Abstract:Light and electron microscope studies were conducted on the nasopharynx and the nasopharyngeal tonsil of 15 young horses. The nasopharynx and nasopharyngeal tonsil was lined with pseudostratified columnar ciliated epithelium and goblet cells. The lymphoepithelium of the nasopharyngeal tonsil was folded forming crypts, the mucosa of which was modified into follicle associated epithelium characterized by stratified cuboidal epithelium, loss of cilia, absence of goblet cells and infiltration of lymphocytes. The lamina propria mucosae of the nasopharyngeal tonsil contained well‐developed lymphoid tissue and clusters of seromucus acini. Scanning electron‐microscopy revealed a dense mat of cilia covering the nasopharynx and nasopharyngeal tonsil. The follicle‐associated epithelium consisted of different populations of microvillus cells in addition to M cells with very short microvilli and a few squamous and intermediate cells. Microvillus cells in the deeper part of the FAE had larger microvilli and their cytoplasm contained a dense population of mitochondria, smooth and rough endoplasmic reticulum, Golgi complexes and lysosomes. The flat surfaced M cell had a more electron‐dense cytoplasm and contained small supranuclear vacuoles in addition to the organelles seen in microvillus cells.

Abstract:The interaction of heat shock protein with molecular mass 25 kDa (HSP25) and its point mutants S77D + S81D (2D mutant) and S15D + S77D + S81D (3D mutant) with intact and thermally denatured actin was analyzed by means of fluorescence spectroscopy and ultracentrifugation. Wild type HSP25 did not affect the polymerization of intact actin. The HSP25 3D mutant decreased the initial rate without affecting the maximal extent of intact actin polymerization. G-actin heated at 40-45 degrees C was partially denatured, but retained its ability to polymerize. The wild type HSP25 did not affect polymerization of this partially denatured actin. The 3D mutant of HSP25 increased the initial rate of polymerization of partially denatured actin. Heating at more than 55 degrees C induced complete denaturation of G-actin. Completely denatured G-actin cannot polymerize, but it aggregates at increased ionic strength. HSP25 and especially its 2D and 3D mutants effectively prevent salt-induced aggregation of completely denatured actin. It is concluded that the interaction of HSP25 with actin depends on the state of both actin and HSP25. HSP25 predominantly acts as a chaperone and preferentially interacts with thermally unfolded actin, preventing the formation of insoluble aggregates.

Abstract:Hepatic tumors unassociated with cirrhosis were encountered in seven patients aged 10 to 19 years. Four patients had received androgens for aplastic anemia. Two patients had transfusional hemosiderosis. One patient had had a renal transplant 2 1/2 years ago. Two patients are alive at 2 3/4 and 2 1/2 years after surgical resections. Nodules were found at autopsy in the others. The tumors were well differentiated and, in the androgen-related cases, differed from the others in the following features: canalicular bile retention, mild nuclear atypia, and acinar formation. No mitoses, vascular invasion, or metastatic tumor were evident. The clinical setting was variable; different factors, including iron overload and androgen therapy, played a role in the development of tumor. Although the androgen-related cases showed mild cellular atypia, biologic evidence of malignancy was lacking as in most previous reports.

Abstract:Individuals and society are dependent on transportation. Individuals move about their world for work, school, healthcare, social activities, religious and athletic events, and so much more. Society requires the movement of goods, food, medicine, etc. for basic needs, commerce, cultural and political exchanges, and all of its dynamic, complex elements. To meet these critical daily demands, the transportation system operates globally and around the clock. Regardless of their role, a basic requirement for the individuals operating the transportation system is that they are awake and at optimal alertness. This applies to individuals driving their own cars, riding a bike or motorcycle, as well as pilots of commercial aircraft, train engineers, long-haul truck drivers, and air traffic controllers. Alert operators are a basic requirement for a safe and effective transportation system. Decades of scientific and operational research have demonstrated that the 24/7 scheduling demands on operators and passengers of our transportation system create sleep and circadian disruptions that reduce alertness and performance and cause serious safety problems. These challenges underly the longstanding interest in transportation safety by the sleep and circadian scientific community. An area currently offering perhaps the most significant opportunities and challenges in transportation safety involves vehicle technology innovations. This paper provides an overview of these latest innovations with a focus on sleep-relevant issues and opportunities. Drowsy driving is discussed, along with fatigue management in round-the-clock transportation operations. Examples of cases where technology innovations could improve or complicate sleep issues are discussed, and ongoing sleep challenges and new safety opportunities are considered.

