Abstract:Objective: Investigating the relationship between systematic fine-needle aspiration and testis biopsy histology in predicting sperm presence in order to provide relative counseling to patient with azoospermia. Methods: Firstly open testis biopsy was performed in 67 azoospermia men, including obstructive and nonobstructive azoospermia (NOA). Then all the patients underwent testicular systematic fine-needle aspiration (FNA), and wet preparation of the obtained testis tissue was investigated by microscopy to find spermatozoa. Results: The numbers of obstructive azoospermia and NOA were 12 cases and 55 cases respectively. The testis histology patterns of total cases included normal spermatogenesis, hypospermatogenesis, sertoli cell-only, maturation arrest as well as mixed patterns that involved at least two of those histologies. Overall spermatozoa were found by systematic FNA in 49% of NOA patients. A comparison of histology and systematic FNA showed that the pattern of hypospermatogenesis had a higher rate of sperm detection. Conclusion: For azoospermia patients systematic FNA is an effective, minimally invasive method to detect the sperm presence. Sperm detection by systematic FNA shows a close relationship with testis histology.
Primary Evaluation of Testicular Biopsy and Pathological Examination on Status of Sperm Detectable Rate from Patients with Obstructive Azoospermia

Abstract:Objective:To primary evaluate the diagnosis values of the testicular biopsy and pathological analysis of testicular tissue as a means for obstructive azoospermia. Methods:Totally 84 patients who had previously undergone a testicular biopsy and pathological analysis were recruited for this study. After a clinical re-evaluation,the diagnosis of obstructive azoospermia and the etiology of each patient's disease were obtained. The patients who had no spermatozoa retrieved via the previous biopsy were then subjected to either a percutaneous epididymal sperm aspiration(PESA) procedure or a testicular fine needle aspiration(TEFNA) procedure. Results:Out of 84 patients with OA,56(66.67%) cases were congenital,26(30.95%) cases were inflammatory,2 cases had a history of bilateral herniotomy. Out of 33 cases whose pathological analysis failed to retrieve any spermatozoa,the spermatozoa could be retrieved in all 29 cases after the second procedure(PESA or TEFNA) . ICSI was performed for 57 cases with 62 cycles,and the pregnancy rate was 46.8%. Conclusion:The testicular biopsy and pathological analysis has a high false-negative rate and is not sensitive enough to be a good diagnostic test for OA,and PESA or TEFNA with less trauma is benifit for further treatment.
Successful Sperm Retrieval and Clinical Pregnancies Following Micro-TESE and ICSI Treatments in Patients with Nonobstructive Azoospermia Due to Various Etiologies

Abstract:(1) Background: Nonobstructive azoospermia (NOA) etiologies affect the sperm retrieval rate (SRR) by microdissection testicular sperm extraction (micro-TESE) and the clinical outcomes following intracytoplasmic sperm injection (ICSI); (2) Methods: We investigated seven NOA etiologies. The SRR and clinical outcomes of 627 patients were analyzed between November 2017 and July 2022 in the Reproductive and Genetic Hospital of China International Trust and Investment Corporation-Xiangya (CITIC-Xiangya); (3) Results: The overall SRR was 39.4% (247/627). The SRR according to NOA etiologies were: Y chromosome azoospermia factor c microdeletions (26/46, 56.5%), Klinefelter syndrome (KS), 36/85, 42.4%), idiopathic (110/398, 27.6%), cryptorchidism (20/29, 69.0%), chromosome anomalies (7/13, 53.9%), orchitis (45/50, 90.0%), and cancer (3/6, 50.0%). The SRR were different for spermatogonia arrest (26/96, 27.1%), maturation arrest (76/177, 42.9%), and SCOS (30/80, 37.5%) according to histological examinations. The clinical pregnancy rate was similar among the NOA etiologies. The high-quality embryo rate differed between successful (54.7%) and unsuccessful (40.9%) pregnancies. Moreover, the successfully pregnant women (28.99 years) were younger than the unsuccessfully pregnant ones (30.92 years); (4) Conclusions: The SRR from patients with NOA was associated with the etiology and histological categories, while the clinical outcome was associated with the high-quality embryo rate and the female partner's age.

