Abstract:Objective: The aim of this study was to identify the prognostic factors of cardiopulmonary arrest (CPA) patients transported by a physician-staffed helicopter who received cardiopulmonary resuscitation (CPR) using AutoPulse (ZOLL Circulation, Sunnyvale, CA). Methods: A total of 110 CPA patients who had CPR performed on them in the helicopter using AutoPulse were enrolled in this retrospective study. We used logistic regression analysis to examine the prognostic factors of CPA patients who were transported by a physician-staffed helicopter. Results: Of these patients, return of spontaneous circulation (ROSC) during transportation was observed in 19 (17.29%); 1 (.9%) survived through hospital discharge without neurologic disability. In multivariate analyses, bystander CPR (P = .023) and the time from the first call to the arrival of a helicopter medical crew (P = .041) were selected as independent factors associated with ROSC. Conclusion: In our study, factors such as early contact from the first call to the arrival of a helicopter medical crew and the presence of bystander CPR appeared to play an important role in attaining ROSC of CPA patients who were transported by a physician-staffed helicopter using AutoPulse.

Abstract:Objective To explore the factors raising the effective ratio of cardiopulmonary resuscitation (CPR) in cardiac arrest (CA).Methods A retrospective analysis was done on the clinical data of CPR in 87 CA patients.The clinical data included the age,the basic disease before CA,the starting time of electric defibrillation and CPR,the duration of CPR,the starting time of artificial respiration.Results In 87 cases,49 were successfully resuscitated(56.3%),while 38 were unsuccessful(43.7%).There were significant differences between the patients who were successfully resuscitated and those who were unsuccessful in the distribution of etiological factor,the age,the starting time of electric defibrillation and CPR,the duration of CPR (P0.05),but the starting time of artificial respiration were similar(P0.05).Adrenalin of 1mg was used at first,then the dosage of adrenalin was added,which was applied to 21 patients who were successfully resuscitated,and to 9 patients who were not.Conclusion The major factors affecting the effective rate of CPR are the primary disease,the age,the starting time of electric defibrillation and CPR and the duration of CPR;Building the artificial circulation immediately and the effective combination of the dosage and the time of using adrenaline are the key measures to raise the effective rate of cardiopulmonary resuscitation.
The Evaluation of Helicopter Emergency Medical Services With a Physician for Acute Myocardial Infarction in Japan: A Registry-Based Study of Prognosis and Time Reduction

Abstract:Objective: The aim of this study was to compare the prognosis and time reduction between helicopter emergency medical services (HEMS) with a physician and ground emergency medical services (GEMS) in acute myocardial infarction (AMI) cases. Methods: This is a registry-based study of the Japan Helicopter Emergency Medical Service Registry from April 1, 2015, to March 31, 2018. Results: A total of 605 cases of AMI were registered in the HEMS group and 794 cases in the GEMS group. In the cases of non-cardiopulmonary arrest (CPA), the prognosis between HEMS and GEMS did not differ significantly. Regarding the road distance, for ranges of 20 to 40 km and > 40 km, the times from the call to the angiography room were significantly shorter with HEMS than GEMS (median 91 vs. 97 minutes, P = .036 and 101 vs. 132 minutes, P = .002, respectively). In cases of CPA, HEMS had a higher rate of return of spontaneous circulation than GEMS (55.3% vs. 36.8%, P = .038), but HEMS had a lower prognosis than GEMS (22.9% vs. 38.9%, P = .036). Conclusion: The present study suggested that HEMS had an advantage in reducing the time to angiography in AMI cases of non-CPA. In cases of CPA, HEMS increased the return of spontaneous circulation without improving the prognosis.