Abstract:Study Objectives: This study investigated the association between driving safety and seven sleep disorders amongst 3541 participants of the Second Strategic Highway Research Program (SHRP 2) naturalistic driving study.Methods: SHRP 2 collected naturalistic driving data from participants between 16 and 98 years old by instrumenting participants' vehicles. The analyses used logistic regression to determine the likelihood of crash or near-crash involvement, Poisson log-linear regression to assess crash or near-crash rate, and ordinal logistic regression to assess driver maneuver appropriateness and crash or near-crash severity. These analyses did not account for any medical treatments for the sleep disorders.Results: Females with restless legs syndrome/Willis-Ekbom disease (RLS/WED), drivers with insomnia or narcolepsy, are associated with significantly higher risk of crash or near-crash. Drivers with shift work sleep disorder (SWSD) are associated with significantly increased crash or near-crash rate. Females with RLS/WED or sleep apnea and drivers with SWSD are associated with less safe driver maneuver and drivers with periodic limb movement disorder are associated with more severe events. The four analyses provide no evidence of safety decrements associated with migraine.Conclusions: This study is the first examination on the association between seven sleep disorders and different measures of driving risk using large-scale naturalistic driving study data. The results corroborate much of the existing simulator and epidemiological research related to sleep-disorder patients and their driving safety, but add ecological validity to those findings. These results contribute to the empirical basis for medical professionals, policy makers, and employers in making decisions to aid individuals with sleep disorders in balancing safety and personal mobility.

Abstract:Previous studies found the introduction of the today well established media radio and television to affect political participation. This paper evaluates the effect of the relatively recent introduction of a new medium, broadband internet. OLS results suggest a positive association between DSL availability and voter participation across German municipalities. However, the roll-out of DSL networks is not random. The paper exploits the fact that DSL availability depends on a unicipality’s distance to the nearest interconnection point to the existing voice-telephony network. Instrumental-variable results using this distance to predict DSL availability confirm the effect of DSL availability on voter participation.

Abstract:It will probably surprise many readers of this Journal to know that occupational exposures to chemicals are rarely assessed with quantitative data, either for epidemiology or for control of hazardous conditions. Only about 13% of epidemiology studies published in the 1980s, on the aetiology of chronic diseases, used any exposure measurements,1 and even those tended to rely on trivial sample sizes (median n = 4).2 Thus, uncertainty in exposure levels is the largest source of error in defining exposure–response relationships. Likewise, industrial surveys of air contaminants typically obtain only a single air measurement per occupational group.3 Surprisingly, this was not always the case. In the period between 1920 and 1960, engineers and health professionals routinely collected many air samples from a given working population, despite the primitive state of measurement technology.4–7 The likely motivation for such effort was the tremendous variability of air concentrations observed over time and among locations; thus, many measurements were needed to accurately assess exposures. The resulting databases allowed early investigators to delve into the sources and magnitudes of exposure, relationships between exposures and health outcomes, and strategies for comparing air levels with occupational exposure limits (OELs). Such applications are beautifully illustrated in a classic set of contributions to the British Journal of Industrial Medicine ( BJIM ), published more than 50 years ago. I vividly recall discovering these papers in the Berkeley library shortly after joining the faculty of the University of California in 1976. Abstract:To evaluate the prevalence of moderate-severe snoring and sleepiness in professional drivers with potential effects on the crash risk,449participants were involved in complete questionnaires and calculate the prevalence of moderate-severe snoring and sleepiness.Among 449drivers,20.9% were referred for moderate-severe snoring,12.2% were confirmed to have sleepiness.Regression analysie method identified the ratio of moderate-severe snoring[OR=7.714,95%confidence interval(95%CI)4.173～14.259],sleepiness[OR=2.138,95% CI1.044～4.379](P<0.05).The prevalence of moderate-severe snoring and sleepiness is high in professional drivers,which is an essential element in crash risk and attracting more attention with guidance about correlate administration department.

