in order to avoid the underestimated biases and obtain more precise information about the true “cortisol condition” of the patients. To reach this purpose we assess both cortisol and dexamethasone concentrations in several samples: saliva at 11PM before the drug administration, diurnal saliva and serum at 8AM and also the urine collection from 11PM to 8AM. Analytes levels are measured using a validated liquid chromatography-tandem mass spectrometry method. In this study we included 20 subjects without morphological adrenal alteration (MRI assessment), dyslipidemia, hypertension and impaired glucose tolerance (healthy controls) and 20 patients with adrenal incidentaloma showing different cortisol levels ranging from normal to ACTH-independent hypercortisolism. In both series, LNSC were similar to salivary cortisol pDST, even if they were greater in the patients with adrenal incidentalomas and subclinical cortisol secretion. Serum dexamethasone levels were in reference ranges, while salivary and urinary dexamethasone found in these matrices require additional sample numbers in order to establish appropriate cut-offs. Our preliminary results suggest that the combination of these findings could represent an improvement to assess the individual cortisol status.
Comparative analysis of the effects of collection methods on salivary steroids

Abstract:Background Steroid hormone test for saliva was a promising area of research, however the impact of different collection methods on salivary steroids was underexplored so far. This study was designed to compare the effects of different collection methods (unstimulated or stimulated by chewing paraffin, forepart or midstream) on salivary flow rate, concentrations and secretion rates of steroids in saliva. Methods Whole-saliva samples were collected from 10 systemically and orally healthy participants, whose forepart and midstream segments of saliva were collected under unstimulated and stimulated conditions, with the salivary flow rate of each sample recorded. The concentrations and secretion rates of salivary steroids including testosterone, dehydroepiandrosterone (DHEA) and progesterone were measured by ELISA, with the multiple of change calculated. Results The results indicated mechanical stimulation used in collection of saliva samples could affect concentrations and secretion rates of steroids, whereas forepart and midstream segments had little differences in levels of salivary steroids, which effects could be partly influenced by individual specificity. The asynchronism in change of secretion rate of steroids with that of salivary flow rate might play an important role during this course. Conclusion Based on these findings, we suggested to use the same collection method throughout one analytical study on salivary steroids or in longitudinal observations to ensure the comparability of the saliva samples collected.

Abstract:Background Many studies investigating the cortisol awakening response (CAR) suffer from low adherence and from the lack of precise and objective methods for assessing saliva sampling times. While attempts to address this methodological issue have been made in the past, most of them complicate the study protocol and drastically increase study costs, often making them infeasible for large-scale studies. Methods For that reason, we developed the CARWatch framework. It offers a smartphone application to record sampling times by scanning a barcode on the saliva sampling tube. To increase study protocol adherence, CARWatch can set alarms for saliva samples which can only be turned off by scanning the correct sampling tube. All study materials can be configured and generated by the CARWatch web page. In addition, awakening times can be recorded from sleep trackers and imported into CARWatch. Results A proof-of-concept study (N=117, 24.2±8.7 years, 79.5% women, two consecutive days) revealed that the use of CARWatch lead to considerably decreased interquartile ranges of sampling delays (self-reports: 8min, CARWatch: 2min) indicating a more consistent sampling behavior and stronger adherence to the study protocol compared to self-reports. Furthermore, it had a significant effect on AUCI (F[2,154] = 67.16, p < 0.001).

Abstract:Background Saliva composition has diurnal variations. Citric acid stimulation plays a major role in the change of salivary flow rate and salivary composition. However, diurnal variations and sex differences in salivary alpha-amylase (sAA), pH, salivary flow rate (SFR), and salivary cortisol before and after citric acid stimulation remain unclear. Methods We recruited 30 healthy volunteers, including 15 women (24.7 ± 1.0 years old) and 15 men (25.3 ± 1.3 years old). At four time points (T1, 7:00; T2, 10:00; T3, 16:00; and T4, 20:00), saliva was collected from healthy volunteers before and after citric acid stimulation; and sAA, pH, SFR and salivary cortisol were measured and compared between men and women. Results There were circadian fluctuations in sAA activity, SFR, pH, and cortisol level both before and after citric acid stimulation, and the diurnal fluctuations of these indexes were not affected by citric acid stimulation. There were significant differences in salivary cortisol between men and women before and after acid stimulation in T1. Neither SFR nor pH showed sex-related differences before or after acid stimulation. The variation trend of sAA activity was contrary to that of cortisol, with a significant negative correlation. Conclusions Our data suggest that sAA and cortisol showed diurnal fluctuation, and the variation characteristics of male and female under resting state and acid stimulation were basically the same. The variation trend of salivary alpha-amylase activity was opposite to that of cortisol, with significant negative correlation. Our findings may enable the selection of the correct sampling time for research and the selection of appropriate sampling strategies in studies investigating chronic psychosocial conditions.