Abstract:Background: Pre-clinical work has shown automated head and thorax elevation with active compression decompression CPR and an impedance threshold device (AHUP-CPR) improved 24-hour neurological survival. Propensity matched studies of out-of-hospital-cardiac arrest (OHCA) patients have shown a time dependent association between survival to hospital discharge (SHD) and application of AHUP CPR.Hypothesis: Rapid application of AHUP CPR for OHCA will have a higher incidence of SHD compared to national data. Methods: This is an ongoing prospective observational study of patients from an AHUP-CPR registry of non-traumatic adult OHCA receiving AHUP CPR as part of first responder care. OHCA patients from pre-hospital sites that consistently implemented rapid AHUP CPR were included. Patient demographics, arrest characteristics, ROSC, SHD, and SHD with favorable neurological function, defined as a Cerebral Performance Category score of 1 or 2, were recorded. Data are reported as means ± SD. Combined 2021 and 2022 data from the Cardiac Arrest Registry to Enhance Survival (CARES) were used for comparison of the primary study endpoint, SHD, and secondary endpoints. Chi-square testing was used to compare outcomes. Results: Of 11 registry sites, 7 (US States AL, FL, IL, MN, OK, TN, VA) met inclusion criteria and 1457 patients from January 2020 to June 2023 were enrolled. Average age was 65 ± 16.4 years, 61.8% (901) were male, and 71% (1035) received AHUP CPR. Average time (mm:ss) from 9-1-1 call to start of CPR was 9:52 (6:47) and time to start of AHUP CPR was 12:59 (5:51). Presenting rhythms were: asystole 47.8% (696), PEA 25.8% (376), and VF 20.5% (298). For all AHUP patients, SHD was 13.6% (198) versus 9.2% (26316/286525) for CARES (p < 0.0001). SHD with favorable neurological function was higher for AHUP (10.3% (150) versus 7.3% for CARES (20889), p < 0.0001) and ROSC (29.4% (428) vs 27.0% (77301), p = 0.039). Conclusions: In an unadjusted analysis, the incidence of SHD, ROSC, and neurologically favorable SHD with rapid implementation of AHUP-CPR was higher than nationally reported data. These results support prior findings of a time sensitive association between survival and time to AHUP-CPR. Additional analyses and study are needed for confirmation.

Prehospital Advanced Airway Management and Ventilation for Out-of-Hospital Cardiac Arrest with Prehospital Return of Spontaneous Circulation: A Prospective Observational Cohort Study in Japan

Abstract:Background: The relationship among advanced airway management (AAM), ventilation, and oxygenation in patients with out-of-hospital cardiac arrest (OHCA) who achieve prehospital return of spontaneous circulation (ROSC) has not been validated. This study was designed to evaluate ventilation and oxygenation for each AAM technique (supraglottic devices [SGA] or endotracheal intubation [ETI]) using arterial blood gas (ABG) results immediately after hospital arrival. Methods: This observational cohort study, using data from the Japanese Association for Acute Medicine OHCA Registry, included patients with OHCA with prehospital and hospital arrival ROSC between July 1, 2014, and December 31, 2019. The primary outcomes were the partial pressure of carbon dioxide in the arterial blood (PaCO2) and partial pressure of oxygen in the arterial blood (PaO2) in the initial ABG at the hospital for each AAM technique (SGA or ETI) performed by paramedics. The secondary outcome was favorable neurological outcome (cerebral performance category [CPC] 1 or 2) for specific PaCO2 levels, which were defined as good ventilation (PaCO2 ≤45 mmHg) and insufficient ventilation (PaCO2 >45 mmHg). Results: This study included 1,527 patients. Regarding AAM, 1,114 and 413 patients were ventilated using SGA and ETI, respectively. The median PaCO2 and PaO2 levels were 74.50 mmHg and 151.35 mmHg in the SGA group, while 66.30 mmHg and 173.50 mmHg in the ETI group. PaCO2 was significantly lower in the ETI group than in the SGA group (12.55 mmHg; 95% CI 15.27 to 8.20, P-value < 0.001), while no significant difference was found in PaO2 by multivariate linear regression analysis. After stabilizing inverse probability of weighting (IPW), the adjusted odds ratio for favorable neurological outcome at 1 month was significant in the good ventilation group compared to the insufficient ventilation cohort (adjusted odds ratio = 2.12, 95%CI: 1.40 to 3.19, P value < 0.001). Conclusion: The study showed that in OHCA patients with prehospital ROSC, the PaCO2 levels in the initial ABG were lower in the group with AAM by ETI than in the SGA group. Furthermore, patients with prehospital ROSC and PaCO2 ≤45 mmHg on arrival had an increased odds of favorable neurological outcome after stabilized IPW adjustment.

