Characterization of Single Lesion Nonfunctioning Pancreatic Neuroendocrine Carcinoma Via Computed Tomography

Abstract:Nonfunctioning pancreatic neuroendocrine carcinoma (NPNEC) is a poorly differentiated neuroendocrine carcinoma and a rare type of pancreatic cancer. Computed tomography (CT)-based imaging is currently the most effective method for diagnosing and classifying NPNEC. The present study investigated the clinical features and CT manifestations of this disease. The CT manifestations and clinical data of 13 patients with NPNEC, confirmed by surgery or needle biopsy, were retrospectively analyzed. None of the 13 patients exhibited endocrine-related symptoms and a single lesion with a variable position in the pancreatic was observed in all patients. The tumors were generally round or irregular, with inhomogeneous density and an average diameter of 7.45 +/- 3.92 cm (range, 4.0-16.0 cm). Cystic necrosis zones were observed in 5 patients and calcification was observed in 2. A total of 4 patients exhibited intrahepatic metastases, and 2 patients demonstrated spleen and colon invasion. Enhanced scanning indicated that tumors were markedly enhanced. There were 11 cases of middle or high-grade enhancement (84.6%) and 2 cases of mild enhancement (15.4%). Marked envelope enhancement was observed in 3/11 patients with high-grade enhancement. In conclusion, the majority of patients with NPNEC lacked characteristic manifestations. While the CT examination revealed certain characteristics, histological analysis was necessary to confirm diagnoses.

Abstract:OBJECTIVE:To analyze the prognostic factors of pancreatic neuroendocrine neoplasms (PNEN).METHODS:Clinical data of 61 patients with PNEN from March 1992 to December 2012 was retrospectively analyzed. There were 23 male and 38 female patients, with a median age of 52 years (ranged from 22 to 68 years). Forty-one patients were non-functional tumors, and 20 patients were functional tumors. Fifty-nine patients received operation, 13 (22.0%) patients underwent laparoscopic operation, 2 patients underwent puncture biopsy under CT guidance. Survival was analyzed with the Kaplan-Meier method.RESULTS:Among these patients, 53 (86.9%) patients underwent curative resection. The cases of grade G1, G2, G3 were 41 (67.2%), 9 (14.8%), 11 (18.0%), respectively. The cases of stageI, II, III, IV were 47 (77.0%), 7 (11.5%), 2 (3.3%), 5 (8.2%), respectively. Liver metastasis, neural invasion were found in 5 cases (8.2%), 5 cases (8.2%), respectively. The median follow-up period was 40 months (ranged from 3 to 209 months). The overall 1-, 3-, 5-year survival rates were 92.0%, 89.7%, 86.3%, respectively. Univariate analysis showed WHO classification (χ(2) = 18.503), TNM staging system (χ(2) = 23.401), liver metastasis (χ(2) = 18.606), neural invasion (χ(2) = 10.091), resection status (χ(2) = 25.514) were prognostic factors of PNEN (all P = 0.000).CONCLUSIONS:Surgical resection in PNEN results in long-term survival. WHO classification, TNM staging, resection status are effective in predicting the prognosis of PNEN. Liver metastasis, neural invasion predicted poor prognosis.

Abstract:A search was made with the High Resolution Spectrometer at PEP for events with an isolated pair of charged tracks e/sup + -/X/sup - +/ where X is a muon or meson. Such pairs are a possible signature for the decay of massive neutrinos. Only one event was found, for a integrated luminosity of 300 pb/sup -1/, satisfying certain selection criteria and with a mass m/sub ex/ > 1.8 GeV. This result places strong constraints on the existence of new neutrinos with masses from 1.8 to 6.7 GeV and with mixing parameters to the known neutrinos in the range 10/sup -6/ < absolute value of U/sup 2/ < 1. 12 refs., 6 figs.
Analysis of the Clinical Features of 20 Cases of Non-Functioning Pancreatic Neuroendocrine Tumors