Diurnal and day-to-day biological variation of salivary cortisol and cortisone

Abstract:There is a growing interest in the relevance of salivary cortisol and cortisone concentrations in stress-related research. To correctly attribute the magnitude of salivary cortisol and cortisone variation as an effect of a stressful event, a coherent understanding of the day-to-day intra-individual and inter-individual variability across the diurnal cycle of the two steroids is required. However, such information is currently lacking. This study aimed to overcome these existing limitations by performing an investigation of the biological variation (BV) of salivary cortisol and cortisone within one day and between five days using an LC-MS/MS method. Saliva samples were collected from 20 healthy volunteers immediately after waking up, at 8:00, 12:00, 15:00, 19:00 and 23:00 on each day over five days. All samples were analyzed in duplicate in one run. Nested ANOVA was used to calculate the sums of squares for analytical and biological components of variation. The within-subject BV of salivary cortisol and cortisone (CV I ) ranged from a minimum of 29.3 and 19.0 % to a maximum of 56.5 and 49.1 %, respectively, while the between-subject biological variation (CV G ) ranged from 29.7 and 29.0 % to 51.6 and 43.6 %. The reference change values (RCVs) ranged from 96 to 245 % for cortisol and from 55 to 194 % for cortisone. A medium index of individuality was observed for both compounds at all time points. This study provides updated BV estimates and RCVs for different times of day that can be used to assess the magnitude of change in biomarkers in future stress-related research.

Salivary C-reactive protein exhibits a diurnal pattern and relates to biobehavioral health in military men

Abstract:C-reactive protein is a systemic inflammatory biomarker that is positively associated with the development of disease. Salivary C-reactive protein (sCRP) has previously been reported to have a diurnal rhythm with higher levels upon awakening and lower levels thereafter. The aims of this study were to evaluate the stability of sCRP across two days, characterize the daily sCRP pattern, compute morning sCRP parameters, and evaluate associations with biobehavioral health in US Navy Explosive Ordnance Disposal (EOD) technicians. Seventy male EOD technicians (age = 34.9 ± 6.5 years) participated in this study, which included a tablet-based survey, measures of health and fitness, and saliva collection. In a free-living setting, participants self-collected saliva on 2 consecutive days at WAKE, WAKE+30, WAKE+60, 4p.m., and 9p.m., for a total of 10 samples. Parameters (e.g., area under the curve) were computed to characterize the morning sCRP magnitude and pattern. Pearson product-moment correlation analyses were used to assess the stability of sCRP samples and parameters across the study period and to examine associations with biobehavioral health. Average sCRP concentrations for the 2-day period were evaluated using an analysis of variance with repeated measures. The stabilities between corresponding time points on Days 1 and 2 were very high (rs = 0.87-.94, all ps ≤ 0.001). sCRP concentrations were highest at WAKE, decreased by 73.6 % at WAKE+30, and then plateaued for the rest of the day. Parameters stabilities were good to excellent (rs = 0.77-.98, all ps ≤ 0.001). We also observed associations between sCRP parameters, self-reported health behaviors, and objective measures of health and fitness. In this study of a military population, we characterized sCRP as diurnal with robust stability across 2 consecutive days, which demonstrates the feasibility of sCRP as a biomarker. These results have significant implications for study methodology and for using sCRP as a marker of dysfunction or disease.

Abstract:Biological measures (e.g., salivary sex hormone levels [estrogen, progesterone, testosterone]) are often used to understand the physiological mechanisms underlying various psychological phenomena. The choice of analytical technique is crucial to maximizing the validity of the results. Our objective was to compare the expected hormonal profiles of healthy young adults generated by two analytical techniques (high-sensitivity enzyme immunoassay [IA] and liquid chromatography-mass spectrometry [LCMS]). In a project investigating the role of oral contraceptives (OC), 72 OC users, 99 naturally cycling women (NC), and 47 men were recruited. Participants provided saliva samples at two visits. NC women visited the laboratory during the early follicular (low estrogen and progesterone levels) and pre-ovulatory (high estrogen and low progesterone levels) phases of their menstrual cycle. The concentrations of the three sex hormones were quantified using IA and LCMS. First, we found that the two methods correlated strongly for testosterone, but weakly and inconsistently for estradiol and progesterone. Second, compared to IA, the LCMS technique was able to detect the expected differences in estradiol and testosterone levels between NC women in different phases of their menstrual cycle (e.g., greater estradiol and testosterone concentrations in pre-ovulatory women versus early follicular). Lastly, for NC women, only the LCMS technique detected the expected within-subjects change in estradiol levels between the early follicular and pre-ovulatory phases. These results suggest that the LCMS technique should be prioritized for quantifying salivary estradiol levels. The greater specificity of this technique will allow for a more accurate measurement of salivary sex hormone levels.