Abstract:Objective Several studies have examined the impact of mechanical cardiopulmonary resuscitation (CPR) devices among multi-jurisdictional emergency medical services (EMS) systems; however, the variability across such systems can inject bias and confounding variables. We focused our investigation on the effect of introducing the Lund University Cardiac Assist System 2 (LUCAS-2) into a single basic life support (BLS) fire department first response jurisdiction served by a single private advanced life support (ALS) agency, hypothesizing that the implementation of the device would increase prehospital return of spontaneous circulation (ROSC) rates as compared with manual CPR. Methods A retrospective observational analysis of adult non-traumatic prehospital cardiac arrest ALS agency records was conducted. Descriptive statistics were computed, and logistic regression was used to assess the impact of CPR method, response time, age, gender, CPR initiator, witnessed status, automated external defibrillator (AED) initiator, and presence of an initial shockable rhythm on ROSC rates. A Chi-square analysis was used to compare ROSC rates among compression modalities both before and after the implementation of LUCAS-2 on July 1, 2011. Results From an initial dataset of 857 cardiac arrest records, only 264 (74 pre-LUCAS period, 190 LUCAS-2 period) met inclusion criteria for the primary objective. The ROSC rates were 29.7% (22/74) and 29.5% (56/190), respectively, for manual-only and LUCAS-assisted CPR (p=0.9673). Logistic regression revealed a significant association between ROSC and two of the independent variables: arrest witnessed (OR 3.104; 95% CI 1.896-5.081; p<0.0001) and initial rhythm shockable (OR 2.785; 95% CI 1.492-5.199; p<0.0013). Conclusions Analyses support the null hypothesis that there is no difference in prehospital ROSC rates among adult non-traumatic cardiac arrest patients when comparing mechanical-assisted and manual-only CPR. These results are consistent with other larger multi-jurisdictional mechanical CPR studies. Systems with limited personnel might consider augmenting their resuscitations with a mechanical CPR device, although cost and system design should be factored into the decision. Secondary analysis of independent variables suggests that prehospital cardiac arrest patients with a witnessed arrest or an initial rhythm that is shockable have a higher likelihood of attaining ROSC. The power of our primary objective was limited by the sample size. Additionally, we were not able to adequately assess the quality of CPR among the two comparison groups with a lack of consistent end-tidal carbon dioxide (EtCO2) data. .

Abstract:Out-of-hospital cardiac arrest (OHCA) remains a life-threatening condition that requires immediate intervention to increase the prospects of survival. It affects more than 356,000 people annually in the United States, with an undesirable survival rate of approximately 10%. [ 1 ] Providing immediate and efficient cardiopulmonary resuscitation (CPR) can double or triple the likelihood of surviving an OHCA by providing temporary circulation to vital organs. Inefficient compressions and insufficient decompressions lead to decreased CPR efficiency. This directly affects the return of spontaneous circulation (ROSC) and long-term outcomes such as survival. [ 2 , 3 ] Studies have debated whether the utilization of manual CPR increases the likelihood of survival as opposed to mechanical CPR. [ 4 , 5 ] There are various approved devices on the market, namely "AutoPulse" and "Lund University Cardiopulmonary Assist System" (LUCAS), with a large number of studies performed comparing the efficacy of such devices. On account of the number of studies available, the conflicting results of such studies raise hesitancy and reluctance with respect to the superiority of either manual or mechanical chest compression.