Abstract:Objective To investigate the clinical features of non- functioning pancreatic neuroendocrine tumors and to summarize experiences in the diagnosis and treatment. Methods The clinical pathology and follow-up data of 20 patients with non-functioning pancreatic neuroendocrine tumors treated in Nanjing Drum Tower Hospital Affiliated to Medical School of Nanjing University, from January 2008 to February 2018 were retrospectively analyzed. Results Among the 20 patients, 9 patients (9/20) were asymptomatic, and the tumor was found unexpectedly by physical examination. Eleven patients (11/20) with symptoms were admitted to the hospital due to non-specific gastrointestinal symptoms or compression symptoms caused by tumor space occupying. None of the patients had endocrine disorders. There were nineteen patients who received surgical treatment. According to the grading system, there were 7 patients (7/19) with G1, and 12 patients (12/19) with G2 . Among the 14 patients with diameter of tumor greater than 2 cm, there are 4 patients (4/14) with lymph node metastasis, and 4 patients (4/14) with liver metastasis. Five patients with diameter of tumor less than or equal to 2 cm had no distant metastasis. Conclusions NF-pNETs is often with nonspecific symptoms. Imaging examination is an important diagnostic method. Operation is the primary therapy for NF-pNETs. Because most of the small NF-pNETs with no symptoms are benign and grow slowly, and the metastatic rate is very low. It is important to weigh the pros and cons of surgical treatment for these patients.

Abstract:The aim of this study is to summarize the experience of preoperative diagnosis for nonfunctioning pancreatic neuroendocrine tumors (NFPNETs). The medical records of patients with NFPNETs were identified from 2001 to 2009 in a single center. They were all reviewed and analyzed for their demographics, clinical presentation, preoperative laboratory evaluation and radiographic characterization. Twenty-six patients (11 men and 15 women) in the past 8 years were diagnosed with NFPNETs. Almost all patients had the normal level of exocrine tumor markers. Although it was not difficult to find tumors by traditional imaging modalities including CT and MRI, the misdiagnosis rate was as high as 81.2%. Perfusion CT was helpful to predict the biological behavior of tumors and even identified NFPNETs as benign or malignant. The preoperative diagnosis for NFPNETs can be confirmed by one or the combination of CgA, perfusion CT, FNA-EUS, PET and octreoscans. NFPNETs should be aroused suspicion of normal exocrine tumor markers and relative specific imaging appearance, which can be proved by one or the combination of CgA, perfusion CT, FNA-EUS, PET and octreoscans.

Abstract:The incidence of pancreatic neuroendocrine tumors (PNETs) is on the rise primarily due to the increasing use of cross-sectional imaging. Most of these incidentally detected lesions are non-functional PNETs with a small proportion of lesions being hormone-secreting, functional neoplasms. With recent advances in surgical approaches and systemic therapies, the management of PNETs have undergone a paradigm shift towards a more individualized approach. In this manuscript, we review the histologic classification and diagnostic approaches to both functional and non-functional PNETs. Additionally, we detail multidisciplinary approaches and surgical considerations tailored to the tumor's biology, location, and functionality based on recent evidence. We also discuss the complexities of metastatic disease, exploring liver-directed therapies and the evolving landscape of minimally invasive surgical techniques.

Abstract:Neuroendocrine tumor of the pancreas (pNET) is a rare group of neoplasms with widely heterogeneity.Complete surgical resection remains the only potentially curative treatment for pNET.Compared to open surgery,minimal invasive approach treating pNET could decrease bleeding and shorten postoperative hospital stay,while didn't show difference in operation time,postoperative complication rate and overall mortality.Therapeutic strategy was formulated by synthesizing clinical symptoms,tumor size and tumor stage.Minimal invasive approach can achieve radical curve of oncology and provide refine manipulation condition to improve surgical safety and efficacy,meanwhile realize function preservation.Minimal invasive approach is a safe and efficient method in treating pNET,but further prospective,randomized control trials are required.

Abstract:A 36-year-old woman underwent left lateral hepatic lobectomy and tumor enucleation of the pancreatic head due to the diagnosis of mixed neuroendocrine nonneuroendocrine neoplasms (MiNENs). Her past medical history revealed an ill-defined mass in the pancreatic uncinate (approximately 1 cm in size) and liver nodules in segment II, suggesting a pancreatic neuroendocrine tumor (pNET) and liver metastases. After the operation, pathological examination showed that the lesion in the pancreas was MiNEN. The neuroendocrine component consisted of grade 1 (G1) NETs, and the nonneuroendocrine part was well-differentiated adenocarcinoma. Additionally, in hepatic lesions, the type of pancreaticobiliary duct in moderately differentiated adenocarcinoma indicated the source of tumor cells. We present the following case in accordance with the CARE reporting checklist. MiNENs of the pancreas are extremely rare and heterogeneous malignancies. Owing to the rarity of pancreatic MiNEN, no structured guidelines on preoperative diagnostics, therapy and follow-up exist thus far. Thus, problems in differential diagnosis and appropriate therapy may arise when treating suspected MiNEN patients. We performed a rigorous search on pancreatic MiNEN via PubMed and Web of Science. In total, 167 studies were identified, and 28 cases in 15 studies about pancreatic MiNEN were assessed. All the available information was listed and fully discussed.
Clinical Efficacy of the Laparoscopic Resection for Pancreatic Neuroendocrine Neoplasms