Abstract:The effects of brain irradiation on the enzyme activity of monoamine oxidase (MAO) and catecholamine O-methyltransferase (COMT), as well as on the rate of incorporation of [14C]tyrosine into catecholamines (CAs), were examined in brain regions of the squirrel monkey (Saimiri sciureus). Results indicated that proton irradiation of the striate visual cortex at either 10,000 or 20,000 rad drastically affected the MAO activity, moderately affected the COMT activity, and significantly altered the incorporation of [14C]tyrosine into CAs in several brain regions. After 10,000 rad, MAO activity was increased in caudate nucleus (by 135%) and cerebellum (by 41%); significantly decreased in putamen (by 49%), gray matter (by 19%), and hippocampus (by 18%); and unchanged in brainstem. After 20,000 rad, MAO activity was markedly increased in caudate nucleus (by 114%), but decreased in cerebellum (by 24%), putamen (by 38%), gray matter (by 30%), and hippocampus (by 36%), and unchanged in brainstem. After 10,000 rad, COMT activity was significantly decreased in putamen (by 27%) and brainstem (28%). After 20,000 rad, COMT activity was significantly decreased in putamen (by 22%) and cortical gray matter (by 37%) but unchanged in the other brain regions examined. The 10,000 rad significantly increased the rate of incorporation of [14C]tyrosine into CAs in both the cerebellum (by 68%) and hypothalamus (by 26%), but 20,000 rad increased the rate only in hypothalamus (by 17%) and decreased the rate in hippocampus (by 24%). The whole-brain weights were significantly decreased after both 10,000 or 20,000 rad, by 17% and 11%, respectively. The only significant change in regional tissue weight was observed in putamen after 20,000 rad (a decrease by 17%). The protein concentration was significantly increased in hippocampus at both 10,000 (by 13%) and 20,000 (by 30%) but not affected in other brain regions examined. These data support our previous conclusion that a sustained chemical change occurs at the irradiated site and that the effect was transmitted to nonirradiated brain regions. Results also suggest that catecholaminergic pathways may exist between striate visual cortex and other cortical or subcortical brain areas where chemical changes were observed after visual cortical lesion by proton irradiation at various doses.

Abstract:One major challenge in male factor infertility is nonobstructive azoospermia (NOA), which is characterized by spermatozoa-deficient semen without physical duct blockage. This review offers a thorough overview of the histopathology of the testes in NOA cases, clarifying its complex etiology and emphasizing the possible value of histopathology inspection for both diagnosis and treatment. Variable histopathological findings have been linked to NOA, such as tubular hyalinization, Sertoli cell-only syndrome, hypospermatogenesis, and germ cell arrest. Understanding the pathophysiology and forecasting the effectiveness of treatment are further enhanced by both morphometric and ultrastructural analyses. The potential significance of testicular biopsy in forecasting reproductive outcomes is assessed, especially concerning assisted reproductive technologies like intracytoplasmic sperm injection (ICSI). Besides, testicular microlithiasis, serum hormone profiles, and testicular size are investigated concerning NOA histopathology. It is concluded that understanding the histopathological patterns in NOA is crucial for its accurate diagnosis and appropriate management. Further research is still warranted to improve understanding of the complex pathophysiology underlying NOA.
[Testicular Histology Does Not Affect the Clinical Outcomes of ICSI in Men with Non-Obstructive Azoospermia].