Abstract:Driver drowsiness contributes to 10-20% of motor vehicle crashes. To reduce crash risk, ideally drivers would be aware of the drowsy state and cease driving. The extent to which drivers can accurately identify sleepiness remains under much debate. We systematically examined whether individuals are aware of sleepiness while driving, and whether this accurately reflects driving impairment, using meta-analyses and narrative review. Within this scope, there is high variability in measures of subjective sleepiness, driving performance and physiologically-derived drowsiness, and statistical analyses. Thirty-four simulated/naturalistic driving studies were reviewed. To summarise, drivers were aware of sleepiness, and this was associated to physiological drowsiness and driving impairment, such that high levels of sleepiness significantly predicted crash events and lane deviations. Subjective sleepiness was more strongly correlated (i) with physiological drowsiness compared to driving outcomes; (ii) under simulated driving conditions compared to naturalistic drives; and (iii) when examined using the Karolinska sleepiness scale (KSS) compared to other measures. Gaps remain in relation to how age, sex, and varying degrees of sleep loss may influence this association. This review provides evidence that drivers are aware of drowsiness while driving, and stopping driving when feeling 'sleepy' may significantly reduce crash risk.

Abstract:Background Sleep disorders have a multifactorial origin, and can generate a state of tiredness, drowsiness, affecting physical and mental health, increasing the risk of developing chronic diseases, implying good performance during the workday, affecting productivity. Objectives Evaluate the profile of road drivers, identify lifestyle habits, health conditions, sleep quality and ability to work. Methods This is a cross-sectional descriptive study. The participants were 100 workers in the role of road drivers, from a large transport company, who travel between the states of São Paulo to Rio, Minas, and Curitiba, who agreed to participate in the study. Socio-demographic and occupational data (age, marital status, sex, education, monthly income, time with the company, working hours) were collected. Practice of physical activity, anthropometric variables, health conditions such as current illnesses diagnosed, use of medication, and hours of sleep. Sleep quality and current work ability were assessed using a 0-10-point Likert scale (0-worst to 10-best). Anal, mean values, standard deviation, and absolute and relative frequency were discrepancies. Results The average age of the 100 workers was 47.5 ± 7.64 years, men (99%), married (70%), high school education and monthly income between 1 and 2 minimum wages. The time at the company was, on average, 5.89 ± 5.3 years, with shifts in three work shifts (morning, afternoon, and night). The anthropometric characteristics of the workers were an average weight of 87.67 ± 13.53 kg, height 173.5of 173.5 ± 7.9 cm, and BMI of ± 3.7 kg/m2. Regarding life and health habits, 60% of workers do not practice regular physical activity, only 22% have diagnosed diseases and 22% use medication. Mean sleep was 6.9 ± 1.2 hours and mean sleep quality was y 7.9 ± 2.2 points. Current work ability averaged 9.5±7.8 points, with the response varying greatly among workers. Conclusion Road drivers are older, male, sedentary, but with few diagnosed diseases and in good health and sleep quality, despite their profession with shift work and long itineraries. Implications It is essential to understand the profile of this group of workers due to their lifestyle so that there are strategies that reduce risk factors, improve health, and maintain the safety of users of this service.