Abstract:Abstract Background Mechanical chest compression (mCPR) offers advantages during transport under cardiopulmonary resuscitation. Little is known how devices perform en-route. Aim of the study was to measure performance of mCPR devices of different design during pre-hospital transport. Methods We tested animax mono (AM), autopulse (AP), corpuls cpr (CC) and LUCAS2 (L2). The route had 6 stages (transport on soft stretcher or gurney involving a stairwell, trips with turntable ladder, rescue basket and ambulance including loading/unloading). Stationary mCPR with the respective device served as control. A four-person team carried an intubated and bag-ventilated mannequin under mCPR to assess device-stability (displacement, pressure point correctness), compliance with 2015 ERC guideline criteria for high-quality chest compressions (frequency, proportion of recommended pressure depth and compression-ventilation ratio) and user satisfaction (by standardized questionnaire). Results All devices performed comparable to stationary use. Displacement rates ranged from 83% (AM) to 11% (L2). Two incorrect pressure points occurred over 15,962 compressions (0.013%). Guideline-compliant pressure depth was > 90% in all devices and compression-ventilation ratio was 40% (AM), 87% (CC, L2 respectively) and 93% (AP). Electrically powered devices showed constant frequencies while muscle-powered AM showed more variability (median 100/min, interquartile range 9). Although physical effort of AM use was comparable (median 4.0 vs. 4.5 on visual scale), participants preferred electrical devices. Conclusion All devices showed good to very good performance although device-stability, guideline compliance and user satisfaction varied by design. Our results underline the importance to check stability and connection to patient under transport.

Abstract:In pre-hospital settings, efficient cardiopulmonary resuscitation (CPR) is challenging; therefore, the application of mechanical CPR devices continues to increase. However, the evidence of the benefits of using mechanical CPR devices in pre-hospital settings for adult out-of-hospital cardiac arrest (OHCA) is controversial. This meta-analysis compared the effects of mechanical and manual CPR applied in the pre-hospital stage on clinical outcomes after OHCA. Cochrane Library, PubMed, Embase, and ClinicalTrials.gov were searched from inception until October 2021. Studies comparing mechanical and manual CPR applied in the pre-hospital stage for survival outcomes of adult OHCA were eligible. Data abstraction, quality assessment, meta-analysis, trial sequential analysis (TSA), and grading of recommendations, assessment, development, and evaluation were conducted. Seven randomized controlled and 15 observational studies were included. Compared to manual CPR, pre-hospital use of mechanical CPR showed a positive effect in achieving return of spontaneous circulation (ROSC) and survival to admission. No difference was found in survival to discharge and discharge with favorable neurological status, with inconclusive results in TSA. In conclusion, pre-hospital use of mechanical CPR devices may benefit adult OHCA in achieving ROSC and survival to admission. With low certainty of evidence, more well-designed large-scale randomized controlled trials are needed to validate these findings.

Abstract:Introduction: Mechanical CPR mitigate rescuer fatigue and ensure adherence to guidelines, potentially enhancing overall survival rates. Contrarily, manual CPR relies on human skill, presenting challenges in maintaining uniformity and endurance. Unveiling the complexities within this juxtaposition becomes imperative in shaping future resuscitative paradigms. The purposeof studi is to determine the effectiveness of mechanical CPR in cardiac arrest management compared to manual CPRMethods: Database used were Pubmed and Proquest using the keywords "Mechanical CPR", "Automatic CPR Device" and "Cardiac Arrest". The inclusion criteria were last 5 years published, analyzing the effect of automatic CPR devices on cardiac arrest management, as well as full text articles. The inclusion criteria are literature review, systematic review or is not original research. The number of articles eligible for review is 10 journal articlesResults: The use of automatic CPR equipment is still inferior compared to manual CPR during cardiac arrest with the use value being 4-5% of all cardiac arrest patients. There are 4 journals that state that ROSC in cardiac arrest management using automatic CPR devices is higher than manual, but there are 2 journal articles that report the opposite. Automatic CPR device is very vulnerable to injury with fractures reaching 85.5% of total injuries compared to manual CPR which does not experience injury.Conclusion: The use of automatic CPR device does not produce better results than manual CPR overall for ROSC, but is very useful and meaningful in certain conditions such as during the transportation process, therefore, the automatic CPR device is more meaningful for pre-hospital useKeywords: Manual CPR; Mechanical CPR; CPR; OHCA