Abstract:OBJECTIVE:To investigate whether testicular histology influences the clinical outcomes of intracytoplasmic sperm injection (ICSI) in men with non-obstructive azoospermia (NOA).METHODS:We retrospectively analyzed the clinical data about 73 cases of NOA undergoing ICSI, including 105 ICSI cycles and 79 embryo transfer cycles. The infertility of the patients was attributed to male factors only or both male and female tube factors and the females' age was ≤38 years. Based on testicular histology, we divided the ICSI cycles into three groups: hypospermatogenesis (HS, n = 72), maturation arrest (MA, n = 21) and Sertoli cells only (SCO, n = 12). We recorded and analyzed the age of both the males and females, infertility duration, base follicle-stimulating hormone (FSH) level, dose and days of gonadotropin (Gn) administration, estradiol (E2) and progesterone (P) levels on the day of human chorionic gonadotropin (hCG) administration, endometrial thickness, number of metaphase II (MII) oocytes, and rates of fertilization, transferrable embryos, high-quality embryos, clinical pregnancy, and abortion.RESULTS:The rates of fertilization, failed fertilization, transferrable embryos, and high-quality embryos, and the average number of transferred embryos were 67.03% (553/825), 9.52% (10/105), 85.66% (472/551), 35.03% (193/551), and 2.10, respectively, resulting in 44 pregnancies (55.70%) and 42 live births (53.16%), with no birth defects. No statistically significant differences were observed among the HS, MA and SCO groups in the mean age of the men and women, infertility duration, base FSH level, Gn dose, Gn days, E2 and P levels on the hCG day, endometrial thickness, or number of MII oocytes, nor in the rates of fertilization (68.51% vs 64.39% vs 61.45%), transferrable embryos (85.05% vs 90.48% vs 83.05%), or high-quality embryos (33.09% vs 41.67% vs 38.98%). The rates of clinical pregnancy and embryo implantation were higher in the HS (60.00% and 37.61%) and SCO (62.50% and 50.00%) than in the MA group (37.50% and 21.21%), but with no statistically significant differences (P >0.05).CONCLUSIONS:Once testicular sperm is retrieved, desirable clinical outcomes can be achieved in ICSI for NOA patients, which is not affected by testicular histopathology.
Rate of testicular histology failure in predicting successful testicular sperm extraction

Abstract:Background: The management of Non-Obstructive (NOA) Azoospermia or Obstructive Azoospermia (OA) patients relies on testicular sperm extraction (TESE) followed by intracytoplasmic sperm injection (ICSI). In NOA patients the sperm recovery is successful in only 50% of cases and therefore the ability to predict those patients with a high probability of achieving a successful sperm retrieval would be a great value in counselling the patient and his partner. Several studies tried to suggest predictors of a positive TESE (e.g. FSH concentration), but most concluded that diagnostic testicular biopsy (histology) is best. Methods: This is a retrospective analysis of 526 TESE patients. After the extraction of the testis, the resulting sample was immediately given to the embryologist, who examined the tubules for sperm cryopreservation. During the same procedure, a different specimen was destined to the histological analysis. The comparison between the two methodological approaches was carried out through a score. Results: Concordance between TESE and testicular histology outcomes was found in 70,7% of patients; discordance was found in 29,3% of patients. Among the discordance outcomes, in approximately 95% we found at least 1 sperm in the TESE retrieval, while the histology report did not find any spermatozoa or found not enough compared to our evaluation; in only 5% of cases we did not find any spermatozoa or found not enough compared to what was detected in the testicular histology. Conclusion: Based on our experience, to increase diagnostic accuracy, a larger biopsy should be sent to the histopathology laboratory; another option may be to use TESE cell suspension (the same embryologists employ for cryopreservation) for cytological evaluation of spermatogenesis.