Effects of Sleep Deprivation and Sleepiness on Society and Driving

Abstract:Sleep deprivation represents a major public health issue, and increasing the proportion of the population that achieves adequate sleep has been identified as a national health priority. Despite this, insufficient habitual sleep duration is still highly prevalent, with approximately a third of US adults achieving less than 6 h of sleep on average. Habitual short sleep duration cuts across all segments of the population and is associated with weight gain and obesity, heart disease, diabetes, poor cognitive performance, stress, and other adverse outcomes, including mortality. This chapter begins by conceptualizing the effects of sleep in the context of society. This is done largely through a social–ecological framework. Then, this chapter aims to summarize the health effects of sleep deprivation across dimensions of cardiometabolic disease risk, neurocognitive function, and psychological well-being. Then, the chapter will further explore these effects across age, race/ethnicity, gender/sex, relationship status, and occupation. A section will devote specific attention to the role of sleep deprivation in driving accidents. Finally, the chapter ends with a summary and conclusions, with recommendations for future directions.
Awareness of sleepiness and ability to predict sleep onset: can drivers avoid falling asleep at the wheel?

Abstract:Objectives: Regarding the causes of sleep-related accidents, this study assesses whether individuals can anticipate sleep onset accurately and how individuals acknowledge and use physiological and cognitive cues to make judgments related to sleep onset. Methods: A group of 41 partially sleep-deprived subjects predicted the likelihood of sleep in 30 consecutive two-minute intervals and noted physiological and cognitive signs of sleepiness, including involuntary eye closure, head-nodding, wandering thoughts, yawns, and instances of sleep, collectively referred to as "sleep complaints". Continuous polysomnographic recording compared these predictions to actual instances of sleep. Results: Subjects varied in their ability to predict sleep onset. For all subjects, the mean prediction of the likelihood of sleep prior to sleep was significantly higher than the mean prediction of the likelihood of sleep prior to intervals in which no sleep occurred (78% vs. 42%; p<.05). However, subjects tended to predict much lower likelihoods of sleep onset before their first sleep event (55%) than before later sleep events. On average, the rate at which subjects reported miscellaneous sleep complaints (such as head-nodding, eye closure, and wandering thoughts) was higher prior to sleep than prior to intervals in which sleep did not occur. Conclusion: Subjects who acknowledged a limited number of physiological and cognitive indicators of sleepiness tended to be poor predictors. Subjects whose physiological and cognitive signs of sleepiness failed to provide a strong indication of whether or not sleep onset would occur also tended to be poor predictors. Inability to judge sleep onset and, hence, susceptibility to sleep-related accidents, may be attributable to both a scarcity of meaningful warning signs and a failure to acknowledge the importance of physiological and cognitive indicators.

Abstract:Abstract Introduction Road Traffic Accidents (RTAs) are a major cause of mortality and disability in developing countries. Previous studies suggest that one-third of road traffic accidents are due to drowsy driving. The frequency and causes of drowsy driving in Nigeria are unknown as there is limited data and publication on the subject. Hence, this study was carried out as part of an awareness program for the 2023 World Sleep Day to explore the perceptions and determinants of poor sleep among long-distance drivers in Ile-Ife. Methods A cross-sectional qualitative study was conducted with a focus group conducted among 12 leaders of drivers in Ile-Ife. Ethical approval and consent were obtained beforehand. Participants were asked about their perception of sleep with a pre-developed interview guide. Their responses were recorded and transcribed verbatim, and ATLAS.ti 22.3 was used to manage the data. Thematic analysis was done, and sample quotes were documented. Results All respondents were male aged 45 to 52 years. The drivers described their sleep as not sufficient as they leave for work early and return late. A recurrent response was; “sleep is very important, but for people like us, sleep is not sufficient.” Four factors were identified that affect sleep quality: financial debt, responsibilities, drinking alcohol for recreation, and distance traveled. Participants reported that they work longer hours to be able to pay for the hired buses they drive, alongside responsibilities such as payment of children’s school fees, as described; “Had it been that one has a basic salary; one can sleep conveniently till 9 am.” The drivers also added that they prefer to drink alcohol and have fun after work, which affects their sleep. Additionally, drivers who travel intra-state return early and sleep more. Conclusion Commercial drivers are at an increased risk for drowsy driving and, consequently, are at risk for RTAs. This study highlights the multifactorial and socioeconomic challenges that influence sleep quality among long-distance drivers in South West Nigeria. Further studies are required, and educational and economic measures are recommended to be implemented to reduce this risk. Support (if any)