Abstract:Background Limbs pneumatic compression during cardiopulmonary resuscitation (CPR) for cardiac arrest patient were rarely reported. Purpose To evaluate the effect of this therapy on the patients of cardiac arrest. Methods A randomized, controlled and trial, a total of 62 elderly (75.8±8.3 yrs) cardiac arrest patients were included and randomized to "CPR group" (n=31) and "CPR + Pneumatic Compression group" (n=31). Each individual in CPR group received standard CPR. In addition to the standard CPR, all patients in "Pneumatic Compression group" accepted intermittent pneumatic compression (IPC) by Air Compression Therapy System, which digital six-tiered rippling full-body pressure (0.45–0.6 kg/cm2) and 1 minute interval every 5 minutes. Results Compared with standard CPR group, significant improvements of cardiac output (CO) and survival of patient were found in Pneumatic Compression group (Figure 1). Patients with pneumatic compression had lower incidence (19.4% vs. 45.2%, p=0.03) of acute kidney injury (AKI) than that in standard CPR group. Besides, the lower occurrence of hypoxic ischemic encephalopathy (HIE) was observed in Pneumatic Compression group (Figure 2). There was no ischemic myonecrosis of limbs caused by compression. Conclusions Intermittent limbs pneumatic compression during cardiopulmonary resuscitation (CPR) maybe could improve the blood flow of the crucial organs (e.g. brain) and increase survival of cardiac arrest patients, which is superior to the strategy for CPR alone. Abstract:SESSION TITLE: Education, Research, and Quality Improvement Posters SESSION TYPE: Original Investigation Posters PRESENTED ON: October 18-21, 2020 PURPOSE: The rapid response team (RRT) is designed to capture patients with early deterioration and allow for early intervention to prevent cardiopulmonary arrest (CPA). It has been widely adopted by most hospital systems as a targeted approach to improve hospital mortality. Currently, in hospital CPA outcomes demonstrate a 60% rate of return of spontaneous circulation (ROSC) and a 20% survival to discharge rate. The American Heart Association (AHA) identifies factors which lead to improved CPA outcomes including early identification of distress, chest compressions and defibrillation. Studies to date have been inconsistent in determining improved mortality. We aim to identify if CPA events that were preceded by an RRT event improved survival outcomes, as these patients would have had early identification of their distress and chest compressions. METHODS: A retrospective chart review over between January 2015 – December 2019 was conducted at our 660 bed university affiliated community hospital. 206 patients were included in the study. 103 CPAs preceded by an RRT and 103 without an RRT event on general medical floors. Patient data including age, gender, time of event, time of last assessment, last vital signs and laboratory abnormalities was collected. Data was analyzed with parametric and non parametric testing by SPSS. RESULTS: While the number of RRTs have increased by 1.5x per month (72 to 110) over the study period, the number of CPAs have not. The rate of ROSC and survival to discharge for CPA preceded by an RRT compared to those not preceded by an RRT were similar (60% vs 64%, 20% vs 20%, p=0.57, 1.0). There was no significant age (p=0.95), gender (p=0.78) or initial rhythm difference between the two groups. Patients with ROSC were found to have a shorter time since last assessment (1:53 vs 2:53, p=0.02) when preceded by an RRT. Shorter CPA durations were demonstrated between those with and without ROSC in both groups independently (p<0.001). No other analyses were significant including number of SIRS criteria, vital signs and time of arrest between the two groups. CONCLUSIONS: The AHA identifies early identification and chest compressions as keys to successful ROSC in CPA events. While early identification of deterioration was determined via the RRT event and early compressions initiated as the RRT event transitioned into a cardiopulmonary arrest, the rates of ROSC and survival to discharge were not significantly different than CPAs not preceded by RRTs. CLINICAL IMPLICATIONS: As RRT events have increased over the years, current outcomes have not drastically improved leading us to believe that we are not identifying the appropriate patient population. Potentially by the time of the RRT event, a patient may be past a point of decompensatory return. Effectiveness of Extracorporeal Cardiopulmonary Resuscitation for Non-Shockable Out-of-Hospital Cardiac Arrest: An Analysis From the SOS-KANTO 2017 Study