Abstract:Objective: Reproductive hormones are a traditional good method to evaluate spermatogenesis but might not accurately represent local spermatogenesis. To find a more accurate method, seminal reproductive hormones were studied. Methods: A bidirectional cohort study was performed. A total of 126 infertile men from 2018 to 2019 were retrospectively analyzed. They were divided into nonobstructive azoospermia (NOA), oligozoospermia (OLZ) and normal (NOR) groups. A prospective study was conducted on patients in the NOA and OLZ groups for 2 years. Microscopic testicular sperm extraction was performed for NOA patients, who were divided into a focal spermatogenesis group (FS) and an idiopathic azoospermia group (IA). Drug treatment was for OLZ patients, who were divided into a valid group (VA) and an invalid group (IN). The differences in sperm parameters and reproductive hormones were compared. ANOSIM analysis was used between and within groups. Pearson correlation analysis, CO inertia analysis and Proctor's analysis were for relationships. ROC curve for the specificity and sensitivity. Time series analysis was for the trends between hormones and time. Results: The b-FSH, b-LH, s-T and ΔT in the NOA group were significantly higher than those in the OLZ and NOR groups. However, the s-FSH, s-E 2 , s-P, ΔFSH, ΔLH, ΔP and ΔE 2 were lower. Thirty-one NOA patients underwent MTSE, of whom 12 had sperm (FS) and 19 had no sperm (IA). The s-FSH and s-E 2 of the FS group were higher than those of the IA group. Twenty-six OLZ patients completed 30 days of treatment, of which 11 had an improved sperm count (VA) and 15 had no (IN). The ΔT of the VA group was higher than that of the IN group. After follow-up for 2 years, 18 patients' results showed that b-FSH, b-LH and s-T were different over time, with delays of 19, 3 and -19 days. SC is closely related to pH, s-FSH, s-LH, s-E 2 , s-P, s-T, b-FSH, b-LH, ΔFSH, ΔLH, ΔP, ΔE 2 and ΔT. There were complex common trends and relationships between different kinds of hormones. s-FSH, s-LH, s-E 2 , s-P, s-T, b-FSH and b-LH were useful to judge spermatogenesis, of which s-T, b-FSH and b-LH were more sensitive. If s-T, b-FSH and b-LH reached 64.4, 9.4 and 4.7, respectively, their prediction performance was the strongest. Conclusion: Seminal testosterone is sensitive for judging local spermatogenesis in nonobstructive azoospermia patients, which may be the direction of local spermatogenesis in nonobstructive azoospermia. Testicular Sperm Aspiration Has a Poor Effect in Predicting Micro-Tese Outcomes in NOA Patients with AZFc Deletion

Abstract:Background Testicular sperm aspiration (TESA) is widely used in the diagnosis and management of nonobstructive azoospermia. However, its ability for predicting microdissection testicular sperm extraction in nonobstructive azoospermia (NOA) patients with AZFc deletion remains uncertain. To investigate whether TESA affected the sperm retrieval rate (SRR) in NOA patients with AZFc deletion, a retrospective analysis of the clinical data of NOA patients with AZFc deletion who underwent microdissection testicular sperm extraction (micro-TESE) was conducted. The effects of age, testicular volume, follicle-stimulating hormone (FSH) levels, luteinizing hormone (LH) levels, testosterone (T) levels and TESA on the SRR were analyzed in this group of patients. Results A total of 181 individuals had their sperm successfully collected and underwent micro-TESE, with an SRR of 67.4%. The patients were separated into two groups based on their micro-TESE results (sperm acquisition and nonsperm acquisition), with no significant variations in age, testicular volume, FSH levels, LH levels, or T levels between the two groups. There was no significant difference in the SRR between any of the groups into which patients were classified based on reproductive hormone reference value ranges. Binary logistic regression was used to explore the absence of significant effects of age, testicular volume, FSH levels, LH levels, and T levels on sperm acquisition in patients undergoing micro-TESE. In the preoperative testicular diagnostic biopsy group, the sperm acquisition and nonsperm acquisition groups had SRRs of 90.1% and 65.1%, respectively. More significantly, there was no significant difference in the SRR between the negative preoperative testicular diagnostic biopsy group and the nonpreoperative testicular diagnostic biopsy group (65.1 vs. 63.8%, p = 0.855). Conclusion There is a high probability of successful sperm acquisition in the testis of men undergoing micro-TESE. In this group of patients, age, testicular volume, FSH levels, LH levels, and T levels may have little bearing on the micro-TESE outcome. In patients whose preoperative TESA revealed the absence of sperm, the probability of obtaining sperm by micro-TESE remained high (65.1%); negative TESA results appeared to not influence the SRR (63.8%) in patients undergoing micro-TESE.
Sertoli cells from non-obstructive azoospermia and obstructive azoospermia patients show distinct morphology, Raman spectrum and biochemical phenotype