Evaluation of a radioimmunoassay and establishment of a reference interval for salivary cortisol in healthy subjects in Denmark

Abstract:A commercial radioimmunoassay (RA) for salivary cortisol was evaluated using certified reference material in water and spiked to pooled saliva in the range 2.1-89.1 nmol/L. A variance component model for describing the effects of age, body mass index (BMI), diurnal variation, gender, days of sick leave during the past year, and smoking habits was established. Reference intervals for salivary cortisol in 120 healthy individuals performing their routine work were established according to the International Union of Pure and Applied Chemistry (IUPAC) and the International Federation of Clinical Chemistry (IFCC). The method evaluation of the certified reference material in water did not show any bias of the method, i.e. recovery was 97% [CI: 94%; 100.9%]. LOD (detection limit) was 1.59 nmol/L. The ratio between analytical and within-subject variation (CVa/CVi) was 0.14, indicating that the method was adequate for measurement in healthy subjects. Reference intervals were estimated to be from 3.6 to 35.1 nmol/L for samples at the time of awakening (05.27-07.27), 7.6-39.4 nmol/L for peak level in saliva samples collected 20 min after awakening (05.47-07.47), and LOD 10.3 nmol/L for late afternoon samples (17.00-19.00). Reactivity (increase from awakening to 20 min after awakening) was estimated to be 82% [CI: -179; 345%] and recovery (decrease from 20 min after awakening to 18.00) to be 80% [CI: 51; 109%]. Eighteen percent of the subjects showed a decrease in cortisol in saliva from awakening to 20 min after awakening. Salivary cortisol was not affected by age, body mass index, gender, smoking habits or days of sick leave during the past year.

Abstract:Internal circadian phase assessment is increasingly acknowledged as a critical clinical tool for the diagnosis, monitoring, and treatment of circadian rhythm sleep−wake disorders and for investigating circadian timing in other medical disorders. The widespread use of in‐laboratory circadian phase assessments in routine practice has been limited, most likely because circadian phase assessment is not required by formal diagnostic nosologies, and is not generally covered by insurance. At‐home assessment of salivary dim light melatonin onset (DLMO, a validated circadian phase marker) is an increasingly accepted approach to assess circadian phase. This approach may help meet the increased demand for assessments and has the advantages of lower cost and greater patient convenience. We reviewed the literature describing at‐home salivary DLMO assessment methods and identified factors deemed to be important to successful implementation. Here, we provide specific protocol recommendations for conducting at‐home salivary DLMO assessments to facilitate a standardized approach for clinical and research purposes. Key factors include control of lighting, sampling rate, and timing, and measures of patient compliance. We include findings from implementation of an optimization algorithm to determine the most efficient number and timing of samples in patients with Delayed Sleep−Wake Phase Disorder. We also provide recommendations for assay methods and interpretation. Providing definitive criteria for each factor, along with detailed instructions for protocol implementation, will enable more widespread adoption of at‐home circadian phase assessments as a standardized clinical diagnostic, monitoring, and treatment tool.

Abstract:Adrenal insufficiency (AI) is potentially life-threatening and accurate diagnosis is crucial. The first-line diagnostic test, the ACTH stimulation test, measures serum total cortisol. However, this is affected in states of altered albumin or cortisol-binding globulin levels, limiting reliability. Salivary cortisol reflects free bioactive cortisol levels and is a promising alternative. However, few studies are available, and heterogenous methodologies limit applicability. This study prospectively recruited 42 outpatients undergoing evaluation for AI, excluding participants with altered cortisol-binding states. Serum (immunoassay) and salivary (LC-MS/MS) cortisol levels were sampled at baseline, 30-minutes and 60-minutes following 250mcg Synacthen administration. AI was defined as a peak serum cortisol level <500nmol/l in accordance with guidelines. The study recruited 21 (50%) participants with AI, and 21 without AI. There were no significant differences in baseline characteristics, blood pressure, or sodium levels between groups. Following Synacthen stimulation, serum and salivary cortisol levels showed good correlation at all timepoints (R2 = 0.74, P<0.001), at peak levels (R2 = 0.72, P<0.001), and at 60-minutes (R2=0.72, P<0.001). A salivary cortisol cut-off of 16.0 nmol/L had a sensitivity of 90.5% and a specificity of 76.2% for the diagnosis of AI. This study demonstrates good correlation between serum and salivary cortisol levels during the 250mcg Synacthen test. A peak salivary cortisol cut-off of 16.0 nmol/L can be used for the diagnosis of AI. It is a less invasive alternative to evaluate patients with suspected AI. Its potential utility in the diagnosis of AI in patients with altered cortisol-binding states should be further studied.