Abstract:Extracorporeal cardiopulmonary resuscitation (ECPR) using veno-arterial extracorporeal membrane oxygenation (V-A ECMO) has the potential as a viable treatment for refractory out-of-hospital cardiac arrest (OHCA). While mechanical circulatory support devices, such as Impella® and left ventricular assist devices, are being increasingly used, initial ECPR often relies on V-A ECMO. Previous studies, including randomized controlled trials, reported the prognostic benefits of ECPR for shockable OHCA (SOHCA); however, its effectiveness for non-SOHCA (NSOHCA) remains unclear, with poorer neurological outcomes and the lower return of spontaneous circulation rates than for SOHCA being reported. The present study utilized data from the SOS-KANTO 2017 study to examine the impact of ECPR on the neurological outcomes of NSOHCA. Data from 2,502 OHCA cases were analyzed, with a focus on the relationship between ECPR and 90-day neurological outcomes. The results obtained showed significantly higher survival rates at 30 and 90 days and significantly better 90-day neurological outcomes in the ECMO attempt group than in the non-ECMO attempt group. A multivariate analysis identified ECPR as one of the significant independent predictors of favorable neurological outcomes. The prognosis of NSOHCA cases with CA was improved by ECPR using V-A ECMO, particularly in those where CPR was initiated within one minute of onset and the patient arrived at the hospital within 45 minutes. Factors associated with a favorable prognosis included a shorter time from onset to hospital arrival and the likelihood of acute coronary syndrome being the cause of CA. The present results suggest the potential of ECPR to improve the survival and the 90-day prognosis of NSOHCA, particularly when bystander CPR is initiated quickly and hospital arrival is prompt.

Abstract:Introduction: Cardiopulmonary resuscitation (CPR) in microgravity requires specific methods to counteract weightlessness. Automatic chest compression devices (ACCDs) could improve CPR in microgravity. We aimed to compare ACCDs versus manual CPR in microgravity simulated through parabolic flights. Methods: This prospective, open, controlled study compared 3 ACCDs (LUCAS 3©, AUTOPULSE©, EASYPULSE©) to manual CPR during the 66th CNES (Centre National d'Etudes Spatiales) parabolic flights campaign onboard the Novespace Air Zero-G A310 aircraft. Chest compression depths and rates were monitored by a Laerdal© Resusci-Ann-QCPR manikin. Results: The LUCAS 3© device had a median compression depth of 53.0 [53.0 - 54.0] mm, significantly higher than the EASYPULSE©, AUTOPULSE©, and Manual CPR (Handstand method), measured at 29.0 [26.0 - 32.0] mm, 29.0 [27.5 - 30.7] mm and 34.5 [29.6 - 43.3] mm, respectively (p value <0.001). Compression rates were 101 [101 - 101], 100 [100 - 100] and 80 [80 - 80] compressions per minute (cpm) for the LUCAS 3©, EASYPULSE©, and AUTOPULSE©, respectively. Manual CPR provided a significantly higher compression rate with 115 [109 - 123] cpm (p value <0.001). Conclusion: Only LUCAS 3© provided effective CPR according to international guidelines. ACCDs should implement microgravity CPR algorithms.