Abstract:Study question: Are there differences in the morphology, spectrum and biochemical phenotype between Sertoli cells from non-obstructive azoospermia (NOA) patients and those from obstructive azoospermia (OA) patients with normal spermatogenesis? Summary answer: Sertoli cells from NOA patients are distinct from those from OA patients in terms of morphological features, Raman spectrum and phenotype including the expression of genes and proteins (e.g. SCF, BMP4 and GDNF). What is known already: NOA affects 10% of infertile men and has been diagnosed in 60% of azoospermic men. In contrast with OA patients who have normal spermatogenesis, NOA patients have an impaired spermatogenesis. Study design, size and duration: This case-control study included 100 NOA patients (as cases) and 100 OA patients with normal spermatogenesis (as controls). The study was performed between January 2012 and January 2013. Participants/materials, setting and methods: Karyotype analysis was performed to check the chromosome content and multiplex PCR was carried out to determine the expression of numerous Y chromosome genes in NOA patients. Human Sertoli cells were then isolated from the testes of NOA and OA patients by two-step enzymatic digestion and differential plating. Transmission electron microscopy was used to determine the ultrastructure of the Sertoli cells and real-time Raman microspectroscopy was used to assess their spectrum. We further compared the two groups of patients for expression of SCF, GDNF and BMP4 in Sertoli cells, using RT-PCR, microarray analysis, immunofluorescence, immunohistochemistry and Western blots. Main results and the role of chance: NOA patients had normal chromosome karyotypes and Y chromosome microdeletions were excluded. In morphology, Sertoli cells isolated from NOA patients had a series of abnormal ultrastructural features compared with the control Sertoli cells: (i) existence of small and spindle-shaped nuclei, (ii) smaller diameter, (iii) deficient nucleolus or endoplasmic reticulum and (iv) more vacuoles. Spectral intensities in Sertoli cells of NOA patients were distinct at four typical Raman peaks compared with the control Sertoli cells. In phenotype, SCF, BMP4 and GDNF transcripts and proteins were significantly lower in Sertoli cells of NOA patients than in the control Sertoli cells. Limitations and reasons for caution: The Sertoli cells of NOA patients were not compared with Sertoli cells of normal fertile men due to the fact that it is hard to obtain adult testes from normal donors. Wider implications of the findings: This study provides novel insights into understanding the underlying causes for NOA and might offer a basis for developing new therapeutic strategies for patients with NOA.

Abstract:To investigate the correlation of the testis volume and reproductive hormone level with the results of testicular sperm aspiration (TESA) in non-obstructive azoospermia (NOA) patients, and to explore the cut-off value of the testis volume and reproductive hormone level in predicting the results of TESA so as to provide reliable information for the diagnosis and treatment of NOA.We enlisted 121 NOA patients in this study, divided them into a sperm group and a non-sperm group based on the results of TESA, and measured their testis volumes and reproductive hormone levels.The left testis volume, the right testis volume, and the levels of prolactin (PRL), follicle-stimulating hormone (FSH), luteinising hormone (LH), estradiol (E2) and total testosterone (T) in the non-sperm and sperm groups were (7.07 +/- 1.06) ml vs (11.75 +/- 1.38) ml, (7.37 +/- 1.37) ml vs (11.70 +/- 1.98) ml, (12.43 +/- 11.69) ng/ml vs (9.60 +/- 4.55) ng/ml, (15.77 +/- 10.84) mIU/ml vs (8.01 +/- 7.43) mIU/ml, (6.12 +/- 2.92) mIU/ml vs (8.11 +/- 20.11) mIU/ml, (119.36 +/- 43.52) pmol/L vs (141.12 +/- 48.33) pmol/L, and (11.43 +/- 4.05) nmol/L vs (12.46 +/- 4.60) nmol/L, respectively. The mean levels of serum FSH and PRL were significantly higher in the non-sperm than in the sperm group. Although the mean testis volume of the former was less than that of the latter, there were no significant differences between the two groups, and nor were any significant differences in age and the levels of E2 and T. The cut-off value of the testis volume was 9 ml, with sensitivity of 93.8%/89.6% (left/right) and specificity of 100%/94.3% (left/right). The area under curve (AUC) of the left testis volume was 0.984 and that of the right was 0.961, indicating a high diagnostic accuracy. The cut-off value of the serum FSH level was 8.18 mIU/ml, with a sensitivity of 71.2% and a specificity of 75.0%. The AUC of the FSH level was 0.743, suggestive of a moderate diagnostic accuracy.The testis volume and FSH level are important for predicting the TESA results of NOA patients, and the former has even a higher diagnostic accuracy than the latter.