Abstract:Objective To investigate the clinical efficacy of laparoscopic resection for pancreatic neuroendocrine neoplasms (pNENs).Methods The clinical data of 31 patients who received laparoscopic resection for pNENs at the Sir Run Run Show Hospital of Zhejiang University from March 2005 to June 2014 were retrospectively analyzed.The surgical procedure was selected according to the location and size of the pNENs.Patients were followed up via phone call or out-patient examination till August 2014.Results Laparoscopic resection was successfully carried out on all the 31 patients,including laparoscopic enucleation in 4 patients,laparoscopic pancreatoduodenectomy in 2 patients,laparoscopic central pancreatectomy in 2 patients,laparoscopic spleenpreserving distal pancreatectomy in 6 patients and laparoscopic distal pancreatosplenectomy in 17 patients.The operation time,volume of intraoperative blood loss and blood transfusion rate were (205 ± 74)minutes,125 mL (10-1 000 mL) and 9.7％ (3/31),respectively.The time for out-off-bed activity,time to anal exsufflation,time for fluid diet intake and duration of postoperative hospital stay were 2 days (range,1-3 days),3 days (range,1-4 days),3 days (range,1-7 days) and 10 days (range,5-63 days).The incidence of postoperative complications was 35.5％ (11/31),and the complications were cured after symptomatic treatment.No patient died perioperatively.Thirty-one patients were followed up with the follow-up rate of 100.0％.The median time for followup was 21 months (range,5-111 months).One patient had symptom of pancreatic exocrine insufficiency after the operation; 3 patients had diabetes at postoperative month 1 ; 1 patient died of tumor metastases at postoperative year 1 ; 1 patient had Whipple's triad at postoperative year 3,and was confirmed with tumors in the head and tail of the pancreas by magnetic resonance imaging.No tumor recurrence or metastasis was detected in the other patients.Conclusions Laparoscopic resection is safe and feasible for the treatment of pNENs.The surgical procedure is selected based on the location and size of the pNENs.Laparoscopic resection of pNENs has the advantages of small trauma,quick recovery of the patients and favorable short-and long-term efficacy.

Abstract:Pancreatic neuroendocrine tumors (PNETs) arise from neuroendocrine cells and are a rare class of heterogenous tumors with increasing incidence. The diagnosis, staging, treatment, and prognosis of PNETs depend heavily on identifying the histologic features and biological mechanisms. Here, the authors provide an overview of the diagnostic workup (biomarkers and imaging), grade, and staging of PNETs. The authors also explore associated genetic mutations and molecular pathways and describe updated guidelines on surgical and systemic treatment modalities.
Clinical Features Of Patients With Pancreatic Neuroendocrine Neoplasms: A Retrospective Analysis Of 61 Cases In A Cancer Center

Abstract:Objective: To assess the clinicopathological features of patients with pancreatic neuroendocrine neoplasms (pNENs) in relation with clinical outcomes. Methods: A total of 61 patients with pNENs were enrolled in this study. Patients' characteristics, treatment procedures, and clinical follow-up were retrospectively analyzed. Results: Among the 61 patients, 29 patients (47.5%) were men and 32 patients (52.5%) were women. 6 patients (9.8%) had functional pNENs and presented with diarrhea, flushing, sweating, palpitations or glucose abnormalities. At diagnosis, 31.1% (19/61) had a localized disease, 6.6% (4/61) presented with regional spread including lymph node metastases and/or extrapancreatic organ invasions, and 62.3% (38/61) had distant metastases. Liver was the most common site of metastasis. 58 cases were recorded with detailed pathological information, among which 42 patients (72.4%) were diagnosed as NET G1/G2, 16 patients (27.6%) were diagnosed as NEC G3. The chromogranin A (CgA) levels were elevated in the serum of patients with liver metastasis, unresectable tumors or a Ki-67 proliferative index >= 20%. The univariate analysis suggested that the WHO classification, stage, occurrence of liver metastasis and operative approaches were closely related to the prognosis of pNENs; while sex, age, tumor size and functional status were not. Conclusion: The pNENs are a heterogeneous group of neoplasms with diverse clinical manifestations and prognosis. Serum CgA is an indicator of tumor burden and proliferation. The WHO classification, stage, liver metastasis and operative approaches might closely relate to the prognosis of pNENs.