Abstract:Abstract Background Mechanical chest compression (mCPR) offers advantages during transport under cardiopulmonary resuscitation. Little is known how devices of different design perform en-route. Aim of the study was to measure performance of mCPR devices of different construction-design during ground-based pre-hospital transport. Methods We tested animax mono (AM), autopulse (AP), corpuls cpr (CC) and LUCAS2 (L2). The route had 6 stages (transport on soft stretcher or gurney involving a stairwell, trips with turntable ladder, rescue basket and ambulance including loading/unloading). Stationary mCPR with the respective device served as control. A four-person team carried an intubated and bag-ventilated mannequin under mCPR to assess device-stability (displacement, pressure point correctness), compliance with 2015 ERC guideline criteria for high-quality chest compressions (frequency, proportion of recommended pressure depth and compression-ventilation ratio) and user satisfaction (by standardized questionnaire). Results All devices performed comparable to stationary use. Displacement rates ranged from 83% (AM) to 11% (L2). Two incorrect pressure points occurred over 15,962 compressions (0.013%). Guideline-compliant pressure depth was > 90% in all devices. Electrically powered devices showed constant frequencies while muscle-powered AM showed more variability (median 100/min, interquartile range 9). Although physical effort of AM use was comparable (median 4.0 vs. 4.5 on visual scale up to 10), participants preferred electrical devices. Conclusion All devices showed good to very good performance although device-stability, guideline compliance and user satisfaction varied by design. Our results underline the importance to check stability and connection to patient under transport.

Abstract:Objectives: To evaluate the diagnostic accuracy of carotid artery compression using a point-of-care ultrasound probe (POCUS-CAC) in reducing pulse check times and facilitating the detection of the return of spontaneous circulation (ROSC) during cardiopulmonary resuscitation (CPR) compared to manual palpation (MP). The secondary aim of the study is to assess the ability of POCUS-CAC to detect ROSC during ongoing chest compressions. Methods: This prospective study was conducted in a tertiary emergency department between January and June 2023. During CPR, POCUS-CAC was performed by placing a linear ultrasound probe transversely on the lateral neck to assess the compressibility of the carotid artery. Complete compression of the artery without any visible pulsation indicated no ROSC, while resistance to compression or partial compression suggested the presence of ROSC. Simultaneously, another clinician performed manual palpation of the femoral artery. The primary outcome assessed in this study was comparing ROSC detection between POCUS-CAC and traditional methods, and the secondary outcome was comparing the time taken to detect ROSC with each method, and the ability to detect ROSC during ongoing chest compressions. Results: The study included 41 cardiac arrest patients and analyzed 496 MP pulse and 1984 POCUS-CAC checks. The mean time to identify a pulse using POCUS-CAC was significantly shorter, at 2.3 (0.5–7.8, SD ± 1.2, 95% CI [2.25, 2.35]) s, compared to 4.7 (2.0–10.5, SD ± 1.8, 95% CI [4.54, 4.86]) s with MP (p = 0.004). Additionally, 52.9% of ROSC cases were detected earlier using POCUS-CAC, even during ongoing chest compressions. The sensitivity of POCUS-CAC was 100% (95% CI [80.5–100%]) and the specificity was 87.5% (95% CI [67.6–97.3%]). The POCUS-CAC method required less than 5 s in 99.996% of cases. Conclusions: POCUS-CAC significantly reduces pulse check times and enhances the early detection of ROSC during CPR, offering a reliable and rapid alternative to traditional manual palpation methods in emergency settings.

Abstract:Aim In cases of out-of-hospital cardiac arrests (OHCA) occurring at home, Japanese emergency medical services personnel decide whether to provide treatment on the scene or during transport based on their judgment. This study aimed to evaluate the association between the timing of advanced life support (ALS) (i.e., endotracheal intubation [ETI] or adrenaline administration) for OHCA at home and prognosis. Method This retrospective cohort study used data from the Japan Utstein Registry and emergency transport data collected from patients who underwent pre-hospital ETI ( n = 6806) and received adrenaline ( n = 22,636) between 2016 and 2019. The timing of ETI or adrenaline administration was determined as "on the scene" or "in the ambulance." Multiple logistic regression analysis was used to estimate the association among the timing of ALS implementation, pre-hospital return of spontaneous circulation (ROSC), and survival at 1 month. Result ETI on the scene was significantly positively associated with pre-hospital ROSC (adjusted odds ratio [AOR], 1.81; 95% confidence interval [CI], 1.57–2.09) and survival at 1 month (AOR, 1.81; 95% CI, 1.47–2.23). Adrenaline administration on the scene was significantly positively associated with pre-hospital ROSC (AOR, 2.51; 95% CI, 2.33–2.70) and survival at 1 month (AOR, 2.13; 95% CI, 1.89–2.40). Conclusion Our analysis suggests performing ALS on the scene was associated with pre-hospital ROSC and survival at 1 month. Further efforts are needed to increase the rate of ALS implementation on the scene by emergency life-saving technicians.