The effects of age, testicular volume, FSH, LH, T, and preoperative diagnostic testicular biopsy on SRR were compared cross-sectionally in this group of patients.Results: A total of 181 individuals had their sperm successfully collected and underwent micro-TESE, with an SRR of 67.4 %. The results of micro-TESE were separated into two groups: sperm acquisition and non-sperm acquisition, with no significant variations in age, testicular volume, FSH, LH, and T between the two groups. Binary logistic regression explored the absence of significant effects of age, testicular volume, FSH, LH, and T on sperm acquisition in patients undergoing micro-TESE. Based on whether or not a preoperative diagnostic testicular biopsy was conducted, the above parameters were compared between the two groups. Testicular volume was higher in the group with preoperative diagnostic testicular biopsy than in the group without preoperative diagnostic testicular biopsy, while FSH and T were lower. In the preoperative testicular diagnostic biopsy group, the sperm acquisition and non-acquired sperm groups had SRR of 90.1 % and 65.1 %, respectively, with the sperm acquisition group being significantly higher than the non-acquired sperm group.Conclusion: There is a high probability of successful sperm acquisition in the testis with micro-TESE. In this group of patients, age, testicular volume, FSH, LH, and T may have little bearing on the micro-TESE outcome. In patients whose preoperative diagnostic testicular biopsy revealed no sperm, the probability of obtaining sperm by micro-TESE remained high (65.1%), while negative preoperative diagnostic testicular biopsy results appeared to not influence SRR by micro-TESE.

Abstract:Abstract Clinical efficacy of treatments against non-obstructive azoospermia (NOA), which affects 1% of men, are currently limited by the incomplete understanding of NOA pathogenesis and normal spermatogenic microenvironment. Here, we profile >80,000 human testicular single-cell transcriptomes from 10 healthy donors spanning the range from infant to adult and 7 NOA patients. We show that Sertoli cells, which form the scaffold in the testicular microenvironment, are severely damaged in NOA patients and identify the roadmap of Sertoli cell maturation. Notably, Sertoli cells of patients with congenital causes (Klinefelter syndrome and Y chromosome microdeletions) are mature, but exhibit abnormal immune responses, while the cells in idiopathic NOA (iNOA) are physiologically immature. Furthermore, we find that inhibition of Wnt signaling promotes the maturation of Sertoli cells from iNOA patients, allowing these cells to regain their ability to support germ cell survival. We provide a novel perspective on the development of diagnostic methods and therapeutic targets for NOA.

Abstract:We performed this study to evaluate the clinical outcomes of microdissection testicular sperm extraction-intracytoplasmic sperm injection (micro-TESE-ICSI) treatment that used fresh or cryopreserved sperm in patients with nonobstructive azoospermia (NOA). A total of 338 NOA patients with 344 consecutive cycles received treatment in the reproductive medicine center of Peking University Third Hospital in Beijing, China, from January 2014 to December 2017. Fresh oocytes and fresh sperm were used in 222 patients with 234 cycles (Group A). Fresh oocytes and cryopreserved sperm were used in 116 patients with 110 cycles (Group B). We compared patient characteristics, embryonic development, and pregnancy outcomes between Groups A and B. There was no statistical difference in the patient characteristics, and no differences were observed with fertilization or quality embryo rates between Groups A and B. The rates of clinical pregnancy and live birth were both higher for Group A than those for Group B (both P < 0.05). In conclusion, fresh testicular sperm appears to produce better ICSI outcomes than cryopreserved testicular sperm in patients with NOA.