Abstract:Objective: To investigate the clinical characteristics, diagnosis, treatment, and prognostic factors of pancreatic neuroendocrine neoplasms (PNENs). Methods: From August 2004 to August 2016, the clinical data of 63 patients with PNENs in the Sir Run Run Shaw Hospital, an affiliate of the Zhejiang University School of Medicine, were retrospectively analyzed. The data included age, gender, function, tumor location, tumor size, pathological characteristics, lymph nodes, metastasis, and treatment. Association of these factors with PNEN prognosis was proven by univariate analysis and multivariate analysis. Results: In our study, the patients' overall survival time was between 5 and 127 months; the median time was about 46.6 months. Moreover, the survival rates in three years and five years are 88.8％ and 84.1％ respectively. Univariate analysis showed that factors such as lymph node metastasis, liver metastasis, vascular invasion, TNM staging and pathological grading, and radical operation were associated with prognosis (P<0.05). We did not prove any of the factors to be an independent factor associated with poor short-term outcome by multivariate analysis. Conclusion: PNENs are rare low-grade malignant tumors with heterogeneity, which is why they can be erroneously diagnosed. AJCC TNM staging in 2017 and the WHO pathological classification in 2010 can actively guide the prognosis evaluation. The lymph node metastasis, liver metastasis,and vascular invasion affected the prognosis of PNENs. Patients with radical operation had improved prognosis.

[Clinical Analysis of Minimal Invasive Surgical Treatment for Pancreatic Neuroendocrine Tumors].

Abstract:Objective: To analyze the safety and effectiveness of minimal invasive surgery treating pancreatic neuroendocrine tumors (pNETs), and to summarize surgical characteristics and share experience. Methods: The clinical data of 80 pNETs treated by a single hospital from January 2015 to December 2019 were retrospectively analyzed. The patients were divided into laparoscopic group and robot group. And surgical procedures included pancreaticoduodenectomy (PD), distal pancreatectomy (DP), central pancreatectomy (CP), and tumor enucleation. Results: Of 80 patients, 76 cases (95%) underwent minimal invasive surgery and 4 cases (5%) changed to open surgery. There were 38 females, with median age of 54.4 (20-80) years and median BMI (17.0-38.0) kg/m(2). Among them, 24 patients (31.6%) underwent PD, 36 patients (47.4%) underwent DP, 8 patients (10.5%) underwent CP and 8 patients (10.5%) received tumor enucleation. The postoperative incidence of grade B/C pancreatic fistula was 35.5%, the incidence of abdominal infection was 10.5%, the postoperative bleeding was 7.9%, and the reoperation rate was 6.6%, without perioperative deaths. There was no significant difference in postoperative complications among different surgical methods, including postoperative pancreatic fistula (P=0.396), postoperative bleeding (P=0.297), postoperative abdominal infection (P=0.339) and reoperation (P=0.396). Conclusions: Surgical resection is an effective treatment for pNETs. pNETs are suitable for minimally invasive surgery with earlier stage and smaller tumor diameter. Minimally invasive surgery for pNETs is safe and feasible, and functional preserving surgery could take into consideration.
The Role of Endoscopic Ultrasound-Guided Ki67 in the Management of Non-Functioning Pancreatic Neuroendocrine Tumors