Effects of alertness management training on sleepiness among long-haul truck drivers: A randomized controlled trial

Abstract:Education is a frequently recommended remedy for driver sleepiness in occupational settings, although not many studies have examined its usefulness. To date, there are no previous on-road randomized controlled trials investigating the benefits of training on sleepiness among employees working in road transport. To examine the effects of an educational intervention on long-haul truck drivers' sleepiness at the wheel, amount of sleep between work shifts, and use of efficient sleepiness countermeasures (SCM) in association with night and non-night shift, a total of 53 truck drivers operating from southern Finland were allocated into an intervention and a control group using a stratified randomization method (allocation ratio for intervention and control groups 32:21, respectively). The intervention group received a 3.5-hour alertness management training followed by a two-month consultation period and motivational self-evaluation tasks two and 4-5 months after the training, while the control group had an opportunity to utilize their usual statutory occupational health care services. The outcomes were measured under drivers' natural working and shift conditions over a period of two weeks before and after the intervention using unobtrusive data-collection methods including the Karolinska Sleepiness Scale measuring on-duty sleepiness, a combination of actigraphy and a sleep-log measuring sleep between duty hours, and self-report questionnaire items measuring the use of SCMs while on duty. The data analysis followed a per-protocol analysis. Results of the multilevel regression models showed no significant intervention-related improvements in driver sleepiness, prior sleep, or use of SCMs while working on night and early morning shifts compared to day and/or evening shifts. The current study failed to provide support for a feasible non-recurrent alertness-management training being effective remedy for driver sleepiness in occupational settings. These results cannot, however, be interpreted as evidence against alertness management training in general but propose that driver education is not a sufficient measure as such to alleviate driver sleepiness.

High risk of near-crash driving events following night-shift work

Abstract:Night-shift workers are at high risk of drowsiness-related motor vehicle crashes as a result of circadian disruption and sleep restriction. However, the impact of actual night-shift work on measures of drowsiness and driving performance while operating a real motor vehicle remains unknown. Sixteen night-shift workers completed two 2-h daytime driving sessions on a closed driving track at the Liberty Mutual Research Institute for Safety: (i) a postsleep baseline driving session after an average of 7.6 ± 2.4 h sleep the previous night with no night-shift work, and (ii) a postnight-shift driving session following night-shift work. Physiological measures of drowsiness were collected, including infrared reflectance oculography, electroencephalography, and electrooculography. Driving performance measures included lane excursions, near-crash events, and drives terminated because of failure to maintain control of the vehicle. Eleven near-crashes occurred in 6 of 16 postnight-shift drives (37.5%), and 7 of 16 postnight-shift drives (43.8%) were terminated early for safety reasons, compared with zero near-crashes or early drive terminations during 16 postsleep drives (Fishers exact: P = 0.0088 and P = 0.0034, respectively). Participants had a significantly higher rate of lane excursions, average Johns Drowsiness Scale, blink duration, and number of slow eye movements during postnight-shift drives compared with postsleep drives (3.09/min vs. 1.49/min; 1.71 vs. 0.97; 125 ms vs. 100 ms; 35.8 vs. 19.1; respectively, P < 0.05 for all). Night-shift work increases driver drowsiness, degrading driving performance and increasing the risk of near-crash drive events. With more than 9.5 million Americans working overnight or rotating shifts and one-third of United States commutes exceeding 30 min, these results have implications for traffic and occupational safety.

Abstract:This study investigated the short-term effects of daily recovery, that is, impaired psychological detachment from work and various actigraphical indicators of sleep quality, on near-accidents when commuting to work the next morning. Furthermore, the mediating effect of actigraphically assessed sleep quality on the relationship between impaired psychological detachment from work and near-accidents when commuting to work was analysed. Fifty-six full-time employees of a Swiss assurance company participated in the one-week study. Multilevel analyses revealed that impaired detachment was highly related to a decrease in sleep duration. Furthermore, impaired daily recovery processes, such as impaired psychological detachment from work and disturbed sleep quality, were related to commuting near-accidents. Impaired sleep quality mediated the effect of impaired psychological detachment from work on these near-accidents. Our results show that occupational safety interventions should address both impaired psychological detachment from work and sleep quality in order to prevent near accidents when commuting to work. Practitioner Summary: Commuting accidents occur frequently and have detrimental effects on employees, organisations and society. This study shows that daily lack of recovery, that is, impaired psychological detachment and impaired sleep quality, is related to near-accidents when commuting to work the next morning. Primary prevention of commuting accidents should therefore address daily lack of recovery.