Abstract:Objective To compare the outcome of intracytoplasmic sperm injection (ICSI)in men with obstructive and nun-obstructive azoospermia.Methods This study retrospectively investigated azoospermia patients receving ICSI treatment in the First Hospital of Beijing University from Jan.2012 to Dec.2015,whose infertile reason was only male factors,and in these patients the basal hormone was normal and the female age was ≤ thirty-five years.They finished 344 cycles of egg retrieval and 256 transplantation cycles.They were divided into OA group (n=255)and NOA group (n=79) according to histopathological types.Male and female age,infertile period,base FSH level,Gn dosage,Gn days,E2 and P level on hCG day,endometrial thickness,received oocyte number,fertilization rate,cleavage rate,quality embryo,clinical pregnancy rate,fetal miscarriage and live birth rate of all ICSI cycles and different groups were evaluated.Results The fertilization rate,fertilization failure rate,transferable embryo rate,good quality embryo rate of all azoospermia patients were 72.68％ (2208/3038),4.36％ (15/344),87.87％ (1920/2185) and 36.93％ (807/2185) respectively,with 144 pregnancies (56.25％,144/256)and the live birth rate was 50.78％ (130/256).The demographic and clinical factors,including male and female age,infertile period,female base FSH level,Gn dosage,Gn days,E2 and P level on hCG day,endometrial thickness and received oocyte number were similar between two groups;there were also no difference in the embryo cleavage and quality,clinical pregnancy rate,implantation rate,fetal miscarriage or live birth rate between two groups,though the fertilization rate and 2PN fertilization rate of OA group (74.61％,67.29％)were higher than NOA group (65.93％,54.96％,P ＜ 0.01);the fertilization failure rate of NOA group was higher than OA group (8.99％ vs.2.75％,P ＜ 0.05).Conclusion The fertility capability of testicular sperm in men with NOA is lower compared with OA,but the clinical pregnancy outcome of ICSI is similar between two groups.

Abstract:PurposeThe purpose of the study is to explore testicular sperm cryopreservation in patients with nonobstructive azoospermia (NOA) whether affect the outcome of subsequent intracytoplasmic sperm injection (ICSI).MethodsA systematic review and meta-analysis was conducted by searching the MEDLINE and EMBASE databases for relevant published studies in English language (1997–2017). Studies were eligible if they included the comparison of using fresh and frozen-thawed testicular sperm followed by ICSI. Two reviewers independently performed data extraction, quality assessment and assessed the risk of bias. The overall summary risk estimated the number of events. A meta-analysis was conducted using a random effects or fixed effects model analysis according to the test of heterogeneity.ResultsA total of 17 studies with 1,261 ICSI cycles were identified. Analysis of the present data showed no difference in the fertilization outcome when comparing fresh versus frozen-thawed spermatozoa (RR = 1.02, 95% CI 0.86–1.09). Similarly, no difference in CR (RR = 1.01, 95% CI 0.96–1.05), good embryo rate (RR = 1.01, 95% CI 0.95–1.09), and IR (RR = 0.93, 95% CI 0.66–1.30) was observed if the spermatozoa was fresh or frozen-thawed. Finally, no difference in CPR or LBR was noted when using fresh or frozen-thawed cycles were analyzed separately (RR = 1.03, 95% CI 0.86–1.24; RR 1.11, 95% CI 0.88–1.41, respectively).ConclusionsIn men with NOA, the ICSI outcome is not affected by whether the retrieved testicular sperm is fresh or frozen. Sperm cryopreservation ought to be considered in every surgical sperm retrieval case, which remain feasible even in patients with few testicular sperm retrieved.