Abstract:BACKGROUND/AIMS: The management of small, incidentally discovered nonfunctioning pancreatic neuroendocrine tumors (NF-PNETs) has been a matter of debate. Endoscopic ultrasound with fine-needle aspiration (EUS-FNA) is a tool used to identify and risk-stratify PNETs. This study investigates the concordance rate of Ki67 grading between EUS-FNA and surgical pathology specimens in NFPNETs and whether certain NF-PNET characteristics are associated with disease recurrence and disease-related death.METHODS: We retrospectively reviewed the clinical history, imaging, endoscopic findings, and pathology records of 37 cases of NFPNETs that underwent pre-operative EUS-FNA and surgical resection at a single academic medical center.RESULTS: There was 73% concordance between Ki67 obtained from EUS-FNA cytology and surgical pathology specimens; concordance was the highest for low- and high-grade NF-PNETs. High-grade Ki67 NF-PNETs based on cytology (p=0.028) and histology (p=0.028) were associated with disease recurrence and disease-related death. Additionally, tumors with high-grade mitotic rate (p=0.005), tumor size >22.5 mm (p=0.104), and lymphovascular invasion (p=0.103) were more likely to have poor prognosis.CONCLUSION: NF-PNETs with high-grade Ki67 on EUS-FNA have poor prognosis despite surgical resection. NF-PNETs with intermediate-grade Ki67 on EUS-FNA should be strongly considered for surgical resection. NF-PNETs with low-grade Ki67 on EUSFNA can be monitored without surgical intervention, up to tumor size 20 mm.

The present study aimed to evaluate the risk of malignancy of small NF-PNETs and their outcomes following curative resection.Methods: Patients with NF-PNETs undergoing surgical resection at the First Affiliated Hospital, College of Medicine, Zhejiang University, between 2012 and 2017 were included. Clinicopathological characteristics, perioperative results, and prognosis were retrospectively analyzed.Results: A total of 73 patients were identified, including 28 with small NF-PNETs and 45 large PNETs; 32.1% of NF-PNETs <= 2 cm underwent a parenchyma-sparing pancreas surgery, which was >6.7% in large NF-PNETs. No statistically significant differences in perioperative results, postoperative complications, and long-term outcomes were found between small tumors undergoing standard and parenchyma-sparing pancreatectomy. Eighteen small tumors (64.3%) developed a perioperative complication, with a clinically significant pancreatic fistula rate of 25%; however, only 2 patient needed reintervention. Small NF-PNETs in 3 patients were malignant. Multivariate logistic regression showed that grade >= 3 and lymphovascular invasion were independently related to malignancy in NF-PNETs.Conclusions: Small NF-PNETs (<= 2 cm) are not immune from potential malignancy. Surgical resection may be considered for small tumors and can provide favorable postoperative and long-term outcomes. Parenchyma-sparing pancreatectomy may be an alternative surgery for selected small local NF-PNETs.
A Comprehensive Comparison of Clinicopathologic and Imaging Features of Incidental/symptomatic Non-Functioning Pancreatic Neuroendocrine Tumors: A Retrospective Study of a Single Center

Abstract:BACKGROUND:Incidentally discovered, nonfunctioning pancreatic endocrine tumors (NF-pNETs) are being increasingly diagnosed with the widespread use of imaging examination. The objective of this study was to analyze the different clinicopathologic and imaging features between incidentally discovered and symptomatic NF-pNETs.METHODS:From March 2010 to October 2014, we retrospectively analyzed 102 patients with pathologically confirmed sporadic NF-pNETs, among which 49 (48.0%) had lesions that were discovered incidentally. Differences in clinicopathologic features and various computerized tomography (CT) and endoscopic ultrasonography (EUS) findings were evaluated between incidental and symptomatic NF-pNETs.RESULTS:Among 102 patients with NF-pNETs, 49 (48.0%) had lesions that were discovered incidentally. Incidental NF-pNETs were more highly associated with low tumor grades, stages and lymphatic metastasis compared with symptomatic tumors (p = 0.007, 0.029 and 0.003, respectively). Moreover, incidental NF-pNETs had a lower rate of hypoenhancement (p = 0.018), main pancreatic duct dilatation (p = 0.043), and unclear border (p = 0.022). In addition, hypoechoic lesion was the most common and had a slightly higher rate in symptomatic tumors (p = 0.032).CONCLUSIONS:Incidental NF-pNETs are associated with lower rates of aggressive behaviors of clinicopathologic and imaging features compared with symptomatic tumors.

Abstract:Neuroendocrine tumors (NETs) are uncommon malignancies that develop from neuroendocrine cells which most commonly occur in the GI tract, lung, and pancreas. Treatment courses for these tumors are largely dictated by the primary origin site, which can present diagnostic and therapeutic challenges in NETs of unknown primary origin. Herein, we present a case of an NET of unknown primary origin with significant liver metastases. Our aim is to highlight the key components of the workup of NETs of unknown primary origin and detail the biochemical, histopathological, and imaging modalities as recommended by current literature. We highlight the importance of a multidisciplinary approach to both diagnosis and treatment of these patients as well as touch upon therapeutic options.