Abstract:Aim: Out-of-hospital cardiac arrest (OHCA) has a poor prognosis in children; however, the annual patterns of prognosis and treatment have not been fully investigated. Methods: From the Japanese Association for Acute Medicine OHCA registry, a multicenter prospective observational registry in Japan, we identified pediatric patients (zero to 17 years old) between June 2014 and December 2019. The primary outcome was one-month survival. We investigated the annual patterns in patient characteristics, treatment, and one-month prognosis. Results: During the study period, 1188 patients were eligible for analysis. For all years, the zero-year-old group accounted for a large percentage of the total population (between 30% and 40%). There were significant increases in the rates of bystander-initiated cardiopulmonary resuscitation (CPR; from 50.6% to 62.3%, p = 0.003), dispatcher instructions (from 44.7% to 65.7%, p = 0.001), and adrenaline administration (from 2.4% to 6.9%, p = 0.014) over time, whereas the rate of advanced airway management decreased significantly (from 17.7% to 8.8%, p = 0.003). The odds ratios for one-month survival adjusted for potential resuscitation factors also did not change significantly (from 7.1% to 10.3%, adjusted odds ratio for one-year increment = 0.98, confidence interval: 0.86-1.11). Conclusion: Despite an increase in the rate of bystander-initiated CPR and pre-hospital adrenaline administration, there was no significant change in one-month survival.
Effects of real-time feedback on cardiopulmonary resuscitation quality on outcomes in adult patients with cardiac arrest: A systematic review and meta-analysis

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Aim</h3><p>To investigate the relationship between the implementation of real-time audiovisual cardiopulmonary resuscitation (CPR) feedback devices with cardiac arrest patient outcomes, such as return of spontaneous circulation (ROSC), short-term survival, and neurological outcome.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We systematically searched PubMed, Embase, and the Cochrane CENTRAL from inception date until April 30, 2020, for eligible randomized and nonrandomized studies. Pooled odds ratio (OR) for each binary outcome was calculated using R system. The primary patient outcome was ROSC. The secondary outcomes were short-term survival and favorable neurological outcomes (cerebral performance category scores: 1 or 2).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>We identified 11 studies (8 nonrandomized and 3 randomized studies) including 4,851 patients. Seven studies documented patients with out-of-hospital cardiac arrest and four studies documented patients with in-hospital cardiac arrest. The pooled results did not confirm the effectiveness of CPR feedback device, possibly because of the high heterogeneity in ROSC (OR: 1.42, 95% CI: 1.03–1.94, I<sup>2</sup>: 80%, tau<sup>2</sup>: 0.1875, heterogeneity test <em>p</em> < 0.01) and survival-to-discharge (OR: 1.27, 95% CI: 0.74–2.18, I<sup>2</sup>: 86%, tau<sup>2</sup>: 0.4048, heterogeneity test <em>p</em> < 0.01). The subgroup analysis results revealed that heterogeneity was due to the types of devices used. Patient outcomes were more favorable in studies investigating portable devices than in studies investigating automated external defibrillator (AED)-associated devices.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Whether real-time CPR feedback devices can improve patient outcomes (ROSC and short-term survival) depend on the type of device used. Portable devices led to better outcomes than did AED-associated devices. Future studies comparing different types of devices are required to reach robust conclusion.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Protocol registration</h3><p>Prospero registration ID CRD42020155388.</p>