Abstract:Introduction: The application of microdissection testicular sperm extraction (micro-TESE) to retrieve the sperm of patients with non-obstructive azoospermia (NOA) has greatly increased. Patients with NOA often have poor quality sperm. Unfortunately, there are few studies on artificial oocyte activation (AOA) performed on patients who successfully retrieved motile and immotile sperm by micro-TESE after intracytoplasmic sperm injection (ICSI). Therefore, this study sought to obtain more comprehensive evidence-based data and embryo development outcomes to aid consultation of patients with NOA who opted to receive assisted reproductive techniques and to determine whether AOA needs to be performed in different motile sperm after ICSI. Methods: This retrospective study involved 235 patients with NOA who underwent micro-TESE to retrieve adequate sperm for ICSI between January 2018 and December 2020. A total of 331 ICSI cycles were performed in the 235 couples. Embryological, clinical, and neonatal outcomes were demonstrated comprehensively between motile sperm and immotile sperm using AOA and non-AOA treatment. Results: Motile sperm injection with AOA (group 1) showed significantly higher fertility rate (72.77% vs. 67.59%, p =0.005), 2 pronucleus (2PN) fertility rate (64.33% vs. 60.22%, p =0.036), and miscarriage rate (17.65% vs. 2.44%, p =0.018) compared with motile sperm injection with non-AOA (group 2). Group 1 had comparable available embryo rate (41.29% vs. 40.74%, p =0.817), good embryo rate (13.44% vs. 15.44%, p =0.265), and without an embryo for transfer rate (10.85% vs. 9.90%, p =0.815) compared with group 2. Immotile sperm injection with AOA (group 3) displayed significantly higher fertility rate (78.56% vs. 67.59%, p =0.000), 2PN fertility rate (67.36% vs. 60.22%, p =0.001), without an embryo for transfer rate (23.76% vs. 9.90%, p= 0.008), and miscarriage rate (20.00% vs. 2.44%, p =0.014), but significantly lower available embryo rate (26.63% vs. 40.74%, p =0.000) and good embryo rate (15.44% vs. 6.99%, p =0.000) compared with group 2. In groups 1, 2, and 3, the rates of implantation (34.87%, 31.85% and 28.00%, respectively; p =0.408), clinical pregnancy (43.87%, 41.00%, and 34.48%, respectively; p =0.360) and live birth (36.13%, 40.00%, and 27.59%, respectively; p =0.194) were similar. Discussion: For those patients with NOA from whom adequate sperm were retrieved for ICSI, AOA could improve fertilization rate, but not embryo quality and live birth outcomes. For patients with NOA and only immotile sperm, AOA can help achieve acceptable fertilization rate and live birth outcomes. AOA is recommended for patients with NOA only when immotile sperm are injected.

Abstract:Retrieving spermatozoa from the testicles has been a great hope for patients with non-obstructive azoospermia (NOA), but relevant methods have not yet been developed to the level necessary to provide resolutions for all cases of NOA. Although performing testicular sperm extraction under microscopic magnification has increased sperm retrieval rates, in vitro selection and processing of quality sperm plays an essential role in the success of in vitro fertilization. Moreover, sperm cryopreservation is widely used in assisted reproductive technologies, whether for therapeutic purposes or for future fertility preservation. In recent years, there have been new developments using advanced technologies to freeze and preserve even very small numbers of sperm for which conventional techniques are inadequate. The present review provides an up-to-date summary of current strategies for maximizing sperm recovery from surgically obtained testicular samples and, as an extension, optimization of in vitro sperm processing techniques in the management of NOA.

Abstract:OBJECTIVE:To primarily study the influence of recombination abnormality in human spermatocyte meiosis on the pathology of the patients with non-obstructive azoospermia (NOA).METHODS:We obtained testis tissues from 6 NOA patients by testicular biopsy and divided the tissue of each patient into 2 portions, one for pathological examination and the other for immunofluorescent staining. We observed the synaptonemal complex and the numbers of the recombination sites on homologous chromosomes, and analyzed the relationship between abnormal recombination and pathological findings.RESULTS:Pathological examination showed that the basement membrane of the seminiferous tubules was thickened in 3 of the cases and atrophied in the other 3, the number of autosomal MLH1 foci in a spermatocyte ranging from 10 to 50 in the former 3, and from 30 to 50 in the latter 3.CONCLUSION:The increased range of the homologous chromosomal recombination frequency may be one of the possible factors for the thickening of seminiferous tubule basement membrane and even lumen occlusion in NOA patients.