Abstract:Background: The detection of driver fatigue as a cause of sleepiness is a key technology capable of preventing fatal accidents. This research uses a fatigue-related sleepiness detection algorithm based on the analysis of the pulse rate variability generated by the heartbeat and validates the proposed method by comparing it with an objective indicator of sleepiness (PERCLOS). Methods: changes in alert conditions affect the autonomic nervous system (ANS) and therefore heart rate variability (HRV), modulated in the form of a wave and monitored to detect long-term changes in the driver’s condition using real-time control. Results: the performance of the algorithm was evaluated through an experiment carried out in a road vehicle. In this experiment, data was recorded by three participants during different driving sessions and their conditions of fatigue and sleepiness were documented on both a subjective and objective basis. The validation of the results through PERCLOS showed a 63% adherence to the experimental findings. Conclusions: the present study confirms the possibility of continuously monitoring the driver’s status through the detection of the activation/deactivation states of the ANS based on HRV. The proposed method can help prevent accidents caused by drowsiness while driving.

Abstract:Maintaining human alertness and behavioral capability under conditions of sleep loss and circadian misalignment requires fatigue management technologies due to: (1) dynamic nonlinear modulation of performance capability by the interaction of sleep homeostatic drive and circadian regulation; (2) large differences among people in neurobehavioral vulnerability to sleep loss; (3) error in subjective estimates of fatigue on performance; and (4) to inform people of the need for recovery sleep. Two promising areas of technology have emerged for managing fatigue risk in safety-sensitive occupations. The first involves preventing fatigue by optimizing work schedules using biomathematical models of performance changes associated with sleep homeostatic and circadian dynamics. Increasingly these mathematical models account for individual differences to achieve a more accurate estimate of the timing and magnitude of fatigue effects on individuals. The second area involves technologies for detecting transient fatigue from drowsiness. The Psychomotor Vigilance Test (PVT), which has been extensively validated to be sensitive to deficits in attention from sleep loss and circadian misalignment, is an example in this category. Two shorter-duration versions of the PVT recently have been developed for evaluating whether operators have sufficient behavioral alertness prior to or during work. Another example is online tracking the percent of slow eyelid closures (PERCLOS), which has been shown to reflect momentary fluctuations of vigilance. Technologies for predicting and detecting sleepiness/fatigue have the potential to predict and prevent operator errors and accidents in safety-sensitive occupations, as well as physiological and mental diseases due to inadequate sleep and circadian misalignment.

Abstract:Cardiovascular disease (CVD) risk in train drivers is associated with health conditions that can result in sudden incapacity. Drivers are at high risk on several CVD risk factors with research suggesting that sleep may predict CVD risk, however this relationship has not yet been explored. This study investigated the link between sleep and CVD risk, in relation to hours of work day and days off sleep. N=309 Australian drivers completed a cross-sectional survey. A CVD risk score was calculated by summing scores from behavioural and biomedical risk factors. Sleep was most frequently cited as the main reason for decline in perceived health status. Main analyses showed that shorter work day sleep (M=5.79 h) was a significant predictor of increased CVD risk (p=0.013). This relationship was moderated by days off sleep, such that when days off sleep (M=8.17 h) was higher, the effect of work day sleep on CVD risk was weaker (p=0.047). Findings indicate the amount of sleep a driver obtains on non-work days may compensate for adverse health outcomes. Successful management of fatigue in safety critical occupations appears essential not only for the prevention of safety hazards, but also for the long-term health of shift workers. Further investigation is warranted.
